Accident type, without injuries

A planned fall occurrence on the #4 section in the #3 entry at spad #44702. Falling outby for 240 feet and 8 feet high. The area started working about 11am, the section foreman stopped loading coal to build crib around the area. The fall occurrence about 130pm, the area was bolted with 5 foot torqued tension bolts.

A roof fall was found in the E1-bolt on preshift, area is approx. 1000' inby the working section fall measures approx. 20' wide x 350' long 10' high. Area was bolted with 6'-42"" fully grouted bolts and cable bolts in some intersection (10').

A pillar fall occurred on the MMU 004-0 in the #20 crosscut in the #5 pillar, catching the continuous miner, size of the fall undetermined. No injuries, ventilation was not interrupted.

A bolt became loose in the magnetic door latch that activates the latch mechanism. As the elevator was being raised from the bottom of the shaft (no one was on the elevator) the latch vibrated loose, causing the elevator to stop 26 feet above the bottom of the shaft. Elevator was out of service for approximately one hour before repairs were completed.

An unintentional fall of roof occurred on the 2nd South panel, off the 2nd West Sub Main, off the 4th South Sub-Main in front of #4 seal. The area was bolted with 6 & 10 foot point anchor roof bolts. The roof was fractured stack rock. The fall will not be cleaned up. A seal plan will be sumitted for this area.

The drip pan under the slope belt fell on top of the slope track resulting in blocking the slope cat and pulled the wire out of the conduit. The hoist was down from 12:32 pm to 6:20 pm. MSHA was on surface at time of accident.

An unplanned roof fall was discovered in the return air course in a room panel off 11 Right Main. This fall is located 60 feet inby spad #5323. It is approximately 10 feet high and 200 feet long. The area was supported with 42 inch fully grouted resin rods. This area was dangered off. The fall did not affect the ventilation at this mine.

A roof fall, 20'x20'x5' thick, occurred in the No. 1 entry of the Cazy mains 2 breaks inby spad 13050. The area has been breakered off.

While cutting with a torch on the feeder in the Raw Coal Tunnel at the Preparation Plant, an ignition occurred. A flame ignited and then immediately went out.

A spoil slide occurred at 2:45pm on 2/28/08, in the Oak Hill Mine DII Area pit. The slide was approx 250' wide and 125' across the pit to the toe of the highwall. The slide blocked in a euclid hauler and a caterpillar loader. The operators were able to walk out of the pit. No injuries and no equipment damage. MSHA was notified. Investigator arrived at 3:45pm.

A unplanned roof fall occurred in the # 4 entry on the 001 section at S.S. # 5694. The fall was 1250 ft. from the face. The fall will be rehabilitated and the size was 30ft in length X 19ft. in width and app. 10 ft. in height. The area was inspected by M.S.H.A inspectors and State.

A roof fall was found during a weekly examination of the seconday escapeway. The fall was called outside at 8:57 am. It is located in the old dollar branch works located in the #4 entry and break #7. It is approx. 7 ft. thick by 25ft. by 45 ft. Additional support will be added to the area.

A roof fall was found during a weekly examination of the mine. The fall was found at 10:00 am. and was called out when the person making the examination got to a mine phone. The fall was located in the south mains break 81, entry #4. The fall is approx. 7 ft thick by 20 ft. wide by 40 ft. long.

At around 8:00 AM an unintentional roof fall was found in the intake airway by the No.3 beltline.

A roof fall was found by an examiner while examining outlying faces away from the working section. It was located at 5+15 #6 Entry of Zone Sub 2. The area was developed 1-27-08 & was abandoned 2-1-08. The fall was 20'L x 18'6"" W x 8'H. The area was originally supported w/6' fully grouted resin bolts. No injuries, air flow not disturbed, passage of men not affected.

A roof fall occurred in the 7 East Panel at crosscut #31 in the intersection of #3 entry measuring approximately 18ft. x 18ft. x 5ft. to 7ft. high. The area was supported with 5ft. fully grouted roof bolts and 9ft. mega bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the 2 west 4 (#1 unit) @ 31 crosscut, #4 entry. The fall measures approx. 18' long, 18' wide and 12' thick. The area is supported with 5' fully grouted resin bolts & 10' point anchor bolts.

An unplanned roof fall occurred on the 3 right longwall headgate on 2/26/08. The fall occurred in the #3 entry at approximately 56+65 for a length of 65 feet. The event made the headgate of the longwall impassable and was immediately reported to MSHA and WVOMHST.

