A rock fall occured at the number 8 entry while making returns. The fall measured 40 foot long, 20 foot wide, 10 foot thick. The top was bolted with 6 foot resin bolts and 10 foot cable bolts
There was dirt, rock, coal & mud that had fallen in at the back end of the fan house. There was insufficient air. Had to shut down and remove dirt, rock and mud with excavator and rebuild fan house.
A ROOF FALL WAS DISCOVERED IN THE 2 NORTH #3 MAINS, 28 CROSSCUT, #4 ENTRY INTERSECTION. THE FALL MEASURES APPROXIMATELY 30 FEET LONG, 18 FEET WIDE, AND 6 FEET HIGH. THE AREA IS SUPPORTED WITH 5 FOOT FULLY GROUTED RESIN BOLTS AND 6X6 TIMBERS.
Slope brake car came off the tracks. It was put back on the tracks and in service at 3:15 p.m.
Mining 1 intersected D-Gas hole, water full pipe coming out of hole. Normal strata, aprox. 96"" high-24"" draw slate. No sand rock. Normal roof conditions. Plugged hole aprox. 9:30pm. Ignition was self extinguishing. Location C-Bleeders CH.section #1 entry. Methane quality at face .040% CH4 when ignitioin occurred-K-order issued
Roof fall on # 5 belt between spads 825-826, 280' from #5 head drive. Size of the fall 20' X 70' X 5'.
Unintentional roof fall #29 xcut, 3rd North sub main belt line. 18' long x 18' wide x 6' high. Bolted with 5' double lock bolts. Area was mined 3-15-03. Fall was propped at approaches and will be cleaned up.
Roof fall in the intake. Measured 30'(L) x 16'(W) x 3'(H) in the #3 Belt Intake.
Distance from active face: 1,500 ft. Airway: return, escape ways cribbed. Size: l - 20ft.W-19ft.H-4ft. Roof support: 48 in.fully grouted resin. No cribs or timbers installed prior to fall. Strata: sandstone, mixed, laminated, wet. No coal rider seam present. All entrances are breakered off.
The Contrary Service Cage was down for approximately 1 hour. There were 5 persons on the cage for 20 minutes. There was some dirt and grime build up on the rope at the jam conveyance switch. There were no injuries due to this incident.
A roof fall was found on an idle shift. The fall was found at 0+80 #4W. It was 30' long, 18' wide and 15' thick. The area was initially supported with 5' install III bolts. 12' install II long bolts had been installed as secondary support. Water in the top was determined to be the cause.
At 12:39PM the 20 shovel operator called and said there was smoke coming out of the house and he was exiting the shovel. May Day was called and the Fire Brigade headed to the scene. At 1:19 the Incident Command released the May Day, fire was out. Fire was in main trasformer and fire brigade stayed on scene to cool down and fire watch was set up. MSHA contacted. Spoke with Hank.
An ignition occurred on the N-1 longwall when the top fell behind the headgate shields.
Slope hoist overtravel was out of adjustment. Repairs were made and hoist was put back in service at 9:50 p.m.
Unintentional roof fall above anchorage for a distance of approximately 100 - 150 feet.
The hoist was inoperable for appr. 2 hrs due to mechanical problems. During muck hoisting operations, the hoistman experienced problems with the control joystick. A maintenance inspection revealed a faulty rheostat inside the joystick. No injuries or damage resulted from this incident.
A roof fall was found during the weekly air course examination at 19+00 between main entries #6 and #7 west. The fall was 30""long, 17.5'wide and 6.5'high. The cross-cut was initially supported with 6' fully grouted resin bolts. Water was determined as the cause. The cross-cut was timbered and dangered out.
A roof fall was found during the weekly examination of the air courses at 2+40 #1west main entry. The fall was in the intersection. It was 15'long, 18'wide and 6'thick. Water was determined to be the cause. The intersection was initially bolted with 5' install III bolts. The intersectioin was timbered off and dangered out.
A methane ignition occurred at drill platform over an 8 3/4 inch slurry injection hole leading into the sealed Gail panels.
ROOF FALL (7000-1-001-06) : LOCATION : Outby 1 North 3 West Seals - #4 Seal. DIMENSIONS : H:6-10', W:19', L:280'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Hytech point anchor bolts. The area was mined in June 1993. Prominent roof jointing or slips in the roof were the apparent cause for the fall.
The Enlow Hoist was down for more than 30 minutes because a cover came off and damaged the door linkage and wiring to door switch. There was no entrapment.
An unintentional roof fall occurred on the MMU 002 working section. This roof fall occurred at a 4-way intersection located in the #7 entry at the #21 crosscut. The area was supported with 10' point anchor roof bolts. The roof fall measured 12' x 18' x 12'H. and will not be cleaned up.
UNINTENTIONAL ROOF FALL. FALL MEASURED 20' LONG X 18' WIDE X 40' HIGH. AREA MINED 2/22/06. NO INJURIES OR PROPERTY DAMAGE. ROOF PREVIOUSLY SUPPORTED WITH 6' TORQUE TENSION AND 6' DOUBLE LOCK BOLTS. ONLY ENTRANCE WAS DANGERED OFF AND BROW SUPPORTED WITH 3 CRIBS. FALL MOST LIKELY WILL NOT BE SUPPORTED OR CLEANED.
The Cedar Creek Service Elevator was out of service on 2/22/06 from 1:20pm to 4:03pm due to the armature voltage signal not being picked up which did not set the brake all the way causing the car to drift into the overhead frame. Bagby elevator co. came and made repairs to the ""M"" contactor and then the elevator was put back into service.
Began 70' right of spad 1111 extending from # 6 entry to # 8 entry & outby 50' down the # 8 entry roof was supported with 4' resin rods. Laminated shale roof.
A roof fall measuring 24'L X 28'W X 7'H occured at the number No.3 NW mains No. 2 entry spad number 5099 area was bolted with 6' bolts.
An unplanned roof fall occurred in the #3 belt panel return entry at the #2 entry intersection of #8 X-cut. Fall was 50' in intersection and cross-cut and 30'long, 7' to 9'high. Area bolted with tension rebar.
14 breaks underground roof fall on intake 55'long, 18'wide found on preshift.
The hoist inoperable for approximatley 2 hours as a result of brake adjustment problems on #1 side. This occured during the normal muck hoisting cycle a maintenance department inspection determined brake sensor adjustments were required to remedy the problem. No injury or equipment damage resulted from the incident.
AN UNINTENTIONAL ROOF FALL OCCURRED AT BREAK #81, #8 HEADING OF THE RIGHT RETURN. THE FALL MEASURED APPROX. 30' LONG X 19' WIDE X 5' HIGH.
Elevator went down due to movement of rails caused by temp. change. Sensors were adjusted - running @ 9:40 am. This is a new system and is still having a few ""bugs"" worked out.
Had roof fall inby survey 179. Roof fall was approx. 18' wide x 30'long, thickness was 2.5' to 4.5'. There were 4' glue bolts used in this area of mines. This fall was found by EE on pre-shift exam.
A roof fall was discovered on 1 section in #1 entry. The fall measured 5 1/2 ft high, 18 ft wide and 50 ft long. No injuries were reported as a result of this roof fall.
As the Hoistman was conducting the daily Pre- Operational saftey checks, The #1 left brake failed to properley release. A maintenance inspection identified a faulty brake hydraulic tank level sensor to be the cause. No injuries or damage resulted from this incident. MSHA notification was made.
#152 D11 dozer caught on fire while in operation, SMET team responded, almost got fire put out when dozer started on its own. This happened several times due to starter wires shorting. No injuries were sustained. Campbell County Fire Investigator, Catterpillar, and Firemaster came out to help with investigation. The cause of fire still has not been determined.
DUE TO AN UNDETECTED POCKET OF GAS (MEASURING APPROX. 4' WIDE BY 6' LONG) 2 EMPLOYEES WITNESSED AN IGNITION OF METHANE GAS ON THE LEFT SIDE OF THE MINER. HOWEVER, NO PHYSICAL EVIDENCE OF AN IGNITION WAS FOUND.
AT ABOUT 8:00 AM, 2 GOLF CARTS WERE PUT ON CHARGE AT SHIFT CHANGE. AT ABOUT 9:55 AM, 2 MINERS SMELLED SMOKE ON TRAVEL WAY. THE MINE FOREMAN WAS NOTIFIED & TRAVELED TO SLOPE BOTTOM & FOUND 2 GOLF CARTS ON FIRE. THE FIRE WAS PUT OUT ABOUT 10:15 AM. CAUSE OF FIRE IS UNDETERMINED (INVESTIGATION ONGOING). ALL ELECTRICAL CIRCUITS SUPPLYING POWER TO GOLF CARTS FUNCTIONED OK.
WHILE PRE-SHIFT EXAM WAS BEING CONDUCTED, ROOF FALL WAS FOUND ON THE #5 BELT CONVEYOR.
Roof fall old #4 unit, #1 entry & x-cuts spad 35+07 to 37+19. Approx. 432' long, 20' wide, 6.5' height.
A methane ignition occurred in face of #3 Right crosscut on 5 Right Section, 26' right handed of survey station #23740. Ignition occurred when sandstone roof fell in cut while continuous miner rippers were running. Ignition occurred approx 30' from face on left rib. Flame duration was approx 6-8 seconds, & was extinguished w/fire extinguisher. No injuries, no equip damaged.
Roof fall old #4 unit, #4 entry & x-cut spad 33+99. Approx. 40' long, 20' wide, 6.5' high.
A rock fall unplanned occurred 90' inby 4171 in the intersection of #8 to 9 face. The fall measured 5 to 6' thick x 18' wide and 20' long. Strata consisted of sand rock layered with streaks of coal and fossilized slate.
On the south section 1st s.e. submains in #2 entry at spad #15 & 95 roof fall occurred. The fall was 25' long 18' wide and 10-12' thick. no equipment involved, vent not affected.
Hoist Failure Electric Problems.
Roof fall 5R+to #6+ outby 1 brk. Roof had been supported with 4' resin and 8' cable bolts, laminated shale roof. Fall was 140' long X 20' wide X 8' thick.
Hoist got caught into upper track limit while slinging some pipe. Electricians did some trouble shooting. They called G.L.Tiley and Associates LTD in Canada. They got on line and reset the computer.
FRIGID COLD WEATHER CAUSED WATER TO AIR COMPRESSORS TO FREEZE. THE MAIN HOIST WAS DOWN FOR ABOUT 3 HOURS BECAUSE OF NO AIR. ALL UNDERGROUND WORK ACTIVITY WAS HALTED. #2 SHAFT WAS OPERATIONAL, BUT NO ONE WAS EVACUATED.
The elevator serving the #6 portal was out of service from 5:48am until 6:20am due to a malfunction in the interlock for the inner door. Affected miners were directed to use another route of escape in case of emergency.
The track slope car was unavailable as a secondary escapeway from 9:05am until 2:25pm due to failure to the 4160 cable serving the hoist house. Allegheny Power had to respond to correct the problem and cable was re-entered in a connection. All affected miners were instructed to use another secondary route of escape from their work areas.
A roof fall occurred in the 11th crosscut in the track entry along the no. 4 belt measuring 20'x30'x3' thick. The area had been developed approx 2 years ago.
A roof fall occurred on the 8O section. The roof fall occurred in the #1 heading just inby the #27 crosscut and extended inby for 60 feet. The fall was 10 to 12 feet high and 22 to 23 feet wide. The area had been bolted with 5 foot install B bolts.
Had an unintentional roof fall approx 9 breaks from the main outside track portals. Fall is 100' long x 10' high x 20' wide.
The #8 elevator was out of service for a period exceeding 30 minutes from 8:55 AM to 9:38 AM due to the car door switch being out of adjustment. Adjustments were made to the car door switch and the #8 elevator was placed back in service.
An unintentional roof fall was discovered at #33 block, spad station #3731, #9 entry, along #4 belt. This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been investigated. The dimensions are 18'W x 75'L x 8'H.
An unintentional roof fall has occurred int he #3 hdg of 002-0 mmu inby spad. This fall was 20' wide 6' to 8' high and approx. 70' long. Fall did not impede ventilation or didn't involve any equipment. area bolted with 5' bolts.

