Accident type, without injuries

An unplanned roof fall was discovered in the No. 7 entry of the 5 North intake at crosscut 73. The fall is approximately 15' long, 8' high and 10' wide. We are in the process of bolting the area.

An unplanned roof fall was discovered in the old 6 North Mains #1 Entry between 8-M and 7-M. The fall was approximately 16' wide, 25' long and 6' high. The fall was likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

The Yazzie pit is an inactive part of the mine, last coal mined in this area was approximately 2010. There are currently no miners with exposure in this area. A supervisor passing in the area, noticed smoke coming from an outcropping and determined that it wasn't extinguishable in the 30 minute time frame.

An underground 2yd. loader in use, a hydraulic hose burst spraying fluid on the running engine causing fluid to ignite. Employee shut loader off, turned master switch off, hit fire suppression, left area, shut auxiliary fan off to work area, called mine dispatcher to report & stated could not confirm fire was out. Mine was evacuated, MSHA hotline was called to notify.

The Winifrede Slope hoist was removed from service due to a knot being found in the hoist cable. The cable was repaired and the hoist was returned to service.

AN UNPLANNED ROOF FALL WAS DISCOVERED INBY SPAD # 7073 IN THE #8 ENTRY RIGHT RETURN AIRCOURSE ON WEST SECTION. THE FALL MEASURED APPROX. 90'X20' AND FELL ABOVE ANCHOURAGE.

An employee was letting car warm-up after scheduled shift, when the car caught on fire and could not be put out by the employee. The local fire department was called to extinguish the fire.

The elevator at Patterson Creek portal was out of service due to the governor cable malfunctioning. The cable and sensor were cleaned and the elevator returned to normal service.

A Roof Fall occurred in the Main West #2 Entry, in the intersection of Crosscut #18. The fall is 20' Sq. with 1' to 5' of top falling. No-one was injured or entrapped. No Ventilation was interrupted or blocked. The Fall was Dangered-Off. Timbers &/or Roof Jacks were set to prevent access till the area can have new Permanent Roof Supports installed.

The drum on the hoist was out of sync with the hoist statistic screen. The hoist was re-programmed to the proper settings.

The air compressor used on hoist for pinion and accumulator is also used to aid in dumping stench. Valve was left open on stench manifold and compressor would not keep up creating air loss on the pinion brake, which faulted the hoist computer.

4900-132 Ramp - Mucker 2-11. Miner was operating LHD; mucking out heading when the LHD caught fire. The fire suppression system was activated and miner withdrew from heading and called the No. 3 Hoist. The mine was evacuated.

The Main Slope Hoist was kicking out due to an incorrect position on indicator. Adjustments were made and put back in normal operations at 10:30 p.m.

While cutting out a broken liner on the SAG2, a rubber lined ""pulp lifter"" began to smoke and caught fire. May Day was called, plant evacuated. The fire exceeded the 30 minute time frame, MSHA notified by phone. Fire put out by rotating the mill and spraying with water. No injuries, minimal damage.

An unintentional roof fall occurred at TG-4 xc 88. This roof fall occurred in the primary escape way #2 entry travel road.

An unplanned roof fall occurred in the intersection of cross-cut #7, entry #6, of the 3B7 Panel at an unknown time on November 14, 2018. The roof fall is approximately 11 feet high. Entries approaching the fall measure 18.4 feet, 17.5 feet, 17.9 feet and 17.6 feet wide.

5 South #3 escape facility hoist was inoperative for a time due to a faulty up over travel switch blowing a fuse. The fuse was replaced and problem fixed.

IGNITION - occurred on 9 section in the No. 1 entry while installing a rib pin on the third row of bolts. Ignition lasted 4 to 5 seconds and was extinguished with one fire extinguisher.

An unintentional roof fall occurred in 3 East Mains 23 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and the strata shifting.

Miner was trapped in an elevator, the elevators safety emergency stop activated stopping the elevator car between level 2 & 3. Called elevator company, elevator car was able to move up but not down. Miner was instructed to take elevator to the top floor and use ladder to come down.

At approximately 8:10, a fire was discovered in the solid waste land fill and was not extinguished within 30 minutes.

An unplanned roof fall was discovered on mainline 16 belt at the number 10 crosscut. The fall is approximately 20' long, 20' wide and 15' wide. The fall has been cleaned and bolted and will be steel set and back filled.

An unintentional roof fall occurred on HG-13 XC 35 primary escapeway, #2 entry travel road.

