FALL WILL BE CLEANED UP.

ROOF FALL 80' LONG, 6' HIGH, 20' WIDE - NO INJURY, NO IMPAIRMENT.
An unintentional roof fall was discovered on the B-2 East section belt. It occurred in a 4-way intersection with 60 degree crosscuts. The roof conditions were good in the area prior to the fall. Roof bolts - 5' resin. Width - 19', Length -60', Height - 5'plus. Equipment damage - belt, structure and 6"" water line.
KUHNTOWN ELEVATOR STOPPED MID-WAY DOWN WITH AN EMPLOYEE ON IT, FOR MORE THAN 30 MINUTES. ELECTRICAL PROBLEM.
A THUNDER STORM CAUSED A PROBLEM WITH ONE OF THE FUSES WHICH RESULTED IN THE ELEVATOR AT THE #8 PORTAL BEING OUT OF OPERATION FOR OVER THIRTY MINUTES, THE BLOWN FUSE WAS FOUND AND REPLACED, THE ELEVATOR WAS PLACED BACK IN OPERATION

AN UNINTENTIONAL ROOF FALL OCCURRED @ #35 XCUT ON ""H"" TRACK. THE FALL MEASURED APPROX 18'L X 20'W X 10' TO 12' HIGH. NO INJURIES & NO DAMAGE TO EQUIPMENT. AREA WAS CABLE BOLTED.
UNINTENTIONAL ROOF FALL. G-NORTH TRACK ENTRY AT #45 INTERSECTION. FELL APPROX. 20'LONG, 15'WIDE & ABOVE ANCHORAGE OF 8'ROOF BOLTS.
Roof fall in No. 6 entry between Spad No. 58 and No. 64, fall is 30'-40' long x 18' wide x 8'-10' thick. Fall is in intake entry and air will be re-routed into No. 5 entry. Reflectors and danger tags are hung. Area will be breakered off with 100 ton Heintzmann, Pizza Jacks.
The mine was evacuated at 4:25 p.m. because the main hoist went down due to IO Module fault alarms. Electricians responded and contacted ABB service representatives. On vendor recommendation, a IO Module was replaced and hoist operation was returned to normal. Crew returned underground at 6:25 p.m.

The main hoist went down due to fault alarms. Electrictions responded and reset the hoist. Crew returned to underground at 6:25 p.m.
ROOF FALL (7000-1-004-05) - LOCATION - Main E #4, Crosscut 105, Entry #4. DIMENSIONS - H:6', W:19', L:40'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORt - 5' fully grouted bolts.
An electrical fault of undetermined origin caused a fire in the finish mill motor control center and associated switch gear.
An unintentional roof fall occurred along the #2 entry at #6 crosscut in the Main North roadway. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 20' long x 18' wide x 12' high and this roof fall will be cleaned up.
Fall in between #7 and #8 entry in the cross-cut only on #1 unit mmu-001, unit was idle.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED DURING A PRE-SHIFT EXAMINATION OF AN INACTIVE AREA OF THE MINE. THE AREA IS IN THE 1ST NORTH SUB-MAINS, #2 ENTRY, BETWEEN CROSSCUTS 21 & 22. ROOF FALL DIMENSIONS ARE: 18 FEET WIDE, 20 FEET LONG & 5 1/2 FEET HIGH.
A ROOF FALL WAS FOUND AT 3:00 AM ON 6/27/05. LOCATION WAS ENTRY #0 19+60 7LMS. FALL WAS 10' HIGH X 55' LONG X 25' WIDE. 4-WAY INTERSECTION WAS MINED 5/19/05 AND BOLTED WITH 6' RESIN BOLTS. NO PROPERTY DAMAGE OR INJURY RESULTED. PLANS ARE TO CLEAN FALL UP AND RE-SUPPORT AREA.
Unintentional roof fall on 2 Left #5 entry 1 break inby spad #1413. Dimensions:90' (l) x 20'(W) x 10'(h).
Electronic drive on hoist kicked-out rendering the hoist inoperable. The Chief Electrician reset the breaker and power system and the hoist became operable. The situation will continue to be monitored.

UNPLANNED IGNITION AT 2 LOCATIONS CONSUMING 1/4 & 1/2 ACRE OF GRASS NEAR ABANDONED PREP. PLANT. APPEARS TO BE CAUSED BY ELECTRICAL SPARK THAT FELL ONTO DRY GRASS.
WHILE DOING PRESHIFT EXAM ON HOIST, FOUND BROKEN WIRE IN HOIST ROPE. HOIST WAS IMMEDIATELY TAKEN OUT OF SERVICE. HOIST ROPE HAS BEEN CHANGED OUT.
Hoistmen at #1 shaft was bringing up the South skip when he heard and saw the North bell come on in the hoist cab as he was dumping the skip. So he immediately set the brakes. The hoistmen tried calling on the mine phone and using the bells to get a hold of the cager but their was no response so maintenance was called. Hoist back and running at 11:15 pm.
A ROOF FALL OCCURRED APPROX. 1200' OUTBY THE 001 SECTION. IT MEASURED 80' LONG X 20' WIDE X 8' HIGH. THE LOCATION WAS IN THE INTAKE JUST OUTBY SURVEY STATION #1056 IN THE #6 ENTRY.
The elevator at the #1 portal did not operate properly from 4:30 PM to 6:30 PM due to overheating in the penthouse area. The ventilation was restored to the area. The elevator resumed operation at 6:30 PM.
The rock burst consisted of approx 40 feet of the hanging wall. The length of the stope from the face is 70' from the intersection. This stope will be permanently closed down and sealed (sand filled) from future mining activity in this area.
ELEVATOR WAS OUT OF SERVICE FOR MORE THAN 30 MINUTES WITH 2 EMPLOYEE ON IT - AT THE KUHNTOWN PORTAL. ELEVATOR STOPPED WITHIN 100' OF THE SURFACE LANDING DUE TO THE BRAKES SETTING UP AS A RESULT OF VIBRATION AND A GREASE BUILD-UP ON THE GOVENOR AND SENSOR MECHANISM.

An unintentional roof fall was discovered between spad numbers 9206 and 9048, Xct between 6 & 7 entry on E-7 Section. Fall measured approx. 18'W X 80'L X 6'H. Area was mined on 6/1/03 and was supported with 5' torque tension bolts. Cover in this area is approx. 255'.
A FRICTION CAUSED METHANE IGNITION, IT OCCURED ON THE LONGWALL PANEL 50 FACE BETWEEN 171 & 172 SHIELDS. METHANE WAS IGNITED BY FRICTION WHEN THE SHEARER STRUCK A PYRETIC INTRUSION LOCATED BETWEEN THE MAIN COAL SEAM & THE DRAW SLATE DIRECTLY ABOVE A DEGAS HOLE LOCATED 1 1/2 FEET FROM THE MINE BOTTOM. A RED/ORANGE FLAME, 2 - 3' LONG BURNED APPROX. 45 SECONDS BEFORE EXTINGUISHED.
An unplanned rock fall occurred in the #5 entry of the #5 left panel, 8 breaks inby the section headdrive located off the #3 Left Sub-Mains off the #3 West Sub-Mains. Miner was extracting coal on 1st run on the right wall cut when intersection fell in and rode thru the breaker timbers. Rock fall measured approximately 40'x20'x42"".

Water truck parked at end of shift. During am hours, short occured at battery cable. spark ignited grease on frame grease dripped onto fuel line, melting small hole in line. Diesel dripped onto ground. Sparks ignited diesel on ground. Passing coal train saw smoke underneath truck @ 5:30 am called wright fire dept. fire dept arrived @ 6 am. extinguished fire @ 6:03 am.
A fire was started by spontaneous combustion on top of shot coal in the east oilwell pit. The wind spread the fire preventing it from being extinguished immediately. The fire was cooled by applying water and the coal was dozed up and buried in overburden.
AREA HAD BEEN MINED APPROX. 20 YEARS AGO AND HAD DETERIORATED FROM TIME AND MOISTURE.
Fall occurred in the 30th crosscut in the #8 entry of the third east panel. Supported by 8' and 6' tension rebar pins. The fall blocked the travelway. Trafic was diverted and the area propped. Fall dimensions are 70' x 18' x 8'.

Fire boss found roof fall on weekly examination of mine. The fall was in the #7 entry at x-cut 30 of the north mains. The fall measured 40' long 20' wide and 6-8' high.

A roof fall occurred in the 3D return, crosscut 13, entry #2, near spad #25002. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was located outby the tailpiece of #3 unit.
A roof fall occurred in the 1 South in #8 entry in the pillar line between cross-cut #46 and cross-cut #47 measuring approximately 30ft. x 19ft. x 6ft. The area was supported by 6ft. tension rebar bolts. An undetected slip was determined to be the cause of the fall. The area was mined in April 2004.

