Accident type, without injuries

An unintentional roof fall has occurred in the #10 Entry of the Main South @ 19+20 - 20+40. This is an extension of a previous fall which has made it a reportable roof fall. The fall was approx. 120' feet long x 17 1/2' wide and 9' high. This area had already been cribbed and dangered off prior to fall.

An unplanned roof fall occurred in the #4 room of 1 Right Room, 2 X-cuts inby survey station #691. Fall was approximately 19' wide, 30' long, 4'plus high. Fall did not affect ventilation, equipment, but did affect travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered at approximately 11:15PM.

In 14th West Pillar section in the #5 entry a roof fall occurred at approximately 12:45 pm. It fell 18' wide x 31' long x 6' high. 5' resin bolts were installed during development. The cave buried two Mobile Roof Supports. There were no injuries. Two MSHA inspectors had been in the section observing mining prior to the fall.

A roof fall above anchorage occurred in the belt entry one crosscut from the outside. The fall was approximately 25'x18'x7'high. The fall was reported and investigated by msha. The area is being cleaned up and arches installed in the area.

Fall did not affect any ventilation, equipment, or travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered on 05/30/2007.

hot coal or spark ignited pulverized coal in dust collector bin, explosion occured on 4th floor, area is off limits and locked at all times. Safety features worked as designed and intended, rupture discs blew out, dust collector bags and cages, and tube sheet were damaged.

An unplanned roof fall occurred in the #3 room of 1 Right Room, 1 x-cut inby survey station #569. Fall was approximately 19' wide, 25' long, and 4' plus high. Fall did not affect any ventilation, equipment or travelway. Fall was timbered and dangered off. Roof fall will not be cleaned up. Fall was discovered on 05/30/2007.

Roof fall of an intersection in 2 South Mains, #2 entry inby 6 ft. of crosscut #13. Roof measured 17' wide by 20' wide by 6' to 8' high. The cave measured approximately 37' long. Roof fall was reported to MSHA Emergency Response Toll Free Number 1-800-746-1553.

While mining in number five entry, the mine roof fell, pulling out one row of bolts. The fall was approximately 25ft long by 20 ft. wide by 5 ft. thick.

A fall occurred on the 8E belt at xcut #8. 1 break inby spad #41027 in the #5 entry. The fall was bolted with 5' torque tension bolts, and spotted with 12'rope bolts. The fall measured 25'l x 18'w x 5-6; h. The was cleaned up and a canopy put through and crib built around it.

A roof fall was observed during the preshift exam in the crosscut between #9 & #10 entry @ 10+33. The area was a sump that was cut after mining. The intersection on both ends was cable bolted w/passive cable bolts & cribbed. The fall was approx 70'l x 12'h x 17'w.

At 8:30 pm power was lost to the mine. Evacuation called. MSHA was notified. After evacuation completed, electrician and supervisor entered the mine and discovered the 4160 volt cable had been damaged in an abandoned area of the mine, by a roof fall. The cable was disconnected, re-routed and repaired. This took place on the 500' level.

A roof fall was found on preshift on # 4 belt it was about 4'or 5'thick, 40'l x 20' w. It had been heaving in the #13 area. It will be set with timber or crib, then start cleaning it up bolting it back.

The mine experienced an unintentional roof fall on the MMU 001 working section, at S.S. 22+80, entry #2 of the 2ndNorthwest Panel. A 103(k) was issued at 2:00am.

A roof fall was discovered at the #2 seal of the third south set of mine seals. The fall measured approx 15'h x 20'by 30'. No injuries resulted from this fall. The time of accident is listed as the start of the shift.

A roof fall was discovered by the examiner on the 3 Right 2 East area in the cross cut adjacent to station 5231 between no. 2 and no. 3 entries. The fall was approximately 6' high x 20' wide x undetermined length. The fall will not be cleaned up but will be breakered off.

An electrical connector on the -50 hoist hydraulic unit shorted, resulting in a blown fuse in the control power circuit. This resulted in the hoist not operational for greater than 30-minutes. The failed component was replaced, system was reset and operational. There was no damage to the hoist.

Disruption of power supply because of electrical storm. Caused elevator to stop for about 15 mins. with 5 men on it. Back up generator was started & elevator was put back into service.

Subsidence that occurred in XC7, R6 of the west side of the mine. A sinkhole was found by the land owner and mine management was notified. The hole was approximately 25' x 40' and went to the mine floor.

