EE WAS ATTEMPTING TO REMOVE A LOCKING COLLAR ON A SPPED REDUCER WITH A PIPE WRENCH. THE LOCKING COLLAR SUDDENLY BROKE LOOSE AND EE FELL(APPROX 5 FT) AND LANDED ON HIS LEFT WRIST AND HIP.
EE WAS WALKING AROUND FUEL TANK STEPS TO DISPENSE INTO A CONTAINER. EE LOST HIS FOOTING & FELL CATCHING HIS LEFT FOREARM ON THE SHARP EDGE OF STEPS
EE WAS LIFTING A 55LB BOX OF EXPLOSIVES INTO THEBACK OF THE POWDER TRUCK. AS HE WAS TURNING TO LIFT A BOX ONTO THE TRUCK HE FELT A POP IN THE MIDDLE OF HIS BACK JUST BELOW THE SHOULDERS ID# FOR TCI
EMPLOYEE WAS TIGHTENING THE SLACK ADJUSTER TO SET THE BRAKES ON THE TEREX HAUL TRUCK. THE WRENCHSLIPPED AND CAUGHT HIS FINGER BETWEEN THE WRENCHAND ADJUSTER. HIS FINGER WAS CUT, REQUIRINGSIX S TITCHES.

EE WAS CLEANING UP AROUND PLANT WITH A CASE UNI LOADER AS EE DISMOUNTING LOADER. HE BUMPED HIS LEFT SHIN. EE WAS NOT SURE OF EXACT XXX. AS TIME WENT ON EE NOTICED HE HAD FLUID ON LEFT KNEE . EE WENT TO DR ADN FLUID WAS REMOVED. DR SAID THAT BUMP ON SHIN MAY HAVE CAUSED FLUID TO FORM ON KNEE.
EE WAS DELIVERING COMPUTOER DISCS TO SCALE HOUSE.AS EE RETURNED TO PICKUP HE STOPPED AND COMPLAINED THAT HIS BACK WAS HURTING. ANOTHER EE WITNESSED EE RUNNING TO PICKUP. WHEN EE WAS ASKED WHY HE WAS RUNNING, EE REPLIED THAT THERE WAS NO REASON EE TO BE RUNNING BACK TO PICKUP.
EE WAS WORKING CLEANING UP PARTS ON BENCH GRINDER. HE WAS WEARING HIS SAFETY GLASSES. SOMEHOW A SMA;; PIECE OF WIRE MUST HAVE CAME OFF AND GOT BEHIND HIS GLASSES. THIS SMAL PIECE EVENTUALLY WA S FOUND FLOATING AROUND IN HIS EYE. IT WAS REMOVED BY A DR. EE WAS GIVEN EYE DROPS FOR THE DISCOMFORT. EE DID NOT MISS ANY TIME OFF.
#1 NOT SURE IF EE DID THIS AT WORK OR NOT. #2 EE DOES A LOT OF MOUNTING AND DISMOUNTING ON SKID LOADER AND EUCLID TRUCK. POSSIBLY CLIMBING ON AND OFF. EE NOTICED DISCOMFORT IN BACK. WENT TO DR 'S & WAS PRESCRIBED IBUBROFEN. EE'S PAIN IS FOR THE MOST PART GONE. WE DID NOT HAVE TO GO BACK TO DR. NOT SURE IF THIS IS REPORTABLE BUT WILL JUST BE SURE.
EMPLOYEE WAS NOT SURE THIS WAS A WORK INJURY. HE THOUGHT MAYBE HAPPENED NEAR THE TIME HE WAS HELPING LOAD A SHOT. HELPED MOVE 50 LB BOXES OF TOVAN SUPER 70. THE PHYSICIAN JUST DETERMINED IN FE B THAT MAY HAVE BEEN WORK RELATED.

EMPLOYEE WAS BULDING UP THE WORN IMPELLER BARS IN THE CEDAR RAPIDS IMPACT BREAKER. HE WAS WELDING WITH HELMET DOWN & RAISING IT UP TO CHECK WORK. THIS WAS IN TOTAL DISREGARD TO HIS OWN SAFETY. REFLECTION OF THE ARCH IS WHAT DIRECTLY CAUSED THE SLIGHT BURN TO HIS FACE. HELMET WAS IN GOOD CONDITION.EMPLOYEE RECEIVED A FULL RELEASE TO RETURN TO WORK.
