On 7/31/2007, an unintentional roof fall occurred at cross cut #6, entry #3, on the 1st East panel, off the 1st South Sub main. The roof fall measured 18' long X 18' wide X 12' high. The area was bolted with 6' and 10' roof bolts. The fall was caused by broken roof strata.

Fall was measured at 20' length, 20' width, 6 1/2' height. The area was bolted with 6' torque tension bolts. Area was breakered off using timbers and the intake was rerouted. Near spad #6279 2600' from face. No injuries reported, no restrictions of air.
The Production Hoist stopped operating. It could not be re-started. It was discovered that an overtravel switch at the bottom of the Production Shaft was damaged. The switch was replaced and the hoist began to operate again.
During the examination of the no.1 conveyor belt it was discovered that the mine roof had set down & broke some cross collars that had been set along with cribs to support the area making the area unsafe for travel. The area was located at the 3rd crosscut inby the portal & measured approx 8'L x 8'W with the thickness undetermined due to the fall being caught by the cribs.
Unintentional roof fall 3rd west panel #2 entry 1519'. 25' long x 35' wide x 8' high. Bolted with 6' & 8' tension rebar bolts. Approx date of development 7/1/07. Will not be cleaned up.
300' from active face. Access was already timbered and dangered off before fall occurred. Size-25' X 5' thick. It is located one break infy survey station 1039 in the intersection.
MINE FIRE - A mine fire was discovered @ the #1 slope portal @ 05:39pm by guards. The fire dept was dispatched & the fire was extinguished @ 06:04pm. The fire occurred from work being performed during demolition. The established guidelines for reportability is a fire not extinguished within 30 minutes of discovery. We do not consider this fire to be reportable.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, X-CUT #38, #4 ENTRY INTERSECTION. THE FALL WAS APPROXIMATELY 20' W x 20' L AND FELL ABOVE THE ANCHORAGE.
A roof fall was discovered in the abandoned 2 North 5 Mains, 25 Crosscut, #2-3 entry. The fall measured approximately 10 feet thick, 18 feet wide, and 100 feet long. The area is supported with 6 foot fully grouted resin bolts.
A roof fall occurred in the face of #4 entry inby spad # 8334. The area was not bolted and when it fell it pulled a few rows of bolts out. The area that was pulled out was bolted with 4' double lock bolts. The fall was approx. 40'L x 20'W x 4'to 6' high. The area will be cleaned up and a clean up plan posted in the face area.
A roof fall occurred in the 3 south panel, in the last open crosscut, approximately 15' on either side of the intersection of #6 entry. The fall measured approximately 50'x18'x6'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts.
A roof fall occurred in the #5 entry of Southwest Mains 75 feet outby SS #1202. The fall measured 120' long x 18' wide and 8' to 10 feet high. No injuries occurred and no equipment was involved. Ventilation was not affected. The fall will not be cleaned up.
A roof fall measuring 20'L x 18'W x 8'H occurred 1 break outby spad number 7331 in the No. 4 Southeast Mains, 003 MMU. Area was bolted with 5' TT bolts.
A roof fall was found in #6 entry 2500' outby the face, one break inby spad #7993. The fall was 20'W x 40'L x 4' to 6' high. The fall has been dangered off and timbers have been set. This area was bolted with 4' resin bolts and will not be cleaned up.
A rock fall was found in the #4 entry at the 42 break adjacent to the #2 belt. This fall was approximately 5 feet high, 20 feet wide and 80 feet long. This area was in a low cover area supported with 4 foot roof bolts. The top is shale and is layered.
A roof fall occured in the 18 North Panel, between crosscuts #13 and #14, #5 entry. The fall measured approximately 35'x19'x5'. The area supported with 5' fully grouted bolts.
A roof fall was discovered in the 2 north 3 mains, 15 crosscut, #1 entry. The fall measures apprx 8' thick, 18' wide and 40' long. The area is supported with 6' fully grouted resin bolts.
A non-injury roof fall was found at this idle mine during an inspection by the pre-shift forman (supt). The fall was approximately 20 cross cuts outby the face at spad #109. The fall was 4' x 20' x 60'/ the fall did not impede travel or ventilation. (Pond Creek Section)
A frictional methane ignition occured on the longwall face, shield No. 130. The flame was extinguished in 3-5 seconds.
A roof fall occurred at spad 1169. The fall is approximately 50 ft. long, 19 ft. wide and 5 ft. thick.
The elevator was down due to a planned power outage by Allegheny Power. The transfer to the diesel backup caused an overcurrent condition which interupted the operation of the elevator from 10:40am until 1:00 pm when the power was restored.
Roof fall occurred near spad # 3116 on 2nd Southeast mains. In the crosscut between track entry and the neutral roadway. Fall was 6' H x 20' W x 20' L. The area had been bolted with 4' resin and 6' torque tension bolts.
The roof fell prematurely in front of the #3 and #4 MRS units pulling 4 rows of roof bolts. The miner breaker had knocked while in the #32 lift and the miner had to be retrieved. This delay allowed the roof to work and load up the MRS. When the MRS units were released to move, the roof fell. The roof was previously supported with five foot Hy-Tech bolts.
Kiln 4 was in the process of being shut down when the oil in the bull gear housing caught fire. The fire was fully contained within the gear housing and was extinguished in approximately 30 minutes.
We experienced a fire on our 6400 Dragline in the area of the main engine. Two operators began to fight the fire & then firefighters from the Fire Department extinguished the blaze & pumped foam to avoid re-ignition. No injuries were sustained. Investigations showed a lost 24 volts carrier from starters near fuel lines could have caused it.
Rock fall occurred in 2 West Mains on 002 section in the #2 entry from break #6 to break #9. Fall measured approx. 20' wide x 140' long x 4'-6' thick.
An unintentional roof fall occurred at the 7N Headgate between Entry #1 and #2 at Crosscut 40 1781 North. The fall was of unknown length, approx 18' wide and of unknown thickness. The fall did not affect ventilation or the passage of EE's. The fall will not be cleaned up.
A fall of roof occurred in the 3E Panel at x-cut 32 between the # 5 and 6 entries. The fall was approx 60 ft long x 19 ft wide x 8 ft high. The area was supported with 5 ft fully grouted bolts.
Unintentional roof fall 6th East Panel #4 entry #58 x-cut. 30' long x 35' wide x 8' high. 4 way intersection bolted with 6' & 8' tension rebar bolts 3-27-07 propped off, will not be cleaned up.
An unintentional roof fall occurred on the #1 section one break above spad #5362 in the crosscut between #2 and #3 entries. The fall measured 50' long, 19' wide and 0' to 8' thick. The fall does not affect the air flow or the travel of men.
Brush fire behind the Bazza Dump. No equipment or personnel were in the area when fire began. Fire was contained and completely extinquished at 14:00. No property damage; no injuries.
Unintentional roof fall measuring 18'W X 80'L X 12'H. Previously supported with 6' full grout resin bolts. Mined on or about 3/10/2007. Cover approximately 267'. Fall to be cleaned up, bolted, and screened. Fall located on 12 Track, No. 7 entry, one block inby station 1405.
A roof fall measuring 18W x 18L x 7H occurred 3 breaks inby spad number 9189 in panel 5 off of panel 1 submains, 005 MMU. Area was bolted with 6' bolts.
Roof fall above the anchorage at 1 break inby spad 2215 #3 entry #5 belt conveyor.
A fault and rotten top caused a fall to occur on #8 belt near spad 4394. The fall was approximately 130 ft. by 19 ft. by 5 to 6 ft. thick.
Roof fall in D entry at 12+00, 2 Sec 1 South off of main West C. 4-Way intersection. Had no effect on ventilation. Laminated shale, 4 & 6 foot fully grouted resin bolts.
A roof fall was found in the secondary escapeway 11breaks inby #5 head drive. The fall was approx. 6' to 7' thick, 20' to 25' long. The fall area was cribbed off, the secondary escapeway was re-routed.
A roof fall occured approximately 4 feet inby spad 6517 in the intersection of the #2 face on 1-right section. The fall measured approximately 18' 6"" wide, 33' long by 9' high.
A roof fall was found in the #5 entry from crosscut 13-15, the fall is approx. 120' long, the fall is approx. 3' to 7' thick, the fall area was cribbed and dangered off. Additional roof support has been put up in entry 5 & 6 out-by the fall, 8 ft. tension bolts were installed.
On 7/25/2007 at 11:35 am, the mine experienced a roof fall on the 3rd main Northwest at S.S. 54+60, including the 54+60 crosscut. The Secondary escapeway (haul road) was rerouted to an adjacent entry to allow the travel of men.

