Employee opened an airlock door, and it blew shut, catching EE's hand between the doors.
Employee was using a grinder with a cutoff wheel, the grinder them jumped making contact with employee's left little finger. This contact caused a laceration that required suturing. The employee was wearing gloves at time of incident.

Employee was pushing the roof bolter cable over to hang it and while pushing on the cable the employee's left hand slipped causing the left hand to strike a roof bolt plate resulting in a laceration. The employee was wearing gloves at time of incident.
Employee was dropping a cable for a roof bolter and got mud in left eye.
Employee was struck with a piece of draw rock on the right forearm, the rock was approx. 2 inches thick, 6 inches wide and 18 inches long.
Employee was working on a belt move and installing structure. A top cradle accidentally fell over causing employee's finger to become caught between a top cradle bracket and the structure rail causing injury.
Employee was cutting some tape with a razor knife and accidentally cut right hand.
