The elevator was called to bottom and faulted just before it arrived. There were no occupants when this occurred. Through investigation, it was discovered that the braking contactor/resistor and door switch malfunctioned. Items were replaced and back in operation at approx. 7:00am. This elevator is not used as an escapeway.

Roof fall occurred at x-cut 20 between entries 2 & 3 of the Main East. The roof fall measured 55""L X 18'W X 5""T. The roof fall will be cleaned up utilizing MSHA approved roof fall clean-up plan. The roof was supported with 6' fully grouted roof bolts and 8' mega roof bolts. Date of development was 9/2017.
An unplanned roof fall was discovered in 13 West. The fall is located in 0 entry between crosscuts 3 and 4. It is approximately 8' high, 25' long and 16' wide. The area has been dangered off and will not need cleaned as no travel or ventilation was affected.
An unplanned fire occurred in the #55 conveyor belt transfer building. The taper lock on the #5 roller failed causing the belt to run out of alignment. The roller rubbed against the frame until it stopped and the friction of the belt continuing to run caused the belt lagging to catch on fire.
Roof fall 60 ft. outby spad 1273 4 ft. thick 18 ft. wide 35 ft. long.
A hot spot was found in the 3rd Main East return at crosscut 158, between entries #1 and #2.
Roof fall was found while traveling the weekly air ways. Fall was found on 3rd North West Sub Mains starting 1 break inby spad #10338 at the beginning of the inter section and stopping 1 break outby spad #10377. Fall is dangered off and supported as per roof control plan. The fall will not be cleaned up.
Electrical components of the secondary egress were below operable temperature due to heaters not functioning. Underground work ceased until the elevator is operable and ice conditions in the raise allow for safe access.
Employee was trapped in K-2 elevator between the 7th and 8th floor due to a mechanical malfunction. Employee stayed in communication through cell phone and radio. There were no health concerns and employee stayed in good spirits.
Haul truck brakes had locked up; a mechanic was called. Mechanic had parked in front of haul truck. When ee completed repairs, driver was to back up to test brakes. Driver stated truck began rolling forward, over berm, hit service truck which caught on fire, which in turn caught the haul truck on fire. NO ONE WAS INJURED.
JLG Forklift caught on fire. Mine was evacuated. Mine air was monitored until mine rescue teams were able to go into the mine and check on fire. Fire was finished and unit destroyed by fire when mine rescue teams got to fire location. Mine was checked for safety and determined safe for workers to return.
Safety Dept received a call @ 12:32am from manager that an employee was stuck in elevator. Several attempts failed to reset the elevator. MEI was contacted & they stated they would be there in 1 hour. Manager notified Safety who called MSHA at 12:39 reporting the incident. MSHA operator was told that the manager was in contact with the employee by radio and phone.
A power outage to Lansing Township, which included the mine, occurred. During a controlled evacuation of the mine, the generator for #3 hoist had trouble starting and people began to evacuate through #2. The generator was able to be restarted, however it took longer than 30 minutes.
There was an unplanned roof fall on Old 3b Supply Rd at X-5 to X-6.
During normal inspection, the rope gripper pads were found to need repair. The Hoist was taken out of service for an extended period of time to repair.
The elevator at Patterson Creek portal was out of service due to the limit switch on the cage malfunctioning. The switch was replaced and the elevator returned to normal service.
Heavy wet snow caused tree limbs to fall and trip out power line coming to mine. No. 3 Hoist down from 10:30 PM January 20th to 12:45 AM January 21st.
At approximately 3:00 a.m. the service hoist went down due to electrical issues. All employees that were required to evacuate exited the mine out the belt slope. No persons were trapped or injured. The hoist was back to normal operation at 6:05 a.m.
A fall was discovered on the day shift of January 18th, 2019. The fall area was located at left return, #2 entry, 254cc to #255cc. The fall was cribbed and dangered off.
An unintentional roof fall occurred in the #1 entry (belt) of the 6L longwall section. The fall occurred on top of the stageloader approx. 15 feet from the longwall face. The fall was 70ft long by 12ft high by 12ft wide. The fall was caused by adverse geological conditions in the form of laminated sand stone in the anchorage zone.
Roof fall occurred at x-cut 54 between 10 & 11 entries of panel 5. The roof fall measured 50'L x 18'W x 8'T. The roof fall will not be cleaned up. The roof was supported with 5' fully grouted roof bolts. The date of development was 11/2018.
Roof fall occurred in the 9 seam in the 1st Southeast sub main entry 2 between footage markers 5220 and 5140. The roof fall measured 25'L x 20'W x 10'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts. The date of development was 12/2018.
A coal smoker was discovered in the porcupine pit and was not extinguished within 30 minutes.
A roof fall occurred along MW#4 Belt, XC6, Entry#4, Sta 116+66.88. The fall is approximately 30' Wide X 40' Long X 15' High. It is dangered out and 2 rows of props have been installed on all accessible sides.

Roof fall in the old 9 East intake was found by preshifter.
At approximately 3:35 p.m. the service hoist went down due to electrical issues. All persons were evacuated the slope and no one was trapped or injured.
An unplanned non-injury roof fall was found on the 2E belt on the 4th Southeast Submains. The fall was isolated to the intersection of break 10. Minimal damage other than the loss of approx. 60' of belt, belt structure and control line in the immediate area. The area was cleaned up and the belt restored.
Contractors in the cement silo passenger elevator became trapped when one of the safety switches tripped (opened). It took longer than 15 minutes to reset the elevator and get the men down.
A fall was located on the B-3 belt in the #3 entry at crosscut 15. A fall had previously occurred here in 2017. An additional 2 feet of rock fell in this location on this date. The area was previously supported with 6' resin bolts and 12' cable bolts. No persons or ventilation was affected. The fall was cleaned up and the area was re-supported.
Due to a failed door switch the Elevator was reported down to MSHA around 8:00 p.m. on the above date. The switch was replaced and the Elevator was released to run at 1:00 a.m. on 1/10/2019.
A non-injury roof fall was found in the No. 6 intersection and the No. 6 right crosscut at crosscut No. 7 on the MMU No. 002-0 section on the 1-South Mains. The roof fall is approximately 20 feet wide x 35 feet long x 12-16 feet high. The roof fall will be cleaned up and the mine roof re-supported. The MSHA Hot-line was notified of the roof fall. See map. There were no injuries.
A roof fall measuring approximately 60'long X 20'wide X 4' thick was found in the Powellton No.1 Mains Intake, entry #2 @ 101 cross cut. This fall is located 1 cross cut outby survey station #970. This fall does not impede travel or affect the ventilation. The fall has been cribbed and dangered off.
Unplanned roof fall occurred in the last open crosscut between the #1 and #2 entries inby spad 1548. The fall was approximately 16 feet wide and 30 feet long and about 6 foot high.
A roof fall occurred at #8 entry. The estimated size of the fall is 18 ft. wide x 50 ft. long x 6 ft. high. No injuries were reported as a result of the roof fall.
A coal smoker was discovered in the coal pit and was not extinguished within 30 minutes. Was extinguished shortly after.
Minor amount of smoke was observed from spontaneous combustion of coal occurred in area 6.
The fire was in our cooler system. Equipment failure caused part of the flow of material to plug. With hot material spilling there was a small fire at the bearing. It was behind guarding. The fire caused a lot of smoke and the fire department showed up. They were not needed.
An aluminum hydraulic filter housing on the hoist rope winch cracked causing an oil leak. This oil leak caused the mine management to remove the hoist from service until repairs could be made.
An unplanned roof fall was discovered at the 15-15 crosscut between the track and belt. The fall is approximately 20' long, 7' high and 12' wide. All accesses have been dangered off.
As a result of excess grease build up the production hoist pinion brakes would not set properly to perform the static test.
A fall of roof occurred on the 2nd Main South at # 4 xc Entry # 6. The fall measured approximately 75 feet long x 18 feet wide.
An unintentional roof fall was found between breaks 16 and 17 on the #3 Belt line. The fall was approximately 60' long by 20' wide and approximately 8-10' thick.
Inby fall area was sub-normal roof, additional support was installed when mined. At a later date area was observed taking weight on roof support and cribs were added. Over the extended holiday the crosscut outby the cribbed area started to work and fell four to six feet above the coal seam. Area was unstable due to laminated shale rock.
Wesco bulk truck arrived on the pattern and was positioning to load the first blast hole. Operator saw smoke then flames come from behind the cab and exited truck, May Day initiated and mine evacuated to 5000ft.
Due to rainy weather, gravel was washed into the safety brake system of the hoist causing a mechanical failure around 3:30 a.m. on 1-1-2019. MSHA was notified at this time. The safeties were cleaned thoroughly and tested multiple times to verify proper function. The hoist was released to run and MSHA was contacted.
While the surveyor was setting up the instrument in #4 belt entry, the other surveyor was getting height measurements down #3 entry. A shuttle car loaded with coal coming from #2 entry tramming to the feeder struck and killed the surveyor near spad 24.
A miner was fatally injured when they were pinned between an air-lock equipment door and a concrete rib barrier located near the shaft bottom.
Employee states ee was backing out of 4 heading when drill cable quit taking up. EE states ee turned reel off and reached to grab cable to throw on back bumper. The cable tightened catching fingers between bumper and the cable. This caused an avulsion to the end of ring finger on right hand.
Frozen material jammed auger. Pellet mill operator attempted to free jam by using a metal bar. Bar slipped off of leverage point, pulley recoiled back and pulled hands into belt. Incident resulted in pellet operator losing two fingers on left hand.
Cement silo was not flowing properly, ee shot air into silo to increase flow. EE put hand inside silo and paddle wheel caught fingers on left hand. Index finger was removed by doctor and middle and ring finger were broken but were saved.
Using a slate bar to shift a track rail over to install track ties and had pain in groin area.
WHILE EXITING THE 544 LOADER THE EMPLOYEE SLIPPED. THE EMPLOYEE GRABBED THE HANDRAIL WITH LEFT HAND CAUSING A ""JERK"". THE EMPLOYEE FELT PAIN IN LEFT SHOULDER AND NECK. EMPLOYEE CONTINUED TO WORK UNTIL MARCH 2ND, 2020 WHEN EMPLOYEE SAW A DR. AND WAS TAKEN OFF WORK UNTIL NEXT APPT.
Employee received a laceration and a fracture to the left thumb when it got caught between the latch mechanism and the release mechanism of a cable come-along while attempting to remove tension from a belt clamp.
Changing a grease cartridge in a grease gun. Pulled back on the handle and felt something pull in neck.
Helping install a hold-down roller on a conveyor. Employee was watching co-worker use a hammer and chisel to seat a bearing onto the roller shaft. As the co-worker struck the chisel with the hammer, the employee was struck in the right eye by a flying object (small piece of metal)
While servicing the shearer, employee was struck by an open, 1/2 filled can of oil that was being tossed back into the shield/gob. The employee was struck by the can in the right knee, causing injury.
