The mine experienced a roof fall in the 5th Northwest Panel at the 8+10 XC between entries #2 & #3. The fall did not affect ventilation, but did involve the secondary escapeway. The fall measured 25' X 10' X 7'thick. The area was developed on 12/21/2007, with 6"" fully grouted bolts. 103(k) issued over the phone.

Extenduation of Pillar Line from STA #2108 230' to 2133 in the process of moving MP-25. 26.
Safety door switch malfunction caused interruption to elevator service for more than 30 minutes. Repairs made to switch to return elevator to service.
HOTWORK hired for curing of refractory. 10AM Alternate Energy vaporizer pressurized, leak checked and lit-then Hotwork burners lit. Fittings checked on vapor outlet & inlet lines. 10:15AM Hercules plant worker let know that vaporizor was afire-2.5' flame - Pressure outlet valve shut off & flame went out. Exam of piping shows that elbow was not tight at connection.
A roof fall was foundby an examiner during an idle day fire check. The fall was located at 1+80 #6 room off the return side at 29+20 of Zone P1 of Zone sub1. The fall was 60'Lx19'Wx7'H. The area was originally supported with 5' fully grouted resin bolts. There were no injuries involved, air flow was not disturbed and the passage of men was not affected.
HOTWORK (D843) contracted for curing of refractory. High capacity vaporizer converts LP to a gas for use in their heating systems. Equip was set up, leak checked, then lit & operating for 1hr. 9:30am HW employee saw 8' flame burning at vaporizer burner regulator. Supply line was shut off, fire dept called-advised gas be allowed to burn off.
Received a call that one of the oil well locations had been struck by lighting, resulting in a fire; this was in an inactive part of the mine. MSHA number called, talked with MSHA EE and shortly after that another MSHA. # 1-51906711.
Elevator down more that 30 min. Elevator started to move from bottom landing to the surface when the door opened. The elevator stopped in the ""cardoor open in motion"" mode. The door lock on the inside car door was adjusted and test ran. Once inspected the elevator returned to service. No one was on the elevator at the time of the incident.
Return off the longwall was being re-habilitated due to upcoming longwall move. A roof fall occurred inby the area of rehibilitation for an area of roof 20 ft. long x 11 ft wide and 4 ft in height fell. There was still a four foot walkway that had been supported that did not fall.
An unintentional roof fall occurred in the 10I Longwall Tailgate entry. The fall measured 15ft long X 16ft wide X 12ft high. There was no injury or equipment damage with this occurance.
Elvator stalled between 3rd and 4th floor for approximately 35 minutes.

A roof fall was found at crosscut #55 to #57 on #2 beltline, the fall was approx. 65'x20', between 8' to 10' thick. Additional roof support of cribs and cable bolts were put in affected area, all breaks around the fall were checked, no injuries, beltline structure and possible waterline structure damage.
Employee was operating #74 scoop when he saw a flash that lasted approiximately 2 minutes coming from the scoop breaker box. No injuries occured. After investigation it was determined that the tree of leads coming from the control box to the Jct box had shorted together causing the flash he had seen.
A fall measuring approximately 18' W x 40' Long x 6' High occurred on section no. 4, entry 2.
An examiner found a roof fall on the Main South at crosscut #49 between the roadway and belt line. The roadway had been rehabilitated with 8' bolts and the belt line had been re-bolted with 6' bolts. The fall did not continue into either the belt line or roadway, because of the extra support present.
A roof fall occurred in the 6-East #1 Entry at break #23. The fall was 25' long by 20' wide, and 6' in thickness. There was no equipment involved, or any injuries. The fall cleaned up and the roof re-supported. The area was originally supported with 5' hi-tech roof bolts.
#1 Hoist stopped and would not reset. UPS for the PLC system failure causing hoist to go down. UPS replaced, hoist recalibrated, reset, checked, balanced, tested OK. Two means of egress were still operational (#3 Hoist and ""Chippy"")
Roof fall in D entry in 3 South @ station #33+09. Approximately 30 ft long, 17 ft wide and 8 ft high.

Escape Hoist in Zone 2 was missing the tube block. So they downed the Escape Hoist at 4:00 P.M. and brought it back up at 5:00 P.M. that same night. No miners trapped or injuried.
A rock fall was discovered approx. 300' inby portal, in the intake roadway. The fall was one rock that measured 8' wide extending 6' down to a point. The rock was triangular in shape and was approx 3' high. The rock fell sometime between the morning preshift and when the foreman exited the mine, approx 3:00 pm.

Fall had been dangered off & cribbed off for 3 months. When we drove through the area it was laminated sandstone with pressure from block of coal left in the room above the pressure causing the laminated sandstone to break up causing the rook fall in an intersection. Fall was approx 4' to 8' high, 18' wide and about 40' long in #6 intake entry, inby spad #6516 1 break.
A roof fall occurred at the South E panel #21 xc #7 Entry in a 4-Way intersection. The fall did not trap men or material. It did not block travel or ventilation. The fall was 17 x 17 x 10 ft high. The area was bolted with 5 ft fully grouted resin bolts.
The #1 elevator was having intermittent problems that could have resulted in it being removed from service. The elevator was never down for a period of time exceeding 30 minutes for trouble shooting the problem. A bad TRL timing relay was found and replaced. No escapeways were affected and no individuals were on the elevator during the times.
A roof fall was discovered on 3/24/08. A belt line was finished out that seperated the belt from the reform. The fall was approx 12' long X 17' wide X 20' long. The fall did not affect escapeways or the equipment. The area was bolted with 5' fully grouted roof bolts.
A roof fall measuring 30' length, 20' wide, and 7' thick was discovered in the return entry 1500' outby the working face. The area was developed Nov 07 with 5' torque tension roof bolts. The fall occurred due to shale mixed with coal streaks above the anchorage zone. Fall location was in # 2 entry on 1st left panel near spad 58748.
A roof fall measuring 50' length, 20' wide, and 6' thick was discovered in the return entry 1500' outby the working face. The area was initially developed Nov 07 with 5' torque tension roof bolts used. The fall occurred due to shale mixed with coal streaks above the anchorage zone. Fall location was #6 entry 1st left panel.

The Joy Miner was cutting bottom in the No 3 Entry on No. 6 Section that was driving the South Entries when an ignition occurred in the bottom of the Face. It took approximately one minute to put the fire out. This was a frictional ignition.
An unintentional roof fall occurred in the No.4 entry, one break inby spad No.3194, 2000' outby the working section. The fall measured 80' long, 19' wide and 0' to 6' high. The fall did not affect the travel of men or the passage of air.
Mine associates left the main office area and traveled to the mine work area at 6:20 a.m. One EE notified another EE at 6:25 a.m. by FM radio the mine office trailer had been destroyed by fire. The fire occurred sometime over the weekend. The mine was not in operation and no injuries. MSHA was contacted at 6:34 a.m. to report the fire. Cause has not been determined.