A roof fall occurred on the Main North in entry #5 between crosscut 70 and 71. This fall was in the travelway and the travelway was re-routed around the fall. The cause of the fall was weak laminated shale. This fall is in an area of the Solar Sources mine that Black Beauty recently purchased.
Spad 34+56. Fall extends from #3 entry over to the edge of #1 entry, in worked out panel (old workings). Approx 125' long x 7 H x 20 wide.
A ROOF FALL OCCURRED NEAR SPAD LOCATION 1869 IN #7 ENTRY. FALL WAS APPROX. 75' LONG X 20' WIDE X 4' THICK. THERE WERE NO INJURIES; NO DAMAGE TO EQUIPMENT. THE FALL WAS ABOVE ANCHORAGE. IT DID NOT AFFECT VENTILATION OR IMPEDE PASSAGE.
Employee became stuck in elevator between floors.
Had unintentional roof fall in the #3 entry of the 011 / 012 MMU supersection. Fall was 60' L x 8' high x 20' wide.
A rock fall occurred on #1 unti -#8 entry in the intake. Fall was from 17+55-18+65 a total of 110 ft. fall started outby feeder and extended 55 ft inby-110 ft from face.
THE HOIST FOR THE MAN MATERIAL SHAFT FOR THE GATEWAY MINE WAS UNABLE TO BE OPERATED DUE TO A MECHANICAL CONDITION. THE MAN MATERIAL SHAFT IS THE PRIMARY ESCAPEWAY FOR THE GATEWAY MINE.THE EE'S WERE CALLED OUT OF THE MINE AND MINE WAS IDLED UNTIL MECHINICAL CONDITION WAS CORRECTED. THE HOIST WAS UNOPERABLE FROM 7:10AM UNTIL 8:30PM
A fall of clay rock occured in the west mains #1 entry. Fall was 20 ft long, 16 ft wide, 13 ft high. Area was bolted with 6 ft double lock bolts and was also spot bolted with 12 ft (2) piece double lock bolts. Fall was outby the working section. no disreption of ventilation, no injuries or entrapment.
Roof Fall on the 2nd Panel West off of the 3rd South 0+60 Room #6. The intersection was bolted with 5ft resin bolts that have fallen out. This area will not be cleaned up and does not influence the passage of men or the air course. The fall measured 60ft long, 19ft wide and 6.5 in thinkness.
600 KW - transformer failure causing fire. Damage to transformer only. *Timeline of incident attached.
An unintentional roof fall occurred at #16 crosscut, in the 4b intake entry #3, the fall measured 25' long by 20 wide by 6' thick.
Supply cars were being let down the slope. One came uncoupled allowing it to runaway down the slope striking a support beam which allowed the roof to fall in and take the belt line out of service. Area will be re-supported and cleaned. MSHA and the OMSL have investigated this incident. The fall is app 40' in length x 16' wide x 7' high.
During regular maintenance inspection of the elevator a damaged stran wire on one of the hoist ropes was observed. Elevator was taken out of service and hoist ropes were replaced.
A fall occurred on a section of storage unit of longwall belt at spad #3272, approximately 100 ft long and 10 - 12 ft high.
An unknown condition started a fire in the engine compartment of a Fletcher scaler while it was in use back-scaling. The operator activated the fire suppression system and initiated the mine evac system. All personnel exited the mine safely and MSHA was notified immediately.

This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been fully investigated. The dimensions are 90'L x 19'W x 7.5'H.
The cedar Creek Service elevator was out of service on 2/9/06 from 6:55 pm until 10:00 pm due to trash in the truck for the doors and then they could reset the door operator. bagby elevator company came and made the repairs and then the elevator was put back into operation.
An unintentional roof fall was discovered at spad #3870, #6 entry, along #4 belt, #51 block. This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been fully investigated. The dimensions are 40'L x 19.5W x 7.5'H.
The Oakdale Portal Elevator was inoperative from 9:45 am to 11:40 am. A malfunctioning door switch on the car was replaced. Returned to service @ approx 11:40 am. No entrapment, no injuries.

A roof fall has occurred in the crosscut between no. 5 & 6 entries of L-main set of entries at spad 9+85. The fall measured 7' high x 8' wide x 40' long approximately.
Setting 5' & 4' resin bolts fall occured in intersection.
During inspection. Hoist at brushy was foung inoperative. Escape way was re routed & hoist repaired. Hoist was reinstated 2/14/06.
In the #5 entry north supply road at 5721 to 5801 in pillar line was developed about 8/10/04. The roof was pinned with 5' tension rebar bolt. the fall was approx. 44' long 16' wide, 8' high.
Mining operation was in process of changing the main hoist cable. Employees were using the escape hoist to enter & exit the mine. While 3 employees were on escape hoist, the hoist malfunctioned, suspending the 3 for approx. 15 mins. Temp. repairs made to extract miners. Mine shut down until permanent repairs made. MSHA notified at 0500 hours & left voice message.
On Feb. 7, 2006 the #2 hoist was down from 9:00am to 12:00pm.
Roof fall occured out-by the working section at cross-cut 18+20. The fall was 100' in length crossing the #2W & #3W entries. Fall was 18'wide and 12' high. The area was initially bolted with 6' fully grouted rebar bolts with 10' cable bolts as secondary support. A slip was exposed at the top of the fall.
A rock fall occurred just outby spad # 9+25 in #4 entry of the 1st North Mains. The fall was 18'W x 20'L x 8'H.
(2ND OF 2 FIRES - 1ST FIRE ON 2/4/06.) Contractor baling hay in reclaimed area E. Apparent mechanical failure on hay baler 567 resulted in an overheating condition which ignited grass. Fire consumed 7.35 acres of reclaimed vegetation with total burn time of 1.5 hours. Extinguished by mine fire brigade and SMEC power plant fire brigade.
An unintentional roof fall occurred in the last open crosscut of no. 5 entry. The fall was approx. 35' long, 19' wide & 7' thick at highest point. The area was bolted with 4' resin bolts and some 5' resin bolts.
An unintentional roof fall occurred approximately 20 feet inby SS# 1211 in the #1 entry. The fall dimensions were approximately 40' L x 14' W x 4' high.

Roof fall was discovered across from spad 3630 near Horse Creek #3 belt. Fall was Approx. 13,000 ft outby the #1 section. Fall was at the Intersection of the #5 entry. Did not disturb Primary escapway. Fall was 40' long X 19' wide X 6' high. Area timbered off at all approaches.
During an inspection of the intake air course a roof fall was found in an intersction approximately 900 feet from the mine portal. The fall measured approximately 18 feet by 20 feet by 5 feet in thickness.
A fall occurred between #4 & #5 entries at crosscut #32 inby spad #24813. The fall was approximately 20' x 10' wide x 5' thick. The area had been supported with 5' high tech bolts and supplemented with 8' super bolts.
Normal Hoisting made when a fuse blew on the main hoist motor cooling fan. This shut the hoist down and set the brakes. The fuse controlling the blower fan is located in the dc 2000 thy ristr control panel and is set up to shut the motor down and set the brakes on hoist if this blower fan fuse blows. This is a 4 amp fuse.
40' l x 20' w x 7 1/4' roof fall in #4 entry on #1 section due brk outby spad 2119
Roof fall occurred in #1 return entry at spad #89 approximately 350' outby section. Fall is 3-5' thick, 16' wide & 23' long. No injuries/equipment involved. Area was fully bolted with 4' resin bolts. Area that fell resembles a horse-back slip. Fall will not be cleaned up - has been breakered off.
When water was pumped down in the entry, a reportable roof fall was found in 19 Tailgate, entry 1, crosscut 40. The fall was approx. 12' high, 20' wide & 30' in length.
An unintentional roof fall occurred on the 1st South Panel between xc 14 and xc 17 in the #5 entry (8 + 55 - 10 + 20). The roof fall was approximately 220' long, 17' wide and approximately 8 1/2 thick. The roof fall did cover up the high line. Area was timbered off after fall.
AN UNINTENTIONAL ROOF FALL OCCURRED IN X-CUT 17, ENTRY #5 OF THE 1ST NORTH SUB-MAINS, ON THE 2ND WEST CONVEYOR BELT. THE FALL WAS IMMEDIATELY REPORTED TO THE MSHA FIELD OFFICE AND WAS INVESTIGATED BY THE OFFICE. THE FALL MEASURED 30'W x 40'L x 5'H. THE FALL WAS IN AN AREA BOLTED WITH 5' BOLTS PRIOR TO OPERATORS CHANGE IN ROOF CONTROL PLAN TO 6' BOLTS.
A fall of ground occurred between EB76-EB77 bench bottom and top slice. This area was an inactive area. The ground fall did go past point anchor on resin bolts. Area is flagged off and barricaded from access. The time of fall is not sure; it happened at shift change.
2/4/06 Bearing in baler went out grass was ex- ceptionally dry grass caught fire. 2/5/06 pickup assembly on baler picked up rock which logged against metal sparked and caught hay on fire.
A rock fall occured in number one entry on left return for 007 section on number four main belt. A continous fall from previous rock fall. The rock fall was 130' long, 20' wide, 4-10' thick.
(1ST OF 2 FIRES - 2ND FIRE ON 2/7/06.) Contractor baling hay in reclaimed area E. Bearing failed on hay baler & overheated, causing grass to ignite. Recent drought contributed to increased fire danger. 68.73 acres of reclaimed vegetation were consumed, with total burn time of approx. 3 hrs. Extinguished by local vol. fire dept. & San Miguel fire brigade.
A roof fall occurred in the 3rd west, #8 entry, #52 crosscut, 3640' Station. The fall was approximately 20' long, 20' wide and 8' tall. The area was bolted with 6' and 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.
Surface electrical transformer at surface facility caused power to trip out #1 shaft hoist - resulting in evacuation of UG operations via #2 shaft hoist.
A roof fall occurred on MMU 002-0 last open crosscut in #4 heading. Fall measured 20' long, 20' wide and 4 to 5 feet thick.
Roof Fall was discovered in 1st right off of slip ridge mains in #2 entry near spad # 6710 fall measured 20'long X 4' wide X 4' High. 4 ft. Fully Grouted Resin Bolts were being usedin this area, fall will not be cleaned up. All approaches have been supported. No injuries occured. Fall was 13,200 ft from working section. Area Examined on a weekly basis.
Roof fall was discovered in 1st right off slip ridge mains in #1 entry near spad #6782 Fall measured 20' long 4' Wide X 4' High. 4ft. Fully Grouted Resin Bolts were being used in this area, fall will not be cleaned up. All approaches have been supported. No injuries occured. Fall was 12,800 ft from working section. Area Examined on a weekly basis.
Track entry fell in at 3875 and from tail across to 10 tail fell in.
An un-intentional roof fall occurred in the number 3 entry, 1 break inby 4074. The fall started in an intersection and fell through the crosscut from no. 3 entry towards no. 4 entry for a distance of about 30 feet. 48 inch resin bolts were being used. Fall was 20' wide, 30' long, and 8' high in highest point. MSHA and the State were called when the fall was found about 1:00 P.M.
The #1 elevator was out of service for a period exceeding 30 minutes due to a problem with the Uninteruptible Power Supply (UPS). The UPS was removed and sent away for repair. The elevator was placed back into service on 2/6/06 at 12:35 PM. No escapeways were affected by the outage.
A roof fall has occurred int he main east entries at spad 38 & 75 in no. 3 entry. The dimensions of the fall are: length 18', width 22', height 8'.

