Roof fall on the Main Southeast between entries 4 & 5 at 14+70. The entries were bolted with 5ft grouted bolts and will not be cleaned up. The fall does not affect the passage of men or the mine's air course. The fall measured 25ft long and 18ft wide with a thickness of 10ft. Originally, developed 11/14/03.

Airway:return. Size: l - 50', W-19',H-9'. Type and length of support: resin 48""/72"", cribs, 5 cribs installed prior to fall. Strata: sandstone, mixed, laminated. All entrances are breakered off. Area will not be rehabilitated.
A roof fall occurred measuring 0-6' high x 20' wide x 30' long beginning in the #7 heading lick fork #3 mains, break 35, right return, 90' outby spad 1948, 4,900' outby face. No equipment was involved, no ventilation was disturbed, no injury occurred and the will not be cleaned up.
A false roof had been installed in the belt entry of 8 west between x-cut 30 and 32. After a roof fall on 12/15/2005 at x-cut 31, a rib fall above the false roof damaged the false roof between the stage loader and the face.
A hot spot occurred in the 4th west off of the 1st north at approx. #38 room on the left side of the panel.
A ROOF FALL OCCURRED IN THE 3RD WEST, #8 ENTRY, 2520' STATION. THE FALL WAS APPROXIMATELY 25' LONG, 25' WIDE AND 8' TALL. THE AREA WAS BOLTED WITH 6' AND 8' POINT ANCHOR BOLTS. THE FALL DIDN'T AFFECT VENTILATION, TRAVEL, CAUSE INJURY, OR DAMAGE EQUIPMENT.
XCut 4-3 mined to the depth of 20', no break above 3'-4' where encounterd an area that was supported with 7' Installs and 12'cable.
The cave 480' outby the face of the B-Main Section started in the intersection of the belt entry, running through the crosscut belt through the L-1 entry and stopping approximately 25 feet from reaching the L-2 entry. The caved area is approximately 20 feet wide and 95 feet long. Four foot bolts were used in the affected area, and the cave is approximately 4 feet high.
A roof fall occured in-by the feeder location at intersection 19+80 #6W. The area was initially bolted with 6' fully grouted resin bolts. The fall was estimated at 10' high. Water was exposed at the 10' level.
Unintentional roof fall occurred in #3 entry at spad #1183. 20' x 20' x 6' height. Type of bolt used 6' torque tension and 8' point anchor.
Emergency stop switch not turned on, hoist rope unspooled from drum, caught cable in sprockets & damaged cable.

On 1/27/2006 a roof fall occured on the #1 working section of white queen mine. the fall measured 90' long X 18' wide X 10' thick. no injuries were reported. The fall was cleaned up and re-bolted with 6' fully grouted bolts, 8' and 12' cable bolts.Additional bolts were installed on all sides of fall.

An unintentional roof fall occurred on 2nd Northwest Mains, #4 entry spad #573 20' x 20' x 6' to 8' high. Appeared to be shalie weather roof in intake aircourse. Not affecting escapeway. Type of roof bolt used was 6' resin.
Rock fall occurred in the #5 entry of the #12 Submains starting just inby break #54 and falling thru break #55 and stopping just outby break #56. Fall measured approx. 20' wide x 60' long x 5' high.
Substation on surface caught fire could not be extinguished. Internal comments of the substation were destroyed by fire.
Hoist malfunctioned, slope car works on manual not automatic.
A roof fall occurred on the 3 Left - 4 West section in entry #7 at crosscut 30 to 31. The fall was located in an extra entry of #5 units intake. When mined the area been recognized as a weak area and cribs had been set. Water was present in the area which helped contribute to the fall.
On the #1 panel off #5 Belt, near Spad 1263 between #1 and #4 entry on 1-26-2006 a roof fall was discovered. The fall measured 130' long X 20' wide X 6' Height. Fall occured in an out area. Area is only traveled on a weekly basis. 4' resin bolts were being used. Area was cribbed and timbered off. Ventilation was not disturbed.
On #1 panel off of #5 belt near spad # 1261 a roof fall was found on 1/26/2006. This area is traveled on a weekley exam. (Outby Area) No injuries occured and ventilation was not disturbed. Area dangered off and cribbed. Fall was 20' long X 20'wide X6' high.
Brakes locked up on brake car 300 feet from top. Adjusted speed sensor and put brake car back in service at 2:00 pm. on 1/25/06.
A roof fall occured in the 3rd west, #8 entry, 3990' station. The fall was appr. 25' wide and 8' tall. The area was bolted with 6' and 8' point anchor bolts. The fall didn't affect ventilation, travel or cause injury. Fall did cover the fuel car.
Minor went into inactive heading 3150 panel 48 to get ready to backfill. EE noticed left rib area getting loose. Notified shifter and area barricaded off. Day shift, foreman and engineer went to this area to investigate. Sometime between late nite and early day shift, there was a fallof ground from the left rib.
Roof fall found in the abandoned area of the 004 section. Fall was in the 50th crosscut in the no. 7 entry of the previous no. 7 belt, measuring 20'x 20'x 8' thick. This area will be cleaned and re-supported.

The Tyro Creek Service Elevator was out of service on 1/24/2006 from 6:15 am until 7:30am due to a defective limit switch. Bagby Elevator Co. replaced the switch and the elevator was back into operation.
While examining returns & bleeders, I found a roof fall on the 5 north 1 left panel in the x-cut from #6 to #12 entry at the end of this panel. The fall is 400' long, 16-18' wide, 8'10' high. The area has been dangered off and breakers set. It will not be cleaned up and is not obstructing air or travel.
Roof fall occured in 1 South Mains in #5 entry near spad 74. Fall was 50' 20' x 5' and discovered on 1/24/06 at 3:30 pm. Crosscut was cribbed off and does not effect escapeway.
A roof fall occurred on #5 unit, #5 entry @ the face. The fall measures approximately 6 feet high, 18 feet wide, and 20' long. The area is supported with 5 foot fully grouted resin bolts.
Entry No. 7, spad No. 1482 - 2 breaks inby. Airway-intake; Size of fall: l -50', w- 18', h - 4'. No cribs or timbers installed prior to fall. Type of strata: shale, laminated, wet. No coal rider seam present. All entrances are breakered off. Area will not be rehabilitated.
Unintentional roof fall occurred in #5 Entry- 1 break inby spad #15127. Measurement: 20'(L) x 20'(W) x 6' to 8' (T).

