Accident type, without injuries

There was some smoke leaving the #2 coal tank that couldn't be seen from the ground. We notified MSHA at their 800 number to report a possible fire in our #2 coal tank. We then ran a fire hose up to the top of the coal hopper from the fire hydrant. The foreman and a laborer flooded the coal inside the tank with fogging spray.

Our examiners found a roof fall on the 4 West at crosscut 150 between entries 4 and 5. The fall was located in a crosscut in the intake. The fall did not extend into an intersection. The fall was in an entry adjacent to the examiners route of travel.

IGNITION--The Continuous Miner was cutting in the No 4 Entry. While cutting in the middle of the face, an ignition occurred. The ignition lasted less than a minute according to the Miner Opr. Used wash down hose and sprays to put fire out.

During normal operation of the hoist. The hoist stopped running and would not reset. The code given was another brake solenoid in fault. Repairs were made changing one of the main brake accumulators, and solenoid reset. Hoist has resumed normal operation.

IGNITION-- The Longwall Shearer Machine was trying to cut a large rock at No. 101 Shield when the H/G Shearer Opr. saw a flash around the drum of the Shearer. When the machine and drum was cut off the flash went out. It lasted approximately 30 seconds.

Fall of material from the back on the level 1 to level 2 decline most of the material that fell was caught in the mesh on the back. The loose ground in the mesh above the decline impeded use of the primary escapeway

An unintentional roof fall occurred in No.4 entry, 100 ft inby spad 12371, H3 belt, approximately 20 ft wide x 45 ft long x 5 1/2' to 7 ft thick. Roof support in the area is 5 foot fully grouted roof bolts.

An unintentional roof fall occurred in the Main South Belt Entry at the 122+15 foot mark. The fall occurred due to the fact that the roof strata broke just above the anchor point. The fall area was originally supported with 4,5, and 6 foot conventional type roof bolts. Approaches to the fall area will be reinforced, fall area cleaned up and the fall area supported.

An unintentional roof fall occurred in the 10 I longwall Tailgate entry. The fall occurred in a three way intersection approx 8 ft high by 40 ft long by 20 ft wide. There was no injury or equipment damage with this occurrence.

Our examiners found a roof fall in the 3 Left 4 Main North return at crosscut 35, entry 2 to 3. The fall was located in the crosscut and did not fall in an intersection. The fall was adjacent to the examiners route of travel.

A roof fall occurred in the 2 west 4 mains @ 39 crosscut, between # 4 and # 5 entries. The fall measures approximately 20 feet long, 18 feet wide and 5' thick. The area is supported with 5 foot fully grouted tension rebar bolts.

Two mechanics were on an airboat at the tailings dam reclaim area pulling a barge that was hooked up to the front of the airboat. The front of the airboat tilted downward and started taking on water too fast to pump out. The water caused the boat to flip over on the right side. The employees managed to climb on top of the left side of the boat that was not submerged.

Unintentional roof fall discovered this date. Fall measured approximately 18'W X 60'L X 10'H. Area previously supported with torque tension roof bolts and cable bolts. Area was developed on or about 4-08-05.

# 29 crosscut fell in approx. 8' to 9' thick. No equipment involved. Crib off all four corners on 1st right mains, moved mandoor to # 28 break.

FALL OCCURRED 5 BREAKS INBY THE #2 HEAD DRIVE IN THE #6 & 7 ENTRIES IN THE INTAKE ENTRIES. THE FALL WAS 10' THICK, 80' LONG, AND 20' WIDE. THIS AREA HAD BEEN SUPPORTED WITH 4' FULLY GROUTED ROOF BOLTS.

An unplanned roof fall occurred on the 2nd West Sub-Main, off the 5th North Sub-Main, off the 4th West Sub-main in entry #5 at cross cut #4. The fall was on the supply road and will be cleaned up. The area was roof bolted with 6' and 10' pins. The fall appeared to be fractured stack rock.

Material fell from roof of Tunnel A, near 14 glory hole. Tunnel is a retrieval operation. Discovered at 11:30am, time of fall is unknown.