HOISTING INCIDENT : Cowling Portal Elevator - On Thursday, 02/02/2006 at 08:45 pm the Cowling Portal Elevator went down due to the door switch at the bottom of the shaft being out of adjustment. The elevator was repaired on Thursday, 02/02/2006 at 10:45 pm when the door switch was adjusted after locating the problem.
EE was operating a mechanical scaler when he heard a loud boom. Scaler caught on fire and mine was evacuated at that time. It is beleived that this was an electrical flash fire & self-extinguised in less that 30 minutes. We notified MSHA at the time because it was thought we had a reportable mine fire. Upon further investigation, it revealed it probably was not.
Upon entering the secondary escapeway (return entry) in the 6th right section, smoke and carbon monoxide were encountered. It was determined that a thermal event had occurred in the #2 panel gob area.
A roof fall was discovered during the weekly examinations in the primary intake escapeway 1 block outby Station No 105. The fall measures approximately 30' ling x 17' wide x 5' high. The fall will not be cleaned up. An alternate primary escapeway entry will be designated and properly identified with reflective indicators.
Roof fall was 15.5' wide X 40' long X 8.5'high the roof fell prior to being centerbolted. Mined 1-31-06 997 feet of cover.
A roof fall of clay rock occured in the west mains belt entry. Fall was 20' long, 17' wide, 18' high. Area was supported to place by left double lock bolts, additioinal support of T-5 channel and 12' (2) piece doulbe lock bolts installed on 4' centers. No injuries, 30' of 60"" belt structure damaged.
A small 2.8 amp fuse flew ont he dc drive unit that controls the emergency stop circuit. This caused power loss and brakes to set. Replaced fuse and ran safties. Everything ok.
Section of roof fell about 7 ft hi. 18 ft wide 30 ft long and due to water in top and hill seams.
Somerset elevatorwas our of service due toa malfunction of a relay switch causing elevator to stop short of the landing. Two employees were stuck on the elevator from 7:15 am to 8:00 am. Elevator was repaired and inspected then returned to service.
The Jeffrey Locomotive EE was operating struck a longwall support that was loaded on a low boy parked at 66 brk switch on E-1 track. Locomotives were being staged at this location for transport to Submains. Locomotives were advancing one at a time.
Employee was walking up to a stopping that had just been knocked down by a scoop when a piece of rock fell from between bolts and struck him on the back.
3 employees working on elev. platform to remove pitman from Crusher B. Crane owned & operated by contractor had lifted pitman & it was hanging stationary. The men headed for the stairs to descend. Pitman suddenly swung and hit EE, fatally injuring him (cause of death crushing blunt force trauma). 2nd EE was grazed by pitman, but was not injured. 3rd EE uninjured.
Right rib roll in No. 5 entry, No. 4 left mains pinned employee against roof bolting machine.
EE was cropping the clarion level when for unknown reasons he traveled to the top of the ridge and inadvertently contacted a low pressure high volume 16 inch gas line.
Moving sections of 3"" diameter pipe into the pipe rack, one section bound up on another and as he moved it he caught his left middle finger between them and took the tip of the 1st joint off.
Employee had his right hand fingers between a wire rope sling in a basket hitch configuration. When the sling was tightened to lift a piece of equipment, it pinched and twisted his fingers, severing the two middle digits at the first joint.
Employee complained of back pain and muscle pain. Was diagnosed with back strain, groin pull and hernia. Did not report accident until 2/27/06. Employee could not provide cause of injury.
Employee was coupling railcars to locomotive. When he noticed at the last minute that the draw heads were not lining up, used his left foot to push the draw head on the railcar. His foot slipped and was caught in the couplings, crushing his toes. Three toes were surgically moved later that night. (Little toe and next 2 amputated - all proximal phalange.)
EE WAS GRINDING ON A DOZER BLADE WHEN THE GRINDING DISC CAME APART AND STRUCK THE EE'S FACE SHIELD AND SAFETY GLASSES CAUSING A SEVERE LEFT EYE INJURY.
EE WAS LOADING SUPPLIES ONTO THE SCOOP FROM THE FLATCAR WHEN HE GOT HIS LITTLE AND RING FINGERS CAUGHT BEHIND THE RAM OF THE SCOOP. THE RAM CRUSHED HIS FINGERTIPS, RESULTING IN THE AMPUTATION OF HIS RIGHT LITTLE FINGER TO THE MIDDLE PHALANGE AND HIS RIGHT RING FINGER TO THE DISTAL PHALANGE.
Pulling auger tools, employee grabbed end of auger when it came off and snapped back onto fitting smashing fingers on right hand.
While working with a heavy bar, the bar slipped and he got a laceration of 5th finger left hand.
EE felt a sharp pain in his lower back and his right groin area when he picked up miner cable and placed it on the rib of the mine.
Employee was carrying a 4' x 8' x 1/8"" sheet metal with 1 other employee. He was walking backwards when the other employee tripped, pushing the load onto him, causing him to fall backward dropping the sheet metal onto both legs.
A Tank Loader was injured when his right arm was pulled into #18 Tripper Conveyor between the head pulley and the cleaning brush. Injuries included accidental avulsion of some of the worker's right forearm muscle tissue. The employee is hospitalized at this time.
EE was pulling a tub of oil from under a loader, the row rod pulled loose and he fell backward landing on his wrist.
While stepping down out of trailer van, the wind caught van door and slammed it around against employee, knocking him to the ground. He sustained trauma to the right knee and leg by way of bruising and abrasions. He went to emergency room at hospital and had x-rays, which do not reveal any fracturing. Leg was wrapped in a soft-brace, therapy and several days lost time.
A haul truck was traveling empty from the pit. It struck the berm on the pit edge with its left front tire. Skid marks indicated the truck slid about 4' as a result. The truck then traveled about 200' before stopping half way up the berm. The driver was taken to the hospital with visible head injuries. Initial investigation presumed he fell asleep and was not wearing his seat belt.
EE was pushing bolt up into the mine roof while bolting on the left side of the 3 north left miner. Roof bolt caught on roof strapor in bolt hole, causing the bolt to bend, catching his left middle finger between the bolt and atrs. EE sustained a fracture to the tip of his left middle finger. EE was wearing gloves.
Lost balance and fell hitting left knee on rail. EE returned to work on 3/1/06, then went back to Dr. on 3/2/06.
Employee was sweeping in return, dust 6"" deep and covered a block. Employee tripped over the block and twisted ankle.
Not paying attention to what he was doing he burnt left hand with cutting torch.
Employee was trying to put a piece of drill steel back into the rack when the steel slipped and caught his hand and thumb, also bending his wrist enough to break it. The drill steel landed on the employees thumb, breaking it in 2 places.
A roller was stuck on the 1 east belt at break #6. The EE attempted to free the roller without locking out or turning off the belt. His hand slipped and caught his lower right arm (forearm) between the belt and the roller. The Dr. took him off work on 3/7/06.
WHILE UNLOADING 95"" X 4' SCREEN CLOTH OFF OF BACK OF TRUCK, THE SCREEN SLIPPED FROM DRIVER SIDE OF TRUCK TOWARDS PASSENGER SIDE. IT FELL TOWARDS GROUND, CUTTING A 4"" CUT IN LEFT LOWER LEG OF EMPLOYEE.
He was walking down a set of steps, slipped on ice and fell. He struck his lower left back causing fracture to three ribs.
Working on bearing (was spring loaded), cut lock nut off, hit impact wrench, spring came loose, impact wrench sprung back and pinched his thumb between wrench and motor.
EE stepped out on running board of truck top step and slipped and fell on four inch pipe on scales. He hit his knee cap on pipe and arm on another pipe.
Employee was emptying mud bin when mud splashed back and impacted him in the face.
The employee was climbing the stairs and did not step firmly, which caused an injury in his right knee with the sharp stairs. They took four stitches on his knee.
Employee was going to the truck shop through door #8. He slipped on snow and ice at the door way and strained left knee. He continued to work since then with no restrictions no lost work days but has informed us that he is having surgery on 1/30/07 related to this incident. He begins losing time for this previous year's injury on 1/30/07.
EMPLOYEE WAS WEARING WIRE CUTTERS AROUND NECK WHEN HE BENT OVER AT SCOOP CHARGER. THE WIRE CUTTERS GOT CAUGHT AS HE STOOD UP AND THEN RELEASED, STRIKING EMPLOYEE IN LEFT EYE - LACERATING THE EYEBALL.
EE reports that in February or March of 2006, he strained his neck and L shoulder while dislodging oversized material at the Tower A grizzley. EE reported the incident to management on July 27, 2006. the incident is being investigated for compensability.
EMPLOYEE WAS PULLING SLACK ON THE #2 SHUTTLE CAR FOR A POWER MOVE WHEN HE FELT A POP & PAIN IN HIS LEFT CALF.
EMPLOYEE WAS LIFTING A CHAIN & HIS BACK STARTED HURTING. STARTED MISSING WORK ON 3/10/06.
EMPLOYEE WAS HANGING CABLE. CABLE FELL DOWN AND HIT HIM ON LEFT SHOULDER (BRUISED SHOULDER).
Employee was dressing up a drill bench when he felt something like a crick in his neck, and after a short time the pain moved into his lower back. Employee stated that he had not done anything different and he didn't know what happened to cause his pain.
Contractor complained of burning sensation on his left shoulder and arm while changing a screen panel plant. Upon observation, his shirt was wet along with the seat of his pants. He removed his shirt and a small area on the left shoulder appeared to be scraped with redness to his elbow. He was taken to a shower to wash chemicals from his body and then transported to the local E.R.
EMPLOYEE WAS BOLTING TOP AND WAS REACHING TO GET ROOF BOLTS AND TWISTED HIS KNEE.
EE WAS AT THE POWER BOX WHEN A ROCK FELL FROM THE TOP, HITTING HIM ON THE ARM (LACERATION TO LEFT FOREARM).
While dismounting a front end loader, the injured slipped and fell to the ground and broke his leg.
Employee was carrying supplies from the service shaft cage to the manbus when he hit his head on a roof bolt that was hanging down from the roof.
While operating operator's side of bolter, drill head went up higher than the canopy and got wedged onto canopy. Employee used left hand to separate head and canopy causing hand laceration.
Employee complained of back pain after operating front end loader (lower left back and leg).
While taking lifting beam off crane, it turned over and struck the leg of the crane helper causing a simple fracture.
Employee was riding in scoop bucket in the direction of the battery end on #1 unit's supply. While the employee was watching the miner cable, the scoop operator raised the bucket because the tire went down into a hole. Scoop came out of hole; the bucket went up against the roof pinning the employee between the floor of bucket and mine roof.
EMPLOYEE'S HAND SLIPPED AS HE WAS UNLOCKING TILT STEERING ON THE KOMATSU 450 LOADER AND JAMED HIS RIGHT MIDDLE FINGER.
Employee was holding steel plate in place when it slipped causing him to twist his back. Sent to hospital, released with a back strain. No lost time, wages or change of job at this time. A later MRI showed a disc problem which was operated on June 2; on this date it became a reportable and lost time accident.
There is no evidence verifying this is reportable. This report is submitted because of citation was issued. EE was opeating shuttle car with limited visibility and uneven floor when another S/C operator struck the EE causing small red spot on his arm Refused to go to Dr. worked shift. EE went to Dr.one month later. MRI showed no problem with elbow but had a shoulder spur.
Employee was laying track and he and his partner who was helping him were going to flip an 85 lb rail. EE did not have his bar in hand and went to get bar. His partner began to flip rail on his own and flipped it on top of EE's foot.
EE was operating Carerpillar 777D rock truck & left the loader with load of rock & drove over the berm & rolled 1 time down embankment approx 20 ft & landed on its tires. EE said he lost sight of the road in the dust. EE was admi to hospital & tests were run but only possible fracture to L collarbone was found.
Employee's right index finger was caught in the locker room door while closing it. It has a deep nasty cut and requires stitches.
WHILE DESCENDING LADDER OF WHEEL LOADER, EMPLOYEE LOST HIS FOOTING AND GRABBED THE HANDRAIL AND HURT HIS RIGHT SHOULDER.
While mining a lift off of a pillar, the rib sloughed off some chunks of coal and one struck individual in right ankle and foot. This caused a fracture to right foot and a severe sprain to the ankle.
Roof material fell from top and struck side of right foot.
Employee was walking down 10 belt in the secondary crusher. She slipped and grabbed onto both handrails & cracked her sternum. She didn't go to the doctor until 3/3/06. I became aware of this lost time accident on 3/6/06.
Employee was walking down stairs to office trailer while carrying out trash bag. He slipped, grabbed handrail and reportedly injured wrist. He made notification to supv. but stated he was ok. On 5/31/06, he was diagnosed w/torn ulnar collateral ligament by MRI. Surgery scheduled on 06/07/06.
EE STATED THAT HE WAS ATTEMPTING TO HAND A CONTINUOUS MINER CABLE ON THE COMPANY NO.002 SEC. WHILE PUSHING AGAINST THE CABLE TO SECURE IT AGAINST THE RIB, HIS KNEE HAVE WAY AND THE KNEE CAP WAS OUT OF PLACE. THE EE STATED THAT HE SET THE KNEE CAP BACK INTO PLACE.
WHILE DISMOUNTING FROM A 24-H BLADE, THE EMPLOYEE LOST HIS BALANCE AND LOST THE RIGHT HAND GRIP ON THE GRAB BAR. HE FELL APPROX. 7 FEET TO THE GROUND, WITH THE RIGHT LEG TAKING THE MAJORITY OF THE IMPACT. DIAGNOSED WITH KNEE SPRAIN WITH POSSIBLE ACL (ANTERIOR CRUCIATE LIGAMENT) TEAR. RELEASED TO WORK WITH RESTRICTIVE DUTY.
EE was in the process of cleaning out the dust box out of roof bolter. EE was on his knee's cleaning out the box when a rib rolled off measuring appro. 51""X56""X6""thick striking him in the head area. The force of the rib roll forced his head into the bolter, causing several cuts to the scalp and facial area. It took several stitches and staples to close wounds.
EE stepped in grease before mounting equipment. When EE was climbing ladder to the truck, his foot slipped on step. He held on with one arm and bumped his arm into the trucks fire extingusher holder.
Caught heel on step between fuel tank and door; fell about 4 feet and twisted ankle. Diagnosed with bone chip and bad sprain.
Note: Employee failed to report incident on a timely basis. The company is investigating whether the incident occurred on the mine site. Initial employee report to supervisor was on 3/6/2006. Employee claims to have twisted left foot while stepping off mobile equipment.
Unit # 2 - Employee was installing a roof bolt. Pinned finger between bolt plate and the mine roof. Injury resulted in fractures and a laceration to left middle finger. Returned to Work 03/02/2006. Lost 4 Work Days.
Sandy terrace and he stepped over pipe to hook chain and got off balance and twisted knee.
Employee was grinnding on metal inside the shop with safety glasses on when a big gust of wind blew in the shop causing dirt and metal from the grinder to blow under his glasses and into his right eye.
EE was walking to section near power center and hurt knee on uneven bottom. EE last day of work was 7/13/2006 and is still off at this time.
Driving a pin into a bull dozer nose cone and a piece of metal flew into his eye.
REPAIRING DREDGE. DISCHARGE ELBOW ON PUMP HAD BEEN LOOSENED FOR REMOVAL. EMPLOYEE REACHED UNDER THIS ELBOW TO GET A CHAIN AND THE ELBOW FELL ON HIS RIGHT HAND. THE BLOW CAUSED A BONE IN HIS RIGHT HAND TO BE FRACTURED IN TWO PLACES.
Employee turned quickly when a rock fell from the roof. He heard a pop in his left knee. He continued to work until 4/13/06. At that time he had swelling in the knee and went to the doctor.
EE was moving a Steam Jenny when the jack pin broke causing the end to fall, striking EE on top of the foot.
EE reported he jumped off miner and strained neck. EE started missing work on 3/4/06.
Employee was making a belt splice. As he stepped backward without looking, he stumbled upon the 3"" water line and twisted his left knee.
Employee was entering parts building on surface. When entering he turned and twisted right knee. Investigation revealed that there was no stumbling or tripping hazards found.
Employee was moving slack out of hi voltage cable, had foot planted against ground, torn plantar fascia.
Was pulling conveyor chain(face)back together with shear when a f-bar broke sending a hook toward the headgate and striking accident victim.
WHILE USING A HIGH PRESSURE HOSE TO CLEAR A CLOG, FROM AN UNLOAD HIS FOOT SLIPPED. HE DID NOT FALL BUT TWISTED . HE HEARD A ""POP"" AND PAIN PROGRESSED TO SEVERE. POSSIBLE TORN TENDON IN LEG.
EE WAS THREADING RAW MILL ENTRY COVER THROUGH OPENING AND HE PUSHED THE COVER TO STRAIGHTEN IT UP SO HE COULD SLIDE IT THROUGH THE OPENING. THE CHOKER SLIPPED, CAUSING THE COVER TO SHIFT AND STRIKE HIS LEFT SHOULDER (BROKEN COLLARBONE).
Employee was working graveyard shift 6.00pm on the 23rd to 6.00 am on the 24th - was leaving to start his vehicle at the end of his shift. Employee slipped on some ice and fell flat on his back. Employee returned the next day and left early and called off on next scheduled shift.
Employee was seated inside mantrip waiting to go up hill on surface track. A second mantrip exited the mine on the same track and struck the first trip. Injury not reported as lost time until 3-1-06.
Attempting to secure the quick couple vic clamp on a 2' pipe with both legs under him for support. Left patella displaced.
Employee was working on the rear brakes of a welding pickup truck. When he lowered the jack, the pickup truck ran over his ankle. His ankle was bruised.
Employee was moving gob out of a crosscut with a diesel scoop. The scoop dislodged a piece of victaulic water line which struck the employee on the right arm. The employee started receiving physical therapy for the injury on 1/25/07.
Employee reported: Operated continuous miner. He had left an uneven floor in the face area. Backed machine to start of uneven floor, standing at machine's bumper, he lowered the cutting drum. This caused the machine's bumper to lower catching his right foot. He initially returned to normal work status. Due to pain & discomfort, he started missing scheduled work days on 3-3-2006.
Draw rock fell from roof striking EE on neck and back.
Was hooking up chain to lift generator. They were ready to lift generator and employee's finger got caught in chain.
EE was using a hammer and chisel to remove a staple from plug on a new leg jack for shield in F-13 longwall set-up face. The plug came out under pressure causing the chisel to strike him in the right hand, resulting in a laceration to his right hand requiring 7 sutures. EE was wearing gloves.
Mine management's investigation revealed that no accident occurred. Employee alleges that he re-injured his knee from a previous surgery when he stepped off the push arm of a dozer.
Working on screen had screen held up with chain & comalongs. Chain broke screen came down hitting victims left hand with spring mount.
Employee was pulling on feeder cables when he strained his low back.
On Feb 23,2006 General Inside Laborer injured his L ankel while working as a section utility person on the MMU 002 working section. He was walking through the last open crosscut when a shuttle car cable drug across the entry striking him on the L foot causing a sprain to his ankle.
Repairing crusher, he jumped from conveyor to catwalk causing left knee strain and contusion.
Slipped while climbing onto Bobcat Loader. Strained back muscle. Was medical only at the time but turned into lost time accident when employee had surgery on 4/26/06.
EE was putting a rope on the tail of the miner to pull cable up. When he lifted the cable, he felt pain in his lower back.
Employee was replacing glass in trackhoe when he slipped and fell fracturing right arm.
EMPLOYEE WAS INJURED WHEN WALKING IN #4 BELT ENTRY WHEN HE STEPPED IN A RUT, TWISTING HIS RIGHT ANKLE.
EE was repairing a dredge bucket. He stepped onto the teeth of the bucket, where he twisted his ankle, causing a hairline fracture.
The employee stated he was loading a piece of curtain in the scoop bucket when the scoop operator dropped the scoop bucket and it landed on his left foot that was placed approximately 6"" under the edge of the bucket.
Road Builders Machinery was reinstalling the tracks on the PC600 excavator. When the lifting chain broke it lashed back and hit employee on his hard hat, nose and lip causing a laceration (stitches in his lip). Employee was using a bar to help move the tracks onto the rollers.
Torching ""I"" beams. Piece of beam fell onto can of spray paint lying on plant pad. Sharp corner of beam punctured can causing worker to be sprayed with paint. Paint ignited with torch flame, clothing caught fire causing burns.
EE was changing batteries on his coal hauler at the time of this incident. He stated that he hit his hear on a low area of the mine roof twice while working to change the batteries. He sought medical attention on 2/23/06 and did not return to work.
The emp. was loading bolting supplies onto the bolting machine when he began to feel pain in left shoulder.
At the end of the day the EE went to the shipping office to put on his jacket. When he put it on he felt a sting from a spider.
While entering a vessel through the mnhole, he twisted back. Finished shift, next day back tight and sore. Unable to work.
He was drilling a bolt hole when a piece of slate fell from the rib and struck him on the head, back and right shoulder. He received a contusion to the right shoulder, neck and head.
Steel hung in hole, fell out striking his right hand.
Cutting rockdust hose with utility knife and cut lt hand.
Employee was dismounting Mack-600 powder truck, slipping on last step falling to concrete pad, bruising right ankle.
Boulder hung in load chute in skip pocket. Boulder was too big to go thru the load out system getting thru grizzly. While trying to move cable, hook slipped off boulder hitting the employee across the nose. Employee was taken to emergency room where he was diagnosed with a crushed bridge and would need surgery. Employee is currently off work.
Piece of rock fell from mine roof stiking EE's left knee.
EE STATED THAT HE WAS ATTEMPTING TO HANG A CONTINUOUS MINER CABLE ON THE COMPANY NO. 002 SEC. WHILE PUSHING AGAINST THE CABLE TO SECURE IT AGAINST THE RIB, HIS KNEE GAVE WAY AND THE KNEECAP WAS OUT OF PLACE. EE STATED THAT HE SET THE KNEECAP BACK INTO PLACE.
While moving a water pump into water hole, stepped on rock and twisted left knee, did not see a doctor until 3/16/06 for treatment. Treatment will be for strained or torn ligaments with physical therapy and knee brace for support and comfort.
EMPLOYEE WAS SHOVELING BELT. HE WAS JUST GETTING READY TO TIGHTEN DOWN BOLT ON J-HOOK ON BOTTOM ROLLER. ROLLER CAME OFF AND STRUCK HIS RIGHT FOOT (FRACTURED METATARSALS).
Dragging belt and was struck on the head by a piece of rock that potted out. The rock measured approximately 11""square by 1""thick. He began losing time the next shift because of a lower back sprain.
Employee was holding the bottom of a steel plate in place for two other employees to repair a chute at Phase I, 6'x16' screen to belt#9. The two people holding the plate at the top lost their grip and the plate forced employee's right wrist into the chute frame and twisted. He has a 6 cm fracture to his right radius and has restrictions that do not allow him to work.
Employee stated the bridge operator bumped the tail of the low belt while backing up. The low belt framework moved and the framework struck his elbow.
On 2-22-2006 employee was getting into the Catipilliar IT-28. As he pushed off the ground he felt a pop in his right knee. He continued to work until 2-27-2006 and due to increased pain he took off work.
Employee stated he tripped & fell hitting his right shoulder. EE started missing work on 11/27/06.
HE WAS SITTING ON A PIN BOARD, WAITING FOR A SHUTTLE CAR WHEN A ROCK FELL OUT BETWEEN PINS. (2'x2'x3"") THE ROCK STRUCK HIM 6"" ABOVE THE KNEES.
We think the EE was cutting sleeves off coveralls when he received a self-inflicted wound to his left chest; subsquently a knick to his heart.
Employee was bolting top in #6 entry. He had put up bolt and then walked back to get a box of glue from back of pinner. When he started back to front of roof bolter, a piece of draw rock fell from between rib and bolt and struck him in the head and back and knocked him to the ground.
A rock fell out of the right side of a 992G bucket into the 777 truck bed, driven by the injured. The impact of the rock jarred the truck and the driver. The driver complained on the next shift of pain in the left shoulder and hip.
Employee standing directly under 2x2x1/4"" angle while millwrights fitting side walls of a duct collector screws housing when the angle fell 1' onto EE's hard hat. He was taken to the hospital for observation. He was released to full duty. During the night he went back to hospital & got restricted duty release. He resigned 2/24/06.
Employee was cutting bolts out and the nut for high pressure on the torch came out causing oxygen to get into the glove and ignite.
The employee was working inside the hopper of a jaw crusher on a 6' ladder when he fell. The employee was torquing the bolt on the grizzly when the wrench slipped of the bolt and he fell. He was wearing his hard hat, but hit his head, back and arms and his hard hat fell off. He indicated that he passed out for couple of minutes.
Employee put steels together and rotated steel and hit thumb. **Employee did not start losing time until 4/12/2006 due to surgery**
EE WAS PLUGGING UP OUT GOING 13,200 CAT HEAD TRANSFORMER LOCATE ON BOTTOM OF B-SHAFT, AGGRAVATED LOWER BACK (FELT PAIN IN LOWER BACK)
Motor grader tire exploded and employee nearby was hit by an air blast and debris.
EE was trimming roof at a cross-cut. A piece of draw rock fell onto his right ankle. He was taken to ER for treatment, X-ray revealed a fracture to the ankle.
While working on a feeder, dust blew into employee's eyes. Worked two more days after incident and was seen at an Emergency Room on Friday the 24th of February. Started losing time on February 24th.
Employee was walking down conveyor catwalk and stumbled. When he reached for the handrail he missed and jammed the fingers on his left hand in the catwalk.
Motor grader tire exploded and employee nearby was hit by an airblast and debris.
EE fell behind tool sled, loading material on sled, stumbled on rock, fell on piece of metal and cut right thigh.
EMPLOYEE CAUGHT HIS RIGHT PINKIE FINGER BETWEEN SIDE OF METAL ON THE CRUSHER AND PRY BAR. HE CRUSHED THE NAIL ON PINKIE FINGER.
Jumped off dozer twisting left knee.
On 2/21/06 the employee went on a personal leave and had surgery around 2/25/2006. On 3/15/2007 Vulcan Materials Company received the DWC Form from the employee informing us it was a work related injury. The employee claimed he injured lower back due to the bouncing of the truck while driving (ALLEGED).
Tightening bolts on hoses & installing high pressure pump.
EE was ascending a ladder. He stepped off the ladder rung and onto the landing. He missed the landing and landed on his right knee. On 04/04/2006, EE had surgery on his left knee to repair a torn meniscus. EE is losing work days.
The emp. alleges that while he and another employee were closing an approx. 130 lb. refractory lined door on the recoup duct when the emp. felt something pull in his back. 1st Lost Time Day 3/10/06
While striking a cable cutter with a 12 lb hammer, a foreign object flew into employee's right eye. He was not wearing his safety goggles.
Employee slipped and fell on track while prying the switch mechanism, resulting in pain in lower back. Not a serious injury.
EE stated that he fell off the platform of a D10 dozer while attempting to look over the berm. Reason for delay: Accident was under deliberation of the company for chargeablity.
Employee was cutting directly overhead when a piece of slag landed on his glove. While brushing the slag off his glove he inadvertently swung the torch across his right leg, resulting in a burn to his leg. He was evaluated by a physician and taken off of work for 2 days: 2/28/06 and 3/1/06.
Employee twisted right knee while stepping down slowly off of train.
EE turned wrong while trying to sit in a chair at the desk in the mine office. Lower back strain.
The employee was roof bolting at the time of the accident. The roof bolter had been advanced to the next row to be installed. While the ATRS was being set the injured stepped up to the operator's drill station. When the center support of the ATRS contacted the mine roof a rock was dislodged that struck the employee on the left leg as it fell to the ground.
Pulling miner cable out of roadway and hurt back.
EE was welding threaded studs from morning till lunch. After lunch he noticed numbness in his right leg from hip to foot and thought that his leg ""fell asleep"". He came into work the next day and his right leg still had numbness.
EE WAS TRAMMING ROOF BOLTER. WHILE WATCHING CABLE TAKE UP, HE HIT HIS HEAD ON RIB (STRAINED NECK).
Ee was shoveling wider the belt in mud and twisted his back the wrong way as he was shoveling mud onto the belt.
Employee slipped his foot went under head of bolting machine, as he tried to break his fall he grabbed the lever that lowered the head onto his foot.
Employee was attempting to push the tailpiece over with a prop gun when his left hand slipped, causing the prop to jerk his right arm.
Employee was pulling the plug out of the #2 scoop. The plug slipped & he fell back & struck his hand on the lightguard.
The pinner needed hydraulic oil; two co-workers went and got two 5 gallon buckets of oil from outside and carried it down slope through door to reach pinner. Once injured reached the pinner, he set the two buckets down. When he rose back up he felt pain in his lower back. Was taken to hospital at 11:00 a.m.
EE was shotcreting & realized that he had a hole in his boot. He made the choice to continue working and did not report it to anyone. That morning in the shower, he noticed a redness in his foot, but still did not report. He took a PTO day next day but did not go to doctor. On 2/22 was in hospital. On 2/23 employer was told.
Employee was walking to the elevator and he claims to have twisted his ankle. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, miner proficiency a factor, protective equip. not a factor, mining equip/systems not a factor.
EE stopped on south entrance ramp to fuel pump generator. He shifted gear to neutral. Brakes wouldn't hold & truck rolled approx 100-125' down ramp toward highwall. He was afraid truck would hit wall & jumped from cab. Spoil material stopped truck as he jumped(temp was about 20 deg) When tested, brakes worked, however system was inspected, cleaned & 1 slack adjuster replaced.
While lifting miner cable to hanging cable, sharp pain in left shoulder.
Lifting a cover & poking at tower felt pulling & pain hernia surgery required.