#1 elevator was approaching landing. At complete stop the automatic doors open and came off bottom rail guides slider. The damage to the door slider effected operation of this elevator for more than 30 mins.

A fire occurred in #2 dust collector during the mill shut down/cool down phase. No plant personnel were exposed and there were no injuries associated with the event. Lost all of 208 dust collector bags, damage to the dust collector was minimal and the unit was returned to operation later the same day.

A possible frictional methane ignition occurred while the face of the #3 entry (approx 54' inby survey station # 1152) was being mined on the MMU 001-0. The continuous mining machine operator reported a flash, that was yellowish orange in color, which self extinguished immediately.

A roof fall occurred in the #9 seam, 3A submain, #9 and #10 entry, crosscut 32-35. The fall is 210' long, 18' wide and unknown height.

A loose connection caused a wire termination to burn off a speed control contactor. The mine evacuation started at 3:28 am and was completed at 3:42 am. The hoist was repaired at 4:07 am and tested prior to use. Miners returned underground at 4:15 am.

During the weekly examination, a reportable roof fall was found in E-Seam, 10 Headgate, entry 1 to 2, crosscut 2. The fall was approximately 8' high, 19' wide, and 15' long.

An unintentional roof fall occured in the #8 return entry outby unit 2. The fall occured do to the strata breaking at and below the anchor point. The area will be propped off. An inspection of all adjoining areas for adverse conditions will be conducted. The fall measured 20' x 20' x 4'. The fall area was located at 9+10 survey station.

The Cedar Creek elevator was out of service from 3:15 pm to 3:58 pm on 5/24/07 due to a door malfunction.

Unplanned roof fall above anchorage zone in 5th North East Belt #1 Entry & Crosscut 2-1 Entry. 20' x 40' in length. The fall was 55' outby the face of the #1 Entry & inby Spad #2883 25'. The roof was bolted with 42"" fully grouted resin & some 6' fully grouted resin spotted. The anchorage zone was 42"". The roof fall was 6'-7' high.

Vibration caused wire to loosen on mechanicl contactor that controls speed steps on the production hoist. The mine was evacuated at 4:10pm and completed at 4:23am. The hoist contactor wire was repaired, the hoist tested, and miners returned underground at 5:30am.

A coal outburst/floor heave occurred near the stageloader; this action moved the stageloader and longwall power controller. The result restricted travel from the longwall face.

A roof fall occurred on the 16th West retreating longwall headgate entry, inby crosscut 34. The fall was approx. 20 feet wide by 20 feet long, approx. 2 to 4 feet high. The roof sagged down on the stage loader impeding travel. Primary support was 7 foot resin bolts with additional 10 foot cable bolts between each row of bolts. Additional supports were installed in the area.

An unintentional roof fall occurred in the #1 entry of 4 south @ SS# 177+20. The fall measured 16'long X 12' wide X 8' high. No injury or property damage to report.

A roof fall was discovered in the 2 West 2 Mains, #2 entry, #4 crosscut. The fall measures approximately 50 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.

At approximately 1 pm a grease fire occurred on the #3 clinker cooler clinker breaker's north bearing. Due to excess heat from the cooler the grease caught fire. The fire was extinguished several times and flared back up. The fire was extinguished on and off for approxiamtely 50 minutes.

A roof fall occurred in the #9 seam, 1st southeast mains, crosscut 24, spad 16+90, between #2 and #3 entries. The fall is 30' long, 18' wide and unknown height.

Unintentional Roof Fall on #2 Track, #4 Entry, 19 block, Station 2055. Fall measured 30'L X 17 W X 6'H. Fall not initially reported due to believing it was an extension of a fall in #3 entry.

A roof fall was discovered by Mine Foreman and MSHA as they investigated 2 previous rock falls in an abandoned panel. The fall extended from spad 982 thru 980, 981 and from the right break in #3 across to #5 right in the belt line in #4. The area had been abandoned and did not have any affect on miners, ventilation nor equipment. The area has been dangered and cribbed off.

A roof fall was discovered in the 2 North #5 Return air course, at # 44xc in #2 Entry. The fall measures approximately 20' long by 18' wide by Approximately 8' High. The area was supported with 5' fully grouted resin bolts.

A roof fall occurred in the 1 Main South #6 entry starting in the pillarline between crosscut #2 and crosscut #3 and ending in the pillarline between crosscut #3 and crosscut #4 measuring 60ft. x 18ft. x 9ft. An undetected slip was determined to be the cause of the fall. This area was mined in September 2005.