On June 22, employees reported hearing a loud thunder-like noise. An immediate investigation was initiated to determine the source of the unexplained noise. Personnel discovered that 3 gaskets were blown out on the oxygen enrichment channel. Deformation on the pipe and flanges was observed. There were no employees working in the area, no injuries sustained.

The fall was located in the No. 3 entry, 1 break, outby survey station No. 1150 and measured approximately 20' long x 20' wide x 6' high (intersection). There were no injuries or equipment involved. IT APPEARS A FAULT IN THE FRONT ENGINE STARTING SYSTEM MAY HAVE STARTED THE FIRE. THE FIRE ENGULFED AND DESTROYED THE ENTIRE ENGINE ROOM COMPARTMENT.

A roof fall occurred in entry #6 3rd s.w. panela @ 15 + 60. The fall measured 20' long x 18.5' wide x 6' thick. the area had been supported with 6' fully grouted resin bolts. all approaches have been timbered.
A roof fall occurred one break out by spad 313, it was approximately 5 feet thick, 20 feet wide and 40 feet long.
THE CEDAR CREEK SERVICE ELEVATOR WAS OUT OF SERVICE FOR APPROX 1 HOUR BECAUSE THE ACTIVATING ARM CAME UNBOLTED. THE BOLT WAS REPLACED AND THE LATCH DOOR LOCKS WERE ADJUSTED ALONG WITH THE CAR GATE SWITCH. THE ELEVATOR WAS OUT OF SERVICE FROM 9:00 AM UNTIL 10:00 AM.
Upon doing pre-shift for Sunday midnight shift, foreman found a rooffall which partially covered the inby end of the roof bolting machine in by B-15 of #3 entry of the L left panel of the L-left submains. Due to face conditions found during end of Friday night afternoon shift, equipment was pulled back from faces. Fall fell above 42"" resin bolts.

The contrary service cage was down from 11:20 pm until 1:00 am to repair the grating that was damaged due to a chain that was being used as a supply car safety chain getting fouled at the surface landing, which bent the grating. There were no injuries due to this incident.

Fire on CAT D11 dozer discovered by operator reporting for his shift. Fire fought with fire extinguisher & Perry Twp. fire dept. Cause remains under investigation, but appears to have started near the wiring to the starters. Damage extensive to engine, torque, hydraulic pumps & valves. Operator cab completely burned out. There were no injuries.

BOLTED ORININALLY 5'BOLTS, RESIN ASSISTED. NO INJURIES.

The Tyro Creek Service Elevator was out of service on 6/12/05 at 8:40 pm due to the pit being full of water, causing the pit switch to short which tripped the control breaker. Bagby Elevator Company repaired pit switch and the elevator was in service at 11 pm.

An Unintentional roof fall happened approximately 1 break inby survey station 3381 in the number 5 entry. The fall was approximately 50 feet long and approximately 0 to 8 feet thick and 20 feet wide. The area has been dangered off and the stopping line moved over in the affected area. The fall may be cleaned up at a later date. No men or equipment were involved.

A roof fall occurred in the 3C return at cross cut 9 in entry #2. The fall blocked the one return entry at this location. The stopping line was set over to re-open the return aircourse from #3 unit. This fall was found during the weekly examination.

While running the #1 production Hoist the hoist person experienced problems trying to pull a loaded skip up from the bottom. After powering the hoist up as much as 6 points the skip still wouldn't move so he shut the hoist down. U/G was evacuated. Down 1:22 am. Started running 3:22 am.

A THYRISTOR WENT OUT IN THE SILVER SHAFT HOIST. THE HOIST WAS DOWN FOR APPROX. 1 HOUR. THE #2 SHAFT HOIST WAS READY TO GO FOR EVACUATION OF 6 PEOPLE UNDERGROUND. THE SILVER SHAFT HOIST WAS REPAIRED BEFORE EVACUATION STARTED. WE DID NOT HAVE TO EVACUATE THE MINE.
POSSIBILITY OF THE WILLIAMSON SEAM BREAKING THRU & LETTING MORE WATER INTO THESE AREAS (H-PANELS AND NORTH WEST MAINS). THE WILLIAMSON SEAM IS OVER TOP OF THE POND CREEK SEAM.

Elevator had noticeable vibration while operating. Elevator was shut down until elevator service company repairmen were able to change some bad guide rollers. The elevator was put back in service at 9:00 pm.
Fall occur between a 3-way and 4-way intersection. The 4-way intersection was supported with 100 ton Heintzmann jacks and I-Beams with 4-100 ton pizza pan jacks on the side where the fall occured. Fall is 30 ft. long, 19 ft. wide by 5-6 ft. thick
While second mining the miner became stuck because of floor conditions. Time was needed to remove the attached bridge system, and to assemble the retriever. During this time a rock fell from the roof onto the miner head. Miner was removed with the assistance of the retriever. Miner was not damaged.

At 5:15 a.m., the hoist engineer was lowering (4) four loaded supply cars into the mine via ""the slope track."" 3 cars came uncoupled, breaking the safety chain coupling device. As a result, the 3 cars traveled a distance of 128' colliding with an empty supply car & diesel locomotive. The incident resulted in the derailment of 3 cars.
Miner went into the 3565 bullion and noticed a fall of ground on left rib. This heading has been inactive since 5-10-2005

STATE AND MSHA LOOKED AT AREA.
While traveling the return airway course, #2 roof falls were discovered int he L14 seal area in #7 entry #1 roof fall measure 5' high 18' in width 25' in length #2 roof fall meausre 5' in high 18' width and 20' in length. No injures were reported as an result of these roof falls.

At approx 2:40, the 20 shovel lost power. The shovel operator called dispatch at 2:43 with MayDay saying that there was a fire in the house of the shovel. The electricians responded, cutting power and disconnecting the potheads so the ERRT members could fight fire. The fire was extinguished at 3:15. The Dallas MSHA office was notified at approx 3:40 am.

Due to high rain fall the Highwall was saturated with moisture and was being monitered. Highwall activity indicated to evacuate employees from the area. Following our safety procedures employees were evacuated. No employee was injured or eqipment was damaged. There was no rock burst just a large slump of wet material from the highwall onto 5175 bench.

An electrical malfunction occurred in the control cable for the Garden B-shaft hoist.
While conducting hoist examination, a defective brake release sensor was found. Due to unavailability of an additional sensor, the hoist was removed from service for more than 30 minutes.
a seal in the main brake valve went out. The mine evacuation started at 8:55 am and was complete at 9:03 am. The seal in the main brake was replaced, the hoist tested and miners returned u/g 9:35 am.

A roof fall occurred in the 2 East at cross-cut #16 starting at the north edge of the pillar line of #9 entry and proceeding north through the intersection of #8 entry measuring approximately 50ft. x 19ft. x 6ft. The area was supported by 6ft. tension rebar bolts. A hidden slip was determined to be the cause of the fall. The area was mined in December 2004.

A roof fall occured in entry 4-5 of the 2nd MNE at survey station 13 & 30. The fall measured 30' long by 18.75' wide by 8' thick. The area had been supported with 6' fully grouted resin bolts and 9' double locks. The approaches have been timbered.