WHILE RIDING IN THE INBY END OF #24 MOBILE, IT STRUCK A PARKED VEHICLE. EE WAS SITTING WITH HIS BACK TO THE INCIDENT AND HE WAS JARRED
EMPLOYEE WAS STANDING ON TIMBERLINE HOIST AND FELL DUE TO SNOW & SLICK CONDITIONS, INJURING KNEECAP (BROKEN KNEECAP).
Employee is a mechanic and was passing underneath a 992 loader from one side to the other. He stood up too soon and jammed his head on the underneath of the machine. He sought medical treatment and was unable to report for his scheduled shift on February 27, 2006.
Injured walked up beside of a tramming scoop without letting the operator know that he was there. As he turn to flag the operator his left foot was ran over by the scoop tire.
The employee and others had been installing 6 inch steel waterline at belt drives to replace the existing 6 inch plastic line. After leaving work the employee stated his back was hurting and sought medical attention. The phycisian consulted adivised him to take off work for one week. The employee does not recall any specific incident where he hurt his back.
Employee was walking from the headgate to the tailgate when he hit the top of his head on a shield. Lost time injury as of September 6, 2006.
Walking down steps, slipped and grabbed railing, pulled right arm causing torn rotator cuff to right shoulder. Also abrasion to left leg calf. Disability did not start until 2/24/06.
Still dark and snowing and visibility was poor. Dozer was pushing dirt up on stockpile when truck slightly struck tractor cab.
While walking between C114 and C14 screens, worker slipped on ice on the floor that was covered with a thin film of dust. He fell and caught himself with his left hand straining his left shoulder in the process. He did not begin losing time until July 6, 2006, when he was scheduled for surgery.
Operating a front end loader carrying a large rock in the bucket. This caused the loader to bounce allowing the operator to leave his seat and be bounced around in the cab of the loader.

