AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #2 X-CUT, BETWEEN THE #1 & #2 ENTRIES OF 2ND NORTH (NORTH-SUB). THE FALL WAS APPROX. 40'L x 18'W x 5.5'H. THE FALL OCCURRED IN AN AREA THAT HAD BEEN BOLTED WITH 5' ROOF BOLTS PRIOR TO THE OPERATOR VOLUNTARILY INSTALLING LONGER ROOF BOLTS.

A unplanned roof fall was discovered at S.S.# 3162 to S.S.# 3192, the size of the fall measured approx. 140' in length X 18' in width X 8' high. The area was dangered off and timbers were set. This area will be rehabilitated.
A ROOF FALL OCCURED AT THE 9TH XCUT ON THE #3 BELT IN THE NUMBER 5,6 AND 7 ENTRIES. THIS FALL OCCURRED SOME TIME ON THE 3RD SHIFT AFTER COAL PRODUCTION CEASED ON THE 2ND SHIFT. THE FALL MEASURED 20' LONG, 68' WIDE AND 5' TO 8' THICK.
While mining through a barrier block between two panels a cut through occurred causing a water inudation.
Longwall shearer was mining at No. 17 shield when an ignition occurred under the panline at No. 8 shield.
The elevator servicing #6 shaft became inoperable when the limit switch malfunctioned. The switch was repaired.
A roof fall was found on the South Mains ""D"" mainline belt at 28 x-cut. Fall was approx. 30' long by 26' wide by 7 to 8' high. Fall did not obstruct ventilation or escapeways. Area bolted with 8' Install III bolts with resin assisted with supplemental support consisting of bacon straps, wood cribs and 4"" H-beams. Cause of this fall was weathering of the roof.
A roof fall of approx 30'L X 20'W X 5.5'H was discovered at 8:30am by the Mine examiner. The falls location was 3 breaks from the bottom of the slope in the return.
An unintentional roof fall occurred in the old 1A panel at station 36+40 entry #1. The fall did not affect ventilation, affect passage of men of cause personal injury. The fall measured 30feet x 18feet x 6feet high. The fall will not be cleaned up. Appoximate date of development was 7/20/2008.
Rock fall occurred on 5 West off 1 North Mains at break #32 in the 3 left crosscut. Fall measured approximately 20' wide x 25' long x 5' thick.
A roof fall occurred in the #9 seam in the 2 Main East, #9 entry, 1/2 the intersection of xc 19 and extending through the pillar line towards xc 20 and from 1/2 the intersection of xc 19 through the pillar line towards #10 entry. The fall measured approximately 60' x 18'x 6'-9'. An undetected slip was determined to be the cause of the fall.
An unintentional roof fall occurred at the Galatia Mine New Future Portal at the Main West intake travelway Entry #4 between 2974 W and 3099 W, crosscut 22.5, the fall was approx 23 feet long, approx. 18 ft wide and approx. 8-10 feet thick. the fall will be cleaned up.
An unintentional roof fall occurred on the old 1A panel at station 35+70 between entries 3 and 4. The fall did not affect ventilation, passage of men ot cause personal injury. The fall measured 18ft x 18ft x 6ft high. THe fall will not be cleaned up. Approx date of development 7/15/07.
A roof fall has occurred in the No. 4 Entry of 1 right off of 1 north at spad No. 2197 and measures approximately 20 ft wide x 60 ft long x 10-12 ft high and involved an intersection. There were no injuries or equipment involved & the fall did not affect the passage of men or equipment or ventilation.
The continuous Miner was cutting coal and rock in the No. 1 Entry on the No. 8 Section and while loading the last Ramcar, a frictional ignition occurred on the right side of the entry next to the face
While mining in the 1 left 4 north section in A entry, a gas and or dust ignition occurred.
A roof fall was found at 6 left, off right fork submains, #3 entry brk.34, the fall is approx. 20' wide, 30'long, 10' wide, the effected area was dangered off.
A fall occurred in a return intersection of 2nd west panel at the 10+20'. The fall was in the #2 entry, the fall was 20' by 20' by 9'. The fall was due to deteriation of the roof above bolt anchorage.
Roof fall occurred on 10left section entry #3 and #5 at spad no.10LTA907, 4500' from the active face, size of fall, length-150' width-18' height-6 to 8'. All entrances to fall area will be breakered off on evening shift 8/27/2008. There were no injuries, no equipment involved, area will not be rehabilitated.
A roof fall occurred on the 4 Main North C belt at crosscut 135 to 136. The fall was located in the belt entry between crosscuts 135 and 136. The 4 Main North C belt is the belt for units 1 and 4.
An unintentional fall occurred in crosscut between the #5 entry of the 2nd west panel travelway at the 15+00' the fall was 20' wide by 30' long by 8' high. The fall was due to slippage above the bolt anchorage.
A ROOF FALL WAS DISCOVERED 1 CROSSCUT INBY SPAD 4512 IN THE #5 ENTRY WHICH IS THE NEUTRAL. THE FALL DID NOT AFFECT PERSONS, EQUIPMENT NOR VENTILATION. THE FALL WAS APPX. 20' X 20' X 8'HIGH. THE FALL WAS DANGERED AND TIMBERED OFF.
A roof fall was discovered North F 12x #2 Hdg 4 way intersection. 6' super twist fully grouted resin bolts and 4 12' post tension cable bolts were in use. No ventilation or travel was affected. No men or equipment were injured or trapped.
A fall of roof occurred in the 1st Left Panel at # 12 entry, at Station 10+00. The fall was approximately 45 feet in length x 20 feet in width and the thickness is unknown. The area was supported with 5 foot fully grouted roof bolts.
A roof fall occurred in the South Sub Main Entries xcut 33 in entry #12. The fall measured 25' wide, 25' long & 5' tall. It did not cause injury, affect ventilation or passage of people. No equipment was affected. It will not be cleaned up.
A belt fire occurred on the tailpiece of the # 6 main line belt and was extinguihed in about 1 hour
The contractor was operating a D8 dozer pushing material from a high angle area on the pit high wall. The upper wall on the dozers left side slid due to the relief support from its toes causing the dozer to become completey covered with earth, with the operator barley escaping.
A roof fall was discovered in the 2 West #4 Mains, #6 entry, crosscut 45. The fall measures approximately 25 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot tension rebar bolts.
A non-injury accident, the unintentional stoppage of the Camp Run Portal elevator due to an electrical component malfunction from approximately 4:05 PM until 8:00 PM.
An unintentional roof fall occurred in the room off of 11+75 Entry #1 in the old 4E panel. The fall did not affect ventilation, passage of men or cause personal injury. The fall could not be measured because it fell into the perimeter rooms. The fall area was timbered off.
The mancage/elevator was down for more than 30 minutes, with miners aboard the cage. Braces supporting the guides for the ocunterweight needed replaced and repaired.
While cleaning muck from a workplace with a loader, a blasting cap from a misfired bottom hole was apparently struck by the loader dipper and detonated. No injuries occurred.