The employee was stripping at gravel pit. The employee had dumped his load of dirt and was returning to the load site. At this time he chose to travel over a pile of dirt. as he was traveling over the pile of dirt he got too close to the edge of the pile and the equipment overturned and landed on him.
The victim was operating a track dozer to clean off a drill bench in preparation for drilling. The bench was 45 feet above the quarry floor. Apparently the victim was attmpting to turn the dozer around on the bench, backed off this area, and fell to the quarry floor.
Employee was standing on a temporary platform just below a jaw crusher attempting to remove two toggle seat wedges from the pitman. Shortly after removing the two bolts holding the wedges in place, the pitman unexpectedly moved fatally crushing the employee.
Employee moved roof bolter outby face to rebolt in the section. Horse back fell out causing fatal injury.
Shuttle car operator stopped to talk to the shuttle car operator in the crosscut behind him. Scoop operator in crosscut in front proceeded to cross in front of shuttle car. Shuttle car operator proceeded with the buggy towards the feeder running over the scoop operator in front of him.
The victim had traveled inby permanent roof support in the #3 entry right crosscut #4 and a section of draw rock fell in the unsupported area fatally injuring the victim. The continuous miner was cutting into #3 right crosscut from the #4 entry when the draw rock fell out on miner and victim.
Employee was discovered by section foreman caught in the pick breaker on the Stamler feeder. He suffered fatal injuries to his lower chest and mid torso.
The employee was lifting a Gardner Denver 83 jackleg drill out of a Boss Buggy and felt pain in his groin.
AT 23-M CONVEYOR BELT TAKE-UP/TENSIONER AREA, EMPLOYEE WAS ASSISTING TO PREPARE TO LOOSEN THE TENSION OF THE MOVABLE TENSIONER CARRIAGE. HE GOT HIS LEFT HAND CAUGHT BETWEEN THE CARRIAGE DOLLY WHEEL & THE RAILING IT RUNS ON.
WHILE PUTTING UP ROOF BOLTS, EMPLOYEE GOT HIS LITTLE FINGER CAUGHT IN CLAMP THAT HOLDS ROOF BOLTS IN PLACE.
IW was on ground ~ 4 feet from basket of manlift. Operator placed manlift in reverse motion, wheels turned right. Boom/basket abruptly swerved left and struck IW on right side of body. Left hand of IW got caught between midrail of basket and conveyor column causing L-hand laceration, left index finger fracture and partially severed L-thumb. Cause: untrained operator for equipment.
When switching tubs on the treater floor the employee has to step up to change a valve. When he was stepping down he felt a pull in his groin area. Upon examination at the hospital it was found that he had a left inguinal hernia.
Employee states while attempting to slam tube down barrel his glove became entrapped between tube nut and barrel. Partial amputation and fracture right index finger.
Employee unexpectedly placed finger in a hole between two pieces of steel while steel was struck by a 10lb sledge hammer, cutting off a portion of the index finger on the left hand.
ON 6-17-05 AT 5 PM, EE AND 3 OTHER MEN WERE ATTEMPTING TO RE-INSTALL THE HATCH COVER OVER THE CLINKER DRAG. EE USED HIS HAND TO CLEAR DEBRIS FROM UNDER THE HATCH AND IT FELL CAUSING THE TIP OF HIS 1ST FINGER RIGHT HAND TO BE CUT OFF. THE HATCH WAS STUCK AND FELL 1 1/2"" TO 2"".
EE WAS PUTTING FLIGHTS BACK IN FEEDER CHAIN WHEN HE WAS RE-INJURED (HERNIA). ORIG. INJURY AND DIAGNOSIS OF HERNIA 11/17/04. HERNIA WAS SURGICALLY REPAIRED APRIL, 2005.
Employee was manually (by hand) turning the drive belts to a screw's drive motor to try to get the screw started. The belts pulled his right hand in between the belts and the pulley causing amputation (from tip to first joint) of his right hand ring finger. He did not follow a known & recognized safety practice (Lock-Out, Tag-Out, & Try-Out).
EMPLOYEE WAS PRIMING WITH A PAINT GUN, IT STARTED TO GET SLUGGISH SO HE WIPED HIS FINGER ACROSS THE TIP & IT BLEW PRIMER INTO HIS FINGER. HE IS STILL UNDER A DR.'S CARE & NOT ABLE TO WORK. THEY THOUGHT THAT THEY MIGHT NEED TO AMPUTATE TO THE FIRST KNUCKLE BUT THE DR. HAS CHANGED HIS MIND AS THE FINGER IS HEALING.
He was attempting to rotate the shearer drum. When he placed his hand on the drum, a piece of rock fell and caught his left hand between the rock and the drum. This caused a crushing injury to his left hand with injury to the middle and ring fingers, and amputation of the left index finger. (left index finger and left middle finger to the second knuckle by Misc Mail 6/23/05 #1)
Injured was using a 4 ft bar to steady an H-beam that was being jacked back into place on an overcast, as pressure was being applied to the track jack that sitting on top of a 4 ft crib (top layer solid) the cribbing and jack creeled out catching his left little finger against the overcast wall, resulting in finger tip being surgically removed.
EMPLOYEE PLACED LEFT HAND BETWEEN BOTTOM AND MID SHELL OF CRUSHER TO ADJUST GASKET WHILE MID SHELL WAS BEING LOWERED BY CRANE ONTO BOTTOM SHELL. EMPLOYEE'S LEFT HAND WAS CRUSHED. (POST-SURGICAL EVAL.: PROBABLY WILL REQUIRE AMPUTATION OF MIDDLE, RING & LITTLE FINGERS, EACH AT PROXIMAL PHALANGE.)
WHILE CARRYING A JENNY PIN IN HIS RIGHT HAND, EMPLOYEE TRIPPED & FELL STRIKING HIS RT. THUMB AGAINST A METAL PLATE. THIS RESULTED IN A BROKEN BONE & A LACERATION OF HIS RT. THUMB. THE LACERATION REQUIRED 10 SUTURES.
Employee was changing cylinder hose on D8 dozer. Wrench broke, causing him to fall off dozer.
While pulling a wire mesh roof screen over his head and onto the ATRS of the roof bolter, EE felt pull in lower abdomen. Doctor visit on 7/12/05 diagnosed with hernia. Surgery secheduled for 7/19/05.
Employee was hanging cable in the #4 entry on right side of bolter, with back turned away from bolter. Coworker was backing bolter out of the #4 entry and watching the cable on the left side of the bolter. Coworker never saw employee and backed over his foot and lower leg.
EE stated that on the morning of the injury, a tire had to be removed from his grease truck. As he was pulling on tire, he felt pain in his lower back area. No report was filled out at the time of injury; safety department was informed on July 11, 2005. Note: Employee did not start missing work until August 25, 2005.
A Bagger Operator was descending the steps from a platform when he felt dizzy. Employee fell backwards on floor and bruised left shoulder. Defective stairs were in use that likely contributed to the fall. Employee complained of pain in his shoulder on the following evening and was provided medical attention.

TWO DRILLERS WERE SCREWING DRILL PIPE WITH PIPE WRENCHES. ONE DRILLER SLIPPED STRIKING THE CONTROLS FOR ROTATION. PIPE ROTATED DRIVING PIPE WRENCH INTO INJURED ABDOMEN. SURGERY REQUIRED TO REPAIR DAMAGED MUSCLE TISSUE
Victim got his arm caught between winch and monorail.
EE STATED HE WAS WALKING AROUND MANTRIP CARRYING TOOLS, LUNCH BOX AND WATERJUG. HE SLIPPED AND TWISTED HIS RIGHT KNEE. AN MRI WAS TAKEN WHICH REVEALED A TORN MENISCUS. FIRST LOST WORKDAY DID NOT OCCUR UNTIL 07/06/05.
Ee was attempting to dislodge by hand a 50# bag of product that had gotten stuck at the corner of the palletizer belt system: he caught his glove between the belt tail pulley and thr guide that keeps the bags on the belt, lacerating the middle finger of his left hand.
Employee was cleaning on walkway side of his belt. While lifting on a piece of draw rock, he felt something pull in his rt. upper arm.
CRUSHER HAD BEEN REMOVED FROM CONCRETE PAD FOR NEW INSTALLATION. EMPLOYEE WAS CLEANING THE PAD & HE SLIPPED ON GREASE WHERE A MOTOR HAD BEEN, CAUSING HIM TO FALL.
Employee was Jacking up #3 Belt when the jack rolled over causing the employee to twist his back.
While lifting a 5 gallon can from the bed of a truck the employee strained his lower back.
Victim was assisting with pulling the cars out of the hole that was finished. He go his right foot caught between the push assembly and one of the cars. Foot was bruised & swollen on top. Missed two days work.
Employee was leaving his work station for the day. He slipped and fell on the ground bruising his lower back.
Employee was cleaning the dump area when he was injured by a piece of 4"" waterline. The waterline was under the gob & got fouled under the scoop & came up & struck him on the right side of his head.

INJURED WAS PASSENGER IN A PORTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. A LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF PORTAL BUS COULD NOT CLIMB THE HILL. IT STARTED SLIDING BACKWARDS AND INTO THE PORTAL BUS.
Employee was struck & knocked down while he was in the process of taking down a high voltage cable. When he cut the strap that was holding the cable- the cable fell on him.
EMPLOYEE WAS REMOVING BUCKET TEETH FROM BUCKET USING A SLEDGE HAMMER TO DRIVE TOOTH OFF ADAPTER. A PIECE OF THE TOOTH BROKE OFF AND STRUCK EMPLOYEE IN HIS LEFT UPPER CHEST. THE PIECE OF METAL HAD TO BE REMOVED IN AN OUTPATIENT SURGICAL PROCEDURE. NO PERMANENT DAMAGE IS EXPECTED.
Employee was using a 8# sledgehammer to drive a bearing race out of the wheel of a euclid haul truck. The hammer handle caught on a wheel stud causing the hammer to deflect and twisted employee's arm. *****It appears that this employee is going to need more treatment than anticipated.
INJURED WAS A PASSENGER IN A POTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. THE LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF HIS PORTAL BUS/ THE LOCOMOTIVE COULD NOT CLIMP HILL AND STARTED SLIDING BCKWARD INTO PORTAL BUS INJURED WAS A PASSENGER IN.
RIDING IN MANTRIP TO 7 WEST BEHIND LOCOMOTIVE AND ROCKING MACHINE. STOPPED AT A CROSSCUT 200' INBY MNO #2 BELT. LOCOMOTIVE WAS GOING UP A GRADE AND LOST TRACTION SLIDING BACK DOWN THE GRADE AND INTO THE MANTRIP. INJURED WAS EXITING THE MANTRIP WHEN IT WAS STRUCK.
As ee was descending northwest haul road of impoundment a left front tire on the cat 631G scraper blew out, causing him to loose control and roll scraper over berm. He was treated at hospital for abrasions and contusions & released to work with no restriction. He has continued to have swelling from a large contusion on left thigh and to do surgery relieve pooled blood.
The injured employee wass loosening hand brake on rail car when he slipped & hit bottom on the car drawhead. The accident was determined to be ""lost time"" on 07/11/2005.
Employee was stepping over retaining wall @ C-8 Tank pump to turn off valve. He tripped over wall & fell catching himself on his right hand.
Employee was hauling and dumping rock in valley fill - he backed truck up too far and slope gave way - truck went back over embankment about 40 - 50 ft - truck turned over.