Wet shale seperated with a rider seam unintentional roof fall occured in intake aircourse 20'x20'x6' thick appeared to be weathered and shalie roof.
Found roof fall in old area of mines during a weekly exam, not sure when it fell. MSHA inspector present during exam. Belt entry 5 to 4 5NW G panel spad 7391(to the left)
A roof fall occurred at 5+60 on the 1B Submain belt line. The fall is 30' long, 18' wide, height is unknown.
Unintentional roof fall in # 1 Entry from spad 1998 to 1696. Measurement: 35'(L) x 20'(w) x 6'(T).
A ground fall occurred at teh 4150-5740 heading. The fall of ground was approximately 22 feet high and 15 feet wide and buried the front deck of a MacLean bolter. The bolter operator had completed installing 3 rows of 12 feet roof bolts in the secondary heading and was in the cab to back the machine out when the rock fall occurred. The operator was not injured in this incident.
The elevator in the Galatia North shaft became disabled. A wire burned into two pieces on the brake motor causing the leevator to malfunction. Repairs were made by 2:15 A.M.
Rock fall occurred between break #15-#16 in the 1st North Mains in the #5 entry. Fall measured approx. 30' long x 3.5' thick.
D11R, company # 0453 caught on fire from what appears to have been an electrical short under the cab of the dozer. The dozer burned for over 30 minutes and was reported to the local MSHA office.
A ROOF FALL OCCURRED INBY THE FEEDER LOCATION AT INTERSECTION 16+60 #5W. THE AREA WAS INITIALLY BOLTED WITH 6' FULLY-GROUTED RESIN BOLTS. SECONDARY SUPPORT WAS ADDED BY INSTALLING 10' CABLE BOLTS (VERTICAL). FALL PEAKED AT 15' HIGH. WATER IS BELIEVED TO BE A FACTOR IN THE CAUSE OF THE FALL. A SLIP WAS EXPOSED AT THE 10' LEVEL.
A roof fall was found in the 5 west panel from the #2 entry to #4 entry at spad 24235.
A fire occurred at the motherbelt storage unit that was not extinguished within 30 minutes.

The Oakdale Portal elevator was inoperable from 11:23 pm to 1:25 am. Nineteen employees were entrapped from 11:23 pm to approx 11:50 pm. No injuries. Door switch caused contactor overload. Repairs made by Schindler returned to service at 1:25 am .
AN UNINTENTIONAL ROOF FALL WAS FOUND AT 2:00 PM ON 1/18/06 IN THE #5 BELT PANEL IN #5 ENTRY, APPROX. 150' INBY SPAD 2282.
A fall of roof occurred in the #3 entry, inby #16 cross cut on West Mains section. The fall was 16 ft wide, 25 ft long, 6 ft high. Area was supported to plan by 6 ft double lock bolts. No injuries or equipment damage.
A fall located at #41 crosscut between the #2 to #3 heading was discovered by a fireboss on his weekly walk. The fall was 8 feet wide by 30 feet long and ranged in thickness from 0"" to 48"". The fall will be breakered off.
A series of roof falls occurred between rooms 1 and 5 at survey station 4+70 at return room neck in 6LMS unit. Area had been mined November 25, 2005. Equipment had all been moved out of this set of rooms and mining is currently being done eleven x-cuts inby this area. Work is being done to secure area with secondary support, then seals will be built across these room necks.
A roof fall occurred at the Spirit Mine on 01/18/2006. The size of the fall was 20'x 12'x 8'. 6' Torque Tension support was being used. All entrances have been breakered off and the fall will not be rehabilitated.
While performing weekly inspection of #2 ore shaft, the control relay K-15 failed closed which prevented a resistor from limiting the current to the main loop contactor close coil. The coil overheated and failed. This caused the main loop contactor to open and shut down the hoist. Repaired at 13:40.
On this date a roof fall was found in #1 entry three breaks inby spad 411. The fall is 20'x20' and 5' in thickness. Ventilatioin was not impaired.
On 1-10-06 at 9:05 am in the old section, spad #10159 off southwest mains a roof fall was discovered. This fall is in the old pannel and did not impead travel or ventilation. The fall measured 40' in length. 20' in width, and was ranging from 6"" to 6' in height.

The fall occured in the no.2 entry approx. 250' out of the face. measuring approx. 8'x20'x25'. the intersection was in a fault area.

Employee was locked inside O'Sepa ductwork for about 2 hours. There were no injuries.
1/16/06, an unintentiona roof fall occurred in the #8 return entry at the #6 crosscut of the #4 working section. The area was supported with 6' fully grouted roof bolts, the roof fall measured 18' x 18' x 10' H. This roof fall will not be cleaned up.
A new clutch was made and installed on #1 hoist, maintenance was just replacing a part that was worn out. The time frame to install clutch was too long so the mine was evacuated until repairs were made. Down 7:30 a.m. to 10:00 a.m.
THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES WITH NO ENTRAPMENT OR DAMAGE TO EQUIPMENT BECAUSE THE DOOR SWITCH BOX ON PORTAL BOTTOM BROKE AND NEEDED REPLACED.

A roof fall measuring approx. 55' x 10' x 6' in the belt entry between x-cut 30 & 31 on 002-0 section. The area had been bolted with 4ft.fully grouted roof bolts and the fall was caused by a hidden slip.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE #1 WORKING SECTION. THIS ROOF FALL OCCURRED AT A 4-WAY INTERSECTION LOCATED IN THE LAST OPEN CROSSCUT, THE AREA WAS SUPPORTED WITH 10'POINT ANCHOR ROOF BOLTS. THE FALL MEASURED 18'X 20'X 20'H. THE FALL WILL BE CLEANED UP & THE AREA RE-SUPPORTED.
At 8:10 am the underground AMS system alarmed comm. failure on the 4E#2 belt. Two mechanics were sent to determine the problem and within 60 min. the location of the fall was known. All persons UG were accounted for. Fall was app. 20'x30'x5'high in intersection on belt at #100xcut of 4E#2belt
Afternoon Shift Shut belts off at about 3:20 to replace a bad roller. After the roller was replaced the belts were restarted and the beltmen checked belts as they were starting, all started down to # 16. Beltman went into # 16 drive and tried to start the belt but it would not start. A beltman went to the tail to check for a plug and found the roof fall (4:20 pm)
A roof fall occurred in the 13 North Panel #8 entry in the intersection of cross-cut #3 and continuing through the intersection of cross-cut #6 measuring approximately 220ft. x 19ft. x 5ft. The area was supported with 5ft. tension rebar bolts. An undetected slip was determined to be the cause.
Fall occur in a 4-way intersection in the No. 3 belt entry 100 ft inby spad No. 17, 1600 ft. from the face. Rock is 30 ft. long by 20 ft. wide by 6-8 feet. thick. This area had been bolted with No. 5, 48 in. fully grouted bolts in April, 2005. Some surface cracks are present.