An unintentional roof fall occurred on the East Submain at station 23+10 Entry #8. The fall measured 26 feet x 20 feet x 5 feet. The fall did not affect ventilation, passage of men or cause pesonal injury. The fall will not be cleaned up. Approximate date of development was June of 2007.

An unintentional roof fall occurred on the Main North at station 87+26 in the #2 entry. The fall measured 30 foot x 18 foot x 5.5 foot. The fall did not affect ventilation, passage of men or cause personal injury. The fall will not be cleaned up. Approximate date of development was October 1992.

A roof fall was discovered in the 2 left panel off main east. The fall was in the NO. 2 return entry about 30' to the right of survey station NO. 236 and measured approximately 20' long by 15' wide by 6' high. The fall was approximately 1,400' ouby the working section and will not be cleaned up. There were no injuries or equipment involved.

A small fire was observed on the bull gear of the # 5 Rotary Kiln. The fire was immediately extinguished by 8:15 am. The fire was due to a build up of grease around the gear. Directions received from the local MSHA office is to report all fires.

A roof fall occurred at #20 belt head on E-panel submains 1-North. Laminated shale rock fell from 6'-10'-12'. Area bolted with 6' resin bolts & header boards & 12' cable bolts with monster mats. Cover 415' sect. was moved outby affected area. Cribbed & dangered off. Called hotline.

A roof fall was discoverd at #13xc, #8 entry in the south E panel. No equipement was trapped, ventilation was not affected, the passage of men or material was not affected. The fall was 17ft wide, 17ft long and 7ft high. The intersection was bolted wirh 5ft fully grouted bolts

A roof fall occurred in the 2 west 1 mains, #4 entry, 39 crosscut. The fall measures approx. 20' long, 20' wide & 12' thick. The area is supported with 5' fully grouted resin bolts.

Escape Hoist went down with a Hydraulic Pump in Zone 4. Moved the mine crews over to Zone 2 and Zone 7 where the Escape Hoist was working, until we could get our hoist running in Zone 4. Got the hoist back up at 11:30 PM that night and the mine crew was able to return to Zone 4. No miner trapped or injured.

An unintentional roof fall occurred at IO Coal Co Europa Mine. The fall occurred on 3 left section in No 4 entry 1 break inby Span No 5141. The fall measured 120' long, 15 feet wide, and approximately 6 to 8 feet thick. The fall had been cribbed off on all sides. No injuries occurred from this fall.

An unintentional roof fall occurred at the Galatia Complex New Era Portal at the #1 Entry, 2W Headgate, Crosscut 28, 4025'. The fall was approx 35' long, approx 18 1/2'-19' wide and approx 9-10' thick. The fall did not affect ventilation or passage of personnel. The fall will be cleaned up.

Unintentional fall of roof measuring 60' x 20' x 6 1/2'. This fall is 7000 ft from any working section and does not involve any equipment or escapeways. This fall will not be cleaned up and has been cribbed and dangered off.

An unplanned roof fall occurred in #130 mine straight behind 6 tail in the return at spad #6097 (2 brks inby). The fall measured 8' high, 30' long and 20' in width. No injuries were reported. The area was developed on 10-12-06.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 ENTRY, BETWEEN X-CUTS #6 & #7. THE FALL MEASURED APPROXIMATELY 25'Lx18'Wx6'H. IT SHOULD BE NOTED THAT A 6.3 EARTHQUAKE OCCURRED ON THE UTAH-NEVADA BORDER AT THE SAME TIME WHICH WAS FELT THROUGHOUT MOST OF THE WESTERN STATES. THE HORIZON MINE BELIVES THIS FALL WAS A DIRECT RESULT OF THE EARTHQUAKE.

The mine encountered a roof fall on the 5th Northwest panel, entry #5, SS 8+10. The fall was in the primary escapeway and measured 30' X 17' X 7' thick. There was no equipment involved, no personal injury, did not affect ventilation. The area was developed on 1/2/2008, and supported with 6' fully grouted bolts. This fall will not be cleaned up.

Roof fall, face of #4 entry of the 1-right submain at what would be the intersection of the #9 x-cut back outby to just inby the #8 x-cut intersection. Fall approximately 40 feet long, 19 feet wide, by 6 feet high. No one injured, no equiptment involved.

A roof fall occurred in the 1st South Mains, #1 entry, between 20+80 and 21+60. The fall measured 60'x 19'x 7' and was caused by an undetected slip in the mine roof. The area was mined in July 2006 and supported with 6' tension rebar roof bolts.

