Accident type, without injuries

At 12/31/2018 at 2:35 AM the supervisor discovered a fall in 12 West section XC 6 entry 4. The fall area was just above the anchorage (7 feet), 19 feet wide, 26 feet long. The support was 7 foot fully grouted primary roof support.

An unplanned roof fall was discovered at the Old 6 North Tipple Track at the neck of 6-M. The fall was approximately 16' wide, 25' long and 10' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

A roof fall was discovered on the A-Mains roadway at crosscut 35 in the #3 entry. The fall measured 24' by 20' and was 10-12' in thickness. The area was previously supported with resin bolts, truss bolts and beams. No persons or ventilation was affected.

The underground natural gas line that supplies the plant failed and started leaking natural gas underground into the nearby access opening to the old abandoned underground mine entrance. When notice the area was evacuated and main gas valve was closed.

At 4:35AM the plant technician was going to the top of the silo in the elevator when the elevator stopped near the top of the silo. The elevator passenger was never in any danger of being injured. This person was safely out of the elevator at 7:05AM.

Mining in adverse geological conditions-low cover. A Roof fall occurred overnight and was discovered at 5:46 am on pre-shift exam. Fall was investigated by section foreman, DMS, and MSHA

Employee was driving a rock truck returning back to the North Pratt washer via Flat Top mine entrance road. While traveling back, on the right-hand side of the road, employee veered off to the left-hand side of the road, running the truck off into a hollow along the mine entrance.

MSHA was called because of an equipment fire on level 3 of the underground mine.

A Roof Fall occurred in the Main West #1 (Return) Entry, just inby crosscut #18. The fall is 16 ft. wide by around 60 feet long, and around 6 ft. high in the highest spot. No one was injured or entrapped and no Ventilation was interrupted or blocked. Access was physically blocked. A 103(k) Order was issued. When approval was given, Timbers were installed on both sides.

At 11:58 a.m., a power outage occurred in the surface and underground of the mine. When the outage occurred, three miners were on the #1 shaft conveyance performing a hoist inspection. The miners were 40 ft. from the 1350 station when the power went out.

Between haul road curves 6 and 7 the truck went into a ditch.

After pulling a truck out of the mud, they got in the pick up truck to drive it down the road and were told that the truck was on fire. They were unable to put the fire out. Cause of the fire is believed to be a fuel leak.

The BMX slope hoist was out of service due to the gearbox for the encoder breaking. The gearbox was replaced and the hoist returned to service.

An Examiner found a Roof fall in the #5 belt entry. The fall was called into the MSHA Hotline at 2:35 A.M.

Unplanned roof fall #3 entry 10 west butt section. The fall started 40 feet inby spad 6213 and extended 60 feet inby and 30 feet into x-cut 3-2. It was approximately 12-14 feet high. Area was cribbed off.

Roof fall information from D-2 in the plant mains found at 7:40am. It was reported to the call center at 7:45am. The roof fall is in #4 and #5 entry beginning in x-cut 5 thru 6. In #4 entry the roof fall is approx. 70'long x 20' wide x 4-6 high (thick).

At approximately 4:40pm the service hoist on the Camp Creek portal went down due to a contactor fault. At 5:48 pm the contactor was repaired and the hoist was put back in operations.

There was an unplanned roof fall in the 1st South Panel, #5 entry at spad 7+65.

IGNITION - Occurred at East A head roller at the North Bunker, while burning a hole at the top of the North Bunker a piece of slag dropped down into bunker and ignited methane that was in the coal, in the Bunker. The ignition was extinguished with one fire extinguisher.

An unplanned roof fall was discovered at 22 block in the #4 Entry intersection of 6 North. The fall was approximately 16' wide, 16' long and 8' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

An unplanned roof fall was discovered at approach to the 6M #5 Seal. The fall was approximately 16' wide, 20' long and 6' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

An unplanned roof fall occurred in the intersection of cross #25, entry #7, of the 2nd Southeast Parallel at an unknown time on December 1, 2018. The roof fall is approximately 12 feet high. Entries approaching the fall measure 17.6 feet, 19.0 feet, 17.8 feet and 17.2 feet wide.

