Longwall Tailgate/4 N. headgare crosscut #4 at 1937 South, between entry #1 and enter #2. the fall was approx. 75-80' long, approx. 18' wide and approx. 12-15' thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. Fall #22PP.

The ventilation fans at the #1, #7, and #8 shafts were not operating for a period of time in excess of 30 minutes due to planned repairs on the reversing doors of the #1 and #7 Joy fans and power repairs on the #8 fan electrical system.

The graveyard longwall foreman came off the face to find the top from 25' inby the belt tailpiece to approx. 40 outby had fallen. The fall was approx. 18"" thick and did not exceed the anchor point of the roof bolts. A horizontal shear had broken the bolts off. The seconary escapeway was blocked and production was interupted until 07/31/2005 at approx. 4:30/pm.

A roof fall occurred 60 ft inby spad 1363 and it measured approximately 20 ft wide, 35 ft long, and 5 ft thick.
The Hart Street elevator malfunctioned. A braking coil moved out of position and caused an electrical short that blew a fuse on the elevator controls. The coil was repaired and the fuse replaced and the elevator was retrurned to service. In the malfunction state the brakes were locked and would not release.
The inspection of the emergency elevator revealed the outside door latch was not working properly. The outside door latch mechanism was found to be out of adjustment. The latch mechanism was adjusted and elevator was put back into service at 1:15 pm.
The Miracle Run elevator stopped about 100 feet up from the bottom. The Schindler Elevator Company repairman found a shorted lead in a power control cable.
An unintentional roof fall occurred on 2nd. Left Section #7 Entry @ Spad # 6046. Measurements: 50' (L) x 20' (W) x 8' to 9' (T)
Prior to start of daily operations, hydraulic pump to the hoist brake system was showing inadequate pressure. After adjustments, the hoist was put back into service at 9:00 AM. Later in the morning the hoist was again taken out of service to replace the pump, oil and filter. Hoist was put back in service at 2:30 PM. MSHA Inspectors on site this day doing quarterly inspection.
BREAK 317 #6 ENTRY FELL APPROXIMATELY 6' TO 8' HIGH AND 40' EACH WAY TOWARD #5 AND #7 ENTRIES.
The escape capsule at the #7 shaft was taken out of service for a period of time in excess of 30 minutes due to planned repairs during the vacation period. The alternate escapeway was re-routed to the #1 portal during the repair period. No other escapeways were affected.

The slope hoist main power control fuse was failing while operating the hoist. A contractor with better experience with the circuit was called in to assist in trouble shooting cause of problem. He found the thyristor(SCR) drive for both directions needed replaced. The items were replaced on 8.3.05 and slope hoist is operating normally at 3:00 pm.

A roof fall occured in the 2 Main North, crosscut #21 and extending approximately 20' inby and outby the intersection. The fall measured approximately 60'x19'x4'-7'. The area was supported with 6' tension rebar bolts and square timbers. The area was mined in December of 2000. An undetected slip was determined to be the cause of the fall.

7-22-05 6:45 AM A ROOF FALL (40'L X19'W X 5'H) OCCURRED IN THE NEUTRAL TRACK ENTRY 200' INBY THE ELEVATOR. THE ROOF STRATA IS LAMINATED MIXED SHALE WITH NO RIDER
ROOF FALL IN THE #2 THRU #6 ROOMS IN THE LAST OPER CROSSCUT. MOVE THE ACTIVE SECTION TO A NEW LOCATION IN THE MINE.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #9 ENTRY @ SPAD STATION #3393 ON #4 TRACK - MAINS. THE FALL MEASURED 7' H X 18' W X 60' L. CLAY WAS PRESENT IN MAIN ROOF (7' ABOVE MINE ROOF). AREA WAS CRIBBED OFF & DANGERED OFF.
Temporary hoist outage. A crack was noticed in a brake shoe on the production hoist at 7:30 am. The mine evacuation system was initiated, but the brake was repaired at approximately 7:54 am, before all underground personnel evacuated the mine. MSHA field office at Newark was notified at 7:50 am.
While performing work on #3 track motor anderson plug that was grounded, employee decided to remove battery cover lid so that he could cut one of the leads going to the anderson plug. While using a pair of kline cutters, he cut one lead that resulted in an arc causing hydrogen gas to explode.
AN UNPLANNED ROOF FALL OCCURRED AT THE LOGAN'S FORK MINE 15,000 FT FROM THE ACTIVE FACE OUTBY AT CROSSCUT 2.3 SPAD #243. THE FALL WAS 19'W AND 4-5 ' HIGH. NO INJURIES WERE REPORTED AND VENTILATION WAS NOT AFFECTED.
The roof in #2 entry between #27 and #28 x-cut fell for approx. 28 feet long 6 ft to 9 ft high and 14 ft wide. The roof was fully bolted and screened. Loose and Fractured roof contributed to the unexpected fall. the location is in the 5th left longwall bleeder return was found on weekly examination of the area.
The page portal service elevator overtraveled due to a bad electrical relay. The elevator was down for 20 hours, from 8:00 PM on 7/21 until 4:00 PM on 7/22/05. The relay was replaced, the elevator was tested and the service cage was put back into service. There were no people on the service elevator at the time of the incident.
Unintentional roof fall on 1st. Right Section, 1 break to the right of spad # 1573. Measured: 20' (W) x 20'(L) x 6'(H).

SLOPE HOIST INCIDENT - A kink was discovered in the slope rope on Wednesday, 07/20/05 at 06:00 pm. The slope was taken out of service. Repairs were made on 07/22/05 and the slope was put back in service.

ROOF FALL ABOVE ANCHORAGE IN MAIN NORTH # 1 ENTRY CROSSCUT #17 AREA WAS MINED MARCH 1984 AND WAS NEAR AN INTERSECTION THAT WAS PREVIOUSLY SHUT DOWN FOR OVERCAST CONSTRCUTION. HIGHT VOLTAGE LINE WAS UNDER THE FALL SO SOME POWER WAS LOST AND IMMEDIALTELY DISCOVERED AS INVESTIGATION AS TO REASON OF POWER OUTAGE.

One of our dust collectors empties into a trailer mounted bin that is then towed to the waste dump for disposal. As an employee open a slide gate and coal dust began spilling out, the dust ignited. Trailer and towing vehicle sustained paint damage.

The 3rd shift crew found unplanned roof fall's in the face area of the White Cabin # 7 mine. The falls were in the faces of #4, 7 and 8. The area was cribbed and dangered off and the equipment was removed from the section and moved approximatly 500' outby to a new section.

