A ROOF FALL OCCURRED IN THE 7TH SOUTH, #11 ENTRY, #4 CROSSCUT. THE FALL WAS 18' LONG, 20' WIDE & 8' HIGH. THE FALL DIDN'T AFFECT TRAVEL, VENTILATION, DAMAGE EQUIPMENT OR CAUSE ANY INJURY. THE AREA WAS BOLTED WITH 6' & 8' POINT ANCHOR BOLTS.

AN UNINTENTIONAL ROOF FALL WAS FOUND IN THE OLD 2ND NORTH MAINS SECTION. THIS AREA IS AN INACTIVE PART OF THE MINE AND WAS COMPLETELY UNDER WATER. WATER WAS BEING PUMPED AND ROOF FALL WAS DISCOVERED. ROOF FALL WAS PROBABLY CAUSED BY WATER SATURATION.

A ROOF FALL WAS FOUND AT 1:20 AM ON 9/30/04 AT X-CUT 167 ENTRY #1 MAIN SOUTH. FALL WAS IN A 4-WAY INTERSECTION THAT WAS MINED MID-DECEMBER 1996, AND HAD BEEN SUPPORTED WITH 8' FULL RESIN BOLTS. FALL WAS 20' WIDE X 20' LONG X 20' HIGH. NO INJURY OR PROPERTY DAMAGED DUE TO FALL.

A roof fall occured in the 6 North Panel at crosscut #46 starting in the pillar line and ending at the edge of the pillar line of crosscut #48 in the #9 entry. The fall measured approximately 90' X 19' X 5'. The area was supported with 5' fully grouted roof bolts. The fall was caused by an undetected slip.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #1 ENTRY OF B-MAINS AT STA. #1755. IT WAS IN A RETURN ENTRY. THE FALL WAS 20' LONG X 20' WIDE AND 6' HIGH. VENTILATION WAS NOT INTERRUPTED. NO MEN OR EQUIPMENT WERE INVOLVED.

Fall occurred in an intersection, caught an old cont miner boom, no injuries, no blockage of ventilation or escapeway, plan posted to remove miner boom- fall breakered off and dangered, plotted on mine map. Fall will not be cleaned up.

A ROOF FALL OCCURRED IN #1 ENTRY OF THE MAIN SOUTH EAST @ SURVEY STATION 7 + 70. THE FALL MEASURED 90' LONG X 18' WIDE X 9' THICK. THIS AREA HAD BEEN SUPPORTED WITH 9' BOUBLE LOCK AND 6' FULLY GROUTED RESIN BOLTS. ALL APPROACHES HAVE BEEN TIMBERED.

Incoder driveline coupling came apart. Fixed Maintenance mechanics repaired the coupling, observed hoist operation, released the hoist back to service. Hoistman resynced the hoist and continued operation.

A roof fall occured in the 2 Main North Entries in the intersection at crosscut #10 in the #1 entry. The fall measured approximately 20' X 20' X 6'. The area was supported with 6' tension rebar bolts. The fall was caused by an undetected slip.

An unintentional roof fall was discovered during a weekly examination of an inactive area of the Horizon Mine. This area of the mine was under water and when water was pumped out, the roof fall occurred. Roof was saturated with water. The roof fall was in the old North Section of the mine.

A FALL OF THE ROOF OCCURRED IN THE 3RD S.W. SUB-PANEL, #6 ENTRY AT X-CUT 12. THE FALL WAS 50' OUTBY THE TAILPIECE. THE FALL MEASURED 90' LONG X 19' WIDE X 8' HIGH. NO PERSONNEL OR VENTILATION WAS AFFECTED. A SHUTTLE CAR WAS UNDER THE FALL. THE CAR HAS BEEN REMOVED.

A ROOF ALL OCCURED AT #50 CROSSCUT ON THE 7 LEFT PANEL IN #2 ENTRY IN THE INTERSECTION. THE FALL WAS APPROXIMATELY 25'X25'X6' THICK. 5' HIGH TECH BOLTS HAD BEEN INSTALLED IN THE AREA.

A ROOF FALL WAS FOUND AT 2:00 AM ON 9/28/04 AT X-CUT 165 ENTRY #0 MAIN SOUTH. FALL WAS IN A 4-WAY INTERSECTION THAT WAS MINED MID-DECEMBER 1996, AND HAD BEEN SUPPORTED WITH 8' FULL RESIN BOLTS. FALL WAS 20' WIDE X 20' LONG X 10' HIGH. NO INJURY OR PROPERTY DAMAGED DUE TO FALL.

WHILE DOING WEEKLY AIRWAY A FALL WAS FOUND AT #4 SET SEALS IN FRONT OF SEAL #5. THE FALL CONSISTED OF LAMINATED SANDSTONE AND WAS BOLTED WITH 4' GLUE BOLTS. THE FALL HAS BEEN CRIBBED OFF FROM ALL SIDES. FALL WAS 5' HIGH, 18' - 20' WIDE & 50' LONG.

