A roof fall was discovered at 16x 3 main #7 Hdg intersection only. 18' x 18' x 12' high. Fell above anchorage of 6' fully grouted resin bolts aand 8 @ 12' cable bolts. No men or equipment were injured or trapped. No ventilation or travel was affected. This fall was approx 4,500 feet outby the section.

Old North, 8, #9 entry, 44 block, MMU 004, 4-way intersection Spad 17949. 18' wide X 30' long X 8' high. Support: 6' 3/4"" resin bolts, 12' cable bolts, pans and brow straps. Fall dangered off, additional roof support installed around fall.
Rock fall 2200ft from the face in the return airway in old works, rock was 5ft high, 20ft wide and 30ft long. No injuries or equipment were involded. Fall will not be cleaned up. Fall is dangered off and supported as per roof control plan.
Pillars were pulled on the left and right side of bleeder line. Didn't leave enough pillars for support of bleeder line. Fall was inby spad # 1306.
The speed switch was found out of adjustment during normal testing and the slope car was out of service from 11:10 am until 7:00 pm. The speed switch was replaced.
A rockfall occurred in the 8th Panel North off West Main between 6+40 & 7+20. The fall was 19 x 8 x 60.
FALL BETWEEN #21 & #22 ON 2-54'' BELT. 16' WIDE X 29' LONG X 6' HIGH.
Brakes were not holding and functioning properly on the secondary hoist. We evacuated the mine, repaired brakes on the secondary hoist around 8:45 am.
Unintentional roof fall inby crosscut #3 in #4 entry in the Headgate #2 Set up Rooms.
A roof fall occurred at the #13 crosscut on the longwall. This fall occurred as a result of this area being flooded in 2006.
A roof fall occurred one break inby E.P #9 in the intersection. The fall will not be cleaned up. All entries to fall have been timbered off. Fall did not affect passage of men.
A roof fall has occurred in the #5 entry near the #2 belt and spad station #398. The fall measured 8' x 18' x 30'. The area will not be rehabilitated, but was cribbed up and dangered off.
Our #11 baghouse bags started on fire. The fire went on for approximately 25 minutes. Once fire was put out it smoldered for another hour as fire crews tried to gain access to the inside of the baghouse. No employees were injured.
An unplanned roof fall above anchorage on #1 section in the #1 entry inby spad #6756 100' from the face.
After hours, the small out building (Approximately 10 ft. X 15 ft.) caught fire and was extinguished by the local fire department. The cause of the fire is under investigation.

Unplanned roof fall in 2nd Northeast Butt #3 Entry 2 Crosscuts Inby Spad 3770. The fall was 20' long X 25' wide X 5' high. The place was bolted with 4' install #2 bolts.
An unintentional roof fall occurred in the return entry of 12 I section @ 2 x-cut. The fall measured 20 ft in length by 16 ft wide by 8 ft high. **No injury or equipment damage due to this incident
A roof fall occurred on the #6 unit supply rd. in the 1st N.W. submains. The length of the fall was 20' long x 20' wide and 10' thick. There was no equipment involved in the fall. The ventilation was not affected. no personnel involved.
An intentional roof fall occured in the #3 entry of 9I section @ SS 20+60. (between 9 1/2 and 10 X-cut) The fall measured approx. 21ft in length by 16ft wide and 8ft high. There was no injury or equipment damage.
A roof fall occurred in the 1st South Mains #6 entry 20+80 measuring 19' x 18' x 6'. The area was mined in May 2006 and supported by 6' tension rebar roof bolts. The fall was caused by a hidden slip in the mine roof.

Fall was discovered # 9 entry 2 brakes outby spad # 578 in the intake. Ventilation was not affected, passage of people not affected and equipment not involved. Fall was 19 ft x 60 ft x 4-5 ft high. Fall area was breakered off. Both MSHA and OMSL were notified.
A fall of roof has occurred in the 1st Left Panel, # 4 entry, Station spad 9+10. The fall was approximately 20 feet in length x 20 feet in width and the thickness is unknown. The area was supported with 5 foot fully grouted roof bolts.
A ROOF FALL WAS DISCOVERED IN THE CROSSCUT BETWEEN # 4 AND 5 ENTRY 2 XCUTS OUTBY SPAD 84. THE FALL DID NOT AFFECT MINERS, VENTILATION NOR EQUIPMENT. THE FALL HAS BEEN DANGERED AND CRIBBED OFF.
Rock fall occurred on 1 North Mains in the #2 entry at break #25. Fall measured approximately 20' wide x 90'-100' long x 4'-6' thick.
Roof fall in #2 entry, 18 break on 2 belt. The fall was 30' wide, 25' long, and 5' high. There were no injuries.
A fall of roof has occurred in the 1st left panel, #5 entry, staiton spad 10+90. The fall was approximately 20 feet in length x 20 feet in width and the thickness is unknown. The area was supported with 5 foot fully grouted roof fall.
6 Mains 4, 12 block, 3 entry, 4-way intersection, MMU 007. Support 6' 3/4"" resin bolts, 12' PT cable bolts, straps. 25' wide x 17' long x 10' high. Fall dangered off; additional roof support installed around fall.
An impassible condition on the tailgate was discovered during the on-shift examination of the longwall face. This condition was caused when gob rock came from behind the tailage shield flushed into the tailage entry. No roof fall occurred. Condition was reported to MSHA & Tailgate Blocked Plan was put into effect. Condition was corrected by loading rock out.
Computer shut down main fan VR-1 at approximately 1130AM. Mine crews tried to re-start fan at 1200 hrs. Computer would not allow re-start. MSHA Arlington was contacted at 1213 hrs. Underground miners were cleared from the mine by 1300 hrs. Pend Oreille Mine will call MSHA when fan is up and going again.
Fall discovered 10:30 am #7 entry intake outby spad #60648. No passage of persons affected. No vent affected, no equip involved. Fall was 18' x 10' x 5' high. Area breakered off. MSHA and OMSL notified and investigated. Moving stopping & life line around fall & danger off.
A roof fall occurred in the North sub main in entry #9 x cut12. The fall was 40ft long, 20ft wide and 8ft high. This fall did not cause injury, affect travel, ventailation or passage of people. The fall will be cleaned up.
A level insicator and a heating elemnet within a double walled 500-gallon tank ignited combustible gasses within the tank. This ignition ruptured the vessel at the top seam resulting in a horizontal gash approximatley 16"" long and 4"" wide. Damage was limited to only this tank.
Main Fuse on Number 1 Drive for the Production Hoist blew out and had to be replaced. Production Shaft is second escapeway and personnel had to be evacuated from the mine until fuse was replaced.
BG-4 loadout building caught on fire. Welding had been done in loadout approximately 2 hours prior to fire being noticed.
A roof fall was discovered in the #1 entry of the 3-Left Panel off 2-Left #2. The fall measured 18 feet by 60 feet and was 6 feet in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts. No equipment was involved. The area was dangered off.
The brakes are not holding on the steam hoist. Hoist will be taken out of service until brakes are repaired. This will not affect our facility, we still have two shafts up and running to get employees in and out of the mine.
Our examiners found a roof fall during their weekly examination of the 5 West worked out area. The fall was located adjacent to the examiners route of travel. The fall was located at crosscut 61 between entries 2 and 3.
The overspeed switch sensor was malfunctioning. The hoist returned to service at 8:30 am.
Found crack in drum break linkage. Removed hoist from service. Adjusted, reassembled & tested brakes. Hoist was returned to service at 3:30 pm on 10-24-08.
The mine lost power due to fuse on K.U. Power Center. The hoist was inoperable. Power came up at 1:55am
A roof fall occurred in #1 entry, 2 breaks outby spad #255 and 7500 feet from the active face. The fall was approximately 25' x 20' x 4.5' thick and will not be cleaned up at the present time.
Smoke discovered at 6:28 a.m. and reported to the control room operator. A loader operator and the water truck were called to the site. They dug out the portion of the coal that was smoldering, sprayed water on it and back dragged. This took 30 minutes. It was reported to MSHA at 6:58 a.m. due to the possibility of reappearance.
A roof fall was discovered north B 13 x # 3 Hdg intersection. Fall above anchorage. 18' x 18' x 7' high. 6' fully grouted resin bolts were in use. No equipment or persons were injured or trapped. No ventilation or travel was impeeded. This fall was approx 3,000' outby the working section.
A roof fall occured 4 Main 20 X # 7 Hdg intersection. Approx 12' high, 18' wide, 18' long above anchorage. 6' super twist & 12' post tension cable bolts were in use. No men or machines injured or trapped. Ventilation was impeeded in face of 7 x 8 @ 20x. Fall was 180' outby face and even with the loading point.

An unplanned roof fall occured on #2 section in the #5 entry at spad #5525 3000' from the face.
An ignition occurred at approximately 82 + 00 in # 2 entry, E-19 Section. The ignition lasted approximately 90 seconds and was extinguished by the use of a wash down hose on the miner and a fire extinguisher. There was no damage to equipment and there were no injuries.
Our examiners found a roof fall during their weekly examination of the 4 Main North returns between crosscuts 148 and 149 in entry #3. The fall was located in their normal route of travel.
A rock fall occurred in the number 2 entry 3 breaks inby spad # 1934 at break number 25 in the No. 2 SE Mains. Fall measured 20 L X 30' W X 5' H and bolted back up with 5' TT bolts.
A roof fall occurred in the #2 entry return in the Mains section at Survey Station #62 approximately 2500"" outby the face, measuring 20' wide, 20' long and 8' high.
Roof fall in the 8HLG bleeders 8K#1 entry was found by the fireboss making his weekly walk. Was reported to the surface at 12:00PM 10/22/08. No one was trapped and ventilation was not affectted.
The number 8 elevator was inoperable for longer than 30 minutes due to a lock on the inside door staying in the locked position. A lock set screw had worked loose and was retightened. There was one individual on the elevator who was out within 5 minutes. The elevator was returned to service at 11:45 pm.
Discovered fall in return, #1 entry of 010 MMU 20 ft inby spad 44529. Fall was 19' X 60' X 8' high. No equip involved, no passage of persons affected, no vent. disturbed, entrances were breakered off, will not be rehab, MSHA and OMSL were notified.
Elevator stopped working at 8:30 pm. Discovered the governor cable had come off of shieve wheel. Lakeshore representative was called in to install a new cable. The elevator was back in service around 6:00 pm on 10/23/08.
A roof fall occurred in the #2 return entry of the 1st north west main return at spad # 2568. The fall was 20 foot in length and 9 feet in width not blocking the entire entry. There was no equipment involved. The area involved was posted and cribbed.

