In the tailgate entry on the longwall panel a roof fall 75' to 90' and 6' thick by 20' wide. Contacted federal inspector on 5-31-05 about roof fall.

A ROOF FALL OCCURRED IN THE 1ST WEST OFF OF THE SOUTH SUB-MAIN, #7 ENTRY, #4 CROSSCUT. THE FALL WAS 25'LONG X 25'WIDE & 8'HIGH. THE AREA WAS BOLTED WITH 6' & 8' POINT ANCHOR BOLTS. THE FALL DIDN'T AFFECT VENTILLATION, TRAVEL, DAMAGE EQUIP. OR CAUSE AN INJURY. NO ONE WAS REQUIRED TO WORK OR TRAVEL IN THIS ENTRY.

Accident type, without injuries

The footwall slip that intersected with the hanging wall contributed to ground failure at 41W 8400 A5. The area was posted and barricaded at 11:30 PM pending further investigation. At 12:40 AM the failure had propagated to approximately 50' wide x 40' along strike x 20' high. The 50' cable bolts installed on 6' centers remained in place. MSHA was notified by telephone.

On Friday, 5/27/05, water inflow into No. 5 Mine exceeded pumping capacity. No employees other than those involved in water pumping were allowed in the mine after the Friday Owl Shift. Pumping efforts continued until 8:15 AM on Monday, 5/30/05. All power into the mine and the 5-3 Fan were then shut off. The mine is currently idle.

A roof fall occurred in the 3C return, crosscut #5, entry #3. The cause of the fall was weak laminated sandy shale commonly referred to as stack rock. The fall was located in the return for MMU-003-0 and was adjacent to the examiner's walkway.

Hoist person running skips thru for testing before mantripping. While testing the limits on the N side he couldn't back out the skip after testing the limits on the Norh side. Down 5-25-05 at 5:00 p.m. back running at 7:00 p.m.

Elevator went past bottom landing and tripped over travel switch. Brakes were making a noise, upon checking found brakes needed adjustment. Schindler elevator service company called to make necessary adjustments. Elevator was tested with test weight and put back in service.

No injuries or equipment was involved. This fall will not be cleaned up.

The trip switch indicator sensor shut down the hoist and it would not reset, upon investigating it was found there was a grease build up on the slack rope sensor. after cleaning off the grease, we were able to reset the sensor. the hoist is back in full operation at 12:07 am.

A ROOF FALL WAS DISCOVERED WHILE FIREBOSSING MAIN RETURN OF THIS MINE AT 4 RIGHT OF NORTHWEST MAIN. THIS IS A CONTINUATION OF A FALL THAT OCCURRED 3/21/05. 20' WIDE, 60' LONG, 6-10' THICK. THIS FALL STARTED AT SS #773. FELL TO FACE.

Contractor was burning debris behind the mine maintenance barn without required notification of approval. According to the owner, he thought the fire was out and left the mine site without the appropropriate fire watch. The fire was extinguished the first time by the fire department at 2123 hours. They were called back again at 0115 am. Approx. 1.8 acres burned.

DISCOVERED A ROOF FALL IN #6 ENTRY, ONE BLOCK FROM THE FACE ON THE 002 SECTION. THE FALL IS 9'H X 17'W X 20'LONG. IT WAS A 3WAY INTERSECTION. IT HAD BEEN BOLTED WITH 6'RESIN BOLTS & SPOT BOLTED WITH 9'INSTALL BOLTS. THE AREA WAS DANGERED OFF, BREAKERS SET & WILL NOT BE CLEANED UP. NO PERSONS OR EQUIPMENT WERE INVOLVED.

We had a roof fall in an area of the mine that had been cribbed and dangered off for about 3 weeks, 20 ft in by spad #5125 or 7 brk out by face in the #3 neutral entry. The fall was approx 18' wide by 70' long and 10-14' high. The fall did not block intake or return airways, belt entry or neutral travelway. Fall will not be cleaned up.

When inspecting the hoist, oil was observed on the brake pad. A leaking seal on the south side brake caliper was found. Due to inavailability of a replacement seal and a new brake pad, the hoist was down for more than 30 minutes.

