Accident type, without injuries

A roof fall was discovered 70' outby spad 521 in the #2 entry of the snd sw mains. The fall was apx. 20'long x 18'wide x 64 "" thick. The fall did not affect persons, equipment nor ventilation.

An unintentional fall occured on the weekly aircouse in the #7 main north entry at 63+80'. The fall was 25' X 20' X 11', this area had 6' tension rebar and had supplemental 10' cable bolts in it.

The Lilley controller lost syncronization with the PLC controller. The brake switch and Lilley controller was out of adjustment. The mine initated a routine evacuation due to temporary loss of secondary means of egress. MSHA was notified at 9:50pm. 23 people were underground at the time of the failure. The hoist was returned to service at 12:25am May 1st 2008.

Brake caliper spring discovered to be broken on the northside brake drum counterweight linkage. Mine initiated a routine mine evacuation to effect repairs due to temporary loss of second means of egress. There were 49 employees UG at the time of the evacuation. MSHA notified at 11:07am. Hoist returned to service at 1:00PM.

An unintentional roof fall was discovered in front of the #5 seal, At #4 seal, this fall is 60' over from spad #1780, time of fall is unknown, but fell sometime between 4/22/08 and 4/29/08. THe size of fall is 6""thick x 20Long x 20 Wide. the roof in this area is 3'slate with laminated sandstone above slate.

A roof fall occurred in #8 intersection in the 5 north return across from spad #44679 in #9 entry. The area was bolted with 5 foot torque tension bolts. The fall was about 20' by 20' and 6' to 8' high. The fall was cribed and danger off and will not be cleaned up.

Our weekly examiners found a roof fall in a worked out area of the 3 Left 4 West panel. The fall was located adjacent to their route of travel into the worked out area. The fall was located between crosscut 42 and 43 in entry #2.

Roof fall occurred in worked-out 2-Left off 3-South beltline just inby spad 3511. The approximate fall dimensions are 650' x 20' x unknown height. The roof in this area was shale with no visible defects. Cable bolts were installed in every test hole. Fall was a possible result of valley/stream influence.

Unintentional roof fall discovered this time and date. Location of fall: No. 2 Section (MMU 002-0), No. 1 entry, station 2618. Fall measured approximately 110'LX18'WX6'-8'H. Area previously supported with 6' full grout resin bolts. Overburden is near 160'. Area mined on or about April 10, 2008.

While traveling the airways I found a roof fall 1 break inby spad 303. It had fallen about 4' thick 12' wide and 70' long. I check test holes around it found no other cracks came outside reported the fall and went back timered and dangered fell off hill seems and water look like cause fall.

#3 hoist tripped on ""PM Vibration trip"" error code. It was determined that a switch was bad. THe switch was replaced and the hoist was running at 1740, there were still two operable hoists available.

Koepe Lilley Controller Overwind Out of Adjustment. Called for routine mine evacuation due to loss of secondary means of egress. MSHA notified at 5:14pm. There were 18 people underground at the time of the evacuation. The hoist was returned to service at 5:50pm April 25th 2008.

Unplanned roof fall in the #1 entry on the 001 section measuring 160' in length X 18' in widthX8'-10' in height. The fall was immediatley reported and all entrences were breakered off. This area will not be rehabilitated.

The west brake canister packing gland failed preventing adequate air pressure on the pressure side to activate the spring release before the PLC controller timed out. MSHA was notified at 6:35pm. There were 17 employees underground at the time of the evacuation. The mine evacuation was complete at 7:35pm. The hoist was returned to service at 10:20pm 04/25/2008.

A fall of the mine roof occurred near spad #2669 the size of the fall was 50'Lx20'Wx0-6'H. There was no injuries and no airways were blocked.

An unplanned roof fall occurred in the #2 section inby spad # 1747. Fall measured 70' long X 20' wide X 8' high. No equipment or persons were involved and ventilation was not affected. The fall will not be rehabilitated, all approaches have been breakered off.

A surface brush fire was discovered on mine property near the Neely Pump. Pictorial evidence gathered the following day indicated that vandals may have been burning off the insulation to a section of pump cable. Evidence from nearby spectators indicate two young men on 4-Wheelers left the scene just moments earlier. The nearby fire department extinguished the brush fire.

An unplanned roof fall occurred in the #2 gas mine, one brk outby spad #417 in the return air course. The fall measured approximately 50' in length, 18' in width and 16' in height. The area was developed on 1/21/08. No injuries were reported.

Zero methane check. Employees were 150 feet above fire. Hot embers fell down shaft, caught bleeder on fire. Took about 3 minutes to extinquish fire. Called in. On D order until inspector came out. Resumed work approximately midnight.

