Employee had breaker bar propped against jaw die trying to hold back up to take bolts off of the jaws toe protector. The breaker bar slipped off the jaw die and swung around and hit EE in the arm.

Employee was shoveling rock from the walkway and twisted EE's back

EE was running fuel and water to drill 569. EE was pulling pump out of water trailer when EE fell backwards and hit the ground. Slipped and fell off water trailer. Hurt right shoulder.

EE was lifting up a grinder and touched the trigger on it and it cut EE's forearm.

Employee was changing screens and felt a pop in EE's right elbow. No restrictions until EE's surgery date of 6/11/2020.

Employee was working on the jaw crusher when they got a hand too close to the motor drive sheave and got a finger pinched by it.

Employee was changing V belts when the guard fell. When EE tried to catch it, EE strained EE's back. EE sought medical attention 6/28/18.

Employee was operating a loader in a stockpile when the bucket struck a large rock and caused EE to whip forward in the cab. EE refused medical treatment at the time but went to be seen 2/7/18 for a left shoulder strain.

Employee was getting a drill core out of the tube and struck finger with a mallet. Employee received 2 stitches.

Employee was carrying a piece of steel and when ee went to lift it to the deadbox, ee felt a pull in shoulder.

Employee was fusing pipe when the pipe came down off the hook and struck EE in the face.

Employee was cutting a sheet of steel and the piece the employee cut off dropped on their foot.

Employee was operating a skid steer when the blade hit a rock, jarring EE inside the skid steer. EE later reported that their back was injured and went to the clinic on 8 July 2016 after 5p.

Employee was operating an excavator when he noticed something fall from the boom. When he was exiting the cab, he got his foot hung up in the track, fell, twisting his ankle.

EE WAS GETTING UP FROM DESK & TRIED TO CLOSE THETOP DESK DRAWER. HIS FINGER WAS CUT ON THE SHARPEDGE ON THE BOTTOM CORNER OF THE DRAWER

DRILLING HAMMER ROLLED ONTO FOOT. WHIEL CHANGING BIT HAMMER WAS SET ON ITS SIDE AND UNEVEN GROUND.

WAS PUSHING ROCK OUT OF CONVEYOR BELT HE WAS BENTO OVER & RIGHT FOOT SLIPPED WHEN HE STOOD UP HIS BACK WAS HURT.

USING WRENCH ON ROCK CRUSHER, FINGER PULLED.

HE WAS THROWING ROCKS OUT OF THE FEEDER AND CAUGHT FINGER ON THE CONE, BROKEN HAND.

MASHED FOR FINGER ON RIGHT HAND. NO DAMAGE TO THE HAND.