An unintentional roof fall occurred at station 26+60 entry #4 in the 4C intake. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20 feet x 21 feet x 6 foot.
The mancage was down for more than 30 minutes due to the guide shoes of the counterweight becoming misaligned.
An unplanned roof fall occurred at the #6 belt. Fall measured 50' long, 20' wide and 4' thick. No persons were involved and fall reportedly fell on a belt that was out of service.
8R section #15 1/2 Break angle entry #1 to #2 fell approximately 6' to 10' high 18' wide x 20' long. Pulled 3 rows of bolts out. This was found while firebossing . Area has been breakered off. Will continue to watch the area and install additional support when needed.
During an onshift inspection of the tailgate area the examiner observed deteriorating roof conditions. The area was dangered off and work to install additional support began outby. A short time later the roof fell. The location was XC33, between entry 1 and 2. No injuries or property damage occurred.
South 7, #6 entry, 27 block 4 way, spad #14630 MMU 006. Support: 6' 3/4"" resin bolts, 12' cable bolts, pans and brow straps. 18' wide X 30' long X 8' high. Fall dangered off, additional roof support installed around fall. Clean-up plan developed and posted at fall site.
A roof fall was discovered in the track entry of the old 1 south track approach to the seals. Fall investigated by MSHA Inspector.
An unintentional fall occurred between #6 and #7 entry of the 2 west panel at the 15+00' on unit 1. The fall was 18' wide by 30' long by 6.5' high.
A water pumper noticed smoke at the begining of his shift as he was going out in the mine. Upon investigation he found a fire on mine property in the old C-area. This area was not accessible with our water trucks, we called Harrison County and Hallsville Fire department responded. Approximately six acres of pasture burned, fire was started by assumed lightning strike.
The shearer cut into the tailgate entry of the longwall and the crew prepared to change direction, cutting back to the headgate. They were not cutting coal, but the drums were turning. At this time the tailgate corner of the longwall block bumped and methane was released. The crew noticed flames in the center of the body of the shearer.
Occured in Main West, intake x cut 119 entry #8 fall measured 20'l x 20'w x 8'h. did not affect ventilation, travel on cause injury and will not be cleaned up.
Stocked coal was observed smoking. While moving the smoking coal it started blazing.
An unintentional roof fall occurred at the Galatia Mine New Era Portal at the 3W Headgate travelway, 3866' W-3390' (Crosscut 28) #2 Entry. The fall was approx.imately 64 feet long, approximately 18-19 1/2 ft wide, and approximately 3-8 feet thick. The fall did not affect ventilation. The fall will be cleaned up.
AT 11:30 AM, AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE BELT EXAMINER IN THE NORTH SUB-MAINS AT X-CUT #14. THE FALL WAS APPROX. 40'L x 16'W x 6'H. THE MSHA HOTLINE WAS CALLED AT 11:44 AM. THE FALL WAS LOCATED IN AN AREA WHERE 5' BOLTS HAD BEEN INSTALLED, PRIOR TO THE OPERATOR VOLUNTARILY USING 6' BOLTS.
An unintentional roof fall occurred on the 5B intake at station 86+45 Entry #3. The fall did not affect ventilation, passage of men or cause personal injury. The fall may be cleaned up at a later date. The fall measured 25' x 20' x 8'.
8P longwall recovery face #5 to #8 recovery face fell out approximately 5' to 6' high x 3' to 5' wide x 20' long. Area has been breakered off, had no effect on ventilation and no escapeways are blocked.
A roof fall occurred 4 main 14 x-cut from #5 to and including #6 intersection. Fell above anchorage. 6' fully grouted super twist bolts and 12' post tension cable bolts were in use. The x cut from 5 to 6 had 12 wood cribs in it. no men or machines injured or trapped no ventilation or travel offected. 3 x-inby feeder.
A large portion on material sloughed from the outside slope of the downstream embankment requiring the pond to be pumped down to minimize failure.
A roof fall was found in #2 entry at crosscut #19 near spad #206. The fall measured 20'X20'X4-5'high. No injuries or equipment damaged as a result of the fall. Area had been bolted with 4' resin bolts.
A report of the hoist brake developed a leak. Supervisor notified MSHA @ 7:53pm after initiating evacuation. Hoist repaired 8:15pm. Employees return to work.
Employee reports hoist drive problems to supervisor. He notifies MSHA @ 10:40pm of hoist problems adn initiates evacuation. Problem found (blown fuses) and repairs were made and completed at 6:30am 8/22/2008. Employees return to mine 6:40am.
During the weekly examination a roof fall was found at E-1 Headgate section #3 entry between crosscuts 25 to 26. The fall was approximately 40' long, 5' to 6.5' high 14' to 16' wide.
A roof fall was discovered in the 2 west mains, #2 entry, 62 crosscut. The fall measures approximately 40 feet long, 18' wide and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.
A fall of roof occurred at the intersection in # 3 entry, # 33 cross cut, in the 1st East. The fall was approximately 25 feet in length x 25 feet in width and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
A Coal bank that was not mapped was mined into. No injury or illness resulted and ventilation nor equipment was affected.
Fire of unknown origins at the cooling tower that was under demolition located by the crystal plant.
A roof fall was discovered 3 main 16-314 xcut #8 hdg. Fall above anchorage 18' x 18' x 6' high. Area was bolted with 6' fully grouted resin bolts and 12' passive cable bolts. No men or machines were trapped or injured. No ventilation or travel was affected.
An unplanned roof fall occurred in the #5 entry at spad #5320, 5000 feet from face, call measured 4' long, 20' wide and 4' thick. No persons or equipement involved. This fall waill rehabilitated neutral air course.
The cooling tower by the Crystal plant that was under demolition caught on fire. Cause is believed to be sparks from hot work conducted earlier in the day.
An unplanned roof fall occurred in the #5 entry at spad #5320, 5000 feet from face. Fall measured 4' long, 20' wide, 4' thick. No persons or equipment involved. This fall will be rehabilitated . Neutral Air course.
Rock fall of shale type at Spad Number 41 + 26 of the 4th Southeast Submain in #6 Entry. Size of fall is 18 ft x 20 ft x 6ft plus high. App 4,500 feet from active workings.
A roof fall occurred on 8/18/08 at 3:45 pm at survey station 8+40 entry #8 in 7LMS, MMU #006. Fall was in a three way intersection that had been mined on 8/8/08 & supported with 6 & 8 foot full resin bolts. Two significant clay slips intersected at this intersection & contributed to the roof failure. No injury or property damage resulted. There are no plans to clean up this fall.
Roof Fall was discovered outby the 001 section in return entry #7, spad #19001. Fall was 10 ft X 20 ft X 6 ft high. Entries to the fall were breakered off, area will not be rehabed. Both MSHA and OMSL were notified, and it was investigated. No persons or equipment were involved.
Hoist was down due to the offside speed sensor.

A rock fall was discovered at Spad 1709 in the 1st left panel. The fall was appx. 50' x 20' x 5'. No persons, equipment nor ventilation were affected. The fall was dangered and timbered off and will not be cleaned up at this time.

A roof fall measuring approximately 60 ft. long x 18 ft. wide x 6 ft. height was discovered in the intersection of the #4 entry of 3 East Mains Bk. 4 at survey station # 18496.
Rock fall occurred on 3 East Mains in the #1 entry at break 38. Fall measured approximately 20' wide x 25' long x 5'-6' thick.
A roof fall was discovered at spad 1726 in the 1st left panel between number 3 and 4 entry. The fall did not affect persons, ventilation nor equipment. The fall is apx. 50' x 20' x6' and has been dangered and timbered off. The fall will not be cleaned up at this time.
An unplanned roof fall occurred in the #2 Gas mine at 29 brk, spad #397 that measured 35' long x 22' wide x 14' high. No injuries were reported. The area was developed on 1/15/08.
An unintentional roof fall occurred on the 2-B belt line at the 19+20 station in entry #5. The fall did not affect ventilation, passage of men or cause personal injury. The fall will be cleaned up and a plan is in place. The fall measued 25' x 20' x 6' and had been bolted with 6 foot fully grouted roof bolts.
The DC bridge control failed which controls voltage for the brake release. This unit was replaced. Four miners were on the elevator until 5:55 AM.
A section of frame for a new wheeloader was being lowered down the main production shaft into the mine. During the lowering, the frame began to rotate and got caught. The main production shaft was out of commission until the frame could be freed.
A fall of roof has occurred at the intersection in # 4 entry, # 5 cross cut, of the Main South travelway. The fall was approximately 80 feet in length x 19 feet in length x 10 feet thick. The area was supported with 6 foot fully grouted roof bolts.
A fall of roof occurred at the intersection in # 2 entry, # 16 cross cut in the 1st Left Panel. The fall was approximately 40 feet in length x 35 feet in width and the thickness is unknown. The area was supported with 5 foot fully grouted roof bolts.
Rock fall occurred on 1 East Mains at break #4 in the #7 entry. Fall measured approximately 20' wide x 20' long x 5'-6' thick.
An unintentional roof fall (above anchorage) was found 500 ft out by the east mains section in one of the intake air entries. The fall was approx. 30 ft. long, 6 foot thick, and 19 ft. wide. The area will not be cleaned, all aproches are cribbed and dangered. The fall did not affect ventilation nor block the travel of the escapeway.
A roof fall has occurred in the neutral airway approximately 1.5 miles from the active face. There were no persons, or equipment involved. Ventilation was not affected. This area will not be rehabilitated.
A roof fall occurred on the 4 Main North at crosscut 179 in entry #4. The fall was located inby the tailpiece for the 4 Main North C belt. The area had been previously recognized as being weak and longer bolts and timbers had been installed in the area prior to the fall occurring.
A roof fall occurred 2800' outby face at spad 2334 in #1 entry. It measured 18' wide x 50' long x 10' thick.
A 25' x 30' x 16' high roof fall occurred in 1st. left mains in #3 entry at spad 2383 approx. 4200' from face. The roof fall area was dangered off with caution tape.
A roof fall 2000 ft outby section in return in #1 entry 75 ft outby # 1126 on 1 left submains. Size of the fall: 6 ft high 19 ft wide and 60 ft. long
A fall of roof occurred at the intersection in # 4 entry, 19 cross cut in the South East Submain. The fall was approximately 30 feet in length x 24 feet in width and teh thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
1000 Feet from the face in old works. Roof fall was found at 1105 on 8/14/08. 8'Hx16'W and 60'L. Entrance to fall is dangered off and supported as per roof control plan.
Rock fall occurred in the 3rd NE panel at spad 36+26. Roof is pf shale type. Fall size is approx 60'x20'x10'High and approx 3800' from active workings. Fall occurred in #6 entry of this panel.
A roof fall measuring 70' L x 17'W x 17'H occurred in #4 heading 1 break inby spad # 10671 in the 4th SW mains off of the 3rd SW Mains on the 005 MMU (003 section). Area was cleaned/bolted up with 6' TT and 10' cable bolts.
Roof fall 3-Left Submain #4 Entry Crosscut #4, 30 feet in lenght, 18 feet in width, and 4 feet in height.
A roof fall occured south E 11x 8 entry 4 way intersection 2000' outby loading point. Area was bolted with 5' fully grouted resin bolts. No men or machines injured ot trapped. No ventilation or travel was affected
An unintentional roof fall occurred at ss# 4215 in the mains bleeder #4 entry. The fall dimensions were approx. 35;L x 18;W x 6'H.
North 4, 7 entry, 71 block, Spad #8273, MMU 004. 18 ft. wide x 30 ft. long X 10 ft. high. Support 6 ft. 3/4 inch resin bolts, pans, straps, 12 ft cable truss bolts, screen. Fall dangered off, additional roof support installed around fall and clean up plan posted.
Hoist was down due to a small problem, possible loose wire. Hoist returned to operating in approximately 15 minutes.
Rupture in hydraulic hose on west pinion brake. Severe enough to warrant an immediate shut down to change hydraulic hose.
When we arrived at the mine for the 1st shift, supervisor's found a D-10 dozer company # 221 had burned during the night. The evening shift had parked the dozer at 2:00 am. There were no signs of foul play.
An unintentional roof fall was found by the weekly examiner in the # 8 entry @ x-cut 69.
An unitentional roof fall occurred on the 4 E belt line at station 20+75 entry #5. The fall did not affect ventilation of passage of men or cause personal injury. The fall did fall on 25 feet of belt line and structure. The fall measured 30 feet x 18 feet x 6 feet high. The area had been bolted with 6 foot fully grouted roof bolts. The area had been cribbed on one side.
An unintentional fall occurred in the #1 entry of the 2nd west main at the 6+80'. The fall was 18' long by 18' wide by 6' high.
During a daily exam, a fall was found in the northwest main belt line (section of the mine is idle) adjacent to spad 325. All accesses to the fall were timbered and dangered off. The fall was about 20'wide, 40' long and 6' high.
An unplaneed roof fall occurred in the #2 gas mine 1 brk outby soad #346. The fall measured 20'Lx19'Wx14'H. No injuries were reported. The area was developed on 12/13/07.
Roof fall on intake. Approx. 120' underground, 5' high and about 60'-70' long.
At approx 20:10 a fire was discovered in bottom of coal silo (T90), while EE was performing his rounds. He contacted fire dept & on-call supervisor who contacted ass't plant mgr. Not knowing how long it was going to take to put fire out, ass't mgr told supervisor to have EE contact MSHA. Fire dept left plant site at 21:31. No one injured and no equipment damaged.
roof fall occured in the 12-H section in the No. 1 entry outby No. 3 crosscut. Fall was 16 ft wide, 20 ft long and 6 to 7 ft high.
Excavator had been removing material from the toe of the slope which surrounds Pond 7. While taking a break, material began to crumble off the slope. The operator attempted to move the excavator away from the slope, but while attempting to do this, the excavator was engulfed with material and flipped partially on it's side.
#3 hoist kicked on ""vibration trip"" on the south motor, west bearing. Electrician was called out of the mine to adjust and reset the hoist and it was back in service at 3:45 a.m. There were still two operable hoists during this time.
Rock fall occurred in pit over intake canopy going into the underground from the highwall. Rock was 15' x 20' x 5'.
A roof occured in the 3-North Intake Entry. The fall was located in the #12 crosscut in the #4 entry. The fall was 20' by 20' and 8' to 9' in thickness. The area was originally supported with 5' Hytech Bolts. The fall will be cleaned up and re-supported. The fall was immediately reported to MSHA.
There was a large push of material into the clinker cooler. The hot clinker is transported to an elevator & to the outside bins. A trip by the motion switch on the elevator that caused the clinker to go to the main clinker elevator where it is dispensed onto the belt. The emergency water sprays were immediatley activated but the hot clinker ignited the main clinker belt.
During maintenance shift a rockfall was found while checking the fresh waterline in the #4 main return, #3entry, @ #5 xcut. Fall is appr. 20'x30'x6-8'thick, no ventilation affected. Affected area will be timbered & dangered off. The fresh waterline will be rerouted away from affected area. By the time the supervisor was contacted & got outside to call MSHA it was 05:20
A roof fall occurred in the 2 west 4 mains, 29 crosscut, #9 entry. The fall measures approximately 50 feet long, 18 feet wide and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.
A fall of roof occurred at the intersection in # 8 entry, 15 cross cut, at spad no. 6+40 in the 1st Left panel. The fall was approximately 35 feet in length x 30 feet in width x 12 feet thick. The area was supported with 6 foot fully grouted roof bolts.
Unintentional fall located at 36 break in the Tailgate #3 entry
A wire in the electrical control circuit of the elevator became grounded causing the elevator to stop while asending to the surface. A crew of men were on the elevator for approximately 3 hours. The grounded wire was replaced and the elevator was tested and found to be operating properly.
An unintentional roof fall was found in the No 2 Entry, at Crosscut No 5. The fall is approx 18' long X 17' wide X 4' thick. Area is supported with 4' conventional roof bolts. The fall was found at approx 8:45 am and the individual who found the fall had to walk outside to a phone and called immediately to have the fall reported at approx 9:20 am.

