Employee was climbing down two steps off the ANFO rig, placed EE's left ankle on the ground (approximately 12 inches from bottom step) and it rolled when EE released the handrails on the step. Immediately reported to management and eventually found to be non-displaced fracture of the lateral Malleolus of the left fibula.
Sections of the back between 01SB-02SB and 10EB-11EB fell past the anchorage zone at some point in time and was discovered on 12/3/25. The entire area has been bermed off since 2020 so the area is considered inactive. All entrances to the entire bench cell have been barricaded against entry at this time.
The back in 4SB and 27WB fell above the anchorage and was discovered at approximately 9:30am on 11/3/25. Workplace exams from the intersection showed no hazards at approximately 4pm on 11/1/25, so the accident occurred sometime between then. The area was immediately barricaded off and it did not block travel routes or impeded escape. No miners or equipment were hurt/damaged.

Miners went underground at or around 0600 hrs, after driving through work areas, a roof fall was found at or around 0630 hrs. The area was coned off, management went and looked at area and called MSHA 1-800 number to report incident. No damage to personnel or to equipment, fall did not block travel routes or impede escape routes.
Two employees were working to shotcrete an area. The shotcrete nozzle handle became stuck and a wrench was used to turn it. The injured employee had a finger in the line of fire and left pinky was struck with the wrench causing a distal phalanx fracture.

Employee delayed reporting the incident. Injury causation determination made known on 4/5/21. EE was performing hot work at an obscure angle. A piece of hot material fell into employee's ear. 4 days post incident, the employee went to a clinic and was diagnosed with a perforated ear drum.
2020 · 1 incident
Employee was operating a CAT 745C underground hauling material from a heading to the primary crusher. While reversing to be loaded, the HT shifted due to a change in ground conditions and the employee felt pain in EE's back. The injury has been under investigation.
2016 · 1 incident
EE bent over to pick up a set of keys. When EE stood back up EE claimed EE felt a pop in EE's back. Employee was at work following incident on modified duty for 9 days before EE's claim was denied and EE was sent home on personal injury leave. Upon investigation it was determined that this injury was not caused in the workplace but it is being filed as a matter of precaution.

While testing the amp on the air hose the hose got out of control and the employee got sprayed in the face with air. Both eyes became irritated. He was seen at the local clinic and treated with antibiotic drops and cream.

Employee reports that he twisted his left knee when placing his left foot on the step of ANFO reg and step swung away causing foot to slam to ground. He was subsequently seen and treated for a knee strain.

EE in basket while raising in unit the cylinder split causing the unit to fall. EE had stitches to left shin and swollen torso.

Incident happened on 8/23/07 but was not reported until 08/25/07:ee slipped, tripped or fell while descending from haul truck. Returned to duty 08/25/07. Doctor prescribed medication for the EE.
EE was organizing a storage trailer. He was cutting open boxes with a utility knife. He set the utility knife in an open box on a shelf in order to reach for another box. While he was reaching for another box, he bumped into the original box and the knife. The box and the knife hit his left leg and gave him a laceration.

Employee states he hit a pothole causing pain in lower back. He went to the doctor for the pain. Employee was newly hired the day before the incident occurred. Prior to his being hired he had medical conditions relating to his back.

EMPLOYEE JUST GOT OUT OF VEHICLE SUBURBAN AND WAS WALKING TO THE PORTAL WHEN HE SLIPPED ON LOOSE GRAVEL & FELL WITH HIS LEFT LEG BENDING UNDER HIS BODY. LEFT KNEE BECAME SORE AND SWOLLEN. MRI TAKEN 7/20/01 SHOWED NO LIGAMENT OR CARTILEDGE DAMAGE.