30 below freezing temperatures with icy walk ways. Employee was walking to work station when they slipped and fell on ice.
Employee was setting timbers. EE picked up a timber and felt pain in lower back.
Tractor trailer was dumping a load when a strong wind gust turned trailer and tractor over. There was about 3 tons of coal frozen in the left front of the trailer due to sub zero wind chill. Truck was on a slight grade. All these factors combined is what caused the accident.
A fully trained miner fell backwards landing on SCSR and bruising right hip. The accident was not the result of any compliance issues nor were any protective guards involved.
On 1/30/2019 while breaking apart 10"" waterline. The pipe swung over and struck EE's hammer while EE was holding it, jamming right elbow. On 2/11/2019 EE was trying to remove bit from miner head, missed bit with hammer causing further pain to right elbow. On January 8, 2020 EE became a Lost Work Day injury due to having surgery on right elbow.
Employee was swinging a hammer overhead to vibrate a bin to loosen material and strained shoulder.
EE was using a hand held drill to clear dirt and other material from the hole that the pin goes into in order to connect two highwall miner cars. The drill bit hung in the pin hole and the drill twisted causing the injury.
Employee was mopping the floor when they cut a finger on an outlet box. We were notified on 2/6/19 from a doctor that there were stitches.
Employee stated that while trying to carry a top structure cradle out of the belt entry on the longwall ee felt a pain in back.
Operator & Oiler were in process of moving power cable to maneuver Machine. Oiler hooked up 2"" sling around power cable & using 1"" sling hooking to fan tail hook. Before Oiler was ready Machine started to swing while Oiler's hand & fingers were still under sling on power cable side. Resulted in four fingers getting injured from the pressure of the sling when Machine swung.
Employee was helping drop hard pressure pipe off monorail by hand and took chain off and was too heavy to hold dropping it on employee breaking hand and foot
As the employee was hand loading cable bolts onto the roof bolter, ee strained neck. EE finished shift but when ee woke up the next morning ee was unable to turn ee neck left or right. The doctor diagnosed ee with a pinched nerve in ee neck. EE began missing work on 1/30/2019.
The employee was on a step ladder plastering a stopping. The employee slipped off the ladder and landed on a block, injuring upper back, right shoulder, and neck area. The employee was trained in the task and was wearing all the appropriate PPE. The employee did not violate and laws or company policies.
Running in pit and tripped. Sprained right ankle.
A fully trained miner was riding in a man trip when it went into a switch and hit another man trip causing EE to jar back. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was walking down the BC-20 incline when the grating shifted and the grating in front of ee came loose from the framing and when ee stepped on it, it gave way. EE fell to the ground about 6 ft. below.
Employee stated was walking backwards as EE was backing the miner up when EE twisted and injured the knee. Employee sought medical attention and was placed off work under doctors care.
Employee(#1) was installing an accumulator on a CAT 980H Loader with the assistance of a Quarry employee (#2). #2 was holding accumulator while #1 was removing bolts. #1 saw #2 struggling with holding up the part and came down to assist. When #1 took hold of part, #2 let go of part and the unexpected transfer of weight caused #1 to strain upper back muscles.
While tramming bolter forward, employee was caught between the bolter and the rib, resulting in bruising to the abdomen.
An employee was fabricating a swing case stand while working on a welding table. During the process the employee attempted to flip the piece over to continue welding and in doing so the steel frame fell off the table. The frame landed on the employee's foot, fracturing the foot.
EE was walking down the #3 entry carrying 2, 27"" pumpable can crib bags. As EE was walking, they stepped on a loose piece of coal/rock and rolled their left ankle, causing them to twist & fall to the mine floor. EE suffered a left ankle fracture & dislocation as a result. EE was wearing required PPE and didn't violate any rules or regulations.
Employee was climbing up onto Komatsu 475 bulldozer. When EE caught the toe of the boot on the track causing EE to fall forward. EE landed on right wrist resulting in a small closed fracture.
An employee tripped and fell causing pain in their knee.
Operator was dismounting a 785B End Dump, off a ladder when incident occurred. Operator missed the last rung, fell backwards to the ground. Put out left hand to break fall, fell on left hand. Fracturing left distal radius.
Employee was dragging a wash down hose on the first floor of the prep plant when it got caught and caused the employee to strain EE's right groin area. Employee suffered a hernia. Employee worked till 01/30/2019 when EE was taken off work.
Employee was rib bolting in the two east mains, when EE's right hand caught between two drill steels causing a laceration to the right middle finger. EE received six sutures.
Employee was changing hose on pan line bolter when EE was struck along the left eye brow resulting in a laceration. Cause of this incident was lack of concentration. PPE was not a factor, mining systems was a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
The employee was riding in a mantrip along 9 South Haulage. The mantrip picked 16D switch, went into the spur and struck a parked ride before stopping. The employee was thrown back against the seat and struck neck area on the SCSR storage box. The employee didn't violate and law or company polices, and was wearing all the appropriate PPE.
EE stepped over the mill motor's tracking rail and slipped on ice which measured 24'x18'. EE lurched suddenly and slightly forward to maintain balance. The tracking rail measured 3.25' tall and 4' wide. EE reported to supervisor within the hour and 2 days later ee informed supervisor that ee had significant pain on the left lower side of back.
Employee smashed right ring finger causing a fracture to the tip of the bone and an avulsion when EE's hand got caught between a diwydag sling and the cable come-along handle while lowering centenary belt structure.
The employee dropped a bottom roller on the offside of the belt and while coming back across the belt they grabbed a belt chain and put their left foot on the belt rail to step down to the mine floor when their foot slipped and their leg went between the belt and the belt rail hyperextending their knee.
Miner was helping to shovel sand and felt a pain in EE's lower back. The next day EE called in sick and reported having aches, chills and fever that morning that EE associated with EE's back injury.
A fully trained miner was loading rollers in a hoist basket when EE twisted right ankle. The injury was not the result of any compliance issues nor were any protective guards involved.
Center bolter cable was being pulled by the scoop. Employee was standing approximately 3' from cable. As the cable was being pulled, it flipped toward center of entry, striking employee in the left knee.
Employee injured (strained) left arm (bicep) while hand unloading track steel off of a supply car at the end of the track. EE was diagnosed with a tear in the bicep that will require surgery.
Lifting and pushing screen head resulted in hernia in abdomen. Returned to work full duty after missing 1 day. Potential for surgery in the future.
The employee entered the cab of the excavator, sat in the seat, and was closing the door when employee left a finger in the door jamb too long and closed the door on thumb. Lost time as of 1/28/2019.
Employee was working around the scoop on a section while moving up the return, when the scoop hit a timber, the timber struck the employee causing a laceration to the forehead requiring sutures and a fracture above EE's left eye.
While swinging boom, a rock approximately 10""x12""x3"" slid off canopy striking right ankle. Employee finished shift and continued to work. Incident became a L.T.A. on 2/4/19.
EE slipped in the parking lot on 1/24/2019 and hit an elbow. EE went to the Dr. on 3/5/2019 to have it seen and was placed on restricted duty at that time.
Employee was struck in the head by a water bottle that fell from above.
Employee jumped off man lift instead of using the ladder, sprained right knee.
While mounting a 24.5 L24 size tire on a one piece wheel, a tire bar slipped and struck EE in the mouth causing a broken front tooth.
While reclaiming High Voltage, employee twisted left ankle and broke 3 metatarsal bones in their foot.
EE was assisting in a power move. While moving a waterline, EE placed right hand on top of a scoop bucket. At the same time, the scoop operator began to move the stinger back catching the little finger in between the stinger and top of the scoop bucket. A fracture and laceration of the little finger was a result of the incident.
Everyday crawling. Employee's knee has been swollen up through out this time. Getting it drained and numerous doctors. They decided to do surgery on 5/28/2019.
Employee was hammering on tail roller shuttle car and a piece of metal went into EE's hand.
Employee stepped forward and slipped on miner cable causing them to fall and twist a knee. after seeking medical treatment, they went back to work. EE has continued to have issues with the knee that has lead to lost time during each follow up with doctors. On April 4th, they underwent an ACL surgery. EE was taken off work until reevaluation on May, 7 2019.
Frozen rain from night before created icy conditions outside making everything extremely slippery. Miner was attempting to apply salt to walk ways and fell doing so.
Employee was moving supplies and twisted knee.
Tire tractor pulled into a cross cut to allow tomcar to pass by. While attempting to back further out of the way the tire tractor moved forward contacting the tomcar operator as EE drove past. The operator received a laceration to the arm and was seen by the on-site medic. EE was transported to St.Vincent Hospital and diagnosed with forearm laceration.
Installing bottom rib bolt, EE fell off bolter and caught self with hands on ground, causing pain in shoulder.
Employee slipped on a patch of ice while walking to a company vehicle which resulted in a sprain/strain injury to the left knee. On 5/22/2019 the employee was placed off work due to a surgery that was performed at which time this became a Lost Time Incident.
Employee was exiting a D11R. EE stated that EE's right foot was on the last step on the push arm as EE was lowering EE's left foot down to the ground. EE lost EE's balance on some frozen material and twisted EE's knee as EE tried to regain EE's balance.
Employee was operating shuttle car and hit a bump causing the hard hat to fall off and EE hit head inside the shuttle car canopy.
EE handed IE a metal liner. When IE grabbed liner and twisted to set down, IE felt a sharp pain up back. Hand and foot went numb - pain in upper back. IE was taken to health care center. Work release reads may return tomorrow if able to work without taking pain medicine to do light duty. Needs follow up 3-4 days for further evaluation.
Employee was moving from seat to seat on the shuttle car, to change direction, when employee felt pain in lower back.
The employee was carrying one base lift jack on EE's shoulder to deliver it to the face of the longwall set up and tripped over a crib. The employee fell down and the jack landed on EE's right lower arm causing a contusion. The employee did not violate any laws or company policies.
The IE was air arcing in the main shop when IE's clothing caught fire and caused burns to IE's skin.
Worker was reaching to pick up a box, bumped into a pallet of finished material and knocked a rock off of the pallet on to the top of their foot .
Employee was exiting D11 Dozer when they stepped on push arm step which had frozen mud on it, their foot slipped and they fell to ground hurting right shoulder and knee.
Employee stated EE was getting ready to lift a piece of sheet metal when the end of an Aqua water line busted and the water struck EE causing EE to fall backwards landing on a I Beam. Employee injured EE's back during the fall and was diagnosed with a fractured vertebrae. Employee was taken off work.
Employee was exiting parked personal vehicle in the employee parking lot when employee slipped on ice. The employee suffered a fracture to leg when employee fell on the ice. Employee had not yet reported for work.
Employee slipped going down ice-covered steps on Hitachi Shovel and fell, injuring back and neck. Employee continued to work normal job until ee was seen by orthopedic surgeon on 3/14/2019 and placed off work.