A roof fall measuring @20'x18'x9' fell in the #2 entry of the #1 unit. The fall was in the 2nd open x-cut outby the face area.
A roof fall occurred in the first north submain entries off the main east entry 3 crosscut 40. The fall measured approximately 16' wide x ? longx thickness ? The fall did not result in any damage to equipment or property.
A ROOF FALL HAS OCCURRED IN OUR REHAB SECTION (MMU #017-0) IN THE #4 ENTRY OF THE A MAIN DEVELOPMENTAL AREA, APPROXIMATELY 260 FT NORTH OF CEDAR CREEK ELEVATOR. THE FALL MEASURES APPR. 21 FT LONG X 16 FT WIDE X 8 FT HIGH.
A NON INJURY ACCIDENT OCCURRED WHEN 4 MINERS BOARDED THE A SHAFT HOIST & STARTED UP THE SHAFT. AS THE HOIST ASCENDED A NOISE WAS NOTIFIED AT ONE OF THE WEAR SHOES FOR THE GUIDE HAD BROKEN BOLTS WERE OBSERVED LYING ON THE FLOOR OF THE CAGE. WHEN THE CAGE REACHED THE SURFACE MAINTENANCE NOTIFIED THE SHIFT SUPERVISOR & THE CAGE WAS REMOVED FROM SERVICE. PUT BACK INTO SERVICE 6:15AM
12:45 am on the 001 section a fall measuring 20'x24'x3 1/2 to 4 inch thick occurred in the intake. fall was reported immediately. area has been cleaned up, and additional support added.
A roof fall occurred in the #11 second west main in #5 entry at xc 54, spad 38+86. The fall originated in an intersection and measured approximately 20 x 30 x 7.
THE ACCIDENT OCCURRED IN #1 SIDE SHAFT COMPARTMENT APPROX. 428'BELOW THE 900 LEVEL. AS THE SKIP WAS LOWERING THE 1ST CREW FROM 1085 TO 50 LEVEL, THE SKIP MAN DOOR OPENED PARTIALLY & STRUCK A SHAFT SET CAUSING THE DOOR TO BE PUSHED UP INTO THE SKIP. THE INVESTIGATION HAS REVEALED THE DOORS LOCK MECHANISM FAILED ALLOWING THE DOOR TO MOVE. MSHA WAS IMMEDIATELY NOTIFIED.
A bounce occured that disrupted ventilation. The face fan was displaced from the force of the bounce and had to be moved and ventilation re-established.
An area approximately 20 feet wide by 45 feet long by 7 feet high fell in a return entry. The top in this area consisted of laminated shell and mud stone. The fall area was wet with water dripping from the fall area.
Brashear Coal Mines in no way accepts responsibility for the mine fire. This fire is the responsibility of the Contractor, Tri Star Mining. They were told several times to extinguish this fire, which they refused to do. However, Brashear Coal Mines is filing this report and will submit a plan for extinguishing said fire since it is on our permit and it is our coal which is burning.

FALL HAS NOT IMPEDED TRAVEL, VENTILATION OR ESCAPEWAYS. AREA WAS BOLTED W/5' RESIN BOLTS PRIOR TO CHANGE IN OPERATOR'S ROOF PLAN TO 6' RESIN BOLTS. AREA WAS TIMBERED & DANGERED OFF.
A roof fall occurred in the 3rd west #2 entry, 3570' station. The fall was approximately 19' long, 18' wide and 8' tall. The area was bolted with 6' and 8' point anchor bolts. The fall did not affect ventilation, travel, damage equipment or cause any injury.
A roof fall occurred on the 2L4W panel in entry #1 at crosscut 33 to 34. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was located in the return aircourse of unit 003.
Somewhere between 1:30 PM and 3:30 PM approximately 50 tons of rock fell from the roof impeding the main haulage way. The rock fell at 5 West daylight a.k.a Roof Grid C-3. When the fall was found the area was barracaded from entry. During the night more rock fell bringing the total to 85-100 tons.
A methane ignition occured in the bottom of the pocket at the production shaft. Miners cutting a bolt off of the head wiper on main belt when a spark fell into the loose coal and ignited a pocket of methane.
ROOF IN THE 18 RIGHT TAILGATE HAD FALLEN AT THE TAILGATE CORNER AND PULLED THE ROOF MESHING DOWN AS A BARRIER TO GAIN ACCESS TO THE TAILGATE TRAVELWAY. TRAVELWAY BLOCKAGE RUN IMPLEMENTED AND CONDITION WAS CLEARED BY 1/12/06.
A roof fall occurred in the #1 East Mains, break 53 in the return entries of #1 & #2. The fall measured approximately 5' thick x 20' long x 80' wide. This area is bolted with 5' glue bolts and has cribs set between 2 & 3 entries.
There was a roof fall measuring 40' long x 20' wide x 5' high.
A fall of roof occured in 4-d iac h3 & 20 (5-d longwall tailgate) the fall blocked travel out of an alternate route off the longwall. The fall was approx. 16-20 ft high, 100 ft long and 15-16 ft wide. the roof had a visible clay veing and was supported by straps with 8 ft double lock bolts, and lincoln lock cribs drun center of entry.

An unintentional roof fall occurred on the 1st South Panel between 32XC in the intersection of the #4 entry. The roof fall was approximately 45' long and 13' wide and approximately 5 1/2 to 6' high. This area had been flagged off due to adverse roof conditions.
A ROOF FALL WAS FOUND IN THE #5 ENTRY OUTBY THE #5 SEAL ON 5 NORTH MAINS. THE FALL IS APPROXIMATELY 150' INBY SPAD #9473. THE FALL IS APPROXIMATELY 5' THICK X 20' WIDE X 40' LONG. THE AREA HAD BEEN SUPPORTED WITH 5' RESIN BOLTS.
ROOF FALL 10'H X 20'W X 50'L. INTERSECTION OF #2 ENTRY & 15'TO THE LEFT + RIGHT OF INTERSECTION. CAUSED BY MINING IN 3 SEAM 150' BELOW BARRIER BLOCK.
The fire started from the result of a cloth tarp being used as a weather shield to prevent a compressor from freezing during cold weather. The tarp made contact with the muffler of a compressor and caught on fire. The wind blew the burning tarp off the well pad and into the grass causing the grass to ignite. The compressor is the property of Anadarko Petroleum Company.
Area has very questionable roof with layers of laminated shale and sandstone with streaks of mud & water at various locations. Fall was approx. 80' in length & 8'-14' high & 20' wide.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 1ST N. SUB-MAINS, XC-19, ENTRY NO. 5 APPROX. 12' L x 10' W DOME. THE AREA WAS BOLTED W/5' RESIN GROUTED BOLTS PRIOR TO OPERATORS CHANGE IN ROOF CONTROL PLAN TO 6' RESIN GROUTED BOLTS. THE FALL HAS NOT IMPEDED TRAVEL, AIR OR ESCAPEWAYS. THE AREA HAS BEEN TIMBERED AND DANGERED OFF.
Hoist went down, hub and splined shaft on main hydraulic pump stripped. New hub, shaft and seal was replaced and hoist was placed back into operation at approx. 4:30pm.
AN UNINTENTIONAL ROOF FALL OCCURRED BETWEEN G1+00-G1+55 (2a) INTAKE ROOMS. 1+30 STATION ENTRY #3 THE FALL MEASURED 65'LONG X 20' WIDE AND 6' THICK

THE AREA HAD BEEN SUPPORTED WITH 4', 5', & 6' CONVENTIONAL ROOF BOLTS. FALL MEASURED 30'X16'X4-6'. FALL AREA WILL BE CLEANED UP & SUPPORTED. SURROUNDING AREAS WILL BE CHECKED FOR ADVERSE CONDITIONS.

FALL WAS FOUND IN RETURN WHILE CHECKING SEALS, 2 BREAK OUTBY SPAD 1835. FALL CONSISTED OF WEATHERED SLATE & SANDSTONE. AREA HAD BEEN BOLTED WITH 6'DOUBLE LOCK BOLTS. FALL WAS 30'LONG X 18'WIDE X 6-7'HIGH. CRIBBED AROUND FALL SO NO ONE COULD ENTER THIS AREA.
Employees were erecting a mine limit fence in the 3-C area. Pickup truck catalectic converter ignited vegetation. Truck was totalled. Less than 1 acre vegetation burned. Fire lasted approximately 30-45 minutes. No injuries.
The emergency escape hoist at ventilation shaft #2 became inoperative due to an electrical problem with a programmable controller. The hoist was repaired and became operative at 8:30 AM on 1/7/06. This escape hoist does not serve any working section or the longwall at this time.