The Cedar Creek elevator was out of service from 6:30 am to 7:15 am due to a fault door locking mechanism.
Two employees were in the process of inspecting topsoil piles while driving on a two track road when they noticed flames coming from the engine compartment of their pickup truck and underneath it. The truck cab burned down along with approximately 20 acres of BLM land. The fire burned for approximately 1 hour.
On 7/24/07 a roof fall was discovered by a belt cleaner at #151 cross-cut along Somerset Track haulage. No persons were injured and no equipment was damaged
Miner operator accidentally lowered the head against sandstone chunk and created a spark leading to a small ignition. This is the second electronic filing for this incident. The original was filed July 31, 2007 but does not show up on the MSHA database.
A roof fall was discovered in the 2nd Rt. intake just ouby #16 xc. The fall measured approx. 20' long, 16' wide and 6 to 7' high. The area is wet and the stratta experienced degradation due to saturation.
Underground intersection at break 13 above #4 head drive about 3 1/2 ft. high, about 19 ft. wide, approx. 50 ft. long.
Unintentional fall of mine roof located at spad #124 in #1 entry on 4 left section, 5000 feet from working face. Fall is approximately 16' wide by 18' long by 6' high. Fall was discovered by weekly examiner. MSHA and the state were notified immediately.
Inundation was at 1st east panel, entry #5, # 4th break.
The mine examiner located a roof fall located outby the #1 Seal of the 1st Southwest seals, off of the 2nd Main Northwest. Instructions from Distrct #8 is to monitor the area near the seals hourly until the fall can be cleaned to check the condition of the seal. Approaches to the area will be resupported with 12' bolts and cable bolts.
Roof material between the shield canopy and the first bolt in the pattern fell out. The material was of sufficient volume that it sloughed in around the taildrive and obstructed the walkway off the longwall face. The bolts and roof to floor support in the tailgate remained intact and the enry remained open.
Fall was 60' length - 20' width - 5' height. 4 ft fully grouted resin bolts were used. Fall was timbered off from all approaches. Not to be cleaned up. 2 section had moved approx. 14 days prior.
A fire was reported in Section 23. It was determined that the fire was started by lightening and was contained by pushing a dozer line around the fire. Company security was notified in order to monitor the area to prevent further spread of fire.
Smouldering coal by the shovel flared up and increased rapidly. When it was apparent that it could not be exinguished in under 30 minutes, MSHA was notified. Mine and MSHA personnel agreed on a plan and the fire was extinguished. Very hot, dry weather that had prevailed for 4 days contributed to the flare-up. No injuries and no damage resulted from the fire.
An unplanned roof fall occurred in the #130 mine in the left return at #44 break on #5 belt. The fall measured 20' in length, 18' in width and 4' in height. The area was developed on 5/31/06, no injuries were reported.

The elevator started down & stopped after traveling 5'. The elevator stopped due to deceleration fault. Elevator was down until 10:20pm. Electrician reset the controls and elevator began running again.
Crack in drum found during routine inspection on #2 hoist. #2 hoist down for investigation & repair. Two other access to mine.

At approximately 8:07 A.M. the hoist at the Galatia North Portal went down when a malfunction of the brake set switch occurred. The brake set and would not release. No personnel were trapped. The switch was repaired and the hoist was running again at 8:52 A.M. MSHA was notified at 8:44 A.M.
The #1 elevator was removed from service after the bottom door switch failed. Schindler was called and the switch was repaired. The elevator was out of service for a period of time exceeding 30 minutes. All agencies were notified. No individuals were on the elevator at the time.
#2 section, #5 entry, spad no. 973, 700 feet from active face. Action taken: breaker access off, past recovery plan, clean up fall 40 ft length, 20 to 25 ft width, 6 ft height. The fall fell to the rider seam and dues have a stress area over it.
We lost power to the bottom of the silos which also runs the elevator. An employee was 3 feet from the top on the elevator. Electrician troubleshooted for 25 minutes and a decision was made to put a harness on EE & have her step out the top of the cage onto the silo deck with a rope tied onto her by the response team instead of hand brake the cage 217 feet. MSHA was notified
An intersection in the left return 90 feet outby spad #3024 fell for a distance of 60'x18'x25' high, 9000 ft outby the mains seciton.
An unintentional roof fall occurred approximatley 40 feet inby survey spad 1209. The fall measured approximatley 20' wide x 35' length x 1' to 10' feet high. The fall area has been cleaned up and re-supported with 8' fully grouted tension bolts.

Cowin Construction company was working in the shaft replacing cage rollers and guides (made of wood beams). Torch cutting was required to cut out old rollers. Apparently sparks entered the cavity of one of the old rotted timbers and began to smolder. Three hours later the spark was causing enough smoke to be identified.
A roof fall was observed approximately 17 X outby the working section in #6 entry at survey station 434. The fall measured approximately 35 feet long, 20 feet wide and approximately 1 foot to 5 feet high. Four foot fully grouted bolts were used in the area. Additional supports (cribs & timber) were being set in the entries to the left prior to the fall.
On Sunday evening 7/22/07 a pre-shift examiner on #3 unit found a roof fall when he arrived on the section inby the dumping point. The fall was located in entry #5 between crosscuts 19 and 20 on the 1 Left - 5 Main North panel. The cause of the fall was weak laminated shale asscociated with water dripping out of it.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY MINE EXAMINER IN NORTH SUB-MAINS, X-C 17, #2 ENTRY. THE FALL MEASURED APPROX. 14'W x 14'L X 5.5'H. THE FALL WAS A CONTINUATION OF A PREVIOUS ROOF FALL IN THE X-CUT. THE FALL OCCURRED IN AN AREA BOLTED WITH 5' BOLTS. OPERATOR VOLUNTARILY CHANGED TO 6' BOLTS SOME TIME AGO.
A roof fall was found in the belt entry at approx. 7:30a.m. near spad #8013. The fall occurred in an intersection and measured 20'L x 40'W and 4' to 5' high.
Electrical problem when operating slope hoist, blowing regulator fuse. Rebuilt rectifier assembly and hoist was put back in service at 5:30 p.m.
MMU-001 experienced an unintentional roof fall at S.S. 29+05, entry #0. 103(k) issued, and modified to allow additional roof support to be completed.

The fall is located 1 break inby spad 1801 and measures approximately 22' wide, 25' long & 8' - 10' high and involved an intersection. No injuries or equipment were involved. The fall did not affect the passage of men or ventilation.
An unintentional roof fall occurred in the #4 rt X-cut and extended into the #5 hdg approx 80 feet inby spad #1880.
At approximately 0320 a tank that contained 500m3 of diluted caustic solution ruptured releasing the solution onto a containment pad. This was an injury free event. Once it was verified that all personnel were accounted for and safe, they notified the safety manager about the incident. The safety manager notified MSHA at 0359 about the event through MSHA Toll Free Number.
Fall occurred on the #1 belt in #1 head. Length 40', width 20', height 9'. Hot weather sweat. No persons involved, no equipment.
At approximately 7:20 PM on 7/19/2007 there was a bounce at the tailgate end of the longwall which resulted in the tailgate being block from travel.

The slope car serving the #1 track slope was not operational due to electrical storm which created power outage. Outage lasted from 3:55am until 6:25 pm. The storm resulted in power surge which caused damage to electrical components on slope car.
The elevator serving the #16 portal was not operational due to an electrical storm which created a power outage. The elevator was down until 7:45am when it began running on a diesel generator.
PLC for weight switch would not reset, brake would not release.

A roof fall was found at 9:45 am in the small block submains bleeder, at brk 14, in entry 3 & 4. The fall was approx. 7' x 20' x 80'. The opening was 20' w & 8.5' h. Additional roof support was added to the area.