An unintentional roof fall occurred on the #4 section in the #8 entry one x-cut inby spad #44917. The fall measured to be about 6' high, 20' wide and 60' long. The area was bolted with 5' torque tension bolts. The fall will not be cleaned up. All entrances were cribbed and dangered off.
During a pre-shift examinaion, Examiner found a roof fall in the #7 heading of the active 003 mmu. The fall was approximately 120' inby survey station number 1000. Fall measured 20' long x 18' wide x 0' to 8' in height. Fall was found at 10:45 pm and reported to MSHA at 10:58 pm.
4th 48 Beltline (x-cut 21). Length 35 x width 17 1/2 x thickness 10.
roof fall in the 8N bleeders, #1 entry, #30-#31 break. Approximately 18'WX12'HX30L.
On the #3 mains section a roof fall occurred in the #48XC, #11 intersection when the CM cut into this area from the #49 XC. The fall occurred in the 3-way intersection that was bolted w/6' fully grouted bolts with 4-12' post tension cable bolts. The cut that happened at the intersection had a slip running through them. The fall was appox 17'X17' X 10-16' high. H20 present.
25 x 20' x 5' high fall in #5 entry and #3 section near spad no 11127.
An unintentional roof fall occurred on the Old 1-A return at station 28+00 in the #10 entry. The fall measured 60 feet x 18 feet x 8 feet. The fall did not affect ventilation, passage of men or cause personal injury. The fall will not be cleaned up. Approximate date of development 2-2-2007.
Roof fall at 8N Bleeders, #1 entry, #30 - #31. Approximately 18'WX12'HX30L. This area was bolted with 5'jenmar combination bolts. We are installing 5' jenmar bolts in working sections and supplementing them with 8' cable bolts and straps where needed.
A roof fall occurred in the #9 seam, 3A panel, spad 1451 #2 and #3 crosscuts. The fall is approximately 150 feet long, 20 feet wide and unkown height.
The arm was mined in march 06. We had a fall in June 06. Soft top and water in a drainage contributed. This was a continuation of the fall. It blocked travel beyond this pain. The area will be sealed.
A fall of the roof occurred on the northwest sub main panel, #3 unit one Break in front of the feeder. The fall measured 30 x 20 x 8-10 feet. No ventilation, equipment or personnel was affected.
East Mains 002 unit, a roof fall occurred in the # 3 entry 130' inby spad 10025 measuring approx. 25' x 20' x 4'. A portion of the Joy continuous mining machine was caught under the rock. No disruption to ventilation, no travelways blocked, and no injuries.
#4 hoist (back up and emergency hoist) down due to electric problems. Lost automatic and part of manual control, still have #2 and #5 shaft as escape routes. Unknown time when down. Hoist back up and running
An unplanned fall measuring 100' in length X 18' in width X 9' in height, fell on the #2 section at approx 3:40 pm. This was immediatley reported and dangered off. Timbers were installed to block the entries off. It will be rehabilitated.
An unintentional roof fall occurred at the galatia Mine New Future portal at the 9 west headgate belt entry #3, Crosscut 47,6970 West. The fall was approx. 20 ft long, approxl 15 ft wide, and approx. 10 feet thick. The fall did not affect ventilation. The fall will be cleaned up.
A fall was discovered in the 1 southwest set of entries. the fall was located in the cross cut located between the No. 5 and No. 6 entries adjacent to survey station No. 198. The fall measured approximately 5' high by 15' wide by 50' long. Tthere were no injuries or equipment involved. Ventilation not affected.
Area experienced a 10"" to 11"" rainfall event with 8"" falling in the first 24 hours. This event followed 8.2"" in feb. and 3.5"" earlier in March. runoff overwhelled. 8"" turbine pump in the sump at surface of drift mine even with the start of. 8"" diesel backup. Water raised such that water entered the mine beginning at 2 pm on 3/18 and mine became inundation at 7 am on the 4th.
An unintentional roof fall outby on Unit 1 in Entry 7 between 5070 south and 5135 south. The fall was found by a mine examiner making his weekly inspection. The fall is approximately 25ft long, 18ft. wide, and 0-6ft. high. The fall will not be cleaned up. Fall area was supported with 5ft. conventional roof bolts.
A roof fall occurred in the 1 South in the #5 entry starting at the inby edge pillarline of crosscut #21 and extending to the face. The fall measured approximately 70ft. x 18ft. x 8ft. high. The area was supported with 5ft. fully grouted roof bolts and bacon straps. A slip was determined to be the cause of the fall.
An unintentional fall occurred at the 46+30' in 10th main north return entry. Fall was 24' X 24' X 10'.
Employee noticed a fire on the pocket belt at the Maerz Complex. The pocket belt (Fairfield Engineering #K462460453) and Beltwall belt #BWX222 caught on fire and burned. The cause is yet to be determined. No injuries.
Hydrolic system on the Escape Hoist at Zone 4 wasn't working properly. So took the Escape Hoist out of service until the system could be repaired. No miners trapped or injuried. Hoist back up at 5:30 P.M. on 3/20/08.
An unintentional roof fall occurred on the #3 section in the #1 entry just inby spad #44808. The fall measured to be about 6' to 7' high, 20' wide and 80' long. The area was bolted with 5' torque tension bolts. The fall will not be cleaned up. All entrances were cribbed and dangered off.

The Cedar Creek Elevator was out of service from 2:30 pm until 4:00 pm due to a microswitch.
A fire occurred on the 2002 sterling fuel lube truck co#135, fire was not extingusted within 30min. The fire was put out by the local fire dept. There was no injuries during this event.
The hoist car servicing the #1 track slope was taken out of service when the overspeed (500 feet/minute) switch failed. The car was down from 7:16 pm on 3/17/2008 until 8:45 pm on 3/18/2008.
Roof fall in B entry 39169 4 way Intersection, approx 30 feet long, 20 feet long and 10 feet high.
Dus to intermittent loss of power to the synchronous motor protector, the hoist was taken out of service @ 445pm. After resetting the field with amps, the hoist was placed back in service @535pm. The electrial engineer who was called in believes that a relay on the synchronous motor protector may be going bad.

