Miner was inside Stedman crusher preparing to weld on reel. As they stood up, they placed a foot on the reel, dislodging wedge and rotating reel upward catching their leg and pushing it up against the apron.
A contractor without a contractor ID installing arc flash labels at the mine walked down a slight incline off a small cement pad, lost EE's balance and broke EE's right leg.

Employees were trying to remove a crusher breaker plate by welding a lifting eye on the plate and attaching a come-a-long to pull it out. They put tension on the lifting eye and after a few minutes the eye released from the plate and struck one of the employees on the left cheek.

Employee claims hit head on a chute while pulling a hose across the floor. This claim was disputed by the employer. Ohio BWC recently ruled this injury was work related. Notification was received by employer on July 15.

The employee was walking along the catwalk along C-7 conveyor when the last section of Gripstrut catwalk slipped out from under his feet causing him to fall about 9 feet to the ground. The section of catwalk was not bolted down.
Employee was driving a pin out of a conveyor when he struck his left thumb with the hammer. Injury was a fracture of the left thumb.