A contractor was working on some demolition work at the mine site. He was standing on a catwalk with handrails cutting loose the bottom of the catwalk from a hopper. He cut both of the supports to the catwalk and fell approximately 23 feet. He showed signs of a leg injury and the hospital confirmed a broken femur and pelvis.
While carrying can of hydraulic oil handle on can broke throwing employee of balance, causing him to injure back.

He was lifting a gear box and had sharp pain in lower abdomen.

EMPLOYEE SAID HE STRAINED HIMSELF WHILE SHOVELING BELT WITH #4 COAL SHOVEL.
Employee was getting on a tow motor. When he pulled himself up with his left arm he felt pain in his left shoulder. He had an injury in the same place which resulted in surgery in 2002.
Alledged incident was to have occurred while this employee was rockdusting the belt. He said he tripped on the steps of an overcast. Cause of this injury was safety awareness. Compliance not a issue, miner attitude a factor, mining equip/systems not a factor, protective equipment not a factor.

Employee was changing out belt structure. While pulling a piece of structure out of the mud he fell back striking right elbow against sailboat. He missed 6/28/05 and 6/29/05. Came back on 6/30/05 then went back off again starting 7/1/05.
Shoveling belt when he pulled something in back.
While reaching for a wrench off a rib protector a rock fell striking left hand. Rock was approximately 14"" long and 8"" wid eand 4"" thick. The roof was 9 foot in height. Diagnosed as a contustion.
EE had parked his service truck in order to work on a grease truck. Although the park brake had been set, the service truck began to roll backwards. He was caught between the driver's door of the service truck and the front bumper of the grease truck.
EE WAS RIDING RAIL LOCOMOTIVE WHEN HIS HEAD STRUCK THE ROOF. EE BRUISED MUSCLES IN HIS SHOULDER AND NECK.
Employee parked ramcar to get off and use bathroom. The car rolled and pinned him against rib.
Some more workers were taking a rubber sleeve off a pipe with a chain band. He was standing in front of them leaning aganist a belt winder when the sleeve came off it just hit his leg and pinned it up aginst the belt winder breaking his right leg just above the ankle.
EE LOST CONTROL OF HIS TRUCK. HE HIT ANOTHER TRUCK & THEN TURNED OVER INTO A POND. EE BROKE BOTH LEGS. TRANSPORTED TO MED. CENTER BY AIR. ALL DAMAGES TO EQUIP. ARE NOT EVAL. AT THIS TIME. (TRUCK LANDED ON SIDE; COWORKER HELD EE'S HEAD ABOVE WATER UNTIL THEY COULD GET HIM OUT. UNKNOWN (7/1/05) IF EE HAD ADDL. INJURIES IDENTIFIED AT HOSP. 2ND TRUCK DRIVER NOT INJURED.)
EMPLOYEE STATED THAT HE WAS ATTEMPTING TO OPEN THE TAILGATE OF TRUCK TO REMOVE GOB UNDER THE TAILGATE. HE SLIPPED, TAILGATE FELL MASHING HIS FINGER
EE was pushing a scoop that was stuck in a hole. The bucket of the scoop that EE was operating slid off of the scoop that that was stuck when it broke free, causing EE to strike his head against the scoop's canopy.
Subject was positioning drill pot to install roof bolt. Swung boon out with left hand elbow of right hand hit coal rib and jammed wrist against drill pot. off-side operator.
Injured Worker (IW) picked up crane's outrigger pad from squatting position, stood up and took 3 steps while turning to his left when he felt a 'pop' in his low back. IW sustained a muscle strain in low back.
While moving steel under motor, channel came loose striking left ankle.
East 42"" Conveyor went down due to being overloaded. EE had conveyor belt de-energized and was sitting on belt shoveling out ""pant leg chute"" that feeds conveyor. Employee reported, after shoveling, that his back was hurting. This was not a reportanle injury until Feb 6, 2006 when employee under went surgery and was taken off work.
Operator said his end dump was over heating. The operator stopped to check coolant level and as he was loosening the filler cap it came off suddenly and sprayed his face and chest and arm with hot coolant. It resulted in first and second degree burns on his face, arm, and chest.
When removing wheel from water truck the cleat from the wheel ejected striking ee in the left hand and wrist area.
Employee had stockpile truck with concrete stone. Went to scale to weigh and forgot to dump his load at stockpile. EE started down road to the pit. He was not going very fast but weight of truck and decline made it difficult to stop. He came to curve and truck tipped over. He was wearing seatbelt. He climbed out and was taken to hospital for x-rays and stitches.

AFTER CLEANING GRIZZLY AND REPLACING DOOR, THE EMPLOYEE TURNED TO WALK AWAY. HE STUMBLED ON LUMP OF MATERIAL AND TWISTED HIS BACK.
EMPLOYEE ALLEGES HE STRAINED HIS GROIN ON LEFT SIDE WHILE HANDLING 7 LONG TIMBERS. HE WORKED UNTIL 7/25/05 THEN HAD SURGERY TO REPAIR HERNIA
Foreman was two rows of timbers back when miner was extracting coal (first cut) on right wall in the #5 entry when the intersection fell in and rode thru the breaker timbers catching his left leg.
Employee was tramming ram car down roadway when he hit a hole in the roadway and felt pain in back. Employee started missing 06-25-05.
Employee was removing derailers from storage shelf when he struck his upper arm on shelf, resulting in a strain of the upper arm. This accident became reportable on 7/12/05, when lost time occurred.
Employee stepped off of a stair onto a rock and his right leg twisted out to the side. He was evaluated by a physician and on 10/12/05 he required surgery resulting in lost time.
EE WAS CHANGING A GREASE BARREL BAG AND WHILE STANDING ON THE DECK, HE TRIED THROWING THE ALMOST EMPTY BAG OVER HIS HEAD INTO THE BED OF THE TRUCK AND LOST HIS BALANCE AND FELL TO THE GROUND LANDING ON HIS FEET BREAKING HIS LEFT HEEL
GLOVE CAUGHT BETWEEN STEEL AND DRILL HEAD. WHEN ROTATION PRESSURE WAS APPLIED, EMPLOYEE'S HAND AND ARM WERE TWISTED AROUND DRILL STEEL CAUSING COMPOUND FRACTURE OF LOWER ARM.
Operating truck in unsafe manner causing it to roll onto side, he hit shoulder and head.
Employee was rock dusting, pulling on hoses he stepped on a rock, fell back and landed on arm.
EE was placing shot crete in the hopper of the shot crete machine, while taking a break, he placed his hand on the cement truck shoot. The cement truck driver was adding a section to shoot. Other ee fliped the shoot down pinching ee left hand between the joints of the shoot. He was taken to the dr, 2 broken fingers.
Employee was riding in a scoop when he hit his back against the scoop bucket.
Was on a Par Machinge tray pushing a used mill liner off toward the scrap luger. The liner slipped off the 2x4 and pinched his finger.

A 10' piece of 4"" pipe filled with concrete was laying on crib blocks when it rolled off of the blocks onto his left ankle.
Making repairs to tailgate pin on haul truck. Used loader to push tailgate arm and pin into position. While tightening last bolt bucket slipped off arm of tailgate catching left hand between arm and bucket.
Forklift driver was backing up after putting pallet down; looked over left shoulder, looked over right shoulder, not seeing anyone, began to back up. Light and alarm were active. Heard EE yell as forklift tire ran over foot.
Employee was pushing cable through shieve. Cable was twisted and his left shoulder was twisted. He started missing on 9/29/05.
The employee was removing the outside bolts of a tailpulley take-up unit. The inside bolt was missing from the unit which allowed the top of the take-up unit to come loose, striking the employee on the left side of his face.