While moving trailing cable, the IE slipped on the freshly bladed ground and felt a pop in the right arm. EE was taken to the ER for evaluation and referred to an Orthopedic Dr. The IE was seen by an ORTHO and scheduled for an MRI. The MRI reading revealed a tricep tear that required surgery to repair.
Supervised contractor was rolling a cyclone vortex into position for a crane pick and in the process tore EE's bicep.
Employee was moving 4"" schedule 40 pipe over about 4"" to hook it up to drisco. Upon lifting the end of the pipe EE felt a ""pop"" in left elbow. EE was seen by the on site medic and referred to a Medical Center to be evaluated. EE was then referred to a specialist and diagnosed with a ruptured tendon.
Employee squatted down to place a tire on a ride when EE went to reposition self EE felt a click in the left knee with an immediately burning sensation.
Employee was swinging a sledge hammer and felt EE's left knee torque. The employee was seen by the company doctor and was taken off work.
Employee was installing boar in the face of #3 entry on south mains. Employee was pushing the 5' bolt up into the hole with the pusher, and the head of the bolt slid off the pusher causing the stored energy from the bolt to make contact with the palm of their left hand resulting in a broken left wrist.
Attempted to roll a pump motor off mantrip when I bolt on top of motor broke causing employee to lose balance falling to the ground. Employee hit hard enough causing EE to chip tooth and jam neck and back.
While performing maintenance pressure washing the 6030 SH002, employee slipped and lost balance causing fall to ground. Employee extended arms attempting to brake fall and felt pain in left wrist after impact with ground. The next day employee was taken to local Urgent Care and diagnosed with possible fracture and scheduled for MRI.
Employee alleges ee was trying to turn a push beam with a slate bar and in doing so hurt back. The event was not reported until Jan 25th, 2019. On Jan 29th ee presented an excuse that covered the days missed as being under Drs care for the alleged injury.
Employee was performing a count of shovel/loader teeth requiring to partially move the position of the teeth. Employee reported discomfort in left groin. Evaluated by surgeon and it was made known on 3-6-19 that employee had a hernia.
Employee was loading bolting supplies in an Underground loader bucket when EE felt a pull in right groin area followed by discomfort. Seen by site medic and referred to the ER, restrictions were issued muscle strain.
Employee was caught in miner cable slack, waterline, when the employee slipped/fell while trying to take down slider curtain causing the employee to sprain right ankle.
Clear weather, dry conditions, employee was clearing excess dirt from under conveyor belt when triangular guard fell on them and bruised chest.
Employee was struck in the right foot by a piece of a kettle bottom while trying to install a roof bolt to support it.
The ee was operating a CAT 740B Articulating truck when the bottom center pin sheared off causing the cab of the truck to tilt forward jolting the operator. The ee stated ee had pain in lower back and left leg numbness. The ee did not violate any laws or company policies and was wearing seat belt. The ee pre-op-ed equipment prior to operation.
The employee was hanging high voltage cable during a power move on 01/17/2019 and felt pain in their left shoulder. The employee continued to work until 03/25/2019 when the employee went out for surgery.
When putting a roof strap onto the strap jack, a rock fell out of the top, on the opposite side of the miner, causing the other end of the strap to bounce up and hit the employee's left hand. The employee worked up until 1-25-2019 and was then admitted to the hospital for infection to hand.
Fall protection was not being utilized correctly resulting in a fall from height. 2 broken ankles and a broken leg.
Mechanics were working on the endloader that was operated by the injured employee. Employee climbed into the back of a mechanic's truck to get a can of starter fluid and when stepping off the back bumper of the truck, stepped into a hole and twisted their right ankle. Employee said they did not see the hole.
A fully trained miner was on a longwall shield when the shield recycled and lowered pinching the employee between the shield and the panline. The injury was not the result of any compliance issues nor were any protective guards involved.
EE was loading roof bolt resin onto the roof bolter and felt pain in lower back. EE continued to work every shift until 2/19/2019. After reporting to a physician it was determined as a lower back strain. EE has to take time off until therapy is complete.
The operator slid down from the excavator being held with the right arm supporting all the weight of the body.
EE was walking ribbons during metal removal. EE states was walking a ribbon and stepped down funny because the ribbons are not level, EE felt pain in the knee. EE was seen at the ER and diagnosed with a strain to the knee and put off work.
Getting out of man trip when ee stepped in hole and twisted left ankle.

Employee was pulling curtain from underneath gob and felt a sharp pain in shoulder and forearm.
As the employee was removing crib blocks from under a shuttle car in which they were finished making repairs to. As the employee reached in to pull a crib block out, a crib block on which the service jack was planted on split in two and allowed the car to sit down on the employee's right hand causing a fracture to right index finger and multiple fractures to thumb.
The employee was re-railing a mantrip, while gathering cribs, the employee smashed left thumb between two cribs.
Employee stated EE was in the process of inserting a stick of glue when a rock 3'L x 2'W x 2"" thick fell out from between bolts behind EE and struck EE's right shoulder and middle back. Employee sought medical treatment and was placed off work.
Employee along with their partner was putting a belt back on an electrical motor that run a shaker table. As employee was rolling it on the oblong pulley it cammed over and pinched the left ring finger between belt and pulley at last joint to tip causing a partial de-gloving. Seen by site medic, referred to ER, X-rays showed a small chip to bone. restrictions were issued.
Was doing maintenance on the crusher. Taking the drive chain off the hopper and the guard tipped over and hit the top of head and hurt neck. EE was wearing a hard hat.
EE was hanging monorail between the longwall stageloader & ribline, just outby the longwall face. A piece of the rib suddenly popped loose above a rib bolt and fell, hitting EE on the back of left leg, knocking them against the stageloader. EE suffered contusions and back spasms. EE was wearing required PPE & has training & experience.EE didn't violate any rules or regulations.
During the process of installing a HDPE pipe a worker's foot became stuck in mud. While EE was pulling it out EE strained right knee.
IE was throwing roof bolter cable over against the rib so it would be out of the way for haulage an the MN 5 section. As IE stepped forward IE felt a pain in the left knee.
After being fueled at the 5100 fuel dock the employee mounted the 621 Haul Truck, the latch on EE's hydraulic stairs did not latch. The employee used EE's right foot to lift up and engage the stairway latch, as they did this they heard a loud pop from their right ankle.
Employee was helping mechanic put left cylinder back on D6 Dozer when the cylinder hit against the grill on dozer smashing employee's thumb against grill. Went to Clinic, right thumb was broke in (2) places.
EE was riding in the passenger seat of a pick-up truck traveling up the 400 hill. The truck bounced on a rough area of the road causing EE's head to jerk back, causing pain in neck. The EE was not performing any job tasks and did not violate any rules or regulations.
While climbing ladder on grader, pain was felt in neck and middle back and spread to shoulder and down right arm. Fell off the ladder and landed on my back. It was a cold, snowy day at the time of this incident.
IE was bolting the face of #2 entry when they adjusted the wire roof mesh and a rock measuring (3' x 1.5' x 3"") struck their right hand. This resulted in a fracture to the hand.
While setting up to lower power cable from the cables tree the brake handle contacted the employee's face resulting in a laceration and a fracture.
Employee was Pre-shifting the F29 Longwall belt at 6 1/2 wall walkway side, when left arm was injured by a moving roller.
Winter conditions. Contract employee slipped and hit head. EE walked to the supervisor to report the fall. There was no accident witness. Took employee to the hospital. EE suffered a severe concussion, cerebral hemorrhage and a skull fracture. Contractor filed 7000-1. Late submission due to not knowing we had to file a report as well.
EE was climbing up onto the 35 ton haul truck after doing the pre-shift inspection. EE slipped on a glaze of ice on the step and mud on EE's boots, causing EE to lose grip with hands. EE fell on left shoulder and fell about 2 feet. X-rays taken and later an MRI indicating rotator cuff tear. EE continued to work until surgery on March 18, 2019.
Employee was taking a bagged PM kit off a shelve in the surface shop at chest height when EE felt a pop followed by pain in EE's mid back. Seen site medic and referred to ER, x-rays were negative, a follow up was scheduled with a physician where restrictions were issued for a back strain
The employee was attempting to move a full wheelbarrow when the employee felt a pain in their lower back.
Employee was placing red tape up to protect workers below an area to be worked. The employee slipped and tore right quadriceps muscle.
The EE was straightening up materials on a supply car that was parked in the Crash Pit. The employee went to step back and slipped, striking EE's right shin on the supply car. The employee had some swelling and a contusion. The employee did not violate any laws or company polices and was wearing the appropriate PPE. The employee worked up to 1/19/19 when EE began missing work.
EE was operating a continuous miner (CM). As ee engaged the cutting head, a rock fell from the mine roof and struck the top of the CM, which caused a rock to slide off of the CM. The rock that slid off the CM struck ee in the right hip, causing a hip dislocation.
Employee stated was climbing in the #382 Shuttle Car on the Unit 1 Section. In the process of getting into the shuttle car EE's left knee gave out and EE fell to the ground.
Employee had just turned off oxygen and acetylene in the back of the work truck. Employee stepped back to get down out of the bed of the truck and their left foot slipped off bumper. They tumbled out of back of truck and fell awkwardly on their left leg injuring the left knee. Knee swollen and unable to walk at this time.
IE was attempting to pull off cart full of belt material off the elevator, cart got stuck on a lip and IE claims to have felt soreness in back. Cause of this incident was proper body mechanics and awareness. Compliance with rules/regs, PPE, mining equipment was not a factor. Miner proficiency may be a factor. Employee worked up to the 15th then went to seek medical attention.
Holding water line that had ice in it and burst, knocking employee to ground injuring left shoulder.
The employees were getting ready to wash the ledge with a water hose when they turned it on they didn't know it had some ice in it. The hose stopped up and caused pressure to build up in the hose. The employee that got hurt couldn't control the hose & got struck in the right eye. Happened fast. Nobody knows if the hose hit them or if they hit their eye when they fell.
While pulling preconstructed steel set with a miner, the frame caught on an obstruction and a piece of v-decking sprung loose, striking the injured employee in the left cheek, causing a laceration.
While attempting to line bolt head up with drill chuck, miner bumped rotation on drill chuck while holding the roof bolt. EE's glove was snagged and EE's fingers became entangled around the bolt.
Powerhouse boiler operator was climbing steps (2) at a time going to restrooms. When the operator felt a pain in right shoulder.
Employee was climbing a ladder when EE slipped and fell on the right leg / right side. Employee climbed on the ladder to about 5ft to get to screen platform. Area was watered down right before the employee started climbing and EE's work boots were very muddy. Employee states had three point contact. Employee fractured left Femoral neck bone.
Employee experienced severe back and chest pain while shoveling sand spilling from a dryer belt. The employee collapsed onsite, EMS transported them to the Kermit Memorial Hospital. Employee was evaluated and released to return to work on Jan. 14th. The employee sought additional medical evaluation; subsequently treated for muscle spasms with followup scheduled for 1/23/19
EE was walking out of the floc room and slipped down three steps and twisted left knee.