A roof fall occured in the #2 north panel in the #3 entry starting at crosscut #19 and proceeding up to the edge of the pillar line at crosscut #20. The fall measured approximately 100' X 19' X 5'. The area was mined in August of 2004 and supported with 5' fully grouted rebar bolts. The fall was caused by an undetected slip.

A ROOF FALL HAS OCCURRED AT OUR WEST-MAIN SECTION (MMU 014-0) IN THE #6 ENTRY AT SPAD 24+90. THE FALL MEASURES APPROXIMATELY 8' HIGH X 20' WIDE X 20' LONG. THE FALL COVERED THE ENTIRE INTERSECTION. THE FALL IS APPROXIMATELY 5600' OUBY THE FACE.

A roof fall occured on working section 001, Entry No. 4 near spad 1677. The area is near outcrop, low cover, some water present in roof strata and some cracks present. Miner was turning a right hand crosscut. The fall was 40 feet long by 20 feet wide and 5 to 6 feet thick.

ON 9/27/04 A ROOF FALL WAS DISCOVERED AROUND 10:30 PM ON MMU 007. THE ROOF FALL WAS LOCATED IN AN INTERSECTION IN #6 ENTRY, 3 LINES OUTBY THE FACE. THE FALL MEASURED 70' LONG X 20' WIDE X 5' THICK. NO INJURIES WERE REPORTED ASSOCIATED WITH THIS FALL.

Spontaneous combustion in coal storage pile. recent reduced usage. there was no damage other than a small amount of burned coal. most burning was under a smoldering condition. little open flame.

The main portal elevator malfunctioned. It was removed from service for repair. The door safe edge was found out of adjustment. The linkage for the safe edge was adjusted and elevator was put back in service at 11:05 AM.

AN UNINTENTIONAL ROOF FALL OCCURRED ON UNIT 4 #6 ENTRY 39+60'. THE FALL WAS 35' LONG X 20' WIDE X 6' HIGH. FALL WAS DUE TO ADVERSE ROOF CONDITIONS ABOVE BOLT ANCHORAGE. BOLTS WERE 4' FULLY-GROUTED.

A FALL OF SUPPORTED ROOF WAS FOUND BY THE MINE EXAMINER AND MSHA INSPECTOR IN THE WEST MAINS NO 2 RETURN AIR COURSE. THE FALL WAS IN THE ENTRY ADJACENT TO SPAD STATION #8641. NO INJURIES OCCURRED AND NO EQUIPMENT WAS DAMAGED. THE FALL MEASURED 200' LONG X 20' WIDE X 8' HIGH.

A ROOF FALL OCCURRED IN THE 2 LEFT SECTION (001) IN THE #6 ENTRY 30' INBY STA. #1829. THE FALL WAS 50' IN LENGTH X 19' WIDE X 7'-8' HIGH. THE AREA WAS BOLTED WITH 3/4"" X 5' TORQUE TENSION BOLTS. NO MEN OR EQUIPMENT WERE INVOLVED. VENTILATION WAS NOT INTERRUPTED.

An employee was operating the front-end loader (Kawasaki 115Z111) when it caught fire. Employee looked back and the loader was on fire. The fire was large and hot. The local Fire Department responded and extinguished it after 1 hour. There were no injuries. An ee reported the fire to MSHA via telephone on 9-29-04, 9:30 A.M.

The production/man hoist is computer controlled. The computer was not able to display information on the monitor to permit the hoist operator to complete his safety checks nor to re-set any faults that might occur. The shift was cancelled and no individuals were sent underground.

A roof fall was found on 9-27-04 around 8:45 am by safety director in the 1st southeast #5 entry between spad 12 and 60 and spad 13 and 30 in the pillar. Fall was 24' long 13 1/2' wide and 5' high. 5' fully grouted roof bolts were used in this area.

Rock fall occurred 15 breaks inby the Big 2 headdrive on the East Mains in the #5 belt entry. Fall measured approximately 18'wide x 40'long x 5' thick.

Employees at Noblesville Stone pumped water out of floor section and found a large pile of rocks that had appearantly fallen out of the back section above it. To date, an investigation has been impaired by the water and mud and it is unknown whether or not the fall occurred above the anchorage zone. No airflow or escapeways are inhibited as a result.

This area was mined in August 2004 and supported with 5ft. fully grouted roof bolts. This fall was caused by an undetected slip.

While examining return with msha inspector ee. I found a fall in #3 entry x-cut 900' into 3 SE but we also found a fall adjacent to this area in #1 entry which impeded our travel. Area had cutter present when mined & was bolted with 42"" resin bolts. 6' resin bolts & cables were installed as supplements. This is an abandoned portion of the mine. it is now dangered off with timbers.