An unplanned roof fall occurred in the intersection of cross-cut #17, entry #7 of the 2nd Southeast Parallel at an unknown time on November 11, 2018. The roof fall is approximately 11 feet high. Entries approaching the fall measure 17.78 feet, 17.82 feet, 17.88 feet and 17.50 feet wide.

CO sensor at 50 ton coal bin quit working and unknown to the operator. The operator ran the coal bin empty in order to perform repairs at the drive sprocket for the screw. There was no ventilation in the system and the agitator was not running causing material accumulation at the bottom of the bin. Co monitor failed giving a false reading pre and post explosion. No injuries.

Loader operator, was stacking material on a stockpile, while traveling up a stock pile with a load, had to down shift, also pulled on the bucket control acidently engaged the auto lift control. Loader was at a slight angle to the right, operator right front tire drove into depresion, caused it to lay onto its right side, was wearing seatbelt, no injuries.

An unplanned roof fall occurred in #4 entry of Cumberland West at 30 xc. The fall encompassed the whole intersection and was above the anchorage point of the roof bolts. All approaches to the fall were supported with static support and dangered off.

Employees were using elevator to take tools to top of the 309 silo. Governor switch tripped in control panel and had to be reset by Otis elevator. The car was not able to move while switch was tripped.

A roof fall was found during weekly examinations on 11/8/18 at 7:30AM at three breaks inby survey stations #1603. The approximate size of the fall was 6-8 feet high 19 feet wide and 60 feet long.

A borehole was drilled beneath a pre-existing mine that were not shown on a certified map the previous rooms that were mine. The water pressure apparently eroded the borehole and water began entering the mine. All employees were removed from the mine and this was reported to the MSHA hotline for immediately reportable accidents. No injuries occurred from this accident.

In reference to citation 9161924. A roof fall was first discovered 11/8/2018, the fall area was on the 2 Left off 3 north panel. The fall occurred 16 brk between 5 1/2 and 7 1/2 entries. The fall is approximately 100' Long 18'-20' Wide.

An unplanned roof fall occurred in 4 North entry of the 70 HG section. The fall was 27' long and 12' wide and above the anchorage point of the roof bolts. The upper strata above the anchorage point was broken and soft above the bolt anchorage point. The area was bolted and cleaned up and static support was also set.

A mine examiner discovered a roof fall along the No. 6 Belt Conveyor at 15 Break between the No. 1 and No. 3 entries. The fall measured approximately 100 feet long x 20 feet wide x 7.5 feet high. There were no injuries and no equipment was involved. Passage of work crews was not affected.

An unplanned roof fall was discovered on mainline track 14 at the #34 crosscut. The fall is approximately 16' wide, 20' long and 10' high. The fall has been cleaned and re-supported.

A hot spot was found on the raw coal stock pile. There was no visible flame witnessed, and the hot spot area was rendered harmless.

On 11-2-18, at approximately 5:20 p.m. a roof fall occurred on the new 3 section set-up in the intersection of the #6 entry. Area was dangered off and both agencies were notified. No individuals were injured.

An unplanned roof fall was discovered at 16-25 crosscut between the track and belt entries. The fall is approximately 16' wide, 20' long and 10' High. No ventilation or travel was affected and the area has been dangered off.

At 7:55 am the surface operations lost complete power causing #1, #4 and #5 fan to shut down. All miners were evacuated out of #2 hoist.

Mechanic was looking over the hydraulic equipment on a service truck when something failed & struck the mechanic in head above right eye. Mechanic fell back onto the garage floor.

A miner was fatally injured when the miner was run over by the LHD loader the miner was operating underground.

Supervisor pick-up was struck by haul truck, resulting in a fatality.

EE was overhauling Mill #5. EEs were adjusting the flanged ring that sits on top of high side by hand to line up bolt holes when the flanged ring slipped off of high side crushing EE's left index & middle fingers. The end of the left index finger had to be amputated by doctors from 1st finger knuckle to the end (approximately 1"").

Employee was cutting left sump out of #7 head, cable pulled under the miner. Employee shut off miner and walked up to remove cable a piece of draw rock fell hitting EE on the outside of EE's left foot. Rock was approx. 3' long, 3' wide and 5"" thick. Employee was seen in the ED where x-rays revealed two broken metatarsal bones and EE received 7 stitches.