The employee was going up the stairs to verify the surge tank level. Before reaching the top he turned around and his hands slipped and he fell setting down between the tank pump. He injured his back.
FAULTY PROPANE CYLINDER. EMPLOYEE WAS UNABLE TO USE FRESHLY FILLED TANK. TANK WAS LEFT UNATTENDED FOR ? MINUTES WHILE HE CHECKED OTHER TASK. ON HIS RETURN, THIN STREAM OF PROPANE FROM VENT HAD IGNITED, BURNING HOSE OFF. OVERHEAD WIRING THEN CAUGHT FIRE. EMPLOYEE, UNAWARE OF PROPANE FIRE, WAS BRINGING EXTINQUISHER FOR ELECTRICAL FIRE AND WAS BURNED AS HE ENTERED THE BUILDING.
Employee was hanging miner cable and strained lower back.
EE stated he was putting a rope on the cable of the miner as the tail was being swung over. EE was struck by the tail on the back and left arm.
When sweeping, she started to turn red & itch. After awhile her throat swelled & breathing was difficult. Post accident drug screen results - Negative. Sweeping in ""dry"" area.
Injured was using a torch overhead on the belt line to remove bent bolts. The mine floor was wet with some water in the area he was standing. As he back away from his work,still looking toward it, he tripped backwards over the 6"" water line causing him to strain his rt. knee. The injury became reportable on 4/8/06 when he had surgery on the knee.
Injured was tramming a scoop up to pick up some rock dust. When he went around a corner of a block he put his left hand outside of the scoop and in between the hinge point of the scoop just outside the operators deck. As he turned the two halves of the scoop came together catching his finger.
Had belt jacked up. Come a long broke and belt came down on right foot.
While being loaded, the shovel came in contact with haul truck bed, jarring the truck. EE was seen in ER initially, then saw Dr. on 2/21 with diagnosis of neck strain.
Said putting a piece of rIb roll on belt. Twisted back, slipped and fell in mud.

