THE EMPLOYEE WAS OPERATING A BULLDOZER ON A COAL STOCKPILE WHEN FOR UNKNOWN REASONS THE BULLDOZER FELL INTO A FEEDER HOLE AND WAS COVERED WITH LOSOE COAL, FILLING THE OPERATORS CAB.