A roof fall occurred in the 5 North Panel in #8 entry starting in the pillarline inby cross-cut 63 and extending through the pillarline outby cross-cut 65. The fall measured approximately 70ft. x 19ft. x 5ft. The area was supported by 5ft. fully grouted roof bolts. The area was mined in July 2005. A hidden slip was determined to be the cause of the fall.
An untentional roof fall occured on 6 #1 belt. The fall is in #5 and x-cut 4-5. Area was supported by 5' resin and truss bolts. Height -9'; Width - 18'; Length - 55'. Fall was timbered and dangered off. Surrounding mine area was re-evaluated for safe roof conditions

A roof fall was discovered in the north main (002) working section during the preshift exam on Monday, July 18 in the last open x-cut of #22 room and extended outby for 200'. The fall was 5' to 8' high and 20' wide. As a result we moved to next room set.
A roof fall was discovered ont he 5 west belt at #15 crosscut int he #3 entry at spad #2350. The fall was approx. 23' long, 20' wide and 7' high. the area had origninally been bolted with 5' bolts, supplemented with 8' super bolts.
The battery power for UPS on the SOBO Brake system on the Man Hoist failed causing hoist to go into a fault. The UPS and power battery pack was replaced, and the hoist PLC system was reset and the hoist was operational. Hoist was down for 1 Hour 15 Mins. Mine was evacuated.
Temporary hoist outage. A resistor controlling the zero biasing for current on hoist motor went bad. The hoist motor kicked out as the third shift crew left the mine. First shift miners coming in to work did not go into the mine until the problem was repaired and the hoist was tested at 12:50 pm. MSHA field office at Newark was notified that morning.

High winds forced spon-com flame in face and floor of SE pit. This pit hadn't been actively mined for several weeks. No one was working in the pit at the time. No damage other than loss of coal.

All we know was lights started getting dim & bright. I told operator to get out & check out. He informed me that dozer was on fire. He used fire ext. on dozer & could not put out. I told him to get away. We covered dozer up & ext. with water & dirt. There were no injuries & I have no idea how much damage.

THE SECONDARY POWER SOURCE FOR THE #2 HOIST WAS BRIEFLY INTERRUPTED DUE TO A LOSS OF STEAM PRESSURE IN THE BOILER. THE EVACUATION OF MINERS WAS COMPLETE AT 5:10 AM THE STEAM PRESSURE AND POWER WAS RESTORED AT 5:25 AM AND MINERS RETURNED UNDERGROUND.

Livingston elevator is out of service due to a malfunction of dolly wheels on counter weight. Counter weight came free spinning around in shaft. Elevator will be returned to service after repairs and inspection of system.

An electrical storm interupted power by tripping a breaker. The miners were evacuated at 1:30 pm and on the surface at 1:43 pm. The electrical issue was resolved, the hoist was tested and miners returned underground at 1:50 pm.

Preliminary investigations indicate that residual heat from the kiln caused plastic residue to catch fire in the tower. The system possibly went positive forcing the flames into the CDF screw and weigh feeder. A complete investigation report when complete will be forwarded to the local MSHA field office.

While burning bolts off the Bunker Belt Head Roller at U/G Prod. Shaft Pocket (with torch), sparks from metal that was being cut ignited methane on top of the coal that was located in pocket. Ignition lasted 20 to 30 seconds & flame was extinguished with water. There was only 0.1% methane found prior to & during burning of the bolts. No property damage or injuries were incurred.

A roof fall occurred in the 4 West intake at crosscut #45 in entry #7. The cause of the fall was weak laminated shale. The fall blocked the primary escapeway for MMU-003. Production was suspended on the unit and the air course and escapeways were re-routed around the area.

THE DESKINS A SHAFT MANUAL HOIST WAS INOPERABLE DUE TO SLACK IN TRAILING CABLE.
At x-cut 62-63 in 4th East in the #5 entry we had a roof fall that was 8' high x 80' long. It isn't an escapeway or travel way. It is in the intake. All access has been dangered off and supported. It does not impede travel or impair ventilation. MSHA was notified on 07/05/05 at approx. 8:00 pm. The area had 5' resin bolts installed.

Power outage from 0115 to 0315 7-1-05 Friday morning in Mine due to heavy storm lost power from KY Utility. Evacuated to the Shop. Called in to Roger Rowe at 7-6-05 at 10:20 am and reported. Mining staff at time was unaware of immediate reporting requirement.
A roof fall was found in the main south supply road #5 entry between spad #31 + 50 & spad #31 + 95. The fall was 4' or 5' high, 18' wide & 40' long. 4' double lock roof bolts were used in this area.
Worker was fatally injured while driving a CAT 769C Water Truck along a standard mine haul road, returning to the water stand to re-fill the water tank. The water truck drifted left, onto the berm, travelling approximately 160' before ""cartwheeling"" and ejecting the worker. The worker does not appear to have been wearing the supplied seat belt.
Employee was caught in the tail pulley of feed conveyor and his hand & arm was pulled in until his chest contacted frame work. This caused him to die of asphixiation.
Intersection roof fall in above roof suport in the #4 entry/belt entry in the last open crosscut causing death of a bridge operator.
EE was moving scrap plate with come-long...grabbed one end of plate to pull thru manway & let it off the come-along and put on floor... went outside for fresh air & noticed pain in groin.
Employee was operating a dual boom bolter when he got his index finger on his right hand caught between a roof bolt plate and the mine roof as he was installing the bolt. This resulted in an amputation of the tip end of his finger.
Employee was installing chain hangers in low top. He was trying to push a roofbolt up through the resin cartridge when he suffered a ruptured hernia.

WHEN A PIECE OF DRAW ROCK SUDDENLY FELL MEASURING APPROX 30""X30""X3"" STRIKING HIM ON THE LEFT HAND, CAUSING A PARTIAL AMPUTATION TO THE DISTAL DIGIT OF THE LITTLE FINGER. THE DISTAL DIGIT WAS SURGICALLY REMOVED LATER THAT DAY
The employee was starting to drill when a piece of rock fell from the roof, striking the small finger of his left hand, and cutting the tip of it off completely. Per phone call, the original injury did not cut the bone, but the Dr had to amputate just below the fingernail to graft skin around severed part of finger.
Employee was on headgate end of shearer at #90 shield while the machine was backing up toward the headgate cutting up rock between the drums. The spray bar sprung out over the cable though and his right little finger was caught between the handrail and end of spray bar. Diagnosed as a fracture.
Employee alleges he was cutting off plastic fitting and the pocket knife he was using slipped and cut the tip of his right index finger off.
SETTING PANEL IN CORE VESSEL, HOOK SLIPPED, PANEL FELL STRIKING EMPLOYEE'S RIGHT FOOT.
Injured was preparing to set timbers where a rib had rolled out and effected rib support for a length of 3 pins. Once material had been cleaned up, individual was waiting for helper to measure , He was standing outby along supported rib. rib behind injured rolled off knocking him under loader and falling on rt leg below knee It was determined it needed to be amputated.
Monday morning (7/25/05) Employee was dispatched to another site and showed up complaing that he must have got a hernia on saturday at work. He went to the doctor and the doctor confirmed he did have a hernia and needed surgery. On saturday employee asked for help from laborer in lifting some but not all items. Did not complain that day of any pain.
Employee was cleaning out a chute during a system clean down, the employee reached into a chute to remove material inside the chute and his glove caught the rotating equipment pulling it to a pinch point. The distance of the employee's reach was 12-18 inches. This is not a standard procedure for the clean down.
Employee was lifting 3 1/2"" pipe (rigid conduit) when he strained his stomach. The injury was reported but follow up was not done till 9/19/2005. At that point employee had been laid off effective date 8/26/2005.
EE was positioning a tail pulley into place when his hand was caught underneath the pulley. The pulley crushed his left hand severing his left hand ring finger from the upper knuckle to the tip of his finger.