A roof fall occurred in the #9 seam, 1st northwest main, crosscut #2, #2 entry. The fall is 40' long, 18' wide, 8 to 10' high.

An unintentional roof fall was discovered at spad #962 in the cross cut between #6 & #7 entries (returns). Size of fall was est to be 35' L X 18'-20'W X 6' H. Area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught and ventilation not interrupted. Fall will not be cleaned up.

A roof fall occurred at approximately 6:05 a.m. 2 breaks outby survey station no. 653 in the #6 entry. Fall is approximately 35' long, 18' wide, and 12' to 14' thick.

A roof fall occurred in the sub main #4 entry #36 crosscut. The fall was 20'l x 20'w x 8'h. The area was bolted with 6' & 8' point anchor bolts. The didn't affect ventilation, travel, cause injury, or damage equipment.

Hoist down for repair on rope. Hoist rope caught on track fish plate. Bolts damaged 4 wires on cable. We voluntarily shut down job until repairs could be made on 05/21/2007.

During normal checks at start of shift, it was determined a limit switch had failed on production hoist. Maintenance persons (5) who had entered mine were evacuated; no injuries. Part replaced; back in service at 12:08am.

On 4/25/08 it was alleged that a reportable coal dust ignition occurred on 5/20/07. In the investigation completed on 4/28/08 it was determined that the accident occurred. Several EEs removed a belt headroller that was bolted to the roof in E Submains, entry 2, XC-5. Coal dust on the headroller I-beam fell & was ignited by the cutting torch being used to remove support bolts

An unintentional roof fall occurred on the 7th North Headgate Entry #2, Crosscut #46 (2613N). The fall was of unknown length, unknown width and unknown thickness. The fall did not affect ventilation, but did affect passage of men. The fall will not be cleaned up.

At 10:42 a.m. a TXU employee smelled gas while traveling down the northwest road. He stopped to investigate and found a small gas pipeline leak on the north side of the haul road approx. 30' from the roadway. A pipeline company representative was called to the location. The pipeline was closed and pressure released from the line by 3:30 p.m.

Our examiners found a roof fall on the 1 Left, 4 Main Notrh sub-mains at crosscut 86 between entries 1 and 2. The fall was located in their normal route of travel while examining the returns of this area. The area was timbered off and the examination route was re-routed around the fall.

5/18/07 unintentional roof fall occurred on 4th main West, at crosscut #182 in #7 entry. This roof fall was supported with 10 point anchor roof bolts. The roof fall measured 18'l x 18'w x 10'h.The roof fall will be cleaned up. The fall appeared to be broken stack rock.

An external power surge from supplier (CEI) was experienced, knocking power to production hoist, etc. Efforts to re-energize revealed the surge had damaged some electrical components to the production shaft hoist. The mine was safely evacuated; no injuries.

The #4 hoist malfunctioned and came to a controlled stop, the problem was not immediately known and the mine was evacuated using the #2 hoist (2nd way out).

A rock fall occurred at 1:30p.m. as the section was being moved from the panel. The fall was above anchorage. The fall was inby spad 980. No persons, ventilation nor equipment were affected. the fall area was dangered and cribbed.

An unplanned roof fall occurred in the #4 entry - 2nd intersection inby the mine opening. Fall was approximately 11 feet high, 19 feet wide and 38 feet long. Area was bolted with 5 foot fully grouted resin bolts along with 6 foot tension re-bar bolts and 10 foot cable bolts. Fall was on intake escapeway so fall was cleaned up.

D-36 Dragline was stripping in the DIII west area, when the bucket cut throught a plugged gas/oil well casing. Residual conderisate liquid inside well casing ignited. The flame was about 2-3' in height. There was no gas pressure being released from the casing. The fire went out on its own at 10:55pm.

Unintentional roof fall discovered while traveling return airway. Fall pulled 4 roof bolts. Top previously supported with 6' full grout resin bolts. Area mined at or about 1/15/07. Fall measured approximately 6'HX15'LX17'W. Fall location: 11 Belt, 19 Block, 3 Hdg. No injuries or property damage. Investigated by T. Smallridge, WVMHS&T and J. Lyll, MSHA.

A section fo shaft guide came out in north compartment, allowing the tipping box to tip and ride the remainder of shaft on the wrong side of guides. As the north skip approached the shaft collar, the box had more clearance to tip further, causing a wedging effect that put force on the tower guides and breaking out four lengths .

