The employee was setting standing support when EE turned EE's back on a standing unwedged timber to grab EE's hammer. The unwedged timber fell, striking EE in the head.

An unplanned roof fall at or above the anchorage zone occurred on the active working no. 3 section, 2 breaks inby spad no. 6687.

Employee was stacking supplies on the roof bolting machine when a piece of draw rock fell from the top and struck them on the thigh. The employee was injured on 6/11/2025 and was treated, released, and returned to work until 6/21/2025, when they revisited the hospital and was written off work.

Employee stepped down from the roof bolter when EE felt a pain in EE's leg, causing EE to fall.

Employee stepped off the back of a walk-through bolter and rolled EE's ankle.

An unplanned roof fall was discovered inby spad no. 548 in the no. 8 entry between breaks 19 and 20. Another roof fall above anchorage was discovered on the same date and time as the previous fall inby spad no. 534 in the no. 6 entry between breaks 17-18.

Employee was throwing crib blocks and got finger caught between crib and miner causing injury.

Employee was riding inside rail man trip and was not paying attention to low clearance signage. The employee raised EE's head as the man trip approached a ventilation overcast. The employee's head came in contact with the overcast pushing EE's hard hat down causing a laceration.

Employee stated was lifting cable and strained back.

Employee was moving the feeder and was hit in the face with the wash down hose causing injury.

Employee was hooking up Chain hoist. The hoist slipped from hooked position, falling onto employee's finger. Causing laceration.

Employee was racking belt and while using a rope and sheave wheel their hand came in contact with the wheel, lacerating the tip of the right little finger.

Employee was on knees getting supplies from tool box & piece of draw rock fell and struck EE on their ankle.

An unplanned, non-injury roof fall occurred in the #6 - #7 crosscut. The fall measured approx. 4 1/2' feet thick, 35' long and approx. 18' wide and no persons were injured or equipment damaged.

Employee was cutting belt, knife slipped cutting top of left hand

While employee was riding in the battery mantrip. Had knee bent and bounced and felt pain in left knee. The same knee was injured also on 5/20/2018.

Employee was installing roof mesh at which time a piece of rock broke away from the mine roof as EE was preparing to install the mesh. The rock struck EE in the head.

After a wait period, Employee was trying to establish ventilation from a blasting shot and went down and slumped over.

While handling roof straps the strap contacted the right middle finger causing a small incision.

Employee was handling materials and got finger caught between material causing laceration.

Employee was in the process of roof bolting when applied up pressure and caused the drill steel to bend pinching left small finger between steel and guide.

Employee was attempting to move the left miner cable out of the roadway when employee suffered a muscle strain to left shoulder.

Employee was tramming bolter across section and T-Channel (straps) slid into the operators compartment and cut hand in several locations.

Employee was removing guard from miner when guard slipped and caught end of employee's finger resulting in amputation of the end of middle finger.

Employee was moving fly pads and piece of draw rock fell from roof and struck EE in the face causing a small laceration.

While walking into the guard shack, the door was flung open, hitting the employee in the left arm, causing employee to fall back, jarring the shoulder and neck.

Security guard was making patrol rounds in parking and guard shack area adjacent to the administrative buildings. While walking she tripped over a parking barrier resulting in spraining her right wrist and bruising her right shoulder and tail bone.

Taking down a seal frame. Caught upper arm on nail and lacerated arm.

EE WAS PULLING ON MINING CABLE AND THE CABLE FLIPPED, PULLING HIS SHOULDER LOOSE.

EE WAS CHANGING A TIRE ON A 10SC22 SHUTTLE CAR. THE TIRE STARTED RIDING HIM OVER AND HE PUSHED ON TIRE AND FELL BACKWARDS INJURING HIS BACK.

EE WAS MOVING MINER CABLE ON A CORNER AND RIB ROLLED ON CORNER ON #3 INTERSECTION KNOCKING HIM DOWN AND BRUISING LEFT LEG, LEFT SHOULDER AND RIGHT FOOT.

THE #ONE ENTRY WHERE PANEL TURNS ON #3 BELT THE INTERSECTION AT SPAD #107 FELL 40'LONG X 18'WIDE X 2 1/3' TO 4'THICK. THE FALL WILL HAVE TO BE CLEANED UP AND BOLTED BACK. REROUTED RETURN DOWN TRACK ENTRY FOUR BREAKS AND BACK INTO RETURN TEMPORARY.

EE WAS RUNNING A SHUTTLE CAR. HIT A BUMP IN THE BOTTOM, THROWING HIM INTO THE CANOPY. COMPLAINING ABOUT PAIN IN HIS NECK.

OUR #1 HEADING CONTINUED TO GET WORSE STOPPED MIING IT ON 7/31/98 AND BUILT SOME CRIBS TOP FELL ON 8/7/98 7O'FT LONG X17FT WIDE X 4-5 FT THICK AT SPAD #175 BREAKERED OFF AND USING #2 AS THE R ETURN

EE WAS ON TRACK RAIL RUNNER & GOT OFF TO USE BATHROOM AND SAID A RIB ROLLED OFF ON HIM

THE BELT AND TRACK ENTRY HAD A INTERSECTION TO FALL ONE BREAK INBY SPAD #127 - 20'WIDE X 40' LONG X 4' TO 5' THICK. STARTED CLEANING UP AND BOLTING FALL. BUT DECIDED BO MOVE BACK OUT OF BAD TOP AND BREAKER OFF. MOVED BACK 1000 FEET OUTBYFACES. ACCIDENT OCCURRED FROM PRESSURE PUSHING DOWN ABOVE PILLAR BLOCKS.

EE WAS HELPING ON MINER AND A PIECE OF DRAW ROCK 2' X 2' X 2" FELL STRIKING HIM ON THE HEAD RESULTING IN PAIN THE THE HEAD, NECK AND BACK.

EE WAS PULLING MINER CABLE BACK BY HAND AND A RIB ROLLED OFF CATCHING LEFT FOOT BRUISING FOOT AND ANKLE.

EMPLOYEE WAS PINNING TOP AND THE DRILL STEEL HUNG UP. EMPLOYEE PUT A WRENCH ON THE BOTTOM OF STEEL CATCHING A SMALL PART OF HIS GLOVE AND TURNING ROTATION ON . TWISTED HIS HAND AROUND STEEL IN JURING HIS HAND AND TEARING HIS GLOVE OFF.