Discovered on a development section 1st Right Mains during removal of mining equipment from area due to encountering adverse roof conditions in both the outby areas & immediate working face. Fall was discovered after decision to pull off this section area. Fall was 30'length-20'wide-5'high. 1st right mains between 4+5 right of spad 4917.

A PILLAR FALL OCCURRED WHILE BACKING THE CONTINUOUS MINER OUT OF THE SECOND PILLAR SPLIT OF THE NO.10 PILLAR BLOCK, COVERING UP THE HEAD OF THE MINER. THE LENGTH, WIDTH & THICKNESS COULD NOT BE DETERMINED. THE FALL WAS IN THE NO. 10 ENTRY, 3 BLOCKS INBY STATION NO.219.

EXACT DATE AND TIME ARE UNKNOWN. FOUND ON WEEKLY EXAM. A ROOF FALL OCCURRED IN #1 ENTRY OF 3RD LEFT BELT AT SURVEY STATION #88. IT MEASURED ABOUT 120' LONG X 5' HIGH X 20' WIDE. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE.

A roof fall was found by examiner doing weekly at at x-cut 258 entry 6 south mains on 9/21/2004. Fall was in a four way intersection that was mined in September 1997 and supported with 8' full resin bolts. No injury or property damage resulted. Travel way will be rerouted.

A ROOF FALL WAS OBSERVED BY THE MINE EXAMINER AT APPROX. 1:35 PM, WHILE EXAMINING ELECTRICAL OUTBY INSULATIONS. IT WAS IN #6 ENTRY OF 2-E PANEL AT SPAD #18 + 90. 18.5' WIDE X 70' LONG X 5.5' HIGH.

#30 hoist power kicked out at 3:20 p.m. Hoistman called electrician at 3:22 p.m. and then called msha at 3:58 p.m. and told them #30 hoist was out of service at 4:30 p.m. hoistman on duty forgot to call msha back. that call was made at the time this investigation was done at 8:49 a.m. on 9-22-04 by ee hoistman on duty day shift.

ROOF FALL OCCURRED IN 1 RIGHT RETURN #2 HEADING, 69-71 BLOCK. AREA MINED IN JUNE. THE FALL IS 19 BLOCKS OUTBY THE SECTION. THE TOP IS GRAY LAMINATED SHALE. THE FALL IS APPROX. 120' LONG & 4' TO 6' HIGH. BOLTED MOSTLY WITH 4' RESIN BOLTS. FALL WILL NOT BE CLEANED UP.

Switch gear for crusher mtr. failed, causing main feeder breaker to trip off knocking power to Hoist & station power UG. Bore hole power for UG was still on. Work stopped UG. Electricians made necessary repairs & re-energized power. Down 8:50 a.m. to 10:10 a.m.

An unintentional roof fall occurred on MMU-007, in x-cut 6-7, 1 block inby 11087 (spad). It measured 18' wide x 30' long x 8' high. This fall occurred in an intake escapeway. This escapeway was re-routed. Additional roof to floor support was added.

While mining on longwall face in Panel 23, a bounce occurred. This resulted in damage to several stoppings in the bleeder system, effecting longwall face ventilation. An MSHA inspector was present on the surface when the bounce occurred. He was immediately notified and informed of the actions being taken to restore proper ventilation.

A roof fall occurred on the 2L4W-2L panel at cross-cut 5 entry 3 to 4. The fall was found during the pre-shift examination for third shift. The cause of the fall was weak laminated sandy shale more commonly referred to as stack rock.

An unplanned, non injury roof fall was discovered by preshift mine examiner while making his checks and exam of mine. the fall occured between 4 45 pm on 9-1 and when it was discovered at 5:15 am on 9-2-04. the fall was at the 20th crosscut on #3 belt entry. it measured 0 to 8' thick, 35' long and 30' wide.

A roof fall occured in the 1 main east entries at cross-cut 25 in the intersection of #3 entry and proceeding thru the intersection of the #2 entry measuring 80ft. x 19ft. x 6ft-9ft. This area was mined in May 2004 and supported with 6ft. tension rebar bolts and 9ft. mega bolts. The fall was caused by an undetected slip.

Employee attempted to replace starter on Hyster H50XM forklift, jacked forklift up with a 10 ton hydraulic floor jack. He did not put any blocks or shorwings under forklift to prevent it from falling. He then got under forklift to repair starter and forlift then fell off jack causing major head injuries. Employee closing bottom railcar doors. Closure requires employee to place 5 foot, 1 inch diameter steel bar in door hardware and using body leverage pull the door closed. During closure of a door the bar slipped from the hardware, resulting in the employee falling backwards to grade, landing on her left hip. Employee suffered severe contusion to her left hip area.

The injured employee was repairing on the unit 2 roof bolting machine. he had removed the rear main control panel and as he was possitiong the cover during reinstallation he felt a sharp pain in his low back. This is incident has been under investigation awaiting information from the dr. ee should always use proper body position and ask for help when handling heavy objects.
