EE WAS CLIMBING A LADDER TO INSPECT A PIECE OF EQUIPMENT AND LOST HIS GRIP AND FELL BACKWARDS ONTO THE GROUND. IN AN ATTEMPT TO BREAK HIS FALL, BROKE HIS WRIST AND INCURRED AN INCISION.

MOVING A TRANSFER CONVYOR. EE PUAHED THE BELTING OUT OF THE WAY TO SEE BETTER. LOADER BUCKET BLED OFF A LITTLE AND THE OPERATOR RAISED THE BUCKET BEFORE ALL CLEAR WAS GIVEN. TIP OF FINGER WAS CAUGHT BETWEEN GEAR BOX AND STEEL ON CHUTE.
DIGGING INTO THE BANK WITH LOADER. FILLED BUCKET AND PART OF THE BANK SLIDE ONTO THE BUCKET CAUSING THE LOADER TO ROCK SHAKING THE LOADER OPERATOR.