Upon examination of the neutral a rock fall was discovered in the #5 entry between spad 468 and 462. The fall was approximately 60' long x 19' wide x 12' high. The fall did not affect employees, ventilation nor equipment. The fall may be cleaned up at some point.
Our weekly examiners found a roof fall in the 3 Left 5 West panel in entry #7 at crosscut #39. This area is in a worked out area. It was located in a crosscut adjacent to the examiners' normal route of travel. The area was timbered.
An unintentional roof fall occurred on the East submain at 28+70 station in the #5 entry. The fall did not affect ventilation of passage of men. The fall measured 22 feet x 30 feet x 9 feet high. The intersection had been supported with 6 foot fully grouted roof bolts. All aproaches were timbered and dangered out.
Unplanned roof fall occurred in the break between entries three and four. 60 ft.long, spad No.243 to No.245. Located in an inactie area of the miner.
Grader operator was grading road and his hydraulics stopped working, so he backed the grader up into a wide place in the roadway. He got out and noticed flames coming from the engine compartment near the cab. The fire was extinguished within 50 minutes of discovering the fire.
We believe an ember came out of an electric toilet and came to rest against the bottom left corner of a wooden stopping 44 ft away. We believe that the ember was from a rag instead of the supplied paper toilet paper. The stopping began to burn and was discovered in the early stage. The location was on its own vent split and went directly into the returns, not endangering anyone.
A non-injury accident, the unintentional stoppage of the elevator at Camp Run Portal, entrapping 11 persons on-board the elevator occurred. The source of the malfunction was due to the unintentional activation of the car top hatch switch.

An unintentional fall occurred at #53 x/cut on main west belt entry at the 32+75'. Fall occurred due to adverse roof conditions above bolt anchorage. 6' tension rebar bolts used. Fall was 19' X 19' X 12'.
A roof fall occurred in the 2 Main North in the intersection of cross-cut #56 in #7 entry measuring approximately 19ft. x 19ft. x 6ft. This area was supported with 6 foot tension rebar bolts. An undetected slip was determined to be the cause of the fall. This area was mined in January 2001.
Hoist down due to maintenance. Hoist was serviced and some worn hardware was replaced.
Rock fall occurred on 1 East Mains in #4 entry at approx. break 8. Fall measured approx. 20' wide x 60' long x 1'-6' thick.
A rock fall occurred at spad # 16145 in the track entry in the old 6A belt area. The fall was 4 to 8 foot high, 20 foot wide and 80 foot long. The area was bolted with 6 foot torque tension bolts. The fall will be cleaned up and rebolted and crib around.
An unintentional roof fall occurred in the 9I section at 24 stopping in the crosscut from #2 entry to #3 entry. Fall was 20' long X 16' wide X 12' high. No injuries or equipment was damaged.
A roof fall occurred in the 3 left 4 West panel at crosscut #39. The cause of the fall was weak laminated shale. The area where the fall occurred had been previously cribbed and cross barred. The fall was found by an examiner while pre-shifting the area for a reclaim crew.
A roof fall occurred on the 8O section. The fall location is in the #2 entry at #26 break in the intersection. The fall is 18 feet by 14 feet by 40 feet long.
Unplanned roof fall above anchorage zone in the 5th north east butt #3 #4 room. The roof fall started at spad 2878 and was 120' in length, 5-6' high. The rooms were oblted with 42' fully grouted resin bolts. All entry to the roof fall cribs were built.
The power went out due to a lighting strike that struck a surface electrical substation. The site wide power outage resulted in the 1085 hoist being down for greater than 30-minutes. The surface electrical equipment was repaired and hoist operational. There was no damage to the hoist.

Truck owned LJF Inc.enroute for load of coal-Bonita Job. Truck shut off going up grade, coasted truck backward to side road. Driver saw electrical smoke coming between front of battery box & rear of front tire. Wires started flaming, put out with fire extinquisher, flames kept erupting, fire out of control. Loader operator got water in his bucket dumped on fire until out.

Hoist down at 11:30 p.m. Motor plate inside motor had broken. Employees were brought to surface. Hoist repairs were made in 1-1/2 hours. Several test runs were made after repairs. No other damage was observed.

Two seismic events occurred in previously mined areas of the active Longwall District which created pressure waves that disrupted ventilation. The event is still under investigation by the mine and agencies.

A fall of ground occurred in ME-163V crosscut. A mechanic was parked in the crosscut with pickup TP-74 when the fall of ground occurred, resulting in a fatality. After the fall of ground covered the TP-74, the vehicle caught fire, promting evacuation of the mine.
Contractor (Hooper and Chandler) was removing old tin and installing new tin on top of shop, when employee fell through the roof and landed on the concrete floor causing fatal injuries. The employee fell approx. 28 feet.

Initial Information: Two EE's working from manlift on West side of mill building over Electrical Room. From initial witness reports, it appears they were in the process of retracting & lowering boom at end of shift with boom fully extended to the South when South side wheels slipped sideways from leveling blocks, causing lift to tilt and capsize. Boom fell, basket hit ground.

Two front end loaders, one on each side, were being used to position an 80 foot section of 18 inch diameter pipe in the waters edge. When the loader on the right was about 25-30 feet from the lake, the bank started to cave in. Within 3 minutes the loader was submerged in about 20 feet of water.
Miner was hit in head by fly rock from blast causing his death.
Employee was getting back on dozer to move for a shot. A co-worker was backing beside the dozer. He struck employee, and ran over him with pickup truck. Employee suffered trauma to his head and chest.

CONTRACT LABOR, NOT AN EMPLOYEE AT APPROXIMATELY 9:30 PM WAS USING A COMEALONG TO PUT A HYDRAULIC MOTOR ON A FEEDER, WHEN THE COMEALONG SLIPPED AND THE HOOK AND HANDLE SMASHED AND CUT HIS RIGHT INDEX FINGER. ALSO SMASHED THE TOP OF HIS RIGHT HAND, CLOSE TO HIS INDEX FINGER.
Injured and another EE were removing the main hyd cylinder on a front end loader. EE was tapping a pin out w/handle end of sledge hammer, with his right hand cupped around the head of the hammer. The pin came out, cylinder dropped on EE's fingers, pinching between head of hammer and part of the machine. EE's right middle finger was pinched clean off.
Employee was assisting a mechanic to manually turn the drive on a Telsmith 68S crusher to prime the oil system when he accidently pinched his right little finger and ring finger in the v-belt against the sheave. The injury resulted in the amputation of the tip of his little finger and reconstructive surgery to the tip of his ring finger.
The injured employee was trying to remove a piece of metal from the #3 belt when the magnet pulled the metal piece back up and caught the tip of the right index finger causing a amputation to the tip of the right index finger

Injured claims to have been picking up belt feeder trailing cable when he felt pain in stomach area. Injured did not report it to supervisor until 7/11/07 because he said he did not think injury was serious.
Employee was working the window at the chopper. He was pushing rock through the window & placed his left hand on a roller. The roller was spinning and his left hand was pulled between rollers. Pulled his hand out, but middle finger was injured. Referred to specialist who cut bone back to 2nd knuckle of middle finger.
Employee fell between the miner power head and a push beam ""car."" The push beam was knocked forward causing employee's leg to be caught between powerhead and push beam.
Employee was adjusting an Aseptrol packaging machine guide. Approximately 1/4"" of the tip of the middle finger was amputated by the knife.
Employee was helping another employee manually turn the v-belts so the bearings could be adjusted. He got his little finger on his left hand pulled between the sheave and the v-belts, which amputated his little finger at the outermost joint, just below the fingernail.
Employee stated that he pulled the disconnect on 219 dust collector and proceeded to remove the guard. The pulley had not completely stopped when the guard contacted the pulley and pulled his right hand into the moving pulley.

This over the road driver got out of his truck @ stockpile (restricted area) while waiting to be loaded to air up his low tire. On the ground he reaches into his truck to turn on a valve to put compressed air to his air hose. He then accidentally released his parking brake which allowed the truck to roll back and his leg ended up under the front wheel of his truck.
The foreman was working on the truss bolter on the operator's side and was resting his left hand on the canopy. The bolt machine raised suddenly, pinching his left hand between the canopy and top. Fracture to left index and middle finger.
Tramming shuttle car - got into rib - caught caplight against rib, jerking his belt tight into stomach.