EE WAS LIFTING SOLID BLOCK AND FELT PAIN IN HIS GROIN AREA (RT SIDE). HE WORKED UNTIL 7/15/05, THEN WENT OFF FOR SURGERY
Working on top of a shovel bucket guiding a becket ends through the cable tentioner. The cable jumped up out of the socket striking him across the lower right leg.
INJURED WAS WALKING FIREBOSS ROUND. INJURED STEPPED OVER A PIECE OF OLD BELT STRUCTURE. HIS RIGHT FOOT CAUGHT ON THE STRUCTURE. HIS LEFT FOOT SLIPPED AT THE SAME TIME. THE SLIP CAUSED HIS RIGHT LEG TO TWIST AND BREAK A BONE.
The operator checked the antifreeze level in the dozer after it had been running for two and a half hours and the hot antifreeze sprayed over his chest, stomach and arms.
Employee was working on the feed hopper using a porta-power unit to straighten the side of the hopper when porta-power slipped and a 12"" x 14"" x 3/8"" steel plate hit him in the face and chest.

Employee stepped on wet coal and while trying to catch himself, turned his ankle.
Injured was exiting off the manlift on the 6th floor and reached to open the gate to step through when he slipped and fell. He was treated by FMTS and transported to Memorial Hosp. where x rays revealed a fracture to the right fibula. He visited a specialist on June 23rd where a soft cast was applied above the ankle on the right leg.
Employee was twisting around in an attempt to hang a roof bolter cable across the travelway when he heard a ""pop"" from his right shoulder. Employee had dislocated his right shoulder.
Employee was cleaning spillage from crusher operation with a shovel. After approximately twenty minutes of shoveling, a sharp pain shot up his back.
EMPLOYEE MISSED A STEP WHILE GETTING INTO THE LOADER. HOLDING LADDER WITH RIGHT HAND HE PULLED SHOULDER.
The injured employee was moving the miner cable when a piece of draw rock fell striking him on the right foot causing bruising and a fracture to the big toe.

EMPLOYEE WAS LOWERING CANOPY JACK TO FACILITATE SLIDING OF METAL ROOF MAT ONTO DRILL HEAD WHEN HIS FINGER WAS PINCHED BY TELESCOPING CANOPY JACK SEGMENTS.
While lowering drill pipe to connect tool joint, helper put left hand under drill pipe being lowered to remove dirt. Middle finger was jammed between drill pipe causing laceration of finger. Helper was wearing gloves.
Walking down stone steps and went to step down on next step and missed the step and twisted or sprained left knee.
While steam cleaning a piece of equipment, the injured accidently squeezed the trigger of the wand and hit the right side of his right foot. The outside of the foot received 1st and 2nd degree burns.
Injured was exiting slope belt when he stepped up to a step his knee gave out resulting in a sprain.
While putting cement bags on packer he twisted lower back. Rx - Naproxen, Tylenol, Acetaminophen, Banalog, Soma, Vicoden, - Receiving therapy.
The operator was exiting a front end loader. When he turned to climb down ladder, his foot caught on step. He fell backwards to ground.
EE was squating down to open the hatches on a bulk truck when he felt a burning sensation in his left knee. EE is scheduked for a MRI on 7/6/05 and the doctor will determine the extent of the knee injury.
EMPLOYEE WAS DRAGGING TWO METAL ROOF SUPPORT STRAPS WHEN HE TRIPPED (AND FELL) WHILE CROSSING A TRACK RAIL RESULTING IN A TORN ACL LIGAMENT IN KNEE.
4 I TRACK HAULAGE, PUTTING ROCK DUST TANK CAR ON TRACK. USING AIR BAGS TO RAISE CAR UP, THEY WERE PASSING A LIFTING JACK TO OTHER SIDE OF CAR. AIR BAG KICKED OUT, STRUCK JACK & JACK STRUCK EMPLOYEE ON THE LEFT CHEEK.
EE was operating the rock truck hauling coal out of the pit area. As he exited the pit area via a road ramped up to the existing haulage road, the wheels lost traction due to the ramp being wetted down to control dust. The truck began sliding backwards into the pit and the operator turned the truck into the embankment, forcing the truck to lay over on the driver's side.
EMPLOYEE WAS MOVING STONE FROM ONE PALLET TO ANOTHER AND FELT PAIN IN HIS BACK.
EMPLOYEE WAS CLEANING AROUND #8 BELT HEAD THAT WAS GOBBED OFF AND TRIPPED OVER TURNBUCKLE AND FELL TWISTING BACK AND LANDING ON LEFT SHOULDER & ARM. WENT TO HOSPITAL - TURNED IN A PREVIOUS CLAIM ON BACK & SHOULDER, AGGREVATING PREVIOUS INJURY.
Employee was bringing steel up out of the hole. He saw the dust and started running. A rock fell from ledge and hit employee's ankle.

Employee was working to remove a dryer lid when he lost his balance falling approximately 4-5 feet and fractured his right wrist. Employee later required surgery. Employee started missing 8-9-05.
STEAMING OUT BED OF MECHANICS' TRUCK THE EMPLOYEE RAN THE WAND ACROSS THE TOP OF HIS FOOT, BURNING HIS FOOT.
EE was pulling a fuel hose out to fuel the rock crusher, when he sprained the middle of his back.
THE EMPLOYEE WAS ADJUSTING THE SCREWS OF THE PRIMARY CRUSHERS BLADES, A PART OF THE WRENCH DISASSEMBLED AND IMPACTED THE MIDDLE FINGER OF HIS LEFT HAND
EMPLOYEE CLAIMS THAT HE HURT HIS BACK WHEN HE HIT A POTHOLE ON 4/18/05 BUT IT WAS NO BIG DEAL, ON 6/20/05 HE WENT TO CHIROPRACTOR AND IS OUT OF WORK UNTIL 6/26/05. NOW HE IS OUT AGAIN
THE INJURED SAID HE WAS WALKING OVER TO GET THE SCOOP WHEN HE RAN INTO A ROOF BOLT, FELL DOWN TO HIS KNEES, AND HEARD SOMETHING POP IN RIGHT KNEE. HIS KNEE WAS NOT HURTING THAT BAD. HE FINISHED WORKING THE REST OF SHIFT.
Employee was changing drive belts on motor, caught finger in belt injuring finger, knee and head. (per phone conversation with company: EE slipped & fell, hitting his head on a cement pier and his knee on a motor base after freeing his finger from the pulley)
Employee was operating scoop, got too close to rib, caught his ear on rib.
Employee had backed cont miner out of #4 room to cut a corner off so that the shuttle car could get around easier. After he had cut the corner off, he backed the cont miner out by intersection waiting for roof bolter to bolt corner. While he was standing beside miner a piece of rock 5' long by 3' wide by 4-14"" thick struck the employee.
The brakes locked up on the #3 shuttle car causing the car to slide into the rib. The employee had his hand on hand rail and his hand was mashed into rib of coal. Cut little finger and ring finger, broke little finger.