Fall occurred in crosscut 27 of intersection 2-3. Approximately 24' long, 18' wide, 6-1/2' high.
A roof fall occurred in the 1st. South main entries between 18+40 and 19+20 in #7 entry. The area was mined in May 2006 and supported with 6' tension rebar roof bolts. The fall measured approximately 18'x70'x6' and was caused by an undetected slip in the mine roof.
A non-intentional, non-injury roof fall occured at spad 2706, The fall was in the intersection and through the left break. Size of the fall measured 50' long x 4.5' - 5' thick x 7' -10' wide. Roof support was 4' fully grouted 75 grade resin bolts. No equipment was involved. Ventilation was not affected. Fall will not be cleaned up.
Our examiners found a roof fall in the 4 Main North returns at crosscut 181 to 182 in entry #7. The fall was located adjacent to the examiners' route of travel.
Smoke discovered in coal pile & reported to control room operator. Called two haul trucks to remove coal from area and create a separate pile with this coal. Loads were watered and back dragged in order to put out the fire (smoke). This took close to 7 hours. Control room operator contacted MSHA at 7:20 a.m. to inform about the incident. No signs of fire after 10 am on 10/21/08.
Roof Fall, 3 West, #2entry belt neutral (outby section tail 1-2 xcuts), xc 36, 40'x40'x10' thick Area was good when mined and 2 weeks later started busting up showing signs of fault. Area was bolted with 8' tension rebar, Depth is over 600' of cover.
This mine experienced a roof fall in #7 entry at the 470' cross cut.

Power was lost to all of mine (surface and underground) rendering hoist unusable. Evacuated employees out escape shaft. Electric company restored power to site at 7:45 AM.
While transporting a Wallace 14 man diesel mantrip (on the flatcar) to the slope bottom, the hoist rope broke allowing the brake car, supply car & diesel mantrip to crash into the rib & destroy all 3 pieces of equipment.
EE was replacing a pipe under a screen tank. He noticed after cutting,couple sparks falling on a rubber hose. He poured a bucket of water into the hose then covered the hose to prevent further problems. He noticed smoke again coming from the pipe. With assistance & fire extin, he determined the fire out of control and contacted

Upon attempting to start the Main South belt for day shift production, the belt would not start and run. An examination revealed that a roof fall had occurred between crosscuts 48 and 49 on the belt and structure. The examination revealed that a slip was present in the mine roof that caused the fall.

The continuation of a previous roof fall was discovered at approximately 1:00 AM. The continuation measured approximately 8 ft in heigth, 10 ft in width and 15 ft in length. No injuries to personel or damage to equipment occurred.
The # 8 elevator was inoperable for a period of time exceeding 30 minutes due to a loose wire connection on the BRO contact. The wire was re-terminated, the elevator was tested and returned to service @ 1:44pm. No individuals were on the elevator.

Unintentional roof fall discovered this date while examining escapeway. Fall measured approx. 50'L X 17.5'W X 6'H. Previously supported with 6' full grout resin bolt. Area developed on or about 6/15/08. No injuries. No property damage. Escapeway not blocked. Approximately 290' overburden.
There was a roof fall in the rooms, one break right of spad #2778. The fall was 100' long x 15' wide x 10' thick. The area had been bolted with 4' #5 fully grouted resin roof bolts.
Elevator at Somerset Portal damaged, two wires shorted in a relay for the dynamic brake causing the elevator to stop while ascending.
The RB #10 mine was on retreat mining. The continuous miner was in the 2nd lift in the 3 entry left pillar about 30 feet into the pillar. A piece of rock fell onto the Emico miner that was 5 feet thick on the miner cutter head and 3 feet thick on the miner tail. The measurement was 5' to 3' thick, 12' wide and 25' long. The rock did not fall into the entry or pull any bolts.
Smoke discovered in coal pile at 6:58 am and reported to control room. A utility leadman, utility man and water truck went to coal pile. Coal was dug out, watered and back dragged by loader. Supervisor didn't believe fire was going to be out within 30 minutes, so the control room operator called MSHA at 7:45 am to inform about the incident. Fire was extinguished by 9:50 am.
A roof fall occurred on the 004-0 MMU in the #2 entry, parallel to a roof fall in the #3 entry. Persons, ventilation, nor equipment were not affected. This area will be cribbed up and otherwise supported for safe passage.

An unplanned roof fall was discovered in north mains in the number one entry at 67 crosscut. The area fell between cribs and on cribs in an intersection. It measured 15' wide by 12' long & approximately 6' high at the highest point above the orgianl roof line.
A rock fall was found by employee while making weekly examination of the return and the old worked out area of the mine. The fall was in the No 4 Panel, No. 5 entry, 9 crosscuts from the No 1 main entry. The rock fall was about 20 feet long, 20 feet wide by 4 feet thick. The No. 5 entry is cribbed off and is not traveled by the examiner

Roof fall at crosscut 26 in entry 6. It was the intersection that fell 5'3"" above roof line.
High volatlity caol and high winds allowed fire to spread quickly, resulting in damage to belt carrying rolls.
THe primary hoist was down from 0150 due to failure of the lily over travel. mining was stopped and workers pulled back from the face. at 0240 the cage was repaired, test run was perfumed and back in service.
Roof fall in the 8N bleeders between 8N and 8O. Approximately 18'WX20'HX60'L. This area was bolted with 5' jenmar combination bolts. We are installing 5' jenmar bolts in working sections and supplementing them with 8' cable bolts where needed.

The fall was about 6,300 feet outby the MMU 003-0 section and measured approximately 30' long x 84"" thick x 18' wide. There were no injuries or equipment involved. Ventilation was not interrupted. The #3 hoist tripped due to a computer issue. The hoist could have been run in manual or evacuation mode if needed

A roof fall occurred in the #9 seam in the 2 Main East at crosscut #65 in the pillarline between #2 and #3 entries measuring approximately 20ft. x 19ft. x 5ft. high. This area was supported by 5ft. fully grouted roof bolts.
An unintentional roof fall was discovered at old #15 belt in the #5 entry @ 33 brk, one brk inby spad 9079. Size: 18'W x 20'L x 4-5'H. The area had been bolted with 4 foot fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will not be cleaned up.
IGNITION: While cutting and cleaning up coal in the No. 3 Face on the No. 8 Section, a frictional gas ignition occurred. The flame was put out with a fire extinguisher and lasted approximately 20 seconds.
While doing Safety Checks on # 4 Hoist, hoist kicked out and wouldn't reset power. Investigation by Hoist supervisor pinpointed the new Safety Interlock device on the Lily control was out of adjustment. Hoist was back up in operation at 8:05 AM. Underground personnel had two other hoists for egress out of the mine.
An unintentional roof fall occurred on the 3C panel at station 27+55 (x-cut 35) between Entry 6 and Entry 7. The fall measured 30 feet x 18 feet x 5 feet. The fall did not affect ventilation, passage of men, or cause personal injury. The fall had been bolted with 4 foot fully grouted roof bolts.
The elevator at Cook's Branch was reported having a problem @ 10:09am that prevented it from operating properly. It was repaired and returned to service at 11:31am. The MSHA hotline was contacted.
In the B-8 Headgate section at cc 26 a roof fall occurred in supported roof. The fall was a 20 ft. wide, 20 ft long, and 7 ft. high. A thin coal ryder seam was located at the anchorage zone.
The roof between the last shield on the face and the supplemental supports installed in the tailgate failed resulting in a blockage of travel off the face through the tailgate. The entry outby the face area remained open.
A fall of the mine roof was discovered in the #6 entry at break 91-92 along 1 south belt. The fall was approx. 20' w x 10' h x 20' l. There were no injuries and no airways or escapeways blocked. The area has been cribbed off.

A roof fall occcurred in the No. 5 entry, 500' outby the working face, measuring 20' wide by 30' long by 4' high. The affected area has been cribbed at entrances to the fall.
While traveling airways a roof fall was identified in the old Mains Section at break #62 in the #1 entry near spad #358. The fall extended right handed into the crosscut and measured approx. 20'L X 25'W X 6'H. The area was supported with 5' torque tension and 12' supplemental supports. The area has been dangered off and timbers have been set.
A roof fall occurred in the #11 seam in the 2 Main North at crosscut #46 starting in part of the pillarline between #5 and #4 entries and extends through part of the pillarline between #3 and #2 entries measuring approximately 100ft. x 18ft. x 3ft. to 8ft. high. This area was supported with 30 inch fully grouted roof bolts.
Main hoist system experienced a field loss when running the daily brake test. After locating the problem, fuses were replaced and the problem was corrected. No men were underground at the time of the problem, and no injuries resulted from the hoist being out of service.
Rock fall occurred on 1 East Mains at break #14 between the #4 and #5 heading and extending over to the edge of the #6 heading. The rock fall was approximately 20' wide x 90'-100' long and 6'-8' thick.

THIS WAS IN AN AREA THAT WAS BOLTED WITH 5' BOLTS PRIOR TO THE OPERATOR VOLUNTARILY CHANGING TO 6' BOLTS. THE AREA WAS DANGERED OFF TO PREVENT UNAUTHORIZED ENTRY.

