Employee was removing a rock that was lodged and the saddle came back and jammed EE's finger. The injury wasn't recordable until they found a fracture and put EE in a rigid brace on 11/19/24.
Employee was removing handrailing from deconstructed conveyor. Employee was cutting support bolts from the top of the handrail using a torch. When the second bolt was cut, the handrail had stored energy and popped free. The handrail then came in contact with employees forearm, causing a laceration.
Employee was jumping up and down on the bumper trying to release the trailer from the hitch and sprained EE's ankle
EE was on EE's knees pushing the new screens into the bottom deck and EE's knee popped.
EE was working on c3 tail pulley guard with the contractors. EE squatted down to put rubber guarding on the frame. When EE stood halfway up, EE's knee popped and EE dropped back down.
Employee was swinging a sledgehammer and felt a pop in shoulder
Employee stepped off trailer onto uneven ground. Just had surgery on 12/1/24
Was putting railroad ties on a trailer to set part of the jaw crusher on, EE miss placed EE's foot and step off the trailer. With doing so EE hit the ground and a railroad tie landed on the center of EE's ribs.

Employee was working with other employees to replace a tail guard on a conveyor. The forks fell off the skid steer loader causing the guard to shift and hit employee's left hand. His middle finger on his hand was cut requiring ten stitches.
EE was lossening the 7' cone crusher. He wrapped cable around the bowl and had the loader operator pull on the cable. He put the cable over a 2-inch pin. The bin pent, letting the cable come loose and hit him in both lower legs.
Customer, EE, had a heart attack and passed away while on mine property.

Employee was working with another employee to tighten the jaw crusher when the toggle plate came out and struck him in the head. He was wearing his hard hat and the band inside the hat cut his forehead. Stitches were required.
EE WAS CUTTING CHANNEL IRON FOR THE QUARRY CON- STRUCTION PROJECT. WHEN HE CUT THROUGH THE IRON,IT BOUNCED UP IN THE AREA AND CAME BACK DOWN ON HIS RIGHT HAND. HIS RIGHT THUMB WAS FRACTURED . EE WAS OFF WORK ONE DAY 3-11, WORKED REGULAR DUTY 3-12, AND THEN WAS PLACED ON RESTRICTED DUTY BEGINNING 3-15 WHEN THE FRACTURE WAS DISCOVERED.

EE WAS WORKING WITH ANOTHER EE TO REMOVE THE V- BELT GUARD ON THE 5/8" CHIP CONVEYOR. THE GUARD SLIPPED AND CUT HIS RIGHT HAND. THERE WAS NO LOST TIME OR RESTRICTED DUTY BUT STITCHES WERE REQU IRED.
EE WAS WORKING WITH OTHER EE TO INSTALL A NEW WATER LINE AT THE 7' CONE CRUSHER. HE WAS USING A POCKET KNIFE TO CUT THE PIPE WHEN THE KNIFE CLOSED ON HIS FINGERS.
EE WAS CHANGING TEETH ON THE BUCKET OF A CATERPILLAR 990 LOADER. WHEN HE STRUCK A TOOTH WITH A SLEDGEHAMMER A PIECE OF STEEL STRUCK HIS STOMACHSTITCHES REQUIRED*

EE WAS DRILLING A HOLE. WHEN DONE, WENT TO PICK UP THE ROTARY HEAD WHICH CAME DOWN ON HIS BACK. COWORKER CAME TO HIS RESCUE AND RELEASED HIM FROM HIS TRAPPED POSITION. HE WAS TAKEN TO THE HO SPITAL WHERE IT WAS DETERMINED THAT HE HAD A SEVERE TO HIS BACK AND HIP.
EE WAS STEPPING DOWN OFF LOADER AFTER CHECKING FILTER. THE STEP BROKE AND EE LANDED ON LEFT LEG & STRAINED KNEE.
INJURED WAS WORKING WITH OTHER EMPLOYEES TO LOAD A JAW DYE INTO A UNILOADER BUCKET. THE JAW DYE FELL OFF THE BLOCK IT WAS SITTING ON. THE DYE STRUCK HIM IN THE MIDSECTION AND PINNED HIM TO THE GROUND WITH THE OTHER PART OF THE DYE RESTING ON THE BLOCK.
EMPLOYEE WAS OPERAING FEEDER HOUSE. HE TURNED TO STEP OFF A PLATFORM AND STRAINED HIS RIGHT KNEE. HE DID NOT GO TO THE DOCTOR UNTIL MAY 30 2000.

EE WAS TRYING TO REMOVE A NUT FROM 20 IMPACT GUN. HE ACCIDENTALLY TOUCHED THE TRIGGER AND WOUND THE NUT UP ON HIS LEFT INDEX FINGER.
EE WAS WELDING A SPARE TIRE RACK ONTO THE DIESEL FUEL TANK IN THE BACK OF CO TRUCK. WHILE DOING THIS THE TANK EXPLODED. THE EXPLOSION KNOCKED HIM BACK A FEW FEET AT WHICH TIME HE BAILED OUT OF THE TRUCK TO AVOID THE FLAMES WHICH WERE COMING OUT OF THE TANK. THE FIRE WAS IMMEDIATELY PUT OUT BY ANOTHER EE. WHEN HE BAILED OUT OF TRUCK BED, HE SMASHED ELBOW ON WATER TRAILER HITCH.
EE FELL WHILE CLIMBING DOWN THE LADDER OF A LOADER
HE STEPPED OFF A 773 TRUCK ONTO A LADDER. HE MISSED THE LADDER AND FELL TO THE GROUND.
WHILE CHANGING OUT LEAF SPRINGS ON A SCREEN UNIT, HIS HAND BECAME PINCHED BETWEEN THE V-BELTS AND DRIVE SHEAR ON THE SCREEN UIT.