He was taking down a ventilation tube when a piece of rock fell striking the tube. He felt pain in his left shoulder when the rock jarred the tube. He is diagnosed with a left shoulder strain.
EE was unloading parts from truck, a trunion and shaft, when the shaft slipped . He reached out to hold it and pulled a muscle in his upper back.

EE was operating a continuous miner on Unit #1. He was loading coal on the right side of the unit. He was standing directly under a roof bolt, when a rock fell out between roof bolts and struck him on top of his left foot. The rock fell such that it struck him behind his metatrasal support on his boot. The rock was apprx 36""X30""X0-6"". He received a fracture to top of left foot.
EE manually opening a bin gate. Gate was hard to open and employee was pulling and pushing in rocking motion. On the pull stroke the gate broke free causing a muscle strain to right shoulder. EE was taken to Emergency Room at Winchester Medical Center and was given 48 hours off resulting in reportable lost time accident.
Employee stated he was lifting and moving some waterline so he could get supplies into a x-cut and as he picked up on one end of the pipe he felt some pain in his shoulder. He continued to work until 04/21/2008. He has since had shoulder surgery.

Employee was operating a continuous miner in the #6 face, where the mine floor was wet and muddy. He had finished loading a coal hauler and as the hauler was pulling away, it began to spin which allowed the rear of the hauler to slide over, striking EE's right ankle.
Injured was trying to lift a spool of wire rope on the bus when she felt a pain in her lower back, right hip, and leg.
He was shoveling a coal pile on the 9th floor of the batch weigh building. He felt pain in his lower back; diagnosed with strain to his lower back.

The production crew was using the roof bolter to pull the miner cable back. The employee activated the tram control in the wrong direction causing the roof bolter to twist. He caught his right leg against the transformer resulting in non-displaced fracture of the tibia.
Employee's injury is the result of continuing pain caused by a mantrip accident that occurred on July 27, 2007. Due to increased pain he required surgery to repair tendon in his left shoulder. He was off from 3-4-2008 to 3-24-2008.
Employee was changing out a sieve box and as he was pulling on it he felt pain in his lower back. Due to complication this employee started missing work on August 13, 2007.
Employee was welding and a piece of hot slag burned his right calf. Burn became infected. Dr. lanced the area; a blood clot and staph infection formed and employee was placed in hospital. We were not notified that this was work-related until 8-6-07.
While bolting top, a piece of cap coal/slate approx 18"" x 24"" x 3"" fell out striking employee on left forearm and left upper leg.
Employee Claims: His foot slipped off of a ladder and he fell to the ground, landing on his feet. He felt pain in his back.
While employee was operating a 980 loader, he felt pain in his back.
Using hydraulic tension equipment to put tension on cable type roof bolts, he twisted his back.
Employee was lifting a 50 pound bag of rockdust when he felt a slight pain in his lower abdomen. This pain was diagnosed as a right inguinal hernia on August 3, 2007.
Employee was standing in intersection preparing to cross over the continuous haulage unit, when a piece of soap stone rock approximately 36"" x 24"" x 1"" on the ends to 10"" in the center, broke loose from a roof bolt and fell striking him on his right foot.
Bending metal plate on truck bed, while swinging the sledge hammer, something in lower stomach pulled.
Employee stated that he was sore from roof bolting the last day or two and was helping hang the Cont. Miner trailing cable. After he and three others hung a couple X-cuts of cable, he told his supervisor that his back and hip was hurting. Employee continued to work and started missing on 10/07/2007.
Employee was attempting to move a wet brake, when he felt pain in left side of his neck. Hospital diagnosis with possible shingles. Later diagnosis with muscle stain, possible tear, on July 30th.
EE slid on piece of metal under rockdust and fell on right knee.
Employee was installing a roller on the under belt of the cone crusher when he twisted wrong and hurt his back. He didn't report this until Monday July 30, 2007. He came to work on Tuesday July 31 but left early to go to the clinic. They put him on light duty so he came back to work on Aug. 2. He then went to his own doctor and he told him to take two weeks off.
Employee lifting tool box when latches opened up causing him to hurt shoulder. Medical received on 7/31/07 indicates a strain/spasm of dorsal paraspinal.
Employee slipped and fell while greasing a D8 dozer. When he landed he hurt his ankle.
Injured was roof bolting when rock fell from roof hitting him in left elbow and forearm (bruise).
Injured was removing blocks for a regulator at a stopping when a section of rib fell and knocked him into the side of trickle duster, hurting leg.
He is a loader operator and has had problems with his back before. On July 26, 2007 he had back pain from using levers in loader. Missed work July 27th for Dr appointment. Returned July 30th for an hour and a half, then went home because of pain. Has not been released to full duty yet.
Injured was installing roof bolts using roof bolter and had hand in a pinch point area and when he lowered the boom it caught his hand and cut his pinky finger on left hand.
Employee stepped back and tripped on a cart. He fell and sprained his right wrist.