Roof fall found by examiner the return air courses of MMU-001. Fall was located at 15+30 #1 entry of panel #1 zone. Area had been isolated with timbers prior to fall. The fall was 50'Lx18'Wx15'H. Area was supported with 5'fully grouted resin bolts. Area was developed on 1/29/08. No inuries/block of air flow or passage.

A roof fall occured in the 2nd south Sub Main at x cut 50 in entry #4. The fall did not affect travel, ventilation or cause injury. The fall measured 20' long, 18' wide and 9' high. It will not be cleaned up. No one is required to work or travel in the entry.

An unintentional roof fall occurred on the 1 East Mains in the #2 entry adjacent to SS# 278. The fall measured 25'L x 25""W x 10' high. There were no injuries, no equipment was caught and ventilation was not disturbed. This fall will not be cleaned up.

A roof fall occurred in the 2 west 4, #3 crosscut, between #5 & #6 entries. The fall measures approximately 15 feet long, 15 feet wide, and 5 feet thick. The area is supported with 5 foot fully grouted resin bolts.

An increase in seepage from the west embankment of our D cell trilings pond was observed. Upon further investigation, a void near the toe of the downstream slope on the west embankment was discovered; resutling in corrective action-including pumping, trenching, diking and diverting water from the plant. Cause still under investigation.

At approximately 11:00 p.m. a limit switch on the brake linkage broke causing the man cage to be taken out of service. The linkage assembly was changed out and the cage was placed back into service at 1:00 a.m. on 2/19/08.

EE found roof fall on #4 south panel while retreaving thing left behind. He transned off the area around and dangered it off. We will net clean this up it don't affect the mines in anyway. Closest spad 1024.

#3 hoist kicked on check point fault. When reset, the drive convertors kicked out. A contactor was replaced so the hoist could be reset.

I found a roof fall in the E2 butt approx 2 xcuts inby spad #3911. Fall is approx 2 1/2 xcuts long x 19' wide x 6-8' high.

Retreat mining in the #5 entry pillar block when the roof started working in the previously mined #4 entry block. The roof fell in the #4 entry and took out the breaker timbers. The timbers were replaced and mining continued. The roof began working again, mining ceased. The roof in the #4 entry fell again, the fall maesured approx. 80'L x 22'W x 5'H. The entry was timbered off.

An unplanned roof fall occurred in the #130 mine on #5 belt, brk. 25 near spad #5295. The fall measured 40' long, 20' wide and 10' high. No injuries were reported. The area was developed 3-31-06.

Overtravel switch on #2 hoist stuck. Once the problem was determined, it was found that the hoist was still operable in case of emergency.

A roof fall occured in the 7 East Panel in the #4 entry in the intersection of crosscut #25. The fall measured approximately 19'x19'x6'. The area was supported with 5' fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occured in the 15 North Panel at crosscut #10 in the pillar line between #6 anb #7 entries. The fall measured approximately 30'x18'x6'. The area was supported with 5' fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

#3 Service hoist was overloaded when a heavy motor was placed in the cage for transport. The safety controls on the hoist prevented conveyance movement. The motor was removed, the cage and rope connections inspected, and a round trip shaft conveyance test successfully performed before the hoist was returned to service at 6:00 PM. The mine crew left the mine via the Decline.

While traveling the return on the way to section found a roof fall in an abandoned area of the mine. Did not affect ventilation and no one injured. The fall was about 4' thick, 20' wide & about 45' long.

Location: Track; Entry: Left brk. off #5 spad No.: 9453 belt entry; Distance from active face: 4,500 ft. No personal injuries or equip, involved. Size: L - 20'; W - 18'; H - 6 - 10'. Type and length of roof support: Resin; Several cribs installed prior to fall. Strata: shale; All entrances are breakered off; Area will be rehabilitated.

Skip door did not close properly after dumping. As skip was entering shaft, skip door was torn off and fell approximately 140 feet down shaft. Main hoist was not usable until skip door was removed from shaft. Small man hoist was usable.

A fall of the roof occurred on the 6th 54 Supply Road. The fall measured 20' L X 18' W X 6' H. No ventilation, equipment or personell was affected.

A fall of roof occurred at the intersection, in # 7 entry at station # 13+50 of the 1st East. The fall was approximately 30 feet x 18 feet x 10-12 feet. No equipment, ventilation or personnel were affected. The area was developed in September 2007 and had been supported with 6 foot fully grouted roof bolts.