Roof Fall discoverd on 002 MMU at spad # 3620 # 6 heading, size of fall 18' wide x 10' high x 60' long fall does not effect ventilation and will not be cleaned up.
Hoist accelerated into overspeed mode and stopped. The hoist was removed from service immediately and all persons withdrawn from the mine via the Decline. Repairs were made to the electronic controls and the hoist returned to service for the evening shift on 08/10/2008.
At 1:35 a.m. the Kuhntown elevator stopped operating. An employee was on the elevator from 1:35 until 3:10 a.m. The door switch was broken.

#3 hoist kicked on ""vibration trip"" and while working on it a sensor in the shaft came loose and had to be remounted. Electrician reinstalled the sensor and hoist was back in service at 5:45 a.m. There were still two operable hoists during this time.
A roof fall was encountered on the track travelway of the # 1 mine portal during a fire boss exaimation, the fall is located outby spad number 2041, the fall measured 19 ft. wide, approximatly 80ft long and 22ft high.

An unintentional roof fall occurred on #10 belt, bk 7, neutral entry, spad #7406. Approx length 20 ft, width 20 ft, thickness 4 ft. Support being used 4 ft resin bolt. No injuries occurred.
At approx. 11:20AM, materials were being hoisted to the surface utilizing the number one service hoist. At a distance of approximately 500ft up from the bottom landing, the ropes began to slip on the hoist drum causing the load to descend back down towards the bottom landing. The cage came to rest approx. 20ft from the station level.
Roof Fall discovered in the 3 South, 2nd Room, Bleeder, Crosscut 10. The fall was 18' wide by 10-12' thick and unknown length. The 3 South is being pillared at the time of the fall and the fall was located in a fault zone.

#3 hoist tripped on ""Drive Converter A"" fault. Electrician was called out to the mine to check and reset the hoist. The electrician checked for a ground fault and reset the hoist and it was back in service at 6:10pm. There were still 2 operable hoists.

While traveling airways in the Old 1st Rt. Section a roof fall was identified in the #4 entry near spad #11144. The fall was approx. 40' L X 20' W X 6'-7' H. The area was supported with 4' resin bolts. The area has been dangered off and timbers have been set.

The Oakdale elevator became inoperative at approximately 7:00 PM. The malfunction was due to a faulty / sticking electrical contactor. A representative from Schindler Elevator Repair service was called to correct the condition. No person or persons were injured in this event.

An examiner found a roof fall during the weekly examination of a worked out area on the 1 Left 4 Main North. The fall was located at crosscut 86, entry 2-3. The fall was located adjacent to the examiners route of travel.
An unplanned roof fall occurred in the #2 coal mine in the left return on the #1 section at spad #725 that measured 40' long x 22' wide x 16' high. No injuries were reported. The area was developed on 4-24-08.
Operator was tramming the shovel when it quit running. Another equipment operator advised the shovel was smoking and the shovel operator evacuated the equipment. Fire suppression went off however, could not extinguish the fire. Shovel was severely damaged by the fire.

A roof fall was found on 8/3/08 at cross cut 159 main south entry by examiner doing preshift inspection, Fall was in a four wasy intersection that had been mined 12/1996. Area had been supported with 8' full resin bolts. Fall was approx 20' high x 24' wide by 30' long. No injuries resulted, plans are to clean up fall and set arches thru area.
A roof fall was discovered on the 22 Right abandoned section that had fallen across the breaker timbers for the pillar line. The fall was approx 50' long X 5' high X 20' wide. The fall will be breakered off. This section is abandoned.

