Employee was performing a routine pre-shift inspection on an excavator when he lost his balance. He reached out to stabilize himself using the machines air pre-cleaner filter housing. The pinky finger was the only part of the hand to make contact with the surface resulting in a tear in the skin requiring multiple stitches and a fracture to the upper portion of the finger.

On May 18th the main sand plant generator shut down on high coolant temperature. The site glass for the radiator did not show any fluid in it. The plant operator got antifreeze to fill the radiator. He attempted to removed the after cooler radiator cap and the pressure caused some antifreeze to be sprayed onto his left forearm. He was treated for a chemical burn.
While getting into the cab of our Cat D8 Dozer to check the seatbelt for proper operation. The employee opened the door while standing on the platform and the door opened all the way. The employee lost his balance slipped off the machine and fell six feet to the ground striking his face. He was taken to the emergency room for treatment of multiple lacerations and fractures.

While assisting a co worker repair the feeder. The co worker was performing grinding activities. The employee was wearing his safety glasses and felt something in his eye. The following day he was seen at the clinic for removal of a piece of metal and drilling to remove rust. He was allowed to return to work on 09/28/2011

While pulling on a bar EE slipped backward; lost his balance fell on the ground then bar struck EE on left side of forehead causing a cut. Landing on his wrist while falling caused a fracture in the right wrist.
The EE was picking up an air hammer when he slipped and fell. He sustained an unkown injury to his lower back.