A roof fall accurred in the #9 seam in the 1 West Panel at crosscut #53 in the intersection of #6 entry measuring approximately 18ft. x 18ft. x 9ft. high. This area was supported by 5ft. fully grouted roof bolts and 9 ft. mega bolts.

While stripping dirt from spoil side with 2570 dragline the spoil became unstable. There was no equipment damage or injuries due to this event. The reason for reporting is that it interrupted coal haulage for over one hour.

A roof fall occurred on 004-0 MMU in the #3 entry at spad # 3013. The fall measured 6ft. in height, 20ft. long, and 18ft. wide. Persons, ventilation, nor equipment were affected. This area will not be rehabilitated.

An unintentional roof fall occurred at the galatia mine new era portal on inby crosscut 27+3865', 2 West longwall beltline. The fall was approximately 37 feet long, approx. 18 feet wide, and approx. 10 feet thick. the fall did not affect ventilation. The fall will be cleaned up.

During normal operation the Main Hoist stopped operating giving a halt signal message on the computer for solenoid brake switch in halt. The problem was identified and solenoid relay reset. Hoist has been operating as normal.

A fire was reported by a nearby land owner on some mine land that was reclaimed years ago and now is leased to an individual who raises cattle on it. This is not near the active mining area. Members of Luminant management were notified about the fire about 5:00pm and by 5:30pm it had been put out by the local fire department. In all it burned the under brush on about 6 acres.

The #8 elevator was inoperable for a period of time exceeding 30 minutes due to failure of the RBO coil. The RBO coil was replaced and the elevator placed back into service @ 5:15 PM. No individuals were on the elevator at the time of the failure.

Unplanned roof fall occurred at 6:00 AM on 9/27/08 in B-Mains section. Fall is located 40' inby Station No. 1735 in #1 Entry of B-Mains. Area is used as an intake airway. Fall is 80'L & at least 6' above coal seam. Area was supported w/6' fully grouted resin bolts & 10' cable bolts. All approaches were timbered off. Ventilation was not interrupted. No escapeways involved.

A roof fall was discovered in the 2 west 4, crosscut 47, # 9 entry. The fall measures approximately 25 feet long, 20 feet wide and 7.5 feet thick. The area is supported with 5 foot torque tension rebar bolts.

A fall of roof occurred in the South East Travelway, # 3 entry, # 18 cross cut. The fall was approximately 30 feet in length x 25 feet in width x 15 feet thick. The area was supported with 6 foot fully grouted roof bolts.

Cutting coal in the No. 1 Entry on No. 8 Section and while cutting coal on left sidt of entry, an ignition occurred. Ventilation was disrupted by the Ramcar just prior to the ignition. Fire was put out with a fire extinguisher. The ignition lasted approximately 30 seconds.

A roof fall approximately 30' wide x 35' long x 7' high fell around 2:30am at the #8 intersection in the 003 section return airway. This fall will be dangered off and will not be cleaned up.

An unintentional roof fall occurred in our bleeder system for our MMU 003 section. The area didn't affect the bleeder ventilation quanities, but travel will have to be rerouted. Fall occurred in x-cut inby spad 3546, starting in the # 5 entry and extending to the # 7 entry. This fall was found during the weekly examination.

During a routine check, a guide cable was discovered to be pulled loose off of a ground plate. All miners were evacuated from the mine. After the guide cable was repaired and operational, miners returned to the mine at 10:25 am.

An unintentional roof fall occurred in the # 1 entry of 6-Mains starting at SS# 3907 extending to SS# 3847 and continuing inby SS# 3847 into the #4 entry of 6-Left panel for approximately 80'. The fall dimensions were approximately 18' w x 6' h x 260' long.

The left side miner MMU-001 was mining coal in the face of #1 entry and cut a hole approx 2' X 3' in the upper left corner of face, into an old mine that was not known to be there.

An unintentional fall occurred due to adverse roof condtions above bolt anchorage. The fall was at the 23+10' in the #2 entry of the 2nd main west entry's. Fall was 25' long X 18' wide X 8' high.