Drowning

The victim drowned when the dredge ee was operating sank.

While operating the 2nd/back MBC (mobile bridge carrier) the employee became pinned between the bridge of the MBC and the rib.

Victim was operating front-end loader in coal pit area, when a highwall failure occurred, covering the front-end loader completely.

While performing lifting operations of a liner, the rigging came loose causing the liner to flip. The liner began to swing, striking employee on leg and pinning against stand. Approximate weight was 7000lb.

Employee was using a rag to plug a hole in a screw conveyor that was leaking product. The material got caught in the screw flight and pulled the employee's right index finger in to the hole. The screw flight severed the right index finger to the first joint.

Employee opened a door, reached into equipment with right hand & mill rollers grabbed glove pulling hand into mill damaging multiple fingers. EE was transported to Emergency Room. Three fingers were amputated and one finger was sutured.

Employee was tramming a forklift when EE drove off the road into the ditch. The forklift leaned on its side against the berm. As the employee was getting out of the forklift EE slipped and rolled over the embankment approximately 15 feet.

During a probable seizure, this employee sustained a fractured right upper arm and a laceration above the left eye. The event was unwitnessed and occurred during end-of-shift change in a loader that was tied down.

Employee stepped off the shield toe onto uneven ground and twisted EE's left ankle. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Over-exertion in pulling or pushing objects

Removing rear differential fill plug from a 777G. Tried pushing down on breaker bar plug and it didn't move. Felt popping in right shoulder and shooting pain down right arm.

Over-exertion in lifting objects

While helping 4 other men hang high line cable, employee felt a pull/pain in right shoulder. The employee worked up until 1-27-2019 as a ""weekend warrior"". Employee called us on 1-30-2019 and said was still experiencing pain and wanted to get it checked out. The doctor put employee off work on 1-30-2019.

Struck by flying object

While the Employee was helping pull the end plate on the mono rail up the hill using a sheave wheel. The chain on the sheave wheel broke and the cable hit the employee lower leg and back areas causing ee to hit the ground.

Employee was attempting to repair a support on a float barge when EE's foot slipped and EE fell over onto hip causing a bruised hipbone.

The employee was walking on the slack pile for the miner doing perm, the employee's foot got tangled in the cable and the employee twisted their ankle. The employee continued to work until 1/3/19. An x-ray was taken on 1-3-19 and a fracture was discovered.

Employee was installing bolts on the 4-E longwall recovery face. As EE was tightening a bolt using a breast drill, the machine jerked unexpectedly pulling on EE's left arm. Had pain and soreness in EE's shoulder. EE continued to work until 1-21-19 when EE sought medical treatment and began missing work.

Pulled on tailgate to clean bottom rail and slipped on ice. Tailgate came back to closed position and smashed left hand.

Fall onto or against objects

Miner tripped on gravel and a cable and fell into a piece of tin lacerating face and neck. Received stitches.

EE was pushing shot rock. EE observed rock in mirror roll down behind dozer. EE turned to look and see where the rock went as EE was turning the dozer. The dozer trammed over the rock. At this point the EE felt a bump and also felt a pain in EE's lower back.

Employee was walking through fly pads when check curtain flapping in the wind on the bottom caught employee's foot causing employee to slip/fall resulting in a re-tear to right upper arm (bicep) and right shoulder (rotator cuff).

Employee was at the crusher spraying inside of chute with 115 psi. The hose tip was just inside the chute door. Something broke loose above and it fell down hitting tip of the hose which caused employee's arm to go downward causing bone fractures in left arm.

Employee was crossing belt. When stepping down EE turned EE's right ankle.