Roof fall Intersection of #4 entry of the 1-Left Submain at break 11 . Approximately 30 feet long 20 feet wide 8 feet high. No one injured, no equiptment involved.
Mine was idled and the Examiner preshifting for 3rd shift found roof fall during exam at 9:50pm in the 1st Mains #3 entry, xc 6. The fall was in intake travelway in oldest part of mine, mined around Oct. 1996 and measured 25'w x 12'L x 6'-8' hi. The area was supported with 6' fully grouted bolts in a fault area with some cribs.
An unintentional roof fall occurred in Unit 1 Entry 6 at +6045. The area was supported with 5ft. conventional bolts. The fall is 18ft. wide, 30ft. long, and 5-6ft. high. The fall will not be cleaned up.
#3 Hoist kicked out on ""Drive converter A"" code. An electrician had to be called out of the mine to reset the hoist. #3 hoist was reset and back into service at 11:50 pm.
Approx. 12 tons of material fell from the highwall onto the canopy at the No. 1 Portal. The material measured approx. 0""-2' in thickness, 12' wide x 8' long. The material was removed from the canopy and a replacement canopy was installed.
A fall of roof occurred in the 2F panel on # 2 Unit, the fall was located in the # 4 entry at x-cut # 8. The fall was approx 18 ft wide x 30 ft long x 9 ft high. Determination was made that it was due to a slip in the roof
A roof fall occurred in the face of No. 6 entry inby crosscut No. 860. The fall measured approximately 19 feet wide, 6 feet high and 30 feet long. No injuries, no one trapped, and no disruption of ventilation. Agencies were notified as required.
Fall occurred in the #7 hdg 30' inby spad #54 to the face. Approx 55' in length by 18' in width by 6'-9' in height. A clean up plan will be posted. Reviewed with crew and submitted for approval with OMHST and MSHA.
Haul truck driver was entering the shovel pit when he noticed no power on the shovel & smoke coming out of the back of house. The fire suppression system activated & a mayday was called at 11:26. The electrician killed the power-fire brigade was dispatched. The fire was out at 11:54 & a fire watch was put in place. At approx 12:10 it started to burn again & was called out at 12:26.
The fall occurred in the left rooms off #7 Mains 180' inby spad 938. The rooms were toward the outcrop and water had developed in roof.
An unplanned roof fall occurred int he #2 gas mine, in the #2 entry near spad #497. The fall measured 30' in length, 28' width and 11 to 14' in height. No injuries were reported. The area was developed on 3/5/08.

An electrical problem caused operation of Koepe cage to cease. Extensive investigation revealed dust partially clogging air sensor for SCR cooling fan & allowing over-heating. Mine evacuated; no injuries; no problems. Hoist back in service at approximately 12:05pm.

A roof fall occurred in the #5 entry three breaks inby survey station no. 932. The fall is approximately 800 ft. from the working section and measures approximately 90 ft. long, 20 ft. wide, and 0 to 8 ft. in height. Fall was discovered at 5:22 am and MSHA was notified immediately.
An ignition occurred at approx 65+70 in #2 Entry. E-18 section. The ignition lasted 15 to 20 seconds and was extinguished by the use of a wash down hose on the miner. There was no damage to equipment and there were no injuries.

Rock fall occurred on 002 bleeder cut through between 4 and 5 left panels off 2 right off 1 left off 1 east off 1 north mains. Fall measured approx. 20' wide x 4'-6' thick (length could not be determined).
An unintentional fall occured in old main north entry at the 61+70' #5 entry. Fall was 33' long 18' wide and 10' high. This was a 4 way intersection.
Fall was measured 20'x20'x5' thick. 3000 from face. 1 block outby 21087. Ventilation or passage was not affected, no injuries. Fall was phoned in to MSHA and state of WV.
Roof fall occured on the 11-I section at the no. 19 crosscut in the number 3 entryand extended to the no. 1 entry. fall is about 16ft wide, 210 ft. long and about 15 ft. high.
At or around 3:00pm a roof fall occurred on the 004 section bleeder entry, just outby Spad No 23014. The fall measured 18 feet wide x 18 feet long x 8 feet high. No injuries were reported as a result of this fall of roof rock. The fall will not be cleaned and all entries leading to the roof fall have been timbered and danger signs installed.

Roof fall discovered by section foreman in worked out area adjacent to active mining section. The cave measured 1000'long by 19'wide by unknown height. There were no visible defects when originally mined. Test hole (8'deep) were drilled in every cut and in every intersection. The area was bolted with 4' FG resin bolts & 8' cable bolts placed in the test holes.
Unintentional roof fall found by on-shift examiner and federal mine inspector at x-cut #14 in #4 entry on #2 belt line. Fall was 6' high, 20' wide and 60' long. Found at 1:30 pm on 3/11/08.
During pre-shift tests (no employees on cage), the operator's console froze and would not reset. An electrician performed a power restart which made the system operational. The system had to be resynchronized and the pre-shift tests performed again. MSHA was contacted regarding the hoist control issue at approximately 7:15 pm and the system was operational at 7:45 pm.

A fire was discovered at the golf cart charging station as 3rd shift entered the mine. The fire was discovered at approximately 12:10 AM. The fire was under control at approximately 1:20 AM and was extinguished by 2:30 AM. A ""Club Car"" golf cart was involved and there were no injuries or eye witnesses.

Creek/Pond from private property breeched on 03/9/08 due to excesive rain. It then came thru the quarry over the high wall taking out the #10 screen pile leaving from 4ft to 50ft of water into the pit. Noone was working at the time and no quipment harm was done. All areas were secured on Monday morning 3/10/08 before the quarry was open. DEP was contacted on for this event.
The operator of the DMM2 blasthole drill finished drilling the drill pattern around midnight, shut off the machine and left the area around 12:15am. The dragline oiler noticed the fire around 12:50am and called a MayDay. The fire was under control about 45 minutes after the call. Maintenance supervisor notified MSHA around 1:30am.

A roof fall was found by an employee while driving on the haul road. The fall was located at 41 + 30 #4 entry of zone sub 1 on mmu-01. The area was developed 11/29/07. The fall was 25'L x 15'W x 8'H. The area was originally supported with 6' fully-grouted resin bolts. There were no injuries involved, air flow was not disturbed and the passage of men was not affected.
Our examiners found a roof fall in the worked out area of the 1 Left, 5 West panel during their weekly examination of the area. The fall was located in a crosscut adjacent to their normal route of travel into the area. The fall was located at crosscut #8 between entries 4 and 5.

Rockburst occured in crosscut during the day shift, caused withdraw of the miners in work area.Normal activity was disrupted for more than one hour and MSHA was notified. Rockbust created about 6 cars of muck at the face, a couple of smaller burts followed and consensus was to back away for the remainder of the day.

As retreat mining was taking place the left side MRS unit was being trammed forward after the #9 cut. A fall occurred in the #9 cut, coming out into the entry trapping the MRS units. A roof fall occurred in the 1 South, crosscut #18 in part of the intersection of #5 entry and extending inby toward the face and covered the back 10' of a roof bolter. The fall measured approximately 35' to 40'x3'to 19'x7' to 8'. The area was supported with 3' fully grouted bolts and 5' fully grouted bolts. A slip was determined to be the cause of the fall.
Hoist was down due an electrical breaker wouldn't trip.

Upon entering the mine, the night shift crew encountered smoke and 12 PPM carbon monoxide. The shift boss removed the crew from the mine. No fire was seen. MSHA was contacted as a precaution. After receiving approval from MSHA, the mine was checked for hazards and none were found. A high voltage vacuum breaker blew out, causing the smoke.