When the day shift production crew arrived at the section, the boss discovered a small roof fall at approx 7:50am. The fall measured 10' long, 6' tall and 6' wide.

Our examiners found a roof fall during their weekly examination of the 4 West return. The fall was located at crosscut 153 from entry 1 to 0. The fall was approximately 70 feet long, 18 feet wide, by 9 foot high.

An examiner found a roof fall in the 1 Left, 4 Main North panel. The fall was located in the primary escapeway for #1 unit. The section was shut down all persons were evacuated. The fall was located at crosscut 16 between entries 5 and 7.

An unintentional fall occurred above bolt anchorage. The fall was in the face of #1 entry at the 21+70'2nd west entry's.

An unintentional roof fall was discovered in the #7 entry 85' in by spad 1366, this is the bleeder airway. Fall was estimated to be 18-19' wide, 6-7' high and length could not be determined. No injuries, no equipment caught, travelway to check bleeder system was blocked. Area to fall was breakered off. Fall will not be cleaned up.

Unintentional roof fall was found at 3:30pm on 4/21/08 at X-Cut #11 on #2 belt #1 entry (intake). Fall occurred in intersection. The roof fall was 4' high, 20' wide and 20' long.

An unplaned roof fall occured in the #4 entry app.2100 ft. from the working face. The size of the fall was 50' in lengthX 20' in width X 9' in height. The fall was inspected by MSHA and the WVOMHS&T. This area was supported with 6' torque tension bolts and was rehabilitated.

During pre-shift testing, #3 Service Hoist Computer recorded an inadequate brake test. The hoist was removed from service. No persons were underground. Further investigation revealed a failed hoist servo. When repairs are completed and hoist fully tested and found safe, it will be returned to service.

A brush and grass fire was observed on mine property and was called in to the emergency dispatcher. The Lemon Springs Fire Department was dispatched and was on scene in 17 minutes. The fire was controlled and the responding units left in one hour and 46 minutes. The fire was immediatly reported to MSHA. The cause of the fire was not determined.

EE's reported to work and discovered the electircal trailer and control tower was on fire. Supervisor called fire department and MSHA then.

MMU-001-0, #8 right entry While cutting coal a possible gas or dust ignition occured. A small orange glow which went away immediately was observed by the miner operator. There were no injuries. There was a kettle bottom that had been cut into.

A roof fall measuring approximately 20' x 30' x 6' high occurred at break 25 in No. 5 neutral entry, 15,000 ft. from the face.

Roof fall on #3 belt, brk 14. Fall was 14"" long and approx. 2-3"" thick. no injuries. No ventilation was affected. Passage of men not affected fall was cleaned up area rehabilited. MSHA notificed via hotline.

The mine encountered a roof fall on the 3rd MNW, S.S 66+10, XC between entries #3 and #4. The fall measured 35' L X 19' W X 8' thick. The fall DID NOT affect ventilation, passage of men, no personal injury, no property damage. This fall will not be cleaned up. The area was developed on 11/15/2006, with 8' point anchor bolts installed.

A pumper found a roof fall while in the 5 West intake. He found a roof fall at crosscut 93 in entry #7. The fall was located in the intersection and was more like a pot out because it was only 4 foot high. It left a number of 6 foot bolts in the roof but pulled a few out of the center of the intersection.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY MSHA INSPECTOR AND SAFETY DIRECTOR IN THE 2ND WEST MAINS, #6 ENTRY, BETWEEN X-CUTS 46 & 47. THE FALL MEASURES APPROX. 25'Lx18'Wx5'-6'H. MSHA WAS NOTIFIED AT 1:20 PM. THE AREA WAS DANGERED OFF.

Blasting a spillway ditch for a new pond when grass caught fire. The blast holes were 4"" in diameter and 10 feet deep. The grass and leaf fire was extinguished in approximately one hour. There were no injuries and no equipment damage.

A roof fall was found in the South Mains Break # 86 the fall extended from the cross cut in #1 entending into the crosscut in #3 entry. the fall is approx. 6ft. thick by 20ft wide by 140 long.

The brake limit switch for the Koepe lily hoist access cage had moved due to vibration, causing the cage to stop. It took time to reposition them and adjust the brake cannister. Mine evacuated; no problems; no injuries. 15 persons in the mine. Hoist back in service at 6:15am prior to completion of evacuation.

Unintentional roof fall on section 2 at pillar block #43, 1 x-cut inby spad 3309. Stopped mining and moved back two full rows of pillars and restarted second minng.