A methane ignition occurred as the shearer was cutting toward the headgate. The flame lasted 2-3 seconds. Was extinguished by the bit sprays.

A roof fall occured in the 1 Main East, #8 entry, in the intersection of crosscut #60. The fall measured approximately 20'x20'x6'-8'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.
An unplanned roof fall occurred in the #5 entry approximately 2400' from the active face. The fall measured 22' long, 18' wide, 3.5' to 6' high. No persons were injured, passage of persons was not affected. Ventilation for return was affected.
Outburst occurred at the top of the 3700-230 raise. Both miners were working under supported ground, scaling the area above the east compartment of the raise when the burst took place. No one was injured - crew left the raise. Immediate inspection not possible due to activity in area. Work area currently shut down; when mining resumes, will monitor & de-stress.

A pocket of CH4 ignited as the shearer cut toward headgate. The flame lasted 1-2 seconds and self-extinguished.
The Hart St. elevator would not stop at the proper position on the bottom. An investigation revealed that the self leveling function on the bottom was not working properly and this resulted in the footage numbers getting off in the computer.

An employee was cutting a bearing off conveyor #8 with welder. He looked up and saw screen deck on fire. Unable to put out with extinguisher; accelerated out of control. Conveyor #11 also caught fire. Fire Dept. was called; they extinguished fire. A fire blanket was being used - located on top of the screen deck. Poss. cause: slag falling onto the rubber screens.

On 1/1/06 at approximately 3:00 pm a roof fall was found in the 19 HG Section between 18 & 19 XC. It is approximately 15' wide, 35' long & 7' high.

A truck driver loaded the 1984 Ford LT9000 truck, then proceeded down the access road and then for some unknown reason, he got out or left the cab of the truck which therefore led to bodily injury and death (fatality) to himself approximately 100 feet from bottom of hill.

Employee working in calciner unit operating a cordless circular saw to build wooden forms in preparation for guniting. As he was making a miter cut on 3/4"" piece of plywood @ 2""x8"" long, the saw blade came in contact with left index finger cutting the tip off.
Installing cylinder on dozer. Had put hand in between cylinder and guard. Cylinder slipped and smashed finger.
Leadman(EE) was assisting repairmen remove the 32"" Dia. steel end cap from the NE Trunion of the #4 kiln inside the burner bldg on the 1st pier. While handling and lowering the end cap to the concrete pier, EE got his middle left finger caught between the 3/4"" eye bolt attached to the cap and concrete causing crushing to the tip of finger. Surgical repair on 1/26/2006.
Roof bolter operator drilling hole for rib pin. EE turned back to the face after reaching for a R.B. and plate he was struck on head/neck/shoulder by piece of rock from between permanent roof supports. Knocked to mine floor. Right hand caught between R.B. and falling rock. Right thumb amputated to end of thumb.
Right index finger caught in pinch point while adjusting shims on a shovel bucket. Tip of right index finger was crushed.

LIFTING 2"" X 20""BOLT OUT OF DRAG BEARING CAP, USING HOUSE CRANE & SLING. BOLT BECAME JAMMED, EMPLOYEE GRABBED BOLT & THE SLING BROKE CAUSING THE BOLT TO FALL STRIKING RT. MIDDLE FINGER TIP. CAUSED CRUSHING INJURY TO FINGERTIP, LEADING TO PARTIAL LOSS OF DISTAL PORTION OF FINGERTIP.

THE PULLY TURNED, CATCHING INJURED LEFT LITTLE FINGER IN A PINCH POINT BETWEEN THE HOUSING AND THE PULLEY RESULTING IN AMPUTATION OF END OF LITLE FINGER.
Employee fell off the ladder while mounting a 7858 hauler. His hand slipped off the hand rail and he fell backward to the ground.

Terex haulage truck operator washed the unit & then started to move to a parking area when the unit died. He exited the cab to check the engine compartment. When lifting the hood he slipped on the wet vehicle and slid to the ground (gravel road) landing on his knees.
EE was talking on IP phone when a piece of rib fell off landing against his lower leg pushing him into the Sub Frame.
Entering cab of wheel loader and foot slipped off of step. The slip caused him to bruise his thigh. The swelling was of concern so he went to see a doctor to make sure everything was ok.
Heavy Equipment/Diesel Mechanic with over three years of experience was using a pry bar approximately 5 feet long to straighten a mirror bracket on an end loader. The bar slipped out of the bracket and struck the employee in the lip. Employee's lip was lacerated requiring stitches and lost time away from work.
EMPLOYEE FELT PAIN IN LEFT SHOULDER AS HE LIFTED CONDUIT.
EE was struck by a corner brow causing multiple severe lacerations and punctures and crushing injuries to his right foot. Also injured his back and right shoulder. The brow pulled away from the solid rib at the end of 2-4' torque tension bolts. EE stated that the brow gave no warning and he didn't have time to get away.
Cleaning grizzly kneeling down, small rock under right knee. Injury was not reported to us until 2/1/06.

Making a belt splice, he reached to move knocker and finger was hit by hammer.
EE was getting onto the mantrip to travel into the mine when he slipped and hit his left foot on the Rib.
While ee was servicing the TDD 388 dozer, he climbed on the right side of the dozer tracks. He leaned over to place tools on track. As he straightened up he lost his balance causing him to fall on the push arm then falling the rest of the way to the floor hitting his head. He was transported by ambulance. EE had a sprained knee & was given RX for pain & 1 day off work.
Employee was replacing O-ring of steering cylinder with 14mm wrench and it slipped off bolt, right hand jerked back hitting against the corner edge of metal frame on the loader. Employee could not open his hand afterward and was told to go to the doctor.
Employee was helping coworker check out scoop that did not have power. When employee touched the battery bell it blew out on him and burned his right hand. Apparently the battery leads were pulled out of the bell and when he touched the bell the leads went positive to negative and produced an arc that burned him.
EE was folding up a portable conveyor for transport, when it was almost folded a undetected piece of limestone dislodged from head pulley area, striking him on the head and face. He has a skull fracture received 4 stiches in head knocked out one footh his next dr. appt. is 2/13/06 to see when he will be returning to work.
Employee allegedly suffered back pain and claims it is related to his responsibilities as Packaging Operator.
The 1300 BE Dragline had moved back six steps. The injured employee was moving the power cable back due to the move. He stepped in a hole and twisted his right knee.
Employee was helping a co-worker repair a broken port hole door on the 3rd floor of the tower riser when some hot dust flowed through the opening and got in his eyes. He had PPE but the dust went underneath and got in his eyes.
While attempting to reattach a scale sensor unit the set screws on the coupler caught employee's glove and started to twist his hand around the shaft. The glove was torn apart allowing his hand to be released. Employee suffered a strain to the wrist and was returned to work with a splint.
EE was connecting rock dust hose to Track Duster. While holding hose with left hand, EE struck coupling with an axe to latch coupling and struck left little finger with a glancing blow, resulting in 7 sutures in little finger. EE was wearing gloves.
Employee strained back while moving pails of oil.
EE strained his neck after driving his vehicle into a concrete barrier, causing the airbag to engage.
EE right shoulder dislocated after slipping and falling off the second rung of a step ladder in the shop.
Employee was operating the shear and had just cut through the tailgate end of the face when a bounce occurred in the floor and jarred the employee's back. This employee has had previous back problems and continued to have problems with his back and eventually required surgery. The first day away from work from this incident was April 14, 2006.
While operating a drill to bolt the longwall face, the rotation stuck on the drill when tightening a bolt. The drill swung around and struck him in the right rib.
Employee was standing on flatbed of lube truck, helping to put a barrel on forklift. One forklift fork was underneath lip on edge of truck, unknowingly. When the fork was tilted, it created pressure so when the driver backed out, the fork swung upward and hit the bottom of his foot.
Employee twisted her left knee jumping a parking lot ditch filled with water.
Using a stationery ladder to get off of the 59 Shovel. The ladder was stuck when she tried to lower it, so she stomped on it to get it unstuck. When it gave way she had both hands and right foot on the ladder as it lowered approx. 9 feet causing a fracture to her foot.
He said suction hose stopped up going to drill pot on bolter. He layed hose across shoulder and was pulling on it. Said both sides were hurting.
Employee stated that after lifting tire & wheel with forklift, he was holding tire up when the wheel slid out of the tire and hit him on top of the foot.
Employee was servicing a crusher on ground level when a off-road truck was dumping a load of limerock on top of a ramp. A large rock rolled down the hill striking the employee knocking him into a 10 foot pit which was dug under the crusher to catch the screenings. After examination and MRI, patient had torn ligaments and contusions.
EE ran truck out of road, rolled over approx. 4 times.
Employee was turning a large metal cable spool as helpers were loading high voltage cable onto a trailer. The shaft that the spool was turning on was not secured and it came off the A frame holder. The spool struck the victim, knocking him into the A frame and then onto the ground. Suffered a fractured rib.
Employee was splicing reclaim belt in bunker when right index finger struck by hammer form co worker.
EE WAS STEPPING DOWN FROM LADDER OF PLANT (SCREEN) AND TWISTED HIS LEFT ANKLE. HE CONTINUED WORKING FOR THE REMAINDER OF THE SHIFT. AFTER GOING HOME, DUE TO PAIN & SWELLING, EE WENT TO ER AT LOCAL HOSPITAL AND FOUND HIS ANKLE WAS BROKEN.

