An 80 year old security guard was found deceased in the Guard Shack at the entrance to the mine property. The victim was found at 11:06 PM and based on written security logs, the victim died at some point after 8:20 PM when EE made EE's last written log entry.
Employee was installing a seat in the 773 truck when EE twisted and felt something pop in EE's shoulder.
On 5/5/25 the employee fell and hit their left thumb on a jersey barrier resulting in a strain/sprain. Employee continued to work and sought medical treatment on 5/8/25 when they were taken off work and returned back to work on 5/12/25.

The employee informed management on 12/7/23 that EE was seen by a physician on 12/6/23 for back pains from an injury that EE suspects happened on the job on 12/1/23. EE did not notify EE's supervisor of any injury until 12/7/2023 and was released to return to work with no restrictions on 12/7/23 and continued to work until 12/13/23 when EE was written off.

Security officer was sick and hurting on her right side. Her supervisor was concerned she might have appendicitis. Security officer took her to the doctor. This was not a workplace injury. She has a herniated disc in her back that was causing the sharp pain in her right side making it look like an appendix problem according to her dr. Her dr gave her a slip to RTW on 2/6/14.

Employee was in process of removing broken pieces of a cast volute from a fine heavy media pump and as he tried with a bar using right hand, the broken discharge section of pump came loose and caught his left ring and middle finger between the broken discharge section and an assembly bolt resulting in lacerations and fractures to the tips of both fingers.
