EE suffered a low back injury when a boat flipped over in the water. Boat came to rested on dredge pipe EE retrieved by fire/rescue. EE went to a clinic/diagnosed w/a contusion to the back. Released with restrictions did not prevent normal job. Full duty 5/5. 6/5 EE provided supervisor with a Psych Eval removing EE from duty due to PTSD due to the 4/17 incident.

Miner was repetitively operating a chain hoist which re-injured EE's right shoulder. EE reported it 5/8/19 when EE started having pain. EE saw the specialist on 5/14/19.

A tire was stuck in the cutter head of the dredge. Employee was cutting the tire with a grinder when the grinder kicked back and struck employee in the right palm; causing a laceration approximately 1 1/2 inches. Employee was wearing gloves and grinder was in good operating condition. LED lighting on the dredge was all functioning.
Injury became reportable on 04/02/18 when EE had surgery. Employee was digging a headline cable that was buried in the sand and did not release pressure on the winch. Employee went to step over the cable, when it released from the sand it caught EE's leg it flipped EE. EE landed on right shoulder.

Mechanic was pulling on a winch cable, with his life jacket on. As he strained to pull the cable, the life jacket strap became overly tight and broke a rib on the right side of his chest.