Hoist went down due to a blown fuse. Men were pulled out with emergency hoist. Hoist was repaired and back in service at 10:30 A.M.
Foreman found roof fall while making weekly exams. Fall consisted of weathered stack sand stone shale mix. Roof control plan has been changed to use longer roof bolts when these conditions exist.
Our examiners found a roof fall in the 1 right, 3 left, 5 West panel at crosscut 8 between entries 4 and 5. The fall was located in a worked out area. The fall was located adjacent to the examiners route of travel.
An Unintentional roof fall has occurred on the 002-0 MMU in the 4 left crosscut. The fall started in the unbolted area and continued out into the intersection. The approx. lenght of the fall is 60 ft. and 5 ft. high and the width of the intersection. No injuries occurred as a result of this fall.
For reasons unknown at this time, the victim came out of the truck and was run over by the truck.
The contract employee was helping to install a counterweight on Gradall XL5110 Scaler. As the weight was lifted into place, one of the connector links of the chain used for lifting broke, causing one end of the weight to strike the ground. This in turn broke the other end, which allowed the weight to strike the employee as it fell.
Employee (electrician apprentice) and a co-worker (journeyman electrician) were working on a light fixture of the SX upper tailings booster when he came in contact with energized wire.
EE was found unconscious, with no resp or pulse pit between hydroseparator and floc building. CPR was administered and EE was transported to CMS where he was pronounced dead. Autopsy report has not been recieved. A heat trace tape was found arcing near where EE was found.
Operator removed guard to clean the steel tire of a rotary sand drying drum. The operator was holding a rag on the tire while it was running. His two finger tips were smashed between the moving steel rollers. KU Med doctors amputated the distal tips of both his ring finger and middle fingers on his left hand. The bandages will be on for at least another week.
The employee was pulling off steel beams from the fork of the forklift and got his left small finger caught between two pieces of steel beam. The employee pulled his finger out hard and noticed the end of the left small finger was missing.
Contractor finished drilling hole, started to lower mast, pinched little finger between the mast & window frame of operator's cab. Resulted in the amputation of his little finger.
Two men were attempting to move the stacker. They did not follow standard procedure and removed the chock blocks before the loader was there. The wind from Tropical Storm Fay caught the stacker. The men tried to stop the stacker from rolling, but IE slipped and it ran over his right leg crushing it from the knee down. His leg was amputated on 8/25/08 below the knee.
Employee was bent over parts washer rack and was getting ready to lift starter from rack. As he picked up on the starter, he sneezed and felt a pop in his belly area. He has been diagnosed with an umbilical hernia. A 2-ton job hoist is available to be used to lift items out of the parts washer and employee has been instructed to use it more frequently during all lifting.
Employee was holding up a power cable. A rock fell on the power cable causing the employee to drop his hand and strike a rock on the ground.
Drill steel uncoupled from shank and fell on operators big toe on left foot, partialy severing toe. Toe was reattached at hospital.
Employee put his hand in harms way and the tip of his right ring finger was cut off.
While repairing screw conveyor employee followed company lockout/tagout procedures but failed to realize that the screw shaft was free wheeling inside of collar when he stuck his finger into the bolt hole of the screw drive shaft.
While trying to unplug a dust collector chute the employee raised up the provided rubber guard and stuck his hand in too close to the auger screw and momentarily became entangled in the screw.
A hose/fitting became disconnected from a tank (acid delivery trailer)
Working in damp conditions. Pulling drill cable up and was shocked.
The employee was in the process of blocking under the undercarriage of a raised 14/15 CM continuous miner to replace the crawler pads. As he stuck his hand under the undercarriage to retrieve a wooden block the previously installed wooden crib under the cutter head shifted allowing the machine to fall catching his hand between the wearing strip and the mine floor.
Employee stated that after returning home from his shift on Sunday evening 8/31/08 he noticed a caustic burn to the back of his leg (ankle area). He did not report it until 9/5/08. GA personnel transported him to an offsite medical facility for treatment.
Employee was attempting to board the right side of 546 dozer. Using his right arm to grab and pull himself up onto the platform. As he elevated himself he felt a sharp pain in his upper right arm. In the cab of the dozer he noticed that when he flexed his arm, a pain would shoot up into his shoulder.
Struck head on low timber, injuring neck and back.
Employee was unloading a roll of rubber with 3 other employees when it rolled onto his leg causing a compound fracture.
Employee was struck by a large piece of ore on the hard hat which caused neck and shoulder pain
EE was sitting down trying to pull a nip off a transformer but couldn't do so and as he rolled over to get his knees to get up he felt a sharp pain in the middle of his back.
Employee lifted a case of bottled water - lower back pain.
Member was changing a 100 hp motor on the direct ship cruiser located on the 2nd floor of the plant. The spring loaded base was in the up position & when the motor was removed it released the motor base. The base came down and hit him on the left hand.
The employee was unloading belt structure from the scoop when he slipped and fell forward into the scoop bucket hand first. The incident resulted the employee having both Fx and sutured fingers of the right hand.
Employee was cutting rubber in #3 vibrating screen. His knife slipped and cut his right hand.
Employee was helping crib the miner under the stab jack. When the stab jack was pressurized a board kicked out and struck him on the right arm.
The employee stated he pulled his s/c up too close and on the miner bumper and his head was slammed into the canopy when the miner moved forward.
Employee was using a hydro-blaster to unplug a sanded hydrosizer. The hydro-blaster removed most of the sand that plugged the discharge. Employee pulled the hydro-blaster hose out to realign the nozzle with the discharge line. As he repositioned his hand on the hose, the discharge struck the outside edge of his left forefinger cutting his finger.
Employee was on the mantrip on the opposite operator's side in the midde seat with his foot on the middle rail. The mantrip came around #7 header on the supply road when his right foot was caught between the mantrip and rib.
Putting battery powered mantrip on charge, when it rolled into him, catching him between the mantrip and the power center.
EE was walking from one roofbolter to the other roofbolter and stepped in a hole and twisted his ankle.
Employee strained his lower back while swinging 8 lb sledge to open a valve.
EE was replacing a tire on the std car on section 008. The tire slipped, resulting in EE straining his back (upper) and his neck. Pain in back and neck.
The man was carrying a post when he felt a pain in his back.
Employee was helping another employee lift a rock from the conveyor car on the High Wall Miner. The other employee lost his grip on the rock and the rock dropped onto the frame of the car. The employee who was helping remove the rock, pinched his finger between the rock and frame of the car. The first digit of his ring finger was fractured.
Employee said he was pushing the miner cable out of the way with his feet, as he was crawling back to the miner and hurt his back.
EE was riding on a diesel mantrip and had his left foot outside the frame of the machine and got it caught between the mantrip and the coal rib.
In the late morning on 8/28/08, a small speck of metal got into EE's right eye. By 7 PM his right eye was very irritated and swollen. He went to ER and received medical attenion (metal sliver removed). No damage to eye. EE returned to work the following Monday, 9/1/08.
EE was riding to work area on the mantrip, mantrip was going thru an airlock and the door hit the SCSR storage box which then struck ee's right hand. *Restricted duty from 9/2/08 to 9/12/08. Started missing work 9/15/08.
EE was bolting top and dropped the head of the pinner down to remove the drill steel, when the drill steel came back and struck the EE on the right index finger. The finger was broken in two places.
Lifting roller into conveyor, pulled muscle in lower back.
Employee was using a track duster to dust belts. The hose became clogged, he pulled the hose off to look for the clog. He looked inside the coupling and the clog came loose and the dust hit him in the face. On 8-30-08, he was diagnosed with a scratched eye and was put off work until 9-5-08.
Employee was exiting a tool shed at the office area and slipped on wet surface and fell on right shoulder.
Miner crushed his right toes due to movement of cone while clearing rock jam. As he used pry bar while straddling cone last rock released stored energy of cone squeezing the side of his right foot.
Section foreman noticed that the rib needed scaled where work was being performed on a shuttle car. He scaled the rib and was rolling a large piece of rock from the work area when another piece of rock fell striking his left index finger causing a severe laceration.
Employee was operating a scoop and when he applied pressure to brake pedal he felt a pain in his left knee. Pulled ligament in left knee. Did not report the injury until 9-4-08.
Operating battery powered S&S scoop. While exiting operators deck, slipped and left knee made a popping sound.
Victim sat down at the rear of the roof bolter to eat lunch and a portion of the roof fell striking him on the head and shoulder. Laceration to his head and contusion to the right shoulder.
Helping another person remove some cap rock off of a power cable, while he was bent over a piece of cap rock fell out of void striking him on the back of the hard hat.
He was operating a haulage motor when the motor went off track causing him to strike his right knee and left middle finger against the motor. He sustained contusions to the right knee and a fracture to the left middle finger.
The employee was getting down from a forklift, when he felt a pull in his lower back.
Employee climbing access ladder to enter cab of machine pulled tendons in right wrist.
EE was attempting to insert a roof bolt into the top, in order to do so he had to bend the bolt to get it started. When he attempted to bend the bolt he felt a pain in his wrist.
EE is an independent sales consultant for TPH. While consulting on the subject of flocculant equipment, he slipped on the wet concrete and banged his knee. The area was marked ""slippery surface"", ""watch your step"". EMT called and transported EE and then he was released. He will follow up with his primary doctor and notify us if there are any complications.
Was extending the survey sights in #1 room. A piece of rock fell out from in between the bolts hitting him in the head and back. Started missing work on 9/3/08.
Employee was connecting relay bar to pan, and the chain on the back of the ram made relay bar jump up and it crushed left finger
Employee was removing impeller from pump. Reached up & grabbed impeller with left hand & while pulling down on impeller cut his left little finger down to the bone. Was wearing a pair of jersey gloves.
Employee was starting to drill hole and a piece of draw rock fell on his shoulder.
Employee was roof bolting when auger came out of chuck and struck right hand between thumb and index finger. (Required 5 stitches.)
As the employee was descending a step ladder, the employee slipped and fell backwards on the third step from the bottom. The employee landed on a beam and concrete floor with his back and hip area.
Drill steel hung in hole, He pulled on steel with one hand, had other hand on jaws. Co-Worker accidently hit controls and pinched employee's hand and fingers.
Employee was attempting to move a piece of plate steel that was setting on steel grating. He was using a pry bar to wedge in grating to move the plate. Instead of the plate moving, the grating shifted off the support beam, allowing the grating and employee to fall to the ground below. Sprain wrist and contusion.
Injured was taking down 3/4"" pipe that was beside feeder and transferring it across #3 belt. Pipe had been used as part of fire deluge system, drive had been removed. As he attempted to move it across belt, he felt a pain in his lower back on the right side.
Driving haul truck. Not using mirrors to back up.
Employee was changing air filter on Komatsu Portable Jaw Crusher, when he attempted to get up from a kneeling position, at which time he was unable to do so.
Employee was working with another man placing rocks in pallets. The other man tried to place a rock before EE moved his hand from the pallet.
Operating pug mill when foreign body entered eye.
Employee was operating a ""articulate truck"", he was ""dumping a load"" when it. ""jolted"", he said he began to feel pain in his lower back. He stated he had previously injured his back, and was being treated by a chiropractor.
The employee stated he was working on a Mac 8 and felt a pain in his back. He has been diagnosed with degenerative disc disease. He did not miss work until 8-28-08. His back has been bothering him for several weeks.
EE was walking on uneven concrete floor, when she turned her left ankle. EE was taken to ER on 8/25/08 and on 8/27/08. She was seen by company physician and diagnosed with a fracture of the foot. She began loosing time on 8/27/08.
EE was walking around the roof bolter and tripped and fell, striking his knee on a rock that was lying on the mine floor.
Employee was helping lower concrete forms for the next shaft pour. The form became hung up and then sprung free crushing the employees hand between the steel form and the concrete shaft.
The employee was helping to hang a miner cable when he felt a burning sensation in his shoulder.
The employee complained of chest and neck pain. He was administered oxygen. EMS arrived and transported employee to the ER for evaluation. Testing was inconclusive. Employee was released from ER on 8/27/08.
Other EE was loading a truck and flat bed by taking bulk bags from inside the warehouse and placing bulk bag on flat bed. While picking up bulk bag injury was backed into by the forklift. Injury said his legs and back hurt. Northshore Regional released him to go home and was referred to Dr. for a follow up visit.
Was attempting to align drill steel with the pot. When he raised the pot he caught the tip of his finger under the steel. No fracture, just a contusion.
Employee stepped on a rock while climbing down from a loader, twisted and rolled his right ankle.
Injured employee was using a hydraulic boom to hold a hydraulic pump in place on a D 11 dozer. The boom control was beside him on the dozer, when he inadvertently activated the lower switch and lowered the boom onto his back.
The employee tripped over a 6"" inch water line (vic coupling) spraining his knee.
EE was working under raised hood on loader in shop. Hood fell striking employee on shoulder.
Employee is a Mechanic/Welder and was lifting a piece of metal and while lifting he felt a sharp pain in his lower back.
While removing a bottom belt roller, the employee got his gloved left middle finger caught between the roller and the frame of the roller assembly resulting in a fractured tuff and a laceration requiring 6 sutures.
Heavy rain when the operator was climbling onto the loader. WHen his foot slipped causing him to fall 6ft to the ground breaking his left leg.
Employee was placing jumper bar on cell line and smashed right index finger between jumper bar and hanger bar on cell line. There was little damage to finger and no impairment.
Moving miner from #4 entry to #5 entry and the rib rolled off while he was moving the miner and hit him in the hip.
Employee was unplugging rocks from a discharge screen on a rock crusher. When the rocks went through, the lid slammed down on his left index finger.
The employee was walking when she fell down fracturing right leg and ankle.
Operator stuck hand into air conditioner fan in top of cab. Possible disorentation, refused drug screen at hospital.
Employee backed over a large rock while operating a D11 dozer causing him to be jarred in the cab. He was wearing a safety belt at the time. The following day he had some discomfort in his neck and was scheduled for an appt. An MRI evaluation later indicated a cervical disc displacement and he was ordered off work effective 10/24/08.
Employee stated he was walking underground on the section when he stepped in a small hole and twisted his knee.
EE was installing a troughing roller and became struck/pinched by the roller when it slipped.
Employee twisted his back when he lost control of his drilling machine while long holing. He went to his own doctor 8-27, reported work-related accident. Reported accident to BlueRock on 9-2-08.
The weather on the day of the incident was hot with temperatures in the mid to high 80s with little breeze and brief periods of rain. Believe the hot humid weather contributed to the employee fainting. Injured contractor's injuries included dislocated right shoulder, hairline fracture in right shoulder and minor cuts and abrasions to contractor's head and face.
Employee shut down a 4"" pump that had aired up. When disconnecting the discharge hose from the pump, hot water and steam blew back onto the employee's right arm.
Assisting the section foreman to attemp to scale loose roof at a roof slip location. When positioned under supported roof, he pryed a piece of rock down. When it came loose, unsuspected roof was dislodged and it fell to the mine floor, striking the victim. A piece of the rock struck his left foot, resulting in a fracture to the metarsal bone of the foot.
Employee stated that while attempting to cross the 1 north belt he caught his leg in the crossover and fell, twisting his right knee.
Employee descending a ramp off the overcast stepped on a rock twisting his ankle. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equip not a factor, miner proficiency a factor, mining equipment/systems not a factor.
EMPLOYEE STATES HE WAS OPERATING THE CONTINUOUS MINER WHEN A RIB BLEW OUT, STRIKING EMPLOYEE IN THE MOUTH.
Employee was pulling connecting pin from trailer with pry bar when he strained his back.
EE was walking out of the west blend silo room and he allegedly hit his head on a pipe causing pain. All x-rays were negative and he worked without restrictions and did not miss work until 11/24/08. this then was a lost time accident right before layoff.
Using a hand grinder and it kicked back and struck him on his arm, 3"" above his elbow. Laceration is 3"" long. Did not cut into muscle.
EE was putting new brake pads on dump truck when he felt a strain in lower back. Employee went to doctor and was put on light duty work for 4 days.We are looking into this accident. We had a helper with him, he did not report any injury friday before he left work but came in monday morning first thing and reported back injury.
1 1/2"" piece of angle iron (which supports walkway sections) failed allowing miner to fall through walkway to concrete floor below (approx 14'6"" distance).
Small piece of draw rock fell and hit him on the back of his neck.
Injured was backflushing the high pressure filters and a 3/4"" hose was not secured when it struck him on the right knee and right forearm causing contusions to both areas. He went to the doctor for treatment.
Employee was in the process of hanging miner cable when his feet got tangled up in the cable causing him to fall to the mine floor. When he fell, he tried to catch himself. This resulted in a bruised right wrist. The employee continued to work and completed his shift. EE did not return for next shift.
On 8/22/08 EE was pulling on the water line that was falling out of the car when the car dropped off the pad. His left foot was positioned under the car when the car dropped off the pad, crushing his left foot.
He was walking to his work site, stepped in a hole and felt pain in left knee. He continued to work at his regular duties but continued to have pain. He went to a physician and was removed for surgery on 11/20/2008 to repair a tear of the medial meniscus.
Employee was climbing down from 100 ton haul truck when his hand slipped from handrail resulting in employee falling to the ground.
Stated he was stepping off of a dozer and hurt his knee.
Injured was running shuttle car and hit hole in belt entry causing head to hit canopy. Was not officially reported until 9/2/08.
Employee stepped down from from a trackhoe. He stepped down upon uneven ground with his left foot and twisted his left ankle.
At about 2 pm on August 21st, 1st Class Mechanic reported a loss of strength in his right bicep as he was lowering a ~40lb air filter access panel while working on a loader in the Truck Shop.
On 8/21/2008 this foreman was loading posts in a scoop bucket and he felt a pain in his lower back. The foreman continued to work and on 9/10/08 decided to see his physician. Therapy and rest was prescribed.
Employee was stacking 50# bags on a pallet and pulled muscles in lower back. Employee was released to come back to work on 8/25/08 but chose not to return until 8/26/08. Treated at Custer Regional Clinic by Dr for lower back strain.
Walking to east side of tank house (EW) to turn on water to clean out slime outs, on return of same path, tripped on uneven surface and fell forward catching himself with both hands, but twisted his left knee.
EMPLOYEE WAS WALKING UNDER CAR CABLE THAT WAS HANGING FROM ROOF. THE CAR BECAME ENERGIZED AND PULLED THE CABLE DOWN ON EMPLOYEE'S RT. RIBCAGE AND BACK.

