An unintentional roof fall was discovered 12:00pm, No Person or Equipment involved. In the #2 Entry-spad 267 on 1st North Mains in the Return. The fall was appx. 20' wide X 25'-30' long and 8' thick.
EE was helping to get the Continuous Miner blocked up to remove planetary when chain ratchet came loose hitting EE's left jaw and shoulder. EE had x-rays and was released to RTW Full Duty NO LOST DAYS

An unintentional roof fall--Off the Section-Spad #52-Old #1 Belt at Brk 13 in #5 entry-Intersection in OLD travelway-fell 18' long X 12' wide.
While riding underground the man trip ran over a rock and EE hit his head on the top. EE continued to work until 7:00 then decided to get it checked at the ER at Logan Regional.
EE was tramming scoop, ran over a piece of structure in roadway-it kicked up and hit his right elbow causing bruise. EE: worked light duty 2 days-RTW full Duty 10.18.13.
EE was loading bolt supplies and said he felt pain in lower right side
EE was working on roof bolter tire, taking off lugs-- the socket extension broke and EE fell into the metal frame cutting his bottom lip-sutures required.

An Unintentional roof fall was discovered at 10:55am--One X-Cut inby spad #164 in the #7 Entry, 1,300'from the mine portal. The fall was approx.19' wide x 30' long x 8' thick. No Person or equipment were involved. This area was bolted with 3/4"" x 60"" fully grouted bolts.
EE was placing a jack under the planetary of the Miner, it fell and caught the right hand. Ring and Pinky Fingers were bruised. Note: Metatarsal Gloves in Use

The size of the fall is appx. 8 ft thick X 20 ft wide X 20 ft long in the intersection. The area was bolt with a 60"" X 3/4"" rebar bolts. Fall was discovered at 8:47AM
EE was getting out of the Scoop when he felt pain in back-EE refused ambulance and drove himself to hospital
EE was trying to move a 5 gal bucket to pick up a dropped solaris spotter. While attempting to move the bucket EE stated that he had hurt his neck-back and had pain in bolt legs. EE was transported to Logan Hospital via Ambulance

EE was attempting to cut a piece of belt with a box cutter, it slipped and cut his right wrist. Required 3 stitches.
A roof fall was discovered in #7 entry in the intake, it begins appx. 100 ft inby Spad 172...60ft long, 18-20 ft wide, 5-6 ft thick. Approval has been given to move the intake stopping line and primary escapeway to allow for the intake air course to bypass the fall. The inby end of the fall will be cleaned and supported according to the approved clean up plan.
EE was putting water in 6 volt battery and got fluids in eyes and face.
Traveled Return Entry A fall in #2 entry. Airway was NOT blocked. Travel ways was routed to the #1 entry. The area of the fall was dangered off. MSHA/State was notified via hotlines. Area was then cribbed, roof jacks set and Danger Boards were put in place. The fall was 4-5 feet thick 20 - 25 feet long- 18 feet wide. safety talk with all three shifts.

Employee was working on equipment in mine yard. When he went to get his tools, he fell over some metal stacked on the ground.
Employee was lifting a shuttle car tire to get it in position to roll to the repair site, the tire was on its side next to the rib. When attempting to lift the tire his foot slipped in loose rock next to the rib causing him to slip, at this time he felt extreme pain in his lower middle back.
A roof fall occurred #5 Entry--spad #185, 20ft x 20ft x 5ft. Timber and Cribs were set.
Scoop operator was taking the scoop to the charger, it quit tramming in forward-put in reverse-stuck in tram, went to hit Panic Bar, scoop hit rib. Threw him partially out-got hand behind the rubber stopper. Mashed his little finger. His little Left hand finger required stitches.

Brian made sure that there were no other hazards and that all men were accounted for...Both Federal and State Offices were contacted at appx 1:40 am--the fall was 20 x 20 by 5ft thick. The fall was cribbed at the intersection
Employee was cutting a hose and the knife slipped and jabbed his left hand between the thumb and forefinger.
