Accident type, without injuries

# 1 Unit # 3 entry at 30+80 survey tag there was a roof fall in this intersection. The fall was the whole intersection and fell 8' to 10'. The ccnveyor belt tail was inby this area in the # 6 entry at 31+50 Tag. This area has been timbered and flagged off.

Main Portal Elevator lever arm bent and out of adjustment. Replaced arm and made adjustments. Test ran elevator and put back in service at 2:57 p.m.

A roof fall has occurred in No. 4 entry of No. 2 left off No. 1 North Section. The fall is located 1 break inby spad no. 1721 belt entry, and measures approximately 22' wide x 22' long x 6-8 ft high and involved an intersection. There were no injuries or equipment involved. The fall did not affect the passage of men or ventilation.

An unintentional roof fall occurred at cross cut #69 in #5 entry on the 2nd 48 West Belt Line. The roof fall measured 25' long, 20' wide and 10' thick. The roof fall will be cleaned up. The fall appeared to broken stack rock.

Rock fall occurred on 1 North Mains at break #114, brow rolled off on track. Pulled approx 4 or 5 bolts out. Brow measured approx. 2'-3' wide x 20' long x 48"" thick.

#1 elevator, door closer motor burned up. The motor to the automatic door closer. effected the operation of this elevator for move than 30 minutes. the elevator was operational at 8 pm.

Production Hoist stopped operating. Discovered that there was a blown fuse in the drive unit. Fuse was replaced and Production Hoist began to operate again.

Employee used the elevator for transport to top floor after finishing a task on the bottom (fourth) floor, when the elevator stopped between the first and second floors.

Employee was operating a 651 scraper when he lost control which resulted in the scraper tipping over. Operator was wearing a seatbelt and sustained no injures, but was taken to the hospital to get checked out and was released.

A roof fall in the intake airway that is not used for travel or is not an escapway and is in the 1st Left panel mined around 1998 with 6' full grout resin bolts. The fall was in the #3 entry x-cut 10-11 and was 20'x50' long 3-6' thick. Area will not be cleaned up, just dangered and cribbed.

A fire was reported at the landfill. Water trucks were dispatched. The fire brigade responded several minutes later. Because of the nature of the landfill the fire fighters were not allowed out on the landfill. The fire was extinguished in approximately one hour.

Lightning strikes in the immediate area knocked out power to both #1 and #3 Hoists. An evacuationtion was ordered. #1 Hoist was returned to operational capacity and dressed for evacuation. #3 Hoist was returned to service shortly after and evacuation was no longer necessary, as all conveyances were operational.

A direct lightning strike on a seven foot deep bore hole loaded with one foot of explosives, caused a premature detonation of the borehole. Flyrock from the detonation shattered the rear window on a Komatsu D475 bulldozer, no injuries involved.

The mountain #11 outy @ 20xc @ plus 15+69 on the South ""C"" section fall. The fell was above and 6 to 8 high, 50 ft long, it connected to a area that was left unbolted and cribbed before the area was abandonded. The area was bolted w/5th fully grouted rebar 3/4"".

The roof fell in #5 & #6 intake & X5-6 at 43bl. Worth main return MM V Idle. Cover of approx 365', top Gray Shale. Area bolted with 6' fully grouted resin bolts and hole board & 12' cable bolts used as supplemental support with 16"" X 18"" mountain mats. Fall was discoved on weekly exam.

A roof fall occurred in the 6th West #2 entry at #8 x-cut in an adjacent intake entry that is not a manway, travelway or escapeway. The fall was measured at 25' by 15' by 8.5' high. There were no injuries or equipment damage.

An area approx. 20 feet by 25 feet by up to 8 feet high fell in the number 2 entry of the Rilda break outs. This fall was above the anchorage zone. initial roof support was 6 foot bolts. This is one of 3 entries.

An interruption in power supplied by the public utility company interfered with use of #1 & #2 hoists. Miners remained underground for 50 minutes until an electrician arrived to restore electrical service or activate the emergency generator. Electrical service was restored to the hoists by resetting a breaker in the substation.

