THE VICTIM WAS IN THE PROCESS OF DUMPING HIS LOADED TEREX TRUCK AT THE WAST DUMP. THE TRUCK TRAVELLED OVER THE EDGE AND THE DRIVER WAS FOUND PINNED UNDER THE LEFT FRONT TIRE.

EE WAS ATTEMPTING TO REMOVE A LOCKING COLLAR ON A SPPED REDUCER WITH A PIPE WRENCH. THE LOCKING COLLAR SUDDENLY BROKE LOOSE AND EE FELL(APPROX 5 FT) AND LANDED ON HIS LEFT WRIST AND HIP.

EE WAS WORKING CLEANING UP PARTS ON BENCH GRINDER. HE WAS WEARING HIS SAFETY GLASSES. SOMEHOW A SMA;; PIECE OF WIRE MUST HAVE CAME OFF AND GOT BEHIND HIS GLASSES. THIS SMAL PIECE EVENTUALLY WA S FOUND FLOATING AROUND IN HIS EYE. IT WAS REMOVED BY A DR. EE WAS GIVEN EYE DROPS FOR THE DISCOMFORT. EE DID NOT MISS ANY TIME OFF.

WENT TO DR 'S & WAS PRESCRIBED IBUBROFEN. EE'S PAIN IS FOR THE MOST PART GONE. WE DID NOT HAVE TO GO BACK TO DR. NOT SURE IF THIS IS REPORTABLE BUT WILL JUST BE SURE.

EMPLOYEE WAS BULDING UP THE WORN IMPELLER BARS IN THE CEDAR RAPIDS IMPACT BREAKER. HE WAS WELDING WITH HELMET DOWN & RAISING IT UP TO CHECK WORK. THIS WAS IN TOTAL DISREGARD TO HIS OWN SAFETY. REFLECTION OF THE ARCH IS WHAT DIRECTLY CAUSED THE SLIGHT BURN TO HIS FACE. HELMET WAS IN GOOD CONDITION.EMPLOYEE RECEIVED A FULL RELEASE TO RETURN TO WORK.