A roof fall occurred in the 1st west off of the 1st north, #7 entry, #56 crosscut. The fall was 20' long, 20' wide, and 8' high. The area was bolted with 6-8' point anchor bolts. The fall did not affect ventilation, travel, damage equipment or cause any injury. No one was required to work or travel in this entry.

CITYWIDE SWEPCO (UTILITY COMPANY) POWER INTERRUPTION OCCURRED AT 8:45 AM ON 5/21/05. THE MINE EVACUATION WAS COMPLETED AT 9:00 AM. POWER WAS RESTORED BY SWEPCO AT 10:20 AM. THE HOIST WAS TESTED AND MINERS RETURNED UNDERGROUND AT 10:35 AM.

AT APPROX. 9:20 AM ON MAY 20, 2005, AN EXPLOSION WAS HEARD COMING FROM LINE NUMBER ONE. THE DEFLAGRATION WAS OBSERVED TO BEING AT THE ELEVATOR BOOT & CONTINUED TO THE TOP OF THE ELEVATOR. THIS EXPLOSION WAS CAUSED BY THE IGNITION OF A DUST CLOUD INSIDE THE ELEVATOR.

(2ND OF 2 ROOF FALLS THIS DATE.) A ROOF FALL OCCURRED AT 2 MAIN WEST #4 BELT LINE AT 62 CROSSCUT. THERE WAS A COAL RIDER IN ROOF THAT GAVE WAY. FALL WAS 25' X 25' X 10'. THE AREA IS SUPPORTED WITH 5' FULLY GROUTED RESIN BOLTS WITH TIMBER SET IN WALKWAY.

A roof fall occurred in the 3B return at cross cut #8 in entry #1. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was located in the #1 entry of the return for MMU-003. The fall was located adjacent to the walkway used by the weekly examiner.

An unintentional roof fall was discovered 05/19/2005. Fall measured approximately 6' high X 100' Long X 18' Wide. The intersection was supported with 6' fully resin grouted tension bolts during development on 06/2001.

A PARTIAL COUNTY POWER OUTAGE CAUSED A TOTAL MINE SITE BLACKOUT. THE SITE POWER OUTAGE RESULTED IN THE MINES HOIST BEING INOPERABLE FOR A PERIOD EXCEEDING 30 MINUTES. MSHA WAS NOTIFIED AS PER 30 CFR PART 50. HOISTING OPERATIONS RESUMED ONCE THE MINE SITE POWER WAS RESTORED.

Employee was driving a rock truck down a hill when the left front tire of the truck came into contact with a berm causing the truck to flip over.

The fire started sometime after 10 00 pm.It was discovered at 5:30 am on May 17 & was reported to the fire dept. They were on the scene at 5:50 am. & remained until 2:00 pm. They were recalled at 4:00 pm & remained until 7:45 pm.The fire seemed to be out at the time. The fire chief suspects possible electrical cause, but it is yet to be determined for sure.

A non-injury roof fall accident was discovered at 6am on 5-14-05 in the #1 entry at crosscut #71. It measured approx. 19' wide by 0"" to 7' thick by 80' long. The fall apparently occurred sometime between pre-shift on 5-13-05 and pre-shift on 5-14-05.

The failure plane was noted as a regional plane foliation, extending above the curent ground support depth. Geological observation revealed F2 folding striking north/northwest parellel to the orientation of the 30 decline. Approximately 400 tons of ground fell.

Employee was making dredge repairs using hand tools: 3/4"" ratchet & 4'-long cheater pipe. He took dredge pump apart and left the ratchet and pipe on the end of the shaft. He started the engine and then engaged the clutch. The pipe came around and hit him on the left side of his skull. Employee was hospitalized and died of his injuries on 7/3/05.

The victim was walking in front of the #11 scoop that was traveling in #1(intake) entry of the 14A longwall to deliver posting material to the worksite. The operator of the scoop stated that he saw the victim walking on the side opposite of the operator. He then looked back & did not see him. He exited the cab & walked to the front of scoop & found the vicim injured on the ground.

EE was in process of assisting in the removal of transport safety block on dragline bucket. He was assigned to cut the safety braces on the left side of the bucket. As he positioned himself, he was verbally told to hold up. As he began to climb down he placed his hand on the spreader bar and the hoist wire rope moved and pinched his right index finger and middle finger.

