An unintentional roof fall was discovered at survey station 281 (belt entry). Fall dimensions estimated at 15 feet high by 20 feet wide by 20 feet wide.

Had a roof fall above anchorage in No. 7 Entry one break inby survey station 258. There was a slick sided slip that turned loose. 18' wide - 50' long - 7' high. This fall was not in face and did not hinder travel to and from face area.

Consolidation Coal Company · Fall to the walkway or working surface

Employee was in process of positioning a distribution transformer onto a concrete pad using a boom truck. The transformer was approximately 1-2" above concrete pad when the frame mounted clack bolts for the Manitex boom sheared and the individual jumped from the operator's deck from a height of 42" landing on the ground, twisting his left ankle fracturing it.