While walking behind the roof bolting machine as it was traveling across the section, the bolter helper got their head hit from the top of the canopy up to the roof. Due to the height of the section, the helper was walking bent over where helper's head was out in front of helper, unaware of how close helper was to the bolting machine.
Employee was moving the cable reel cover back into place on the mule when they pinched their left middle finger between the mule and the cover. Fracture to the left middle finger.
Employee was stepping off bed of a truck and twisted EE's left knee.
Miner was turning crosscut 3-4, stopped to remove a rock from tail of miner when a piece of coal/rock rolled out from rib between the posts striking right hand causing fracture. Rock size unknown.
Employee was moving a miner cable out of the roadway while ee was pulling something pulled in right arm.
IE was struck by a rock that fell out between roof bolts. EE received a laceration to EE's left ear and a fracture to the second metatarsal of EE's right foot.
Minor aggrevated back from an injury sustained at home prior to this incident. strain
On 1/10/19 at 6:00 pm a shuttle car operator started experiencing pain in left side back/kidney area. The pain started as EE was descending a grade in the mine floor. EE finished shift & completed a report. On the way home the pain increased to the point EE called spouse to come meet EE. Spouse took EE to hospital where 1st diag. was pneumonia, 2nd was contusion.
Employee was shoveling belt in low top and felt a pain in lower back. Employee started missing work on 1/10/2019
1-17-19 Employee stated ee fell over miner cable or miner pull strap on #2 unit on 1-9-2019 stated didn't report to supervisor thought it would be fine. 17th ee reported and we took ee for medical attention, released no restrictions! 1-21-19 I sent ee back to Dr. ee states ee was having trouble 1-22-19 is the first lost day of the incident.
Employee caught right index finger between the pinner chuck and the mine roof. This happened while employee was attempting to get a drill steel out of the mine roof. The right index finger was fractured.
Employee was building cribs, as well as participating in other routine belt line maintenance acts during shift, and stated that EE strained EE's back.
Employee was operating diesel mantrip on 3B supply road and a piece of rock fell struck employee's left shoulder.
Employee was throwing roof bolts off of the bolter toward the unbolted face when employee felt pain in lower back.
As EE was stepping over a tree branch, left foot had cleared and as right foot was going over a gust of wind mixed with snow, sand and dust hit ee in the face right foot hit the branch causing ee to fall. EE caught self with right hand resulting in a fractured wrist
EE was working on the miner when ee took the rub rail off. It caught left index finger between the miner and the rub rail requiring 14 stitches.
EE was hanging roof bolter cable. EE was hanging the cable above their head when they felt a pop in the left shoulder. EE continued to work until 8/8/2019 when they had surgery to repair a tear to the left labrum.
The miner was doing a plant startup when they noticed smoke from the screen tower. The miner proceeded toward the screen tower to inspect the problem and fell on loose material and injured their shoulder. Miner then went to the doctor and did an x-ray / MRI resulting in a torn rotary cuff.
The miner was bolting up last ring with a jackleg when EE's right hand was struck by a rock from the footwall/ back contact. EE was seen by the on-site medic and transported to Livingston Medical Center to have EE's hand evaluated. EE was diagnosed with fracture to a metacarpal and issued restrictions.
Employee was filling the continuous miner up with oil and when ee was opening up another can hand slipped cutting left index finger. The metal can had a rigid place where ee cut with a knife.
Employee was removing rocks from the grizzly bars at the feeder with a sledge hammer. While doing this, the employee felt a pain in back. Injury reported 1/9/19 and lost time as of 1/10/19.
Employee was exiting a man trip on Kuhntown bottom when EE's boot lace got caught on the safety chain hook, which caused EE to trip and fall against the rib causing a injury to EE's lower back and right hip. It did not involve job skill, proficiency, training, PPE or protective devices.
Employee was burning bolts off of the bottom shaker screen. When switching hand to burn it, employee knocked the oxygen lever on torch. The burst blew coal dirt under employee's glasses into eyes.
While stepping out of the portable toilet next to the dispatch office the dispatcher fell backwards striking back of neck and head. Physician advised dispatcher to remain off work for 2 to 4 weeks.
EE climbed the ladder to get back in position to operate the loader. As EE was climbing, EE missed a step & fell forward which caused EE to reach out to the hand rail to catch self. When EE grabbed the handrail EE had pain in left shoulder & the pain has continued in left shoulder since this incident happened.
Employee was placing filter fabric over drain, climbing layers of rock. While climbing, employee rested hand on rock and it moved. Employee fell with rock striking right lower leg and foot. ER exam showed fractured fibula 3in. above ankle.
Employee (miner) lost footing on stairs & slipped right leg was caught between stringer and bottom of handrail.
While cleaning up spilled floc material employee slipped and fell onto 6 inch supply pipe that was crossing the slab, injuring tailbone. Bad bruise only. No breaks or other injury.
While manually moving a pogo (scissor lift) the employee slipped and fell causing the lift to fall on their leg.
Employee was laying rail in the 6 West Section and was bent over tightening up fish plate bolts when EE felt a sharp pain on the lower left side of EE's back. Cause of injury was improper body positioning. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
IE claims that when closing the door on a storage container, EE felt a pain in arm. After follow up for persistent pain on 1/30/2019, IE was placed on restricted duty until an orthopedic surgeon could decide if a steroid injection is necessary to control inflammation of shoulder joint. This injury is still under investigation.
Employee stated was cleaning out the deck of the shuttle car when EE re-injured an old injury in EE's middle back area. Employee called in the following day.
Employee tripped and stumbled over a pallet in the maintenance shop yard and struck the front bucket of a parked backhoe causing the fracture of 3 ribs.
The injured employee got EE's left thumb caught between a forklift mast and a fork causing a laceration to the left thumb.
Employee slipped and fell on ice/snow straining upper back, arm, and both legs. Went back on January 21st, Local clinic sent employee home for 3 days to rest and recover. *EE was diagnosed with fracture to left ankle on February 22nd; taken off work as of February 25th. Surgery scheduled after appointment on March 26th.
Worker was working closely between 2 pieces of steel causing the finger to get caught between the 2 steel structures causing a finger nail avulsion on the thumb.
EE caught EE's hand between a chain and discharge head when pressure was applied.
Employee had just set another 4' x 8' section of screen on the bolter's stinger to continue covering the back. As EE was walking away, a rock fell onto a partially secured screen & sprung off the end of the wire catapulting the rock 10' at & contacting the miner in the shoulder and forearm as EE had back to it. X-rays were positive for a fracture to right forearm.
Employee was standing up hanging line curtain above EE's head when EE felt a pain in lower back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. Employee worked AFT shift 1/4/19, staying in between shifts, and getting injured on 1/5/19 at 1:00am.
EE was changing bags in the baghouse, when sand/dust became lodged under EE's contacts. EE was wearing Safety Glasses at the time.
Employee miss stepped on the cell line during harvest and fell, rolling right ankle.

Employee was walking in the 2b beltline when EE stepped in a hole causing EE to fall and twist EE's right knee. EE continued to work until EE starting missing on February 4, 2019 in order to schedule surgery for a meniscus repair.
Employee was dismounting a front end loader & while climbing down the last set of steps to the ground, the second rung down broke causing the employee to fall to the ground. Employee's left leg went inside of the next rung down causing a small abrasion to the left calf. Employee landed on right arm causing sharp pain in the right shoulder & a bruise to right arm.
IE was undoing a clevis from the cab of the John Deere truck, and another EE took hold of the cable and in doing so caused IE to lose IE's balance and fall on IE's right forearm, striking the lift cylinder of the truck and causing a fracture.
An arc flash occurred as an electrician was attempting to check voltage on the secondary side of a 4160 Volt transformer. The electrician was seriously burned on both hands and one side of face and neck.
Miner scheduled eye doctor appointment after complaining of eye irritation. Finished shift and went to doctors following day. Piece of steel was found in eye.
Removed top rollers on a PC650 utilized a bottle jack to get some slack for the placement of the roller. That is when the jack slipped catching the employees hand
A fully trained miner was recovering a section of discharge line when they stepped in a hole and strained their back. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee had stepped on top of a covered duct near the fan turbine shaft near an overhead metal grating. EE reached up to hold the edge of the metal grating to step down slipping backwards catching wedding band on a 1/8"" tip of metal that was on the grating. The ring due to the downward force created an avulsion type wound to left ring finger. This resulted in stitches.
Employee was prying on the snake head trying to get an E-bolt started through the slot in the pan line, when EE reached down to try and start it by hand, the other employee did not see EE put EE's hand down, accidentally hitting EE's hand with a sledge hammer causing fracture and sutures to fingers.
The injured contract employee was assisting to extract shield #89 from the extraction face when a rock measuring 21"" long by 11"" wide by 2"" thick fell from in front of the walker shield and struck EE on the left foot.
Employee was examining belt in low area. Employee twisted awkwardly while bent over and felt pain in lower back.
Employee states that they slipped and fell on some ice near the 5280 fuel dock. This incident was reported on 1/22/2019. Employee had no restrictions. HR informed on 3/20/2019 that employee went out for surgery.
Employee fractured and lacerated the end of the middle finger on left hand when replacing a conveyor belt tail pulley bearing assembly.
The ground conditions were wet due to rain. The employee attempted to grease the front bucket of a front end loader by climbing onto the bucket. The employee slipped and fell hitting left shoulder and neck area. The employee suffered a strained/sprained left shoulder and a strained/sprained neck.
The employee stated was unloading a desk from a truck and pinched left thumb between the desk and the tailgate.
Employee stated employee bent over to pick up a bundle of plates when employee felt a pain in employee's back. Employee sought medical treatment and was taken off work.
While the miner was stepping off the dredge onto the gang plank, the plank shifted causing the miner to fall into the water. Associate sought medical attention on 2/11/2019 and was placed on restrictions.
Lifting a speed reducer weighing approximately 80 pounds and cleaning up spillage. Strained both shoulders as a result of over exertion.
Employee was installing scalper plates on # 4 rod mill discharge box. The scalper plate was already installed and employee was in the process of installing a 2nd one. As employee was sliding down the scalper plate, caught right ring finger in-between the 2 scalper plates causing a laceration.
Employee slipped on icy surface hurting right shoulder, continued to feel pain and was found to have shoulder problems requiring surgery on 04/11/2019, was originally reported as 1st Aid. Is now lost time.
Employee lowering landing gear on portable welder trailer. Trailer hooked up to forklift. Putting stabilizer leg of trailer in place, aprox 15 lbs. Pulled pin with dominant left hand, held leg with right hand. Leg slipped out of hand, reached for it awkwardly, hyperextended elbow. Received first aid. Injury reportable on 2-11-19 when employee received work restrictions.
Striping insulation off an electrical wire with utility knife slipped and cut pinky left finger at 3rd joint.
While Crossing #1 tailpiece subject slipped and fell and twisted Left ankle. Employee then fell backward landing on right wrist and sprain the right wrist.
