Over-exertion in pulling or pushing objects

Employee was working on a front end loader and while pulling on a wrench employee felt a pop in left shoulder

Struck against stationary object

A liner being replaced in the primary crusher was held in position by a maintenance truck crane and the miner. The liner slipped and the miner went to hold it when EE heard a ""pop"" and felt a sharp pain. Management notified of accident on 9/22/20 but was not notified treatment was sought for injury until 10/27/20. Miner was placed on leave by the doctor on 10/28/20.

Changing screen cloth on top deck & screen panel in a bind. When it came free the weight & momentum pinched his hand against an I-beam.

Secretary slipped on ice and twisted knee but claimed no injury until January 5 when she reinjured it the previous weekend. Then stated she had actually fallen on January 22.

Absorption of radiations, caustics, toxic and noxious substances

EE was driving water truck and was stung by a wasp on the right side of his back. Reported incident to supervisor of possible allergic reaction. EE self administered epinephrine injection @ 12:50 PM and went by doctors office on his way home.

After loading a truck from 411 bin, returning to mill entrance he slipped and fell on ice, catching himself with his right hand and arm palm down.

LEFT ANKLE TURNED UNDER STEPPING DOWN FROM HAMMERMILL LOADING STEP, FELT SOMETHING ""SNAP"". NEXT MORNING ANKLE WAS SWOLLEN & PAINFUL - SOUGHT MEDICAL ATTENTION AT THAT TIME.