STIFFNESS & SORENESS CONT'D OVER TIME AND ON 3/7/06 1ST SOUGHT MEDICAL TREATMENT. WAS DIAGNOSED WITH POSSIBLE ROTOR CUFF INJURY. STARTED MISSING WORK DAYS AS OF 3/8/06. THE MEDICAL REPORTS PERTAINING TO THE TREATMENT REC'D WILL BE REVIEWED IN DETAIL
Employee stumbled while walking down the face and struck his right lower leg against a relay bar. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, miner proficiency a factor, protective equipment not a factor, mining equipment/systems not a factor.
On the night of 1-27-06 we had our mine evacuation drill. I went to pick up 2 co-workers to take them out of the mine. One of them went to get out of the drill and stumbled and fell to the ground. He got back up and into the pickup. I asked him if he was alright and he said yes. Nothing was mentioned again until 3-12-06. He had surgey on 5-4-06.
Rail runner went through 1st set of doors, skidding through second set of doors that were closed.
The individual was cleaning the water filter at the end of the hard line. While working on the filter, he caught his finger between the water filter and the conveyor belt tail piece resulting in a laceration to his left middle finger.
Pulling a pump out of the mud. Felt a pull in his back.
While setting timbers said he felt something pull in his groin on right side affecting right leg.
EE was working in the quarry generator room and while attempting to pry a nail out of some wood, the pry bar slipped off the nail head causing some minor back strain as he apparently re-aggravated a pre-existing back problem.
Employee was working on the miner. He was using a chain rachet to pull on a bolt. The bolt broke and flew back striking employee.