Non-injury, non-intentional roof fall was discovered at 2:30 PM by Mine Foreman on Panel #2 one break outby the #9 Seal. Size of the fall was approx. 90' long, 20' wide and 5+' high.
A roof fall occurred in the #1 return entry, spad # 928 and in 2nd N.west mains. No equipment was involved. Area was cribbed off.
IGNITION. The Continuous Miner was cutting the bottom rock in the No. 3 Entry on the No. 4 Section on the curtain side of the Face when a frictional ignition occurred.
A roof fall occurred in the 2nd south at X cut 6 in the entry #3. The fall measured 30ft long, 20ft wide and 81/2ft high. The fall did not cause injury, passage of people or ventilation. The fall will not be cleaned up.
An unplanned roof fall was discovered at old #15 belt in the track entry, size was est. to be 30'L x 18-20'W x 4-5'H. The area had been bolted with 5' fully grouted resin bolts with supplemental support consisting of 9' cables bolts, no injuries, no equipment caught or ventilation was not altered. fall will not be cleaned up. location of fall was #33 break track entry spad #9075.
A roof fall occurred in the #9 seam in the 2 Main East in the pillarline between crosscut 64 and crosscut 65 in #3 entry measuring approximately 40ft. x 18ft. x 5ft. high. This area was supported with 5ft. fully grouted roof bolts.
A roof fall occurred in the #9 seam in the 2 Main East at crosscut #65 in the intersection of #4 entry measuring approximately 25ft. x 18ft. x 5ft. to 9ft. high. This area was bolted with 5ft. fully grouted roof bolts and 9ft. mega bolts.
A roof fall was found inbetween #3 & #2 xcut on #4 belt at #4 xcut. It was 6' high, 18' wide & 50' long. Mine prop was put at end of fall & cribs at other end.
Rock fall occurred on 1 East Mains on #4 belt at break #45. Fall measured approximately 20' wide x 90' long x 5'-6' thick.
Unplanned fall on the #4 track entry measuring 60 ft in length x 19 ft in width x 6 ft high. The area was breakered off and is being rehabed. The fall was investigated by person from WV OMSH & T and from MSHA.
A roof fall was discovered in the 2 North 3 Mains, 36 crosscut, between #5 and #6 entries. The fall measures approx 45' long, 18' wide and 6' thick. The area is supported with 5' fully grouted resin bolts.
Our examniers found a roof fall in the 1 Left 5 Main North panel during their weekly examination of the area. The fall was located in a crosscut adjacent to the examiners' route of travel.
Had roof fall approx 15'L8'W3'H. in our #7 entry 1 brk inby spad #26 in intake airway approx. 20 brk. Outby working faces. Top was heavily Laminated. Additional cribs and headers were installed to make area safe.
Loss of power to our # 4 man shaft that would interfere with use of the equipment for more than thirty minutes. We actually have three different man Hoists.
A roof fall occurred in the C-1 butt section approx 100' from the face of C-1 butt, not being mined. The roof fall was 16' to 18' in width, 25' in length and 4 1/2' in height. Area was bolted with 4' resin bolts. No one was working in area. Mining was being done in return rooms. No ventilation or escapeways were affected.
Roof bolter was tightening a rib pin when an ignition occurred. No injuries occurred. No damage to equipment or ventilation controls occurred. No citations were issued.
#3 hoist (production hoist) lost power and tripped due to a transformer malfunction. There were still two operable hoist for the duration of the hoist power issue.