Employee stated that EE bent over to pick up a battery jumper cable when EE felt a pain in EE's lower back. Employee was taken off work by physician.

Struck by falling object

Employee had completed installation of bolt into the mine roof when a rock fell striking employee on the right elbow. Injury resulted in an abrasion/contusion. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Fall from machine

Contract employee was exiting vehicle with mud on soles of boots when the contract employee slipped on the step, landing awkwardly on the ground, spraining ankle. Cause of this injury was employee awareness and muddy conditions. Compliance with rules/regs, PPE, mining systems was not a factor, Mine proficiency may be a factor

Over-exertion in pulling or pushing objects

EE was opening 1"" valve and reported pain in shoulder two days later.

Absorption of radiations, caustics, toxic and noxious substances

Working on excavator hydraulic line released pressure and struck EE in left cheek and fluid sprayed in left eye.

Struck against stationary object

Employeee was exiting DCS out the rear door when the employee hit the cement curb at floor level with left foot and twisted lower ankle/leg area and was unable to walk on it afterwards

Fall onto or against objects

Employee was exiting a sand screw basin, while starting to exit the basin retractable lanyard seized causing the employee to stumble backwards and strike middle back on the sand screw (auger) causing a contusion.

On 12/23/2018 employee slipped and fell on left side of body and was initially given full duty release. After additional evaluation of the employee's shoulder in April 2019, the Dr. determined surgery was required on employee's left shoulder. Surgery was performed on 4/22/2019

Struck by flying object

IP noticed air and water headers come down off the rib in a muck bay. IP decided to move the water line first by bending it back on itself and stuck the header into the ground. As the IP was in the process of retrieving the air line, the water line came dislodged and the valve turned on. The water line struck the IP's right forearm resulting in a fracture.

Over-exertion in lifting objects

Employee was lifting belt clamp from waist to overhead. EE used improper body mechanics while lifting, and then ee heard a pop/crack sound and felt pain in left hip.

Employee was pulling a rope across the spillway between pond #1 and pond #2 when ee slipped twisting left knee.

Driver stepped up onto tire to clean windshield and slipped & fell breaking wrist.

Employee was replacing guard on Portable Plant head pulley conveyor

The job required removal of previous system that ran from the drivers console through the ""man whole"" work area, to the rear of the machine. The space was very confined. While installing the new system, the employee took many short breaks to stretch due to a timely duration of removal and installation of new system. At the end of shift the employees back was sore and tense.

Over-exertion in lifting objects

Employee was helping to load I-beams on a dolly. EE lifted the end about a foot and a half and set on dolly. EE felt shooting pain during lift and EE's hip to ankles felt bad. EE continued to work until end of shift at 7:00 pm. Management was made aware of the incident on December 23rd.

Employee was removing debris from machinery when a piece of material fell and pinched their hand between material and machine resulting in a laceration on their finger.

EE was removing bolts from a final drive on haul truck. Later that afternoon, back started bothering EE (strain).

The employee was helping to empty a water treatment sludge bag after it had been dewatered. EE was pulling open the bag with a pair of vice-grip pliers and the pliers slipped causing EE to fall backward onto the frozen ground. EE landed on the right side causing a contusion on EE's right elbow.

EE was hanging miner cable with other employees when the cable was dropped and the injured held on and strained EE's left elbow.

Employee was stepping out of rail car mover and when EE stepped onto the ground EE twisted EE's ankle. Went to be seen at clinic. X-rays were taken and EE was diagnosed with a slight sprain. Missed two shifts then back to work normally. Ground was level and no work conditions were noted as cause.

Employee was operating MT5811 rock truck when EE experienced pain in EE's lower back.

Employee was working with three others scraping dirt off the front of a dump bed to prepare for liner replacement. EE turned slightly on the metal surface, when EE slipped, fell backwards and automatically reaching out with EE's left hand to catch self. EE felt pain in EE's wrist. EE suffered a fracture in the left wrist.