A rockfall occurred at break of the 6a belt areaq starting at the right xcut at spad #42712 in the #2 heading going over to the #3 heading & up to spad #42716. The fall was 8' thick, 19' wide, and 80'long.The area was bolted with 5' torque tension bolts. The fall will be clean up & rebolted & add cribs around it.

The plant maint crew reported to complete repairs on screen. As 1 EE was cutting loose a guard with a torch, another EE was wetting down the urethane screen media with a water hose, which is proper procedure. A piece of slag fell down the sand flume, starting a fire from below. (30 min to extinguish fire). MSHA immediate notification was made.

Four men became stuck on livingston elevator, while trying to exit the mine on 6/24/07 at 2:23 pm. The elevator started ascending as normal, stopped 2' above mine level (bottom). Maintenance forman opened doors and all men exited elevator at 2:33pm. Investigation-door cable caught, causing elevator to stop.

Roof fall happened between shifts in the E2-Butt intake about 40' off of the main travelway, it was an intersection. Fall measured 19' wide X 40' long X approx 5' high. Area was cribbed and dangered off. Area was bolted with 42"" & 6' fully grouted bolts.

On 6/23/07, an unintentional roof fall occurred on the 1-A supply road, entry #6 at cross cut #30, on the 2nd West Sub Main off the 3rd South Sub Mains. The roof was supported with 10' point anchor bolts. the fall measured 20' long, 18' wide, and 12' thick. The fall will be cleaned up. The fall appeared to be broken stack rock.

An unplanned roof fall occurred in the #130 mine on the old 1st right section in the #9 entry. The fall measured 25' in lenght, 20' in width and 5' in height. No accident reported. The area was developed 12/21/05.

An unplanned roof fall occurred in the #130 mine on the old 1st right section in the #1 entry. The fall measured 100' long X 19 1/2' wide and 4-6' high. No accidents reported. The area was developed 12/21/05.

A fire was reported at approx 1:30 pm. I immediately went to investigate & found a fire located approx 1 mile east of the plant facility. Apparently a train passed through the area & a spark ignited the weeds in the railroad right away. The fire spread to the plant property. The fire department was notified and MSHA was called at 1:50. An area of 400' x 200' was burned.

A ROOF FALL OCCURED IN AN INTERSECTION OF THE #7 ENTRY 4 BREAKS INBY SURVEY STATION # 1215. THE FALL WAS IN THE SECOND TO LAST OPEN LINE OF CROSSCUTS ON THE 004-0 MMU. THE FALL MEASURED APPROXIMATELY 10' WIDE, 20' LONG, AND 8' THICK.

A maintenance operation was going on a 8X20 Screen deck which involved cutting. Plywood was placed over the screen media to protect it from damage. The operation was going 4 hours when the fire watch noticed smoke coming from the screen deck. Four fire exstinguishers were emptied to control the fire at which time it grew out of control and the employees evacuated the area.

An unintentional roof fall was discovered in the return aircourse of 001 section, at approximately 10:15 am on 6-19-07. The aircourse was last traveled on 6-12-07. The fall was approximately 50' long x 19' wide x 7'-8' thick. The fall consisted of weathered laminated sandy shale.

A roof fall was found by our weekly examiners in the 3 Left 4 West return at crosscut 17 in entry #3. The fall was located in the normal weekly travel route of the examiners. The travel route was routed around the fall area.

A ROOF FALL WAS DISCOVERED IN THE OLD 2 NORTH 5 MAINS, AT 61 CROSSCUT, BETWEEN #3 & 4 ENTRIES. THE FALL MEASURES APPROXIMATELY 20 FEET LONG, 18 FEET WIDE, AND 5 FEET THICK. THE AREA IS SUPPORTED WITH 5 FOOT FULLY GROUTED RESIN BOLTS.