EMPLOYEE WAS CLEARING TREES FOR STRIPPING. HE WAS STANDING APPROX 8' FROM THE FRONT OF THE SKID STEER LOADER WHEN THE LOADER CAUGHT A 4"" SAPPLING. THE SAPPLING CAME UP AND STRUCK THE EMPLOYEE KNOCKING HIM BACKWARD INTO A LARGE TREE TRUNK HITTING HIS RIGHT SHOULDER.
Injured employee walking up the walkway to #1 east neutralization. Called supervor. Another employee was walking beside injured employee when the accident occurred - other employee said injured employee tripped but did not fall and after that injured employee said he felt he had just broken his ankle.
EE WAS CHECKING THE WATER LEVELS IN A RADIATOR ON A DIESEL SCOOP. WHILE ATTEMPTING TO LOOSEN THE RADIATOR CAP, THE CAP BLEW OFF AND HOT WATER AND ANTIFREEZE BLEW OUT, BURNING HIS RIGHT ARM AND SIDE. 1ST, 2ND & 3RD DEGREE BURNS TO ARM; 1ST DEGREE BURNS TO SIDE.
Employee was helping build a crib and felt a pain in his right groin area.
EE WAS WORKING A TRACK OFF OF EX550. HE WAS HAMMERING IN A TRACK PIN, WHEN A SMALL PIECE OF PIN BROKE OFF AND WENT INTO HIS LEFT UPPER ANKLE. THE PIN WENT THROUGH COVERALLS, TIP OF BOOT, APPROX 11/2 INTO UPPER ANKLE. HE WAS DRIVEN THE THE ER AT NAVARO REGIONAL HOSPITAL.
Operator was operating a bob tail jeep in seam height of 26 inches. The operator ran over a rock in roadway that bounced him into the top hurting his ribs.
Employee was removing screw conveyor covers in order to clean out the screw conveyor. While moving the covers, employee strained his left leg and hip area.
Employee was helping make a belt move, lifting belt with a bar & putting structure in belt when he felt pain in lower right side of back. Went to dr had strain. He came back to work on 2-20-06, worked 4 hours and left again - will be off a few days.
Employee injured shoulder back in February and pain persists. I just received the (MRI) results, which show a Rotator Cuff Tear and surgery is required. He is scheduled for surgery on 4-20-2006. This will become a lost time on 4-21-06 (which would be his first scheduled shift for that week).
EE was backing the miner out of #5 entry and walked between the #4 mobile bridge unit and a rock that was laying against the coal rib. The mobile bridge unit swung around and caught his left leg against the rock.
EE BACKED HIS HAUL TRUCK UP TO A BERM TO DUMP HIS LOAD. THE LOAD LEFT THE BOX AND HELD THE BOX IN THE RAISED POSITION. EE PUSHED THE LOWERING LEVER DOWN AND PULLED AWAY FROM THE BERM. THE RAMS BEGAN LOWERING AND THE BOX DROPPED SEVERAL FEET, JARRING EE'S NECK. CHIROPRACTOR & THERAPY & DAYS OFF.
Installing roof bolt, when straightening bolt, the bolt twisted with bend in bolt. It pulled employee to the point he fell to the ground. At this time he felt strain in his shoulder & back.
A rock fell from top and landed on employee's left shoulder.
EE was walking toward a mandoor when he stepped into a rut and turned his ankle causing a sharp pain to go through his knee. Reportable lost time injury on March 3, 2006.
Employee was shoveling under track, slipped & fell on left knee. Knee bruised & swollen.
EE was attempting to reposition a metal section of the vent tubing so that the tube mated better with the section already mounted in the back. EE was wearing belt & lanyard but when attempted to reach a clamp he loosen his lanyard & he stepped on the rail to reach the clamp; when he reached the clamp & cable, the tubing gave away & fell. The EE fell on vent tubing and then ground.

EE was filling up fuel truck. He climbed up ladder to see if truck was full. He was climbing down and he slipped on ladder and fell on ground. He said his right shoulder and neck were hurting. Sent him to hospital (A.R.H.)
The miner was attempting to water down the #2 belt at #2 belt drive when apparently the hose got between the belt and the moving discharge roller causing his arm to be dragged around the guard and under the scraper.
Employee reached for the brake on the backhoe and felt sharp pain in right elbow, forearm and hand. Was lifting pipe prior and thinks this could have contributed as well.
While lifting the top plate off of stabilizer jack on continous miner, the plate slipped and caught the man's finger between the plate and swivel shaft that is attached to the top plate.

EE was cutting with acetylene a piece of metal. Another worker ask him to use briefly the equipment. When EE received again the torch, he grabed it by a hot point causing him a burn in his right hand.
He was having pain to his lower back from the previous day. He said the pain goes and comes but he is able to continue his shift. He elected not to see a physician and decided to wait until the next day to see how he felt. On 02/15/06, he reported for work, decided to see a doctor and was escorted by another employee.
Walking around emulsion pump - right knee caught - made a ""popping"" sound - knee strain.
REPAIRING VEHICLE. LYING UNDER VEHICLE, PULLING ON WRENCH. FELT PAIN IN LOWER LEFT ABDOMEN (HERNIA). ACCIDENT OCCURRED ON 2/15/06, WENT TO DOCTOR ON 2/20/06, DISABILITY BEGAN ON 2/22/06.
Employee stated he was unloading supplies from the section supply cars onto the scoop. While doing so he lifted a large box of glue with his arms extended too far out. When he lifted he felt pain in his right arm on the inside of his elbow. The glue box was 36"" long, 8"" high & 10"" wide. Estimated weight 30 lbs. The box was in good shape, and the plastic bands were intact.