Employee was leaving inside of mill. As EE was leaving mill EE's grip slipped and EE fell. As EE fell EE caught self and ""strained the right shoulder muscle"". That was diagnosis from ER
Administration Employee was Inspecting an area where reclaim of equipment is to take place. Stepped in hole and fractured RT fibula. Employee returned to work with restrictions.
EE was discing piles in a disc tractor and they were chunky from the frozen weather on 1-28-19 and 1-29-19. Two days later EE woke up in the middle of the night complaining of back pain and thinks it is from bouncing in the tractor while discing piles.
Employee was walking down stairs, tripped and fell down the last three landing on posterior and dislocating left shoulder.
Employee was thawing out bridged material above BC-107 and a large piece broke off. Employee pulled the torch back that EE was running and in doing so twisted shoulder. Required no medical attention at the time. 8 months after the incident EE was having issues with the shoulder and went to the doctor for an examination.
The employee was in the process of installing roof bolts in the #6 heading. After drilling with a piece of starter steel, EE lowered the drill pot. The starter steel was stuck in the mine roof. EE raised the drill pot back up to connect to the steel. EE's right middle finger was crushed between the drill pot and the piece of steel in the mine roof.
Employee tripped on bottom step and landed on floor grating. EE scraped EE's knee. It continued to hurt and EE was placed on restrictions on 1/29/19.
Flipping piece of screen rail over to knock pins out. Rail fell causing hand to be pinched took glove off saw bloody finger and call for supervisor.
EE was driving into work, when EE was pulling into the office parking lot at the Plant when EE's phone fell off the magnet holder in the air vent causing the phone to go between the floor and the accelerator pedal. When EE went to pick it up the vehicle veered into a rock blocking the electrical panel causing the vehicle to stop immediately.
Employee slipped and fell on ice at blast site. Did not seek immediate medical attention. Discomfort in left shoulder persisted. Diagnostic screening did not result in restrictions until 3/6/19. Originally reported to MSHA on 3/6/19. Report not found on-line. Resubmitting 8/15/19.
EE was clearing frozen material and belt spun frozen material into EE hand
Employee was walking down a catwalk near the primary dump point and fell down a single 8"" step near the operator booth, dislocating EE's right shoulder.
Employee was carrying chain fall in their arms and over their shoulder. Due to unsafe ice conditions on the walking surface of the normal route, employee had to step over pipe and caught their foot in the chain, falling onto their arm and resulting in a compound fracture of the left radius.
Working installing bolts in 38"" high section, twisted knee and felt a pop and pain resulting in a torn meniscus, worked until surgery on 3-6-19.
EEs were lowering jaw die into chute for wear purposes when the die got hung on the walls. They cut one side of the die off, the die fell, and the chain hit the EE in the right knee. The EE has a fractured patella on the right leg, causing the injury to be reportable.
Employee felt a pull in stomach while lifting an empty CO2 gas cylinder over a 1-inch lip on welding cart. The employee reported that in June 2018, had a non-work related herniated belly button repaired and thought may have damaged the repair.
Employee was driving a cart, became distracted and drove into a hole striking a pipe. Employee was taken for medical care at Gulf Coast and (R) elbow x-ray was negative. First visit to Dr. on 2-6-2019 and EE was returned to full duty. 2nd visit was on 2-20-2019 and MRI was ordered and EE was placed on 0-10 lbs. upper extremity restrictions.
Employee stood up after working on short head crusher. Started walking around the catwalk carrying a wrench in right hand. Right ankle gave out and fell to catwalk causing sprain to right ankle. Walkway was dry and clear of obstruction. There were no slip/trip hazards.
On 17 Jan employee was grinding a piece of metal, a particle went into right eye. EE was wearing safety glasses & face shield. On 18 Jan EE went to ER at 2100 hrs, diagnosed with viral conjunctivitis. On 21 Jan EE went to an eye doctor, as eye was still irritated. Second doctor identified a foreign metal body in right eye. Operator was not notified of injury until 23 Jan 2019.
Line electricians were working out of a bucket truck basket when one of them attempted to move a jib. The employee felt a pop immediately followed by a lot of pain in shoulder.
Burnt right index finger while cleaning slag off torch.
While opening the hood on a rental compressor to fuel it the ee felt pain in left arm. Inspection of compressor hood shows it to be in good working order. EE was lifting it up normal without any strain while lifting the hood. EE diagnosed with bicep tendinitis.
At 1/16/2019 1:00 pm. Employee right knee was hurting while crawling. Went to the doctor on 1/24/2019 and found out it was a meniscus tear. Employee worked until 2/15/2019. The day of surgery.
Employees were inside the coal mill replacing the rubber sleeve. The employee's left leg was standing on a lip and right leg was kneeling on the roll in order to repair the rubber sleeve. When the employee lifted the sleeve, EE's left knee ""popped"". Employee diagnosed with a lateral collateral ligament sprain and had surgery on June 13, 2019.
Employee was shoveling frozen mud from the track frame of a D-10 dozer when EE felt a strain in right shoulder. Employee went to the doctor for observation two days later. EE was referred to an orthopedic surgeon. This injury went LTA on March 14, 2019.
Employee was cleaning chute and fell from 4th step of ladder causing fracture to left leg

Employee was loosening a bolt on an excavator during radio installation. EE was using a breaker bar, and when the bolt loosened, they felt a pop in the shoulder. EE was taken to the clinic, given restrictions, and released to work. Currently ongoing. MRI pending.
Employee was attempting to clear ice and snow from the containment area. While descending into the containment, employee's footing slipped on the last (3rd) step and employee fell. Employee's left ankle was fractured as a result of the fall.
Employee felt knee pop as employee was lifting 50 pound bags of ANFO.
Due to engine failure, employee thought haul truck was on fire, and in an attempt set off fire suppression system, injured/bruised wrist. EE was taken to ER and given a full release (Fri., start of days off). On Monday, morning of next shift, contacted company, complained of ongoing pain. Taken for further evaluation, given restrictions, pending MRI, currently ongoing.
Employee parked truck in blind spot of loader operator to get lunch money. Loader backed into the truck with employee in it and it jarred employee's neck.
Employee was working inside an open hopper when rock was dumped into the hopper. This caused rock to contact employee's legs resulting in bruising and hyper extension of the right ankle.
Employee trouble shooting pinched electrical cable for an actuator on the boom of boat tower. Stood up from kneeling position and ""tweaked back"".
Employee was helping to assemble a conveyor in the field when employee experienced pain in left arm.
Injured was working at our Sister Mine on 1/10/2019. EE tripped while working on a battery coal hauler and bumped knee. EE seen a Dr. on 3/18/2019 and was placed on restricted duty due to a knee contusion.
EE stepped off a platform and twisted right knee resulting in a knee strain. 1/15/19 EE was seen by doctor for observation 1/28/19 EE received physical therapy which resulted in the reportable injury.
Roof Bolt Operator smashed right ring finger between bolt and mast. Evaluated by doctor; required stitches. Released back to next scheduled shift with restrictions.
Injured was stepping out mini trac and twisted EE's ankle. Injured was placed on restrictions and was given a prescription for pain.
Tightening up a hose that was leaking, standing between tire and step fender on loader #260068 foot slipped, fell backwards onto blacktop approx. 4 feet, injuring center left side of back.
The employee and 2 others were replacing a 1.5"" ball valve. Once completed, the valve was energized & the employee tried to open the valve. The entire pipe assembly started rotating. The valve was isolated & the employee tried to tighten the assembly by hand. The valve was re-energized & the employee was struck by the assembly rotating off the header pipe extension.
On Monday, January 7, 2019 a miner was injured while operating an articulating truck on surface mine property. The miner was driving the articulating truck down a hill when it struck a berm. The associate was treated at a local hospital for injuries incurred in the accident. Minor damage was sustained to the articulating truck.
Employee slipped and fell back down when walking down the stairway of the HT592. The employee's back was hurt as a result.
Employee was in the process of emptying a trash can in the shop. EE strained stomach as EE attempted to lift the trash can.
Miner was maneuvering wooden blocks away from a dozer receiving service, using a hand truck. EE's angle for moving the wooden blocks required EE to duck under some equipment and as EE did so, EE felt pain in EE's lower back.
Employee was sitting in wheel axel housing hooking up electrical wires and hydraulic lines. When the employee stood up employee felt left knee lock up. Received notice 4/18/2019 that the knee is an industrial injury.
Employee was driving a bolt with an impact wrench. Brake clean solvent was in the hole and when compressed, combusted and blew the bolt out of the hole. This drove the impact upwards. The employees left hand was on top of the gun. EE's left index finger was mashed in between the gun and the equipment breaking it, requiring surgery.
Employee received a back strain while riding in a shift change bus.
EE was shoveling under conveyor and a small rock hit EE's back
While maneuvering an electrical component employee felt discomfort in EE's back, continued to work the remainder of the shift. Diagnosed with back strain on 1/4 restricted from work until 1/16.
While moving a shovel bucket pin, the pin inadvertently contacted the IE's left leg causing a fracture to the fibula.
Employee felt pain in their shoulder when they slipped and fell while stepping across a ditch on 1/2/2019. Employee felt reoccurring pain and on 1/27/2021 it was determined that the employee had a rotator cuff tear consistent with the 1/2/2019 incident.
Employee was operating bulk truck which caught fire. Employee jumped from the truck to the ground.
A Lab Tech, was exiting truck at the Step 2 Agglomerator at line 5 and slipped on ice striking left elbow and felt pain in left shoulder. After telemedicine exam, ee returned to work without restrictions. Incident became a recordable on 2/20/19 when we were notified the employee needed Surgery
Employee was using the stairs to the 5/6/7 Kiln control room. Employee struck hand against the handrail leading onto the stairs. They felt a pop and a burning sensation in the left hand / wrist area. It started swelling over the next few minutes, so we wrapped the hand into a ace bandage.
An employee stepped down from the boxcar loading platform, to get into position to close the boxcar door. When ee went to step, left foot landed on a tool, twisting ankle in the process.
The EE was helping put the conveyor chain of a C/M together. EE was hitting a master link pin with a small sledge. When EE hit the pin a small splinter of metal hit EE in the right pinky finger.
Ee was replacing hoist cylinders on a CAT 777. The cylinder was hooked to a crane. Slack in the crane attachment allowed the cylinder to move, crushing the ee's right hand between the hoist cylinder and the fuel tank. The ee was diagnosed with a fracture to the right ring finger.
IP was in the process of closing the service cage at the 4450 station. In that process, IP's left ring finger came into contact between the handle and latch. The injury resulted in a laceration which required sutures to close. IP is currently on restrictions.
The Injured Employee was opening a rail hopper car door with a bar when the bar slipped causing the IE to fall to the ground landing on right wrist.
Driving down to clean railroad tracks. On way back up to main road passing rail car hit frozen piled up material and hit head on plastic heater column. Seat belt held me down head and neck hit heater column.