Employee was watching the left side bolter scale a piece of stone on the left side of the miner when a piece of stone fell 3 straps outby on the right side of the miner striking him on the head. The material slid down cutting his right ear.
EE STATES THAT HE WAS PULLING A HOSE AND SLIPPED WHILE WALKING ON A LIME PAD. STATES HE JARRED RIGHT KNEE AND CAUGHT HIMSELF WITH RIGHT LEG SLIPPING UNDER HIM. EE DID NOT FALL TO GROUND. EE'S TREATING PHYSICIAN TOOK HIM OFF WORK ON 08/20/2007.
EMPLOYEE WAS TEST RUNNING A 1710 CUMMINS ENGINE WHEN A FREEZE PLUG BLEW OUT STRIKING HIM ABOVE THE BROW AND SPEWING HOT LIQUID OVER EMPLOYEE'S NECK, HEAD AND ARMS.
While the injured was splitting a rock, the rock shifted and the injured lost his balance trying to move out of the way and tripped. The 30""x36""x5"" shifting rock hit the injured's leg causing a hairline fracture between the ankle and knee.
Employee was operating a roof bolter with his right leg outside the operator's compartment. The roof bolter came in close proximity with the mine rib. Employee's right knee was pushed into circuit panel in operator's compartment.
Employee working on jaw crusher using a porta power when pin slipped out and hit him in nose
Employee was operating the dozer when suddenly it got stuck in reverse and caused jarring effect.
Hooking chain on turntable of saw, operator told loader operator to lower the boom on loader. The chain pinched end of finger with chain. Replaced short chain with long chain.
EMPLOYEE WAS WELDING ON ABOVE STRUCTURE WHEN HOT SLAG ENTERED RT EAR CANAL. EMPLOYEE WAS SEEN FOR MEDICAL TREATMENT AND RELASED FULL DUTY ON 7/26/2007. EMPLOYEE HAD OUT PATIENT SURGERY TO REMOVE A PIECE OF SLAG FROM HIS EAR AND REPAIR EAR DRUM ON 10/30/2007.
Raining most of the day, ground was muddy and slippery.
On July 25, 2007, the injured was walking on the crossover of a drill when his foot slipped, injuring his knee. As the months went by, his knee pain continued to get worse. Please note, on May 1, 2008, surgery was performed to repair a torn cartilage, thus making the injury a reportable event at that time.
Employee bent down to pick up a piece of metal when he stood up, made note of slight pain. He continued working making no other complaints and finished his work day. Came to work the next day, no complaint, finished work day. Third day he stated at 11:30 am he needed to leave for a doctor appointment for back pain. He left work at 11:30am.
Employee said he hit a hole in the road and it caused him to go numb from his chest to his feet.
Employee was helping to lift a pipe in valley fill, causing muscles in his back to be pulled and strained.
Employee was lifting the tongue of the diesel pod duster to connect it to the Getman tractor when he felt a slip in his lower back. He felt pain and a burning sensation in his lower back that continued to worsen as the night progressed. He sought medical attention after the end of his shift and was removed from work for three days.
Employee is a stock truck driver. He was stepping down from his Euclid R22, slipped from the step and sprained his ankle.
Employee was dumping a 777 haul truck, when the fire extinguisher became dislodged and fell onto the frame of the truck. When the employee lowered the bed of the truck, the fire extinguisher was crushed, filling the cab of the truck with the contents of the fire extinguisher.
While handling pipe, pipe rolled and squeezed his right wrist. Fractured it.
Employee grabbed the pipe and put it in position to line it up with the top head so the driller could stab it and screw the sub onto the pipe. The driller came down a little too fast and the savor sub caught the edge of the pipe pushing down on the pipe to kick outward at the bottom causing a crush injury to his right small finger.
Employee stated that he was assisting in replacing a 6"" pipe. He stepped on a piece of angle iron which bent and his foot slipped. This resulted in his foot going between two pipes, causing a possible strain to his lower back and right thigh. Did not start missing work until 7/30/07. Doctor has him off work pending an MRI.
Employee was helping hang a roof bolter trailing cable when he felt pain in his back. He continued to work until June 20, 2008 when he began missing time.
Employee pushing a 4 wheel cart #1 conc ball mill floor, back wheels stuck on water hose. Lifted rear of cart to clear water hose felt a twinge in his low back. He did not seek medical treatment until two days later 7/27/07. We received medical 7/27/07. Employee has numerous back injuries over the years this seems to be a recur of old low back injury.
EE was carrying a screen (4' by 6') up the stairs to the plant and strained a muscle in the middle of his back. He stated ""I do not feel like I need to go to the doctor."" He called in sick on the morning of 07-27-07 and stated he felt he needed to go to the doctor at this time to get his back checked. He worked all day 7-25-07 and 07-26-07.
Employee parked scoop in roadway, walked around it between the canopy and rib to move scoop when the rib rolled striking his left calf.
Employee states while standing on a ladder topping off some cribbing, he lost his balance on the ladder. As the ladder fell sideways, the employee jumped off, landing on the ground and spraining his right ankle.
Cleaning oversized rock out of grizzly bars at the pit hopper. Swinging 10 lb hammer lost footing, slipped and landed on his motorola radio and a piece of steel causing a large hematoma to the right buttock.
Employee complained about severe pain in left chest area and side. This is not work-related. EE is having tests run and results are not known at this time.
Employee was walking through a cross-cut to reset a water pumping new location, when a section of draw rock fell from between permanent roof support and struck employee, knocking him to the ground.
Employee was moving from one dozer to the other when he slipped due to muddy and wet conditions.
Employee was driving too fast and did not slow down properly for the embankment. He lost control. The box started bouncing around and went off the embankment which caused him to tip over causing the employee to bump his head. Employee was wearing seat belt at time of accident.
EE was carrying belt clamp on left shoulder with his arm extended and hand on one end. He twisted his torso, and in doing so, the end of the clamp came into contact with the tripper car. The tripper car forced the clamp into the rib, pinching his left middle finger.
Employee reported: He had unloaded (manually) two golf cart chargers from slope supply car. When he reached and lifted a Fletcher bolter blower motor from the supply car, a sharp pain was felt to his right shoulder. The weight of the blower motor is approximately 35 lbs.
Employee was making a hose for a loader and smashed the ring finger on his left hand. The tip of his finger was amputated.
Employee stated that when he picked up a bucket of oil and then later unloaded his tools his back became aching and stiff. He had a previous back injury and this incident has been under investigation. It has now been determined to be a new injury and is now reportable.
Employee was getting off the bus, fell and landed on his back on the SCRS.
2 EEs were assigned the task of hanging a solid curtain in the x-cut between No. 5 & 6 entries. EE was working off a 6 ft step ladder & was descending ladder when he slipped to the mine floor as he was stepping from the 3rd rung of the ladder to the 2nd rung. He complained of pain in back but finished his shift. The area was wet & muddy and there was mud on the ladder rungs.
Helping change batteries in scoop. When he leaned over to unplug jumper, his back started to hurt.
Injured was riding on a mantrip going out of the mine; mantrip pulled out of travel way into crosscut to get out of oncoming traffic's way. When pulling out of crosscut injured's foot became wedged between the lip of the foot rest of the mantrip and an overhang on the rib, severely bruising his left foot.
EE was removing the panel board on a roof bolt machine and while pulling on his wrench it slipped which resulted in a fracture to his little finger on his right hand.
Employee backed into cement block pillar causing concussion.
Moving a pallet by hand, lost his footing and strained his lower back.
Employee was descending a fixed ladder between mills #1 & #2 after unplugging the feeder. Upon stepping down onto one of the runds, his foot slipped and he struck his left knee on part of the ladder, causing a small laceration just below the kneecap (required sutures).
Operator was tramming s.c. to face, car hit hole in mine floor jamming back.
He was walking back to the continuous mining machine from the loading machine when a piece of rock fell from between the straps striking him on the head and neck causing strain and contusion.
On 7/24/07 employee stepped up on shield hauler forks that were at an angle, slipped in oil that was on forks, and twisted his right knee. Employee continued working until 9/18/07 when he sought medical attention and began missing work. On 10/22/07 information was received on work-relatedness of this absence.
Employee stated that he and another person were trying to remove a steel cover on a Cont. miner with a pry bar. The cover came loose and the bar slipped, and when it did, the cover went up then down and struck the employees thumb. This caused a fracture to his thumb nuckle. Surgery had to be preformed on the thumb.
Employee stepped off of 0320 980 loader and twisted right ankle on slick rock. He was taken to company doctor and referred to orthopedic specialist. He was restricted to limited duty and began losing time 07-25-07.
Employee was shoveling coal onto conveyor belt. The shovel struck the belt and roll jerking the shovel from the employee's hands. Employee reported burning sensation in his lower back later in the shift.
Employee was removing AFC chain links after they had been cut. A flight was also removed. Employee picked up flight to throw on headgate when he injured his left shoulder. Employee continued working until 12/5/07 when he sought medical attention and was prescribed restricted work duties.
Employee states he pulled his lower back muscle ""sometime during the day"" while either helping to lift a travel shaft from a dragline shovel or while handling rolls of wire mesh to make screens for the coal plant.
EE was operating left side of bolter, installing a roof bolt, when a hose busted and sprayed hydo oil in face and chest area.
Employee was returning to pit and lost control of rock truck and hit head on back of cab.
Changing grease bucket in the Alpine miner, when he climbed down he slipped and strained his right shoulder.
He was watching the welder. He was told by the welder and two other employes not to watch it.
Operator fell from ladder while exiting 769c Rock truck scraping arm and complaining of back pain.
Employee was struck by a piece of draw rock while checking supplies on a roof bolter. He was transported to a local ER where x-rays and a CAT scan indicated he has a compression fracture of a vertebrae in the lumbar area of his back. He was released and is scheduled to begin physical therapy. He is expected to return to work in 4 to 6 weeks.
Attempting to attach a weld trailer to the hitch on a pick up truck, trailer rolled forward and caught his hand between the hitch and the truck.
Standing on a step stool changing fuel filters on a haul truck when the stool gave way and he fell. He made contact with the upper part of his body and the ball joint on the haul truck.
Employee was on his back tightening a hydraulic hose that was overhead when he strained his shoulder as he put pressure on the fitting.
He bent over to untangle miner cable. He felt pain in lower back.
Employee was walking past a haul truck which had stopped in the drift. The haul truck driver thought the miner was clear & began to move forward. As he did the employee's right foot was run over by the back tire & when he pulled it out from under the tire he felt like his belt twisted him around causing the left femur to fracture.
Putting rib bolts 8 x R 2nd row 1st cut 18"" x 18"" x 6"" thick coal hit the back of neck.
Employee was cleaning up for shovel with Rubber Tire Dozer when he backed over some uneven ground and felt pain in his neck and shoulder.
Installing 8 ft. rope bolts, bolt bent, attempted to straighten by spinning chuck. Did not notice head out of chuck. When the bolt began to spin, it came out of the chuck. The plate and bolt head struck his left shoulder area. He completed the shift, went home and later sought treatment.
Rock wedged between oval wheels; putting chain around rock to pull it out, a tire blew up and broke pinkey finger.
While cleaning and servicing a Great Western Screen, the employee put his hand down the discharge sock (17"") and his finger impacted an airlock. The employee lost the fleshy front tip of his middle finger. The employee was taken to the hospital and treated for the wound. The employee failed to follow the documented company policy for Lock-Out/Tag-Out and was terminated.
Injured stated that he was removing draw rock (from roof) with hammer when ""wrong"" rock fell out. Operating roof bolter operator's side waiting for other guy to get finished. EE states that he bent over and the rock fell and hit back of head causing contusions.
Employee was making a flexco belt splice with a co-worker. As he reached for the nail plunger, the co-worker accidentally hit his hand with the hammer.
Employee stated that when he threw a chain across a set of batteries to secure them on a truck, he felt a sharp pain in his back.
The employee was sweeping the shop floor and kicked a wheel chock out of the way and strained his groin muscle.
Employee lost control of the rock truck he was operating and went off the road, ran over a berm where the truck dropped 2' to 3' down into pit area.
Employee was injured in July but did not miss work. The first day she missed was Sept 11, 2007. She was walking along B-5 belt & slipped, spraining her wrist & hitting her left knee.
Walking beside continuous miner and slipped on rock, fell and strained left knee. Began lost time on 7/23/07.
Employee was helping move a discharge hole pump and slipped and fell.
EE set the parking brake and was preparing to exit the truck to inspect it, when he noticed the truck fire. He shut down the engine and activated the suppression system by pulling the pin and striking the actuator. EE jumped from the drivers side catwalk. EE sustained a fractured left hip and dislocated his left elbow. Fire was extinguished in approx 20 minutes.
Using a torch to cut out master bad bolt when slag popped in and around side shield. Employee did not seek treatment until 07/24/2007 and did not inform employer until 07/31/2007.
On 7/21/07 the employee slipped and fell while working in slope. He continued to work until 9/12/07 at which time he started missing work due to pain in rt shoulder.
Employee was trying to move a rock that was hung in the section tailpiece with a piece of pinner steel. The steel slipped & one end hit the employee in the chest.
Had hand on drill pot, when cap coal came down hitting hand.
Worker on conveyor tail pulley on maintenance shop floor knelt down on left knee, grabbed pulley and pushed forward with right hand when something in left lower back popped.
The chain on drill mast was broken (it was wedged into the top sprocket). When pried loose it pulled his glove into the chain and the sprocket, pinching his right hand, thus requiring 17 stitches.
Employee was cleaning gob on off side of #3 belt and the rib rolled and covered him up. He was covered up for about 1 hour; it took 7 men to get him out.
Changed air filter on the dozer, standing on the track and went to throw the used filter back on the service truck, lost footing and fell, spraining neck.
Employee was loading supplies on his machine (and strained his back).
EE stated that while walking toward the continuous miner, a piece of rock fell from the mine roof, striking him on his neck and shoulders.
Employee reported left eye irritation on 7/21/07 that had continued from previous night shift. Medical exam located a foreign body and rust ring. Metallic foreign body was removed 7/21/07 and rust ring was removed 7/23/07. Doctor took employee off work until 7/25/07.
Employee was assisting in a shovel move. The cable wrapped around a rock and as the employee reached with a cable hook to get the loop off the rock, the shovel moved taking up the slack. The cable slid up over the rock, breaking off a piece of the rock, which flew and hit him in the mouth, chipping 3 teeth.
Mains Section. Setting timbers in Mains when he turned and pulled his left hip.
Employee was stepping off a track drill when her foot slipped causing her to fall on the ground.
EE was replacing some lacings on his conveyor belt. He was using a hammer and chisel to split the lacings, when a small piece of the hammer broke off and struck EE in the left chest.
At the direction of MSHA's conference & litigation rep, Solid Energy is reporting that EE claims he suffered an injury to his neck on 7/20/07 which he alleges resulted from him running into a rope bolt. This report is being submitted solely to terminate Ciation #6645307. By submitting this report, Solid Energy does not concede or admit that this was a reportable accident or injury.
He lost his balance while rockdusting and fell against the rib, cutting his forearm.
Employees was assigned housekeeping duties on the second level of the work site. A hole covered with plywood and wired in place to prevent movement was apparently removed by the injured employee and he fell into it.
Employee reported to his section foreman he twisted his knee at 7:30pm on 7/20/07, when he was hanging a cable in the #10 entry. He missed 3/27 & 28/08. He returned to work on 3/31/08. He started missing 4/1/08 and he had surgery on his knee 4/10/08. It was determined after his surgery it was work related.