A roof fall was found 80' L x 19' W x undetermined H at 90' outby station 2513 in #2 entry along 8 belt. This fall does not impede ventilation or travel.

An unintentional roof fall occurred in the 4 North Mains #3 entry at crosscut #96 at SS #18557. The fall measured 25'L x 25'W x 10'H. No one was injured, ventilation was not impaired and no equipment was caught. This fall will not be cleaned up. The area surrounding the fall was cribbed off.

A methane gas ignition occurred on the #1 section at approximately 7:30 am. The continuous miner had cut approximately 20 feet out of the face area in the #3 left cross cut and was cutting in the top when the methane ignition occurred.

An unplanned roof fall occurred in the #130 mine on the #1 section 50' inby spad #8246. The fall measured 8' in height, 20' in width and 55' in length. The area was developed 2-8-08. No injuries were reported.

Mining was stopped in the area 45 days before the fall occurred because of weight problems. There is a coal seam elevation in the area. It occurred inby spad #874 & 882. It is examined weekly.

A roof fall occurred on MMU-001 at the intersection of 2+30 in entry #1 of the sub #1 panel #1. The fall pulled the 6' fully grouted bolts. The fall measured approx. 18'Lx18'Wx10'H. The fall occurred in the return 1800' out-by the working section.

Intersection 0 Entry of North Main Return. Fell above the belt anchorage approximately 20' X 30'. There was no impairment of ventilation, there is one other entry unaffected by the Fall.

On 2/12/2008, the entire plant and mine lost electrical power from the supplier. The power outage caused loss of power to the vertical hoist at the mine, however the back diesel power kicked on immediately, and the hoist was operable again. The mine was evacuated at that time. Full power has been restored.

1 1b conveyor belt started on fire. Not sure of exact cause at this point. Belt stopped and motor kept running. Fire Dept was called. Fire lasted appoximatly 30 minutes. No one was injured or entrapped.

A roof fall was discovered in the 2 North 3 Mains, #4 entry, #37 crosscut. The fall measures approx 80' long X 18' wide X 9' thick. The area is supported with 5' fully grouted resin bolts and 10' point anchor bolts.

Roof fall was found in the No. 1 entry of South Mains B return (outby) at 34 x-cut. Fall did not obstruct ventilation or escapeways. Fall will not be cleaned up and is supported over top. Size of fall was approx. 25 feet long by 14 feet wide and 7 to 10 feet high. Bolted with 8' Install II bolts and point anchor resin. Cause was deteriorating roof conditions.

A rock fall was discovered in the pillar inby spad 1041 in the #3 left entry outby the working section. The fall is approximately 5' high X 15' long X 20' wide. The fall was discovered at 1:40 pm. The area has been dangered and cribbed off. The fall did not have any affect on persons, ventilation nor equipment.

A contractor was reteiving some drag line parts from an abandon truck shop. One of the contrator employees was attempting to disengage a parts rack with a cutting torch. Sparks from the torch caught the back wall on fire. The fire quickly spread to the roof and was immediately out of control. Local fire departments were summoned, but the building was a complete loss.

While restarting the #5 Raymond coal mill at approx. 10:15 AM, an unplanned ignition occurred from a small amount of coal dust left inside the emply mill. There was no damage to the damper control system and the fan housing. The event wasn't reported until 2/8/08 at which time it was called into MSHA. There were no injuries and no fire as a result of this ignition.

Operator experienced an unplanned inundation of Hydrogen Sulfide in the upper and lower levels of the V-512, this is an area where miners are working at times. There is no one known to be injured or ill because of this release. The operator did not realize that this incident was an actual inundation at that time.

Fall down stairs

Truck Driver/Customer(350 pounds plus)lost his balance & fell down stairs (older man who just got off light duty work status for his company. Still dealing with high blood pressure.) He was hospitalized and transferred to a rehabilitation center where he died on May 10, 2008.

EE traveled to get backsight in order to advance survey station in the #5 entry of the 2nd Butt Right off of South Mains section. The backsight was outby a run-thru check curtain. He was struck by a loaded shuttle car as it traveled from the #6 face to the feeder through the run-thru check, and incurred fatal injuries. He was found by the shuttle car operator on his return trip.

Drowning

The victim was using an excavator to free a dragline from an ice-covered pit floor. The excavator traveled over an ice-covered, 10 feet deep ditch and broke through the ice. The victim was ejected and was recovered from the water about 6 hours after accident.