Fieldman Leaderman was stepping up onto the back of a float truck and felt knee pop. Injury was not to be determined to be reportable until 4/30/2019 when employee was prescribed Physical Therapy.
Employee was riding in the back of a tractor. The tractor hit a hole and the employee bounced up and back down striking the bottom of their foot on a hose that was on the floor. Their foot flexed up against the metatarsal boot causing a fracture.
An employee squatted down to remove a valve cap on a slurry car. When EE stood up, EE felt a ""pop"" in EE's right knee and it started to hurt.
Employee opened the valve of our air lance. Force of air hyperextended left arm back resulting in left shoulder strain.
EE was working on clean up in the Primary Tunnel. The material was compacted so a chipping gun was needed to break up the material. During operation of the chipping gun, the employee felt a pop in wrist. EE was taken to the clinic; ee was re-evaluated on 01/29/19 and was issued restrictions.
Doing a walk-around at beginning of shift and slipped on the ice, landing on elbow and shoulder. Eventually received an MRI and Doctor issued restrictions on 5/2/19. Employee currently on Restrictive Duty.
Injured Employee (IE) was working on an air slide in the mill building. IE was breaking for lunch and walked away from the worksite, but returned to drop off knee pads. After dropping them off, IE turned, took a step, tripped on air slide screen, and fell onto left shoulder. IE sought medical treatment, was diagnosed with a shoulder sprain, and placed on restricted duty.
Operator fell while dismounting equipment and fractured right ankle.
EE scoring wiper with utility knife held in right hand and cut index finger on the left hand with the utility knife. Evaluated by site responder and then taken to local hospital where EE received medical treatment.
While operator of HTK280 was being loaded at SHV005, the operator felt the haul truck go up and down and felt pain in the lower back.
While a Production Utility Operator was lancing at the finish mill, EE stopped what EE was doing to talk to another employee and lifted the face shield. When EE lifted the shield there was cement on the brim of the shield and some dust got into EE's eye. EE was wearing all appropriate PPE, safety glasses, face shield etc.
Injured twisted knee on 1/26/19 and continued to have discomfort. On 2/5/19 had an MRI of the knee and was discovered the employee had suffered a meniscus tear to the knee.
Employee was going to go clean up around wash plant, when EE stepped in an area of mud and EE's foot sank about 6-8 inches in the mud, twisting the ankle.
Employee was changing out the claw tooth of a scaler when the tooth fell onto employee's 4th digit of left hand causing immediate pain and swelling.
EE lifting and pulling on a pump and felt pain in low back area. EE was placed on restricted duty on 2.6.19 and remains on restricted duty at this time.
Employee was raking under a conveyor belt and felt pain in their lower back but refused treatment at the time. Employee later reported back injury on 02/04/2019.
EE opening a gate, right thumb smashed between the gate and metal brace fracturing right thumb. EE will require surgical procedure to repair fracture and will be able to remain on full duty after surgery due to scope of job requirements.
Employee tripped while walking to assigned work area. EE averted falling by grabbing a steel gusset with the left hand. In doing so, EE dislocated left thumb.
Miner was working on bearing housing when they felt a pop in the lower back.
EE was emptying contents of 55 gallon trash drum into the bobcat bucket, EE's foot hit a piece of steel liner material that was leaning up against a wooden cabinet. This caused the piece of steel to fall forward, striking the employee on the left side of the head/ear, knocking the hard hat off of head.
EW employee was removing a cathode within the bandsaw machine. As employee ran their hand along the edge of the cathode it sliced their finger through the glove they were wearing.
Lifting a piece of steel and something tweaked in lower back.
While tack welding on rock knockers on a haul truck bed in the overhead position, a hot chunk of molten slag fell from the weld puddle and landed on employee's upper right thigh causing a burn.
Employee reported sharp left shoulder pain, while using a tool to clean a locked out conveyor belt. Doctor prescribed arm sling till followup medical visit.
At the end of shift, while leaving the dry to exit the mine, the employee slipped and fell due to the snow that had accumulated on the stairs. The employee landed on the stairs contacting rib cage, resulting in several fractured ribs.
540 HT was loading at the 12 shovel. As the operator was backing the haul truck bed scrapped against the dipper of the shovel. After the truck was loaded the operator drove to the 4560 tie down where they reported that their neck was hurting from the impact.
While carrying a 5 gallon bottle of water up the stairs, employee collapsed and slid back down the stairs.
Employee was walking down the stairs at the original re-grind. As the employee reached the bottom of the stairs they walked through a puddle of water and twisted their ankle on even ground.
Employee was riding in covered mantrip while wearing safety glasses and felt something in eye.
Injured employee was pulling conveyor belting with a cable and clamp to prepare a splice. The clamp came loose and struck the employee on the left heel.
IE and four other EE's were working to break the ice in the flex pipe lines on the hydrosizers. They had all became frozen solid, and we were removing the ends by undoing the clamping on the end, and knocking the end off of the fitting using a dead blow sledge hammer.
Driver of the 525HT backed into the right side of the shovel to get loaded. The shovel operator honked for the driver to stop. The haul truck operator felt what they believe to be the dipper door drop open and hit the haul truck bed followed by the material being dropped into the bed. This caused a rough load to the driver of the haul truck.
Miner was operating a hand powered winch in order to lift a pump out of a sump. when the injury occurred the miner was using EE's left arm to operate the handle.
Employee was removing scales with a chipping hammer when a piece of debris was knocked off and punctured the glove on employee's left hand. When the glove was removed there was a fine point on left thumb.
Was helping to pick up ne plates from the gradall straight across to the metal belt. When employee was putting them on the belt it slipped out of hand and fell on their other hand cutting the palm of their hand right under the fingers.
Employee was using an Oxytherm torch to cut a section of pipe. Unbeknownst to employee, the trigger was leaking oxygen, saturating the glove. EE's glove caught fire, and employee received 2nd degree burn to the right hand and finger. Employee seen by Dr., placed on restricted duty (one handed) until hand heals.
Lifting a turbo using an A frame hoist, while picking up the shut felt a pain in EE's arm.
Mud fell from dove tail of haul landing between employee's safety glasses and eyes causing pain in left eye
Employee was removing material from base of a pipe which was cut to be removed with a bin it was attached to. The weld broke at the top causing it to fall, hitting them on the shoulder. The employee was taken to be medically evaluated which stated that they had a contusion to the right shoulder and was able to return to work the next day with lifting restrictions for a week.
The injured employee was helping to install electrical cable through conduit when a spool of cable unexpectedly shifted and fell off the stand striking lower left leg injuring knee, calf and ankle. On 6/11/19 the employee was given work restrictions which limited their work capabilities.
Employee had out patient surgery on right knee to remove a small fragment of chipped bone. Employee on 1/16/19 reported knee discomfort when employee went to start up a power washer that had a hand pull power cord. On 3rd pull, felt knee discomfort.
Employee was trimming the rail rubber with a utility knife so the wire mash screen could clear and go underneath the lip. The knife slipped and cut their left hand.
On 1/15/19 employee was taken to the company clinic and received stitches for a bursting laceration. Employee's left hand, while performing clean-up duties, was caught between a metal spray bar and the side wear plate of the loader bucket, resulting in a bursting laceration to the left middle finger.
Employee was helping a fellow employee pull a wheeled hopper out of a trash chute when EE felt EE's back tightened up. EE reported the incident the next day, January, 16.
The employee was walking in the coffee break area when the employee tripped on a rug. The employee fell back into the break area ice machine.
Employee cut own thumb with a knife while repairing an oxy/acetylene hose.
EE was assisting mechanic work on D11 dozer. EE stepped off the push arm and was walking away from the dozer when they stepped in a hole and rolled an ankle. EE finished shift but sought medical treatment the following day. EE was diagnosed with an ankle sprain and placed on light duty.
Employee was lowering a raised step assembly and dropped steps. Hand railing hit employee on side of head. Employee received laceration on forehead.
Employee entered the Packaging Rainbow doorway and felt a pop in the right knee. EE stated they did not feel any pain at the time. They continued walking about 50-feet and stopped at a desk to get something to drink. When leaving the desk they turned and felt a pop again and experienced pain in the right knee.
Employee was cleaning belts and catwalks from the recent snow event. EE slipped on catwalk while cleaning, caught self on the hand rail. EE felt pain under shoulder blade. EE suffered chest wall strain.
Maintenance Supervisor twisted leg and fell while removing snow from sidewalks around office, worked the rest of the day, then taken to hospital due to not being able to put all EE's weight on it the next day, told it was a sprain and put on restricted duties.
Employee was operating a forklift on uneven ground causing the seat to bounce which resulted in a sore back that was reported the next day.
Samplers were up on phase 14 drill patterns taking samples of drill cuttings when a handheld drill hit rock chips causing the drill to torque and sprain the employee's lower arm.
While washing down the CV13 drain, a pry bar fell over onto right foot in the metatarsal area causing a bruise.
Truck shop employee was working on haul truck 551 right side front tie rod. When raising the rod into place, the tie rod slipped off the jack and caught EE's left hand between the tie rod and jack. Employee received 13 stiches.
Operator of the 632 HT just finished dumping at the Lone Star dump. As the 632 HT was traveling to next assignment the truck hit a pot hole and caused the bed to raise and slam back down jarring the cab. Operator then drove to a nearby tie down and reported injury to lower back and damage to the suspension.
The employee was carrying a large piece of metal. While walking towards the work area, the employee's hand bumped a nearby wall. The impact jarred the piece of metal from their grip causing the metal to fall and impact the top of their right boot. The employee reported the accident promptly, but did not seek medical attention until 1-28-19.
Employee was in a kneeling position and when they got up they stumbled off of a muck pile and came down on their left leg hurting their left knee.
Team-lifting a timing gear at the 233530 rotary valve for installation on the rotor. EE's fingers slipped off of the gear, and EE felt a strain in the L wrist and palm causing sharp pain throughout EE's arm from the finger tips to the elbow.
Employee was operating the 530 Loader when EE dismounted the loader to check the left front tire. As EE was dismounting the hydraulic ladder EE lost EE's footing on the last step and hit EE's left heel hard on the ground. Was not a restricted until 1/16/19.
Employee was climbing in drill when foot slipped off ladder and pulled shoulder out of socket.
Employee climbed in loader to load a truck. As EE was climbing down EE placed EE's right foot on the icy ground, let go with hands and right foot slipped from underneath EE. EE fell on EE's right side tried to catch self with right hand. EE broke EE's right wrist, a compound fracture of the ulna and radius.
Employee was using a small tractor to clean under a stationary screening unit. This particular instance the tractor operator went in too far under the screen striking left wrist, that was positioned on the steering wheel with the steel frame of the screening unit. EE then notified Crusher Superintendent who took EE to a hospital for treatment.
The employee stated they stepped forward off a platform with their right foot onto a 4-in drop onto a lower platform, felt their weight shift, and reported pain in their back & hips on January 11. On January 14, OccMed placed the employee on limitations and prescribed 800mg of ibuprofen.