STRUCK EMPLOYEE'S RIGHT HAND JUST BEHIND KNUCKLES - RESTING ON RIB PROTECTOR - AMPUTATION OF RIGHT RING FINGER.
Employee states that while shoveling, he felt pull in his stomach region.
Tightening a spring bolt and was using an air wrench, slipped and cut some of his pinky finger off.

Employee was collaring a hole in the back when a rock fell. It slid down the jackleg steel and fell on his right foot. The foot was fractured from the impact.
During dragline repair, employee was lifting a metal house roller pin, strained abdomen. Later determined to be abdominal hernia. Did not loose time until 8/29/05 for hernia repair.
In the process of attempting to remove a drill bit on a secondary drill, the employee got caught up in a rotating drill steel.
The employee was walking into the Longwall panel when he stepped on a rock on the side of the rib. His foot went out from under him and he fell. He was taken to seek medical attention and it was found that he had a broken left leg just above the ankle.
Operator was being loaded at a shovel when a rock fell off of the shovel dipper and struck his truck.

Employee reported right knee pain while jackleg bolting on 7/30/2005. Injury considered FA strain at that time. On April 3, 2006, employee went off work for right knee surgery. Physican related knee problem to FA injury on 7/30/2005.
Employee was performing cleaning duties with a shovel at conveyor F. He was working with other employee. Accidentally his left arm was pinched between the belt and a steel structure. The conveyor was unguarded.
Tightening nut on bolt & socket slipped of nut ee then scraped neck on chute at wash plant.

EE stated that he threw a track switch to let a man bus go by. He was standing by switch when another manbus came through the switch. the handle of the switch came over and hit the ee in the left foot, fracturing a toe. First lost work day was 8/2/05
Submitted to comply with Part 50.20 30 CFR. EE was operating bulldozer in a hollow fill reclaiming. EE stated he was hurting all over & was sick. Taken to hospital; diagnosed as viral infection. At a later date 2nd physician diagnosed degenerative joint disease (arthritis). EE informed Dr. that he was jarred by the dozer & was work related. Invest. ongoing.
Strained right ankle. Contributing factor uneven ground twisted ankle!
EE was working in the prep plant shop and when he lifted an electrical breaker box, he felt a sharp pain in his lower back.

Employee was pulling on approximately 200 feet of pipe on floaters, with a rope, on the lake.
A piece of coal fell over the rib screen, hitting the ee on the left knee, re-injured the knee he had hurt at home the week before
The employee stated that while he was in the process of roof bolting procedures. He was pulling on some drill steel that was stuck int he drilled hole of the roof. The drill steel broke, throwing him off balance twisting his right knee. The employee stated that he hurt this same knee in a car wreck in the year 2000 but never could afford to seek medical attention at that time.
Employee was shoveling #10 belt tailpiece. He turned belt off to cross over tailpiece. As he put his right leg up on tailpiece, his left knee twisted. This employee has a history of unusual injuries.

Employee was pulling a piece of granite curb on the rollers when it fell onto its side. Employee jumped back and ""wrenched"" his hand (strained left wrist). (NOTE: Employee returned to work next day so I did not file MSHA. Noticed my oversight now, when doing my year-end summaries).

The employee was removing material buildup from the walls of the Pit hopper from a standing position above the hopper. He was using a hollow bar about 11' long and weighing about 25 lbs. He performed about 6 downward thrusts with the bar striking the hopper walls and when he continued the movement a 7th time, he felt pain in his lower back.

Injured was bolting top when a rib rolled striking him on the right ankle fracturing his ankle.
Lifting pump casing-casing struck right knee-had pain and swelling no time lost and no restriction-rechecked because renewed pain-mri showed a tear in muscle went off work on 9-22-05 to have tear repaired.
KNEE ALWAYS HURTS. WHEN EMPLOYEE WAS COMING DOWN LADDER, HE STEPPED ON GROUND FROM LADDER AND KNEE FELT LIKE IT WOULD GIVE OUT.
EE WAS CARRYING AN 8 LB HAMMER FROM HIS TRUCK TO THE MACHINE, WHEN HE SET THE HAMMER DOWN HE FELT A SHARP PAIN IN HIS CHEST AND LEFT ARM. HE HAD TROUBLE BREATHING AND WAS IN SEVERE PAIN. NOT KNOWING IF HE WAS SUFFERING FROM A HEART PROBLEM WE CALLED AN AMBULANCE AND THEY TRANSPORTED HIM TO A LOCAL HOSPITAL. DIAGNOSED WITH A PULLED CHEST MUSCLE AND SOME TORN BLOOD VESSELS
Employees were working on 8"" flyash line to unclog it. System was shut down at time. When the removed 2"" cap to insert air lance, material flushed out striking employee in face. Safety glasses were worn, but dust flow blew them off. No serious or permanent injury. System has been redesign to reduce clogs and all caps now have valves to allow controlled opening.
EMPLOYEE WAS ON KNEE'S SHOVELING COAL AND TWISTED KNEE.
Employee was bolting roof top and a rock fell striking his left hand pinky finger and mashing it against pinner.
D-11 Carry Dozer was being taken down rear road to a Low Boy when it overheated and was believed to be low of water. Employee took cap off radiator and his right side was dowsed in anti-freeze.
Ee was measuring product silos: he stepped onto a bin door and twisted his back when he moved backward off the door. The door is approx. 3"" high. EE was taken to community hospital for evaluation. Initial diagnosis was muscle spasms.
Employee claims that he was shoveling the collection belt and felt a pain in his lower back. Strained lower back muscles.
Picking up a compressor motor - strained low back.
Employee said that while operating the bag machine, his right shoulder became more painful from an old work-related injury. He felt burning pain & stiffness in it whenever he had to raise it overhead to reach an empty bag. Finally became so painful tht he could not push empty bag onto the machine's filler spout on his right side. Had torn rotator cuff surgery on 8/26/05.
Employee was leaning over a wheelbarrow cleaning next to a tall pallet of bricks when one of four bricks fell from top of stack & struck him in lower back.
The emp. was on the inby side of the #50 motor in the process of chaining down the motor. The motor was contacted by another motor, caused #50 motor to knock the injured emp. to the ground.
Employee was changing bags in the dust collectors and the repetitive motion of using a screwdriver and ratchet may have caused the swelling. Note: Lost time began 8/26/05.

Employee was leaving work in his personal vehicle. He was involved in a two vehicle incident involving a coal truck. Employee was diagnosed with whiplash.