The EE was cutting off the sideboard of a dozer blade. After making the cut, he struck the metal with a hammer to knock off the blade. The metal did not fall. He grabbed metal with hand and metal fell striking him on top of right foot behind the hard toe cap.
EE was trouble shooting a power problem on No.221 roof bolting machine. He was walking from No.1 entry to the tool car in No.5 entry, when he slipped on slick bottom in No.4 entry just inby the feeder breaker and fell striking his left elbow on the mine floor.
Employee was hand scaling pulling on a slab from back in m3 and strained too hard. He was taken to company dr and referred to specialist. He began loosing time as of 7/20/07 and is scheduled for hernia surgery on 7/25/07.
Employee fell through opening in walkway in coal plant.
Emp. had pressure applied to pipe that he was holding in place to be welded. Emp. sneezed a few times and felt a pop in his mid stomach area. He later noticed a bump. Emp. has not had anything done to repair the hernia at this time.
Employee was operating the No. 187 continuous miner in the No. 9 entry and as the continuous miner was backing up, a loop of water line folded under the tail of the continuous miner. Employee stated that he stopped the continuous miner and reached down and was pulling the water line out of the way, when he felt a tearing feeling in his right shoulder.
Employee stated that he started to feel sensation in his thumb and fingers. A few weeks later this tingling started turning into pain in his neck and shoulder. The pain was moderate but constant.
EE was standing on shield #128 when a bounce occurred on the face and the concussion of the bounce threw the EE into the legs of the shield and onto the toes of the shields.
Operating roof bolter putting up middle bolt when a piece of rock fell striking him on left leg. Continued working until physician took him off work on 8/3/07.
Employee was walking to a survey control station when he slipped and fell on a concrete flume.
Employee was dismounting a water truck at the water fill up when he felt pain in his left arm. Employee did not report incident until 7/20/07.
Employee stated that his arm got caught in piece of top (belt) structure that he was loading into car. He stated it hyper-extended his right elbow.
Employee was resetting a 480 volt breaker, when he recieved an alleged electrical shock.
Employee got something in his eye while a welder was grinding on the crusher jaw. He began losing time 7-23-07.
Climbing on gear box, lid slipped and he fell on his head. Head injury and cervical strain.
EE and co-worker were removing some old discharge pipes from the screening deck. EE made his 1st cut and handed the torch to co-worker to walk around the other side. When he walked around the pipe, he stepped in a hole where a 6 inch pipe had been cut out the evening before. When he stepped into the hole he twisted his ankle and fell against a hummer shaker.
While roof bolting the drill steel became stopped. Harper towered the drill boom to check the selction, when he raised and back up. He placed his hand on top of the drill pot causing it to be pushed into the drill steel
While trying to remove broken bag, employee slipped on bag chair conveyor (stopped) and struck left elbow on framework, dislocating left elbow.
EE was standing on shield #128 when a bounce occurred on the face and the concussion of the bounce threw the EE into the legs of the shield and onto the toes of the shields.
Employee was in process of positioning a distribution transformer onto a concrete pad using a boom truck. The transformer was approximately 1-2"" above concrete pad when the frame mounted clack bolts for the Manitex boom sheared and the individual jumped from the operator's deck from a height of 42"" landing on the ground, twisting his left ankle fracturing it.
EE was replacing a casing in a pump and while lifting the pump casing the cable rachet puller broke, causing the pump casing the fall and strike the employee on the right ankle.
Using die grinder in shop, employee wearing safety glasses, a sliver of metal got around glasses. Off one day after.
Employee was operating a motor and while dismounting vehicle. She placed her foot on motor step and twisted around to step down. When she did she twisted and felt pain in her right ankle. She could not get in to see an orthopedist until 7/20/07 and was dr with a fracture of the fibula.
A mantrip and maintenance personnel vehicle were passing in a travelway when the back of the mantrip hit the back of the personnel vehicle. Employee's right leg was hanging outside the mantrip and was struck.
Crushing crew was adjusting the cone spacing and were removing the shims when one of the crew members accidentally moved the pump into the downward position catching the left thumb of the other crew member.
While hanging a cable employee felt pain in right shoulder and arm.
EE was driving a 777 CAT rock truck, while a loader operator in a 992G was was loading him. A rock came out of the loader bucket and hit the off side of the rock truck bed, jarring him and causing him to go side to side in cab of rock truck.
While employee was entering his truck he felt a pain in his left knee. He then could not put any weight on his leg.
HE WAS PULLING ON HIGH LINE, WHEN HE STRAINED HIS LEFT SHOULDR/NECK. ** STARTED MISSING **08/21/07**
Employee was climbing down the ladder on the 992-G loader when his foot slipped hurting his shoulder.
While dismounting from the water truck the employee experienced a sharp burning pain in the left ankle. The employee states her ankle was not twisted or rolled and that she has had ankle surgery in the past. Upon seeing an orthopedic doctor, the doctor agreed this is a chronic or a relapse of the prior injury.
Employee claims he stepped on a rock and twisted his knee while walking from the primary to a conveyor walkway. The result was torn cartilage in his knee.
Employee stated that he was on top of the cont. miner and had been servicing the head hing pins. As he stepped off he put his left foot down on the ground, slipped and struck his right knee on the top cover of the miner.
Employee was positioning bolter and set stabilizer. Bolter moved unexpectedly, because of uneven floor, struck left knee. Employee was released for full duty-7/27/07 Restrictions added. No lost time work days prior to 8/3/07
Employee was walking on welded angle and fell when angle weld broke. Employee suffered cervical sprain when he fell. He was wearing proper safety equipment and did not fall completely to the ground.
The 131 Van was leaving the 6150 Que, hit a bump, which caused the passenger in the rear seat to strike her head on the roof of the van. She complained of neck and arm pain, was taken to the ER, checked out, and released to next shift (7-22). Employee called off next shift (7-22), went to her physician, and was taken off work. ONGOING
EE was on top of sand silo doing electrical work. He slipped, grabbed a bar above his head, trying not to fall, injured right shoulder.
Employee stated that he was pulling on rockdust hose and felt pain in his groin area. He continued to work until 7/23/07 at which time he was diagnosed with a bilateral hernia.
Feedbox on #5 mill was jammed up with ground silica material. Employee shut down mill and cleaned feedbox out. Employee started mill back up after completing cleaning. Feedbox again jammed up, due to fines from seperator. Air blast occurred splashing product into employee's eyes.
Tailgate drive cover came off. Cover came down coveyor to shearer. Cover jammed into shearer gear box and sprocket. Gear box bolts sheared sending bolts flying. One bolt hit back of employee's left hand against shield leg ram.
Employee was crossing top of #1 1 North Mother belt when his right foot slide out from under him and twisted his right knee. The employee continued to work until 8-29-07 when he began missing due to the injury.
Operating miner taking second cut in #2 right. He set miner over to the left side and started trimming bottom a piece of rock came up and hit him in the face and mouth it split his lip and broke front tooth. So stitches in bottom lip. off 5 days came back 7/27/07
Employees were backfilling a lining overlay. SOS employees were waiting to complete their backfilling when the wind from a passing thunderstorm picked up and began to move the liner. Our employees ran across the liner to help and were thrown 50 feet into the air by the liner. Two SOS employees were injured.
Stepping over ""I"" beam on belt, twisted his back.
Employee was hanging bolter cable, reeling up cable. The cable struck him in the mouth breaking a tooth and busting lip.
Employee was loading a piece of track rail on a rail car and it slid off striking him on the left ankle.
EE was installing a roof bolt and strap when a piece of draw rock fell striking him on the head and back.
A loose piece of rock fell and struck the employee in the lower back.
The laborer was under a moving conveyor; when he reached up the belt caught his hand and pulled his arm between the return roller and conveyor belt pressing his forearm against the frame of the conveyor.
Walking around the plant to start servicing, he stepped on rock on ground and twisted ankle.
Employee was caryying to Toyo Pump across the E16 track at #1 wall when he slipped on a track & felt pain in his left knee. Employee began missing work on 7/19/07.
Employee was lowering a spout to load a truck with Lime Kiln Dust. The spout needed to be adjusted to go into the truck hatch properly. The employee jerked the spout with the cable and felt a sharp pain in his right groin area. Doctor determined it to be a hernia.
Employee was changing screens and bumped his elbow. Employer was notified on 7/19/07.
EE was sliding skirt board plates into hopper. He twisted as he attempted to push the skirt board to a co-worker on other side of opening.
Heard noise in motor area, went to investigate. Turned to leave engine room, radiator hose blew a 2"" hole in it, spraying water on lower legs & down into boots.
HE WAS CHECKING SUMP PUMP , WHEN HE RAN INTO I- BEAM , JAMMING NECK. ** STARTED MISSING WORK ON 02-12-08**
EE was moving a roof bolter using the boom controls. A large rock fell out between roof bolts and struck him on top of the head, between his shoulders and down his right leg. The rock pinned him to the ground. He had abrasions to the top of his head, soreness to the shoulders and right knee. July 26 he was taken off work by a Dr. to repair a torn meniscus on his right knee.
EE was helping haul supplies to the # 1 section. As the track motor rounded a curve on a steep grade, the motor man couldn't hold the motor back and it slid down the grade and struck another motor that was sitting at the bottom of the hill. EE was in the opposite end of the motor and received bruises to the back form being jolted against the track motor frame.
Was going to wipe dew off right side mirror of Euclid, off set on platform (also there was dew that morning), slipped and fell off the front.
Employee was pulling shields when rock fell off the top of the shield, knocking employee to the ground. Lost time injury as of May 27, 2008.
The employee was operating a coal hauler when it hit a hole in the bottom causing him to strike his head against the canopy. Due to unavailability of the doctor and the mine being on miner's vacation, this incident is now being reported. The employee has degenerative changes along his spine.
EE was loading resin onto a roof bolter, as he lifted the resin, he felt a sharp pain in his lower back.
Employee was working at the semi-automatic palletizer, handling 100# product bags, when he felt a pop in his left shoulder. Employee sought medical treatment the following day, was advised by the treating physician to remain off work the remainder of the week, and he returned to work full-duty effective Monday, July 23, 2007.
Miner was climbing down ladder on wheel loader to check hydraulic oil level. He thought he was on bottom rung of ladder but was one rung higher. This mis-step caused him to fall to the ground resulting in a fractured right wrist.
Employee was grinding inside of fire-box when a piece of metal by-passed his protective eyewear and lodged in his right eye. He went to doctor the following morning and had it extracted, causing him to miss one day of work.
Employee was tramming his shuttle car and went through a break hitting a parked scoop. He injured is left knee on the deck of the shuttle car. First Aid administered.
Employee complains of back spasms/pain and has a past history of spasms/pain in the same area of his back. After driving a tractor down the 4b travelway the employee felt spasms/pain in his lower back. Employee stated that the rough road aggravated this condition. Employee did not start missing work until 8/4/2007.
EE stated he was dragging a section of line curtain when he felt a burning sensation in his left groin area. The incident investigation revealed that the line curtain was wet & muddy causing the curtain to be heavier than normal. EE was diagnosed with a hernia but continued to work until 8/27/07 when he started missing work as a result of this injury.
Employee was changing bucket on skid loader. He had it lifted off the ground and it fell landing on his ankle.
Operator was installing bolt into hole while attempting to straighten bolt, it spun around twisting his upper back.
Employee claims fell from loader steps two week ago. He claims lower back pain. He never reported his fall to any of his supervisor until now.
Employee was operating a rubber tire dozer in the pit when the air seat lost pressure causing a strain to the right rib cage area.
While changing rollers on refuse belt, the member said he felt a pain in Rt shoulder. This accident was not determined to be reportable until 8-10-07.
Employee was building a bridge just outby 4 North head. When he turned to pick up a timber he felt pain in his back. He injured himself at 11:30 am and reported it at 3 pm. No witnesses were present. He visited specialist on 9/6/07 to confirm injury.
The injured was in the process of retrieving a B.O. Getman tractor out of the slope. In the process, the tractor started sliding and struck the arches in the slope causing an avulsion to the back of his head.
Injured employee walked away from half empty coffee cup. After working for a short time employee returned to his coffee and drank a couple of ""gulps"". Employee noticed ""chemical"" taste in coffee and began to vomit. It was discovered that a co-worker had put a liquid chemical de-icer into the coffee causing a possible poisoning situation.
While ee was leaning over greasing the miner, a 25 lbs rock fell fromt he roof striking ee in the back.
EE WAS LIFTING COVER OFF OF A RECLAIM HOPPER. A SACK HOLDER FRAME THAT WAS OVERHEAD FELL AS A RESULT OF VIBRATION AND HIT EE ON LEFT LOWER BACK, KNOCKING HIM DOWN TO A SITTING POSITION. EE DENIES PAIN OR DISCOMFORT TO ANY OTHER PART OF BODY EXCEPT FOR LOW BACK. ON 07/17/07, EE'S TREATING PHYSICIAN TOOK HIM OFF WORK. EE CURRENTLY LOSING WORK DAYS.
Employee was working on pump discharge line, fixing a leak. He was pulling tape off pipe when a hose clamp band came through his glove and cut the palm of his left hand below his thumb. It took six sutures to close cut.
Employee was helping foreman fix a broken vertical pin on the No. 3 shield. The foreman hit his right index finger with the hammer. Cause of this injury was safety awareness and proper protective equipment (gloves). Compliance with rules was a factor, protective equipment was a factor, miner proficiency a factor, mining equip/systems not a factor.
Ee was using a cutting torch when a hot piece of slag rolled into his boot. (He was wearing an open type boot.) By the time he got it off, his leg was burned badly.
Employee was hanging a piece of line curtain when a piece of slate measuring 3' long x 1' wide x 1-2"" thick fell approximately 6"" striking him on the left shoulder.
Piece flew off dozer track hitting him in the right side of face.
Employee was swinging sledge hammer, missed and hit finger. Finger required stitches and end of finger fractured (3rd finger left hand).
Injured was drilling a bolt out of a shaft on top shaker. He remembers rubbing his eye. Later that evening his right eye started paining him. He went to ER where they removed a piece of metal and advised him to follow up @ eye clinic. They cleaned out a rust ring that had developed and excused him from work the following day. He returned to full duty July 18th.
Employee stated he was attempting to open a door, he pulled on it first then jerked on the handle and his hand slipped off straining his finger. First report of injury was received 7-18-07. Employee had surgery Aug 3rd. The difference between the reporting date, surgery date and accident date was due pending a medical report verifying injury.
Employee was hanging miner cable when he experienced pain in the groin area.
Employee was traveling over a belt overcast when he fell through the top landing on the mine floor below. Cause of this injury was damage to the overcast occurred and no-one dangered off the area. Compliance with rules was a factor, protective equipment not a factor, miner knowledge a factor, mining equipment/systems not a factor.
Employee injured himself on 7/13/07 but did not seek treatment. He went to the doctor a few weeks later and has been off work since 09/06/07.
Employee was climbing up 10 ft berm. When he grabbed limb for support it gave way causing him to lose balance and fall injuring his back. He began missing work due to this injury on 7/14/07.
EE was moving supplies from on top of the hill to the pit area. He was climbing down the ladder on his loader; he stepped in a hole and felt a sharp pain in his left shoulder. He went to the doctor and was put on anti-inflammatory medication and put back to work on 8/21/07; he went back to see orthopedic and was put off work.
Employee slipped while entering the basket on a manlift and fell. The basket was about two feet from the floor.