On the surface at Vent Shaft #8, The injured ee and one other ee was attempting to lift one end of metal grating (36 in. x 20 ft). It slipped catching the injured ee right middle finger tip between the grating and the asphalt. Employee was operating a Cat articulated truck hauling spoil to dump area of the hollow fill. Upon second load he attempted to back into the dump site and for whatever reason backing over slope he exited or was ejected from truck suffering severed right foot, cut and abrasions to rest of body. Per company call 5-27-05, leg was amputated to 5 inches above ankle - coder 12.

Employee was pulling a LW shield off the cage. Shield came off dollies and fell on employee. Lower left arm was amputated. Also, injuries to lungs.

The employee states he reached under the guard to remove a piece of leggen on a 60"" belt with his right hand while the conveyor belt was moving. The moving belt caught his right hand forcing the hand into the roller of the conveyor belt, cutting off the first joint of his right index finger. All guards were in place.

Injured employee was using a cast hammer mill hammer to try to open a slide gate. The employee slipped when swinging the hammer and struck his left hand with the hammer. The force and hammer's sharp edge amputated the tips of the 1st index and middle of the left hand. Rubber mallets are available for this and all workers had been informed to use only the rubber mallets.

Employee was walking beside supply car when he slipped on a rock an fell on right shoulder hurting his right shoulder and neck. (did not start missing work unitl 4-10-06) He last visit to a Medical facility on his shoulder and neck was on 1-5-06. He had a heart attack an had to undergo by pass surgery and on 4-10-06 had been released to undergo surgery to his neck.

ROOF CONDITIONS WERE CHANGING AND STRAPS WERE STARTING TO BE INSTALLED. AS THE SECOND BOLT WAS BEING INSTALLED IN THE STRAP, A PIECE OF ROCK FELL AND STRUCK THE EMPLOYEE ON THE FOOT. X-RAY DID NOT SHOW A FRACTURE; HOWEVER, AN MRI GIVEN ON 5/27/05 SHOWED A HAIRLINE FRACTURE.

On March 3, 2006 employee started missing time from an accident occurring on 5/19/2005. On that day he slipped and fell against the tail of the continuous miner while working on the trailing cable. He underwent surgery on March 3rd to repair a damaged rotator cuff. Management was informed that ee was missing time on March 24, 2006.

Injured was walking from parking lot to the bath house. As he was disposing a piece of gum, the gum landed on his tennis shoe. Injured tried kicking the chewing gum from the top of his shoe by kicking his foot, injured kicked the concrete base for the thickner fracturing his right big toe.

As employee was driving a service truck along a haul road the truck encountered a large bump that caused him to bounce upwards out of his seat and strike his head on the roof of the cab. As a result of striking his head and falling back into the seat he began to experience pain in his lower and middle back. At the time of the incident he was wearing his seat belt.

HORSEBACK OR SLICKED SLATE FELL FROM RIB, PINCHING EE AGAINST ROOF BOLTER AND CAUSING INJURY TO UPPER BACK AREA. BRUISING & SWELLING WERE OBSERVED AT INJURY. ROCK MEASURED APPROX. 4' X 18"" X 4-6"" THICK.

Ldr oper. was helping change out training roller. Employee tried to pick up roller by himself from the dollie without employee assistance. This resulted in an abdominal strain. Employee felt that he was alright, but he went to the doctor on 5/10, 1 of his 4 days off per his schedule. The Foreman didn't find out that the doctor wanted the employee off for 1 week, until 5/14.

EMPLOYEE WAS USING A 3/4"" RATCHET AND SOCKET TO TIGHTEN WHEEL LUGS ON A DUMP TRUCK AND THE SOCKET WRENCH SLIPPED LOOSE AND HE PINCHED HIS FINGER BETWEEN THE HANDLE BAR AND THE GROUND.

Employee was lifting jacks for a belt head on 5/6/05. He came to work the next day, worked 6 hours then left due to severe pain in his right side. He went to the E.R.; doctor thought he might have a hernia. A surgeon on 5/13/05 confirmed a hernia on the right side. Surgery would be required. He worked until 5/16/05.

Ee was dismounting dozer at end of shift and his knee seemed to be dislocated or pulled.