It was a clear and bright day about 60 degrees. EE, the mechanic was called to the Cat D9R Bulldozer. EE had to replace the track roller cap. EE used a 2 ton Hyd. jack to lift the roller up. After replacing the roller, the jack caused the impact gun to crush the thumb and index finger on left hand.

A fully trained miner had changed the blade on EE's knife and set it down. EE then turned away and when EE turned back the blade contacted EE's right forearm. The accident was not the result of any compliance issues nor were any protective guards involved.

Over-exertion (Not Elsewhere Classified)

Employee went to the Doctor on November 30 and was put on restricted duty on December 3. Employee was walking downhill when EE slipped, picked up speed and stopped hyperextending EE's right knee. Employee did not go to the doctor immediately.

Employee was working on #86 shield. The shields were pulling under automation. As the shearer passed, #88 shield pulled in automatically. The employee's left foot became caught under #88 shield pontoon causing injuries.

Over-exertion in lifting objects

Employee stated was hanging miner cable with the assistance of 4 others when EE re-aggravated a lower back injury. Employee did not work the following shift.

Struck by flying object

Employee was assisting to remove a piece of rock wedged between 2 wheels on truck. Removal of the rock cause the tire to explode causing small cuts to the face

Caught in, under or between a moving and a stationary object

Employee was operating a Porta Power hydraulic jack. The employee's right hand little finger got caught in the pinch point at the base of the jack handle.

Over-exertion in lifting objects

Employee reported the injury on 12/5/2018. Employee stated that shoveling and digging with a shovel made EE's left hand become swollen.

Struck against stationary object

Employee hit top of left kneecap on underside of 2nd rung of 5 ft. fixed ladder while climbing to retrieve a coal sample from Kiln 2 under normal operating conditions.

Fall to the walkway or working surface

Employee was traveling in snow conditions, the engine of the fuel truck ee was driving stopped running. After stopping the truck, the employee exited the truck to survey conditions when ee fell in slippery conditions and landed on the right shoulder. The employee continued to work until placed off, missing the first shift on Dec 6, 2018

Fall onto or against objects

After starting DOZ 128, EE stepped back out of the cab and missed the step. EE fell to the push arm and strained right knee. EE continued working until EE had surgery on 12-20-18.

Operator was changing steel on drill when wrench spun back and hit EE's foot.

Struck against stationary object

The injured employee exited the operator's compartment of a diesel locomotive and stepped onto a board lying on the mine floor, twisting EE's left knee. The employee was not paying attention to where EE was stepping/walking. No regulations were violated and employee was wearing required PPE. Employee is experienced in job.

Over-exertion in lifting objects

Employee was lifting rocks off of the mine floor and putting them on a conveyor belt and soon after felt a pain and numbness in the shoulder/arm. The belt line is typically around 42"" high. EE recently sought medical treatment and was diagnosed with a disc herniation and went off work on 12/28/2018 pending surgery.

Flash burns (electric)

Employee was kneeled down in front of the unit sub when the cat head arc and a flash blew back burning employees right hand & arm.

Struck by flying object

Employee was welding when a piece of slag went past safety glasses and got in left eye.

When employee stepped out of a tractor, EE stepped onto a piece of 8 inch pipe twisting EE's right knee. Incident happened on 11-28-2018. Employee worked until 2-26-2019.

Struck by falling object

Operator sustained a fracture to right big toe when a Skidsteer Loader Bucket slipped while the employee was in the process of training coworker on the bucket change out procedure. The bucket which dislodged from the hydraulic lift assembly, dropped approximately 6 inches striking the employee across the steel toe of work boot.

Employee was in a laydown yard, tripped over uneven ground, and hit ribcage on a rock. EE saw PCP, who didn't have X-rays done, but took employee off work. Employee requested 2nd opinion, was x-rayed on 11/28. X-rays were negative, however, physician did not release to work until 11/29/18. One day lost time.

While thawing ice on a bin pulley, a propane thawing device became entangled, causing the employee to become entangled. Investigation continues.

Employee was operating a 777 cat rock truck and getting loaded by a 992k cat loader. When the loader man dropped the first bucket into the bed of the truck, a big rock hit the truck and jarred the truck driver, causing EE to hurt EE's back.

Absorption of radiations, caustics, toxic and noxious substances

Operator was charging a battery in a loader and it exploded. Minor burns on face small cut on THE chin. Operator was wearing safety glasses so other injuries were avoided.

Struck against a moving object

Appears operator came too close to the coal edge while running parallel with the coal face. Right track on the excavator sloughed down the coal face causing the excavator to roll onto the passenger side of the machine. Employee was extricated by the emergency response team and taken to the medical clinic.