a methane gas ignition occurred at the 8 east rock dust borehole installation project. no injuries or damage to equipment occurred. proper methane test had been and were being conducted when the ignition occurred. a feeder suddenly released methance from around the borehole pipe just as employee stated to burn the pipe into. the ignition was immediately extinguished.
7 Mains 2, 32 to 33 block, 7 entry, MMU 002, #16721. 17 ft wide x 40 ft long x 8 ft high. Support: 6 ft 3/4 inch resin bolts, pans, brow straps, screen. Fall dangered off, additional roof support installed around fall. Clean up plan developed and posted at fall site.
Discovered a tailgate blockage. Investigation revealed that the majority of the TG entry was open, roof fell between last shield on face and the installed roof to floor supports, thereby blocking travel off the face.
IGNITION. The Continuous Miner was cutting coal on the No. 8 Section in the cross cut between the No.2 & No.3 entry on the curtain side when they saw a flame on the mine floor. They put the flame out with a water hose and one fire ext.
Foreman was making his weekly examination on the primary escapeway. When he came across a rockfall. The fall was located 15 feet inby the fanhouse in the #4 entry, the fall was 10 feet in length 16 feet wide and 5 feet to 6 1/2 feet high.
Unintentional roof fall discovered at 11 Zone, 29 block, No. 1 entry, 1 block to left of station 919. Fall measured approximately 40'L X 6'H X 18'W. Area was previously supported with 6' full grout resin roof bolts. Area was mined on or about 02-02-07.
Roof fall in #3 entry of C-Butt last open x-cut adjacent to station # 7368. Affected area was approximately 7-8 ft high and 18 ft wode. Roof fell on miner and Long Airdox bridge
On 10/3/08 the roof set down at 3+70 in the tailgate of 2B longwall. Face was at 3+90. Roof didn't fall but as a result the approved blockage plan was put into effect. At the time that this was called in to Safety Supervisor, it was not known if it was a fall or not so the agencies were called just in case. It was found that it was not a roof fall.
The Production Hoist kicked out and would not restart. It was determined that the lower over travel switch in the shaft had shorted out because of water. The switch was replaced and the hoist started again. The mine had to be evacuated when the hoist went down, because the Production Shaft is the second escapeway out of the mine. The hoist started again at 5:45 p.m.
A roof fall at 2nd left mains panel I at intersection of spad # MP 12 of old works. No ventilation problems. Was found on weekly airway travel. Area has been timbered off.
Roo ffall was due to weak mud stone top. Fall was at #7 x-cut. #2 entry of 8th left section. No damage and no injury.
Had a fall on 3rd shift in last crosscut. 18-20' wide, 80-85' long & 6' thick. Fell from 2 right to 4 left. Laminated sandstone
EE was cleaning and rockdusting the #3 heading when he found a 28 inch auger hole on the right rib at 2:40 pm and reported this to Hillard Belcher Mine Superintendent and was called in to MSHA at around 2:48 pm. At the time the auger hole was found it was checked for hazards and none found.
MMU 002-0, #7 entry, while cutting coal a gas or dust ignition occurred. There was a brief flame confined to the center of the head which extinguished immediately. There were several small kettle bottoms in the top at this location.
South 1, #7 entry, 39 block, spad #2528, MMU 006-0, 4 way. Support: 6 ft 3/4 inch resin bolts, 12 ft cable bolts, pans, brow straps, screen. 20 ft long x 20 ft wide x 7 ft high. Fall dangered off, additional roof supports installed around fall. Clean up plan developed and posted at fall site.
A non-injury accident, the unintentional stoppage of the elevator at the Oakdale Portal entrapping 6 people on-board from 5:23 pm until 7:15 pm. A repairman from Schemdler was called to correct the malfunction.
Non-injury, non-intentional roof fall occurred on the 4d Panel 130' inby SS 3068 in the #2 entry. Fall measured 7' -8' high, 18' wide, and 12' long. Area is dangered off. Ventilation not affected.
Hoist was down due to changing idler shaft roller.
An unplanned roof fall occurred in the #3 entry of 1 Right, 1 x-cut inby survey station #683. Fall was approximately 4' plus high, 19' wide, and 35' long. Area was timbered and dangered off. Did not affect any ventilation but did cover about 35' of belt and structure. Will not be cleaned up. Area was bolted with 4' compression bolts and was also center posted.
A caterpillar 777 haul truck rolled over an embankment coming to a complete stop and landing on all four wheels.
Fell at x-cut 38, 5th 48 # 7 entry. Length 30' Width 20' Thickness 6'
Roof fall occurred in the neutral entry 25 crosscuts outby the face area. The fall was 42' long X 18' wide X 4'-5' high. It does not impead any travelways, intakes or returns. All routes to the affected area have been cribbed and timbered off. We do not plan to clean-up the fall. No persons were affected.
A non-injury roof fall was found in the #3 entry of 1st north at X-cut 15. The fall measured 20' long X 30' wide X 7'-8' high. The area was dangered off and marked on the mine map. This area is a common entry not walked during weekly examination. During an MSHA inspection on 2/13/09 the roof fall was observed, a citation issued, form filled out to terminate citation.
An unintentional roof fall occurred on the Main East at 39 crosscut between #6 and #7 entry station 30+20. The area had all approaches timbered off pryor to the fall. The fall measured 30 feet x 18 feet x 4 feet. The fall did not affect ventilation passage of men or cause personal injury.
Across from spad 573 #10 Break on the Northwest Section in the intersection an unintentional roof fall. 18 x 18 x 10' high.
There was a frictional ignition on the 6-mains section. The remote control miner ee started his machine and his bits were not penetrating the coal. His right side bits contacted the right rib that had a sandstone streak. The operator stated that he saw a yellow flame above the right drum that lasted a few seconds.
An unintentional fall occurred in the main north, entry #11 at the 23+60' The fall fell in a 3 way intersection. The fall was 20' long by 25' wide by 11'high.
An unintentional roof fall occurred at the Galatia Mine New Era Portal on the 2nd West longwall, Corsscuts 15-16. The fall was approx 59' long, 19.6' wide and approx 8-10' thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up.
Three people loaded elevator w/4000 lb liners for installation on 2nd floor. They proceeded down to floor riding elevator. They left 7th floor and when elevator traveled approx. 10' it stopped. It wouldn't clear safety stop alarm. Three persons were trapped from 2:15 am; extracted 3:40 am.
An unplanned roof fall occurred in the #130 mine on #5 belt, brk. 21 that measured 40' long x 20' wide x 8' high. The area was developed on 4/12/07 no injured were reported.
An unintentional fall occurred in the 2nd west return just inby the 13+30'. The fall was 18' wide by 30' long by 11' high.
A non-intentional, non-injury roof fall occured on the the 002 section at spad 3888 on Panel I in the #5 entry. Size of the fall measured 19' wide x 40' long x 12' high. Roof Fall will not be cleaned up and will not affect ventilation.
The mine foreman and the safety director were traveling to the section when they noticed the top working near #7 belt/track entry in the nuetral airway. The men cribbed the area off then the top fell shortly after.
All we know is a call came 10/29/08 about 8:00 a.m. that we had a fatal accident at Kennecott; our driver was killed by a pipe falling off the top of load on trailer. All Bob Orton Trucking does is haul the load (this load from WL Plastics Corp, Cedar City, Utah). We do not load or unload product.
Employee received fatal crushing injuries while operating haul truck, parked in line waiting to be loaded at the shovel. The employee exited the cab and was standing behind the cab, between the cab and air receiver tank when the truck in front started to move in reverse, pinning the employee.
Building a road when dozer overturned. Zero witnesses to accident. Dozer rolled down steep 80 foot incline.
Employee was disconnecting drawbar from dolly on outby end when trail motor collided with dollies and pushed dollies and employee into outby motor fatally injuring employee.
Victim was scaling in level 9 north 8 when a large rock fell from the roof crushing the cab of the scale machine causing fatal injuries. The accident is still under investigation by MSHA and the mine operator. The root cause analysis is not complete at this time.
WHILE OPERATING CONTINUOUS MINER, THE OPERATOR PINNED HIMSELF BETWEEN THE MACHINE AND A COAL RIB.
The Employee was on the surface of a clean dry barge in daylight hours with no movement and light wind. The Ee passed through the #2 port hatch opening and made contact with the empty hopper floor. Cause is unknown. Ee was installing a dust prevention tarp across the port hatch opening before the incident. The operator contests MSHA jurisdiction.
Two employees (welders) were dismantling a large tool rack. The welders were cutting 8'X4""X1/4"" plate steel from the frame. The rack became unstable and tipped over on top of the employees. One employee sustained fatal injuries and the other received injuries to his back and legs.
Employee was assisting in the movement of supplies at the Justice Mine of Independence Coal Company, Inc. During this process he was apparently pinned between a parts car loaded on a rail car and a rib near the first left switch.
The employee was attempting to add a piece of angle iron to the discharge gate to the surge pile feeder. After welding, the gate did not fit properly. He crawled to the gate to investigate. He asked a co-worker for a tape measure. While waiting for the tape measure the material started collapsing around him. As a result the employee was engulfed in the material.
Employee was driving along roadway at approx. 5:30 a.m. The vehicle he was driving went through a berm and landed in approx. 45' of water. Employee was found outside the vehicle.
The injured a section foreman was in the process of placing his gloves on the roof bolter when a piece of rock fell from the mine roof and struck him on his left hand. The incident fracured his left pinky finger and cut it. Surgery was required to repair the injury and a portion of his finger was removed due to damage.
EE was shoveling gravel at crusher and felt slight pain in left abdomen area. Continued to work and later noticed a small lump in left abdomen area. Did not report anything until 11/3/08 when he felt a small pain and the lump reappeared. Sought medical attention and was diagnosed with a hernia in left abdomen.
Employee got finger caught between frame of scoop bucket and hydraulic ram in scoop bucket, resulted in fractured distal phalanx right middle finger.
Employee caught glove between sandpaper and shaft while sanding a shaft with a lathe, The sudden motion severed the tip of his right thumb.
Employee was changing bits while he was bolting when a rock fell from the roof and struck his right foot causing fractures to the metatarsal bones. *Amended - Due to infection, injured's toes were amputated. He remains off work.*
Employee states that while setting inner tube his left ring finger was amputated, and he sustained a cut on right thumb.
Employee was changing a floating axle out on shuttle car when the chain he placed on axle tightened catching 3rd digit finger tip on left hand cutting tip off. Employee has returned to work on regular duty.
EE was removing a bag from the packer when the bag felt slightly stuck. He pulled harder and felt a pop/snap in his abdomen and then felt pain.
Employee reached out to pull fall block towards him and block was still lowering, which pulled left hand into fall block sheave, causing amputation to four fingers on left hand. Fingers were to badly damaged to be reattached.
Injured was trying to pry fender back on S/Car, when fender fell. He grabbed the fender to keep it off of him, as it fell, resulting in twisting his lower back and right leg.
Lifting a piece of 3"" pipe and strained abdomen.
As employee was unloading rock dust from the rail car, the pile of rock dust tipped over and caught his leg between the rock dust and the rail car.
EE stated as he was exiting the 463 bob cat, he felt a pain in his thoracic area of his back. He stated that he took some advil to relieve the pain, but after 2 hrs he states that it is feeling worse. He aslo stated that when he was moving scrap vessel chain around yesterday (10/30/08), he may have aggravated his back.
An employee fell 4 feet from the deck of a lowboy while dismounting a front end loader vertical ladder.
Stepped out of ladder and twisted his ankle.
Employee stated that he fell on an icy patch, twisting his right leg underneath him. He complained of back pain and was taken to the Emergency Room for evaluation. Employee lost 1 work day and returned to work 11-3-08. He began losing time again on 11-5-08. (Dr slip off work until further notice)
While changing steels got hand caught between steels.
He was running to shut off feeder on breaker; while on his way he stepped wrong and hurt his left knee. He continued working that day. Oct 31, he came to work and realized that he needed to have a doctor look at his knee. Doctor told him to take it easy over the weekend and let the swelling go down while using crutches, and follow up with him on Monday, Nov 3rd.
Employee was tramming a buggy down traveling on #2 belt line and caught his head against the top. Injured his neck and left arm.
While employee was walking to the Secondary Crusher work station, he stepped on some uneven fines and small rocks and twisted his right ankle.
Still early in shift, workers were using skid steer to clean underneath conveyor. Loader moved by and obstructed lighting, causing skid steer operator to not be able to completely see around him. When loader operator backed out and swung away from conveyor, cleaning plate on bucket of skid steer struck ground man on side of foot.
The miner was working on a 30"" ventbag. The wire holding the bag in place broke and the end whipped around and struck the miner in the face knocking out his bridge work and breaking a tooth.
Employee stated that he got off the ram car and was walking to the dinner hole when he tripped over a waterline. He put his arms out to catch himself and felt a pain in both shoulders. Employee's FLWD did not occur until 1/7/09 due to surgery.
The employee was cutting a steel plate that was 1""x48.75""x7.75"" in dimension when the steel plate fell approximately 4 feet onto the employee's foot. The employee was taken to the Colstrip Medical Center for treatment where it was discovered that the right big toe and the toe adjacent to the big toe were broken. The employee missed their next scheduled shift due to surgery.
Person pushing wheelbarrow when it tipped over resulting in strain to right shoulder. Person returned to regular duties 10/29/2008. On 03/02/2009 person missed work as a result of surgery on shoulder.
Miner stepped up onto the roof bolter and felt a pull in her shoulder.
Operator states that he was dumping a load of rock when the truck bed came down and jarred his tailbone. X-rays revealed no broken bones. Bruised tailbone.
was putting pipe together and smashed left thumb between the pipe and the work boat. The left thumb is broken.
Injured reported that he had ran his head into a roof bolt while walking to elevator. It was reported that he went to the hospital and have not heard from him since.
Injured Employee was part of a scaling crew. He was pointing out a loose rock on the opposite rib when a rock fell from the roof striking him on the left shoulder. Injuries are fractured left clavicle and 3 compressed lumbar vetibrae.Employee should have not been standing in the immediate area of the scaling.
Walking to truck, slipped & fell in mud. Bruised ribs. Went home, changed clothes & came back to work. Hauled 2 loads of coal, complained about ribs hurting. Went to United Hospital Center & got doctor's excuse to take rest of day off work. Called in next day & said he was too sore to work. Returned to work next day.
While welding on an end dump truck, sparks landed on the injured's clothes and caught them on fire. He suffered 1st, 2nd, & 3rd degree burns on his upper left torso from the wind blown fire.
Injured was exiting motor and stepped on a piece of coal 4"" x 4"" twisting his left ankle. He was transported to Welch Emergency Hospital. He was treated for a sprained left ankle.
Person had turned off a fire valve and after doing so, he felt a pain in his lower back. Valve had been left on to flush fresh water line after belt move. Person jerked on valve to close causing injury.
Tightening roller bolts on a D11R with a ratchet and prybar, felt a pop in right forearm.
Getting into scoop cut wrist on water band.
Employee slipped and fell off the back of a maintenance truck. Fractured right collarbone and sustained a concussion.
Member was standing in the #7 entry talking to the miner operator when a small piece of draw rock fell and hit him. Note: The Dr. did not take member off from work until 11/6/08
On 10/28/08 the employee stepped on a piece of coal, lost balance and fell on their right arm and back. The employee followed up with a doctor for futher examination. A cyst was found in the right shoulder (pre-existing). The employee chose not to do any medial treatment at that time. On 3/30/09 the employee notified that he would like to have surgery on the right shoulder.
Checking radiator leak on truck, burned wrist/forearm.
Employee was using a large impact wrench and the weld on the bolt broke, driving the wrench downward, pinning the hand. The right middle and ring fingers were fractured and surgical repair was required.
Employee states that he was pulling miner cable when he felt a pain in his right groin. Initially thought to be a muscle strain was later diagnosed as a hernia requiring surgery on January 5, 2009.
EE clocked out of work and drove his personal vehicle down to the southeast pit area. He stated that the sun blinded him for a moment and he drove his personal vehicle over a berm and into the pit. When this happened the car rolled over and EE's collar bone was broken.