In entrance #3 (beltline) the conditions were wet and sloppy, no bottom had been cut and the floor was slick. EE slipped and twisted right knee resulting in torn mcl and miner cartilage damage.
Operating mantrip and steering wheel got caught and struck right hand causing fracture.
Operator of Caterpillar 735 haultruck (#30) was coming off 1 Dam to 1x Dam. Just before the ramp he hit a small pile of dirt in road which jarred his left shoulder.
EE was loading Idle Roller into scoop bucket. The roller shifted in the scoop bucket making it roll towards where EE was standing. The roller landed on top of the right foot of the EE.
Employee was walking when he stepped in a hole, in the mine floor, causing a strain/sprain to his left leg and knee. This did not become a Lost Time Accident until 8/22/2008.
Miner was moving the left side miner on the Mains section so he could scoop the 2nd right intersection. While tramming the miner the boom was swung to the right and miner was struck by the boom. Miner continued to work after the incident but decided to go to the hospital due to pain. ER doctors found no injuries but told miner to take off until 8-24 due to soreness.
Employee was cleaning off roof bolt machine when he pulled on some loose wire that moved underneath his safety glasses causing lacerations to his right eye.
Employee was a passenger in a personal car, which pulled onto a public hwy from non mine property, into the path of an oncoming car and was hit in the side. Was initially told by msha a 7000-1 was not required.
Ee was loading bolts, plates glue and straps onto the back of a roof bolter when his left hand struck the sharp edge of a strap, causing a laceration that required 4 stitches.
During a weekly exam run, a piece of stone fell from between the roof straps striking employee on the back of the neck and head. **Lost Work days began 8/28/2008
Employee was sitting on the mine floor behind the roof bolter when a piece of rock measuring 30"" wide X 2"" thick X 12' long fell out between the 7th and 8th row of bolts from the face in the #4 entry.
Employee stated that he was getting up onto the road grader. He grabbed hold of the hand rails to pull himself up the ladder. As he did, he felt a pop and had pain in his right shoulder.
Injured was roof bolting in entry #6, on unit #3, inby crosscut #21 when this incident occurred. He was bolting when the unbolted cut started to work and rocks started to fall. As he moved away from the bolting machine from under the bolter canopy a rock fell in front of him and landed on its end on top of his right foot, fracturing three metatarsal bones in the process.
Employee was breaking strap on a bundle of timbers; when the strap broke it hit him in the left eye. Employee required surgery.
Employee was cutting a 2x4 with a skill saw when the guard stuck up.
Truck was coming down bank and driver could not get it back into gear, so he jumped out of truck, causing multiple contusions and abrasions.
The employee was in the process of removing a manifold off of a 1-1/2 inch fire valve, which was attached to a 4 inch water line at Break 17 on 4 North Mains. The manifold flew off striking the employee in the face causing lacerations to the face requiring sutures to close and a broken nose.
Employee was washing the Caterpillar 226 skid steer loader. When he opened the engine cover the radiator cap came loose and hot water was spilled from the engine to his abdominal area.
Employee stated he was standing under LW shield #3 removing rock props when the shield operator bumped out the shield flipper. The flipper struck him on the left shoulder. He was examined for the bruised and strained shoulder; he did not miss any scheduled time from work. He had surgery on Friday June 19, 2009 which is the first missed work day.
Employee was removing a modulating valve from rock dust tank car. The valve fell off pinching his left index finger between the valve and a reflector bracket that was mounted on the car. Fractured left index finger.
While attempting to remove the miner cable from a hanger (arms stretched out overhead), employee experienced pain in right shoulder.