The #1 MRS was 150 inby spad #635 when it became disabled and would not tram. After attempting to correct the problem the roof conditions deteriorated and fell in on the MRS. AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 ENTRY, BETWEEN X-CUT #14 AND X-CUT #15. THE FALL MEASURED APPROXIMATELY 22'L x 12'W x 6' TO 6.5'H.
Victim was removing a machine bottom guard (belly pan) on a Caterpillar D-10 N dozer. Apparently the chain he was using to hold the belly pan while he was removing the bolts that held it in place slipped when the bolts were removed. The belly pan swung down and struck the victim inflicting fatal injuries
While walking outside the warehouse door, next to the loading dock, EE stepped onto the slope part of the loading dock and encountered black ice that could not be seen in the darks. He slid back and landed on his back and his right leg went behind him.
Employee was operating the left side of a dual boom bolter when he got his left hand entangled with the drill steel and a wrench. This resulted in an amputation of his left hand and arm near his elbow. He was treated on site and then taken by ambulance to a local hospital. He had surgery and his arm was reattached. He was released from the hospital on 4-4-08.
EE was operating the continuous miner. As he was backing up, he noticed line brattice behind him. He reached through the opening of the canopy, directly behind the operators seat, to push the brattice to the side. While doing this, his left hand was above the canopy and his left index finger was squeezed between the roof and the canopy.
As the employee was walking down the rail tracks, he reached over to release the handle on a rail car, when he slipped on the ice and fell to the ground. On 3/28/08 he was still having abdominal pain. He was taken to seek medical attention and was diagnosed with an abdominal hernia.
After installing a roof bolt, victim was dropping the mast jack to remove the wrench. He had placed his left hand on the stabilizing cross member on the 2nd stage of the mast jack assembly. As the 3rd stage of the jack lowered, the bar on the 3rd stage caught the end of his middle finger and glove between 3rd stage and bar, severing tip of middle finger between 1st and 2nd joint.
The EE was straightening a 6' torgue tensioned bolt and the bolt spun around, injuring the EE's left shoulder.

Employee was replacing the blade on the d6 dozer when he bumped the hydraulic cylinder with his shoulder causing the cylinder to fall on and cut his left thumb. He severed 2 tendons and required surgery to repair tendons. Follow up appt. is 4/11/08 followed by physical therapy. Released for light duty w/restrictions.
Employee was attempting to remove a plastic end cap from a piece of electrical conduit when his right hand slipped and the blade of the knife he was using penetrated his left hand causing tendon damage which was surgically repaired.
The man was pulling up slack cable for the DBT mobile bridge conveyor. The man felt pain in his right neck and shoulder area.
Employee was walking down a flight of stairs in the concentrator. Employee missed a step, but did not fall. Employee was holding on to the hand rail. Employee felt pain in his back and right leg.
Ee was shoveling when he pulled something in his back. It was only a pulled muscle. Ee went back to work, with no restrictions on 4-30-08
EE stated that he was helping rib bolt with a twin boom bolter at #2 empty belt rehab project. He & another EE were going to rib bolt a corner but there was an oxy & acet. cart there. EE attempted to move cart & feet slipped causing him to fall to knees while cart moved forward. This caused his head to whip down forcing his nose into the cart resulting in nose fracture & cut.

Injured was trying to hook a rockduster to a mobile vehicle. The rockduster was bumped by the jeep in an attempt to center the hitch. As he began to install a pin into the coupler, the jeep rolled back and caught his left middle finger.
Preparing to service equipment, was in bucket of scoop, stepped out, level ground-no debri, twisted left ankle. Splint ankle and taken to E.R. diagnosis; Subtalar Dislocation. Employee 6' 4"" wt. 350 lb.
Injured employee was in a scoop being pushed out of the roadway when his bucket slipped off the other scoop bumper resulting in a back injury from the impact of the scoop falling.

Employee injured 3-28-08; rock fell while installing bolts, struck right side and knee area, went to E.R. x-ray negative, saw specialist 4-2-08 schedule MRI for 7-7-08, after MRI surgery to be scheduled, had surgery and began to miss work.
Injured was crossing the #6 tailpiece when his foot got tangled up on a turn buckle anchoring the tailpiece. This twisted his left ankle. Causing a sprain to the left ankle.
The roads were wet & slippery, EE was trying to pass a coal truck, lost control of his S10 and went to into a slump hole around Marker #22. It skinned his cheek, put a laceration in his knee requiring 5 stitches, and a contusion.
Emp. was walking out of pump house & stepped in hole on uneven ground & twisted knee.
Was using an acetelyne torch, the torch sparked back after being shut off burning left hand.
Victim was moving his anchor for his shuttle car. After doing so was going back to his car. Crossed cable for other car when other car was energized, tripping victim & causing him to fall. Had small abrasion on nose, lip busted and victim complained of a loose tooth. Victim refused to go to hospital and did not visit a doctor until 3-31-08.
Working as a roof bolter helper while bolting the longwall face with Tensar screen. The face rolled in and the victim fell into the face conveyor in a attempt to avoid the falling coal. His left leg was covered by coal to the knee causing an abrasion and he sustained two fractured ribs & abrasion & contusions to his shoulder when he struck the face conveyor.
Using hammer-hammer slipped pinched thumb between hammer & plate. accident occurred 3/28/08- did not seek medical attention until 4/1/08.
Was cleaning up the work area at the end of the shift, (crew was spraying concrete).
Attempting to empty a gypsum rail car from the top of a stair case platform. The load inside shifted and the rail car moved, bumping the platform and knocking it over. The worker sustained fractured right foot upon falling to the ground.
While lowering the hood of a truck, felt sharp pain in right shoulder.
While removing a wheel unit from shuttle car, employee had a breaker bar on a wrench. He stood on the breaker bar which slipped off the wrench and struck him on the leg.

EE was operating s/car in the #3 South, #2 entry and tire went into a pot hole and hit his head, jamming neck into the canopy.
Employee, along with co-workers, were lifting rail up onto a rail truck to be removed from the section. Cause of this injury is not having a mechanical means to lift rail onto rail trucks. Compliance with rules/regs, protective equip was not a factor. Mining systems is a factor for this job and miner proficiency a factor.

Pulling miner cable over when he felt something pull in his right shoulder. Doctor took him off work effective 3/31/08.
No treatment or lost time related to injury until Aug 28 08. Stepping off ladder twisted left leg.
According to the employee he drilled a hole and was in the process of trying to get the pinner steel out of the hole when the drill pot fell on his knees and thighs. He was in a kneeling position. The cause of the fall is unexplained. When checked the machine operated properly. Due to complications he started misssing work on 4-4-08.
Operator was cleaning ditch line and berms with excavator bucket, hit tree limb causing it to strike windshield cutting EE's hand.
While working to bolt a sandline together the chain holding the pipe off the ground came loose causing the pipe to come loose and hit the miner in the chest. The force of the pipe hitting him in the chest knocked him on the ground causing him to strike his head on a piece of metal used to hold conduit off the ground.
Employee was being loaded when the dipper of the shovel came in contact with the dove tail of the bed. The bed raised and dropped causing him to be jarred. Employee was taken to ER and taken off work.
EE reported: He & a co-worker had moved the lunch bench from along a ribline due to unstable roof in the area. He was attempting to pry down the loose roof. A piece of roof fell from behind where he was prying, striking his head & shoulder. He was knocked to the mine floor, striking his shoulder against the power cable tub & his head struck the tub resulting in 1"" cut.
Employee was installing a roof bolt on a corner, when a piece of coal fell off the rib striking him on the right hand causing a fracture. He did not seek medical attention until 04/01/2008. He started missing work on 04/08/2008.

