EE was removing scrap metal from a muddy area when EE felt a pain in EE's back. EE visited the hospital on 2-24-26 and received a full release. EE worked until 2-26-26 when EE followed up with a PCP and was given another full release. EE went back to PCP on 3-5-26 when EE was taken out of full-duty work
A loaded coal truck collided with an empty coal truck while traveling in opposite directions. The operator of loaded coal truck experienced soreness and was written off work.

Employee was climbing off the belt after clearing the grizzly bars and twisted EE's right ankle on 2/18/2025. The employee was later treated on 2/20/2025 and received a full release. The employee later sought treatment again on 2/26/2025 and was written off work at that time.

Employee was carrying a bucket with an electric saw inside of the bucket. The saw started to fall out of the bucket and the employee went to catch the saw and contacted the bar of the saw creating an incision. The saw was not operating nor on at time of contact, the action of grabbing the bar as it was falling caused the injury.

Employee was cleaning out belly pan discharge and material fell from the chute and knocked EE down causing EE's hard hat to fall off and the employee lost EE's balance and fell. When EE fell, EE struck EE's head on a piece of metal.

Employee was backing up to dump an Artic truck and the bed rolled to the side. The employee stated that when the bed rolled to the side, the seat belt tightened up creating pressure on EE's left hip. The employee was transported to LRMC and was written off work because of a possible pinched nerve.

The Employee stated, the EE was exiting an articulation truck. While climbing down the ladder EE fell from the ladder hitting the ground, landing on right hand. Employee was released to return to work in two days, but went for second opinion, and referred for MRI at a later date.

The employee stated that EE injured EE's shoulder on 10/14/19 and was taken to a medical facility and examined and it was determined EE had a sprain and considered a "first aid case". On 3/30/2020, the employee stated that EE's shoulder was aching again and EE went to a medical facility and was written off beginning on 3/31/2020.

Employee lost balance while cleaning up work area and tried to catch self with right arm. When body weight came down on arm, employee felt sharp pain in arm. EE was taken to the hospital and examined and the result was a torn muscle in bicep area.