While washing the outside of the cable shop with a water hose, he felt something crawling on his head below the rim of the hard hat. Attempting to remove the bug, he felt something ""get him"". He was stung by a wasp in the webbing next to the pinky finger on his right hand. He was taken to the ER, given meds for swelling & infection. Unable to return his next shift due to swelling.
EE stepped onto an uneven walking surface and felt a ""pop"" in his right knee. Taken off of work by the plant physician on 07/28/05.
Putting metal in shoot at quarry, when it came loose it jarred his arm, causing a knot 8/8/05. Went to doctor 8/15/05 returned to work 8/22/05.
THE EMPLOYEE STATED HE WAS MOVING A ROLL OF 3"" PLASTIC WATER LINE OUT OF THE ROADWAY WHEN IT GOT CAUGHT ON THE ROOF. HE LOOKED UP TO SEE WHAT THE LINE HIT AND A PIECE OF ROCK DISLODGED AND STRUCK HIM ON THE NOSE. THE ROCK WAS 7""X9""X1"". HE SUSTAINED A SCRATCH ON HIS NOSE. THIS IS BEING REPORTED TO MSHA TO ABATE CITATION 7244154 WHICH THE OPERATOR INTENDS TO CONTEST
The employee has been experiencing back pain. An MRI performed on 6/6 reported disc degeneration of the L2-3 and L3-4 and L5-6. The employee did not return to work after doctor appointment on 10/5/05.
On 3-5-05 was moving 55 gallon barrel - felt pain in lower back. Pain reoccurred on 7-27-05.
Shoveling out elevator full of product; product was 370F. Employee's feet would get hot and he would stop shoveling for awhile. Didn't notice anything except a red big toe until several days later when his feet started to hurt and he noticed blisters on little toes.
Employee was building a block stopping ventilation control. He was cutting a block to size by using a hatchet and striking it with a hammer. While striking the 2 metal tools together, a chip of metal broke off hammer and lodged in his left eye. He was not wearing eye protection.
The employee stated that he had been on the operator's side repairing the dust collection hose. He had returned to the side of the roof bolter to return a screwdriver to his dinner bucket. While standing approx 1' outby the rear at his bucket, a rock from the top fell from between the bolts striking him in the head and forcing head into the roof bolter.
A bag of material had fallen off the pallet and he reached down to pick it up. as he extended his arm and pushed up the bag he felt a pop in his right shoulder.
EE WAS OPERATING CMO IN NO.5 RT CROSS CUT. HE LOADED SHUTTLE CARS AND WAS IN PROCESS OF GETTING CABLE IN PLACE FOR REMAINDER OF CUT. AS HE BENT DOWN TO MOVE CABLE A ROCK FELL FROM MINING ROOF PINNING HIM TO MINE FLOOR. ROCK MEASURED 80"" X 24"" X 16"". ROCK FELL BETWEEN RT RIB AND MIDDLE ROW OF BOLTS.
He was assisting miner shotcreting an intersection, and machine blew back dry shot crete and injured received dry shot crete in both eyes.
EE operating defective equipment (steering), operating track mounted machine on solid stone, and lifting heavy solid outrigger pad, all contributed to lower back strain
Employee was driving an on-site fuel truck, after re-fueling the dozer, he begins to drive down a declining haul road. He claims brakes failed to slow down truck. He decided to jump from the moving truck, resulting in a laceration to his head. The brakes were inspected and found to be in working order. Cause of accident- excessive speed.
Employee was jacking gob car back on slope track when the jack jumped out striking his left knee. Employee's first lost day did not occur until 9/29/05 due to surgery.
EE was dismounting from the back counterweight step. He had positioned a 6' folding ladder 26"" from the step to assist. As he stepped with his left foot onto the counterweight step, then stepped with his right foot facing the ladder, his left foot slipped through the step opening, which measured 5"" X 11"". He then fell spun around ending upside down, hunging by his leg.

EMPLOYEE WAS GOING UP THE STAIRS FROM THE THIRD FLOOR TO THE FOURTH FLOOR. THERE WAS AN EXTENSION CORD BY THE STEPS GOING UP TO THE FOURTH FLOOR. EE MOVED THE EXTENSION CORD. THERE WAS A DAMAGED PLACE ON THE CORD. HE WAS BURNED ON HIS LEFT HAND.
Accident not reported until 8-10-05. Employee was lifting bags of rock dust when he pulled something in his shoulder. Right
Employee was fueling his dump truck at our Boulder Hill pit in Oak Harbor, WA. While fueling, the employee was stung by a bee and passed out.
Injured employee was shoveling material from under equipment and was twisting while bent at the waist. Caused strain and disc injury to lower back. Employee instructed not to twist but to lift feet and turn when shoveling.
Operator was coming down ladder off an off-the-road truck when he apparently lost his footing and sprained his left ankle. Continued to work until about 9:00 pm when it started hurting and he went by the E.R. where ace bandage was applied. He is to wear wrap 48 hours, apply ice, elevation and use crutches until 7/29/05.
Walking on uneven ground, employee slipped and fell, injuring his back.
At the 3EP filter bank man slipped in mud and twisted left knee. Started missing 10-11-05.
Employee was welding overhead and got something in his eye (left).
While the employee was hanging up tubing for a fan, the wire struck him in the abdomen. After a few days it was showing signs of infection, he was taken to seek medical attention and was put on lost time until the infection could be identified.
CHANGING ROLLERS OUT ON #1 BELT. EMPLOYEE WAS REMOVING CLIP TO REPLACE ROLLER WITH SCREW DRIVER, CLIP FLEW OUT OF BELT STRUCTURE AND HIT HIM IN RIGHT EYE.
While walking to get a knife, employee back hit the head of a cable bolt installed in the roof.
EE WAS CLIMBING ON TO A TRACK MOUNTED ROCK BREAKER FOR THE FUELING/LUBE PROCESS AND STRUCK THE TOP OF HIS LEFT FOOT ON THE BOTTOM OF THE LADDER RUNG. X-RAY AT THE TIME SHOWED NOTHING BROKEN AND ALL OK. ON 8/26/05 HE WENT TO A CHIROPRACTOR FOR AN ADJUSTMENT ON FOOT. EE was diagnosed with a broken foot on 9-21-05. He had surgery. Dr released him to lite duty on 9-30-05.
EE stated that he and other ee were loading 48"" belt conveyor structure (rollers) into the back of a mantrip. His left hand was caught between the belt conveyor structure and the canopy of the mantrip, as Other ee soved the belt structure into the mantrip. EE suffered a fracture and laceration to his left middle finger. The laceration required 4 sutures to close.
A rock was stuck in the grid of our hopper. EE tried to break it with sledge hammer. Did not work. He then tried pulling on rock to push off to one side. Felt something snap in shoulder and forearm area.
EE: 2W Unit - A piece of rock 2"" thick by 2' by 2' fell out of top and hit employee on top of the head, neck, left shoulder, and left thumb. Complaining of lower back pain a day later also.
EE 2W Unit - Employee was helping a co-worker put cable bolt in hole. The glue was starting to set up. The bolt sprung and the roof plate struck employee on the right forearm causing a puncture/laceration wound requiring 9 sutures.
EE was operating roof bolter when a kettle bottom fell on canopy and slid off striking his right shoulder.
Two employees that were not paying attention to the people or the area surrounding them.
Employee was in T-75 and was under shovel getting loaded. A rock came off shovel and hit headache rack. Employee complained about neck pain.
EE was installing a cable bolt, positioning the bolt plate so it would not hit T-bar. He got his right index finger pinched between the plate and the roof.

Concrete block fell on his left foot. While trying to get his foot out from under block, strained his left knee while twisting and pulling foot.