Vacuum truck hose broke and employees arm was sucked in while grabbing to retrieve. Arm was under suction for approximately 10-15 seconds.
While pull starting a chainsaw the miner felt a pop in the left wrist. X-Rays were taken and came back as negative but the miner was placed on restricted duty by the doctor to rest the wrist.
Employee was cleaning out from under a catwalk and was struck on right shin by an attachment from the skid-steer that was assisting in the cleanup process, and received a contusion to the shin.
Employee was working from a 60 foot Telescopic Handler, unloading a piece 36inch by 80 inch 2x4 bar grating. As ee was sliding the bar grating onto the preheater platform, ee felt a pop in right shoulder.
Emp was reinstalling a guard. Emp caught R index finger between the guard and the frame, causing a laceration. Emp was taken in for treatment and received 4 sutures
An employee got out of loader to check on the plant, took four long steps up the ramp and heard a pop in leg.
Employee was setting a steel column and pulling up on a sleever bar when suddenly EE had pain in lower back. The employee did not report the incident till 1/14/19.
Employee was completing a pre-operation check of a skid loader when EE stepped off of the skid loader track into a hole and twisted EE's ankle.
A mechanic's hand was injured by a fan sheave and pinched by the V-belt mechanism resulting in a laceration and a fracture of the hand.
The employee was observing trainee from a 10"" high platform. When the employee stepped off the platform employee came down on right foot and it rolled causing popping noises and immediately swelled up.
EE was opening an air door on a motivator and it ""sprung"" open unexpectedly, striking the EE on the left shoulder, knocking them to the ground. Initial xrays did not show dislocation, but subsequent follow-up found the shoulder was dislocated. The EE is released to restricted duty.
Miner was using mine phone when they were struck in the back by a sheave block which broke free from the bolt/utility plate it was connected to. Miner received soft tissue injuries only.
Slipped and lost balance causing employee to stumble backwards and twist ankle.
Employee was hooking a wash hose, to wash out the tunnel. Upon turning on the water supply valve only a little water came out and then it stopped. While employee was reaching to turn off the water and the hose blew out below the valve, resulting in skin abrasion and local swelling to the right wrist. The hose that was being used was a 2 inch lay flat PVC hose.
Miner was using a box cutter to open the other packaging of some press bags for the wash plant when the cutter slipped and cut EE's right pointer finger. This resulted in the miner receiving medical treatment in the form of a Tetanus shot and stitches. Miner returned to work the next day on restricted duty.
Employee was performing maintenance on an exploration drill rig when employee was struck in the nose by a wrench
EE was walking down an access ramp in the pit to see the best way to pull pipe down the ramp. EE stepped on uneven ground and lost footing. EE began to fall and knees and struck a sharp rock. The contact caused a laceration to right knee. EE also sustained a contusion to left shin. EE was taken to the clinic and ee received stitches for laceration.
Employee was walking from the underground restroom back to the crusher cab when knee gave out. When ee fell ee caught self with left hand causing some swelling on this hand. After seeing a Dr.it was determined employee had suffered a fractured wrist.

Employee was replacing an inspection door after performing work when EE pinched/crushed right index finger between the approx. 10 lb. door and the housing.

Employee stated that ee was operating the #420 Fletcher Roof Bolt Machine on the Unit 2 Section. In the process of placing the drill steel into the drill pot a piece of rock fell from the mine roof and struck the injured employees hand. This caused one of the employee's fingers to be fractured.
Employee hyper extended left arm while moving a 2x4.
Employee was installing piping for aeration pads. Employee strained their back from pushing and pulling on pipes.
The injured employee stepped on a rock and their foot slipped. Employee twisted their left knee and was on restricted activity for one day.
Employee tripped on the edge of an energy isolation mat in front of an electrical panel causing employee to fall to the ground. Employee caught self with left hand and felt pain in the hand. Employee was examined at a clinic and diagnosed with a possible fracture in hand, given a brace and returned to work with restricted use of the hand.
Individual was cleaning on the tower with a water lance felt a pinch in their back then felt pain in their back.
As the contractor was turning around inside the cab of shuttle car, contractor scraped right pinky finger against a piece of sharp metal on a lid inside the cab causing a small laceration. (6) sutures were required to close the wound.
While employee was cutting blanks, employee jammed left ring finger on roller ball of the cutting machine. Incident didn't become reportable until physical therapy was prescribed in February.
A rib burst happened on the face and hit shearer operator. The piece of coal that hit them caused their safety glasses to cut their face.
While placing pin in a scoop bucket jack, injured was lining pin up by placing finger in the hole. The jack fell, striking the injured employees finger resulting in a fracture and avulsion of the right index finger.
IE WAS ADJUSTING THE SEAT ON THE HITACHI SHOVEL PINCHING FINGER BETWEEN THE SEAT FRAME AND SLIDE.
REPORTABLE ONLY DUE TO SUTURES - Employee was helping to load blocks in a scoop, when EE saw a piece of wood in the way EE went to toss it out of the way and that piece of wood came back down on EE's left hand finger causing fracture to the tip and required sutures.
Employee was clearing a chunk of slag out from the #3 product feeder with a pneumatic jack hammer. When the chunk split in half and fell, it pushed the jack hammer bit down while pivoting off of the side of the feeder and the body/handle end of the jack hammer pushed upwards. The employee's right index finger was pinched between the jack hammer handle and an overhead water line.
EE was sweeping the floor near a conveyor. As ee turned around to leave the area, ee struck right shoulder against the edge of a catwalk. EE lost balance, and fell forward but caught self with right hand on the conveyor structure but ee spun clockwise and left hand/wrist passed between the belt and the troughing roller. The EE received two stiches to hand.
EE was stripping cable with a utility knife. The knife slipped cutting the right thumb. EE seen at clinic, stitches required to close wound, EE released to full duty.
Employee used hammer to free impeller motor from a vertical pump. Hitting the impeller, a piece of metal broke off & entered employee's hand. Employee treated onsite by contract medical. On 2.25.19, employee notified employer that wound not healed correctly. Employee referred to offsite medical facility where a metal fragment was found embedded under the skin.
Employee was using a hand grinder to clean up bore holes for pin fitting, when a piece of debris went in behind safety glasses and entered right eye. Employee was examined and no debris was found in eye. Employee did receive prescription eye drops as treatment.
Employee was pulling downward on drill steel during roof bolting operations when they felt a strain in their shoulder.
EE was performing maintenance work on aerial lift. EE was working in a tight area underneath vehicle using a hammer to tighten hydraulic hoses. Hammer slipped & slid about 18"" down the hose, hitting EE in mouth. EE reported damage to crown on tooth. EE was sent to dentist for evaluation & damage to tooth reported. Employer did not receive knowledge of damage to tooth until 3/5/19.
Employee was cutting a band that was securing a bundle of rods for the rod mill. When EE cut the band, the band struck EE on the left forearm causing a laceration which required 6 stitches.
While hanging bolter cable, a piece of shale dislodged and hit the employee
Employee was using a vac truck to clean the cone of a cement silo when material came out. Cement entered the employee's right eye. The employee was taken to the eye doctor and was prescribed eye drops.
Injured employee was assisting an employee in removing the engine from a haul truck. They were removing the spreader bar when the shims shifted, the injured employee then stuck left index finger in a bolt hole to prevent them from falling to the floor. As the last bolt was removed the spreader bar and shim pack shifted and pinched the IE's finger between the shims and frame
Worker was removing insulation on an electrical splice when the razor knife worker was using broke causing worker to cut left hand. The laceration required 9 sutures.
Another employee was using a hammer during a belt set up, the hammer slipped from EE's hand and struck another employee on the right side of the face (cheek). The injured employee sustained a small cut that required sutures.
Employee was grinding on a flange on the #4 rotary kiln ring gear. Some dust got into EE's right eye. EE rubbed the eye probably imbedding the material from the dust. Later that day EE's eye became irritated.
Employee attempted to move a dolly draw bar when EE lost control of grip and got left middle finger caught between the mine bottom and the draw bar. The injury resulted in a laceration and a fracture to the left middle finger. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency was not a factor.
A driller was manual handling the tube assembly when the tube unexpectedly stopped. The force that they were applying caused the spear head to lacerate the driller's hand. This resulted in six stitches. The employee received no work restrictions (released to full duty).
Employee was reclaiming belt structure. As EE was putting stands on a slide, EE's left index finger got caught between the stand and the slide fracturing the tip of the finger.
Employee was gouging in a mill. Wearing required PPE. After gouging employee felt an irritation in left eye. Employee flushed eye. The irritation went away. Employee returned to work with no concern. The irritation returned and employee talked to supervisor. Suprvisor took to DR. Dr prescribed ointment and eyedrops.
Ee was driving the haul truck and hit a deep pothole. EE struck head on the window of the haul truck. EE was transported to the ER with neck pain. Diagnosed with a cervical strain and put on restricted duty for one day.
PLACED HAND ON THE EDGE OF TOOL BOX TO PUT UP TOOLS AND THE LID CAME DOWN SMASHING LEFT THUMB.
Employee was operating loaded haul truck on the Pondarosa ramp en route to the Lone Star 5220 H dump when the fire suppression system chirped. Employee noted smoke and flames coming from exhaust pipes. Employee discharged fire suppression and while exiting the unit they inhaled smoke and fire suppression agent.
During removal of an old B bonded stopping EE was attempting to recover the mandoor, EE's hand was overtop the sharp edge of the mandoor. An overhanging block free fell hitting EE's hand, forcing right wrist to come into contact with the sharp edge of the mandoor resulting in an avulsion type wound. Sutures were required to close the wound.
Employee had EE's left thumb struck by a spring-loaded gate while descending a ladder in the Plant # 2 processing plant. EE was examined and diagnosed with a contusion and was prescribed an anti-inflammatory. Employee inattention and failing to SLAM contributed to the incident.
Employee was hauling coal to the preparation plant on the haul road when they drove the truck into a ditch.
Mechanic was replacing the leaf springs on a fuel truck when the leaf spring moved striking them near the eye.
IP was conducting a post blast inspection in the 4200-212. IP was standing on top of the muckpile checking for misfires. As IP was walking down the muckpile, the IP fell forward resulting in laceration to the left wrist. One suture was needed to close the wound.
Employee was on a catwalk there was a slick spot of snow and ice, employee slipped fell and hit nose and lip on the rounded toe board requiring 4 sutures to close
Removing a bolter wrench chuck by striking it on a metal surface, missed catching finger between the drill steel and metal surface, requiring stitches.
Employee was in the underground shop area picking up rockdust bags to throw in dumpster when EE struck right ear on a sprinkler head on the fire suppression system. This resulted in a laceration to the right ear causing sutures.