Employee was holding a step ladder while a second employee was taking down a c-clamp. It fell striking employee on top of head.
EE was turning around in deck of shuttle car when he caught his left foot under the seat and twisted his left knee.
Worker was trying to clear rock jammed in conveyor when a 10 & 15 pound rock struck him in the middle of his back, resulting in bruising and swelling.
HE WAS PICKING UP TRASH ON HIS SECTION, WHEN A CAR CABLE STRUCK ON THE SIDE OF HIS HEAD. ** STARTED MISSING ON 07/10/06**
Employee was trying to start a fuel truck. The battery/battery box that was on his dozer fell and landed on his left hand.
EE was assisting a roof bolting machine move from one heading to another by removing the cable ties that held the cable to the mine roof, allowing the operator to take up cable slack while tramming cable reel first. The cable struck the EE in the feet knocking him to the mine floor. He fell on his back and on top of his self-rescuer causing a simple fracture to the lumbar area.
EE was tramming a scoop in the #6 entry, 3 breaks outby the section, and ran over a crib block causing him to be jolted in the operator's compartment, striking his head over the underside of the scoop canopy causing injury to his neck and back.
Roof bolter struck by rock while adding support to loose rock in bolted room. #3 crosscut of section at 13 + 20. Miner suffered laceration to right side of neck resulting in 4 sutures.
Electrician was trouble-shooting IGBT tower and failed to allow capacitor banks to discharge completely and was shocked by the capacitor banks.
HE WAS STACKING BLOCK ABOVE HIS HEAD AND PULLED SHOULDER AND UPPER BACK. ** STARTED MISSING WORK oN 03/21/07**
The witness stated he was bolting not knowing that the cable reel was engaged. His co-worker was at the rear of the bolter at this time and was flipped by the cable when the reel began taking up. The co-worker complained of low back pain.
Located at the 8-a switch off of the 6 north track: employee injured his left little 5th finger when monorail dollies he was loading shifted, twisted catching his hand between. Began missing work on 1-30-06.
Two employees were on the manlift to check screens in tower 3. A bolt fell off the manlift. They yelled ""look out below"". The contractor looked up and was hit on the left side of the face - left high cheek bone - eye area & chin.
Walking in #7 entry towards power center, walked into wire screen, cut left ear and left side of neck. He has ringing in ear and stiff neck.
Person was walking down belt entry, stepped down of wood bridge on to wood crib block used for step. Crib block rolled causing knee to twist. Person continued to work thru feb 3, 2006. First day off work for injury feb 6, 2006.
Attempting to unclog chute, slipped and fell on back, fracturing rib.
Employee was moving water pumps and attempted to move a pump that was frozen to the ground and felt a pull in his left shoulder.
EE was having to use a bar to close railcar bottom door when the bar slipped out of the door socket causing EE to hit right hand onto the concrete.
EE was shoveling walkway around the cone crusher when 1/2"" water pipe fell, striking thumb of left hand.
Tripped and fell landing on his back on a chunk of coal. Several months later he sought medical treatment and was diagnosed with a pre-existing degenerative back disease. He was placed on a therapy program. On July 27, 2006 surgery was performed to repair the degenerative disks in his back.
Ee pinched his finger between the drill steel and chuck on a roof bolter.
Removing tire from machine. Extremely tight bead on wheel due to no lubrication, resulting in ram slipping off pressure point and striking EE in the face.
EE was preparing to set up his machine to start a cut in #1L. He was standing beside the miner head examining the roof conditions inby the miner. A piece of rock fell out between the bolts above the head of miner. The rock struck the head & flipped off striking the employee in the shoulder. He was taken to the hospital for x-rays. The x-rays were negative for broken bones.
While attempting to open the discharge door to the No. 1 Dry Plant's hot tank with a 6-foot pry bar, the employee scrapped his left hand's knuckles on both the little & ring fingers. A deep laceration required staples and physician issued work restrictions.
Employee was driving #999 Jeep to work area. He hit a rough spot in roadway causing a jarring sensation to his back. Asked to see Doctor 2-3-06. Started losing time 2-3-06.
Dump trailer raised to discharge load. Load partially adhered to dump bed causing a weight imbalance. This shifted the center of gravity to the right causing the vehicle to roll over onto its side. Driver removed seat belt & attempted to exit the vehicle at the beginning of the roll. Attempt failed causing him to fall back into the vehicle striking the door fracturing his clavicle.
EE WAS TRANSFERRING A METAL PLATE THROUGH A MANDOOR. PUTTING IT DOWN, HE FELT HIS BACK POP.
Employee went to turn off duster; hose blew off pipe and struck employee in the nose.
EE was climbing up the dozer and felt a sharp pain in his left knee.
Slipped and fell as he exited the S-9 discharge hopper.
The employee was preparing to hoist a 10' peice of polyethylene pipe that included a ceramic fitting on one end, the pipe was resting on a hand rail, the fitting caused the pipe to roll falling off the hand rail. The ""plastic"" pipe bounced striking employee on both the bounce and the fall
Was knocking dust from a fan and stuck his finger through the guard and cut his finger. The cut needed stitches. He will return to work when the stitches are removed.
Employee was pulling up on a wrench to loosen the bolts on the plates for a jaw crusher. Later that day the employee began experiencing pain in his lower back.
While attempting to remove stuck drill steel EE strained lower back.
EE was getting ready to mount the mechanics service truck he slipped off a small wooden block and fell to the ground he caught himself with his right hand and broke his right wrist.
EE said he was backing up pinner, and ran over a rock in haulway, got head in top, said his neck was strained.
Employee was walking in to work when he slipped on the stairs and caught himself. He strained his right ankle. He was evaluated by a specialist on 4/11/06 and was taken off of work.
EMPLOYEE WAS OPERATING THE HELPER SIDE OF THE FLETCHER ROOF DRILL. HE FINISHED INSTALLING HIS BOLT, WHILE WAITING ON THE OPERATOR TO FINISH HIS BOLT HE SQUATTED BESIDE THE MACHINE TALKING WITH THE FOREMAN WHEN A COAL RIB FELL FROM NEAR THE TOP MEASURING 6"" X 4'X 2', STRIKING HIM ON THE BACK & SHOULDERS CAUSING NECK PAIN.
Injured did not release pressure before removing staple while trying to add hydraulic line. It struck him in nose and left eye.
During the course of his shift on 1/24/06, the employee developed a blister on the top of his foot from his boot rubbing. On 1/26/06, the employee was treated for infection due to the blister.
Moving miner cable out of road way when he picked up cable he felt pain in his lower back.
After EE completed grinding a weld on a handrail with a 4"" grinder, he raised his face shield and a small debris particle came off the faceshield, went between his eyebrow and glasses and got is his left eye. He missed one day of work for a doctor's exam to check his eye. He was treated and he was prescribed eye drops. There is no permanent damage to the left eye.
While unstopping the head - rock broke from around the canopy and hit employee.
Employee was mounting rock truck when hand rail gave way. Employee fell approx. 3 ft and landed on his lower back. Employee did not begin to miss work until 2-15-06.
EE was riding with crew into section on a long-airdox personnel carrier when he struck a roof bolt with his hard hat wrenching neck.
Employee was removing sand stone that had lodged on a conveyor belt. When the conveyor belt rocked, a stone fell off the belt landing on the employee's left ankle.
BRUISED RIGHT HAND WHILE REPLACING SCRAPER RUBBER. ALLOWED TO RETURN TO LIGHT DUTY.
Employee stated that he was throwing a track switch. He pushed the switch handle down and before he could release it, the switch arm came back up jamming his shoulder his first lost work day did not occur until 1/26/06.
Employee was using a steam cleaner to clean power engine unit. When finished he was cleaning oil and grease off of his steel toed rubber boots. The high water pressure cut through the top of his left boot burning the top of his foot.
Injured was pulling rock off miner when rock flipped striking leg and ankle.
His fall was from top of fuel storage tank on the grease truck.
Located on the 10-A gate section, employee was walking from the mine (outby direction). While attempting to walk around the loading machine, the ""berm"" of the ribline collapsed, causing his knee to slide out from under him.