Employee was walking around a parked boom truck to go to the fines prep plant. He bent down to walk under the boom. He hit his head on the boom and he jerked backwards causing pain and injury to his back.
Employee was installing grates on equipment. Fell off equipment approx. 10 feet, and hit the machine as he fell.
INJURED WAS DRAGGING 2 BOXES OF RESIN UP #6 ENTRY ONE BOX CAUGHT IN THE BOTTOM AND CAUSED A STRAIN TO LOWER BACK.
Rock fell from top hitting employee while he was walking the belts.
While climbing up truck ladder employee slipped on step and fell to the ground, hitting both knees.
He was rolling a wheel unit around and felt pain in his lower back.
Employee was at floor level and was moving ladder. Coworker was working nearby, 10' above floor level. The coworker was handling a handheld power tool (impact wrench). The handle on the impact wrench slipped off the tool and the tool then fell, striking employee (working on floor level) on his hard hat (strained neck).

Injured was grinding on a piece of metal when a metal partical flew up and struck the injured in the eye.
The injured employee stepped onto a 6"" discharge pipe and lost his balance. His right hand grabbed for the belt structure and was pulled into the bottom roller. The employee was able to free his arm.
Employee was coming into work when he slipped and fell on some ice outside his work area.
HE WAS WALKING DOWN RETURN TO TALK WITH FOREMAN ABOUT SUPPLIES, WHEN RIB RASHED OUT INTO ENTRY AND KNOCKED HIS FEET OUT FROM BEANEATH HIM. FALLING TO THE GROUND.
EE was climbing from ground to equipment cab and pulled upper right stomach area. EE began missing work 6/6/06.
Employee slipped while crossing metal grating in the motor barn. While catching himself before falling his back struck a jenny-coupler causing two fractured ribs.
EE stated that he was rolling a piece of rock off the water line when he felt pain in his right shoulder and neck.
While carring bolter supplies employee felt pain in his groin. He was diagnosed with a hernia. On 3/7/06 he had surgery to repair it.
EE STATED THAT HE OPENED THE DOOR ON THE HEAVY MEDIA CYCLONE UNDERFLOW BOX TO ADD SOME CERAMIC BEADS. WHEN HE DID, HE FELT A SUDDEN SHARP PAIN IN HIS LOWER BACK WHICH RADIATED DOWN HIS LEFT LEG (STRAINED BACK).
Manbus was leaving the surface when the brakes malfunctioned and the box containing the SCSRS fell striking employee in the right arm.
Employee lifted a bucket of samples and felt something pull in right elbow. It was diagnosed as a pulled tendon and was to be watched for a while. No time was lost. It was later decided to operate, which took place on June 23rd, return to work was July 17, 2006 for a total of 16 day lost time.
Employee alleges injury to left knee. Specific incident not known.

Employee had come down off the plant and onto the ground when he slipped on ice and fell onto his back. He got up and thought he was fine until he woke up the next morning and his back was sore.

SAID WAS JUST A BRUISE. EE RETURNED TO WORK ON 2/15/06. EE WENT BACK TO DR. OF THE FAMILY. TOLD TO REMAIN OFF WORK TIL 2/20/06.
WHILE HANGING VENTILATION TUBIN, A SECTION OF TUBING SLIPPED OUT AND EE REACHED TO CATCH FALLING TUBE. FELT DISCOMFORT IN LOWER BACK AND TAKEN OFF 2/20/06
EMPLOYEE WAS USING SCOOP TO PULL CONVEYOR CHAIN WHEN THE STEEL CABLE BROKE, STRIKING HIM ON THE FACE AND RIGHT EYE, CAUSING BRUISING.
While examining the belt slope, he twisted his right knee resulting in left knee pain & swelling. Diagnosis of minimal osteoarthritic changes possibly from chronic repetative stress, indicating Occupational illness due to repeated trauma. Medially displaced flap tear of the medical meniscus as described with an additional oblique tear just lateral to the flipped fragment.
Bolting top, small piece of draw rock approximately 15"" square - 1 1/2"" to 2"" thick fell and hit employee on lower back.
Employee was sitting in mantrip when the brakes malfunctioned and smashed employee's finger between the seat and batteries.
Employee was getting a drink of water from the water dispenser when a snake (side-winder) came out from under the water dispenser and struck his left leg (on calf).
I BECAME AWARE OF A LOST TIME ACCIDENT ON 2/18/06, WHEN EE DID NOT SHOW UP FOR HIS REGULAR SCHEDULED SHIFT. EE (A LUBE MAN) INJURED HIS LEFT SHOULDER WHILE PULLING OUT A HOSE REEL ON HIS LUBE TRACTOR. EXTENT OF HIS INJURY IS UNKNOWN AT THIS TIME.
The injured person was walking across face area and stepped in hole and twisted body.
While operating a 631, # 12 scraper, EE was loaded and headed to the fill. As he came out of the 8 ditch onto level ground, he lost control of the scraper and ran over a windrow on the left side of the road. After traveling about 20 yards, EE got control and crossed the windrow back onto the road to his right and pulled the scraper off the road to the right side of the road.
Working on the backhoe, got off the trackhoe, stepped in a hole and fell hurting leg and hip.
Employee was roof bolting. When he stopped to take wrench out of drill pot it was stuck. He took a hammer and started hitting the roof bolt wrench. A small piece of steel flew into his right eye. #1 entry, 002 section.
Injured was using slate bar to move rock off miner when rock rolled causing slate bar to smash finger.
EE was bolting roof. When he let canopy down a piece of draw rock fell scraping right side of face, shoulder and foot.
Employee was lifting the chock block into the holder when something popped in the left shoulder. Felt some numbness in left hand. Employee went to a doctor during days off. Doctor did not see anything. Employee continued to work for about a month. Then went in for exploratory surgery on 3/13/06.
Employee was exiting the cab of a drill. He was walking on a platform when his left ankle was caught between the cab and a deck wrench lying on the deck. Employee twisted his ankle causing a sprain.
Slope Belt - Employee was resetting pull cord. In the process of getting down off the belt, employee slipped, resulting in a strain/sprain to right leg above and below knee.
Employee was working on the emergency brake on a Caterpillar TC-30 Forklift and strained a muscle in his neck. He reported it on 2/15/2006 and went to the doctor on 2/15/2006 and a follow up visit on 2/17/2006 when the doctor put him off duty till further notice.
Employee was removing a metal cover on the face drill when he felt a sharp pain in his lower abdomen, resulting in a hernia/rupture.
Workers were moving a floatation pontoon with a Trackhoe and as they were setting it down the employee grabbed it to steady the position. The pontoon swung and bumped him on the shin of his right leg causing a contusion.
While tightening axle cap, axle cap bolt broke causing employee to fall.
While using a shop saw to cut a piece of metal, the employee sustained a laceration to the right index finger.
The injured EE was changing a screen at the plant and was sliding the replacement screen over another screen to get it into place. There was slack in the come-a-long and his right middle finger was caught between the screens requiring stitches.