Injured was driving miner bits with a hammer when the bit burst and a piece embeded into his left hand above the thumb. He was sent to the hospital where they sent him to a specialist to remove the metal.
Employee was drilling starter hole when a piece of rock fell striking him on the arm. Employee started missing 10-30-08.
Employee was helping recover track rails when one of the track rails slipped in the scoop bucket and caused the other end of the rail to hit him in the right ankle.
Injured was shoveling belt line when he felt something give in his right shoulder. He stated that he had previously hurt the shoulder. He was transported to Welch Emergency Hospitol.
Dismounted the haul truck, misjudged the distance from the bottom step to the ground; held onto the handrail and stretched the upper body - pain in abdomen: hernia.
EE had completed a pre-shift of the belts & was pulling into the belt entry on a 3-wheeler to mark the date board. he hit a hump in the entry and was bounced into the mine roof. He was taken outside by the beltman and transported by ambulance to Medical Center. CT scans and x-rays were negative for fractures or deform. hospital
EE was climbing up onto his D8L dozer. He slipped attempting to pull himself up the first step onto the track pads to reach the second step. He fell backwards first landing on his feet, then hitting his tailbone & then his back on the ground. Weather conditions were good and temperature was approx 41 degrees. He was covered with a blanket and was transported to a local hospital.
Injured was walking and stepped into a hole approximately 8"" deep by 12"" wide by 24"" long fracturing his right leg at the ankle. He was transported to the hospital where they are treating him for a factured leg.
The roof bolter oper. had right hand on top of drill pot while moving the roof bolter into position, a piece of rock fell from top striking him on the hand.
Removing a piece of drill pipe that needed replaced and as employee was lifting the pipe he got a sharp pain in his left arm-bicep area.
Employee was mounting the haul truck and slipped on the third step down from the top and hit her knee on the steps. She had a small scratch and bruise, and was complaining of pain in her upper leg and lower middle back. She was taken in for examination at the ER, diagnosed with back strain, referred to her personal physician and taken off work. ONGOING.
While pulling a hung steel with jackleg, strained left shoulder and neck area.
The employee was changing a bushing on the continuous miner. A piece of the bushing came off and struck the employee in the upper arm.
Employee was walking down a dirt incline and stepped on some lose material. Her right leg gave way and she caught herself with her right leg. Employee felt a sharp pain in her right lower back that radiated down her right leg. At clinic employee told nurse that pain had been bothering her for the last several weeks.
Employee was walking around in loose coal around the hwm when he broke his left ankle.
Employee was opening a box of resin when some unknown object struck him in the eye causing an irritation.
Employee was removing a fuel injuector line while lying on his back and stretching & twisting to reach the clamps on the line. He felt a popping in his back. Employee did not seek treatment until 11/12/2008.
Working on the #5 belt, putting in stands after all stands were put in, EE was resting hand on stand when a rock fell from top, broke EE's wrist and thumb.
While installing a dust seal on a main shaft, the seal supports shifted causing the dust seal to fall, striking employee's right arm and hand.
HE WAS HANGING TIN AND TURNED IN THE MUD, TWISTING KNEE. ** STARTED MISSING WORK ON 12/08/08**
The employee parked the bulldozer that he had been operating in the equipment parking area at the end of his work shift. He then got out of the cab of the machine and either jumped or fell to the ground. He was taken to the hospital where he was examined and released, but claims his back is hurting too much to come to work.
The employee stated that he was walking around the metal storage rack behind warehouse when he tripped on a metal band on a skid. He fell striking his left elbow on the ground. At the time of incident power was out on the surface so there was no lighting. This incident became a reportable, when he had surgery on his elbow on 1/16/09.
Employee was lifting a coupler from a Getman, felt a burning pain in his stomach. Employee continued to work. Employee went to doctor where his diagnosis was found to be a hernia. A surgery was scheduled for 11/18/2008.
EE was hosing up rib protectors on Alpine miner. When power was energized, the EE was stamping between rib protector and machine. The rib protector struck EE causing pain to his lower abdomen.
miner was helping lift parts for overcast steps when the part slipped and lacerated his finger.
EE was trying to remove grizzly bar from stacker belt. Turned on magnet to help lift from belt. Two middle fingers got caught between magnet and bar. EE was wearing gloves but tip of middle finger was amputated and finger was broken.
EE walked on rollers to unjam a broken bag when her foot slipped, causing a roller to pop out and her leg to fall in the hole. She received bruising to her right thigh.
Deck of shovel was muddy and his feet slipped from under him and he grabbed for the handrail and strained his left shoulder, elbow, wrist and hand.
While walking in the Flotation Deck area, EE encountered a small decline and fell forward onto her hands and knees. She experienced pain in the back and neck area also.
The employee was acending a belt crossover ladder when his foot slipped and wedged between the belt/belt structure. The belt was not in operation and as he fell foreward his foot did not release. This resulted in a sprained ankle with a simple leg fracture just above the ankle.
Employee slipped on a pile of salt while walking and strained back.
Employee was entering the mine on the chippy hoist when it suddenly came to an emergency stop. Employee was jarred inside the cage and experienced pain in his neck, back, and shoulder.
Employee laid tools on screen up on vibrator and when screen moved, the tools started falling and employee reached to grab them, causing him to fall.
Employee was climbing up the steps on the push arm of a dozer. Employee slipped striking the left side of his face on the steps.
Miner was standing in the door of the equipment discussing work instructions when the door inadvertently closed on his thumb.
Employee's shift began at 7:00 p.m. on 10/23/2008. Employee was operating the #641 haul truck and stated that his left knee struck the window crank on the haul truck after being loaded by the #11 shovel.
While employee was going to start the feeder he tripped over a rock and fell hitting his left knee against the feeder. (He did not start missing work until 5/7/2009).
Walking past gate drive & piece of head coal fell out & struck left foot.
EE was looking in the bins on shuttlefloor. He turned around to go back to the control room and struck his head/hard hat on a piece of angle iron on the gathering conveyor. His forehead above eye was cut and he suffered a mild concussion. Bruised left arm and side.
12"" Drisco pipe was being pulled by utility vehicle. Employee was standing next to parked Dodge truck. As pipe cleared Dodge truck it quickly slid over and struck EE on lower left leg.
Employee stated while removing structure from the 4 Right Longwall belt. He grabbed a top roller and went to throw it onto the top of the belt and felt pain in his right shoulder and bicep area.
Miner was walking, stepped, twisted ankle.
Pinched his finger between the pot and head of roof bolter.
EE stated that he noticed a pulling sensation in the groin area at about 1:00 AM. After his shift, he noticed a ""knot"" in the groin area while showering. He later went to his doctor who told him he had a hernia. EE is unaware of any specific event or time that he would have caused the hernia.
He was in the process of moving the emergency post sled when one of the posts struck him in the left knee causing a contusion to the left knee area.
When walking from shop to transfer building, employee stepped on uneven ground and felt a pop in his right knee.
Miner picked up a piece of steel I-beam approx 2ft long weighing about 100 lbs. He carried it for approx. 5 feet and set it down. At this point he felt pain in his lower back diagnosed as acute low back strain.
Employee was lifting objects, putting up supplies and felt pain in the lower back and in left leg.
miner was walking slipped and fell on shoulder.
Mis communication with Forklift driver. Forklift driver set pallet down on wrap machine platform-did not realize injured employee s hand was still there.
EE operating the 243 Locomotive. He reported as he was approaching the concentrate hopper cars he went to apply the brakes & they did not work. He hit the hopper cars at approx. 5mph causing him to hit the control panel with his body, two facial cuts below his left eye from his safety glasses & he said his right chest area was sore.
Reparing primary crusher, strained back while lifting part/componet.
Employee was taking rub rail off miner. The rub rail fell on right hand.
Injured was carrying 2 metal ties around a scoop parked in the track entry when he stepped on a soft part of the walkway causing him to lose his footing and twisting his left knee. He was transported to the hospital where he was treated for a sprain to the left knee.
Employee was loading shuttle car with miner when a piece of slate fell off of the miner conveyor beltline hitting him on the side of his right foot bruising it.
Member was riding in a mantrip and started up an incline when they lost traction resulting in the mantrip sliding back into a second mantrip. Employee complained of neck pain.
Drill spiral burr cut the hand while cleaning steel away from fabrication
Employee stated he was using the mechanical scaler when a rock broke under the outrigger causing the machine to shift and straining his shoulder. Employee did not report incident right away and did not miss work until 11/26/2008.
Employee states he has a hernia, right side groin area, which he feels is from various work activity over an extended period of time requiring heavy lifting, pushing and pulling. On 10/22/08 he started experiencing pain in his low back and groin area and requested medical care. Employee had surgery on 11/11/08 to repair his hernia. This claim is filed as an occupational illness.
Employee stepped out of truck and his right knee gave out.
Employee was in the shop preparing new conveyor belt for conveyor & while pulling around on it he had hurt his back. On January 26, 2009 He was put on worker's compensation due to his medication which would not allow him to run any equipment. To this date there was no lost time.
Alleged incident was to have occurred while this employee was spinning the shearer drum to change bits. He claims to have strained his shoulder. Cause of this injury was physical ability. Compliance with rules/regs was not a factor, miner attitude a factor, mining equip/systems a factor, protective equip. not a factor.
Employee was hanging miner cable. When he lifted the cable he felt pain in his right shoulder. Employee continued to work until 11-7-08. He went off work on 11-10-08.
Was setting timbers in #5 Entry and hurt right shoulder when lifting. NOTE: Employee began missing work on 10/31/08.
Driver pulled into safe area to allow another truck to pass. When driver backed out of safety zone, he backed up too far resulting with wheels over embankment causing rollover.
Employee was operating the dump truck. He slipped on the platform of the truck as he was exiting the cab.
The employee states that as he was walking, he tripped and twisted his right ankle.
Employee was climbing down the ladder of the 777C rock truck and as he approached the last step, it swung under the bottom of the truck resulting in the employee falling to the ground landing on his right side.
Employee was tramming the Miner away from the rib to service it when he got caught between the deck and rib.
EE exited the roof bolter deck to get cable crossers when a loose rock rolled off of the rib and hit his right upper leg, it bruised his right leg.
This accident occurred in the morning, calm weather, sunny day. The employee was working in the warehouse counting bulk bags of product for inventory. While moving around during the count, the employee caught his foot in one of the lifting loops on the bag and twisted his ankle. The employee was sent to the emergency room for x-rays.
Underground Maintenance employee recieved foreign body in eye. The employee did not seek medical assistance and the object was not removed immediately. The employee saw the site medic the next day and the employee was sent to the hospital for removal as the skin of the eye had grown over the object.
The EE was getting out of his truck on the job site, when his back gave out and he said his back hurt. The EE stated it felt like he pulled his back. Panel doctors said it was a pulled muscle.
EE reported: Attempting to unload steel cont. miner cutter motor cover from a supply basket to mine slope car. The supply basket was raised using surface fork lift. When the victim lifted the 100# object from the basket &d turned to place in the supply car, pain was experienced to his low back region. Victim is 6'5"" tall, weighs appx 350 lbs, should be able to handle this load.
A rock fell between the shields and struck the miner's foot.
A brier got caught in chain saw. The chain saw cut strap on chaps and rolled chaps up. The chain saw jerked back and cut his leg.
Employee was bolting and slickenslide roof fell from the face to the ATRS without warning and struck him on the side of his leg and ankle. Cause of this injury was safety knowledge. Compliance with rules/regs was not a factor, protective equip not a factor, miner knowledge could be a factor, mining equip/systems not a factor.
Employee was lifting and carrying 16' OSHA planks.
Operator dropped drill wrench in cradle of bolter. Operator kept raising and lowering pot until wrench was free - this action caused damage to machine - when he removed his wrench the boom on machine dropped hitting right leg.
The employee was moving No. 4 MRS after mining the second lift from the bottom end of the pillar block. He was standing at the crosscut to the right of the intersection when a piece of rock fell from the top striking the employee on top of the hard hat.
Employee was installing a dipper stop; as he lifted it to install it he felt his wrist pop. He was taken to his doctor, diagnosed with a sprain, given a soft splint and referred to an orthopedic specialist. He was also given a prescription for pain and a 5 lb restriction for his right hand.
EE was assisting other ee in offloading 50# sacks of Anfo from a delivery truck. One employee in the line dropped a bag and this employee picked it up. He felt a pain in his lower back but finished the job. About 15 minutes later the pain increased and he was taken to town to seek medical attention. off work until recheck on 10/27
EE crossed the belt to remove a flag pin. He stepped off belt onto belt rail. His foot slipped off the rail & his leg went between the belt rubber & rail twisting his right knee. He finished his shift with a swollen knee & worked lt duty till 11/13/08, when he notified safety dept he had a torn meniscus & would have surgery 11/24/08. Off a minimum of 2 wks after surgery.
Employee was changing out a bronze liner using a pry bar. The pry bar slipped causing the employee to lose his balance and put his hand against the liner. Employee received a laceration to his right hand.
EE climed on dozer arm to check water, frost on metal, slipped and fell, hitting lower back on metal brace towards the bottom between dozer blade and radiator.
Employee was replacing a 50 lb motor on dredge screen plant. In process of removing motor, the come-a-long came loose and the motor slid down and hit employee's right knee pinning his leg against the pole.
EE exited his dozer, walking up tilt jack to top of blade to talk with another dozer operator. When returning to his dozer the same way, he fell off of blade and tilt jack head first, between blade and hoist jack, hitting his ribs.
EE was in bucket truck moving phase up the pole. He went to move the carter pin from the chute and a 480 volt line arced, resulting in 1st and 2nd degree burns to his face.
EE was putting a hose on an auger when he turned the wrench to tighten the hose, it pulled his back causing a back strain, vertibra out of line
Person jumped up to reach a strap used to hang ventilation tubing. When he came down he landed on his left foot and his foot rolled causing the injury. Person sought medical treatment on 10/21, missed work on 10/22 & 10/23, was not scheduled to work on 10/24 & 10/27. Returned to work on 10/28. Total days missed - 2.
Injured employee was loading belt structure into a scoop bucket when he injured his lower back.
Employee was trying to pry a deck bushing back into place and slipped and fell mashing his right thumb between a slate bar and dust curtain bar. Employee finished his shift and was diagnosed at Boone Memorial Hospital with a dislocated and fractured thumb. After seeing specialist there was no fracture or dislocation only tissue damage.
Employee was using water hose to wash crushing plant and allegedly strained back.
Employee lifted buster onto wedge and his back went out.
Employee was checking motor grounds in plant area. When reaching overhead he felt pain in his left shoulder. Employee did not seek medical evaluation until 11/13/2008. First medical treatment resulted after MRI testing. Employee received injections on 11/26/2008 and is working with restrictions.
Employee was lifting a vibrator with a set of come alongs. The come alongs slipped and the injured felt pain in his neck and shoulder.
Getting into cab when the door swung open too hard. employee grabbed the door but lost footing in the process and catching himself by grabbing a handrail. Allegedly straining the shoulder.