Employee was standing at the E-15 longwall headgate area when a roof bolt broke/shot from the roof striking the employee in the right arm. Diagnosis as a contusion.
EE lifted cover improperly with chain hook in the hole and it would not fit in cover had already fell off once and he hooked it the same again. Then get cover in place and let the crane down, relasing tension. Cover folded and fell on his. where he was standing in front of the cover. He was knocked down and the cover landed in his leg, the hinge area striking him.
While installing roof support employee activated the lever by mistake causing the roof bolt to bend striking him on the right thumb. X-rays indicated no broken bones. Employee released to return to work. Pain persisted and employee went to another ER where a 2nd x-ray indicated a broke bone in right thumb. EE scheduled to have a cast put on and began missing work July 16, 2007.
EE was lifting a slab of stone into position to be split when he sustanined a sharp pain to his lower abdomen. Drs diagnosis: strain to abdomen.
EE was moving a large piece of stone into position to be split. The stone slipped from his grasp and struck his left leg 3 inches below his knee causing a laceration. He received serveral stitches and returned to full work capasity on 7/16/07.
Employee was backing scoop up in low coal when he pinched his head between the roof and the seat of the scoop.
Employee was helping another man lift the miner jack and it slipped from his hand & fell on right hand causing fingers to get hurt.
Employee was helping to put a supply car on track. After walking back to motor he had pain in left knee. Diagnosed with a torn meniscus on 7/16/07.
DID NOT START LOSING TIME UNTIL 12/05/07. While jacking a shield back onto track, hit left knee on the footwall. Surgery required. Did not start losing time until 12/05/07.
EMployee was picking up a barrel for pump when he felt a strain on his left shoulder. Employee was originally given full duty release, but shoulder has not gotten any better. He had surgery on 1/14/2008 and is currently off work.