1 break inby the feeder at spad #370, we had a roof fall that caught 2 shuttle cars about 4' on one of thjem and 6' on the other one. It looks as if their was a vertical crack that caused the fall.

On 9-24-2008 a dust ignition occurred in the 5th L miner section. MSHA was immediately notified and an investigation conducted. It was found that the water sprays had been plugged on several water sprays and the bits had also been worn. The company thought this had been submitted but after a review it was found the report had not been filed. This report is to correct the error.

While traveling the return entry, a mine examiner found a reportable roof fall in E-1 Headgate, entry 3, XC 41 to 42. The fall was approximately 6 feet high, 20 feet wide, and 25 feet long.

#1 Hoist Conveyance door did not fully close, conveyance became lodged in #1 compartment during hoisting cycle. MSHA notified at 0937 hrs. and crews were removed to 900 level where there is secondary egress. Secondary egress was re-established via #2 compartment at 1750 hrs. MSHA notified at 1757 hrs.

An outage of the slope hoist occurred due to an electrical problem with power for the control circuit. The outage was from approx 140pm to 223pm.

On the #1 southwest section at break #21 in the #1 and #2 entry one break outby spad #145 (spad in #2 entry), a fall occurred from the solid coal in #1 entry thru the break and 10' of the intersection in #2 entry, approx 10' high X 19' wide X 80' long.

Employee hit elevator doors with a hand cart while taking equipment off of elevator, knocked the cage door off track while at surface landing, causing the elevator to become inoperable.

At 11:25 am Mine Foreman called out to me that he had found a roof fall in the 6th West bleeder at x-cut 22 in the intersection of the #2 entry that measured 30' x 25' x 6'-7' high. Five foot resin bolts had been installed in this location. MSHA was notified at 11:30 am and a K-Order was issued verbally. Also called into 800 number at 11:33 am.

A roof fall occurred in the #4 entry at the #11 crosscut on the 3-North-1 Mains in the intake entry. The fall was 8' to 9' in thickness and measured 20' by 20'. The area was previously supported with 5' HY-Tech Bolts. The fall was cleaned up and re-supported.

An apparent power spike went through power lines of Western Electric over the weekend. When our hoist operator attempted start up on Monday, hoist did not operate. This was prior to shift starting. Through trouble shooting we were able to track down a small battery backup that needed to be replaced. The power surge caused the logic control of the hoist to fail. Program was reloaded

Roof fall found by mine repairman at 7:30 PM. Fall is on Unit 2, Entry 7 at +1820. The fall is 18 ft. wide, 4-6 ft. high, 50 ft. long. Area was supported with 5 ft. conventional bolts, and 6 ft. double lock bolts. A visible slip ran through the intersection and limestone was absent in the anchorage zone. The fall will not be cleaned up.

Hyd. line broke on 600 L Excavator. Machine was shut down for approx. 10 min. when a fire occured in the engine compartment. No one was on machine at the time. Dozer man noticed a fire and alerted everyone. MSHA was notified immediately.

A non- injury roof fall was discovered in the bleeder 45' inby SS #2557 on Panel 3A on the 003 MMU. Fall measured 30' in length, 18' wide, 5' - 6' high. The type of roof support installed in the area was 4' resin bolts, and 8' cable bolts. No equipment was involved. Ventilation was not affected.

Operator was cleaning spoils up around jaw. Right hand door on backhoe hit a protruding iron beam on upper platform on jaw, breaking the door glass and damaging the door frame, there were no injuries. He will receive a review of task training.

An unplanned roof fall was discovered on 9/22/08 in the return app. 3400ft from the #2 section measuring 30' in length x 15' in width and 6' high. The area had 8 timbers installed prior to fall, the area will not be cleaned up.

A lug on an U/G transformer shorted out causing a GFI on surface to fail. Started evacuation. Changed GFI to get Hoist operational again. Test Trips and all OK at 1:15 a.m. Talked to James Petrie.