Two EEs were taking a fuel tank off a pavement roller. There was pressure in the pump that sprayed when the line was taken off. Both EEs were wearing safety glasses. The EE did not notice irritation until EE woke up the following day (Saturday). Sent EE to be checked Monday morning. Urgent care gave EE prescription eye drops. No lost work time.
Removing sack stand that was wrapped in chains from hand truck the load shifted and thumb got caught between chain & stand.
Ee's haul truck was being loaded. A large boulder was placed in the bed of the truck, causing the ee to be jostled in the cab. EE reported to EE's supervisor, was seen by the EMTs and refused treatment. On February 18 EE sought treatment with a chiropractor and filed a WC claim. Investigation completed and claim was accepted 03/28/19. Reporting for medical treatment.
Employee was pouring concrete when material fell and struck employee on hard hat, shoulder area resulting in laceration on lower head area.
Employee was installing a roof bolt when EE bent the bolt. As EE tried to straighten the bolt EE inadvertently hit the wrong lever and pinched right hand between the roof bolt plate and the mast of the miner that caused a laceration to EE's right hand. The wound required sutures.
While lifting a battery out of a pickup to install in a haul truck, worker strained right shoulder.
An employee was moving a large rock from the oversized belt. The rock rolled, pinning the employees left pinky finger against the side of the screen.
Employee felt flash burn under hood during welding; employee came back to work the next work day. (Misreported as first aid case, MSHA instructed operator to re-submit 7000-1 for this case).
An electrician suffered smoke inhalation while trying to determine the source and extent of a fire. The EE was administered oxygen at the scene and received diagnostic tests, a saline IV, and an albuterol inhaler at the hospital, where EE was observed for approx. 3 hours and released with no restrictions.
MINER WAS PARKING TRUCK AT THE END OF THE OF SHIFT WHEN THEY NOTICED THAT THE MIRROR WAS NOT IN A GOOD LOCATION FOR THEM TO SEE. SO THE MINER GOT OUT OF THE TRUCK TO ADJUST THE MIRROR WHEN THEY WENT TO ADJUST THE MIRROR THE MIRROR BROKE AND THEY FELL TO THE GROUND.
Employee was pulling cable off of the reel of the bolt machine with a yellow rope hanger when it came loose, striking the employee under left eye requiring three (3) stitches.
Employee was using a 2ft. pry bar to raise a tube into position to be welded. The bar slipped causing EE to pull the bar towards self, striking the safety glasses, which then rolled into EE's eye. The employee was prescribed prescription eye drops which resulted in the incident being a medical reportable injury.
Employee was lifting a dumbbell over their head exercising when they passed out and the dumbbell struck them in the head causing a laceration.
EE was holding a new knee pad attempting to cut part of the knee pad off. EE was sitting down during this process and the knife slipped which resulted in the EE cutting self causing a laceration which required sutures to close the wound.
At 08:45am an MTE was breaking down cable for the 49 Drill. While pushing down the handle on the Main Disconnect Switch, the handle sprung up and hit the employee in the chin and mouth. The injured MTE successfully opened the switch, returned to the Mine Office, where the supervisors were notified at approximately 9:00.
Individual cutting strap on a hammer pouch and cut palm of left hand requiring 8 stitches. No LTA necessary.
Missed step tried to catch self and pulled EE's leg.
Employee was carrying roller and slipped and fell injuring finger.
Employee was laying on their back replacing a fuel filter on a pick up truck. They loosed the line and gas spilled out onto their face. A small amount of gas got under their glasses and into the left eye. The eye was flushed & washed, they suffered no damage to the eye. Employee was prescribed medication that makes this event reportable.
Employee was pushing 2"" plastic pipe thru a stopping when the pipe sprung back and struck the employee in the lip causing a laceration that required sutures.
Employee was using a wrench to remove a swivel fitting. The head of the wrench broke and the wrench came back and hit the employee in the face, cutting EE's nose. EE received 5 stitches.
Employee was installing a roof bolt off the miner side bolter when the collar of the bolt caught on the roof strap causing the bolt to bend and smash the employee's right ring finger against the bolter mast causing a fracture and laceration.
MINER WAS CLEANING BUILD UP OF FROZEN MATERIAL IN COAL HOPPER. EE WAS ABLE TO LOOSEN MATERIAL, BUT NOT TURN MATERIAL. EE REACHED INTO HOPPER AND TURNED MATERIAL. MATERIAL FELL AND STRUCK MINER IN R HAND BREAKING TWO FINGERS.
The employee was troubleshooting an electrical problem on the continuous miner in 1st North Mains. EE had opened up a hinged protective cover. Another employee inadvertently bumped the hinged cover and it fell smashing the injured employees right middle finger resulting in a laceration which required 11 stitches.
EE was bending a piece of steel which was inserted into a vice. During the process of bending the steel, it slipped out of the vice and snapped back into EE's face.
Operator was opening draw off valve at TK -34 where their left thumb was caught between the beater and maul. X-Ray was negative on day of incident. On 1/31/2019 after swelling went down they visited a hand specialist and was placed in a splint for 3 weeks.
Mechanic was removing the retaining bolt from the hoist cylinder pin on 522 when the wrench slipped and struck the mechanic in the face which broke a tooth and cut lip.
Employee was drilling into concrete. The drill bound up and spun and hit them in the cheek.
EE was reclaiming belt and placing sections into a high voltage tub. While walking beside the tub, it slid beside the right foot tearing the metatarsal boot. This caused a laceration to the side of the foot required stitches to close the wound.
An employee was in the process of taking down a Kennedy stopping. During this process, they picked up a panel and began to collapse it. The top section of the panel slide down and pinched the individuals left little finger against the bottom section. This resulted in a laceration which required 5 sutures to close. Employee was released back to work with no restrictions.
Employee was grinding on a finishing screen, eyes were irritated the next day, went to the doctor and had small pieces of steel in eye.
REPORTABLE ONLY DUE TO SUTURES - Employee was beside the feeder when the ram car entered the feeder to dump, causing bat wing to move and hit employees left lower arm resulting in laceration requiring sutures.
Employee was positioned back in a crosscut waiting for locomotives to pass. When the locomotive passed, it caught a power cable causing the cable to strike the employee on the left side of the face. The injury resulted in a fracture to the left orbital bone. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency was not a factor.
Employee was changing bits on an under cutter. EE's hand slipped down onto bit shard while changing bits. Employee had a laceration on left hand. Laceration required stitches.
The employee attempted to pick up a heavy load without using proper lifting techniques and aggravated an old injury.
Employee was walking to the mantrip at the end of shift when left ear caught the end of a roof mat causing a laceration. Employee received 4 sutures.
Technician was removing tire from a machine when a chain broke and caused laceration to left shin requiring four stitches
While roof bolting, the EE was trying to separate the top steel from the bottom steel. When the bottom steel broke free, it struck the employee just below the nose and above the upper lip. This caused a laceration that required three (3) stitches to close.
Employee was removing a toilet that had been installed incorrect. Toilet broke during removal. Employee received 8 stiches to pinky.
Employee was getting on mantrip when ee grabbed a hold of building there was a sharp piece of tin sticking out that caused a laceration to left hand. Employee received 6 stitches to close the laceration.
Employee was handling a distribution box cable when it slipped out of EE's hand and hit EE's shoulder causing EE to strain back. The employee continues to work but has started Physical Therapy on 2/11/2019.
Employee was handling a recently removed trougher roller to remove it from the area. As they began to tip it on to its side, the can rotated and a finger was pulled in between the cans resulting in a fracture to the left index finger.
The injured was placing up belt structure along the Grassy Mains belt and as they placed up a piece of top belt structure they slipped and lost balance and as they tried to catch themselves, their right hand went between the top belt and the top roller of the moving belt line.
REPORTABLE ONLY DUE TO SUTURES - Employee was preparing cable to re-enter on Shuttle Car, as employee was cutting away the outer jacket of the wire, when ee cut right index finger requiring sutures.
Employee was cutting performing maintenance. After viewing work area, employee attempted to re-lite torch and welding glove ignited burning right hand. Employee was seen at the emergency room and released to return back to work with no restrictions.
Employee was bolting top when bolt bent, mashing right hand against mast head and crushing right ring finger from the nail bed up. Employee was taken to the ER where ee required sutures and was referred for further treatment.
Injured employee was using an air lance to get material feeding. When material broke loose, it flooded out, knocking glasses off employee. Employee got dust in eyes and flushed them out thoroughly. Employee was taken to ER where eyes were flushed again and employee was given prescription eye drops as a precautionary measure. Return to work at full duty- Medical treatment only
Worker was pulling a scaffold pole from bottom of pile of 10 or so poles when a pole above the one EE pulled on slid down and pinched EE's left hand middle finger near the end.
Ee was grinding on hard nose of MG753 when the grinding disc came apart. A piece of the disc struck the ee on the upper left chest, causing a small wound. The ee was seen at the ER where stitches were required to close the wound. Ee released to full duty for next scheduled shift.
Employee was attempting to remove the packaging from surveying tape. Employee went to cut zip ties with pocket knife. The employee's momentum from the upward force cutting the zip tie. The knife slipped and cut the employee on the left pointer finger. The cut was deep enough that medical treatment was required. The employee received 5 sutures to close the wound.
An employee lost EE's grip while attempting to go up the steps of the CAT 980M Coal Loader. EE's right hand slipped off of the hand hold causing EE to fall backwards and break EE's left wrist.
Laceration to leg.
The employee was hooking up sling chains to the D10T track grouser pad on the right rear side. As EE went to hoist up on the chain sling and using right hand to adjust chain position EE's glove hung between chain and grouser mashing the right thumb.
Production Supervisor was unplugging STG IV using a jackhammer and towards the end of the shift EE started feeling a lower back pain. The extended use of the jackhammer and the access to the area contributed to this injury.
It was determined through a Part 50 Audit that this miner had an injury, that would have been reportable. We submit this in protest and to abate citation #9222851. Employee was using a hammer to hit the C/M conveyor chain, when a piece of metal struck employee on the upper right cheek.
Employee was cutting a nylon sling with a utility knife that slipped and contacted EE's left leg. This caused a 1/2"" laceration to EE's left upper leg which required 3 sutures.
An air reservoir tank was being installed when it was dropped approximately 36"" landing on the injured's right pinky finger causing several fractures. Treatment was given at a local hospital and referred to a surgeon for follow up. Surgery was performed on the broken finger on 1/15/19.
Employee was lifting a railroad tie to hold down a tarp covering a crusher parts, when letting it down it smacked a finger between the railroad tie and crusher.
Employee had completed pre-shift on service truck, turned on PTO and compressor to check fluids, when the hose came off the ball valve, hitting the side of EE's face, knocking off safety glasses and hardhat. EE was taken to the ER, diagnosed with corneal abrasion, and released to next scheduled shift on 1/19/19.
While picking up cut ends from mine timbers, a timber fell over striking the employee on the right hand. Employee continued to work and sought no medical attention until 01/25/2019 due to continued discomfort. Diagnosed with fracture of the 5th finger on the right hand.