Employee was operating a haul truck. Employee felt something hit his knee, looked down and haul truck ran over a berm. Employee obtained medical care in another town; Freeport Morenci was not notified timely.
Two members were loading a tire in the back of a service truck when one of the member's lost his grip and let go of the tire. The other member maintained a hold on the tire and felt pain in his lower abdomen.
Employee alleges he was unloading rails on the travelway from off of the back of his scoop when he slipped in mud and strained lower back. *After further investigation, it has been determined that this alleged incident could have occurred at the above referenced mine and therefore needs to be reported.

At the time of the incident the technician was moving a workbench in order to clean under it. As the tech was pushing on the bench one of its wheels dropped into a divot in the floor. As he pushed harder to dislodge the wheel his left foot slipped on some oil causing him to fall against the bench and twist his back.

Employee injured right shoulder-sprain and has been through dr visits and found a rotor cup tear out for surgery on 11/17/08. moving items/with hand truck when weight shifted tilting hand truck and pulling on shoulder.
Reeling bolter cable up when the cable jerked the hangers down and the cable hit the back of his head and neck. It pulled muscle in neck.
Employee was stepping out of the operator's compartment of a diesel scoop when his right foot slipped causing pain in his left leg. Upon completion of medical tests it was determined his lower back was initially injured and was the cause of the pain in his left leg. This did not become a Lost Time Accident until 6/6/2008.
EE stated that after assisting the supply motorment in cutting cars, he twisted his ankle while stepping and turning away. The EE finished the remainder of his shift before leaving mine property in an ambulance at approx 1:00am. Medical revealed a fractured ankle.
A piece of rock fell from the coal rib striking the employee on his lower right leg and ankle rrsulting in an injury
EE had dumped his shuttle car's load into the feeder. While changing seats in the shuttle car, he stepped into a hole which caused him to stumble and fall backwards onto the shuttle car striking his lower back and ribs on the canopy.
EE was walking in #2 entry. center bolter picked up the slack on his cable reel, the cable came across the entry and tripped him causing him to roll his right ankle.
Employee was building a stopping, after foaming he stepped down on a chunk of coal rolling his foot and ankle causing a fracture to the bone on the small toe.

Miner was lifting a wheel bearing from a cleaning cart and moving to a table. Miner picked up bearing and turned and walked towards table. Miner felt a pull in his back when lifting the bearing to table height. This resulted in a lower back strain.
EE was bolting the rib and the rib rolled out, striking his right leg and ankle.
While walking to the rest room, Jose slipped on a uniform clothes hanger that was on the floor around the dirty uniform barrel containers. No medical attention was initially required. Subsequently-EE lost 29 workdays.
Employee needed second back surgery for prior injury.
Employee was struck by a section tailpiece on 3/24/08 which caused a contusion on the right knee. The employee started missing work due to the injury on 07/01/08.
While driving an 18 wheeler hauling coal to the tipple the driver turned the truck over on the passenger side of the truck. Coming out of the Amos Run curve of Copley Trace haul road.
Employee state he had bent down to stand a 60"" roller against rib. When he started to lift, he felt something pop in his right arm, between his elbow and bicep. This employee didnt start missing work until he went in for surgery on 10/6/08.
EE and another EE were stringing out discharge water hose, EE was trying to untangle the hose, it was twisted. When the other EE pulled on the hose bending his right thumb back (causing sprain).
Driver was getting out of truck, foot slipped and was falling and put right hand on ground to catch himself.
When getting out of the excavator, ee stepped on the track and slipped twisting his right ankle.
Employee was repairing a light fixture on the top level of Plant 1 Tyler System. While repairing the fixture, employee fell down a flight of stairs. The fall dislocated his left shoulder, ring and pinky finger of his left hand.
Employee stated he had traveled with the MSHA insp. and after he left he was assigned to shovel out the drain in the diesel barn. As he shoveled it into a trash can and then emptied it into a small dumpster, he felt pain in his lower back.
Struck head against top of inby air lock door on 1 North belt, while riding in mantrip.
The subject was watching the cable on the miner. A rib blew out and struck him in the leg and ankle
Employee (Mechanic) was attempting to free the power step on a hydraulic shovel due to it being damaged. The employee was standing in the basket when it was freed and the step free fell to the ground. As a result of the free fall the employee struck his face on the basket requiring sutures to close the cut.
While stepping to ground level from stairs, he stepped with left foot, causing the foot to roll.
Employee strained back while positioning a loader tire. He sought medical attention and was taken off work.
Miner was helping to lift a feeder chain into the scoop bucket. Had pain next day.
Injured had just got through greasing a D9N dozer and as he was climbing down his foot slipped causing him to land on his left ankle spraining it.
Slipped coming down the stairs at the South Black Thunder Loadout. The building had just been washed down and the stairs were wet. Employee fell down the last 3 stairs, landing on her elbow. Employee was taken to Campbell County Memorial Hospital were it was determined that she had sprained and possibly fractured the elbow. Employee was given a full work release for 3/25/08.
Employee was assigned to the surface plant area and alleges he sustained a lime burn to his foot, however he did not; nor has yet reported this incident to plant management. The employee notified plant management on 3-29-08 he was going to seek medical treatment for this alleged incident through his family.
On or about March 21, 2008 the injured alleged he was using an impact wrench for a number of hours. Later in time the injured came to his supervisor and claimed he had pain in his elbow and wanted to be seen by the local clinic. Physician treatment on April 3, 2008 discovered an infection and additional treatment has made this injury a recordable.

Employee was exiting ER 4 when his right leg cramped up and his left leg gave out causing him to fall on the stairs. He was carrying tools in both hands while descending the stairs.

Operating roof bolter on West Mains section, #49 cross-cut 2 to 3. Operator got his ring finger of left hand caught between the drill steel and bolter head crushing end of his finger. Was holding steel and running the head up to connect steel into drill chuck. Was wearing meta carpal gloves at the time of accident.
Employee had trouble releasing a latch on a self-dumping hopper. He stood on the latch handle and slipped off, falling backward onto the floor, striking his head and back on the concrete pavement.
Employee was removing sand props under debris and strained his groin. Reportable lost time injury 3/24/2008.
He was troubleshooting a longwall shield when the shield pulled in on his left foot causing a crushing injury to the left foot.

Employee states he felt a sharp pain in his left elbow when he applied pressure to a bolt while doing maintenance at the crusher. He had stated to co-workers earlier in the week that a non-work related injury to his elbow had been causing him problems.
EE stated he removed belt framing from belt trailer. He dropped a piece of belt framing on top of his right foot causing swelling and appearance of a contusion to employee's entire foot. Started missing work on 3/20/08, returned to work on 3/25/08.
Employee was bending a roof bolt in the drill hole, when it twisted, causing his right shoulder to pop. This did not become reportable until 8-11-08.
Hurt his back using a pry bar on a tire. (He did not report it to anybody, we thought he had quit until he showed up to work later)
Employee is a Long-Haul Driver making a delivery and was on top of tanker using a squeegee to push the remainder of product (emulsion) out of truck. After finishing the job he attempted to stand upright and back began hurting. Strained back.