A roof fall was discovered when the foreman was doing his weekly airways. The fall occurred in panel No. 6 and ran from #4 ehtry through the crosscut into #5 entry outby spad #1975. The fall measured 80'L x 20'W x 6'H. There were no injuries or equipment damage. The area had been bolted with 42"" resin bolts.

An unintentional roof fall occurred at the Galatia Mine New Era Portal on the 2W Headgated 6539' West, #2 Entry. The fall was approx 30' long, approx 18-19' wide and approx 8-9' thick. The fall did not affect ventilation or passage of EE's. The fall will be cleaned up.

The tipping valves (airlocks) feedling limestone from the conveyor belt above the kiln hood failed in the open position allowing heat from the kiln hood to travel up the chute to the conveyor causing the conveyor belt to catch fire.

1 cc inby spad 243 on north east mains, #7 entry fell in for app. 70' & 7 rt fell in for app 15', (7rt was in 1 cut and not bolted). The intersection of #7 did not fall. Additional support had been installed. Both approaches to the fall area have been breakered off crib blocks.

A fall of roof occurred at the intersection, in # 4 entry, # 14 xc of the South East Travelway. The fall was approximately 20 feet in length x 20 feet in width x 10 feet thick. The area was developed in april 2004 and had been supported with 6 foot fully grouted roof bolts.

An unplanned roof fall occurred at break 151 spad 1630. Fall measured 80'L by 20'W and 4-8'H. No persons or equipment was involved. Ventilation was not affected,a clean up plan has been submitted to MSHA.

Between 2:00 P.M. & 10:00 P.M. a roof fall occurred at #7 belt, entry #4 spad # 18683. Fall was 30' X 19' X 5'6"" high. All entrances were breakered off, MSHA-D. Fannin & OMSL-W. Taylor were notified. Ventiltaion and passage of persons was not affected. Area will be rehabilitated, cleaned, additonal support installed.

A roof fall occurred in the East Mains in #6 entry in the pillarline between crosscut #26 and crosscut #27 measuring 20ft. x 18ft. x 8ft.-9ft. high. This area was supported with 9ft. mega bolts.

Unknown electrical problem stopped the production hoist. Extensive investigation revealed a bolt holding an electrical contact had ""stripped"" and pulled out, causing contactor to be displaced from normal position. Mine evacuated; no injuries; no problems. 21 persons underground.

#3 Hoist tripped out with a slack rope indication at approx. 8:45 p.m. Then at 9:30 p.m. #2 Hoist stopped with a brake release problem. Mine evacuation began at 9:40 p.m. At 12:15 a.m. on 4/12/08, both hoists were repaired and ready for service.

#2 hoist kicked out due to the 250 volt power supply. The 250 volt DC coil shorted out. Electricians were called out of the mine to replace the coil. The hoist was back in service at 1205am.

While checking a place that I had dangered off earlier in the week, I found that it had fallen. About 90' long 20' wide adn 5' thick. There was hill seams and water coming out of the top.

During a regular daily inspection a necessary repair needed to be made to a pully that would require that the elevator be out of service for more than 30 min. The repairs were made and the elevator was placed back in service.

Unplanned roof fall in 1st north main at spad # 784 in Return Air Course. The fall was 30' wide x 30' long and 5' high. The roof was bolted with 4' fully grouted resin bolts at the time of the fall. Set post to all access to fall.

A fire developed on the upper most elevation of the preparation plant being constructed in Brookwood, AL for Covol Engineered Fuels. It is assumed that slag from a cutting torch unknowingly fell inside a polyethylene pipe, allowing it to smolder and eventually catch fire from the inside of the pipe(s). MSHA's 800 number was called - report # 1-52227611.

The double boom drill was drilling a crosscut from 1E13 to 2E1, when the left drill steel came in contact with some undetected explosives in a bootleg. The charge detonated and damaged the stinger cylinder, knocked the hammer off the rail and shattered the front windshield.

Roof fell between #1 & #2 Entry near spad # 13640 in the Spruce Laurel Mains. The fall was 30' x 20' x 5' thick and will not be cleaned up. Will set cribs, build new stoppings and re-route life line.

The Main East primary escapeway in Entry 2 between 7 and 8 crosscut was blocked due to roof slough age. Material fell out around roof bolts pulling the plates and heads off of the roof bolts. The affected area is approximately 20ftx16ftx3.5ft. The area will be cleaned up and resupported.

An unplanned ignition ocurred on the 4 right section, #2 heading at the face. The ignition was confined to a small area in the face and was extinguished in less than 20 seconds using a water hose supplied on the continuous miner.