Operator was moving rock and doing cleanup with a front end loader. Employee was sitting in the seat in a twisted position with the seat belt fastened and looking over their shoulder as they were backing up. Employee attempted to turn back into neutral position at the same time they hit a pothole causing pain in the neck.

EE ran SC on the afternoon shift and called the next day that EE was having back pain. EE was examined and sent for an MRI. The MRI showed a herniation of the same disc EE hurt on 8/01/2016. EE was taken off work on 12/1/2018. This is being reported as an occupational illness due to repeated trauma.

IW was assisting in installing a draw bar on the pump monorail. IW was helping hold the drawbar in place as the trolley was pushed into position. IW had hand on the monorail for balance when the trolley wheel pinched IW's finger.

Pulling lances out of Niro 5 pulling with right hand and was placing left hand under the lance to grab a hold of it, the lance slipped out of right hand smashing the left index finger.

Employee was loading hog wire panels from bundles leaning on rib. After 8 panels had been removed the bundles fell, pushing miner away. Employee was seen by on site medic and referred to E.D. Employee was seen at the E.D. and referred to Occ Health on 11/26/18 and again referred back to Occ Health. On 11/29/18 employee was issued restrictions.

Employee slipped on ice and injured left leg.

Employee was rotating a paddle blade upwards to align a shaft and set screw when the edge of the blade caught their glove and pulled it into a steel flange fracturing their left thumb.

Employee was on a diesel mantrip and ran over a piece of old belt that was lying in the supply road. The belt caught on the tire and pulled employees left leg backwards and bruised lower leg causing a hematoma. On December 13, 2018, doctors surgically drained hematoma.

A shop mechanic was dismounting a track dozer in the shop. The mechanic's foot slipped off a deck step and when trying to catch self the mechanic's left arm got caught between the hood handrail and the hood. This caused a fracture of the upper arm. The dozer steps were clean, dry and in good condition, as the dozer was washed for maintenance.

On 11/26/18 EE was asked to help maintenance employee then said couldn't do it because EE hurt back on 11/21/18. This was at start-up.

Employee was assisting scoop operator in storing a shield. After EE placed a crib under the shield, EE exited the area between the shield and the rib. The shield unexpectedly shifted and pinned EE's left leg against the rib. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Struck by falling object

Employee was making a shuttle car splice when a rock fell out of the top striking ee on the shoulder, hand and leg. This resulted in lacerations to fingers and a fracture to 5th metacarpal.

While dumping contents in dump truck the truck tipped over and ee bumped head.

Fall from machine

On November 20, 2018 employee was changing the hydraulic fluid on the Komatsu WA 450-6 loader. EE was standing on the rear fender. EE slipped off the fender when EE backed up too far and wasn't paying attention. EE landed on feet, jarring lower back.

Bodily reaction, (Not Elsewhere Classified)

EE noticed soreness/pain around pre-existing injury, torn right Achilles tendon, 11-20-18. EE reports inflammation of said injury due to using stairs to access a platform. EE reported injury 11-26-18 and medical treatment at ER & Orthopedic doctor. Diagnosis - tendon not torn, no surgery needed, tendon simply needs to heal. Cast/walking boot & crutches provided.

At Approximately 2:10 A.M. a miner was loading roof bolts into the back of a tractor. The stack began to fall against the miner's legs pushing miner to the ground and on the way down miner struck face on the back of the tractor. Originally, treated as a first aid, but later diagnosed with a concussion.

Employee stated EE was helping remove 60"" belt structure from the longwall belt line. As EE was pulling a piece of top structure the employee's feet slipped causing EE to fall backwards. The structure fell on EE's feet twisting EE's left ankle.

Over-exertion in lifting objects

A fully trained miner was lifting a rock when the miner pulled a muscle in lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in pulling or pushing objects

Employee was washing out a crusher tunnel when the employee reached above head to remove some guards and pulled something in back.

Fall from machine

Employee was changing screen above coal crusher when employee fell 4 1/2 feet onto to floor hurting head, chest, & back. Employee went to ER & got staples to back of head.

Fall from scaffolds, walkways, platforms

Miner fell through catwalk and struck ground surface. Miner injured left ankle.

Struck against stationary object

The employee stepped off motor at 1D Switch on 9 South Haulage, foot landed on a piece of coal and employee rolled left ankle.