A rock fall was discovered in the #3 entry one crosscut inby spad #982. The fall was approximately 20'x20' x7' high. The area was cribbed and dangered off. Mining ceased and the equipment was moved from this panel. The fall did not affect ventilation nor equipment.

At the start of the cut in the #1 entry, the miner operator started at the top of the face and cut down, cutting into the face 1 ft. to 4 ft. in depth. As the cutter head hit the mine floor a flare-up occurred over the miner bits. This lasted only 3-5 second and self extinguished.

Roof Fall North Sub-Main B-22 intersection of #4 entry, approximately 75' long,18-20 wide, 5-7' height, above anchorage. None injured, fell on waterline and mine power feeder line. Did not affect ventilation, neutral entry.

A 3yd LHD caught fire while the operator was mucking his round. He left the LHD in the muck bay and started the mine evacuation. MSHA was called at 8:20 p.m. The mine was evacuated and the mine rescue team sent down. The fire was out when the reached the scene. They found that the transmission filter had come loose spraying the engine.

An unintentional roof fall occurred in #9 Entry about 45' inby Spad 179. Fall is approximately 80' long x 5' thick x 18' wide. No people or equipment involved.

At 9:15 pm a small bounce occurred on the 12th East longwall, the result of the bounce pinched the shear's headgate pump motor cable requiring the cable to be reentered. This was the only problem found from the bounce. This prevented mining for more than one hour.

A roof fall was discovered in the 2 South #2 Intake Air Course. The fall was located at #18 xc 2 South # 2 in # 9 Entry. The fall measures approximately 40' in length by 18' wide and approximately 6' to 8' thick. The area is supported with 5' fully grouted resin roof bolts plus 10' point anchor bolts.

Around 12:00PM, PCS emp. was driving in mine area and spotted smoke in a muck pile. He notified supv. & dozer oper. who responded. Fire was possibly started by hot ash from nearby wildfire or recent lightning strike. No personnel or equipment were endangered. Fire was isolated & extinguished at time. No smoke was noted the morning of 5/18/07.

The west brake potentiometer failed on the Production Hoist. The Production hoist is the second escapeway out of the mine. The potentiometer was replaced.

A roof fall occurred during retreat mining, pulling two rows of bolts in front of the #1 MRS Unit. The area was previously supported with 5' Hi-Tech Bolts. There were no injuries to persons. The fall was located at 1-Right North-2 at spad 1470, at break #39.

While in process of transferring diesel fuel into mine via the #2 intake shaft, the fuel pipe leaked allowing approx. 400 gallons of diesel fuel to accumlate in #2 sump pump containment area. The mine was evacuated at 9:55am as precaution. Evacuation complete 10:05am, ret'd 7:45pm. The sump was pumped out. There was no damage to the hoist.

A roof fall occurred in the 3D panel, crosscut 13 #3 entry, spad 8+29. The fall is 20' long, 40' wide and 6' high.

An unintentional roof fall occured at 156+11 station, entry #7 on the main north return. The fall did not affect ventilation or passage of men. The fall measured 18 foot x 18 foot x 6 foot high. The intesection was supported with 6 foot fully grouted roof boltes. All approaches are timbered.

An unintentional roof fall was discovered 30 feet outby spad #985 in the #6 entry (return). Size of fall was estimated to be (50'L x 20'W x 4'-5'H.) Area had been supported with 4' fully grouted resin bolts with supplemental support consisting of 10' cable bolts in the intersections. No injuries, no equipment caught, ventilation not interrupted. Fall will not be cleaned up.

After completing the 4 right crosscut, miner backed out to tailpiece and the 4 right cut fell in pulling bolts out into the #4 entry intersection. The fall measured approx. 20' wide x 25' long x 2'-5' thick.

A roof fall had ocurred in the corss cut along the track entry of the old #9 belt between the #4 & #5 entry. Spad #2399 of #5 entry. The fall measured 90'l x 20'w x 10' h. This area was bolted with 6' tension bolts & beams supported prior to the fall. No injuries. Area will not be cleared at this time.

A roof fall was discovered while pre-shifting the mine after a washed idle. The interesection in #5 entry, #13 x c spad plus 12+13) in the #3 main on section MMU #001 fell above primary anchorage. Roof was bolted w/6 for fully govt bolts. supplement with 12 ft cable bolts. water was running out of the rest at the loader full dimension are 18x18x12 for high.