IE was assisting with changing drag ropes on BE 2570 Dragline. IE was positioned behind and below the drag drum. As the IE attempted to step over the deck winch to relocate to the opposite side they tripped, falling onto their extended arm. X-ray evaluation revealed a hairline fracture in the left wrist.

Fall from machine

After pivoting the employee misunderstood footing and stepped off of the crane. Upon stepping off of the crane the employee fell 80"" landing on left side resulting in a fractured hip socket and minor hand laceration.

Contact with hot objects or substances

Using torch without gloves and melted rubber landed on hand.

The contractor was building a brattice on unit #3. The average mine height on this section is 9 foot. As ee was stepping down off a ladder and inadvertently pulled one of the hollow cinder blocks down and it struck ee on top off left foot. X-rays revealed that ee had (4) fractured metatarsals in left foot.

Employee exited mantrip at the end of shift and felt pain in lower back and left leg.

Conveyor belt shifted causing overloads to trip. Maintenance discovered the take ups had failed. They were trying to remove them to do repairs when the load shifted catching the miner's arm.

Employee was operating a diesel scoop when EE ran over a crib block that fell out of the scoop bucket throwing EE up into the canopy of the scoop and then back down in the seat. This resulted in a compression fracture to the L1 vertebrae.

Employee was assisting the underground forklift operator with the placement of supplies. Machine ran over employees right foot causing bruising and soreness.

IE WAS HOISTING A DISCHARGE PIPE WHEN THE PAD EYE BROKE LOOSE STRIKING THE FACE.

Injured employee sprained shoulder while lifting a rock.

Fall onto or against objects

Employee was standing on the miner dust box ledge, cutting wire from around a bundle of bolts and lost EE's footing. As EE tried to catch self EE's right ring finger contacted a bundle of roof bolt plates causing injury.

The 769C haul truck was backing up to the 3450 excavator to be loaded. As the truck was backing up, it struck the raised bucket of the excavator, which jarred the operator, causing neck pain to the operator.

Struck by flying object

Employee opened the air lock door in the roadway on 3 D Belt for the grader. It has to be secured with a hanger. The rope hanger on the plate of the rib bolt came loose and the door struck and knocked them down. EE said they felt something pop in left knee. Due to events surrounding this injury it has been under investigation and deemed reportable.

A fully trained miner was carrying a piece of belt through a man door when their hand was caught between the frame and belt resulting in a contusion. The injury was not the result of any compliance issues nor were any safety guards involved. NOTE: The employee did not begin missing work until 12/19/2018.

Over-exertion in lifting objects

Employee was removing motor from miner and felt a pull in arm / shoulder area. Employee was transported to Logan Regional Medical Center where ee was examined and diagnosed as having a muscle strain. EE was released to return to work the following shift.

Employee was strawing at a pond spillway when ee stepped on a loose rock and fell down a steep slope injuring left ankle & right ankle.

Employee retrieved a 5 gallon bucket filled with oil from the back of pickup truck. After carrying the bucket from the vehicle, the employee using left arm, lifted the 5 gallon bucket, weighing ~40 lbs, up and over the discharge piping for the fresh water booster pump. Causing pain to EE's shoulder.

While performing maintenance of JCB 940-4 Forklift, employee leaned forward against machine and right knee pressed against metal frame. Knee hyperextended backwards causing pain and soreness to right knee area. Employee was taken to local Urgent Care for observation and diagnosed with pain in knee and possible tendinopathy of patellar tendon.

The employee stated EE's right hand was resting on the ground and was struck by a lifting jack that had been lifting a rover to place back on the track. The employee was diagnosed with a fractured thumb, index, and middle finger. This incident is being reported as a lost time incident.

Employee was backing up D11 Dozer when EE back over a rock and jarred EE's left shoulder.