A roof fall occurred in the South Sub Mains at 9Xcut in entery #7. The fall measured 25' long, 25' wide and 9' high. The fall did not affect ventilation, travel or cause injury. It will not be cleaned up.

A piece of rock fell on the miner cable behind the standoff. It was determined that a fall did not cause the miner to be stuck but the cable was grounded. This is not a reportable roof fall.

There was a roof fall discovered, measuring approx 120' long X 20' wide X 6' high.

The overtravel limit switch went bad in the shaft for the Production Hoist. The hoist had to be stopped for one hour while the switch was replaced. The Production Shaft is the second escapeway out of the mine so the mine had to be evacuated. The switch was replaced within one hour and the personnel returned underground.

An unintentional fall of the roof on MMU 009-0 longwall in the belt entry at spad #100+40 at #10 cx outby the face. The fall measured 20'long x 19' wide and 3 to 4' thick. Both MSHA and State agencies were notified.

Excessive rain caused the highwall above the belt portal entry to erode. The additional weight of the wet material on the canopy caused the portal canopy to partially collapse onto the belt conveyor. An electrical problem took the Koepe (access) cage out of service and mine was evacuated after initial troubleshooting. Final investigation revealed a switch at the Koepe shaft bottom had burnt in the ""open"" position. While trying to find problem, the lily hoist controls got out of adjustment. Back in operation at 3:45am. No injuries. Evacuation - no problems.

A 270' long fall was discovered in the bleeder entry next to the gob line of the S H Panel. The fall is in the intersection of #3, 4, 5,6 and 7 entry and possible the crosscuts between tem, this is the #18 X C of the panel. The bleeder in #19 XC is still open, ventilation has not been affected. This is an abandoned area where mining has been completed.

The final shuttle car was being loaded from the face of the place, when a piece of roof bolted rock measuring about 71"" long X 39"" X 18"" thick fell, striking the victim. The injuries received from the occurance were fatal.

We fill this report with prejudice and that the indicient was not reported to supervisory personnell.

The ee was jacking a bus back on track. The bus was raised with a lifting jack & the ee & co-worker were using a post as a pry bar to push the bus toward the rail. Cat-scan revealed a torn meniscus.

He was climbing up a four foot ladder, carrying a 2 h.p. air compressor. His back distorted, he lost his balance and fell on his back on a concrete floor. He suffered from a concussion, minor back injury and a contusion to his left shoulder. He has a history of back injuries.

Injured had xrays done. No signs of broken bone. Ankle is sprained and will go back to the doctor Monday 04/28/08.

While EE was dismounting his dozer he stepped on a rock, causing him to fall to the ground. He complained of back pain and numbness in one leg. He returned to his regular job the following day. During Tree cutting operation for a spoil fill area in advance of mining operation. An employee received multible non-fatal injuries when another employee felled a tree. Then the tree struck the injured employee who was working down slope of the felled tree.

After replacing the air filter and fuel filter the tech started to step from the left rear fender to the rear push block. Employee was moving a highwall drill to a new location when one track ran over a rock in the roadway. When the drill crossed the rock it caused a sudden bounce to the employee. He was released from the medical facility with no restrictions to return to work. He returned to the medical facility on 4/25 and was taken off work because of swelling of the disk of the spine

He was putting a return roller back up under the over load conveyor, when he felt a sharp pain in his neck. Came to work on 4/22/08 and reported it and went to ER that day.

EE states that back was injured at another location in 4/08.While driving the ANFO truck on 4-21 he hit a small pothole & pain went up his back & neck.On 4/25 treatment was started for muscle spasms,on 5-1 notified supervisor that he would be off work & had been put on steroids.Still off work to date.

Employee was a Roof Bolter and stated that he was picking up and loading some roof plates onto his boom tray, while his buddy was installing a corner bolt. When the installation was done that person went back and let the ATRS down a little, causing the bolter to roll backwards onto the victims foot.

Injured was operating a scoop while pushing a disabled scoop to the charger. The scoop he was operating slipped off the disabled scoop after he had ran up ontop of it causing him to hit the canopy when it fell off. Injured is taking treatments for a compressed disc in his neck.

Miner was handling port steel cable on a barge located at a barge loading facility. Cable had settled behind a raised steel plate. When the switchboat tightned up the starboard cable, the port cable became unlodged and struck miner in the ankle, resulting in an acute tendon strain.

This employee, along with 4 others, was hoisting a pump. The wire rope connected to the pump got wrapped around a rock on the ground. When it came loose it snapped and struck this employee across the upper chest. He was then lifted up and fell backwards down over the edge of the mine pit the pump was being lifted out of.