Employee was getting out of Service Truck & got his foot hung between the Cab & step. He fell & twisted left knee in the process.
First aid incident 03/19/08 when tagline came free from load employee was handling and stumbled backward into guardrail. Seen by doctor & released to regular duty that day. 03/27/2008, employee back spasm getting into vehicle. Dr states result initial incident. Instructed by doctor stay home from work Friday, 03/28/2008. Employee returned work Saturday, 03/29/2008.
Employee alleges he and another co-worker were welding at the same time. The next morning eyes were burning and he couldn't open them.
Employee was stepping over 3' sandwall, stepped down an a 10""x10"" timber on the front of his foot, causing the calf muscle to stretch.
Employee reported: Attempting to move roof bolt plates across top of machine, rib cage contacted the head of a roof bolt, twisted his body resulting in him twisting his right knee. Treating physician at Union County Hospital referred him to an orthopedic specialist. He was removed from work status until he could be treated by the specialist.
Operator was moving to check a conveyor on spread and stepped in hole. Awkward step caused pain in back, which made the employee seek medical attention.
Miner was installing a piece of ventilation tubing when the tubing fell and struck his arm against the loader bucket.
Employee was changing bits on shearer cutting drum. As he was turning drum he felt his left knee lock up. First Lost Work Day was 7/10/08 due to surgery to repair tear.
The employee was removing wear plates from the dozer blade face. The plate relieved itself of stress and the top of the plate popped off the face of the blade, hitting the employee, which knocked him off of the step he was standing on. He was examined at medical center and diagnosed with fractured pelvis, and was unable to report for his next shift.
Employee was using a bit wedge as spacer with a porta power to push out bit block insert on the longwall shearer cutting drum. As he pressured up the porta power, the wedge came out hitting him in the palm of the right hand.
HE WAS REMOVING MINER CABLE STRAP FROM THE TAIL HOOK ON THE MINER, WHEN HE WAS PINNED BETWEEN THE COAL RIB AND THE TAIL OF THE MINER.
Inured was in the process of moving Miner cable on 3-18-08, left work without reporting incident. Injured had a doctor?s visit on 04/23/08 with Dr. Frederick Armburst in which he was diagnosed with a intervertebral Disk Injury that was musculoligamentous in origin.
Employee was lifting 45lbs wet hole bag from truck and was placing it into the drill hole (blast hole) Doing so Employees back began to hurt.
Mashed two fingers between drill and granite block.
A length of pipe 116"" fell from a 30"" high pipe stand and landed on the workers right foot.
Employee was stepping down from grader. He said his right hand gave away and let him fall onto the corner of blade and circle of the grader.

On 3/17/2008 the employee was stepping down the steps of a ladder on a belt cross-over. His foot became caught in the step causing him to fall to the bottom twisting his back. He continued to work until 3/26/2008 at which time he started missing due to back pain.

EE was tramming the miner to grease it, the tail of the miner was swung toward him. The EE split the cuts on the miner, pushing him into the rib with the tail. The EE was taken to have X-rays. The X-ray revealed a cracked pelvis.
EE was getting a piece of metal, he dropped it and it hit the sode of his left foot and ankle area.
A longwall production employee sustained a fracture to his left tibia and right ankle. He was standing on #12 support and the shield was being pulled forward. As the shield pulled forward the employees lower legs were pinched between a rock which was on the floor and a base-lift cylinder. He is awaiting an ortho appointment.
Truck driver was stepping across from the left side of the cab deck to the right side of the cab deck to perform a fluid check for his pre-shift inspection, when he missed a step with his left foot causing him to fall to the ground.

The EE stated he was hanging 2"" water line and stepped on a rock and turned or twisted his left ankle. He had injured his right leg a week ealier by pulling himself off a ladder and favoring his right leg by putting all his weight on his left leg when this event occurred.
EE had Joy 10 SC at #3 entry intersection taking support rail from reel compartment. When end of rail fell onto mine floor, other end struck top of right foot causing injury. EE was taken to ER, x-ray was positive.

Employee was operating the tailgate end of the longwall shearer on the. 3 west off 3 cool hill longwall section. A stone was thrown by the rotating drum of the shearer. The stone struck the ee on his left hand. His left small finger was fractured. He was wearing metacarpal gloves stone was the size of a football. He was standing by shearer body.
Employee was doing some housekeeping and as he went to lift a car battery off of his vehicle he pulled a muscle in his back.
EE was checking and airing tires, alot of snow and ice on ground, EE slipped and fell and hurt left hip.
EE's were setting new Ibeam support in plant. Ibeam was being supported by come-a-long and strap at top and was caught at bottom by 1/2"" plate. Ibeam stopped at top by another Ibeam, EE attempted to dislodge it by lifting on it in a crouched position and threw out his back. EE complained of pain and layed on floor. EE was placed on backboard and taken to hospital.
The utility person on the section went to the miner to relieve the miner operator for a lunch break, when the right outby rib rolled out, which caught the utility person in the back of the legs. This caused him to fall to the mine floor. Rib roll measured 3' X 7' X up to 7"" thick.
While entering a mantrip, EE rotated his body to sit down and his right foot got stuck causing him to twist his right knee. He began missing work due to his knee injury on April 3, 2008 making this a reportable injury.
EE strained his back when lifting on the miner cable during a power move. He continued working his regular job until 3/18/08, his first day missed.
The injured was escourting MSHA inspector, he slipped in some mud, as he fell he caught himself with his right hand injuring his right wrist. The incident was reported but no medical treatment was sought. On 4/22/2008 the injured had x-rays that revealed a partially healed fracture. No work missed until surgery performed on 5/15/2008.
Employee was changing shuttle car tire when piece of coal entered right eye. He missed work on march 14, 2008, with infection in eye.
Employee climbed a ladder in the sampling room to check on a plugged crusher. While working from the ladder employee allegedly twisted her knee and back.
EE was getting off top of bridge, got in cables on side. Getting out of cables with rock caught leg in the cables and seemed to have twisted knee area. Was taken to ER by ambulance.

EE was operating a Joy 21 SC Shuttle Car behind a continuous miner. A lump of coal approx 2"" X 2"" X 2"" was thrown from the boom of the miner and struck him on the left eye.
EE had climbed over the gob plate to enter the E-17 Longwall Tailgate Entry and as he went to take a step on the pile of rock, his left foot caught between the rock and twisted his left knee. The EE continued to work until 3/19/08 when he began missing work due to his injury.