A roof fall occurred on the #2 unit, producing section. The fall is in the #4 entry @ crosscut 6. The fall measures approximately 18 feet long, 18 feet wide and 12 feet thick. The area is supported with 5 foot torque tension rebar bolts and 12 ft cable bolts.

Employee was utilizing front end loader to load waste dust into haul truck when he noticed a glow at the rear of the truck. Employee attempted to extinguish the fire with a hand held extinguisher, but was unsuccessful. Outside emergency responders extinguished the fire.

An unintentional roof fall has occured in the 9I section, which is the 10 I Longwall tailgate. The fall occured in the 16 X-cut at SSN 38+07.48. The fall measured approx. 8 feet high 25 feet long by 16 feet wide. No injury or equipment damage to report.

Elevator was returned to service 0615 relay/coil was replaced, miners were not on the elevator during this time.

During normal operation the Main Hoist shutdown and would not run from the control switches in the Hoist House. A relay in the Wonder Ware was receiving a fault alarm in the Slack Rope circuit. Electricians found the tripped relay and the hoist was back in service in approximately 28 minutes, however the call to MSHA was made as a precaution.

A roof fall measuring 20'L X 18'W X 6'H occurred in the #2 entry 1 break outby spad # 1810 in the No. 2 SE Mains inby the #5 drive at break #3. Area was cleaned and bolted up using 5"" TT bolts.

Methane ignition occurred in the #5 face of the North Mains section. Ignition occurred under the rippers when cutting bottom & self-extinguished. No injuries, no damage to equipment & was reported as required. MSHA was onsite investigated & determined compliance with no citations issued.

A fall of roof occurred in the 1st East at 16 cross cut, # 4 entry. The fall was approximately 50 feet in length x 19 feet in width and the thickness is unknown. The area was supported width 6 foot fully grouted roof bolts.

A roof fall was found at 10:40am on September 17th, 2008 at x-cut 18 entry 4 main west. Area had been mined February 2001 and had been supported with 5' resin pins, intersection had also been truss bolted. Fall was reported because it was adjacent to travel way.

A fall of roof occurred in the South East Submain at 51 cross cut, # 3 entry. The fall was approximately 25 feet in length x 25 feet in width x 14 feet thick. The area was supported with 6 foot fully grouted roof bolts.

Roof fall located at #2 return entry 90ft inby spad #966 1500ft inby mine portal. 5ft thick x 20' wide x 20ft long.

At approximatley 4:00 p.m. while mining the #6 entry a cavity in the coal seam was encountered that contained a pocket of methane gas. As the continuous miner was backing out of the face a glow was seen at the ripper head. The gas ignited; there was no explosive force associated. The report was not filed on time due to an oversight. A citation (6641282) has been issued.

A roof fall occurred on the #1 3 North Beltline at #25 wall, #7 entry 3 north. The roof fall was above anchorage. There was no damage to equipment. The fall was 25 feet long, 20 ft wide, and 16 to 20 ft high.

The elevator at Cook's Branch was inoperable for more than 30 minutes and an employee was on the elevator. The elevator was repaired in 45 minutes.

Loader operator was clearng material when he backed into power pole causing an energized 7.7kv line to fall near the loader. Operaator was told to stay in loader until utility company came.

Rock fall on the 4th Southeast Submain. Spad Number 19+46 in #1 & 2 Entries. Size of the fall is 20 ft x 120 ft x 6' high plus. Approx. 7,700 ft from active workings.

Based on interviews conducted with miners on 9/22 and 9/23 it was determined by MSHA that an ignition of gas and/or coal dust occurred on the 4 Right Longwall section on 9/16/08.

A non-injury roof fall has occured on the 001-0 active pillar section while mining in the No 3 left panel off the No. 1 North. The roof fall occured after mining was finished catching the No 1 MRS. The mobil roof support was recovered and moved to the next block. The fall was at survey station No. 2086.

A fall of the roof 90' X 18' X 8' fell in the # 6 entry 90 feet inby SS # 1865, 5600 feet outby the face. 4' fully grouted glue bolts were on pattern in this area with 12' cable bolts as supplemental. Cause of the fall was a coal/shale slip.

