The point (tip) of the rock breaker hammer was jammed in-between two of the fingers at the primary feeder. The EE placed his hand on one of these fingers. The rock breaker hammer moved down and struck the back of the EE's hand. We brought the EE to get X-rays on the hand which came back negative - nothing broken. Hand was bruised.

Manually pulling product hose (50-80lbs.) out of boreholes 40-60 tmes a day, hurt back/neck.

THE DIAGNOSIS OF SILICOSIS IS A CUMULATIVE CHARGEABLE WORK RELATED DISEASE. THERE IS NO SPECIFIC DOCUMENTATION AS TO WHEN THE ILLNESS INITIALLY STARTED. THE EE HAS TWENTY-FIVE YEARS OF MINING EXPERIENCE AND HAS HELD VARIOUS POSITION (I.E., LABORER, DRILLER, BLASTER, FOREMAN, AND SUPERINTENDENT).
EMPLOYEE WAS STANDING ON A CATWALK AND AS A CONVEYOR BELT WAS RUNNING, A PIECE OF STEEL CAME OFF AND HIT HIM IN THE LEFT SIDE.
EE WAS WELDING A CHUTE TO A #4 CONVEYOR. AS HE WAS WELDING EE SLIPPED AND AS HE SLIPPED, HE BRACED HIMSELF AND FELT A TWINGE IN HIS BACK.
