EMPLOYEE WAS LIFTING A GUARD WHEN EMPLOYEE FELT SOMETHING POP IN ELBOW. EMPLOYEE REFUSED MEDICAL TREATMENT UNTIL MARCH 30TH AND NOW EMPLOYEE WAITS FOR SURGERY.

Employee was cleaning up the bone yard and dragging a piece of angle iron, tripping over a pipe and injuring his left shoulder. He sought medical treatment on the same day and was released to regular duty. 10/7/15, we find that he went to the clinic again and was given restricted duty w/o the knowledge or approval of his case manager. Still has not contacted CM.
Employee states that what started as a minor back ache has turned into nerve problems and muscle spasms. He thinks it's from operating the loader. He has no memory of when it started.

Employee was pulling the lever to dump the load and sprained her wrist.
Employee was getting the loader tire chains out and strained his back.
Employee was retrieving metal from the bone yard when he fell injuring his hip and chest.
Employee was unloaded a piece of conveyor when he rolled his ankle, falling and hitting his cheek on the truck and hurting his back. On 4/24/14 he notified his manager that he had gone to the ER because he was having pain in his ankle/foot. It is fractured.

Employee was sitting in a Cat 988 with the window open. When he went to shut the window, the wind caught it, slammed shut, and cut the tip of his right little finger off.
Employee was pulling on a drive shaft and hit his elbow.
This employee refused medical treatment until now. Lost time began 9/13/12 and he will be going to a specialist. He was working with a concrete saw 8/27/12 and alleges injury to his low back.
Employee was using a track torch to cut a piece of steel. When the cut was finished, the piece fell and recoiled and hit him in the temple causing a laceration.
Employee was riding in skid steer all day and states the bouncing around jarred his back.

Approx. 3:20, a grass wildfire was discovered on mine property. It had started on another property and spread onto mine property. Fire Department extinguished fire. No mine structures or people were endangered by the fire. MSHA inspector was already at the site when the fire was discovered.

Employee was using a wrench to replace a hoist cylinder on a Caterpillar 980F loader. The wrench slipped, smashing finger between cylinder and wrench.
Injury was cleaning the catwalk and steps. He slipped and fell, catching his hand on handrail. His little finger on left hand was fractured.
EE strained his shoulder while lifting a brake hub.

Injury was working on a 990 loader. he went to step forward and missed the step. He fell forward but caught himself with his right arm. He injured his right shoulder and back. He did not seek medical attention until 2 Feb 2009 and began working restricted duty 3 Feb.
Employee reported left ankle/foot pain from operating loader. He did not report this until 1 October 2008 and did not seek medical attention and start light duty until 2 October 2008.
Time started shift is unknown. An employee found victim with RK Trucking parked at front gate. He said he had caught his ring on his truck & was waiting for wife to come get him. Probably happened in tarping area. His condition/details are unknown. Any personal information on him is unknown. Don't know if system will let me leave blank or force me to list something.
EE was positioning a piece of beam for welding. His left index finger was smashed between beam and saw horse. The tip of his finger was amputated and re-attached.
Employee reported on 2-4-08 that a few weeks before he had injured his left wrist and hand from running loader all day. He went to doctor 2-5-08 and began working light duty.

Employee complained of hand pain from repetitive use while operating loader. He did not begin working restricted duty until 22 Aug 2007.
Employee was changing oil filter on Caterpillar 980G loader. The filter wrench broke causing him to fall backward and injure his left side. He is working restricted duty.
Employee was dismounting a skid steer loader. The area was wet and he slipped and landed on his right knee. He sought medical attention 7-10-07.
Employee was changing screens on 5 x 10 grizzly and mashed the ring finger on his right hand between screens. No lost time, no restricted duty, three stitches were required.
Employee was lifting a piece of steel plate and injured his left thumb. He did not seek medical attention until 20 March 2007 and began light duty 21 March 2007.

Crew shut down because of bad weather. They had waited 45 min for storm to pass but it showed no indication of stopping so the helper got out of work truck and went to the rig & turned off the master disconnect. As he was getting back into the work truck, lightning struck the work truck. Helper's right arm was hurt along with burn blisters on both feet.
Employee was replacing wear plate on a loader and strained his back. He is working restricted duty.
EE's right elbow and arm began hurting after operating loader controls. (This was EE's first day back after carpal tunnel surgery on left wrist. EE was working restricted duties - no shoveling.)
Employee was removing adjustment bolts from the cone crusher with a 1-inch air impact. The air hose connection broke and the hose struck ee's right knee. He did not seek medical attention until 02/23/2005 and began working restricted duty 02/23/2005.
On 02/07/2005 EE was shoveling and struck a hard buried object which jarred his right shoulder. He did not report this until 02/10/2005 when he further injured his right shoulder while swinging a sledgehammer. He sought medical attention on 02/10/2005 for a strained shoulder and began working restricted duty on 02/11/2005.
Employee was operating a 990 quarry loader in the quarry when the loader hit a solid toe in the quarry floor. The loader shifted suddenly and the sudden movement strained his back.

Employee experienced numbness in his left hand while operating the controls of his loader. He has been working regular duty but will begin losing time 7 March 2005 for carpal tunnel surgery.
Employee was welding. While moving a piece of I-beam, he strained his back. Employee is working restricted duty.

EMPLOYEE WAS SHOVELING MATERIAL IN THE TUNNEL AND FELT PAIN IN HIS LEFT SHOULDER. HE SUFFERED A SHOULDER STRAIN.
EMPLOYEE SUFFERED PAIN IN HIS RIGHT THUMB AND WRIST WHILE OPERATING A FRONT ENDLOADER. HE SOUGHT MEDICAL ATTENTION ON FEB. 4, 2004 AND WAS DIAGNOSED WITH TENDONITIS. IT WAS LATER DETERMINED TO BE WORK-RELATED.
EE strained his shoulder while placing rails on a retaining wall. EE did not seek medical attention until 2 February 2004 and began working restricted duty on 3 February 2004.
EE WAS WORKING WITH OTHER EMPLOYEES TO STACK RAILROAD TIES ON A RETAINING WALL. HIS THUMB WAS SMASHED BETWEEN A RAILROAD TIE AND THE SUPPORT BRACE ON THE WALL. HE RECEIVED APPROXIMATELY 25 STI TCHES IN HIS THUMB. THERE WAS NO LOST TIME DUE TO THE INJURY.

EE INJURED HIS BACK WHILE SHOVELING WASTE MATERIAL UNDER THE R-1 CONVEYOR BELT IN THE SURGE TUNNEL.
EMPLOYEE WAS OPERATING A CAT 990F LOADER IN THE QUARRY. THE LOADER STRUCK A TOE IN THE QUARRY WHICH CAUSED EE'S SEAT TO BREAK. HIS BACK WAS INJURED. THIS ACCIDENT DID NOT BECOME REPORTABLE UNTIL 9-15-03 WHEN HE SOUGHT MEDICAL ATTENTION.
EE WAS LOADING JUNK ROLLERS INTO A LOADER BUCKET. WHEN HE LIFTED A ROLLER, HIS BACK WAS INJURED.
EE WAS OPERATING A CATERPILLAR 990 LOADER WHEN HIS BACK BEGAN TO HURT.
EE WAS STEPPING DOWN OFF A PLATFORM. HIS LEFT FOOT WENT INTO A HOLE AND HE FELL TO THE GROUND.

WHILE HELPING BREAK OUT A JOINT OF STEEL ON THE DRILL, THE BREAKOUT WRENCH SLIPPED OFF, HIT THE INJURED'S HAND AND CUT IT REQUIRING STITCHES.