Employee was operating a shuttle car, ran over a rock in haulway, bumping his head against roof. Went to doctor on 7/12 & 7/18; started loosing time 7/18/07.
Employee was operating bolt machine, he was swinging head and canopy caught the top, causing rock to fall, hitting on left ring finger.
While turning around in shuttle car to change directions, employee twisted his left knee.
Employed injured back and ribs while pulling on drill steel
Employee stated that a small piece of rokc fell from coal rib striking him on the left shoulder.
Co-worker was hitting a drill steel and wrench on the bolter. He was standing to the right and behind the co-worker when the steel came out of the wrench, bounced back and struck ee in the face.
Employee stepped in a hole causing him to twist his back. Did not become lost time until 10-1-07 and management was not notified until 10-8-07.
EE was helping hang the continuous miner cable across the #3 entry on the 001-0 mmu. When he lifted on the cable, he felt a sharp pain in his back and groin area.
Victim was changing air bag on back tandem of wheels on Kenworth tractor. While loosening bolts on clamp of brake chamber, the back section separated from chamber, striking victim above & below left eye. Safety glasses were worn.
EE was aligning belt structure on the 3A conveyor belt aproximately 18 feet inby survey station # 368. EE stated that as he attempted to move the structure, a timber that was set next to him, fell over on him, striking him between the shoulders and injuring his back.
The employee was lifting a glue box when he felt pain/burning and heard popping sounds in his left hand/wrist. The accident was not deemed reportable, however, until the ee began taking off work due to complications from the injury beginning on Sept. 19,2007
Employee was attempting to remove a large lump of coal from the raw coal feeder with a jack hammer. The employee stepped on the handrail resulting in him falling onto the jack hammer.
Person felt pain in right shoulder while lifting a duke (water prop). He contined to work. Went to DR on 7/16/07. First day off work on 7/17/07 for torn rotator cuff in right shoulder.
EE felt severe stomach pain, which kept increasing, while stacking bags of hydrated lime.

Employee was closing valves on the evaporator body, when hot crystals overflowed to the floor, surrounding employee. Employee received burns on bottom of foot.
On 7/10/2007 the employee was walking along a beltline. He stated that he slipped in some mud twisting his right knee. The employee has continued to work until 8/31/2007 at which time he started missing work due to surgery on his right knee.
An employee's previous back injury was re-aggravated when a rock rolled out of a loader bucket and struck the side of the haul truck bed he was operating.