Employee was descending stairs, slipped on mud and while trying to catch self from falling, strained right shoulder. Employee was taken to the ER, referred to an ortho, who then prescribed a sling, and restrictions the company cannot accommodate, pending an MRI. Currently ongoing.

A fully trained miner was installing a roof bolt when a piece of stone fell from between the straps striking their left hand. The accident was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

Employee was beating two hammers together in an effort to remove a splice pin from a mechanical conveyor belt splice. A piece of metal chipped off one of the hammers striking employee in cheek below the right eye.

Over-exertion in wielding or throwing objects

EE reported swinging a sledge hammer when they felt a pinch in their mid back. Did not want to be seen by site responder. Finished shift. Sought medical care on their own later that day. Department unable to accommodate restrictions.

Struck against a moving object

EE was driving a diesel ride to pick up a part. As ee was driving under an overcast, ee bumped into a Battery Ram Car that was hauling rock dust. The ram car has stopped in the road. The employee bumped knee on the dashboard area of the diesel ride causing a hairline fracture just below left knee. EE had to wear a brace causing ee to be unable to return to work.

Over-exertion in lifting objects

Picked up 2 cribs over EE's shoulder and as EE lifted them over EE's shoulder EE felt a pull in EE's right groin area and dropped the cribs when EE felt the pain. EE was diagnosed with a hernia and taken off work Friday, January 18th for surgery becoming a Lost Work Day incident

Struck against stationary object

Employee was manually spreading chloride to melt ice along the inside edge of the railroad rail passing through pellet loadout. The employee stepped on the rail and slipped resulting in pain to the left knee. Employee was able to continue working. The pain didn't go away and ultimately EE had knee surgery on February 14th to repair the knee. (Meniscus)

Employee stated was riding in a mantrip when the mantrip hit a kink in the rail then wrecked causing the injured employee to hit head on the seat. Employee complained of head and neck pain. Employee was taken to the hospital and taken off work under doctors care.

Struck against stationary object

Employee was operating 4 man diesel ride in reverse. Employee struck head on high voltage cable, panicked and instead of applying brake the employee floored the accelerator causing injury to neck.

Employee slipped on Cat 365 machine and fell. Their finger went into a small hole in machine and cut the right middle finger.

Over-exertion in pulling or pushing objects

Employee was pulling a motor from Air compressor using a Come-a-long. When ee felt a sting between shoulder blades. Employee continued working until 2/19/19.

Caught in, under or between running or meshing objects

The crushing plant had gotten plugged. After getting the plant unplugged EE was helping up a access door back on when EE's coat was snagged by the conveyor belt. EE's arm was pulled into the conveyor and tail pulley.

Struck by flying object

A fully trained miner was moving a pump when the hose that EE was lifting with came off the pump and hit EE in the hard hat causing a bruise to EE's forehead. The accident was not the result of any compliance issues nor were any protective guards involved.

Employee was repairing cable on a shuttle car when the employee stepped back into a hole, twisting left foot causing sprain to EE's foot. Employee was put in air walker boot.

Was sitting in truck while being loaded and struck head on the steering wheel.

The injured employee picked up heavy steel rollers and frame with another employee when ee felt a sharp pain in back. Please note that the employee continued to work unrestricted until 12/20/2018. Officially off of work 12/21/2018.

Employee was taking a break when EE went to stand up and put pressure on EE's leg had a bad pain in EE's knee that also hurt when EE tried to straighten EE's knee. EE had no other accidents or problems prior to getting out of EE's chair. Was told not to put weight on EE's leg until received a MRI to see what was the problem. Today was told EE has a torn meniscus.

While mixing seal material, dust ""burped"" back in the employee's face. Dry material got in EE's left eye.

EE was using a 15 ton motor to move a VFD box. The motor derailed and as ee was exiting the motor it slid approximately 12 inches and dropped on ee foot. EE sustained 3 fractures to right foot.