Person was lifting large piece of rock with a slate bar on top of miner when the bar slipped causing him to twist his lower back causing an injury. Was diagnosed with a strained muscle by a orthopedic specialist on 10-22-08.
Employee was hand digging material with a shovel and felt pain in his shoulder.
EE slipped and fell on icy walkway, fell and twisted his knee.
While operating a loader, the loader was struck by a truck.
EE states that he was hurt while driving a water truck. He stated that he hit a bump in the road and the seat bottomed out, causing him lower back pain. On 10/20/08, he was diagnosed with a strained lower back.
While walking from the ready line to the shop, the employee twisted his left ankle and fell. He was taken to NNHR. He had a follow up visit on the 21st and was classified as LTI at this visit.
EE was stripping wood forms. He pulled the board and it slipped causing a puncture wound to his left knee from a nail in the board.
Removing bolt using impact gun. Finger crushed between truck and gun.
Carrying material, stepped on a piece of wood and twisted ankle.
Employee was driving rock truck and raised bed to dump and the truck jerked and twisted his back. Started missing work on 1/15/2009.
Employee said excavator shut off and the seatbelt jerked him back in his seat.
Injured was tramming shuttle car to the face when he hit a hole in the roadway causing his head to strike the canopy injuring his neck.
EE and a co-worker were attempting to unplug a plugged up 5 hp pump. As they lifted the pump up out of the mud, EE felt a sharp pain in the middle of his lower back.
Employee was operating a dozer. Employee slipped and fell off the dozer resulting in a leg fracture.
EE was operating a roofbolter, after he put up his inside bolt, he stood up and twisted his left knee. EE did not miss work until his physician took him off, effective 10/28/08.
The EE was in the process of advancing the face ventilation during the roof bolt cycle. He was behind the line curtain using his back & hips to hold the curtain away from the coal/rock rib. When he was bent over raking loose material to place on the curtain, a rib roll unexpectedly fell striking the injured on the arm & back of head. Cut to his forehead required sutures.
Employee was fueling equipment at dinner. He was getting out of the fuel truck and stepped on a piece of coal, twisting his ankle.
The employee was helping make a belt splice at #1 belt drive between the take-up and drive. He & other ee were hammering in splice nails. During the swing the hammer caught an over hanging metal fire supression line. When the hammer caught the line he strained his left shoulder due to the abrupt stop of his swing. The area was confined & had limited space to make a splice.
Fueling rock truck. It had been raining, mud had caked under truck. Chunk of mud fell hitting him in head. No cuts, it was just pressure from the mud pushed head down causing pain in neck.
EMPLOYEE WAS WALKING BEHIND MINER WHEN HE STEPPED IN A HOLE. HE TWISTED HIS BACK AND LEFT LEG.
Employee was directing the fork lift operator on changing batteries on scoop. He directed the fork lift operator to set batteries down; at the same time he reached for cap on battery jack on scoop. He got left thumb pinched between cap of jack and battery.
While changing screens on screen #5, employee began pounding the wedge down on the screen when pieces of metal flew into his left eye. Employee was wearing safety glasses, however they were not the appropriate type for the work he was doing.
Employee states that he suffered a knee injury from walking up and down stairs at the plant.
Employee tripped laying out a mat of rebar.
A haul truck driver states she was injured when the truck she was driving traveled over rough roads. The motion of the truck caused back pain. As a result, a physician prescribed pain medication, physical therapy, and time away from work.
While helping a mechanic and using a chisel and hammer, the injured hit the chisel and a piece flew off and got in his eye requiring a doctor to remove it.
EE was moving 50lb bags of product when he felt a strain in his lower back.
EE slipped on paper cushion on packer seat and fell between seat and deck, dislocating left knee.
Was pulling cable. Cable snapped and struck right foot, fracturing the right ankle.
Employee was loading shuttle car when the rib rolled off hitting right knee.
Drilling hole in roof and left hand, middle finger, got caught between roof and steel. Negative x-ray, but now referred to hand surgeon. Released to light duty.
Employee was moving pipes and tripped, causing the pipe to fall on his shoulder. He returned to work after treatment. He underwent physical therapy unsucessfully and had later had surgury. This is a lost time as of 1-12-09
Employee stated that he was using a 12 lb. hammer installing new teeth on the 992G loader bucket and felt pain in his back.
EE was dismounting a Cat 773 haul truck when his right hand slipped off the handrail. EE swung backwards and jumped to the ground, spraining his right ankle.
The injured was making a belt splice and was not wearing his safety glasses when a piece of a metal hammer broke off and struck him in the left eye.
Caught his foot between lift of roof straps and scoop bucket while loading supplies.
EE was assigned the duty of washing the overland belt stucture and the structure under the belt. During the course of the day the EE was on his feet and knees repeatedly. Towards the end of his shift he noticed that his lower back was bothering him. The next day he went to the Dr. and was treated for a back strain.
Employee was running the Ramcar and hit a hole causing him to strike his head against the canopy.
While backing up in a Mac 20 EE hit his neck on a water line, causing pain in his neck and left arm.
Employee was driving fuel truck, traveling to Johnson Hollow Pit. Employee stated that the inside tire of the rear right axle blew out. He stopped the truck to look at rear wheel tires & discovered a rock wedged between the tires. He then reached over into the wheel well to remove the rock when the outside tire blew out striking his right hand.
Employee was operating a three wheel mantrip going down a slope, when he steered to make a turn the three wheeler turned over causing injury to his shoulder and neck. Due to complications he started missing work on February 05,2009.
Miner was helping to move the section feeder when the cable fell striking him on neck and back.
Employee alleges his ankle got caught between two steel beams and he fell.
Employee was descending down the road and lost control of the mantrip. He sustained head injuries.
Driver jumped down off step injuring knee when he landed. The accident occurred 10/08 but the employee did not miss work until 2/3/09 thru 2/6/09.
Employee was using a pull cart to move a pedestal grinder. Employee lost control of the cart and tried to catch the grinder before it fell resulting in the employee smashing his finger between the cart and the cement floor.
He was using a com-a-long to pull a belt head roller and frame back into place, (that had broken free of its welds). A monkey face was then welded to an ""I"" beam to attach a com-a-long. As he was using the com-a-long, the monkey face broke free and struck his right knee.
While using a hydraulic cylinder to straighten bent flanges on a dragline mast, the cylinder dislocated, sprung from position and struck 2 employees approx. 8 feet away from the cylinder in their protective safety headgear. Employee 1 recieved a minor contusion to the forehead and Employee 2 recieved contusions, cuts and fracture from impact of the cylinder to his safety glasses
Walking up & down a hill carrying loading poles, pulled groin.
Supervisor and witnesses were attempting to remove the gearbox from the 3 Right Longwall face. While doing so the tensioner motor was knocked loose by the scoop bucket and struck the injured employee on the lower left leg fracturing it in 4 places.
Employee was shoveling wet refuse from the plant floor when he felt a pain in his lower back.
EE was working near the welder, when he got flash in his eyes.
Employee was lifting 5gallon bucket over a 18"" wall and twisted right knee.
Employee was shoveling on beltline when his foot became hung in the mud causing him to sprain his left knee.
Employee stated he was helping the electrician make a splice in the shuttle car cable and his knife slipped off of the cable cutting his little finger on his left hand. Had to have stitches put in finger.
Employee was servicing equipment, he was pulling fuel hose from service truck to loader, when he felt a pain in his lower right back and hip.
Employee was loading a drill steel into drill steel holder and lacerated finger.
He was stepping down off a tractor to refuel and he slipped off the step. He landed on his back on the concrete pad and hit a piece of steel on the pad.
Was hammering studs in scoop tire and shoulder poped with sharp pain. Persons filed comp on 10/27. Started missing work on 10/28/08
The employee was unloading a rail car at the unloading screw. He was opening a rail car discharge slide using a 60"" bar. The slide was difficult to open and the employee stood on the bar to attempt to move the slide. The bar slipped while he was standing on it causing him to fall to the ground. The employee twisted his right knee as he fell.
Employee stayed back on mid-night shift to help set air lock doors out-by D-4 hd. A piece of draw rock fell onto his shoulder & back area. Taken to E.R. x-ray negative, RTW 10-15-08. Worked until 10-22-08, saw another Dr., taken off work for 2 wks.
Employee was removing hose from 2500 Hitachi. The hose slipped out of employee's helper's hands & hit employee in the head.
Man was moving box of resin off the roof bolter. He twisted and turned and his right knee gave out.
Employee was getting off his D475 dozer slipped while still holding onto the handle and strained his left shoulder.
EE had been running a rubber tired loader #4382 and hurt his back while operating the loader. Don't know when injury occurred EE's last day at work was 10/14 then he was rained out/lack of work. Injury wasn't reported until 10/17 to Foreman.
An old beech tree log was laying on the ground. Employee was cutting brush around log when it started to roll and rolled over on him, trapping him under the log.
While walking an aircourse, the employee tripped and fell injuring his right ankle.
Servicing equipment, hit forehead on part of truck at about 1100.He called EE,EE asked him how he was.EE said fine didn't need to go to the doctor. Our Supt.called EE about 2:00 PM from Garza County Health Clinic .He said EE was servicing loader and raised up and hit his back. Our supt. took him for medical treatment.
Employee was climbing down from a hauler. When he stepped onto the ground his foot landed on a can of oil, causing him to twist his knee. Did not seek medical until 10-21-08. Continued to work with no missed time until 4-9-09. He had surgery on his right knee.
The employee stated he was walking to the belt starter to start the belt when he lost his balance and stuck his hand out to catch himself and struck his hand against the rib. The employee has been diagnosed with degenerative changes and arthritis in his hand, wrist, and shoulder.
Employee was scaling tower #3 with air lance. Large chunk of material sloughed off and jarred air lance. Employee dropped air lance and tried to move away. Air lance handle hit employee above right knee. Employee did not notifiy anyone of incident until going to doctor on 10/16/2008.
Employee was dismounting a D-11 dozer. He stepped on a round rock with his heel and he twisted his left knee. The employee finished his shift. On 10-17-08 he went to his family physician who referred him to a specialist. The specialist advised that he had a torn miniscus and needed surgery. He missed his first shift of work on 12-29-08 and had surgery on 12-30-08.
Employee was washing off the pond balls in a the dump truck with a high pressure sprayer. He lost his footing and fell off the top of a 4 ft. ladder. He hit his head on the floor. He was taken to NNRH and found he had a concussion He was given a full duty release but was made a LTI at the follow up on October 21st.
Employee was helping remove 12 west longwall head roller transfer chute. He was stepping off load bed onto a ladder. The ladder bottom rolled when he put his weight on ladder and fell down twisting his knee.
EE stated he was drilling roof bolt hole in mine roof, piece of draw rock fell from roof, struck EE on his left hand and caused contusion to left hand.
EE was using a pry bar when his feet slipped in the snow and he fell forward. He landed on the bar w/his face.
Climbed off D-8 Cat. Rock under my foot rolled and I lost my balance and footing. I put my arm our to stop my fall. When I hit the ground, I twisted my right shoulder.
Employee was walking past the #4 belt header when employee struck his head against a roof bolt head and plate on a installed cable bolt.
Employee was moving cable near a shovel and was struck by a rope causing him to fall back on the ground.
The man was lifting an oil can and felt a pain in his back.
Employee reported: Standing alongside the FCT waiting for the machine to be backed so he could cross the section. Without warning, draw rock fell from between the rib bolt pie pan and the rib. It struck the mine floor and flipped over striking his right leg. The diagnosis is a small chipped bone at the base of his tibia where it connects to the ankle.
Employee was hooking up a trailer and was cranking the lift jack when he strained his back and left shoulder.
Ee was working by the south platform of the north starter sheet rack when he tripped and fell. He stated that his back started to hurt. He was checked out by the ERRT team and transported to the ER for precautionary reasons. He was taken off work and referred to a specialist.
While traveling around sap tank curve, EE lost control of the truck and ran the truck into a ditch.
Employee was climbing down a ladder when he felt a pain in his back. At the time, he did not feel he required medical attention and continued to work. On 10/24/2008 he was lifting 18lb blocks, when he felt the original injury became aggravated and sought medical attention on 10/27/2008. His first lost time day began 10/27/2008 and his return to duty information is unknown.
Employee was operating a haul truck and was in a shovel pit waiting to be loaded by a shovel. The first dipper load did not open. Shovel operator kept trying to open the dipper. When the dipper did open, the load was dropped on the dove tail of the truck causing the truck bed to rise. Employee experienced pain in the neck and back when the load dumped.
Employee was greasing a tail-pulley of a conveyor. While in a squatting position, he twisted and felt a pain in his left knee.
EE returned from rotator cuff surgery and convalescence today and re-injured his left shoulder again while stacking 50 lb bags onto a pallet. We are awaiting report from doctor.
While attempting to walk around some water walking back from crusher employee slipped and fell onto right knee and then while attempting to stand up fell a second time on left shoulder.
Employee was removing safety chains from between supply cars on slope bottom when he slipped striking his right jaw on something (undetermined) resulting in fracture to right jaw. Incident was not reported until 10-15-2008, began missing work on 10-16-2008.
The injured employee was cleaning his work area around the forced air packer and strained his back while moving a 100 lb bag of sand. The incident was first reported on 10/16 and at that time the incident was not a recordable injury. The employee left work on 12/09/08 and we were informed on 12/12/08 that the employee was to be off work.
Security officer saw someone on mine property. As he was running to security vehicle, he felt pain in upper left leg. No fall involved.
While checking oil and antifreeze levels in rock truck, EE was pulling up ladder, when he felt a slight pain in lower back. He continued to work through one more truck, then decided to have it checked out. No emergency care was required and EE refused ambulance.
EE alleges back strain occurring 10/11/08. Incident reported by spouse via telephone on 10/20/08.
Employee injured his lower back when the seat he was sitting on came in contact with an exposed power lead knocking him off the mantrip.
Injured was shoveling on #6 belt when he slipped on gob. He fell on a rock and hurt his right knee.
EE was bldg kennedy stopping with 2nd ee. He was holding angle iron while other ee was driving into the rib. Rib fell knocking ee to the ground. He reported to have broken ribs and puncture lung. The area was in 13W, between sharline and x cut 18 1/2 in the #1 entry. Rib measured 13'L, 4'-6' wide and 9 1/2 high.
While roof bolting the drill steel plugged up; once cleared we began to raise the drill steel to the roof. When the steel struck the roof a clod fell and the steel bent and snapped back striking his left hand.
Employee stated that he was installing a Kennedy stopping, when a rock fell from the roof and struck the employee on right hand and leg just above the ankle. Employee's FLWD did not occur until 10/11/08 due to surgery.
EE was helping repair a broken feeder chain. He was using a hammer to replace the straps on the links & accidently struck his finger with the hammer. He finished his shift & sought medical attention at hospital. He continued to work until 11-4-08 when he went to Pikeville Medical Center due to pain in his finger. X-rays revealed a fracture to his left middle finger.
Employee was working highwall, backing up, backed over a rock hitting hard and causing employee to be thrown toward right hitting his head on a computer panel. NOTE: Employee did not seek medical attention or miss time until 10-31-08.
Employee was riding into the mine on a track mounted mantrip. When the mantrip reached the section, the brakes failed to hold and it rolled back down the track and jumped track causing a fracture to his back.
Employee was helping load up some cable into a scoop bucket when he turned his body causing a sharp pain his right knee. He continued to work until 10/22/08.
Dusting belt line - got close to belt, snagged clothes on belt - belt slammed to ground.