Employee was attempting to dislodge a pinner steel that was hung in the roof. He for some unknown reason had his hand at the steel that was in the roof when he accidently hit his boom lever in the upward position causing him to mash his left hand between the roof and the bolter pot.
Injured had layed down a piece of drill steel on the power center. There was a burr on the piece of steel that lacerated his left hand little finger. Drill steel was taken out of service. (Injured was taken to the hospital where they put 3 stitches in it to close the wound. by 220080840022)
Injured was riding in portal bus #33. His right knee was outside of the bus and struck a fire valve along the West Mains track. Causing a contusion to the right knee.
Employee was respotting at the shovel when the load from the shovel bucket fell onto the dove tail of the truck, causing the driver of the truck to be tossed inside the cab.
Working on crusher using a slate bar, prying on roller to put bearing on. Slate bar slipped causing employee to pull left shoulder chest muscle.
EE was moving a 12x12x8 timber out of a pick-up when he claims to have twisted wrong and pinched a nerve in his back.
He was coming down ladder off of the highwall miner when his feet slipped causing him to fall and strike his right side on the base of the frame ladder. He fell 4' & fractured 1 rib on his right side.
The employee was shoveling belt and struck his neck and head against a roof bolt plate. He started missing work on March 20, 2008.
Moving flock tank when rigging on container turned causing tote tank to flip and strike EE knocking him to the ground. Injury to his right arm. Not scheduled to return to accident site, this was a capital project and was completed.
Employee states that while he was in the process of placing a crib block under a trailer being lifted by a loader, the forks of the loader slipped out from under the trailer and struck him on the inside of his right leg. He sustained a femur fracture of the right leg.
Employee was operating track-hoe excavator on a pad of coal, prying rock off coal and loading rock trucks. Track of excavator got off pad, causing excavator to fall off pad (drop of aprox. 8 feet) and over onto its side. Employee was in cab (selt belt on) when excavator hit the ground, causing injury to his neck and bruising to his head, right elbow, and buttock areas.
While setting steel deck plate into position, contractor lost control of plate, allowing plate to shift/slide into ladder. This sudden movement dislodged worker off ladder, individual fell approx. 6'8"" to ground.
EE was helping dump garbage into dumpster when container slipped backwards away from dumpster. When attempting to grab container, the full weight came back on right arm, causing tendon to snap. One day lost time was required for surgery.
Driver climber on top of the right rear tire on trailer of semitruck to unlatch a tarp strap sliped off the tire, EE's foot landed on a piece of coal and twisted her ankle which blew out her knee.
While working near crusher 4 cleaning the area, employee pulled on the water hose and strained his lower back.
Employee was working on dissassembled bed of a rock truck that was on the ground. He stepped off the bed to the ground, stepped on a rock and twisted his ankle. First aid was applied and he was transported to the hospital. He was diagnosed as having a chip fracture of his right ankle.
The EE bent half-way over with the intention of picking up a crib and felt pain in his low back.
While pulling a water line the victim slipped and fell. He sustained a fractured fibula in his left leg.

EE was standing on the ladder of the maintainer when his knee popped and began to hurt.
Employee was operating a grinder. Employee switched hands on the grinder, his glove got caught on the grinder wheel pulling the employee's finger into the grinder.
While driving a shuttle car it hit a hole in roadway, causing EE to be thrown up into the canopy, hurting his neck and shoulder.
Ran over crib block when he was running a scoop. When it bounced off the crib block, it bounced him up and down in the scoop.
EE was stacking pallets 10 high, 41 lbs each and he felt pain in right side.
EE was in W28 face installing roof bolt, after tightening bolt, let pot down on his right knee.
Employee stated that while operating the loader he felt a sharp pain in his lower back.
EE was fueling and doing maintence Auger, he was removing coal from belly pan. Equipment was wet from rain and possible fuel and other lube. EE slipped on the wet surface and fell into belly pan, bruisung his back.
EE was mounting a doxer when his footing slipped and he fell, while holding onto the handrail. EE complained of lower back pain at the end of his shift, EE was diagnosed with a sprain to his lower back.
The EE was setting bits on the miner. He had one bit that was hard to get out, so he was using both hands to swing the hammer to get it out. He swung really hard and hit the punch hammer to knock it out. A piece of metal broke off, coming back and striking the EE in the left eye. The piece of metal came over the top of his safety glasses that had slide down.
Climbing onto truck, foot slipped off step. Whie holding on with right hand, wrenched right shoulder.
EE slipped and twisted left knee. EE continued to work until today 4/29/08, he will undergo surgery.
The employee was loading sections of 4"" waterline when the pipe suddenly turned. When the employee attempted to stop it, he felt pain in his low back. The incident became reportable on April 9, 2008 when medical evidence revealed that the employee would be missing work.
EE was coming out of the MCC on the 3rd floor of GR3 plant and looking up, when he tripped over a pump/motor that is welded to the floor. He fell on his right side. He was off 3/10,3/11, and 3/12 due to his injury. Returned to work with dr note 3/14.

After working hours employee stayed at company parking lot to perform oil change in his personal car. He was using a hydraulic jack and wood pieces block equipment from movement. The hydraulic jack failed and employee was pinned in the chest between car and floor. The security guard helped him lift the car back and employee got up normally.
EMPLOYEE ACTIVATED ROOF BOLTER CANOPY RAISE LEVER. AS IT ROSE, IT HUNG UP ON A BOLT AND THE STRAIN CAUSED THE CANOPY TO FALL, STRIKING EMPLOYEE'S RIGHT ELBOW.
employee was helping to roll a track rail over. One of the bars being used to turn the rail broke causing the rail to move suddenly. This caused the bar the employee was holding to spring back towards him striking him on the left side of the face. (Fractured jaw and laceration to neck)
Employee was walking backwards while operating an overhead crane when she lost her balance. This caused the employee to fall and hurt her left wrist.
Employee was inserting a five foot roof bolt with plate. The plate slid down the bolt striking his left hand and thumb, possible fracture.
Victim alledgedly was trying to pull a shield off a scoop when he strained his back. A co-worker says the victim told him he was going fishing with a buddy when he left the mine on the 7th. This report if being filed however the status of the ""injury"" is still in question as to whether it actually happened or not.
Employee state that after he cleaned out the chute at the head of the plant feed belt, he stepped down from the grating and his foot slipped causing him to fall. He caught himself with his right hand causing injury. He began missing work on 3/13/08.

Climbing up ladder of #4 bin of stone plant. When he was near top, ladder twisted around. He climbed down back side of the ladder by his hands. His left knee hit something when the ladder twisted around, and has been hurting ever since.
Employee was working on top of feeder (030101) 4th right section. When employee was getting down from the feeder he jumped the last two feet, lost his footing and hyper-extended his left shoulder. Employee is a mechanic and carries many of his tools on his mine belt which contributed to the amount of weight pulling on his body. Injury has now required surgery.
EE was helping load the scoop charger into scoop bucket, when he inadvertentlt placed left foot under charger and charger set down onto foot. EE sustained a simple fracture of a bone in the left foot.
A hernia to left side of abdomen occurred when lifting and splitting stone.
Reported: Working on continuous miner section, preparing to tram miner back to face area when a small piece of draw rock fell striking his head, shoulder and lower back. The piece that struck his head contacted his safety glasses bending the frame and cutting the bridge of his nose. Wound required 5 sutures to close.
While dumping load from truck bed, the bed fell off when raised to full extension of the first jack.
While Employee was traveling down the stairs between Dryer #1 and #2, the Employee's heel caught on the stairs. This slip caused the Employee to loose his balance and footing. The employee was able to catch himself before he tumbled down the stairs. The employee felt the muscles in his back tighten up
A rock flew out of the jaw of the crusher, hit employee in left temple. He fell on his back about 6 to 8 ft to the ground.