EE WAS SCALLING A BROW WHEN LOOSE FELL ON BOOM OF SCALER BOUNCING OPERATOR INSIDE CAB. HE HIS HIS HEAD (HARD HAT) ON ROOF AND HARD HAD CUT RIGHT SIDE OF HEAD. HE ALSO FELT PAIN IN LOWER BACK. TAKEN TO DOCTOR ON 8/22/05. DUE TO RESTRICTIONS FROM DOCTOR, COULD NOT RETURN TO WORK ON 8/23/05 AND BEGAN LOOSING TIME
EE WAS ON THE OPPOSITE OF THE OPERATOR SIDE OF A DOUBLE BOOM. THE OPERATOR WAS HAVING TROUBLE ADVANCING THE BOOM DUE TO AN UNEVEN BOTTOM. THE OPERATOR STOPPED THE BOOM AND THE EMPLOYEE STEPPED IN BETWEEN THE WHEELS TO GET SOME NAILS. WHEN THE OPERATOR BACKED UP, HE RAN OVER EMPLOYEE'S FOOT, DISLOCATING HIS ANKLE AND KNOCKED HIM DOWN CAUSING A FRACTURE TO FOREARM.
Employee was taking a scoop off charge in the #5 entry on the Mains Section and stated that he twisted his right knee in the mud at the scoop charger station.

Ee reported: attempting to back a continuous miner from the face area. The machine suddenly moved to the right, his reaction was jerking his head backwards. Continued to experience pain and discomfort to his neck. Late August received medical treatment for the pain. Dianoised as degenerative disc diease in his neck and also arthritis present along his spine. Started missing
As employee was decending the steps on the rubber tire dozer he twisted his left knee and fell to the ground.

Employee was operating the #18 roof bolter at the face of crosscut #3 to #2 entry in unit #2. He was installing a 4' roof bolt when it bent as he was pushing it up in the roof using his drill pot. He then tried to pull or tug on the bolt by hand to pull it out of the roof so a new bolt could be installed to replace it. At this time he felt a pull in his lower back near his belt.
Lifting two 5 gal cans of oil off trailer & felt sharp pain in lower back
Employee was helping thread belt through a tailpiece. He was pulling on belt with pair of channel locks when they slipped off of belt and struck him in the ribs.
Employee trying to move Slot Drill over w/a bar. The Slot Drill dropped a few inches and the bar came down on his foot.
Employee was attempting to get a bent bolt out of the roof and he accidently hit the rotation when he slipped on a rock. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner proficiency a factor, mining equipment/systems could be a factor.
Employee was shoveling underneath a conveyer beltline and reported that he felt sharp pain in his back when he twisted to empty his shovel.
A technician experienced discomfort in his abdomen over a several month period. A congenital weakness of the navel area required surgery to repair.

Employee was waiting for shuttle car to pass when car bumped tubing. Concerned it may fall, employee moved quickly, slipped and fell catching himself with right hand. Injured felt pain in right shoulder.
Supervisor was walking down the slope for examination and stepped on the track rail losing his balance twisting his knee and ankle. Cause of this injury was awarness. Compliance with rules/regs was not a factor, mining equip/systems not a factor, personnel safety equip not a factor, miner attitude could be a factor.
Emp was attempting to push rocks off of the belt and strained shoulder muscle.
Employee claimed that he hurt his knee on Saturday 07/23/05 but didn't tell his foreman until Monday 07/25/05.
EE WAS LOADING HIGHLINE CABLE INTO SCOOP BUCKET AND PUT HIS FOOT UNDER THE SCOOP BUCKET. THE SCOOP OPERATOR LET THE BUCKET DOWN ON EE'S FOOT AND BROKE THE 3 MIDDLE TOES ON HIS RIGHT FOOT.
EE WAS GETTING ON A HITACHI BACKHOE WHEN HE SLIPPED AND FELL ON HIS LEFT ELBOW

THE CONTINUOUS MINER CREW WAS ADVANCING POWER FOR THE SECTION. THE EMPLOYEE WAS PULLING A CABLE THROUGH A HOLE IN A STOPPING. A CEMENT BLOCK FELL AND STRUCK HIM ON THE LEFT FOOT
3F-T NORTH TRACK ENTRY. MAN WAS PUTTING TOOLS AWAY ON THE END OF TRACK JEEP, A PORTAL BUS COASTED TOWARD HIM HITTING RIGHT LEG
Employee was performing normal duties. Brass coating on forks of forklift was damaged. He was repairing when incident occurred. Employee rotated the forks to the vertical position and was using a hammer to knock the brass coating back into place. The pin holding the top fork came out causing that fork to fall and catch employee's finger between the two forks.
The employee was connecting a special pole in the sub-station of the secondary. The pole rebounded and struck the right part of his face. He had a small cut in his mouth.
While employee was dismounting a grader his foot became hung in the last step of the chain steps resulting in a fall to the ground. He was facing away from the machine when he dismounted. His injuries included a massive bruise on his upper leg, several superficial scrapes on his elbow and a couple small fractures in his pelvis bone.
HE WAS PULLING WIRES THROUGH THE LIGHT FIXTURES AND HIS HAND SLIPPED AND CAUGHT THE SIDE OF THE LIGHT FIXTURE
EMPLOYEE WAS OPERATING #4 TEREX PAN. THEY WERE STARTING A BENCH BY DROPPING REFUSE TO BUILD A NEW ROAD. AS HE STARTED TO UNLOAD REFUSE MATERIAL THE BACK TIRE OF THE PAN SLID OFF THE REFUSE & THE PAN ROLLED OVER.
Employee injured by falling stone from overflow of surge bin while installing chute above 4 1/4 chute. (Belt was operating. Multiple stones fell, but EE was not buried.)
Employee was working on the 513 dust collector and fine dust fell on him from up above causing his throat to be irritated.

The employee was injured while assisting with a conveyor reconstruction project. He was in the process of pulling electric cable from a workboat onto the catwalk of a floating conveyor when he strained his lower back.
EE was moving and relocating roof bolter and canopy hit his left shoulder and neck. Worked remainder of shift and 2 additional days; last day worked 7/25/05.
The part employee was working with was on a pallet. The employee stepped on the pallet and it broke causing him to twist his knee. The employee started losing time due to his injury on August 3, 2005.
EE COMPLAINED OF BACK PAIN ON 7/24/05 FROM THE SEAT IN HIS HAUL TRUCK BOTTOMING SEVERAL TIMES DURING THE SHIFT. HE CALLED OFF FOLLOWING SHIFT BUT GAVE NO REASON. INJURY DATE 7/22/05. THIS SEAT WAS IN THE TRUCK FOR 3 WEEKS AND WAS CHANGED OUT AFTER HIS INJURY. MANUFACTURER FOUND NO REASON FOR IT TO BE BOTTOMING OUT
Employee having trouble with lights on ram car. He looked down at switch and ran into the mine rib. Whiplash type injury to neck and back. Started losing time 7-23-05.
Employee is unsure of his activity that resulted in a hernia.This incident was reported to Management on 8/1/05. Surgery is scheduled for 8/19/05.
Employee stated that while operating shuttle car he hit a hole in the roadway and bumped his head on the canopy.

