Employees noticed the round door for the dust collector was not sealed. Since they could not close the door properly, the control room was called to shut down the fan and close it properly. An EE was standing in front of the dust collector door when the fan was shut down. Immediately, positive air pressure built up and shot the door out, hitting the EE on the face.

IE stated that when disconnecting a hose from an air compressor, the fitting and whip check blew out the end of the hose striking the IE on the lower leg

Employee was performing maintenance involving removal of a hydraulic cylinder on a bulldozer. While loosening a fitting using an impact gun, residual hydraulic pressure was released, causing a high-pressure spray of hydraulic fluid to contact the employee's head, arms, chest, and shoulders. Employee experienced tingling and was evaluated in the ER; no serious injury was found.

While an employee was attempting to charge a hydraulic valve, the fitting/connection failed on the lower hookup and there was a pressurized release of force. The employee sustained injuries to their left ear, where a small piece of metal became lodged and was removed with minimal force. The employee also injured their left hand, but no fracture was identified in the x-ray.

Injured employee was cleaning belt water sprays in a spray bar. They brushed against a fire tap at the location they were working, between the top & bottom belt. When they backed out of the belt and stood up, the fire tap valve suddenly blew off the 8" waterline coupler and struck their left ankle, breaking it. Accident caused by a failure at the coupler tee due to corrosion.

Employee opened an angle cock valve on the back of a rail car to bleed pressure. while opening the valve the hose whipped up and struck their elbow.

At approximately 11:55am, the Line 3 scrubber drop out box ruptured releasing large volumes of water onto the floor. The employee was likely north of the launder when the water swept them down the inclined walkway wedging them beneath the kiln pier stairs.

An OTR technician at a quarry installed two spare tires and a new wheel on a Komatsu WA900 loader. While demounting the scrap tire/wheel in the storage yard, EE removed the O-ring with EE's left hand. Residual pressure caused the bead seat band to spring upward, striking EE's left thumb and index finger. This amputated the tip of EE's left index finger and crushed EE's left thumb.

The employee responded to a high-temperature alarm on the impactor crushers displayed on the HMI. As part of normal correction, the employee proceeded to the impactor building to activate the air horns for each impactor unit. The employee then proceeded to BIC2 and opened the air horn valve. A loose hose end became pressurized and struck the employee in the left eye.

Operator used air pressure to clean a line and opened valve before bleeding pressure. Air pressure burst a catch trap and oxalic acid with debris hit operator's face knocking safety glasses off.

Offloading Ammonia Nitrate Solution, the hose was clogged. Employee closed the line to check the hose but there was still pressure in the line. When they unhooked the line the pressure sprayed solution hitting them in the face and entering eyes. They washed their eyes out well onsite but went to an eye doctor to be checked out and they prescribed antibiotic ointment

EE opened an air line to bleed the system and forgot to turn off a ball valve at the air tank causing the air line that runs under the building to come up and out of the hole and hit EE in the head.

while inflating a bag to separate granite piece from wall the bag blew up causing the rope and hose made contact with EE's hand and caused an injury

REPORTABLE DUE TO SUTURES - Employee was working on an eight-inch Aqua-mine discharge pipe on NE-9 in the #1 Entry at XC #50 when it burst striking the employee on the left side of the body causing a laceration to the left ear and a laceration to the left upper arm which required sutures to both locations.

Operator was having an overheating issue with the equipment. They shut down the machine to investigate. They opened the coolant reservoir which had somehow been pressurized. As a result, the hot coolant burned the left arm of the operator. The extent of damage is limited to their left arm.

Incident occurred while installing a new pressurized air system.

(REPORTABLE DUE TO SUTURES) Employee was working on a 4" air line at 4-7 Fan compressor station when a 4" VIC coupling / bull hose blew off and struck EE causing a laceration to EE's left upper leg which required twelve (12) sutures.

Operator was running pressure washing and fitting to hose came apart and struck operator in the mouth area causing a cut to their lip area resulting in 10 stitches to the lip area.

Employee was observing the removal of a joint of 3" waterline. When the staple was removed, the hose blew out under pressure. The hose struck the employee in the mouth and head.

THE MINER WAS WORKING ON AN EXCAVATOR, WHILE REPLACING A HYDRAULIC HOSE AND WHILE LOOSENING THE HOSE, THE PRESSURE BUILT UP AND THE FLUID HIT THE MINER IN THE FACE AROUND THE LOWER LIP AND CHIN AREA.

Employee was at tool car getting a tool when another employee was filling degreaser sprayer. The hose on sprayer popped off and the liquid got in their eyes. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee had finished bolting the #6 entry on the #4 unit and was standing beside the roof bolter between the dust box and the boom when a hose fitting broke causing the employee to get oil in their eyes. Employee was treated for a scratch to their eye.

Air hose blew off barbed end of quick connect striking employee in left elbow causing a laceration

Employee was loosening a concrete pipe coupler to clear a plugged concrete pipe. When the IE pulled the latch on the concrete line coupler the pressure build up caused the coupler to unexpectedly blow off the line causing the flex line that was coupled to the hard line to strike the employee on top of the right foot causing a laceration and a fracture.