An uninentional roof fall occurred in the main North seal area parallel entry to #3 main no. seal. The fall was noticed in adjacent entry by mine examiner. Area will be propped off & exam of all adjoining areas will be made for adverse condtions. Fall did not affect travel or ventilation in this area.

ONE LAY IN ONE HOIST ROPE ON THE OTIS ELEVATOR SERPARATED AND BECAME ENTANGLED IN A ROPE GUARD. THIS CAUSED THE ELEVATOR TO STOP BEFORE REACHING THE BOTTOM. NO PERSONS WERE RIDING THE ELEVATOR. ALL ELEVATOR HOIST ROPES WERE REPLACED BY KONE ELEVATOR PERSONNEL. KONE ELEVATOR PERSONNEL PERFORMANCE TESTED THE ELEVATOR AND FOUND IT TO BE OPERATING PROPERLY.

A PLC problem caused the #3 Hoist to be rendered inoperable. #1 Hoist utilized to bring an electrician U/G to reset and clear hoist for operation. Test trip conducted, no further incident #3 Hoist put back into service.

The sealed area of the 3 Right 1 Main North apparently had an area subside. This caused a run-off pond on the surface to drain into the sealed area. It was estmated that the water contained in the pond was approximately 1 million gallons. The inundation of water into the sealed area caused water to be impounded behind the seals. The water was drained from behind the seals.

The mine experienced an unintentional roof fall at S.S. 65+40 of the 3rd Main Northwest. The Secondary escapeway(haul road), has been re-routed to an adjacent entry to allow the travel of men.

A momentary power outage occurred while hoisting. This resulted in a drive fault in the hoist program. It would not immediately reset. The DHS computer had a communications error and had to be powered down to reset. The hoist was inoperable for over 30 minutes. Crews were brought above the 900 level. There was no damage to the hoist.

Cause of the fall was the deterioration of the main roof due to weathering. The roof fall did not obstruct ventilation or escapeways.

The intersection of 13xc, #10 entry, plus 12+13 on the #3 main fell. The area was bolted with 6' fully grouted resin bolts and 12' cable bolts. The top was composed of black shale with some quartz. The area in the Pittsburg seam had been mined out. No men were injured. No equipment was trapped. The fall will not be cleaned up.

Roof fall occurred starting 1,200' inby #3 portal on the intake entry. The fall was approx 20' wide X 150-200' long X 10-20' high in between the coal block and through 2 intersections.

Two (2) Commercial Steel Erectors-CSE employees were stuck in the Preheat Tower Elevator. Reset did not release elevator to landing. Elevator mechanic called in to lower elevator . Employees were stuck in the elevator for 50 minutes. Employees were in constant communication with the control roomduring the incident.At no time were they unaccounted for.

Their was a roof fall at brk 13 of 9 f. belt at spad #41473 in #6 entry. Fall will be cleaned up. Fall was approx. 20'x20'x5' thick top was bolted with 5' torque tenion. bolts, excape way was not blocked. Strate was shell.

Had rock fall one break inby spad #1334 in #2 entry on 004 section. Fall approx 6-10' thick X 40' long X 18' wide. Area was bolted with 6' resin bolts & 12' rope bolt over 14' - 16'. Area cribbed off on all sides with crib block. Fall was inspected by MSHA Inspectors on 6/13/07.

Preshift inspection revealed that a large quanity of rock had fallen from highwall into the auger pit. The fall occurred sometime during offshift hours between 3:30pm June 12 & 5:00am June 13, when no miners were present at the mine. Auger substained about 30% damage and water pump minor damage. A clear cause of the fall could not be determined.

Hoistman reported brake failed brake test. After troubleshooting and testing, it was decided to replace the brake blocks. Hoist was put back in service on 6/15/07 at 7:30 pm.

Employee was unplugging center launder going into classifier. Material started flowing to classifier, backed up hot pellets from hopper hit the water, causing excessive steam & water to back into the discharge pipe and spray out. Employee tried to escape the hazard, but ran directly into the hot pellet pieces that were spilling out burning his upper back, shoulders and wrists.