EE WAS FINISHING UP PRESSURE WASHING AT THE AGGREGATE PLANT AND WAS WINDING UP THE PRESSURE HOSE. HE MOVED UP AGAINST THE SINGLE AXLE TAG ALONG TRAILER AND THE JACK FOR THE TRAILER FELL OFF AND THE BALL HITCH FELL ON HIS FOOT. THE JACK WAS NOT SECURELY FASTENED TO THE TRAILER PROPERLY
Employee was pushing a wheelbarrow up a ramp to dump into the bottom ash silo. While ascending the ramp, the axle slid out of the bearing, the wheel barrow stopped suddenly, and the employee jammed his arm/elbow. This action caused a hairline fracture to the employees left radial head (elbow).
EMPLOYEE WAS INSTALLING A SPOT BOLT ON LEFT CORNER OF #4 ENTRY. WHEN HE LET THE ATRS DOWN A PIECE OF ROCK FELL OFF OF THE ATRS AND STRUCK HIM ON THE TOP OF HIS HARD HAT. HE WAS HURTING ON HIS NECK.
While the employee was walking towards the miner he slipped on wet ground and twisted his left knee. He is still experiencing some pain in his knee so he is scheduling surgery.

Employee was fueling the fuel truck from the stationary fuel tank when he stepped from the block wall into the truck and slipped and hit his right leg. Thought he would be o.k. A few days later his leg started to swell. He had received a hematoma/contusion to his right lower leg.
While roof bolting, a rock fell from the mine roof & came off the bolter's canopy & struck employee in his lower back.
As he went to step up on the gen set platform, he lost his footing and fell. When he fell he put his arm out to catch himself.
The emp. was on top of #3 belt removing a section of bagged down screen wire. A rock slid onto belt from the bagged down area. Emp. was diagnosed on 7/22/05 with hernia. Scheduled surgery 8/1/05 - 1st day missed 8/1/05.
EE WAS OPERATING MOBILE VIBRATOR ON THE NEW PARKING LOT BEHIND THE SHOP. THE NEXT DAY HE MADE COMMENTS OF HIS BACK BEING STIFF. ON 08/08/05, HE COMPLAINED ABOUT HIS BACK AND WENT TO HIS DR ABOUT THE BACK PAIN. DR RESTRICTED HIM FROM WORK UNTIL THE 17TH OR UNTIL THE NEXT CHECK-UP.
EE was injured while riding a mantrip. While passing a disabled mantrip, He reached out of the mantrip he was riding to get a SCSR on the disabled mantrip. He became caught between the two mantrips.
Walking up face, he tripped over shield to shield hose, fell down and twisted right knee.
Person was helping to get belt discharge pulley through 6' door in stopping. The pulley moved unexpectedly and person lost balance and fell backwards. Person put his left land out to catch himself and this caused injury to left shoulder. Person is off for surgery to left shoulder as of 9/21/05.

GOOCH, ORA (LTA) : 2W Unit - Employee was trying to remove rock, between telebody and push plate jack. It was too large to move, so employee used a sledge hammer to break the rock. While striking the rock with the hammer, employee hit the rock with the handle where hands were located before hammer head hit the rock. Fracture to left 5th digit.
Employee was putting things away on his mechanic's truck; he placed an oil cooler on the truck; as he turned to walk away, the cooler fell off the truck and caught him on his lower back.
Crew shut down because of bad weather. They had waited 45 min for storm to pass but it showed no indication of stopping so the helper got out of work truck and went to the rig & turned off the master disconnect. As he was getting back into the work truck, lightning struck the work truck. Helper's right arm was hurt along with burn blisters on both feet.

Employee stated that he was in a previously supported area spotting a roof bolt when a piece of drawrock approx 1' x 3' x 4"" thick fell and struck him in the back. He was transported to the hospital where he was x-rayed and diagnosed with a bruised back.
Employee was operating a saw when a rock fell into the cut. The belt then flipped the rock back up striking the employee in the face, left cheek. He was wearing required PPE.
EE claims that the brakes failed at the top of the slope leading into the pit. EE was on his way to get his 9th load of the night. EE had conducted pre-shift on the haul unit. The vehicle str4uck the highwall as he attempted to negotiate a curve and ramp leading out of the pit.EE received a laceration to left forehead area and was hospitalized.

While lifting door of vibrator into place, door slipped and injured employee. Employee experienced abrasion to right hand and strain to right wrist.
Employee suffered a back injury while operating a dump truck. He ran through some ruts on a haul road when he was tossed from side to side. He stated that he was wearing the seat belt provided and the upper portion of his body was what moved back and forth. Additionally, he stated that he has had two major back operations which could also be another factor.
EE was attempting to lift high voltage cable into a sled when he began having trouble seeing out of his right eye.
RATCHET SOCKET PIPE, ATTEMPTING TO TURN MOTOR OVER WITH A PIPE. FRACTURED VERTEBRAE, TO BE FURTHER EVALUATED
While installing a roof bolt, a piece of draw rock fell striking employee in the nose. His reaction to the falling material caused him to pull his head back, striking his head against the roof bolter canopy causing a neck strain. Employee started missing work on 7/25/2005.
EMPLOYEE WAS PICKING UP A REMOTE CONTROL BOX FOR THE CONTINUOUS MINER WHEN A ROCK FELL OVERHEAD, STRIKING HIS RT. HAND CAUSING A LACERATION.
Employee was prying on a victualic coupling with a wrench in order to line it up so it could be tightened on the pipe. The wrench slipped, rotated around and struck the employee in the mouth.
Employee stated that he was roof bolting & caught his left index finger between the drill steel and the roof bolting wrench. He was transported to the hospital where it was determined that his finger was broken & excessive tissue required surgery to repair the damage.
EE alleges that while moving cars he twisted to set the brake and felt a pain in his lower back.
THE COMPANY FEELS THAT THIS IS NOT A REPORTABLE ACCIDENT & IT'S BEING SUBMIITED ONLY TO TERMINATE CITATION # 7249956 WHICH WILL BE CONFERENCED. EE MISSED WORK DUE TO AN UNRELATED TUMOR THAT WAS DISCOVERED. He was struck by a rock that fell out of the mine roof as he was walking away from the miner.
Employee did not report injury until 7/21/05. Employee slipped while walking through the processing area causing water and lime to get inside his steel toe rubber boots. Employee cleaned out boot, and returned to work.
Employee, was operating a continuous miner in the #3 entry crosscut when a piece of draw rock measuring 2 feet by 4 feet by 5 inches fell from an area of the mine roof near the coal rib striking him on the left shoulder and arm. This resulted in the employee's wrist being fractured.