Victim checked pin on stab jack fletcher bolter. Laid hand on boom, raised jack and caught index finger between canopy and boom, causing injury to finger. X-rays showed it was broken and required stitches. Also removed fingernail on ring finger.

EE was bldg kennedy stopping with 2nd ee. He was holding angle iron while other ee was driving into the rib. Rib fell knocking ee to the ground. He reported to have broken ribs and puncture lung. The area was in 13W, between sharline and x cut 18 1/2 in the #1 entry. Rib measured 13'L, 4'-6' wide and 9 1/2 high.
Employee had finished repairing a conveyor chain on a shuttle car. When he dismounted the shuttle car he twisted his left knee. Medical revealed a reportable accident as of 10/24/2008.
Employee was in process of picking up a timber and turned to place on a stack of timbers and felt pain in lower back. Employee had MRI and was diagnosed with lumbar sprain/strain on 10/21/2008.
Two employees were cleaning up some solidified sulfur off the floor. The employee stepped onto a bridge of solid sulfur that had formed over a pool of molten sulfur. The employee's right foot was submerged and when she removed her foot from the molten sulfur, it solidified on her right ankle and foot. She sustained burns to her ankle and lower right leg.
The remote control for the continuous miner would not function due to a weak battery. The employee placed the miner in ""manual"" to move it. When he started to tram the miner, it caught him between the miner and the rib.
Employee was going to change a torque shaft when he sprained his left ankle.
While hooking up the torch car, employee twisted to get the coupling pin and felt a popping noise in his neck. He started missing work on October 14th and returned to work on October 22, 2008.
While changing drill bit his foot was in the way of reverse rotation, pinning his foot against the frame.
Employee was installing belt drive system when an electric motor slid off its mounting bracket injuring employee's left knee and right hand.
Miner was longhole drilling and trying to replace his steel and lost control of the drill, pinching his right hand between the leg and drill. Miner was taken to clinic and X-ray showed a minor fracture of the right wrist. Miner placed on light duty with no time lost.
Employee was transferring sodium silicate from 55 gallon drum to mixer. When opening drum, sodium silicate splashed into his eyes. Employee was not wearing safety glasses. Employee's eyes were flushed out with water at eyewash station & then he wash carried to emergency room.
Employee was helping to load high voltage cable into a sled. His foot slipped on a rock, causing his foot to slide under the sled. The sled was being pulled by a 35C Fairchild scoop.
Injured was bolting top when piece of rock fell striking injured on head behind ear.
Moving material from under the radial stacker with shovel, when he pulled the shovel back he felt a sharp pain in lower left back.
Employee was carrying a piece of metal grating (2 ft x 24 ft) from the track entry to the A West diagonal belt transfer. He tripped and fell to his knees. He felt pain in his left rib cage. (First day missed 10/15/08).
Two employees (welders) were dismantling a large tool rack. The welders were cutting 8'X4'X1/4"" plate steel from the frame. The rack became unstable and tipped over on top of the employees. One employee sustained fatal injuries and the other sustained injuries to his back and legs.
Employee stated he was walking on the unit when he struck his head on a low brow. His neck was hurting so he went to medical services. He started missing on 10/23/2008 and was given an MRI. This stated he had Spinal stenosis. This mostly congenital with the two cause being birth defects and progressive degeneration of the tissue of joints or osteoarthritis. (Occup. Illness)
Employee stated he was walking beside a shuttle car carring 2 empty oil cans, when he tripped/slipped on a piece of rock laying on the floor. He fell on his left shoulder and had pain aftwards. He continued to work until he was taken off on 02/05/2009 for rotator cuff surgery.
While working on changing a screen, injury, Rock Crusher Foreman, got his hand caught and detached his pinky finger on his right hand. He is currently in surgery to attempt to reattach.
Employee was lifting a heavy drill by himself and injured his back. Employee worked a few more days after it happened before he went to a doctor.
Employee was spot bolting along the belt line. He was lowering the head to insert an extension on the drill steel, and lowered the drill head onto his foot.