A fall of roof occurred in the 4C panel at x-cut 23, spad 15+19 in the # 7 entry. the fall was approx 30ft long x 40ft wide x 6ft high.

Excessive rainfall during the previous day has allowed enough water to collect in the pit such that normal operations in the pit have been stopped. No personnel or equipment were affected as a result of this accumulation of water in the pit.

A roof fall was found at 8:45am in #7 entry one break inby spad # 3418. The fall measured 4' H x 20' L x 10' W. The area had been bolted with 4' resin bolts. There was no equipment damaged or injuries but the intake had to be re-routed around the fall.

MSHA has indicated the incident at issue constitutes an accident requiring submission of a 7000 1 Report. The Company doesn't agree with MSHA's position, but is submitting this Report to provide the information MSHA believes must be submitted. By submitting this Report the company is simply cooperating with MSHA and is not admitting the Report is in fact required.

A roof fall occurred in the #11 seam in the 2 Main North in #8 entry in the pillarline between crosscut #24 and crosscut #25 measuring appoximately 14ft. long x 4ft. wide x 30 inches high. This area was supported by 30 inch fully grouted roof bolts.

A rock fall occurred at approx 11:43 am in the belt entry at x-cut 30 on the main north. The fall was in the bottom area and was 40 x 20 x 8. The fall will be cleaned up. The plus is 21 + 36. The area was supported with 8 ft double lock bolts.

Roof fall at the 9Eto9F recovery face #2 entry, #2to#3 CC approximately 40'Lx20'Wx10'H. No affect on ventilation or escapeways, no one hurt or trapped. Area has been breakered off.

This is a non-injury accident. A D-11-R CAT dozer caught on fire and burned longer that 30 minutes. No injury to employee. Fire started between shifts, was found burning by the service man. Fire department was called and they put out the fire.

A rock fall was found in the #1 Mains, number 8 entry approximately one break outby spad number 124. The fall was in the return entry and is not blocking the escape or hindering air flow. The fall is approximately 70 feet long by 20 feet wide by 5 feet high.

The roof bolter operator created a spark when the machine set the bolt. The spark fell to the floor and ignited a small methane bleed. The mechanic (sitting on the borer miner in front of the bolter) and the bolter helper sprayed the bleed and had it out within seconds. There was no injury.

Upon inspection of the East Mains bleeder, a roof fall of approx 30' long x 14' wide was found between x-cut 10 & 11 in the #2 entry. This location is immediately west of the #1 entry at 44 x-cut on development of the 2LEM section. No witnesses. Area was previously bolted & screened with 6' resin bolts & 10' cable bolts with wire mesh.

Employee was pushing material (shot rock) from a break down shot, when his dozer went over edge of highwall. At this time accident is under investigation by MSHA and OMSL.

While working a steep backfill slope doing reclaimation work, the dozer operator was unable to maintain control, resulting in rollover of dozer approx 4-5 times. Although wearing seatbelt, a broken back and swelling of C-spine resulted, causing loss of sensation and paralysis.

The victim had been instructing persons making a belt splice. He fell through a 4 foot by 6 foot opening 28 feet to the floor below.

The employee got his left middle and ring fingers caught between the headgate frame work and the shield clevis rail. The resultant injury was a partial amputation of the end of the middle finger. Both the middle finger and the ring finger required multiple sutures.

EE states he was washing down first floor of the plant, when the clean up pump stopped working. The EE pushed the stop button, opened the cover and pulled belt to see if motor was locked up. A co-worker the reset button in the control room, causing pump to start. Injured EE's (R) hand was pinched between teh belt and pulley.

EE standing on roll feeders & partner on floor at reducer. EE prying roller east w/ 4? pry bar, stepped back slipped slightly. EE reached North w/ RT hand to stabilize self & placed hand on reciprocating carriage rail while the carriage was traveling in a reverse direction. The reciprocating carriage wheel traveled over EE RT hand severing 3rd, 4th,& 5th fingers.