An unintentional roof fall occurred in the #2 entry of 3 west at 65 + 95. The roof fall measured approx 20' L x 15' W x 10'-15' high. No injuries, but damage to a high voltage cable. Re-routed a new cable.

An unplanned roof fall occurred in the #2 Gas mine that measured approximately 90' long, 19' wide and 8' high.

During a weekly inspection 4 falls were detected. The location of the falls are at or near spad 270, 175, 212 & 281. No injuries, no airways were blocked & escapeway are clear to travel. The falls were detected on sliding hill main area.

An unplanned roof fall occurred in the #5 entry of 1 Rt. Rooms 1 x-cut outby survey station #668. Fall was approximately 25' long, 19' wide, 4' plus high. Area was bolted with 4' tension rebar bolts with 6' tension rebar bolts spotted through the area along with straps. Did not affect any equipment, ventilation or travelway. Fall will not be cleaned up.

A mechanic was performing the daily inspection of the elevator & shaft. He was operating the controls on manual. He stopped the elevator & could not get it started. The E-stop switch was stuck in & would not allow the cage to move.

There was a severe thunderstorm around 20:30. Approx 30 min after the storm had passed, two operators on the tire dock noticed smoke coming out of the top of the Keith Bin. EE's notified the control room at 21:15. The fire dept was notifed at 21:38 and arrived at 21:46. MSHA notifed at 22:16 by control room operator.

The power was interrupted to the #5 and #8 ventilation fans. The fans were down from 3:57pm-3:59pm. The #5 fan went back down @4:04pm, due to motor overtemp. An announcement was made to withdraw from the mine @4:19pm. After ventilation was restored, an exam of the mine was made and persons were permitted to re-enter the mine. All agencies were properly notified.

The #8 elevator was inspected by Shindler, MSHA and MSHA technical support. Although the top and bottom door switches checked good, it was decided to replace both swithches. The elevator was taken out of service for over a period of 30 min. All persons underground were notified and the escapeways were re-routed. Victims transported to St Mary's Hospital in Amsterdam and released.

An employee was filling the grease reservoir on shovel MA182. The technician and the shovel operator were within 15 to 18 feet of an uninitiated booster when it exploded. There was no immediately identifiable cause for the detonation of the booster, but a full investigation was initiated on the day of the incident.

The gear box that contols the resolver for the HSS (Hoist Safety Supervisor) and the computer programmer on the Service Hoist started to make noises. It was felt that a bearing was going bad in the gear box. The hoist was shut down and the gear box was replaced.

He had a history of back trouble and an investigation was carried out to determine if this was a new injury and if it was work related. A loader ran into the deck of a three wheeled personnel carrier as it was being trammed through the stockpile area.

Changing bushings on a millroll and adjusting a pinion shaft. Thurned the top of the roll to get a better angle, then the roll fell and smashed finger between the roll and spider gear.

On 6/4/07 employee came in & showed supervisor a bump on his arm. We made a DR's appt for 6/7 at which time the DR told him to remain out of work until he had X-rays and the results back.

Dismounting a Cat D-11 Dozer, stepped off onto a rock, foot slipped off of the rock and twisted right knee. Employee started missing work on 06/25/07.

EE was cleaning mud from 2C-07 stacker. While pulling 1-1/2"" hose, the hose became tangled. While pulling to untangle the hose, EE reported feeling a pain in his stomach. EE reported incident on 6/19/07. Ee did not lose any time at work until, 8/4/07, when surgery was performed.

The employee was moving a roof bolt. A burr existed on the side of the bolt. The burr grazed his forearm causing a small puncture wound. The injury was not reported because the employee felt it was insignificant. Two days later the wound became infected causing him to be hospitalized for treatment.

When employee went back to work after lunch on 05/18/07, he was lifting a heavy stone and felt a sudden pain in his lower abdomen. He felt some pain the rest of the day, but thought it was just a sprain. By 05/18/07, the pain had increased, so he notified his supervisor, and was brought down from the quarry at 4:00 pm.

Employee was injured when a battery powered scoop came into contact with his foot. EE later complained of lower back pain & and independent medical exam was requested. On 7/17/07 a medical dr., stated it was reasonable to relate the back pain to the incident.

While the jacks were in pressure the gear box was heated to clear the interference between the couplings. As the coupling came loose one of the supplements slipped out and hit the employee on his right leg.