Struck by falling object

Employee was in the process of removing debris off of the head drive on the long wall. When EE turned to walk away a piece of rock fell from the mine roof striking EE in the back of the legs.

A fully trained miner was hooking a chain to a longwall shield when EE's left ring finger was caught between the shield and the chain resulting in a laceration. The accident was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

IEs were in the process of inflating a tire to examine it to determine where a slow leak was emanating from. As the tire was being inflated, a lock ring became dislodged. This resulted in a rapid deflation of the tire. The second lock ring came loose as well and struck both employees.

Employee was adding a second steel when a rock fell from the binder and caught the left index finger between the rock and the bolter pod.

IEs were in the process of inflating a tire to examine it to determine where a slow leak was emanating from. As the tire was being inflated, a lock ring became dislodged. This resulted in a rapid deflation of the tire. The second lock ring came loose as well and struck both employees.

Fall from machine

While employee was getting off a haul truck EE came down to the lower stairs. EE bent over to pick up an item and became dizzy upon standing. EE fell forward off the haul truck onto EE's head and right arm.

Employee was in a 126' boom lift attempting to relocate the boom lift unaware the basket was lodged on structure. When basket of boom lift broke free the basket jerked causing employee who was holding on to handrails of basket to smash right hand between basket rails and structure causing employee to smash/pinch right hand and 5th fingertip.

Injured employee was helping lift joints of 6"" pipe (waterline) when they felt pain and tightness in their back. Was driving down the West Submains in a small diesel golf cart when EE ran over a rock. The steering wheel started to spin catching EE's hand between ring finger and pinky finger causing a laceration between the two fingers. Resulting in stitches in hand.

Employee was operating a 4 yd loader, while mucking out a borehole the loader bucket contacted the rib causing the loader to come to an abrupt stop causing the employee to contact chin on the left shoulder pad resulting in a laceration. Seen by site medic, referred to ER, two sutures required, no restrictions issued making this a medical reportable.

EE was cleaning out cab of the drill. EE cut EE's hand resulting in a laceration between the left index and middle finger that required 2 sutures.

Employee was sent to get additional tools when EE fell going down steps and landed on buttocks and back.

Employee mashed finger when lowering a bearing assembly to the press table. A block slipped from under the assembly mashing EE's finger between the assembly and the stand. Diagnosis was a fractured distal phalange (fingertip) no restrictions or treatment. EE was wearing leather work gloves at the time of the injury.

Employee stood up on the bolter to install resin and the bolt. As EE stepped down, EE's foot inadvertently went between the bolter station and the bolter frame as the bolter frame was being retracted. It did not involve any compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was along supply cars and tripped on a lump of coal. As EE fell, EE's left finger was caught on the rib screen and resulted in a dislocation. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was busting rock, attempted to strike a rock 4in thick, 2ft. wide, when EE did, the rock flew toward EE hitting EE's right hand. EE was taken to ER for treatment where EE received x-rays and 6 stitches. EE was released to return to full duty the same day.

Employee stepped from the tool sled in the highwall miner pit and slipped on ice causing employee to fall and stove left hand and cut left thumb. Employee's right side was also scraped.

EE was cutting a timber off so it could be reset. A piece of draw rock fell and hit EE's hard hat first then it slid off EE's hat and struck EE's left hand. EE sustained a laceration requiring stitches to close the wound.

IE was struck several times by a water hose that had been turned on by a scoop bucket causing a laceration that required 5 stitches to close the wound.

A miner was pulling off a driven bolt in the rib and as they did the jackleg pivoted foreword and pinched their left ring finger between the machine and leg causing a laceration. Saw site medic, referred to the ER, x-rays were negative, 3 sutures were required, no work restrictions, making this a medical reportable injury.

EE was clearing a dryer feed chute. Fine material from grating above made its way into their Right Eye and caused irritation

Employee attempted to diagnose a feed flow problem. EE removed the rotary feeder cover and stuck hand in to remove any blockage. EE did not de-energize, or lock/tag as ee was trained. The rotating machinery cut off the tip of middle finger.

