Knife River Missoula · Struck by falling object

Replacing a screen. Lifted screen to remove a rock and screen slipped from coworker's grip causing screen to fall on employee's hand. Lacerating left palm. Required stitches and tetanus shot.

Knife River Missoula · Fall to the walkway or working surface

Injured employee was standing next to a parked skid steer loader talking to emp. sitting inside the skid steer loader. IE turned to walk away from skid steer loader & tripped on the loader's bucket which had a 4' extension attached to the bucket. IE tripped & fell to ground producing a fracture to rt 5th finger. Positive x-ray diagnosed on 8/03/2011. IE initially refused treatment.

WHILE DRILLING A 1/2" HOLE IN METAL USING A DRILL PRESS, THE DRILL BIT CAUGHT & SPUN METAL. THE SPINNING METAL CAUGHT THE GLOVE, PULLING IT DOWN & LACERATING A 1" SLIT IN FOREARM.

EMPLOYEE WAS PLACING A JACKING PLATE FOR THE CRUSHER SCREEN BOX UNDER THE JACK. HE BENT OVER AND TURNED TO THE LEFT, THEN PICKED UP THE PLATE TO SCOOT IT UNDER THE JACK. HE FELT A SHARP PAIN IN HIS LOWER BACK AND DECIDED TO GO SEE A DOCTOR THE NEXT DAY.

EE WAS DUMPING HIS END DUMP TRUCK AT THE PIT. WHILE THE BOX WAS IN THE AIR HE BEGAN TO MOVE FORWARD TO DUMP. HIS LEFT SIDE REAR WHEELS SLIPPED INTO A HOLE OR SOFT SPOT. THE BOX SWAYED TO THE L EFT AND THE WHOLE TRUCK TIPPED OVER ONTO IT'S LEFT SIDE. EE'S HEAD HIT THE TOP OF THE DOOR JAM. THIS CUT HIS LEFT EAR AND HE HAS SORENESS IN HIS NECK AND BETWEEN HIS SHOULDERS.

THIS LOCATION IS ABOUT FOREHEAD HIGH AND EE DID NOT F LAG ITS PROTURDING SECTION. WHILE MAKING REPAIR EE STEPPED INTO THE EDGE OF THE GUARD HITTING HIS FOREHEAD JUST ABOVE THE BRIDGE OF HIS NOSE. THIS CAUSED A SMALL BUT DEEP ENOUGH CUT THAT HE W

EMPLOYEE WAS TIGHTENING BOLTS WITH A WRENCH ON THE LOG WASHER (AT THE WASH PLANT). HE PULLED A MUSCLE IN HIS BACK BECAUSE OF HAVING TO TWIST HIS BODY IN AN AWKWARD POSITION TO REACH THE BOLTS. HE DID NOT SEE A DR. UNTIL 10-8-97 AT HIS OWN REQUEST. AT THAT TIME THE DR. PUT EMPLOYEE ON RESTRICTED WORK ACTIVITY FOR 21 DAYS.