Victim was backing miner out of #1 entry to go to #2 face. He stopped the miner as it rounded a corner to move a cable out of the way. As he bent over to get the cable, he hit the tram levers with his abdomen. The miner struck his upper thigh and pelvic area. He underwent surgery to repair a dislocated hip and fracture to the hip socket.
Employee was adjusting dirt shoe spring; he was down on his hand and knees. He turned the wrench once and he felt his back go out.
Employee twisted sideways to pick up a box of resin and felt a pain in the center of his back. Due to complications he started missing work on March 12 2008.
EE climbed down the ladder of the 980g front end loader. When he reached the ground he turned and his knee gave out. There were no other objects or substances involved. It is my understanding that he was in a motorcycle accident several years ago and injured that knee.

Employee was crawling across a conveyor belt cross-over platform. The left handrail broke causing the employee to strike the left side of his face on the frame of the platform.
Moving excavator to lower level, when it started sliding to the left side. It came in contact with loose soil and sunk up, letting the machine roll over on its top.
The EE heard a noise and he went down to check the machine, in the distance there was a hydraulic hammer, a stone came out and impacted his forehead.
Employee stated that while shaking socks, employee slipped and caught himself on railing, injuring his back and knee. Cause was undetermined until 3/17/08, at which time this became a reportable injury.
EE was helping to remove a piece of plywood from the bed of the pup trailer. It cames loose and landed on top of his left foot. Possible broken bone in top of foot (swelling).
The cutter head on the miner failed. So the operator backed it out of the cut. EE began removing the rub-rail to gain access in the control box. He tried to sit the rail over the miner cable. Something pull in his shoulder. He was diagnosed with a portial tear of his rotator cuff. Note: he was not diagnosed until 3/24/08.
Employee stated that a sock fell on him but that he didn't feel any pain. He stated on 3/6/08 at 8pm that he had suffered an injury the prior day. Employee was considered a reportable injury on 3/12/08 after an investigation and medical evaluations.
The employee was touring the mine following experienced miner training; traveled through a check curtain;slipped and twisted his knee. He started missing time on 6/9/08 from the injury of 3/5/08 and became reportable on 6/9/08.
The employee was working outby the longwall face at a shield repair station. He was trying to remove a side shield pin. The pin is spring loaded and when he rotated pin it came out and smashed his right index finger and left thumb against the side shield. Fracture to tip of right finger and lost nail by left thumb.
Employee was squatting down washing out from under a conveyor, when a slab of rock fell seven feet from the conveyor, striking him on the left shoulder. Employee was checked out at hospital and released and was referred to an orthopedist to have shoulder evaluated. *employee was not able to return to work today making this a lost time incident.
Employee was lifting waterline & carrying it to mantrip when he lowered it down he felt a strain in his lower back.
Feeder having trouble.EE changed the plug to a different supply which then blew 2 fuses and made a conveyor trip.Emptied the feeder by hand rolling the sheave. EE decided that while the guys were fixing the feeder he would go work on conveyor.He forgot they had moved the plug on the feeder.He started the back half of crusher unit,which made the feeder power up.
Miners were adjusting the timing of gates on the Lambert Palletizer Model 1200. Miners must be on equipment while in operation to make these adjustments. The guard had been removed from the left gate. The effected miner's left foot was pinned between the gate and the shaft when the gates were opened.
Employee was repairing the load out chute from a manlift. He was lowering the chute to the manlift basket level, supporting it with both of his arms. He asked the manlift operator to move the manlift forward. The operator moved too far forward and caught him between the chute and the handrail of the manlift basket, straining his back.
EE was setting timbers, when his back popped. He went to Jenkins Hospital, gave him an excuse to RTW on 3/6/08. He came in on 3/7/08 with another excuse to remain off work.
Employee was riding in a locomotive that was approaching another locomotive. He though they were going to collide (they did not) and he jumped out of the locomotive.
EE, a longwall propman attempted to lower a shield on the longwall face. A valve soleniod malfunctioned, which caused the pull ram to activate prematurely, tightening up a chain which broke and a link from the chain struck EE on the leg and caused a hair line fracture.
While palletizing flat rock miner slipped/ tripped over another rock and injured left ankle. Conditions were wet that day.
Employee was removing trash from underground crushing system. He twisted and felt pain in his back. Physician determined employee had a rib fracture. He did not return to work on his next scheduled shift.
Employee got cement over top of boots, did not wash out until end of shift, causing cement burns.
Injured was making a repair to a small pump cable. While making a cut on the cable, the cable and knife slipped and the knife struck the top of his left thumb. This caused a laceration of 1/2"".
Miner was using a chuck wrench to knock a hole in a stopping. As he swung the wrench, it hit a cable and deflected the wrench and it hit his hand, causing a fracture to08 his finger.
While bolting in #6 entry, a piece of draw rock fell out and struck him on the left ankle. The piece of rock was about 24"" X 36"" X 1""-4"" thick. The rock fell ut between roof bolts. EE returned to work for 4 days and stated he was advised not to work, due to possible fx.

Employee was getting out of a truck, jumped to ground, acute lumbar sprain.
Injured employee was operating a 992G endloader loading rock trucks. He was loading parallel with the shot, undercutting the shot material and some rock rolled into the cab resulting in contusions to the left hand and right knee. He had been instructed on two previous occassins to load perpendicular to the shot material.
Magnet was still engaged when worker attempted to retrieve steel. His right thumb was caught between the piece of steel and the magnet.
Employee was inside the drum of the CC Centrifuge #2 installing some bolts when the unit was inadvertantly energized. Employee received cuts on his left hand and left knee which required stitches.
Employee alleged that he lifted the lid to a trash dumpster and strained his shoulder. He did his normal job until 03/03/08 when he had surgery.

The employee had built a warming fire in a small stove in a building for warming on the Quarry. While operating a drill, the employee noticed a box of rags next to the stove were on fire. The employee used a dry chemical extinguisher to put the fire out, but was overcome by the dry chemical fumes.
EE STATES WAS REMOVING A HYDRAULIC HOSE, WHILE PULLING ON THE WRENCH, FELT A PULL TO LEFT GROIN AREA, BURNING SENSATION, DIAGNOSED AS A HERNIA, SURGERY PENDING.
A Komatsu 465 Haul Truck loaded with stripping material fell 33' from a haul road. The weather was cool and clear and the road conditions were dry and in good condition. While operating the truck, the driver veered across the haul road about 219' before proceeding through the berm an additional 29' then falling 33' coming to rest upside down.
The accident occurred in the slope approx. 1260' from the surface when the examiner was heading down the slope in a Wallace 6 man diesel mantrip. The injured lost control of the vehicle, and struck on of the supporting arches with the front drivers side of the mantrip. The injured walked to the surface and called for help.