I BECAME AWARE OF A LOST TIME ACCIDENT ON 2/14/06. EE WAS LOADING A TOP SLICE ROUND WHEN HE SLIPPED AND FELL ON A ROCK (WET ROCKS ON MINE FLOOR), HITTING HIS KNEE. EE FINISHED HIS SHIFT, BUT DID NOT RETURN FOR 2 DAYS. HE THEN WENT TO THE DOCTOR AND GOT A RELEASE TO WORK ON 2/18/06. HE WORKED ONE SHIFT AND DID NOT RETURN FOR THE REST OF THE WEEK.
EE was lifting a pipe when the pipe hanger broke, causing the chain on the come-along to break, which allowed the pipe to fall on EE's shoulder.
The employee was moving the trailing cable on a 15RU Joy Undercutter. During the procedure, the employee slipped. He caught himself and doing so he hurt his neck.
A mack tri-axle dump truck was backed into the shop for service work. Employee walked in front of the truck. The truck driver started the motor with the truck in gear and the truck moved forward pinning employee between the truck bumper and the garage door.
Roadway, Past Cowling Barn - Employee was driving mantrip, just before crossing under belt, the top fell out from between bolts, striking employee on the left forearm. The piece of rock was approximately 19"" x 19"" x 2 1/2"" thick causing a severe hematoma. Possible injuries to left upper arm and shoulder also.
Employee developed a headache & felt sick. Claims it was fumes in the cab. The carbon monoxide detector that is mounted in the cab did not go off. The employee was taken to the hospital and was found to have carbon monoxide in his system. He was told to rest for the day. No medication was given.
EE had a generalized seizure and fell to the floor striking her face on a pallet-resulting in a contusion to her face.
Employee was trying to move a piece of concrete with a shovel. Concrete was bigger than he realized - poor judgement.
While walking between #9 and #10 crosscuts employee twisted her leg behind her knee. This accident did not become lost time until 04/12/2006.
Twisted left knee in deep mud changing structure on #1 belt.
An accumulation of acetylene in a storage cabinet ignited and exploded injuring an EE's right heel.
As EE was coming down the front ladder of a CAT 793 truck, a welder working 15 feet away struck an arc, causing him to see ""spots"". The next morning his eyes were swollen and felt like they had something in them. He went to the doctor and was diagnosed as having corneal ultraviolet burns,
Subject & co-worker working with vent pipe removal. Vent pipe improperly secured. Subject was struck by moving pipe which then caused him to fall to the floor. (Same level fall)
EE operating remote control miner and just moved to a new cut(X-cut 9-10 entry). He moved the miner cable out of the way and at this time a piece of rib (3'X2'X2' thick) fell from the left out-by corner. The piece of rib struck his right leg from the calf down. X-ray showed 4 hairline fractures in the right foot. Walking foot brace applied. Follow-up 2/13/06. 1st day lost 2/10/06.
Miner operator sitting under bolts, running miner when rock fell on boom. Carried to midways, spun around in the bend in conveyor boom, came over the side and hit him in head.
While climbing off a rock conveyor, he stepped on two large rocks on the ground, twisting his ankle.
Climbing off flatbet trailer, he slipped, landed on elbow jamming into shoulder.
The employee was roof bolting when the operator informed him of a thin loose rock above his canopy.The employee took his drill steel and knocked the loose rock down, when preparing to drill his next hole a rock fell out of the top striking his canopy then struck the employee driving him to the ground.Once on the ground two smaller rocks feel striking him in the lower back and leg.
Employee stepped on uneven bottom and twisted right knee.
Employees were about to finish their shift when a part of the steel structure gave way causing the employee and the ladder the employee was working to slide 16' and reached hopper openings (which were covered with plywood - the concrete broke through) and fell through and landed approx 20' below the bin bottom floor (to ground level)
Dry weather, dry conditions. Employee was servicing loader. While climbing down ladder, his foot slipped off step causing a mild sprain to his right ankle.
Employee was cleaning buss bars on hoist, ladder slipped causing him to fall approx. 15'. Injury resulted in a fractured left foot/ankle.
The employee was hanging a ventilation curtain at the long wall head when a rock fell and struck him on the right leg.
While bending roof bolt, EE slipped and fell, striking his chin on a roof bolt plate, resulting in a cut on chin.
EE was cleaning the mine floor in preparation of setting the next timber when a co-worker accidently dislodged a previously installed timber. The timber fell and struck him in the back.
Employee broke bolt loose and it snapped, hitting him in the eye under safety glasses.
He reached across the conveyor belt to turn on the belt when it grabbed his shirt pulling him off balance onto the conveyor belt causing his right hip and right shin to hit the crossover when his body passed under it. ER put hip back in place (manually) and stitched his cuts.
Cleaning a cone crusher. He slipped and hit his knee on crusher.
Employee crawled under dozer to check problem. Another employee inadvertently lowered blade of machine onto back of employee who was under machine.
#7 & 8 crosscut, went to drill hole & piece of cap coal hit him in back of head & neck.
Was cutting scrapping out of the bottom and cutter chain grabbed the bottom and kicked back and briefly pinned him against the rib.
Employee was reaching behind the seat when his upper back muscles knotted up causing severe pain and numbness in his left arm and hand. Employee went to emergency room and was admitted overnite. Employee came back to work until 02/21/06 where he wad diagnosed with a ruptured disc in his neck.
The mill repairman was rebuilding screens on the mill 4th floor when he burned off a bolt causing the lower screen deck to fall six inches onto his left foot. His foot was badly bruised and this incident resulted in lost time.
While working under VC48 dryer, the metal brace welded to chute gave way causing right hand to be caught in v-belt under dryer.
Employee was helping rock dust, standing by scoop when operator went to move scoop and backed over his foot. He went to the hospital; big toe was broken.
Member was adjusting the cat pads on the miner when a chain rachet slipped off of shim and end of chain rachet struck him on the left cheek.
Employee was walking to welding shop; slipped on light covering of snow/ice and fell.
EE was walking by loading dock, door was opened and blew dust from new wood pallets into his right eye.
Employee had hood up on company truck to check engine oil when shocks failed and hood fell onto employee's back. Employee worked his regular shift on 2-8-06 and 2-9-06. He worked 3 hours on 2-10-06 and then went to the doctor.
The employee has been diagnosed with degenerative changes in his cervical spine area between C5-6. **Employee Stated he hit his head against a roof bolt. He has done this numerous times.**
Right knee swelling while crawling in mine, possibly from small rock lodged between knee pad and knee.
The employee refueled the water pump and started the pump. He walked around the pump to see why it was not running when he arrived. He touched the radiator cap and it blew off, spraying his face with hot antifreeze.
WHILE DRAGGING A 16"" X 10' VENT. TUBE UP TO THE FACE BY HAND, EE WAS TURNING THE TUBE AROUND BY LIFTING ONE END. AS HE TURNED THE TUBE, HE TWISTED HIS LOWER BACK WHILE LIFTING AND PULLED A MUSCLE IN LOWER BACK.
HE WAS JUMPING UP AND DOWN ON CC VIBRATOR,TO START WHEN HE TWISTED HIS KNEE. * STARTED MISSING WORK ON 03/22/06*
Employee was walking, stepped in hole injuring left ankle.
While dismounting a D-11 dozer EE noticed the last step on the push arm was filled with dirt and stepped a distance of 27"" to the ground. EE experienced pain in his upper right thigh/hip. Medical notes are not yet available. Obesity (body mass index of 54.6), mobility limitations and repeated mounting and dismounting over 32 years are contributing factors.
EE was taking morning break. Walking in the canteen, his right ankle rolled and he fell. Caused fracture of leg and torn ligaments.
Injured strained his left knee while pulling the miner cable.
Employee and section foreman was attempting to move miner cable out of the haulageway. When He pulled on the cable, he sprained his back.
EE got upset at the job and pushed open the exit door, south end of the ground floor hallway at Buckskin mine. This action resulted in an injury to his forearm, a laceration which was stitched up at the hospital.
EE was shoveling belt line and pulled muscle in lower back
EE WAS SPLITTING A PIECE OF STONE AND HIT HIS HAND WITH A HAMMER
While roof bolting, a small piece of rock fell from the roof and struck his nose. He started missing work on 5/15/06 and had cosmetic surgery.
Drilling hole for roof bolt, the operator reached out and grabbed while still spinning, twisting left hand, fracturing hand. Note: Empoyee took drug test on 2-6-06 and results came back positive for drugs on 2-10-06 and employee was terminated per company policy.
Employee was rock dusting #1 unit. As another employee backed the MSA duster through a curtain, an inoperative scoop was parked on the other side. The employee yelled to the operator and when the duster stopped it caught him between the batteries of a scoop and the duster causing a hairline fracture to his pelvic bone.
Employee was working in the slope when he slipped on the wet floor and felt burning in the groin area. Lost time injury as of 2/14/06.
Running endloader # 2 over uneven road causing bounce and pitch which caused back to hurt.
Stepped off dozer onto frozen ground and twisted ankle. Referred to specialist who removed him from work.
Operator was helping load a utility truck with 4x12 sheets of sheet rock. (Operator continued to work until 2/13/06) (2007100030-the operator was off again on 10/31/06 Short term Diability. Negotiation have been ongoing between the Workman's Comp. Carrier and the Doctor as to whether or not this injury is related to the initail incident.)
EE was breaking up frozen material from bank. Large chunk sluffed off filling loader bucket- jolted loader upwards. EE was bounced up, hitting head on roof of loader cab.
Employee was picking up tongue on rail truck and pulled muscles in his back. He worked until Fri, missed Fri and Sat.
Employee on loader checking radiator, lost balance, pushed himself off and landed wrong on left knee. Tore the meniscus on his left knee.
Stacking 24"" vent tube in crosscut, felt ""pop"" in left shoulder, strain.
Employee was getting up on dozer to add engine oil when he slipped on the track and struck his head on the track (contusion to right eye).
The employee stated that he was using a scrapper tool to clean spillage off of the pans below the top belt. He struck a bottom return belt roller whith the scrapper in an attempt to remove some build-up when the scrapper caught between the roller and the moving belt, causing the tool to turn sharply in his hand and tearing his glove and skin on his right thumb.
Employee claims he felt hot flash in lumbar area while handling continous miner cable. Area where accident occured was four feet high slightly sloped to left, bottom was wet. Employee worked entire shift, did not report injury until arriving at residence two hours later
Was scrubbing walls. A basket fell onto her neck and shoulder.
The employee was operating the continuous miner when a rib sloughed off and struck the employee in the right leg, causing a strain to his leg.
The employee has been diagnosed with degenerative changes in his cervical spine.**Employee thought he pulled a muscle while loosening bolts.**
Employee was using a large wrench to loosen up conveyor belt. The employee then slipped and twisted his back.
EE alleges he slipped and fell while crossing under a belt and struck his back on the track or blocks.
EE was examining the 4 east seals and slipped in a muddy area, causing him to twist his right knee.
Strained elbow using repetitive motion in pulling cord to release dump mechanism on truck bed.
Employee was installing an oil cooler on 2005-2102-02 tower mill when he must have pulled a muscle in his left shoulder. Employee noticed his shoulder bothering him the next morning and reported it to his supervisor that evening and was sent to the medic. Employee reported back to work.
Employee was washing salt inside shaft about 580' from collar. While moving spray gun his left arm was hit by a loose piece of salt from above. Fractured L arm. Started losing time 2-7-06.
Employee was lowering a discharge line to repair a bad coupler. While lowering line the coupler blew apart, causing water and air under pressure to strike him in the face and both eyes, and threw him back against the coal rib. Employee sustained a fracture to the bottom orbit of the left eye.
EE planning to ascend outside stairs. While turning body to do so, his right ankle rolled, causing pain and swelling in right knee.
While climbing the stairs he slipped and he hit the front of his head with the handrail.
Operator bent down to latch a door on a tramway bucket. As he bent down he felt a pain in his lower back; he closed the door and returned to the control room. The pain persisted so he went to a chiropractor. He returned to work the next day but was not able to complete shift and went to the ER. He was perscribed pain medication and was told to rest.
Employee was greasing the screen belt drive when he caught his coveralls on the scrapper bar causing him to twist his lower back. He slipped and fell against the railing.
While EE was laying in scoop bucket using rock duster hose, scoop bucket hit hyman beam jack, beam fell but steel rope caught it. EE said it hit his back a little
Employee was barring a rock from behind screen wire. When rock fell, employee lost grip of scaling bar, bar flipped up and struck employee in the head. Not released to work per his physican treating employee's complaint of dizziness.
Injured stepped down from a flat car into a tire rut in wet uneven ground twisting his left knee.
Rock duster off track and using air bags to lift blockings. Dusted, then tried to straighten tracks. Duster slid about 6 inches, catching right leg between ankle and knee.
EE was struck on the back by a rock brow that broke out from around roof bolts (10-12"" thick, 18-48""wide and 90-96""long). EE sustained fracures to left ribs, lower back and neck. Accident occurred on the MMU 003-0 section.
Employee was assisting in installing beams, took off running, tripped and fell injuring his back. Employee started missing 02-06-06.
Man was walking in pit area and stepped into a hole full of water - twisted right ankle. Foreman took him to hospital for x-ray. Right ankle was sprained.
The emp stepped off one step on to concrete floor at s/r dump. The ee twisted his left ankle.
EE was struck on top of his head by drawrock which resulted in a neck strain, while emptying the dust box. EE worked until 2-7-06 then started missing time.
Employee was pushing forks together on forklift. He lost his footing and his left hand went behind the fork. The fork came down and caught his hand between the fork and the frame.
Hernia--EE notified not to do any heavy lifting.
Miner offered to help truck driver open frozen hatch on top of truck. He climbed on truck to shovel spill; the hatch opened and he fell in, injuring his left ankle.
While pushing golf cart the steering wheel turned at a high rate of speed hitting employee's finger.
The emp. was walking along on 7th floor of prep plant. The emp stepped on a floor drain cover causing strain to rt. knee.
Employee was climbing down off of conveyor belt (2-3' off the ground). His foot slipped off side, twisting and causing a fracture to left foot when his foot hit the ground.
Worker was moving a piece of stone when it fipped and he strained his back.
Employee was loading a drill feed jack on a pallet with a forklift. He go off forklift to shove jack off forks to a pallet and jack slid off forks striking employee on left leg and ankle. He also complained of lower back pain. He started missing work on 2/20/06.
EE slipped and fell - spraining right ankle. EE began missing work 2/7/06.
