EMPLOYEE WAS PLACING A BIT IN A DRILL WHILE PLUGGED. THE TRIGGER STRUCK THE COUNTER TOP AND THE DRILL SPUN WHILE THE CHUCK WAS STILL IN PLACE. THE CHUCK SPUN UP AND CUT THE EMPLOYEE'S FINGER.