While climbing down from a lathe set-up process, an intermediate step was not used which caused an oversized step or fall that resulted in an abrupt and additional force to employee's knee causing hyperextension injury.
As the crew was riding in the Suburban vehicle on the way to the Quarry, they crossed a rough spot, caused by snow melt run-off in the road, slowly. The bounce of the vehicle caused employee to jam his neck on the ceiling. He was in the back seat area.
EE WAS CUTTING CROSS CUT FROM ENTRY #1 TO ENTRY #0 & HAD SET THE MINER OVER TO CLEANUP. HE WALKED BACK BETWEEN ENTRIES 1 AND 2 TO MOVE THE MINER CABLE OUT OF THE PATH OF THE COAL HAULERS. WHEN HE BENT OVER TO MOVE THE CABLE, A ROCK FELL OUT OF THE RIB ABOVE THE COAL SEAM & STRUCK HIM ON THE LEFT SIDE OF THE HEAD & THE BACK SIDE OF HIS LEFT SHOULDER.
Packing holes for a blast, employee was walking between rows of holes and ground gave way. Employee left leg fell into hole twisting right leg.
Hit head on track. Cut above right eye. Fractured right arm.
Employee was drilling a 1"" hole into mine roof. Drill steel bent, striking employee's right thumb. Jammed thumb causing pain. Employee started missing work on 6/21/05.
Employee was assisting another employee to install ribs on bucket and in the process his eyes were burnt.
Putting Bottom Pin in bucet of 992 c. as he rolled the pin to align the bucket hole, he pulled a muscle in his back.
Miner was at the power box when he alledgelly tripped over a trailing cable, miner said he twisted his back. He was checked out by our M.E.T. and was escorted to the Pikeville Medical Center where he was treated and released.
Employee was working on a drive shaft on a mantrip. The mantrip moved & his hand was injured.
EMPLOYEE WAS OPERATING AN EXCAVATOR WHEN A LARGE ROCK WAS THROWN FROM THE BUCKET AND STRUCK THE CAB. THE ROCK ENTERED THROUGH THE GLASS ON THE TOP OF CAB AND HIT THE EMPLOYEE.
The employee was attempting to support the transmission oil pan of a front-end loader with his hand. The mechanic broke the seal between the transmission oil pan and the transmission with a crowbar. The oil pan broke free and fell to the concrete slab. It pushed the employee's left hand down to the concrete slab and fractured the tip of the middle finger.
Employee was helping build cribs and felt tightness in his back.
EMPLOYEE WAS RUNNING CENTERS IN THE FACE OF #4 ENTRY LOOKING BACK AT SIGHT RODS. HE SLIPPED ON UNEVEN BOTTOMS AND TWISTED/SPRAINED HIS RIGHT KNEE. LOST TIME BEGAN 9/9/2005
Lifting draw bar after coupling it up, he started to get up and felt pain in lower back.
He was next to the continuous miner working on it, when he was struck by a 3 wheeler personnel carrier on the left leg. His leg was between the three wheeler and the pan of the continuous miner.
Employee was sitting in the cab of borer miner and siting on his cap lamp cord. He twisted and pulled his head up to free the cord and banged his head against the unistrut that was on the underside of the canopy. He recieved a strained neck requiring 3 days away from work to recover.
SHOVEL OPERTOR WAS WALKING OUT OF THE CONTROL ROOM IN THE HOUSE WHEN HE STEPPED OFF THE LAST STEP IN OIL AND FELL ON HIS BACK. HE WAS HOLDING THE HANDRAIL WITH HIS RIGHT HAND AND HIS LEFT ARM HIT THE HOIST BRAKE IN AN ATTEMPT TO CATCH HIMSELF. HE WAS TREATED BY A PHYSICIAN AND TAKEN OFF WORK.
The employee was eating lunch near a Metal Halide light source that had the outer envelope on one light fixture broken. Although not conclusive, it is possible he received ultra-violet exposure to his eyes from this broken light.
Employee was bending/leaning over the side of the Roofbolter lifting a bundle of plates to move them up to his boom for use. As he was pulling on the bundle a wire which holds them together was hung on another bundle and he jerked on it to get it loose, straining his back.
Employee was mounting #189 777 haul truck, his foot slipped off the swing step, causing him to fall against the truck's bumper causing contusion.
EMPLOYEE STEPPED ON A SHOVEL & TWISTED HIS KNEE.
EMPLOYEE WAS BENDING ROOF BOLTS IN PREPARATION FOR THIER INSTALLATION. HE WAS STANDING BESIDE THE ROOF BOLTING MACHINE WHEN A ROCK FELL FROM BETWEEN THE OUTSIDE ROW OF BOLTS & THE RIB, STRIKING HIM IN THE MIDDLE OF HIS BACK JUST BELOW HIS SHOULDERS.
EE WAS WATCHING A CABLE SO THE COAL HAULER WOULDN'T HIT THE CABLE. THE COAL HAULER HIT HIM.
EE was using a metal bar to force the bottom door of the rail car closed. While pushing the bar he felt his left shoulder pain and some arm numbness and pain in the left scapula. he was taken to urgent care.
Employee was in a parked haul truck and bent over to get something out of his lunchbox and his back popped
Employee was servicing equipment, while walking across the yard he stepped in a hole and twisted his ankle.
He was setting an I beam on top of a shield for recovery. A piece of rock fell and struck him on the right forearm and right wrist causing an abrasion and contusion.
EMPLOYEE STATED THAT HE WAS WALKING ACROSS THE LAST ROW OF CROSSCUTS WHEN HE SLIPPED AND FELL IN AN UNEVEN PLACE IN THE MINE FLOOR. HE STATED THAT THERE WAS WATER IN THE AREA WHERE HE FELL. HE STATED THAT HE FELL ON HIS RIGHT KNEE WHICH CAUSED A TWISTING MOTION. HE REPORTED THAT HE EXPERIENCED PAIN TO THE RIGHT KNEE AREA.
The contracted employee was installing 4"" PUC pipe in the splitter plant overhead from a man lift approx 12 ft above floor and 4-5 ft from ceiling. Throughout the installation procedure PUC glue and cleaner was used. All garage doors open in the building. He passed out, had a seizure, and transported to hospital by ambulance.
ACCIDENT NOT REPORTED UNTIL 7/5/05. WORKING ON DREDGE REBUILD. EE STATED HE SLIPPED AND FELL WHILE TAKING HYDRAULIC PUMP OFF. THERE WERE NO WITNESSES. THIS ACCIDENT WAS ALLEGED AFTER EE WAS WRITTEN UP FOR NOT REPORTING FOR WORK AND TERMINATION WAS BEING CONSIDERED.
THE SUPPORTING CABLE ON SPIDER PARTED CAUSING SPIDER TO FALL AND CAUSING INJURED PARTY TO FALL FROM SPIDER.

SHOULDER WAS DISLOCATED, REPORTED TO ANOTHER FACILITY 6/19/05 DUE TO AFTER SHOWERING HIS RT. SHOULDER CAME OUT OF PLACE, SURGERY PERFORMED ON 6/20/05.
WHILE SPLITTING STONE PIECE OF STONE FELL OVER CAUSING CONTUSSION TO RIGHT LEG
Employee was driving a forklift on a sloped bank with a 2400 pound bulk bag at an elevated position. The employee was injured with no damage to the forklift. Employee was confined in the operator compartment.
EE was replacing a pipe fitting when a hammer that was on an overhead pipe fell and splashed dilute caustic into the EE's left eye. Result was a corneal chemical burn that required prescription medication. EE was taken off of work by his treating physician on 06/18/05.
Employee had just topped a can and was standing by that crew when the next can was set 10 feet out by. The can setter rolled into the can set out by and it fell striking the employee in the right shoulder. This resulted in a fracture tot he right shoulder blade.
Employee reached, with left hand, and grabbed roof support on fork truck. He put his left foot up on forklift and his right foot slipped on a rock or small object causing him to fall. He broke his fall by grabbing with his right hand, pulling his back.
Employee was moving into position to clean jets in the siphonsizer when she stepped into the drain hole and cut her leg.
Employee lifted sample bucket from truck bed and felt pain in right elbow.
Employee was shoveling mud on the beltline when he strained his back.

Employee alleges to have injured his back while dodging a piece of draw rock that had fallen from the mine roof.
Moving miner cable when he was pulling cable. He strained his right arm.
Injured employee was riding in basket of 130' manlift in lowered and retracted position. While reversing to make sharp turn, rear wheel went into 4"" deep hole which caused basket to bounce extremely. Employee was wearing fall protection and was propelled upward several feet. Upon landing in basket, his left leg hit against piece of trash metal causing severe laceration.
Employee was struck by a rock that fell from in between roof bolts.
EMPLOYEE WAS SHUTTING ENGINE COMPARTMENT DOOR ON LOADER WHEN HIS FOOT SLIPPED, CAUSING PAIN TO HIS LOWER BACK.

PROCESS OF REMOVING PIE LINERS FROM FEED AND #3 BALL MILL. ATTEMPTING TO REMOVE LINER WITH HYDRAULIC SCISSOR JACK LINER FELL UNEXPECTEDLY STRIKING EMPLOYEE ON RIGHT ARM RESULTING IN LACERATION REQUIRING STITCHES
THE EMPLOYEE AND CREW WERE EXTRACTING AN OVERCAST WALL. THE OPERATOR HIT THE WALL WITH A WAGNER BUCKET. THE FRAMEWORK THAT WAS BUILT INTO THE WALL POPPED OUT AND FELL TOWARD THE EMPLOYEE. HE TURNED TO RUN AND WAS HIT BY THE FRAME FROM BEHIND. IT DROVE HIM TO THE GROUND. LOST TIME ACCIDENT; FIRST DAY OUT 6/17/05.
Fell off work boat and sprained right wrist. Restricted duty lifting limited to not more than 20 lbs.
Injured was putting breaker in on shuttle car (testing cable) when breaker flashed burning right hand.
Employee was helping install roof bolts along 7H belt for additional support. Co-worker pulled on slack hose causing the stoper drill to fall, injured tried to hold onto drill, but caught his left hand between the drill and the belts and bed rail.

The employee was climbing up the steps on a 988 Cat loader, he lost grip with his right hand which caused him to spin left which threw him off balance and he lost his footing. He fell to the ground resulting in a fractured left ankle.
While loading 50 pound bags of rock dust, employee felt pain in left wrist.
Operator was delivering two connex's of sand to the sand filter tent with the 8233 tractor trailer and 40' high deck. He was going to stop, shifting from 2nd gear to reverse when he lost his brakes and could not get the truck stopped. He drove the truck to the bottom of the hill and rolled it over before inpacting the hill.
Employee was driving a 10 wheel dump truck on haul road at Dixie Mine. Road surface was wet clay. As vehicle was negoiating a curve, the vehicle started slidding. Vehicle rolled over.
Employee was pulling old belt out of way, when he lost his grip on the belt causing him to stumble and fall.
Employee alleges that he had been picking up trash all day and as the day wore on his back got sorer.
While trying to repair dozer, the blade arm slipped against the dozer tracks and pinned the mechanic's leg between the blade arm and the dozer tracks.
Oxygen hose failed below the fitting near the torch-end thus causing a fire and igniting the employee's glove on fire. The burning glove caused second and third degree burns on the right hand of the employee.