Struck against a moving object

While driving the Getman tractor along the travelway, the IE stated IE ran over a rock that was located in the wind row left behind the grader. The IE stated it caused IE to bounce in IE's seat. IE stated IE felt a sharp pain in IE's lower back extending down to IE's right leg. IE continued to work until IE had surgery on Dec. 27, 2018.

I tripped over the fork of a Telle Handler while flying a drone.

Employee was using bobcat and spraying with water hose cleaning under #4 belt. Employee attempted to get off bobcat and slipped hitting their right elbow on boom of bobcat.

The employee was carrying supplies on 6 east Section, when ee finished ee felt a pop in lower back / hip area.

Struck by flying object

Employee was replacing a roller on a beltline outside. Conditions were windy at the time. Employee was wearing safety glasses and at some point debris went behind the glasses into their eye.

Over-exertion in pulling or pushing objects

Employee was pulling conveyor chain over apron assembly when ee felt sharp pain in back and legs.

Caught in, under or between running or meshing objects

The employee was bolting on the left side of the miner, the employee hit the wrong lever and the rib deflector door pinch their arm hyper-extending the elbow.

Struck against stationary object

Employee reported incident to supervisor on 11/20. Employee tripped and hit right forearm against air compressor resulting in a torn bicep. Became lost time 11/21, employee worked until 11/21.

Employee was removing snow from the patrol pick up truck and suffered a slip when stepping on the running board.

moving a bolter when the back end of the bolter went into a hole and hit EE's right foot fracturing EE's little toe

Over-exertion in lifting objects

While hanging miner cable across entry, picked up cable and felt something pop in right shoulder. Employee continued to work until surgery on April 25th, 2019.

The employee slipped on wet road.

A fully trained miner was operating a trail motor when the supplies being pulled came into contact with the 4"" air line. The line was pulled down and struck the trail motor and the employee causing a laceration to EE's left cheek. The accident was not the result of any compliance issues nor were any protective guards involved.

Caught in, under or between a moving and a stationary object

The injured employee approached a roof bolting machine in operation on the 4-E Longwall section. As EE approached the machine, the machine operator on the opposite side was lowered the machine stab jack, which came down on the top of the injured employees right foot, causing a fracture. EE was wearing all required PPE & didn't violate any regulations.

Employee struck by end of 30"" diameter X 40' pipe while pipe being lifted. Truck had been moved into the work zone to allow for traffic to pass. Truck was not moved away and created a pinch point. Pipe shifted when lifted, striking employee, pinching employee between the pick-up truck and the end of the pipe, fracturing employee's pelvis.

Employee was walking near urea dike and tripped on the ground. The employee say no to receive medical attention. Few days after, EE decided go to to receive medical attention.

While fueling haul truck, employee stepped into the parking ditch for the left front tire and fell to the ground. Employee felt pain in right ankle, left knee and left arm.

While trying to put the pin back in the cat track on J-main feeder with a 4lb hammer the employee hit the top of left hand. Employee did not start missing time from this injury until November 16, 2018.

Employee working from manlift next to conveyor beltline. As they were leaning on the belt it unexpectedly started up pulling them in their harness away from the manlift.

Employee was shoveling V wipe at 2B head drive. They had right hand on the belt rail when a piece of draw rock fell from the top and hit the hand mashing their two middle fingers.

Employee exited the water truck and was walking toward the drill. As ee was walking ee twisted ankle on a rock or uneven ground.

Employee was entering the battery scoop and struck EE's knee on the valve block.

On 11/15/2018 the employee stepped out of the service truck and felt a pop in left knee. This injury became reportable on 01/18/2019 when the knee became painful and could not bend it without more pain. The Dr. put the employee off work.

Employee was shoveling belt at Brk 7 on the #4 Belt when a piece of draw rock struck them on the top of the head. Employee was taken off work on 11/16/2018 and has not returned to work.

The IE was attempting to pull a LW shield on to the ""duck bill"" of a shield hauler for transport. A nylon winch rope was hooked to the shield in preparation for pulling the shield onto the bill. As the winch rope was tightened, the tip of the foundry hook hung on the edge of the bill. The IE reached in to free it when it suddenly slipped from the bill striking EE in the chin.

As the employee was bending over to lift a jack pipe on the NE road, xc 104, ee felt a pain in lower back and down left leg. The doctor's assessment was a lumbar strain which should be treatable with therapy. EE began missing work on 11/13/2018. EE is expected to make a full recovery.

