Employee was checking voltages and fuses on an electrical switch at the main plant. While checking the fuses, he had the meter setting on Ohms. When he started checking voltage, he forgot to change the meter's setting. When he checked the voltage, the phase to phase contact caused an electrical flash when the meter's probes came in contact with the current.

Driller was loading empty barrels into the back of the work pickup truck. While in the process he felt a twinge in his lower back. Over the course of the next week and a half, his back started to get worse by the day. Finally he went to the doctor on 12/9/04 who said his back was strained and out of place so he popped his back and prescribed muscle relaxers.

WHILE THEY WERE PUTTING A NEW BEARING ON, THE TORCH FELL AND STRUCK INJURED, KNOCKING OFF HIS HARD HAT AND SAFETY GLASSES. THE TORCH STRUCK HIS RIGHT EYE.

THE RETAINER PIN THAT HOLDS THE STRIKER PIN IN PLACE ON THE ROCK BREAKER BROKE. EMPLOYEE ATTEMPTED TO MOVE THE APPROXIMATELY 250 LB STRIKER PIN BACK INTO PLACE AND STRAINED HIS BACK. HE SHOULD NOT HAVE ATTEMPTED TO MOVE THIS HEAVY OF AN OBJECT WITHOUT ASSISTANCE. A HOIST SHOULD HAVE BEEN USED. HE IS LOSING TIME AS OF 10/15/03.