Driving down hill Sweeper gained speed attempting to brake, brakes failed. Used Ebrake, operator's adjustment of handle being too loose. When fail to stop, drove into hillside. Seatbelt in use. Driver hit forehead on steering wheel, knee on column. Minor front end damage to sweeper.
Injured was handling cable when he got hand caught between cable and bucket of scoop.
Employee was driving the forklift and a foreign substance (either dust or a rock) entered his right eye causing a corneal abrasion.
EE was operating a 785 when the loader loading his truck hit his hauler with its bucket, jarring him.
Employee was working in the Battery Barn shop and was walking in a hurry toward a Ramcar Battery Charging. He stepped on a baseball size rock and bent his toes backwards.
Employee was in haul truck and the loader operator put a load of material in the back of the haul truck. The load had a big boulder that bounced the employee while he was in the haul truck hurting his back.
Employee was operating miner on the 002-0 south section, was taking a lift cut in the #1 gob eye when a rib roll struck employee on the back of his left leg knocking him down. Employee sustained fracture to the tibia bone just above the ankle.
While operator was running the dozer, the air conditioner vent came loose and fell down. The operator attempted to replace the vent cover without turning off the air conditioner and his hand hit the fan blade which is about five inches inside the unit.
Employee was cleaning build-up from pulley on conveyer tripper. The bar he was using slipped causing employee to lose balance and his face hit frame of tripper.
Member had completed installing an 8' cable bolt. As he started around the roof bolter he struck his head on the bolt causing a pain in lower back.
Injured leaned over to try and locate an oil leak under the scoop. Injured instructed the scoop operator to turn on the scoop. When he did he pulled back on the ejector jack which caught the injured's fingers between the scoop blade and the ejector blade.
EE WAS BACKING THE MINER OUT OF ENTRY #5 TO RE-ROUTE THE CABLE TO THE NEXT PLACE. HE WALKED UP TO THE MINER BESIDE HIM TO THROW THE CABLE OUT OF THE WAY AND A ROCK SLID OFF OF THE MINER LANDING ONTOP OF RIGHT BOOT PUSHING THE METATARSAL AGAINST THE TOP OF HIS FOOT. THE ROCK PINNED HIS FOOT TO THE MINE FLOOR UNTIL HELP ARRIVED TO REMOVE THE ROCK
CLEANING OUT MUD TANK, CHEMICALS INSIDE GOT IN BOOTS & CAUSED BURNS ON LEGS WHILE USING HOSES ON VACUUM TRUCK.
While turning a wheel valve, employee caught his right hand's ring finger. Employee continued working. Employee did not require medical treatment until July 26. Employee was able to return to work and his job functions allow him to continue working without violating his issued work restrictions. Treating physician did not immobilize the finger until August 3.
Employee was picking up a rock and sprained lower back and neck.
employee was turning a piece of 1"" plate over to gring off the cutting torch slag when he stepped back on a power cord causing his foot to slip. he lost his lifting momentum causing the weight of the plate to come back at him at which that point he lowered the plate.
Injured miner was hanging miner cable and pulled something in his shoulder.
Cut about a 6 inch bush which had a grapevine growing from it in the ground which was unseen and it knocked him down and landed on a rock which cut his head.
The injured employee was helping spot/align railcars for unloading. The employee assumed the car were moving too fast and tried to jump up onto the railcar to set the braking system. As he attempted to get onto the railcar he was either unable to climb up fast enough and/or slipped resulting in the wheel of the railcar running over his right toes.
The injured employee was operating a mobile bridge unit at the time the incident occurred. The canopy of the bridge unit caught a roof bolt and stopped suddenly, jolting the employee. (Employee notified foreman on 7/21/2005 of injury.)
EE CAME INTO MY OFFICE THIS MORNING & TOLD ME HE HAD STRAINED HIS BACK YESTERDAY (7/14/05) WHILE STACKING PALLETS. EE WAS SENT TO THE DOCTOR FOR AN EXAMINATION IMMEDIATELY & DOCTOR HAS RESTRICTED HIS ACTIVITY TO NO KNEELING, SQUATING, BENDING, STOOPING, PUSHING, PULLING OR PICKING UP MORE THAN 10 LBS. THE DOCTOR WILL RE-EVALUATE HIM ON MONDAY (7/18/05).
Ee was installing a curtain board to the roof in an intersection between the #3 #4 entries on the operators side of the roofbolter. He drilled the hole with c piece of 3' starter drill steel and was bringing the steel back down out of the hole when it fowed and bowed striking his arm causing a contusion.
EE was adjusting the brakes on the rock truck. was pushing wrench with his foot and his knee popped.
EE REPORTED: HE & COWORKER WERE DISSASEMBLING BELT STRUCTURE FROM OLD NO. 3 BELT ENTRY. THE COWORKER HAD REMOVED HIS END. WHEN THE VICTIM WAS ATTEMPTING TO LOOSEN THE OTHER END. COWORKER DROPPED HIS END, STRUCTURE RAIL MOVED SUDDENLY CATCHING VICTIM'S FINGER TIP AGAINST BELT STRUCTURE STAND. EXTENT OF INJURY WAS SIMPLE FRACTURE TO TIP OF RIGHT RING FINGER
Employee was fitting conveyer chain on miner. He was putting steel cable in chain when cable flew back and hit him in the eye.
Employee was tramming a shuttle car in the #7 entry on the #1 unit, heading in the direction of the continuous miner, when the shuttle car trammed over a piece of rock in the roadway causing him to strike his head on the cars canopy. This resulted in the employee sustaining a strain/sprain injury to his neck.
Tripped, and knee struck a rock on the ground near roadway.
Lost balance and fell off of step ladder landing on SCSR while doing standard procedures for survey crew rodman.
Employee was putting scoop on charge and twisted left knee.
EMPLOYEE WAS DISCONNECTING A DISCHARGE HOSE FROM A 6"" WATER PUMP. THE PUMP HAD BEEN RUNNING BUT NOT PUMPING & HAD BECOME VERY HOT, THE HOSE WAS FULL OF STEAM. WHEN THE LINE CAME LOOSE IT HIT THE EMPLOYEE CAUSING BURNS TO HIS RT. ARM & LEG.
EMPLOYEE ALLEGES HE WAS WORKING ON #1 BELT. AFTER COMPLETEING THE JOB HE WAS CARRYING TOOLS AND SUPPLIES AND MISSED LAST STEP COMING DOWN THE STAIRS THEN SLIPPED ON GRAVEL STRAINING HIS GROIN. **INJURY WAS ON 7/13/05 BUT HE DIDN'T SEEK ANY MEDICAL HELP UNTIL 8/12/05.
EMPLOYEE WAS INSTALLING A ROOF BOLT WHEN A PIECE OF ROOF FELL STRIKING THE CANOPY THEN THE LEFT SIDE HIS BACK RESULTING IN BRUISES AND SCRATCHES
WHILE OPERATING A CONTINUOUS MINER FROM A REMOTE CONTROL, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLT AND RIB AND STRUCK HIM IN THE BACK. IT RESULTED IN 3 FREACTURED VERTEBRAE. IT OCCURRED ON THE 005-0 SECTION. AT THIS POINT, SURGERY HAS NOT BEEN REQUIRED.
Employee was taking a drill steel apart with a pipe wrench and the wrench slipped off striking the employee in the face.
EE was inspecting a mobile vehicule at Carpenter's shop. While he was passing around the vehicle he rubbed a piece of glass that was in the cargo area causing him an open wound.
EMPLOYEE WAS USING A 12LB.HAMMER TO LOOSEN A NUT ON A CONE LINER WHEN A PIECE OF STEEL BROKE OFF & LODGED IN HIS LEFT LEG JUST AROUND THE KNEE.