Member was installing rib bolt when a rock fell striking his right hand resulting in a contusion. Rock measured 10"" long x 3"" thick.
While repairing water pipe, moved to avoid water, twisted causing pain to lower back. Accident occurred 8/19/08. Did not seek medical attention until 8-25-08. Off starting 8/25/08.
While cleaning and inspecting new 57"" wheel hardware, injured was standing in front of elevated wheel hardware held by a mobile tire handler he was operating and the hardware weighing approximately 1500 Lbs. slipped from the handler gripping pads and struck injured on the right foot, resulting in crushing injury to right arch and first three toes on that foot.
Injured was climbing down off the top of a eimco scoop when he slipped in some mud and gob causing him to sprain his right ankle. He was transported to the hospital where he was treated for a sprained right ankle.
Employee was lifting a wire cable off of a loaded scow when he felt a pull in his shoulder. Employee was seen by doctor and returned to work in modified duty capacity. He continued to have pain and went for further testing which showed slight tear in the shoulder. Doctor has placed him out of work until further evaluation and treatment.
Grinding cutter drum preparing to weld bit lugs to drum. Metal got into eyes. Employee did have safety glasses on at time of accident.
Employee was putting the operator's end of a 15-ton locomotive back on the track. He had an airbag under the center of the motor fully blown and raised up. He had the other airbag on the left corner of the motor blowing it up to push the motor back over on the track and the airbag popped out striking his right arm.
The employee was operating a RBM and was struck on the right hand by falling drill steel out from the roof.
Changing scoop tire, it started to fall over, he tried to stop it and felt pain in lower back. Note: Employee continued to work until 8/27/08 at which time he was taken off work by a physician.
Employee was a passenger in a golf cart that rolled over. The cart landed on top on his legs. He was evaluated by a physician and was taken off of work on 8/21/08.
Employee was pulling power cable by hand with a hot stick on level ground. While he was backing up pulling the cable, he felt his right knee pop causing pain. Employee was released back to work 8/20/08. He has since seen a specialist and had surgery 9/26/08.
EE entered engine area to grease dredge service pump. When entering EE sliped and his left hand came in contact with dredge exhaust. EE grabbed dredge exhaust to brace his fall.
Down in the tunnel under the sludge sumps, attempting to unstop a drain, EE slipped as he was attempting to step up over a pipe and bumped hie rib cage against a water pump resulting in a possible fracture to one rib. Continued to work all scheduled shifts until 8/25/08. Pain continued, went to DR on 8/26 and started missing scheduled work days on that date.
While climbing down ladder, foot slipped off of step. He fell to the ground spraining right ankle.
Employee was drilling the inside hole when he hit the fast feed lever. The head caught his knee pad causing the boom to hit his left thigh, twisting his knee.
Operator was observing another operator swing a hammer sheering bolts with a wedge. The hammer struck the hood for the flaker belt and bounced back, striking him in the arm.
Employee was pulling plastic water line when he lost his footing on uneven bottom slipping and landing chest first on bottom.
While shoveling the belt, EE was pushing against the shovel with his right knee and experienced pain. After consulting a Dr., it was determined he had a sprain/bruise.
Employee was moving rock from pallet to splitter table when one fell and struck his right foot. Injury was a contusion of the right foot.
Empl. turned ankle when stepping on mill ball. Reported that ball was not covered but just didn't see it. Incident reported as first aid because Empl. indicated he was fine and did not seek medical attention. On 8/31/08 employee was walking down a staircase and felt pain in ankle. EE went to doctor on 9/2/08 and was told not to return to work until diagnostic tests came back.
The employee was working with the gravel conveyor belt. He tried to lift the belt and injured his right shoulder.
Employee was moving boulders with a track hoe with a thumb, the boulder slipped out of the thumb and smashed into another boulder causing a small chip of rock to fly off and hit the employee in the face.
Adding drill steel to drill deeper, hoisting steel to top of tower, load squeal from top of mast. He looked up to see if jammed or broken. Looked back and steel swung contacting operator on righr eye cutting eyelid and rupturing blood vessels behind eye causing pressure, impairing vision temporarily.
The employee stated he was operating the roof bolt machine and was putting up a bolt for a kettle bottom when the rib rolled and struck him on the left leg. The rib was approximately 2' x 2' x 6' high. The employee was bolting the third row of bolts.
EE was stepping down from the bumper of the service truck and slipped, twisting his knee. EE tried to finish his shift, but couldn't due to the pain. He then went to the boone memorial hospital where he was refered to a specialist. He then met with the specialist on August 28, 2008. He was put off work until Sept. 2, 2008. He was diagnosed as having a bruised bone.
Area was about 38' high causing use of different drill steels.
Twisted back by trying to move a piece of iron.
Employee was working with a crew building a transfer tower for a new crushing plant. There was a floor opening on the first work platform. Employee was taking measurements of the opening. He stood up and fell through the opening to the ground fifteen feet below.
The unwitnessed EE stated that when he was lifting a roll of fabric, he mis-stepped and twisted his back. He was taken to the hospital, received Tx and injectable meds and scrips. He failed to show up 2 days later for follow-up visit with Dr. and we discovered that he had taken employment with another employer and denied any problem with his back.
EE was recovering pinner cable from under some debris. After recovering the pinner cable, he stood up and felt a sharp pain in his lower back.
Employee was pulling water hose off water truck and the hose came loose and he fell off the truck injuring his left knee.
Miner was walking and a piece of coal rib fell and struck his knees causing a laceration which was sutured.
EE performs daily equipment repairs. He alleges his back has been bothering him off and on at times (he weight-lifts at a gym every morning) but while at work he lifted a hammer and he felt a low back pain. He notified his supervisor-who then sent him to see a Doctor. It was determined that he has a herniated disc-so he decided he wants it to be surgically repaired.
Employee was grinding welds to reinstall blocks. He thought that he suffered from flashburn and later found that a piece of metal was in his right eye. The metal had to be surgically scraped to be removed.
Employee was lowering a jack and he did not notice that the jack did not latch and the handle snapped up and struck him on his wrist. Cause of this injury was safety awareness. Compliance with rules/regs not a factor, protective equip. not a factor, miner knowledge could be a factor, mining systems could be a factor.
Six men were hanging a drive through door. Employee was on one corner and while lifting the door he felt a pop in his left arm.
EE was installing hood on D9 dozer and pulled muscle in neck & back. Reason for delay in reporting - was not told of accident until 8/25/2008.
Employee was operating miner in #5 Rt crosscut. Rib rolled from face of #5 entry (which was flush) behing the blowing line curtain, striking employee on left knee and upper leg. X-rays were negative, but employee was referred to specialist. He was off work awaiting specialist visit. Examined by specialist and placed off work on 8/26/08. (knee)

preheater tower went positive through the riser duct, pushing small amount of dust into the air, which went underneath EE's faceshield and safety glasses. All ppe was present.