The employee was fueling Haul Truck 11 with Diesel Fuel when the hose popped off and splashed the employee in the eyes, face and chest. The operator was wearing PPE during the time of fueling.

EE completed putting pump PP131 online. They walked by pump and the spool piece around the intake started spraying hot slurry. Hit them on right arm and right side. EE reported injury, seen by on-site EMS, went back to work. EE went to clinic on 8/23, diagnosed with cellulitis. On 8/26 follow-up put on restrictions. Site offering modified duty. Became reportable on 8/23.

Water pump was not pumping and employee attempted to opened the drain valve when the pipe that is attached to the valve broke causing hot water to spray out.

While EE was removing the inner cap to change a conveyor torque shaft, oil, under pressure, came out of the conveyor pot and covered EE's arms, hands and a few places on EE's face. This caused burns and blistering.

Miner picked up can of break cleaner out of service truck walking to back of truck and slipped on some rock causing miner to start falling. Miner reached out with right hand to stop fall causing break cleaner can to hit rear bumper of truck. The can exploded in miners hand causing lacerations to EE's index and middle fingers.

Employee was working on air grease gun and line blew hitting them in the eye.

Slurry pump B feeding the wash plant, plugged. After shutting down, the technician opened the pump drain on bottom of the pump housing. Due to the head pressure, water sprayed out of the drain. Since the pump was plugged, the water was extremely hot, burning the technicians arm. The technician was treated for second degree burns on left forearm at the hospital.

While using a high pressure Jetstream system, the water/air did not completely dead head upon releasing the foot pedal controlling flow, the wand in use became dislodged from port of preheater and stream struck employee on the back of the left leg (knee and calf area).

Miner attempted to puncture aerosol can on vice at which time the can exploded, lacerating EE's face and chipping a tooth requiring stitches and outpatient cosmetic dentistry

Ee was checking a piece of equipment, going to take cap off radiator. The cap popped and coolant splashed on ee's right wrist/forearm. Seen at ER, diagnosed with burns, released to full duty. On follow-up at clinic, released to restrictions and referred for therapy. Site can offer modified duty.

Pulled end flange off refuse screen and was hit with water. Requested medical attention and an MRI. Conducted surgery on knee on 9-13-2025 and was first day of lost time for employee.

Employee was repairing a hose and accidentally hit the valve causing water from a pinhole in the hose to spray into EE's left eye.

Employee drove out into the field to fix a machine. When they got to the machine, the service truck they drove was smoking. Employee went to open the hood to examine the engine to determine the cause of the smoking, but when trying to unlock the hood, the antifreeze sprayed their left hand, causing a second-degree burn on index and middle fingers.

Employee was working on Truck AC when hose and clamp on high pressure side blew off striking EE in the forehead.

Employee was attempting to unstop a water pump discharge 4" line. After leveling the line the employee was stepping away from the line when the coupling broke apart causing the water and other material from inside the discharge line to contact the right side of EE's face, neck, and back causing superficial cuts to the skin.

Employee was greasing track on excavator when the grease hose popped off and struck them on their left hand and caused grease to go under their skin.

Disconnecting a hose to stop a leak in the connection between the hose and customer's plumbing to a sulfuric acid storage tank. Back pressure from the customer's plumbing caused residue in the hose to spray the driver.

EE was removing a valve stem cover plate. After removing the first 4 bolts, they proceeded to remove 2 more. As EE was removing the second, the knife valve split apart, causing slurry to shoot out the side. Slurry came into contact with the EE. EE sustained abrasions and multiple contusions. EE was issued restrictions.

Defective Radiator cap blew off water pump resulting with Burns on employees right side (Arm, armpit, back, stomach area).

The injured employee was attempting to blow argon through the VOCL3 transfer line to clean out the system's transfer pump. When they charged the line with argon, it ruptured the filter housing, spraying hot VOCL3 over the lower half of their body, burning their left ankle.

Employee was heating up a keyway to remove from a valve bushing. In the process of doing so the key popped and hit the employee in their right forearm.

Employee turned a valve on at the #267 shield on the 7W Longwall face to pressurize a 3/4" hydraulic line supplying pressure to a Z-drill outby the face in the tailgate entry. When EE turned the valve on, it broke at the stem where it entered the connection port and struck EE on the right side of EE's face. Suffered a small fracture, contusion to eye and 3/4" cut requiring sutures.

The operator replaced the 612 A strainer and then proceeded to switch the 612 B Strainer. While turning their back to switch the strainer valves, a loud noise occurred, causing slurry to spray out of the strainer housing toward the operator. This incident caused the operator's hard hat to come off & them hitting their head on the beam above.

Miner exited machine, an aerosol can of class cleaner was setting on top of the engine compartment, when miner picked up the spray can, it exploded. Miner received miner abrasions to face. Was checked out on side and at hospital

Employee was filling the outby watering tank, when the pressure in the hose changed causing the hose to rise out of the tank and contact the right side of the employees mouth resulting in 5 sutures.