While walking through the Weld Shop he tripped on a blown over welding screen. Strained right shoulder. He was treated and eventually ended with a surgical repair on 10/10/05.
While shoveling and raking aggregate material, employee stained his lower back.
EE was making a repair on the highwall miner when a blast was detonated. A piece of flyrock or a piece off of the equipment struck him on the right lower leg.
EE was cutting 1/2"" drip line on the side slope of Leach Pad #4 to remove carbon and scale from emitters. In the process, his knife slipped off the drip line and cut his right leg.
While the employee was repairing a piece of machinery with a hammer, he hit a piece of metal, which caused an injury to his fourth finger from the right of his left hand.
Injured was carrying pogo sticks to section when he tripped over firehose streched down belt.
The injured was helping set up a mobile roof support for retreat mining. He stepped over a pile of loose material to get a cable hanger. He stepped back to turn and tripped thus causing him to fall backwards resulting in a fracture to his left lower leg.
THE INJURED WAS POSITIONED ON A LADDER SETTING A ROOF SUPPORT JACK. (MINING HEIGHT APPROXIMATELY 12'). HE FINSIHED SETTING THE SUPPORT AND WAS DESCENDING THE LADDER WHEN HIS FOOT SLIPPED ON THE 3RD RUNG. HE LANDED ON THE MINE FLOOR AND TWISTED HIS KNEE.
INJURED STATED HE WAS GETTING INTO FORKLIFT, HIS BOOTS WERE MUDDY AND HE SLIPPED TWISTING & HITTING HIS LEFT KNEE ON FORKLIFT.
EMPLOYEE WAS FUELING GRADER (MG-6). WHILE PULLING HOSE FROM TANK, SLIPPED AND TWISTED HIS BACK.
After operating a 488 S&S scoop on a rough r/way member complained of a tingling sensation in his legs and arm. The next morning still having that tingling, he reported to the doctor and was told to remain away from work.
A shackle broke while pulling on the mobile tailpiece of the longwall with a ""miner puller"". A piece of the tow chain struck the employee.
THE EMPLOYEE (A MECHANIC) WAS WORKING ON HYDROSEEDER ENGINE WHILE IT WAS RUNNING. THE WRENCH SLIPPED AND HIS HAND HIT THE FAN. IT BROKE 2 FINGERS AND HE RECEIVED A CUT ABOVE THE KNUCKLE.
Reported to work prior to start time. Went to unit 26 truck; placed lunch box on seat, then started to go back to operations office for job assignment. While walking felt he twisted his right leg on uneven ground in front of the electric shop. Felt pinch in calf and upper leg. Reported the problem later in the day.
EE alleges that a rock fell from roof, striking his left ankle. It was also reported that it was a rib roll that hit his left ankle. A coworker was reported as a witness but informed management that he did not witness the injury. EE finished working his regular shift.
Employee was relocating a 2 piece, plywood & 2x4 form wood ramp for wheelbarrows used during a concrete pour. The ramp had been un-nailed/separated on one side. Employee thought both sides were un-nailed and attempted to move the ramp straining his right shoulder while picking upward on the ramp.
WHILE SPLITTING A PIECE OF SANDSTONE WITH A HAMMER AND CHISEL, EE SUSTAINED A SMALL PUNCTURE WOULD TO HIS LEFT ARM. HE REPORTE THE INCIDENT AND RECEIVED FIRST AID TREATMENT, BUT DECLINED FURTHER MEDICAL CARE. ON 25 JULY, HE WENT TO DOCTOR, WHERE THE FRAGMENT WAS SURGICALLY REMOVED AND 4 SUTURES TO CLOSE THE WOUND.
Employee slipped and fell while getting onto mantrip that was loaded with cans of oil causing pain in his left hip. He continued to work thru 7-19-05 when he sought medical attention and was taken off work.
HE WAS MOVING MINER, WHEN THE CABLE CAUGHT HIS FOOT AND TWISTED HIM AT THE KNEE. ** STARTED MISSING WORK **8-03-05**
EE was attempting to start the water truck used to keep dust down when he noticed a spider inside the cab. He grabbed a can of ether and used his own lighter to torch the spider the can exploded burning his hand and forearm.
Employee was working on the ""shooter feed"" cutting bolts with a welder when he sustained a burn to his right hand.
Employee was standing next to the #1 packer filling a supersack, He heard a pop and then the front of packer popped of and hit him in the head, ribs, and left wrist.
Crew was being lowered underground. The power trip caused cage to stop. One employee fell to the floor injuring his right knee. He returned to work with no problems. Three months later he complained of persistant right knee pain. On Dec. 7 he had surgery and on Dec. 8 it became a lost time incident.
EE WAS WALKING AROUND MANTRIP AND WASN'T PAYING ATTENTION TO THE GROUND AND TRIPPED OVER A ROCK ON THE GROUND AND TWISTED KNEE.
EE backed miner out of face of #4 entry, shut off machine and walked towards a roof bolter parked in x cut #4 x #3 when rock dislodged from borw at edge of x cut of #4 entry. Rock struck EE on the neck area and upper back.
Tripped and fell on left side, dislocated rib.
EMPLOYEE STATED THE DROVE HIS PERSONAL VEHICLE TO WORK AND PARKED IN THE EE PARKING AREA. THE EE ALLEGED THAT AFTER HE EXITED HIS PERSONAL VEHICLE, HE WALKED AROUND TO THE PASSENGER SIDE TO TAKE HIS LUNCH COOLER, WHEN HE NOTED SOME ANTS ON THE GROUND. IN TRYING TO AVOID THE ANTS THE EE ALLEGED THAT HE SLIPPED & PULLED HIS LEFT CALF MUSCLE.
After taking slurry sample, injured was exiting the slurry tank pit w/sample in left hand and right hand used for handrail. When he reached the top set of stairs, he fell forward striking his left knee on the bottom edge of the doorway cutting his knee open. He received stitches for his wound. Addendum: Leg that wasn't cut swelled up & EE admitted to hospital.
EMPLOYEE WAS HOOKING UP A DISCHARGE LINE ON A BARGE PUMP. HE HAD MUD ON HIS BOOTS. HE SLIPPED AS HE WAS STEPPING OFF OF THE PUMP BASE AND FELL ONTO HIS BACK AND BOTH ELBOWS. THE INCIDENT OCCURRED ON 7/11/05 BUT IT WAS NOT REPORTED UNTIL THE EVENING OF 7/26/05. AT THAT TIME THE EMPLOYEE WAS IN ER. HE WAS DISCHARGED FROM HOSPITAL ON 7/29/05 & RTW ON 8/1/05 MISSING 3 DAYS.
EMPLOYEE TRAVELING TO SURFACE OPERATING #8 MANTRIP. TROLLEY POLE CAME OFF OF WIRE AND CAUGHT HIS HAND BETWEEN POLE AND WIRE

Employee was removing bolts with a 1/2 inch drive long handled ratchet and, while pullling on the ratchet in an awkward position, strained his lower back.
Employee was moving bolt rollers and frames out of walkway and strained his lower back. Employee started missing work 7/25/05.
The employee was standing in a chute replacing bolts that hold the liner in, and slipped and fell approximately 3 feet.
CHANGING FLASHING ON CONVEYOR BELT WITH BODY TWISTED.
Alledged incident was to have occurred while this employee was getting his tool bag and he felt something pull in his left elbow. Cause of this injury was overexertion. Compliance with rules/regs not a factor, mining equip/systems not a factor, protective equip. not a factor, miner proficiency a factor.
While employee was using a hammer & a bar to remove the pins off the bucket of an excavator, he felt pain on his right elbow.
Loading holes for a blast, he was lifting a box from the truck and twisted his back.
EMPLOYEE WAS HELPING CHANGE OUT BELT STANDS ON NO.2 BELT & WHILE CARRYING A BELT STAND HE STEPPED IN A HOLE & TWISTED HIS RT. KNEE.
Employee injured his back when the scoop that he was operating hit a hole in the roadway. He did not start missing work until 7/18/2005.