EE was a ladder for the purpose of adjusting the tracking of the belt on the feed conveyor. The belt needed to be adjust. He was leaning against a pipe and held onto the machine to reposition himself when his glove got caught an the return roller and pulled his arm into the conveyor. The other man on the floor hit the emergency stop button. The bar prevented accessibility beyond
Injured was found int he #5 entry on the 002 section in the last open x-cut intersection. He was unconsious. EMT and crew adminstered first aid and transported to surface. When he regained consciousness, He stated that passed out, falling to the mine floor and hurt his shoulder. It turnes out that he had a reaction to medication.
He was taking the tractor cover off and it fell and hit tooth
Climbing the ladder of his truck, his knee popped.
EE caught foot between pallet and forklift while stacking bags which caused some bruising.
The employee strained his back while working on a D-8 Dozer. Employee reported the injury and is continuing to work as a mechanic. This incident became reportable when the employee began physical therapy on September 30, 2008.
Moving waste barrel into skid steer bucket and pulled muscle in right knee and hip.
Employee got his little finger on his left hand mashed, when he was trying to get a broken bolt out of the bolter canopy. Canopy fell and caught finger, causing tip of finger to break and sutures.
While shoveling wet fly ash out of the bed of the truck, ee twisted the wrong way and pulled his back out.
Employee shut down the Plant & while exiting the shack, he stepped on a piece of screen that someone welded on the catwalk. He twisted his left foot/ankle. He returned to work on 8/19/08. He had follow-up appt. with another dr. on 8-20-08. He had restrictions, therefore, he will not be returning yet. His next appt. is on 8-29-08.
Employee was stomping on a stem lock bag to activate the chemicals inside when he felt his right knee pop out of joint.
Injured was lowering a roller from the belt scrape mechanism that removes coal from the belt when the silo is full. Injured removed the roller lock from the handle. This allowed the belt weight to push the roller down and cause the roller handle to move unexpectedly knocking injured down. Injured landed on his right lower leg resulting in a fracture to the leg.
Lost grip with left hand coming down ladder of trailer, caught himself with right hand as he was falling back.
Victim climbed onto the railing on the #1 stacker belt to reach some gob material. His foot slipped and he twisted his leg & knee. Has a brace on his right knee and is using crutches.
Trying to pry belt clamp loose from top structure, structure belt broke. Belt and C clamp struck employee in the face, near the right eye.
EE was helping to move a shaft with a strap. The strap slipped and the shaft fell and struck the top of his foot.
Employee was walking across ledge when he slipped in a hole rolling his ankle over and all his weight going onto rolled ankle.
Worker was drilling top, a piece of rock slid down steel striking worker on left wrist. Worker finished his shift.
Employee was on the side of the scoop dumping; cable hooks pulled loose and hit employee in the back forcing him into the duster on the scoop.
Employee was surface when he picked up a spring for a mac-8 diesel ride. When he started to pick the spring on a pallet he felt pain in his back.
EE was crossing a conveyor belt line that was not in operation. For an unknown reason he stepped on top of the wet slick belt causing him to fall forward striking his right hand on a piece of rock breaking his right thumb.
Employee was riding in buggy returning to the elevator shaft. Buggy pulled to the side to allow equipment to pass. Driver released the brake to pull forward, the buggy drifted backwards, smashing the employees hand between the rail of the buggy and the rib.
Alleged incident occurred while EE was attempting to change a drill pot on roof bolting machine. EE lifted drill pot to knee level when he felt a pain in his lower back. EE did not report incident or miss work until 8/18/2008 and also missed work on 8/19 and 8/20. EE did RTW on 8/21/2008. The person filing this report was made aware of the incident on 8/28/2008.
Employee was performing housekeeping. He stood up and hit the top of his hard hat on an I-beam.
Injured was walking over to a crib to install wedges when he tripped over a piece of high voltage guarding twisting his right knee. He was transported to the hospital where
Employee was installing roof bolts in the #5 entry face on mmun9003 section. When the left rib rolled off hitting the EE from behind, the rib injured the lower part of the EE's legs.
Employee was stepping off the crusher and as he stepped on the ground, he felt pain in his left knee.
After bolting a cut through employee noticed a loose rock and proceeded pulling the rock down with a slate bar. In doing so he inadvertently dislodged another rock causing it to fall pinning him down. The rock was removed by other employees present at the time. He was then given first aid and transported to the surface where EMS personnel took over.
Employee was directing a skid steer operator in lifting a metal saw horse when the saw horse slid off the bucket. The sawhorse landed above the safety toe of his left foot causing a deep contusion.
While bolting a cross cut EE went to swing out the bolting head and went too far pushing his body into the rib.
EE received a left rib fracture when the miner slid backwards while attempting to tram forward on an incline of an overcast, pinning him between the rib protector and the rib.
Employee climbed over framework (I-beam) between frame and belt to repair belt clamp. After repair, employee bruised rib area while exiting.
Inhaled smoke from a fire that broke out just after starting haul truck 416. She was using a fire extinguisher to try to put out the fire and breathed some of the smoke. She started to have problems breathing and EMT was called.
Employee was in the process of putting his left foot on the first rung of the ladder to enter loader. The right foot had been on uneven bottom when his foot rooled to the side.
Employee was walking to this truck from the pond. He stepped over a pipe on a slope and twisted his knee.
Victim got a mantrip hung up and was attempting to get it loose by using a come-a-long hooked to a roof bolt. He slipped and fell onto the speed reducer in the mantrip. Has a lower back sprain.
EE was loading slabs of sandstone onto a pallet by hand when he sustained pain to his lower back and left groin areas. Physician diagnosis was strain - sprain lumbar and left groin. He will be off work for an undetermined length of time.
Large piece of angle iron fell from 35' above.
Employee got off service truck, walked behind moving truck while looking the other direction. Truck hit him mid-back per employee, resulted in muscle strain to back.
Mechanic was attempting to put in bearing spacers. He hoisted up cylinder with his crane, the cylinder slipped catching his hand & striking his elbow against the tire. The incident was reported to management on 07/21 as a first aid injury. On a followup visit to the orthopedic specialist an x-ray was taken (8/12/08) & a fracture of the rt. forearm was indicated.
Employee was helping mechanic replace cutting edge on 966H FEL. Mechanic was inpacting off nuts. The bolt became dislodged from holding socket and started to spin freely. Employee was trying to hold the back of the bolt in socket and a burr caught glove, ripped glove off hand. Employee received 8 stitches to left hand.
Employee reported: Helping crew members make splice in FCT conveyor belt. He was up on top of FCT, as he was climbing down from machine, he stepped on loose material on floor, foot turned resulting in a minor sprain to his right knee.
He was putting a roof bolt up into the hole and was trying to unbend it and the roof bolt twisted. He felt a sharp pain in his shoulder.
The injured said he twisted his knee slightly while walking. He then twisted it again while stepping off an offset, causing him to fall backwards. Upon a doctor's exam it revealed degeneration of the medial and lateral compartments of the left knee. The injured suffered pain from an occupational illness. The injured's first missed day was 8/13/2008.
On 8/14/08 Employee reported a strained back which he linked to work he was doing on 8/12/08 when he was working on tail pulley on additive belt, using a pipe wrench to free belt so belt would track. This was an 8 hour job.
EE was assisting another EE spot box rail cars for loading at warehouse. As train came to a stop, EE proceeds to step off of side ladder. While stepping to the ground, he stepped on a large ballast rock and twisted his ankle.
Employee was pulling in shields, shield hung up looked out from pincher next shield over and shield dropped hitting him on head. Neck strain doing physical therapy.
EE was pulling cable off the reel of the roofbolter. The cable reel stuck, he pulled harder and felt pain in his lower back. Note: EE RTW Dec. 15, 2008 but company was not aware till Dec. 29.
The injured, along with 2 others, were installing a mechanical splice in a piece of mining belt when a scoop struck them.
Employee was out of shuttle car @ feeder when 2nd car came to feeder and hit employee's car. Employee sustained a bruised left knee.
Guiding parts on hoist, had feet firmly planted, twisted and felt knee pop.
EE was roofbolting when he stepped off the platform and slipped on a rock, hitting his knee on the mast.
Employee was installing a roof bolt when he felt pain in his left shoulder. Employee stated that his little finger and second finger was numb. Employee started missing 08-15-08.
As employee was helping another employee carry a timber, the timber fell and struck him on the left foot, resulting in a deep bruise and swelling making him unable to wear his work boot.
EE was in process of setting turn timbers in the #13 entry of Mains section for the right barrier lift, when two sections of roof broke loose from the roof, measuring from 0-2"" thick and 45"" X 28"" and 45"" X 27"". The roof fell out from around the roof bolts, striking him on back of the head, middle of back, top left hand and left elbow.
Employee was walking on the section; a rock (approximately 1-2"" thick by 24"" round) fell from the roof hitting his hard hat and right shoulder.
Employee walked through a man door and felt pain in his left leg.
Employee was mounting dozer when he lost his footing, he caught himself by grabbing the handrail and twisting his wrist.
Employee was hauling tires onto job site. While removing binders holding the load, the binder came loose and struck him in the eye.
He was operating a shuttle car tramming to the feeder when the right inby wheel came off causing him to be jarred in the seat of the shuttle car. He sustained contusion and strain to lower back.
Painting door outside by the paint shop,EE stepped on rock & fell to his knees on the hard surface. The EE reported the accident to the Co. on the date of the accident & never received any medical treatment until Friday, December 5th. The EE received an MRI on Dec. 5 & will see an orthopedic doctor on December 22 & probably will need surgery later. Currently no lost work days.
Scoop was attempting to push drill up a hill in 6 Rt. when the bucket slipped over the bumper striking the water box which struck the operator in the back of the head and neck causing injury.
Employee was changing flat tire on dump truck and strained his back muscle.
EMPLOYEE STATES HE WAS IN FORK TRUCK BASKET INSTALLING CRIB. AS BASKET WAS LOWERED, EMPLOYEE HIT RAIL ON BASKET, STRIKING HIS LEFT WRIST ON RAIL, CAUSING A LEFT WRIST FRACTURE.
Employee reported on 8/12/2008 he strained his back on 8/11/2008 while trying to move a spool of wire. He did not need medical treatment at that time. He missed work 8/20/2008 thru 8/24/2008. He presented a slip on 8/25/2008 stating he needed to be off work for this time frame, but could return to work without restrictions on 8/25/2008.
Injured came up from behind layed his right hand on the rib jack of the machine. The operator shut the machine off and when he turn he apparently pushed the lever pulling the jack in trapping the right hand and little finger causing lacerations to the right hand and little finger. Hospital referred him to a specialist.
Walking in the bone yard, took a step back and rolled ankle on a rock.
Lifting blocks, to build brattice and pulled his groin. He began missing work on 10/10/08. He had surgery.
EE attempting to sit in his chair in the primay crusher and fell backwards. EE didn't go to a physician until 8/29/08. Was not aware of his results until 9/4/08. EE received a precription for pain in leg/back.
Employee placed his foot on the track rail attempting to couple a safety chain between shield cars. The car moved and the wheel of the shield car went up against his foot. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equip not a factor, miner attitude a factor, mining equip./systems not a factor.
Employee was lifting shields off of miner, Injured/strained lower back. First day missed due to injury was 8/12/08.
Injuried left middle and ring finger lifting brake drum into the loader bucket.
Employee was driving a Euclid CH-150 coalhauler in pit 11 F section when he hit a hole in the road jarring his back, road maintenance was ongoing at the time of the incident. On 8/11/08 employee was diagnosed with a herniated disk at L5-S1 in his lower back. Employee is unable to work till further notice.
The employee was a passenger in the Terra-Pro mantrip when the accelerator pedal became stuck & the mantrip hit corner at the intersection of 12 west and northwest mains. The employee hit his head on inside of mantrip. 15 staples to close laceration.
Employee was bolting top on 6 right section. As he placed his steel against the top a kettle bottom dislodged from the top striking his right knee. On 9-2-08, MRI results came back positive for 3 tears to his menisci cartilage, was put off work until a surgeon sees what is to be done to repair his knee.
Employee stated that while shoveling coal from under the belt, he twisted his back while he shoveled the coal away from him.
Employee was operating Terra-Pro unit #1555 when the eccelerator pedal became stuck and he failed to nake the corner at the intersection of 12 west and northwest mains strikig the outby rib. Employee's ear was cut on inside of mantrip. Stitches were required to closed laceration.
Driver was going down tipple hill in 2nd gear, saw a T.C. transport truck on straddle berm and stopped truck, set park brake. Felt truck move a little before park brake set up, pushed park brake button back in releasing brake and truck ran away. He had truck out of gear resulting in truck overturning on side.
Employee was checking the cleaner circuit on B1, while walking on the catwalk between the second set of cleaner banks 1 and 2 tripped over a spray bar that had been left in the walkway leaning up against the cleaner bank 1.