Employee and co-worker were building cribs. As they removed cribs from the top bundle some of the cribs shifted. Victim attempted to straighten the pile by pulling on the runner crib. His co-worker attempted the same thing by striking the runner with a crib tie. Without communicating he struck the crib but also hit victim's left hand.
Employee was walking and stepped on uneven surface and twisted his ankle. His ankle continued to bother him so he went to see a doctor. Employee had previous problems with his ankle and the doctor felt it warranted surgery. Surgery scheduled for 6/20/06. No restricted duty or lost time to this point.
WHILE DRAGGING CABLES, EE FELT PAIN IN RIGHT SHOULDER. CONTINUED WORKING UNTIL 2/28/06. SCHEDULED FOR SURGERY 3/9/06.
Injured fell thru a hole in the floor on the gate floor of the dump. Injured did not request medical treatment until 01/24/06 and recieved same day.
Moving a hoist unit - the unsecured control box fell and hit employee on the right knee. Surgery was performed on 4/17/06 and he began loosing time.
EMPLOYEE WAS BENDING OVER TO PICK UP THE CABLE WHEN THE MINER COASTED ON AN INCLINE, THE DRUM OF THE MINER HIT HIM IN THE SHOULDER PUSHING HIM TO THE GROUND.
EE reported: After completion of drilling hole the required depth, he was attempting to remove drill steel. Steel hung in drilled hole. He was pulling on steel and experienced a sharp pain in lower back region. Pain and discomfort continued, he received medical treatment on 1/26/06. Started missing work on 2-1-06. The event became a lost time injury on this date.
TRAVELING OUT OF MINE ON MAN TRIP. THE MAN TRIP DID NOT STOP & TRAVELED THROUGH THE CLOSED DOORS. INJURED EMPLOYEE WAS RIDING IN THE FRONT SEAT. THE RIGHT SIDE DOOR STRUCK THE EE'S UPPER RIGHT ARM.
Employee was getting off loader. His foot slipped and this places all his weight on his arm. This caused a pull in his shoulder.
The pipefitter leader was lifting and placing 20 foot pieces of pipe on brackets in NT-1 tunnel. As he set a pipe on the bracket, it rolled and pinched his thumb. The injury became lost time on 1/25/06 when he missed one day of work.
The EE had just reported for work at 5:00am. As he was walking down to the lab basement he slipped on the first step and fell against the wall, injuring his right rib cage.
Employee stated he was coming down the steps from the bath house (it was drizzling rain, steps were wet, handrail wet also) when he slipped on the third step from the bottom causing him to fall to the second step from the bottom.
Replacing screen on ""A"" raw coal screen, while doing so he felt pain in his left knee. He eventually was diagnosed with a tear to the posterior horn of the medial meniscus.
EE was working on outside belt drive. EE fell and hurt his back. EE worked his shift then went to the ER on 01/23/2006.
Employee was sawing a post on tap drive of main north belt. Hit right elbow on belt structure.
Employee reported: Over the last few weeks of performing his normal work duties as a Supply tractor operator (Getman Operator) has experienced pain and soreness to low back region. This is the same location where he has had three previous disc surgeries. There is no specific event that has caused this new problem. The mine is charging this claim as a work related illness.
EE tripped and fell against coal rib. He caught himself with one hand. A piece of coal/rock fell from rib and struck him in the side of torso. EE began missing work 01/24/2006.
Ladder was placed on cement surface & leveled stone. Was informed by employee of fractured/crushed lumbar L1 & L2, Broken right hand.
While getting out of coal hauler to help move miner, stepped wrong and twisted left ankle
Hurt neck and lower back while riding manbus which derailed.
Employee was straightening out hoist ropes when he felt pain in his lower back. He was working in an awkward position. He later sought treatment and was removed from work on 2/8/06
Employee was in process of installing roof support cribs, for shield removal. A piece of top rock fell and struck him in the lower back with a glancing blow. The employee continued work until 2-7-06 before starting to miss.
Slipped in the mud and broke right ankle.
Employee stated that as he was on knees installing belt structure he felt his left knee lock up and pop as he stood up. Employee's work location at time of injury was on a belt haulage way. Employee started missing time for this injury on 01/26/06.
EE WAS REPLACING DISCHARGE LINERS (K2 CHUTE/PLANT) AND AS HE WAS CLIMBING OUT, HE HIT THE T.D. DISCHARGE APRON. HE SLIPPED AND GRABBED THE APRON METAL AND HURT HIS RIGHT SHOULDER.
At 11 am EE rolled a rock on his right finger rt smashed the tip. Did not lose nail went to dr. and put in finger split.
Employee was climbing down from loader. Ladder was bent, lost footing and fell to ground.
Employee was attempting to straighten a roof bolt and strained her left thigh.
The employee was helping build a stopping. He was breaking a block with a hammer when he felt pain in his right shoulder at #1 Entry, 18 West.
Supply Yard - Employee was helping truck driver untangle cables. Employee was lifting, pulling and tugging on cable, felt a pop in pelvis/lower back area. Pain going down right leg also. Reported incident. On way back to work, collapsed and began to have a sick feelingin stomach.
Operated excavator in rough terrain and aggravated arthritic condition in neck.
WE WERE USING A CRIB BLOCK TO BLOCK OPEN THE TAILGATE OF A MACK MUD TRUCK SO WE COULD CLEAN MUD FROM AROUND TAILGATE, AS WE WERE PLACING CRIB BLOCK AGAINST TAILGATE, CRIB BLOCK SLIPPED & STRUCK THE RIGHT WRIST OF INJURED EMPLOYEE. HE DID NOT WANT TO SEE A DR. AT THAT TIME BUT LATER ON THE 24TH HE WENT TO A DR.
EE suffered a hernia that required surgery as he was lifting belt structure.
Employee was hanging a cable, when he stepped out of shuttle car his knee(R) gave out. Employee did not seek medical attention or miss work until 2/13/06.
Employee was binding a load of supplies down, using a 3 in. strap with a ratchet. As he was ratcheting the strap tight he felt pain in his lower back. Did not miss any work until 02/27/2006. Going for MRI.
He was operating a lo-trac and hauling timbers when he ran over something in the roadway, causing him to be jarred around.
Worker on top of eng. module on Hyd. shovel installing air cleaner assembly, switching safety harness lanyard from one eye to another. Crane swinging away from area, glove became entangled with hook as he was pushing hook of crane out of the way. Worker was suspended until glove tore. He fell to ground resulting in fracture of left wrist, and acute L2 compression fracture.
BOLTING IN THE FACE OF #2. EE WAS WALKING BACK WITH A BOLT WITH A HEADER & PLATE WHEN HE TRIPPED AND FELL OVER A ROCK. THIS CAUSED HIM TO TWIST HIS RIGHT KNEE.
EE was taking down the roof bolter cable that was hung. The offside roof bolt operator started the bolter and the cable reel was left on. The cable hit under his right arm pit, lifting him into the air, jamming his right hand against the roof.
EE was walking down slope, roadway was rocky-lost footing-strained left knee.
EE was welding and chipping slag with a hammer when a piece of slag went under safety glasses and struck eye (right).
The employee stepped on a sharp piece of metal and cut his foot. He was taken for medical treatment due to a diabetic condition. He immediately returned to work. During a routine exam in early April, the employee was informed of an infection in his foot. The employee filed a comp claim and payments started on 4/13/06. The employee remains off of work.
The injured employee had been driving a pin in the ripper chain at the miner when he stepped approximately 6 - 8 feet away to clean his glasses. Another employee was hammering nearby, causing a piece of metal to strike and puncture the injured employee's right eye.
EE ALLEGES HE WAS MOVING SPOIL MATERIAL WITH LOADER BUCKET. THE MACHINE TRAMMED OVER UNEVEN PIT FLOOR. THE REAR TIRES BOUNCED OVER 36"" HOLE IN PIT FLOOR. (SPRAINED BACK FROM SUDDEN JOLT & BOUNCE OF LOADER.)
Employee was trying to remove rock off of belt and another rock fell behind him hitting the right side of his head & shoulder.
Standing in mud, using a wash out hose. EE went to move his feet - upper body moved, but right lower leg & foot did not, causing a ""popping"" in knee. EE did not report this until supervisor noticed him limping 4 days later. EE did not get med. attn. until 3 days following acc. EE has been working during physician's eval. - finally decided surgery required to replace meniscus.
Injured person was hanging line canvas when a loose hanging wire inadvertently struck him in left eye. He sustained a scratched eye which required him to loose one day from work.
Spreading gravel down mountain on end loader, engine died, moving faster, could not restart, lost hydraulic steering and brake pressure, machine turned toward mountain top, jumped from end loader and broke left leg in 2 places
WHILE WALKING PAST STAGELOADER/CRUSHER, EE SLIPPED AND CAUGHT HIMSELF WITH OUTSTRETCHED LEFT ARM. TORN ROTATOR CUFF.