On 4/18/08, EE was climbing up on miner and slipped, hitting right knee on rub rail. Note: EE was injured on the 18th of April, he finished his shift and worked on the 19th. He went ot ER at SMH on 20th. They took him off work starting the 21st.

When opening the top to gain access to the pinned bag the electrical power automatically shuts down as a safety measure but apparently the air lines maintained pressure causing the rake to move foward when he was removing the bag causing him an open wound.

Surveyor was pulling wooden lath out of back of pick-up truck. He got a splinter from the lath in the side of his left hand below the pinky finger. CMO was place changing the miner from #1 to #2 entry while positioned on the inby side of the intersection in #2 as he tramed the miner backwards towards #3 entry. As the machine went by he placed his right foot between the cable and the head to protect the cable and his foot was pulled under the head. He fell backward and fractured his r. tiba and fibula near his ankle.

On 3/21/08 EE stepped from under the cone belt to the over screen, when his feet went out from under him. He used his right arm to catch himself. When he got up he had pain in his shoulder and trouble raising his right arm. He continued working that day and full time until surgery on April 17th, 2008. As discussed with MSHA this injury becomes reportable on surgery date.

Ee was rock dusting the belt line by hand when he heard something pop in his shoulder. After doctor visits, it has been determined he pulled a muscle slightly away from the bone. No surgery required at present. Physical therapy should take care of the problem.

EE was walking up the # 2 entry.he was moving a block out of the road way a Fairchild scoop loaded with a slinger type duster came up the entry the operator was unaware of him, presence and the duster struck and pushed him to the ground injuring his leg.It was not determined until 4-26-08 that he was a lost time accident. While cleaning up the ramp into the pit, a 992 Loader bumped the right rear of a 773 end dump truck parked close to a 2nd end dump facing opposite directions. The truck operators were conversing through open doors and the bumped truck bumped into the 2nd truck causing the open door to slam on the 2nd truck operator's upper arm and caused a strain/contusion to the arm.

The operator of the 121 haul truck pulled into the K86 to delay for a blast. The operator exited the cable to observe the blast, and leaned on the grid box and received an electrical shock. He was taken to the ER and taken off work for 2 days. Currently Ongoing.

When pulling suction out of pond the right rear tire went up on the berm causing to much weight to front of the83 boom and the loose dirt landing sluffing causing the83 to tip resulting in a roll over EE then made the decision to unbuckle and jump causing him to land on his head resulting in a c2 fracture and a fracture to right wrist.

He fell approx 3-5 feet onto ""X"" bracing and slid down to top of classifier another 4-5 feet.

Maint. foreman and co-worker were changing out a shield leg and when he pressurized the leg a 10 mm plug blew out striking him on his lower leg (calf). Cause of this injury was removing the staple lock to the plug. Compliance with rules/regs was not a factor, miner proficiency a factor, protective equip not a factor, mining systems could be a factor.

Injured was in the face of the longwall removing tools used to drill face. He was at #111 shield when the face and draw rock 8' wide gave way falling into the conveyor striking him on his left shoulder. This caused a hairline fracture to his left shoulder area.

Employee was walking behind the roof bolter in the #1 entry and a scoop came up the #1 entry and hit employee in the legs with the scoop bucket below the knee. Employee did not hear scoop due to noise from the roof bolter.

Employee was dragging rock dust hose, caught toe on wedge under rock dust and fell. Arm landed on wedge at the bottom of timber approx 12"" to 14"" above floor, hurting right shoulder. Employee to undergo rotator cuff surgery.

Struck by falling object

The employee was installing an iron plate inside a Cement Mill. Accidentally the plate slipped by few inches hitting his left ankle and foot. The iron plate was located at the Cement mill's floor

Victim was operating a continuous miner via remote control, when he failed to see a piece of roof rock that had sagged, and the rock fell, striking him on the head, resulting in neck and back pain. He is expected to be back to work on 4/7/08, and suffered no apparent injury. this info is the latest info we have at this time.

Fall to the walkway or working surface

The EE stated that he was stepping across a 4"" pipe on the floor. He stepped on a mat used for a jog station. The mat had mud on the surface, whick caused the EE's feet to move out from under him, resulting in a fall.

Struck against a moving object

Track excavator tracked too close to edge of access ramp to working pad. Machine slid sideways 5' onto its side. Ramp and working pad with berms were over excavated. Minor injury to operator. Minimal damage to equipment.

Struck by flying object

Employee was installing input shaft in rotary head, using hammer to install bearing on pinion shaft. A piece of metal chipped from bearing and went into the left eye. He was taken by ambulance to medical center, then to hospital for surgery. He has been instructed to remain out of work for a least 14 days to recover, unsure of prognosis from that point.