Employee was working on replacing a hydraulic cylinder on an underground drill. While driving the pin in the cylinder mount, a piece of the pin hit him on the right leg.
Employee was shoveling spilled material and throwing it out the hole that had been cut in the wall when the shovel hit the conveyor roller jamming his thumb.
While carrying a bottle of compressed oxygen into the beltline, the employee slipped and fell and the bottle landed on right hand and wrist. This caused a contusion to his wrist and he was not allowed to return to work for two days.
An Employee was unloading high-line cable on the main haulage when he slipped and fell, striking his right hip and back against the flat car.
A pickup truck wouldn't start. It was in the way for dump truck to back up. Owner of Alliance Sand told Employee to push it out of the way. EE then started to push the truck & his hand slipped off the door jam, hit the grab handle, breaking it from the force from slipping & then cut his hand on the broken piece that was still attached to the truck.

DID NOT START LOSING TIME UNTIL 8/17/05. While swinging pick, he hit his R.wrist on beam under rail. Was being treated for a sprain until the doctors discovered it was fractured on 8/16/05. Arm in cast.
Employee stepped onto an elevated wood block. The wood block was not stable and rolled. The employee fell and sprained his left ankle.
While roof bolting, employee set the foot jack which broke thru a piece of rock. The rock then struck him on the top of his foot (L).
Employee was pressure-washing a front-end loader. He slipped off the fender of the loader, falling onto the tire and loader lift arms. Injured left shoulder & right hand.
While employee was cleaning up a pile of lime, lime got into his boots causing a lime burn to the lower portions of both legs.
Employee was going up a fabricated step to get to a platform when the step came loose. He then fell 10 to 12 feet.
Miner operator sounded roof and ask bolter operator if he would spot bolt the area for additional support. While spot bolting rock fell and landed on operators right heel.
Employee was injured while attempting to lift an auger flight with a hoist. The flight hung up and when the employee freed it, one end of the flight fell onto the employees left hand.
THE EMPLOYEE WAS CLEANING UNDER 18 WEST GATE SECTION BELT WHEN HE FELT PAIN IN HIS LEFT SHOULDER
EMPLOYEE ALLEGED THAT WHILE WORKING ON THE LIGHTS OF THE FRK30, HE STEPPED OFF OF FORKLIFT ONTO THE ELECTRIC SHOP FLOOR AND SPRAINED HIS LEFT KNEE. EMPLOYEE DID NOT MISS ANY TIME BUT HAD SURGERY ON 10/24/05.
Employee was raising the head of the roof bolter to attempt to couple back onto the drill steel that was stuck in the roof when his finger got between the drill head and drill steel.
EMPLOYEE WAS POKING THE 3RD HEARTH OF #2 ROASTER WHEN HIS GLOVES BECAME TOO HOT. HE PULLED POKER OUT OF HEARTH & THEN REMOVED HIS GLOVE, HE NOTICED ROASTER ARM CAUGHT END OF POKER, FORTETTING HE HAD REMOVED HIS GLOVE & GRABBED THE POKER AGAIN. THE POKER WAS VERY HOT & CAUSED SERIOUS BURNS TO THE LEFT HAND & FINGERS. WENT TO THE E.R.
EE WAS DESCENDING LADDER FROM CATWALK. HIS FOOT LANDED ON OBJECT, CAUSING HIM TO TURN HIS ANKLE OVER. DIAGNOSIS: FRACTURED 2 BONES RIGHT FOOT.
Man working on coal hauler 3EP unit, went to get down and twisted his left knee. Started missing 8-17-05.
Employee was backing up in his Euclid to dump DGA in stockpile. He backed up too far and backed over the edge of the stockpile and flipped over backwards, with the Euclid landing on its wheels.
Employee was pushing rock with a dozer when the dozer lost traction and slipped back. When the dozer caught traction, it jerked the employee's back.
EE WAS WALKING AND STEPPED ON A PIECE OF DRAW ROCK AND TWISTED HIS KNEE
Employee pulled a groin tendon from falling in a hole while replacing grade bars in clinker cooler in a limited work space. It was dark and the floor had debris on it.
Employee's ankle (L) was struck by a rib roll while he was re-entering a miner cable.
Mechanic was working on lights on 301 D10N when operator EE went to get off of dozer to use the bathroom when he slipped and fell on push arm of dozer. NOTE: He missed 6/14/05, worked thru 6/15/05 and missed 6/16/05 and 06/17/05
Employee was tramming bolter from front controls. He got his SCSR stuck in the controls causing the bolter to turn into rib, pinching him.
Employee was moving a 6"" water pump while operating a Cat 910 loader. The cab door was opened and he had his hand on the door frame. A gust of wind caught the door and shut it on his finger.
Employee was unloading brattice block from scoop bucket, and twisted his back.

