THE VICTIM WAS IN THE PROCESS OF DUMPING HIS LOADED TEREX TRUCK AT THE WAST DUMP. THE TRUCK TRAVELLED OVER THE EDGE AND THE DRIVER WAS FOUND PINNED UNDER THE LEFT FRONT TIRE.

THIS SMAL PIECE EVENTUALLY WA S FOUND FLOATING AROUND IN HIS EYE. IT WAS REMOVED BY A DR. EE WAS GIVEN EYE DROPS FOR THE DISCOMFORT. EE DID NOT MISS ANY TIME OFF.

EMPLOYEE WAS BULDING UP THE WORN IMPELLER BARS IN THE CEDAR RAPIDS IMPACT BREAKER. HE WAS WELDING WITH HELMET DOWN & RAISING IT UP TO CHECK WORK. THIS WAS IN TOTAL DISREGARD TO HIS OWN SAFETY. REFLECTION OF THE ARCH IS WHAT DIRECTLY CAUSED THE SLIGHT BURN TO HIS FACE. HELMET WAS IN GOOD CONDITION.EMPLOYEE RECEIVED A FULL RELEASE TO RETURN TO WORK.