Struck by falling object

When attempting to raise the top cover of the belt tail piece, it slipped out of employee's hands causing it to fall on his left hand that resulted in an injury that required amputation of the distal phalange of his left ring finger.

Employee was turning the cap that holds the oil in the trunion bearing. The cap was stuck to the shut off valve and would not release. He continued to try and twist the cap off. When it gave way he reported feeling a strong burning pain in his groin.

Employee slipped and fell while drilling with jackleg. The jackleg fell towards the rib and pinched his finger against the rib. As result of incident, employee's last digit of pinky finger on left hand was amputated.

Roof bolter operator was traming scoop and pulled into a crosscut to turn with his left hand on the scoop swivel bumper block outside the operators compartment. His middle, index and ring fingers were caught between the swivel block and were crushed. The middle and ring fingers were damaged to the base of his finger nails and had to be amputated at the base of the fingernails.

Employee was moving a heavy grinding head with a contractor. They stood it up on its side and the contractor spun it around. The employee still had his hand on it and when the head turned it pinched his finger between the head and the frame of a forklift causing the tip of his left pinky finger to be amputated.

He was plastering a brattice on the return, when plaster splashed in his right eye.

Employee was repairing conveyor drive system. Standing on conveyor system, conveyor belt moved backwards causing employee to lose balance. Employee fell backwards on conveyor and became trapped in hopper at lower end of conveyor. Medical - critical condition - possible internal injuries.

Employee was lifting rebar, twisted, and strained back.

The miner operator bent over to move the cable, when his chest activated the boom swing on the remote box and pushed his left leg into the rib.

EE had just finished taking off covers of the miner to do p.m. work, as he finished up his rountine work he stepped backwards and tripped on the covers causing him to fall on his side.

Piece of iron stock fell from lid of crusher while doing maintence and hit EE on L elbow. The EE was attempting to beat a bolt loose with a piece of steel, EE said he was fine , but foreman offered to take EE to ER, but he refused. Foremen insisted and EE get it checked out by DR. Foreman called everyday till EE went to see DR.

Pushing a wheelbarrow of muck up an overcast ramp and felt pain in his right calf muscle.

Tramming shuttlecar and it struck a hole. He was jostled in the operators compartment and struck the seat and injured his back.

Follow up on 10/01/08 resulted in a full duty release for 10/02/08, and employee missed one scheduled work day due to the doctor visit.

Employee was spotting for the pulling of a 1000-foot piece of disconnected trail cable. The employee who was standing near the cable was struck in both knees when the cable became tight and then dislodged, requiring surgery to the left knee and possible future repairs to the right knee.

Railcar # NS 152963 had uneven surface on discharge chute. Employee hit the uneven area with a sledge hammer. The hammer glanced off and hit employee on top of right safety shoe. This resulted in swelling and pain when standing or walking.

Employee was holding a piece of monorail in a tight area when it was being repaired. It slipped causing him to twist his knee. *Employee did not become reportable lost time until 11/5/08.

Fall from machine

EE was dismounting the rear end of the international grease truck, when he slipped and fell to the ground, catching himself with his right hand. This resulted in a strain/sprain to the arm and shoulder.

Struck by rolling or sliding object

EE was operating a continuous miner on 603 section. He was backing the miner out of a right hand lift when a rib rolled off and struck him in the back, knocking him to the ground. The rock was measured to be 8' x 3' x 19"".

Fall to the walkway or working surface

While walking on rocks the employee slipped and fell, striking his left knee on the rocks, causing pain. Transported to hospital for X-rays and no fracture identified. Advised not to work until released by physician.

Over-exertion in pulling or pushing objects

EE had been pulling on a chain fall for several days and on this day, he started to complain about his arm hurting and going numb. We found out later that he had a pre-existing arm injury we were unaware of.

Fall down stairs

EE was walking down the stairway, when he slipped on the second step from the top of the lanch of the HWM. As he fell, his leg went under him, causing his right knee to twist. He then contacted his supervisor and was transported to Boone Mem hospital, where he was diagnosed as having torn ligaments in his right knee.