Alleged: On 12-2-18, we became aware of employee alleging incident on 11-11-18 where employee claims employee pinched a finger handling grader cutting edges. Employee sought medical care on 12-2-18 for an infection in the finger. Treated with antibiotics. X-ray on 12-11-18 showed a fracture to finger tip. No treatment advised for fracture.

The injured was working assisting others in removing a belt take up system. EE bent over to move a section of the tum table and felt a burning in the abdomen. EE was seen by a physician on 11/16/18 and was diagnosed with an inguinal hernia that will result in surgery to repair.

The employee was helping spot a rib bolt when a rock rolled out of the rib and struck EE behind the metatarsal guard of EE's boot. The rock punched a hole in the top of EE's foot that was too deep the clean, so the medical facility did not clean it right away. This prevented stitching of the wound.

Haul truck operator notified supervision of eye irritation, an EMT responded and flushed eye, employee thought the debris was out. Before next shift, employee reported irritation was worse, went to the ER, was given a prescription, and taken off work 2 shifts.

The injured miner was assisting a co-worker in throwing the flop gate at a head drive chute when the chain ratchet that was holding the gate came loose and allowed the gate to strike EE on the right side of the head. As the gate struck the injured miner, it knocked EE into an adjacent object causing EE to strike the left side of EE's head.

The IE was assisting with hanging hog panel on the coal rib. The IE reached for a bar to help hold the hog panel up and a piece of rock hit EE in the left knee.

Fire Boss bumped IE's head on a pipe; later in the shift began to experience vision issues, i.e. spots/floaters. IE had a broken blood vessel in the eye which in turn caused a torn retina. It appeared to be a result of the bump to IE's head. IE had laser surgery to repair the retina on 11-2. IE has unrelated existing retina pathology in other eye.

Miner's rt knee hit the hopper causing bruising. The miner grabbed ladder to help arrest fall causing a strained rt shoulder. Miner completed shift & was examined the next day.

Employee was using a small sledge hammer to knock a roller loose so the conveyor belt at crusher could be lowered to remove excess material. Employee missed roller and struck conveyor frame with right hand cutting and fracturing right ring finger.

On 11/06 EE was seen by the doctor who decided to put EE off work until a follow-up appointment the next week.

Technician was driving a track pin in because the track press didn't push it in all the way. While hitting the pin with a sledge hammer, a flake of metal flew off the pin and stuck in the Tech's left hand.

While operating a Komatsu HD785 haul truck during loading operations, a large rock fell from the Komatsu PC3000 backhoe that was loading the truck. The rock struck the tail end of the truck bed causing the front of the truck to bounce on the ground. The miner jammed leg causing discomfort to back and leg.

An MRI was performed to confirm and showed a fracture to the talus bone. The results from the MRI were reviewed with doctor on 12-13-2018 and resulted in classifying this injury as an MSHA Recordable.

EE thought ee was fine but swelling would not go down. Employee was taken for Xray on 12/7 that showed imbedded metal in right knee. Antibiotic given. Ortho referral to remove the metal. Placed on restrictions.

Canopy came in contact with a roof bolt plate that was not flat against the roof due to uneven roof and the plate cut the 3rd finger of right hand requiring 3 sutures to close the laceration.

EE was moving channel iron in tight conditions by self. Getting in a hurry in cold conditions not thinking of the proper lifting techniques. Lifting from below knees ee felt the strain in groin area. Welder #2 was cutting a strip of steel loose from the rear of the girder cutout. The strip of steel was cut loose & allowed to free fall uncontrolled, approx. 7'-8' to the ground, where it fell over and struck the work platform & broke it loose from the girder. It caused the ladder that Welder #1 was standing on to topple over, and fall approximately 8'-9' to the ground.

On 11/23/19 around 4:05am an employee was loading powder in 1292 when a rock fell off of the face striking them in the left forearm. While reaching out to prime a hole a 1.5 x 2ft x 4 thick rock slid off about 6-7ft from the face striking the employee in the left forearm causing a 7cmm laceration and deep tissue contusion requiring 14 sutures.

One inch cut on left hand while removing keeper for chain connecting link with pliers. When they pulled back, the link struck the left palm causing a one inch laceration. Due to the location of the cut, the doctor thought it best to place four sutures to keep it closed/help it heal.

The surge blew a lightning arrester and sent the energy through the mine and through the roof bolting machine shocking the employee. Employee had pain in chest and left arm. EE was taken to the hospital as a precautionary measure. EE was found to be in good health and released back to full duty.