Contact with electrical current

Boom truck being used to set concrete safety barriers beside driveway at the request of MSHA inspector. Boom assy. on truck had approx. 18' clearance under overhead power line. Operator of truck and deceased had just moved truck and reset up to have more clearance under the wires. On first lift the boom was extended too far and made contact with wires. Day was clear, dry and sunny.

Struck by flying object

The victim (mechanic) removed fuel injector from engine and then instructed his helper to turn the engine over. Diesel fuel and/or air came out of the 1/4 inch injector hole and struck him in the left eye and entered his brain, causing fatal injury.

Employee was assisting with the retorque of a 1 1/2"" Cap screw and re-agrivated and old hernial injury.

On 6/12/2008 the EE went to see a surgeon and was diagnosed with a hernia. On 6/24/2008 the injury was determined to be work related. To date the EE has not missed any time from work.

While employee was greasing the bearing on the rotor of the crusher, he turned it too far thereby catching his middle finger between the blow bar and side of crusher. This resulted in a severed middle finger from the 1st knuckle to tip of finger.

He opened up the guard and stuck his Right hand in, past bearing for the tail pulley, with a pair of Channel locks to clean. Injured had received load onto his 10 S/C Joy Shuttle Car. Injured carried load to feeder and began discharging load, when a rock fell over sideboard, down into operators deck, striking his left little finger, which was on steering lever. Injured sustained a broken finger, with several lacerations.

Employee was raking coating off of the cooler grates of #6 Kiln. While raking, the kiln puffed back from positive pressure inside the kiln, causing dust to fly out of the door, where he was raking from.

Employee was filling a tote bin with Nitrogen that would be used for loading blast holes. The 2000 lb bag of nitrogen was lifted over the bin and the miner cut a hole in the bottom of the bag to allow nitrogen to flow into the bin. As the material began to flow out of the bag, it shifted and pinned the worker to the top edge of the tote, causing spinal and other injuries.

Employee was making repairs to the #5 Belt Drive. Employee stated that he was climbing down off the speed reducer when he stepped to the ground; he stepped on a small rock causing him to twist his left knee.

EE was operating a skid steer loader, assisting another EE with a blast. Injured EE dismounted skid steer loader, he stepped on an uneven surface/object, severely spraining his right ankle.

Employee was tasked with checking and documenting the water level at east dam. It had rained excessively (1.25"") that day. In performing task, employee slipped on wet 2A mod, went to catch self, and in doing so twisted right knee. Employee did not seek treatment at this time, reported as First Aid - Employee requested & sought treatment 3/31/08.

Miner was carrying belt structure and complained of knee pain. Note: Began missing work on 2/20/08.

Injured worker (employed by ABZ Transport LLC, Contractor ID# A706), reported that he slipped on some ice striking his right arm and hurting his back. He then got back into his personal vehicle and went home. Weather conditions at the T.O.A. were high winds, cold temps., sleet / rain combination.

Employee jumped from his mechanic's truck to put a fire out and when he returned to his truck he noticed his knee was hurting. He went to his doctor and his doctor told him to be off for 2 weeks.

Struck by... (Not Elsewhere Classified)

Original DOI was 11/09/08, when the grinding disc broke and smashed R. thumb. No treatment at that time. Over the next 3 months a bump appeared, so he sought treatment for a bone spur. He had surgery to remove it on 3/12/08 and returned to work on 3/13/08.

Fall from ladders

Employee states while surveying, he was dismounting a ladder, missed his footing and his foot slid to the floor. He felt a pop and pain in his left knee upon impact. Employee continued to work until he had knee surgery on 2/27/08.

Fall onto or against objects

Employee was exiting the cab when he slipped on the floor causing him to fall forward and hit his head and left shoulder on the gear box frame.

Fall to the walkway or working surface

The concrete, non-enclosed walkway from the bulk shack was slick and sloped down hill. The employee slipped on the clay-coated surface and fell hurting his right elbow.

Over-exertion in lifting objects

Two employees were moving a piece of floor grating (4'x10') weighing approximately 130 lbs. One employee noticed a pain in his back after this task. This was initially diagnosed as a slight strain, after a month it has not improved, employee has been taken off work and scheduled for back surgery.

Fall from ladders

Ee was on a ladder on uneven ground hanging bolter cable when a leg on the ladder bent sending him to ground and twisting his ankle. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.
