APAC-Kansas, Inc. · Over-exertion in lifting objects

Dust hose to the dust collector pot became full of mud. EE removed the hose at the lower point to to clear it. The hose fell off. employee continued to unclog the hose by elevating it the middle. EE ended up moving hose off to the side to allow it time to dry out. The next day EE returned to installed it. The employee said EE did not need medical attention till 4-22-26

Employee was underneath an articulated haul truck setting jacks and cribbing. The job site was very muddy and while twisting and grabbing equipment they felt their left knee pop. Called nurse triage, they advised visiting urgent care. Doctor orders from urgent care stated employee needed to be off of the appendage and work until an MRI can be taken. MRI scheduled through work comp.

Not a miner, Commercial DOT Driver, Driver had staged truck in the front parking lot. The driver was climbing out of the truck and lost grip. Tried to catch self and separated shoulder.

Employee was walking on ground level pathway when the employee slipped on surface and fell to ground. Employee straightened left leg out and felt pop. Employee worked rest of shift. Was seen by doctor approximately 7:00pm that night and was diagnosed with a dislocated left patella. Put in brace and given prescription and was released to light duty.

EE had surgery to repair an elbow tendon tare on 1/14/16 for an incident that occurred in March of 2014. EE believed the injury to be minor at the time and did not seek treatment or report to EHS. EE reported the incident in July of 2015 and a WC claim was filed. Ins. Co. investigated the claim and accepted claim in Dec of 2015 scheduling surgery on 1/14/16. Expect 10-14 LTD.

EE WAS OPERATING A DOZER ON THE QUARRY SITE WHEN A BEE FLEW INTO THE CAB OF THE DOZER. THE EE IS ALLERGIC TO BEE STINGS. HE WAS TAKEN TO THE HOSPITAL WHERE HE WAS GIVEN AN INJECTION AND RELEAS ED.

EE WAS ATTACHING ANOTHER STEEL TO A DRILL RIG BIT WHEN HIS GLOVE CAUGHT ON THE STEEL & TWISTED HIS LEFT WRIST. HIS WRIST WAS SPRAINED, WITH NO PERMANENT IMPAIRMENT. ALTHOUGH HE REPORTED HIS IN JURY THE DAY IT OCCURRED, THE COMPANY WAS NOT AWARE HE HAD SOUGHT TREATMENT UNTIL A BILL WAS RECIEVED ON APRIL 7, 1995.