DID NOT START LOSING TIME UNTIL 07/21/05. While climbing stairs, left knee popped and started hurting. Surgery required.
Ee was machining faces on nose cone of a d10r dozer. I stopped the drill and steped around to look at face & I stepped on a small rock and twisted my knee as I stepped around to look at the face.
Employee allegedly was rebuilding a hydraulic cylinder. The end with the pin slid out severing the tip of his middle finger on the left hand.
Loose material sloughed causing rear tires of trailer to drop off stockpile. Trailer turned over on its side causing the road tractor to turn over on its side. The air filter, ferring, mirror, hood were damaged on the road tractor.
A RD-800 MACK TRUCK LOADED AT THE W-1 MINE PIT & WAS GOING OFF THE HAUL ROAD WHEN DRIVESHAFT CAME OUT & THE DRIVER TRIED TO RIDE IT OUT. THE DRIVER JUMPED FROM THE TRUCK WHICH LEAD TO HIM HAVING A SERIOUS HEAD INJURY.
Injured employee put hand in air operated valve to clean out materials. Had valve in local control, but not locked out. Valve closed on left wrist causing possible nerve injury. Employee recieved disciplinary action for not locking out system prior to working on it.
While inspecting tailgate drum for bad bits and sprays, employee turned drum and coal face turned loose and pinched his right hand between face and tailgate drum. This caused an open avulsion laceraction to his right hand and a fracture to his right fifth finger.
Injured was hanging checks when a piece of head coal fell striking injured on his rt. shoulder, causing a bruise.
CLEANING MINER, ROCK FELL OFF HEAD STRIKING TOP OF RIGHT HAND
Employee developed lower back pain while loading stockpile trucks at the bins. He sought medical attention on 13 June 2005 and missed one day of work.
Employee was loading a face round when the rib/back fell in. The rock struck him on the head/hard hat. He was taken to the hospital the next day and the Dr. placed him in a neck collar for precautionary reasons.
Employee was pre-shifting the faces when he slipped, striking his left knee on the ground.
The employee was washing his area with a hose. The hose had a small amount of caustic running through it. While he was dragging the hose he accidently turned the valve open spraying himself in the face.
INJURED ASSISTED THREE CO-WORKERS INSTALLING A 6' X 7' METAL DOOR AT THE PORTAL ENTRANCE. WEIGHT SHIFTED AND EXTRA EXERTION CAUSED THE BACK INJURY.
Employee was attempting to set a mine prop, when he bent over to get a cap wedge, the timber started to fall toward a co-worker. In an attempt to catch the timber employee twisted his left arm.
Running crusher plant while lime silo was being emptied with hydrated lime. Five (5) days later, employee informed office of ""rash or infection"" to feet. Employee stated that he thought it was from the lime.
The employee was using a scaling gun and smashed his finger with the gun.
Welding on chute liners of reclaim hopper #3, welding sparks caught pant legs on fire. When employee bent down to put out fire on leg, he twisted wrong, hurting back.
On 6/10/05, a mechanic working the afternoon shift was walking from #2 entry to #3 entry when a large piece of rock fell out of mine roof from between permanently installed roof supports, it measured 8' long x 2' wide x 8"" thick. This rock fell out of the mine roof between a roof bolt & the rib, striking him on left lower leg & ankkle, fracturing bones in lower leg.
No specific accident. While doing normal duties, while handling a roof support strap, employee experienced a slight pain in his right shoulder.
Employee was exiting the 988 F Loader and missed the last step. twisted right knee as he went to ground.
GOING DOWN THE RAMP IN THE WATER TRUCK & CAME OUT OF GEAR, TURNED RIGHT TO MISS HIGHWALL & ROLLED OVER ONE TIME. THE TRUCK WAS FULLY LOADED WITH WATER. THE SEAT BELT WAS ON. THE INJURED WENT TO THE E.R. & HAD SEVERAL STITCHES PUT IN HIS LEFT HAND, HE NICKED THE TENDON IN HIS HAND.
EE'S FOOT SLIPPED WHEN CLIMBING OUT OF OLD POWER CENTER BEING WORKED ON. HE GRABBED A COME-ALONG HANGING AT POWER CENTER. HIS RIGHT THUMB WENT INTO THE HOLLOW BACK OF THE HANDLE AND WAS LACERATED.
EMPLOYEE STEPPED OFF EDGE OF 15 INCH HIGH CONCRETE APRON OF PROCESS TRUCK WEIGH SCALE. UPON STEPPING DOWN, HIS LEFT ANKLE TWISTED ON A PIECE OF LIME ROCK. EE FELL AND STRUCK HIS RIGHT SHOULDER ON THE GROUND. HE WAS TRANSPORTED TO THE LOCAL E/R AND TREATED FOR A LEFT ANKLE SPRAIN AND RIGHT SHOULDER CONTUSION.
THE EMPLOYEE STATED HE WAS OPENING A LID TO THE SCOOP 610 BATTERY COMPARTMENT WHEN HE FELT A PAIN IN HIS RIGHT GROIN AREA.
Alleged incident was to have occurred while this employee was stepping down and stepped on a piece of coal on the walkway twisting her ankle. Cause is awarness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner proficiency a factor, mining equip./systems not a factor.
PERSON WAS TRAMMING BRIDGE. THE DECK OF THE BRIDGE FLOATED UP CAUSING INJURED TO TWIST HIS RT. KNEE AGAINST THE FRAME.
Employee was carrying a 5-gal bucket and bending over while passing under a belt conveyor structure. Suffered muscle strain to lower back and hip.
EMPLOYEE STATES STRAINED BACK WHILE WORKING ON PIPELINE FOR THE DREDGE. STATED WORKING OUT OF A DESIGNATED WORK BOAT AND A GUST OF WIND BLEW AND STARTED TO SEPARATE THE PIPELINE. EE STATES SHOULDER POPPED & HE STOPPED WORKING & STRETCHED. HE WENT BACK TO WORK FOR THE REST OF THE DAY WITH NO PROBLEMS. ON MONDAY 6/13/05 HE REPORTED TO THE ER WITH BACK PAIN & SAID IT WAS WORK RELATED.
Going up mine road to pick u pump and met a coal truck that was driving left of center of the road in a curve and collided head on.
DIGGING WITH EXCAVATOR IN POND, EAR ON BUCKET SNAPPED AND FLEW INTO CAB AND STRUCK IN FOREHEAD.
EMPLOYEE WAS LIFTING BAGS WHILE LOADING A BLAST. HIS FOOT SLIPPED CAUSING HIM TO TWIST. HE EXPERIENCED PAIN IN LOWER BACK
MAINTENANCE MECHANIC REMOVING AND INSTALLING AN ELECTRIC MOTOR ON SCREEN DECK ON PORTABLE SCREENING PLANT. ANOTHER EMPLOYEE WAS OPERATING A CRANE TO LIFT THE MOTOR. HE PLACED HIS HAND BETWEEN THE SNATCH HOOK AND THE SCREEN FRAME AND PLUMBING TRIED TO STOP THE SNATCH HOOK AND HIS HAND WAS PUSHED AGAINST THE END OF A BALL VALVE.
Employee was moving a lateral for the sandfilter and tripped over another lateral straining his left calf muscle.
Auger was maintained and as machine was being lowered to move over, employee walked up to machine and put his left foot 2"" under the frame of the auger. He had steel toe boots on whereas the steel toe was not crushed but the bottom of the boot came up crushing the tip of his toe.
While trying to stop an oil leak he made a plug by using a handkerchief. He used a screwdriver to put more pressure on the handkerchief when it (the screwdriver) slipped producing a small wound on the left hand.
EMPLOYEE WAS BRINGING A LOAD OF COAL FROM THE HAZY SEAM. ON HIS WAY UP THE HILL, HE HEARD A POP SOUND FROM THE BACK OF THE TRUCK AND THE TRUCK STOPPED PULLING. HE THEN HEARD ANOTHER POP SOUND, THE TRUCK ROLLED BACKWARD ABOUT 35 FT AND HIT THE HILLSIDE BERM AND OVERTURNED ON DRIVERS SIDE. HE WAS TRANSPORTED BY AMBULANCE TO HOSPITAL
Employee slipped on wet track surface causing loss of balance and a fall. Right leg and foot were caught between surfaces. Fall resulted in fracture of right tibia.
EMPLOYEE HIT HIS HEAD AGAINST THE TOP WHILE RIDING IN A SCOOP.
Employee was letting down his basket to put work clothes on and felt a pain in his back, lower portion. Employee stated that he was in a turned position while letting it down.
LOST HIS FOOTING COMING DOWN FROM TOP OF MOTOR. FELL AND CAUGHT HIMSELF WITH LEFT HAND CAUSING HIS LEFT THUMB TO BE OVER-EXTENDED.
ASCENDING LADDER, STEPPED ON ROCK TWISTING KNEE.
Employee initially did not mention incident to anyone. He later stated that he got dirt in his eye while working on a truck fender, but did not know what time it happened.
OPERATING SCRAPER. HIT SOFT SPOT IN HAUL ROAD. TOSSED AROUND INSIDE OF SCRAPER, HITTING HEAD ON RIGHT WINDOW (CONCUSSION) AND HURTING LOWER BACK.
Injured employee lost his balance while reaching over from 3rd of 4th rung of step ladder. Injured employee fell and sprained his right ankle.
EMPLOYEE WAS ALONG SIDE THE CONTINUOUS MINER OBSERVING THE MINING CYCLE WHEN HE WAS CAUGHT BETWEEN THE ATRS CONTROLS AND THE RIGHT COAL RIB RESULTING IN A HAIRLINE FRACTURE TO HIS LEFT PELVIC AREA.
The Employee was adjusting the feed gate on a belt. The wheel handle gave all at once and pinched his fingers between the wheel handle and the brace/bracket.
EE was starting to walk beneath a belt underpass guard and hit his left shoulder on the guard causing him to loose his balance, and fall. The area was muddy and he had his coat and lunch bucket in his hands. When he fell he broke the lower end of his fibula on his right leg.
Hoisting / lifting a section of scaffolding with rope employee strained lower back.
Was attempting to find vinyl hose which was stored in metal container approx 30"" high. Retaining rings were setting on top of lid, he grabbed ring to slide off onto floor. It started rolling toward him, he reached out to grab it to stop it from hitting leg, he felt a pop in elbow area. Strained right bicep tendon.
EMPLOYEE STATED HE WAS MOVING BELT FRAMING & STEPPED ON A PIECE OF CONVEYOR BELT, FOOT SLIPPED CAUSING PAIN TO HIS BACK. EE STARTED MISSING WORK ON 6/10/05.
PRY BAR SLIPPED WHEN PRYING SAND ROCK OUT OF HOPPER (LACERATION AT TOP OF FOREHEAD).
While trimming a piece of belt, the belt knife slipped cutting the victim on the inside of left forearm.
Employee was operating #11 motor in the E-12 track entry. When he went across the #35 wall track switch the motor whipped to one side dislodging the seat from the motor deck. The seat fell over, causing him pain in his lower back.
Employee says that he was ?jarred? in the rock truck while being loaded. Two days rest.
While loading a ore truck, a chunk of ore fell out of the bucket on the right side. The operator did not see the chunk and ran over it, severly jarring the machine, causing a fast side-to-side movement of the neck and head of operator, straining the neck from impact.
Employee was operating an LHD when a steel, hydraulic line ruptured. Hot, pressurized hydraulic fluid penetrated the employee's left calf. The employee did not return to his next shift.
Injured was attempting to lift a rock up on the belt line when he felt something in his back slip.
THE EMPLOYEE WAS MOVING BAGS OF CEMENT FROM ONE PLACE TO ANOTHER. DURING THAT PROCESS HE FELT A SEVERE BACKACHE.
HE WAS PUTTING RUB RAIL BACK ON MINER, WHEN HE STRAINED HIS BACK. ** STARTED MISSING 08-03-05**
TRAMMING FEEDER CABLE WHICH WAS PILED UP ON THE OFF SIDE. EMPLOYEE MOVED FEEDER TO TRAM SIDE & TO CLEAR CABLE. EE PINNED HIMSELF BETWEEN THE FEEDER AND THE RIB.
EE went up a 12 foot fiberglass ladder to perform work on ceiling area. After assesing how to tie off EE started back down the ladder, he lost his balance and fell about 8 feet landing on his back. He was transported to the hospital via ambulance where it was found that he had two cracked vertebrae and was suffering from a concusion.
The 3-Wheeler that employee was operating slipped and jerked the steering wheel causing his elbow to strike a piece of metal.
Injured was installing ""Keeper"" pins in the conveyor chain of CM using two hammers (striking them hammer to hammer) to bend the pin. A chip of metal flew from the masonry hammer striking and embedding in his left wrist.