Employee was dismounting from the left shoe of the dragline, while the machine was stopped, by using the 4-step ladder of this side of the dragline. He lost his grip from the last step (approx. 3 feet high), falling backward and landing on his right elbow.
Helping to make belt splice, he leaned forward to remove nail driver; he was hit on the right side of nose with a 3lb shop hammer, resulting in fracture of the nose.
The injured tripped while stepping over the belt tail piece and landed with his right foot partialy on a crib block causing him to twist his right knee. After an exam by a dr it was determined that there are mild tricompartmental degenerative changes in the right knee. This a occupational illness. The injured continued to work till the knee was examined by a dr. 8/20/08
Two belt rollers came out of the belt line and got jammed in the belt. Belt was locked out. EE pulled out one of the rollers causing the other to fall. It struck his left hand, fracturing his fifth finger.
Employee slipped climbing off the water tank after putting hose in it to fill it. Wet and muddy might have caused the slip.
Injured was operating a 630 scoop. The scoop ran over a 4x4 laying on the mine floor. The scoop tire threw the 4x4 into the operators cab, striking injured in the left kidney and side area.
Employee stated that while fueling the D-11 dozer, he slipped and fell backwards striking his back on the dozer truck.
Miner was stepping down off fork truck and twisted left ankle. There was no swelling and little pain at time. Miner completed shift. Over next few days, ankle continued to bother Miner. Visited doctor on 8/22 and 8/25/2008. Off work on 8/22 only. Diagnosed as sprain.
A pair of scrapers were excavating top soil using (push-pull) method. The rear scraper rode up onto the front scraper's stringer area, when the front scraper hit a soft a soft pocket of material; jarring the rear scraper operator. He was jarred again as he was trying to remove the bale from the front scraper's hook. He felt pain to his back.
At home in evening he removed a flake of mica from his R eye. On Sun, 8/10 went to ER & was treated (found nothing in eye) - given antibiotics. 8/11 went to Advanced Eye Care. Was told eye was scratched & was given eye drops. 8/11 reported issue to Pit Mgr. 8/15 went back to Advanced Eye Care for more treatment - did not work, Returned to work 8/18/08 able to see out of eye.
Employee was a passenger in a utility vehicle. The operator of the vehicle made a left turn and then over-corrected causing the vehicle to flip on its side.
Injured employee was unaware that the bit & insert had come off of drill steel & lodged in hole. As he shoved bolt into hole, it stopped abruptly when the bolt hit the bit & insert. Employee felt severe pain in lower back.
Employee was installing a roof bolt. A piece of material slid off of the boom striking his left forearm resulting in the employee receiving 16 stitches.
While wiping down the inside walls of the dredge classifier, the EE was sitting on a plank with his feet on the floor. He leaned out to clean the walls surface and his hand (holding a cloth) slipped causing him to fall forward. The over-extension allegedly led to a muscle strain.
Operator had truck in too high of a gear and tried to downshift on the hill. He was properly trained by an experienced operator on this truck. The miner only received some scratches and bruises.
EE was lifting a shuttle car tire onto a three wheeler, when he felt a sharp pain in his lower back. EE started missing work on 8/19/08, due to this work injury.
Employee slipped and slid onto the catwalk and landed on his shoulder.
Received a citation from the local MSHA office stating alleged accident occurred 6 months ago. No record of accident. Mine operator nor co-workers were aware an accident occurred. Attorney will be contacted to handle the denial of the claim. Employees are instructed to inform their supervisor immediately after the occurrance of an accident.
EE was putting up a roof bolt, when a piece of rock fell from the top and knocked him against the roof bolter.
On Aug 7, employee was picking up a pump when he felt a pain in his elbow. On Aug 26, 2008, he had an MRI performed on the arm. On Sept 2, 2008, Safety Dept received the MRI results, which was diagnosed with tendonitis in his elbow and was put off work for therapy.
Employee was walking around the tail end of a conveyor belt. When he bent over to get around the ""X"" brace that crosses over the catwalk, he felt a sharp pain in the middle of his lower back. ""X"" brace was about 3 feet above the catwalk.
A mechanic suffered a contusion to his right shin while attempting to access the bed of a service truck. The incident occurred when his foot slipped from the bumper causing him to strike his shin against the bumper. He is now being treated for an infection as a result of the contusion to his right shin.
Employee stated he was getting out of his truck and twisted his right ankle. He stated this has happened before.
Walking toward fan he slipped and fell striking left side of ribs and lower back on the ground.
EE was stacking 6"" hollow concrete blocks, EE turned with a block in his hand when he suddenly felt a sharp pain in his lower back.
Employee was using a pry bar cleaning muck on the shorthead crusher. The pry bar got stuck and was jerked out of the employee's left hand.
A roof bolter operator was installing his second roof bolt in a row, when a piece of a coal rider fell from the mine roof and struck the canopy of his bolter. As he turned to run away from the machine, a piece of the rock broke as it hit the canopy and struck him in the lower back. The incident resulted in contusions to his lower back.
Employee was operating a haul truck and the wind blew dust inside of the cab, which got into the employee's left eye. The employee was diagnosed with a corneal abrasion. The doctor prescribed 2 days off work.
The EE was pilling miner cable when he felt a pain in his shoulder.
Lifted a rock that was too heavy and twisted at the waist feeling a pain on the left side.
Employee was unloading a pipe-threading machine with another employee when the machine slipped. The falling machine jerked the shoulder of employee. Employee missed first scheduled shift on 9/5/2008.
The employee was injured when a steel back plate for a sand screen deck was accidently dropped onto his left foot resulting in a laceration and four broken toes on that foot.
While using pulverinzing machine, I felt my left arm muscles pull from my hand, up my shoulder. Felt numbness, tingle sensation in hands. I then proceeded to inform my supervisor of the pain I was feeling.
Employee was getting a ladder from bed of service truck, missed the step getting off the truck and fell to the ground landing on his hip and shoulder.
While working in a water tank, the employee was standing on one ladder. He lifted a second ladder and twisted to place it into a second water tank. He pulled a muscle in his lower back.
Alledged incident was to have occurred while this employee was pushing a roof bolt into the roof, he said his arm went limp and could not lift it up. Cause of this injury if occurred was overexertion. Compliance with rules/regs was not a factor, protective equip not a factor, mining systems could be a factor, miner proficiency a factor.
Employee stated he was roof bolting when a rock fell from the roof and struck his little finger and against the drill stem pot. He has continued to work until having surgery to repair a bruised tendon.
EE stepped off the back of the roof bolter, into a hole, twisting his left ankle.
On 8/6/08 EE was adjusting brakes and checking fluid levels on a mantrip at the surface motor barn. Upon completion of the checks, he lost grip on the access panel while lowering it into the closed position and dropped it approx. 18inches where it then struck EE's groin area.
The employee was walking to his loader when it started to rain. He then hurried his step and felt a pain in his left leg.
Employee was adding oil to gland water pump while kneeling down. When he pulled himself upright with the use of the handrail, he felt a pain in his lower back. He finished his graveyard shift and sought doctor's treatment later that day, which resulted in time off.
WHILE DRIVING TRUCK ON MAIN DIRT ROAD, HIT SOFT SPOT IN DIRT CAUSING THE STEERING WHEEL TO JERK. THIS ACTION RESULTED IN A LACERATION TO EMPLOYEE'S LEFT HAND BETWEEN THUMB AND FOREFINGER (RESULTING IN 9 STITCHES) AND CAUSED A SPRAIN TO RIGHT HAND
Employee was operating a Komatsu 155 Dozer. Employee backed up over large rock and upon decent from rock jarred his back.
EE was installing a rub rail on the right side continuous miner of the mains section when the rail slipped off the miner, falling on his left foot causing a fracture.
Employee stepped down from a dozer onto an object resulting in injury to right foot. Diagnosed as Plantar Fasciitis.
Employee was using a bar to lift a feeder into place. He strained too hard and injured his groin.
(Note: information received 8/7/08). Rib rolled off on top of operator's canopy. He flagged the injured employee around and asked him to swing the canopy in so the rock will fall off. When he did this, the rock started breaking and rolled off and onto his left foot.
Employee stated he ran over a rock in the roadway causing the coal hauler to bounce. His hat bounced off and then he struck his head on the underside of the hauler canopy.