Temp employee was filling bags and upon release the bag fell. Employee attempted to catch the bag but couldn't stop it. EE continued working the shift and complained of mild pain at the end of shift. EE sought medical care on 12/10/18 and was released to return to work. Dr. requested a MRI and results returned on 1/9/19 revealed shoulder injury. Became reportable on 1/9/2019.

Over-exertion in lifting objects

Employee was loading a hydraulic cylinder into a service scoop bucket and felt a sharp pain in the RT Shoulder. The employee finished the shift but the pain worsened through the night and EE drove self to the hospital after working.

Friday morning 12/7, due to the cone screen stopping, the crew was working to get some of the material off the screen in order to reduce weight and restart it. While wearing cut resistant gloves EE started digging material off the screen. While performing the task EE caught the back of hand on a guard

Cutting a piece of plywood with circular saw and a piece of sawdust entered right eye around edge of safety glasses.

Fall from machine

EE fell off a trailer while unloading a screw. The accident was noted but no action taken at the time. In December ee went in for a hip replacement surgery that resulted from earlier fall that caused a broken femur.

While removing a 2"" diameter pipe using a 18"" pipe wrench, a 2"" connector broke. The pipe which was under pressure broke loose and struck EE on EE's left eye brow causing a laceration. The employee was examined, treated, and eight sutures placed above EE's left eye.

The injured employee was pulling on locker at same time with middle R finger in between the locker and the wall. The locker slid and pinched the end of IE's R middle finger causing lacerations. IE received 5 stitches to close the wound.

Two EEs were discussing an incident that occurred the day before involving both EEs colliding their mobile equipment they were operating into one another. No one was injured during the accident, however, as the conversation escalated, the two EEs engaged in a physical altercation, causing injury to one of the employees who later sought medical attention at a local hospital.

This accident occurred on a drill site off mine property. The injured employee was in the process of removing rods, used for plugging an abandoned oil well. The rod that was being removed at the time of the accident was bent and sprung and hit the injured miner in the lip. 4 stitches were required.

Employee was replacing pin in the 360 Komatsu Track Hoe using the sledge hammer to drive the pin. A metal shard broke off the pin and struck employee in the inner leg approx. 2"" above the knee causing a 1 1/2 laceration.

Injured employee was loading belt roller cradles onto a mantrip for transport. As EE was loading a cradle EE inadvertently smashed EE's fingers between the cradle and one EE had previously loaded. Employee was unaware of EE's hand placement between the 2 cradles. EE didn't violate any regulations and was wearing PPE.

While raising a canopy on a JOY 10 S/C Shuttle Car the canopy slipped smashing employee's left thumb between the canopy and the frame of the shuttle car. Employee received 7 stitches. A hairline fracture was also found.

The employee stated EE was cleaning out the back of the 551 charger when a 20lb fire extinguisher fell approximately 43 inches striking the employees left ring finger causing a laceration and a chipped intermediate phalange area. The employee return to work EE's next shift. This incident is being reported at medical reported due to chipped bone and laceration.

EE was setting up a hose to flush out material under the belly pan (located at eye level). When EE turned on the valve, the pressure caused the hose to move and strike EE in the right eye causing it to bleed.

Injured was using 12lb sledgehammer to install concrete anchor bolts in mill sole plate. While swinging hammer ee struck an adjacent coupling extending into the swing path pinching the tip of right index finger between the coupling and the hammer handle resulting in laceration.

Employee emptied the mop bucket into the sink when EE felt a sharp pain in lower back. It continued to move up EE's spine.

On 12/11/2018 we received notice from our workers compensation insurance company that this employee has been diagnosed with biopsy proven anthrasilicosis secondary to dust inhalation. The date of illness is 2-11-2014.