EE was helping make belt move, he was pulling on structure and slipped hurting his back
There was no scene exactly, the involved employee, came to her supervisor the day after she began to notice pain. The pain continued to intensify through out the pevious day and got worse over night. The next morning she reported the pain to her supervisor.
Employee was lifting a ladder out of the pickup when he felt a strain in his back. He initially denied treatment but over the weekend the soreness got worse. He was taken to the clinic. Employee was taken off work.
Employee was constructing a permanent stopping between #1 entry (return) and # 2 entry (track) on the 006 west cm section. The wall was being built out of 6"" x 8"" x 16"" solid concrete blocks. The wall was almost complete when it fell over and struck the employee's right lower leg. His right knee was sprained.
Injured employee was moving miner cable with his left foot and injured his left knee.
Employee was greasing his loader when he slipped and fell hitting his side on the loader and cracked 3 ribs.
While using sledge hammer, employee felt something ""Pull"" in right side. Dr released to full duty at time of accident. Beginning December 2008 began seeking med treatment from personal dr. Taken out of work 1/21/09 by personal physician. Deemed work related by comp dr on 3/25/09.
Shift began at 7:00 p.m. on 10/7/2008. Employee was being loaded at a shovel when he experienced back pain.
Employee was crawling out of a vibrating screen to the concrete floor. He stepped into the vibrating screen underflow pan and lost his balance. To keep from falling, he grabbed something with his right hand. He felt pain in his right arm/shoulder.
Employee was in the process of making a splice in a piece of mine belt. As he was cutting the belt with a utility knife the knife slipped causing a small laceration to the left knee. He received 4 sutures.
The continuous miner was loading into a shuttle car. A piece of rock bounced off the shuttle car and into the crosscut and struck the man on the leg. The man continued to work until 10/15/08 which was his first day of work.
EE was going to adjust brakes on a scraper. Bent down on knee to go under rear and hurt knee on a rock, causing deep bruise.
Employee was on the motor and it derailed causing employee to be struck in the head by the water manifold which was hanging from the roof.
Employee was working on top of an overcast in the no. 5 entry of 001 section. The employee started to step down or jump down when his light pouch got caught on overcast causing him to twist before reaching the ground, injuring his right leg.
EE was raising the drill head to connect drill steels togather, when the srill steel caught the other steel and bent catching his left thumb between the plank jack and drill steel. The EE sustained a fracture to the left thumb.
EE was releasing stored energy off a hdy. cylinder & it appeared to have been released. He started removing the fitting, loosening it with a wrench & then with his hand. There was stored energy on the cylinder & it blew out striking the employee on his left hand between thumb & index finger. The grease blew a hole thru his glove & made a laceration, injecting grease under the skin.
Spot bolting at 31 wall, employee slipped on rocks and twisted lower back.
Employee was getting out of his mechanic's truck when he slipped and twisted his left knee.
Injured stated he was struck in right knee with a piece of rock causing a sprain to his knee. Injured was given a full release & worked up till a follow up visit on 10-20-08. At that point the orthopedic took him off work due to strained LCL tendon.
Employee was performing routine maintenence on crusher, pushing on piece of steel so that it could be bolted into place. Employee felt his back pop and experienced pain. Employee was transported to the emergency room by crusher foreman.
Attempting to remove bolt with wrench from the coverplate on Wms Mill sump pump, the wrench slipped which caused employee's weight to shift to right shoulder. The pain was not evident at first but got worst during the night. Dr suspected rotator cuff tear, the MRI results confirmed on 10/30/08 (Dr diagnosis). Employee opted to physical theraphy vs seeing orthorpedic doctor.
The forks of the fork lift were not in the channels of the manlift cage. Injured ee and other ee were standing in the cage. Another ee was backing up the fork lift and the cage tipped. Injured ee and other ee fell out of the cage and bottles of Oxygen and Acetylene fell on them.
Driver lost control of tractor trailer when engine failed and he lost air pressure after restarting engine. Trailer went into ditch. Tractor rolled onto haul road. Driver visited ER on 10/08 and was treated for strained lumbar. Prescribed 2 days off work.
Employee was helping another worker take down monorail beams at the headgate area of the 5 west off cool hill 3 longwall section. As he attempted to lift a piece of monorail, he felt something ""pop"" in his left shoulder. He tripped and fell to the ground with the monorail on top of him. As he got up, he felt a burning, pinching in his back.
While employee was climbing down off trackhoe, he felt pain in his right shoulder.
Slipped on steps of a screen plant while pre-checking equipment, grabbed a handrail while falling and injured left shoulder.

Employee was working on the crusher stacker. Standing on a metal ladder which was wet from dew, he slipped and fell to the ground and injured his hip and got cuts.
The man was stepping off the mantrip when the operator inadvertently trammed the mantrip forward. The man twisted his legs causing a muscle strain.
Employee was riding out of the mine in the back of a rubber tired mantrip with his left arm up behind his head. When the operator turned into a x-cut to let an approaching scoop pass, he got too close to the rib and the injured mans elbow was caught by a low brow dragging his arm between the rib and the mantrip.
The employee stated that while replacing a torque shaft in a miner, he felt pain in his lower back

EE was walking around his truck, due to a previous accident, his left knee gave out. He fell to the ground, hitting a crib block with his left knee.
The employee stated that he was running the continuous miner in entry # 6. He saw that a rock was starting to fall, so he turned to back up to get away from the rock and he felt a pop in his right knee.
Tightening a bolt (on the screen) with a rachet wrench. Wrench slipped off and came back and hit employee on the eye.