Cleaning mud from under pusher with bar and pinched gloved hand between roller and frame. Bar slipped off and pinched finger.
Injured party was getting out of the Frontend/Forklift Loader in the pit when he got to the bottom step his foot slipped and he fell to the ground landing on his feet straining his neck.
Welding on cyclone, had pipe suspended in air with come-a-long. Welding inside pipe. Come-a-long cable broke. Pipe came down on right arm, bolt went into interior rt. forearm and fractured rt. radius.
Employee was operating roof bolting machine and felt pain in lower back.
EE stated he was walking past feeder and bent over to walk under the visual pager that was hanging in the walkway. EE said he had been through this area several times but bent over and lost his balance and fell on his knee.
Employee had swelling to right knee due to crawling in low coal as a roof bolter operator.
Employee was bending pin to put in the chuck when he felt a burning in his shoulder.
Employee was cable bolting when co-worker lowered the foot on employee's left foot causing a facture to 3rd toe and contusions to 4th and 5th toe.
OPERATOR WAS CLIMBING UP ONTO THE ROCK TRUCK WHEN HE SLIPPED & GRABBED THE RAILING.
VACUUMING UP MATERIAL, WE HAD TO USE A SLEDGE HAMMER TO BREAK UP FROZEN MATERIAL.
The injured is a roof bolter, he was throwing rockdust on the rib, the upper of machine was setting over, while doing this he ran over the injured EE, right foot, the injured had his back to machine.
Employee was helping to move the roof bolter machine out of #4 entry-the roof bolter operator backed machine upon a rock bolter slid off rock-causing it to move forward and toward rib bolter t bar striking him and pinching him against t bar and rib starting losing time on 1-23-06.
Employee stated that he was drilling the outside roof bolt hole in the No. 7 Room, when a piece of slate rock fell and struck the operator canopy and a piece measuring 2' x 2' x 1"" struck him on the lower right side of his back and knocked him forward into the controls. He stated that he did not observe any loose roof material when he had installed the inside roof bolt.
EE backed 85 ton haul truck into bucket of 1800 Hitachi Shovel causing a rock to fall into the truck bed. EE alleges the impact of the rock caused a whiplash motion to his neck. There is no lost time from the accident, however he is on restricted duty.
EE LIFTED STONE AND WAS PLACING IT ON PALLET. WHEN HE TWISTED, HE FELT PAIN IN HIS BACK (BACK STRAIN).
Employee dropped a bottom roller on his leg when he lost grip of it. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equip. was not a factor, miner proficiency a factor, mining equipment/systems not a factor.
EE reported: Operating a golf cart along the Main North travelway entry. Passed under the No. 1 unit intake overcast as he was coming up the incline on the outby end. Struck his head against the outby brow of overcast, resulting in possible strain to his neck.
Welding on #3 loader bin, eyes burned from welding flash.
Employee stated that after running shuttle car all day that his lower back area was hurting & burning.
EE stated he was pulling on continuous miner cable when he felt pain in his left knee, possibly twisted or sprained. Required surgery on 2/24/06, started missing work on 2/24/06.
Employee was plastering brattice and plaster got inside a boot irritating skin on lower legs.
Torn rotator cuff right arm due to cumulative injury from long-term use of a hydraulic press
Lifting pad and replacing it on dozer when lost footing and twisted knee.
Employee was installing rib bolts when he got his rt thumb caught between the bolt wrench end and the bolt plate-lacerated thumb.
Employee fell while trying to dislodge a rock from a self-cleaning tailpulley.
Drilling a hole to install roof bolts using a 6' steel, he used his wrench to torque bolts to drill a deeper hole and steel bent. He reversed the drill rotation and was holding the steel in his hand when he hit the reverse lever. Wrench came off steel, hit his left arm causing a simple fracture of the forearm.
Injured was standing in a crosscut when a piece of rock measuring 41""x27""x1"" thick fell from between the roof bolts and rib and struck him on the back of the neck.
The Pin Mixer water-line broke and employee was squeegeeing the water out the back door when EE slipped and fell on left hip, bruising it.
Employee was sitting in cab of haul truck being loaded by excavator when a large rock rolled off the bucket, hitting headache rack on the truck. Jarred their back.
Employee was working on a roof bolter, screwdriver fell off boom and struck right eye.
Employee was swinging boom over when a pinner steel flew from the tray, striking employee on right side of cheek resulting in bloody nose and fractures in the cheek area.
EE WAS PREPARING TO START THE LUBE TRUCK TO TAKE IT TO THE FIELD. AS HE WAS TRYING TO START THE ENGINE, AN INSECT BIT OR STUNG HIM IN THE LOWER LEFT ABDOMEN AREA. (GIVEN PRESCRIPTION MEDICATION.)
Employee was handling 4 banded conveyor stucture belt stand when he got his finger caught between 2 stand.
HE WAS TAKING DOWN A's OFF BELT CHANNELS OFF BELTLINE, WHILE TAKING OFF BOLTS, THE A'S FELL DOWN KNOCKING EMPLOYEE TO THE GROUND WITH THE A's LANDING ON THE CHEST.
While hauling overburden, the injured was backing up for the shovel to load him and the shovel bucket hit the bed of his end dump truck, jarring and straining the injured's neck. He missed one full shift and went home early during another shift.
EMPLOYEE WAS WALKING AROUND THE FEEDER AND STEPPED ON A PIECE OF METAL AND TWISTED HIS LEFT ANKLE.
Employee was assisting the mechanic in repairing the loader. He was attempting to tighten a nut and when the wrench slipped off the nut he lost his footing and fell from the front fender to the ground and landed on top of a come-along tool.
Employee was using torches cutting out head jack bushing when some slag popped up got in the corner of his left eye. he had safety glasses on.
EE was attempting to pull slack on the feeder cable to allow the section electrician to connect it to the breaker. A rock Measuring 52"" long X 16"" wide X 7"" thick fell striking him on right ankle. He was then transported to CMAC General where x-rays showed a fractured ankle.
The employee bent down to pick up and move the miner cable. He felt a sharp pain in his lower back and couldn't move.
Employee was working on a scoop when a small piece of rock fell out from between roof bolts & struck employee on his shoulder (L) & neck.
EE was using a 5 foot slate as a prying tool, the bar slipped causing the EE to strain his R shoulder. The 1st day missed was 3/9/2006. EE has surgery on 3/9/2006.
Cleaning dirt and rock out of back of a haul truck using a pick to clean the bed out. He hit the bed of the truck jarring his hands
employee and co-worker were dumping 55 gallon drums of garbage. He lifted on the barrel and pulled a muscle in his lower back
Bolting outer bolt, completed,raised stab jack, went to swing to bolt inner. Roof coal fell out striking on back leg.
The employee was in the process of removing a piece of ventilation curtain out of the roadway. The employee felt pain in right leg and hip. He fell to the ground. Was diagnosed and treated for a fracture to the right femur. (surgery)
Mechanic was climbing down off scaffolding, he felt a pain in his lower back.
EE WAS TRYING TO CLEAN HIS DUST HOSE OUT ON ROOF BOLTER MACHINE WITH HIS PINNER WRENCH. HE STRUCK HIS LEFT FOREARM WITH THE WRENCH. THIS CAUSED A BRUISE ON HIS LEFT FOREARM.
Inj EE & another EE were changing positioin of flop gate at feed end of 6x16 AC screen. Gate consists of a plate attached to a shaft. Gate weighs 100lbs. They tried to reposition gate, shaft broke. EE tried to hold weight of gate. Caused back pain EE went to chiropractor on 1/23/06 has possible compression frac. Excused fro work tl 1/30/06, follow up w/Dr 1/27/206
Walking from shower room to locker, slipped and fell. While falling, he reached out with his left arm to catch himself. Injured left shoulder. He was diagnosed with a torn tendon in his left shoulder. He continued to work until April 17, 2006 when he stopped work to have surgery to repair shoulder injury.
On January 17, 2006, EE was working on a platform inside the kiln feed hood and allegedly experienced knee pain when someone stepped on the scaffold and caused him to tense up. This incident did not become MSHA reportable until 7/11/06, when he had surgery. He did not miss work before that date.
While hanging cable at power center, a piece of shale fell from between pins, knocking hard hat off and causing a laceration to back of head that required sutures. After treatment he was released to return to work. However, the following day he was complaining of tenderness and swelling in ankle and returned to Dr and was treated for a sprained ankle which kept him away from work.
Prying on dozer track with slate bar, bar slipped & he fell back on his right foot.
INJURED STRAINED BACK WHILE MOVING CABLE OUT OF THE ROADWAY.
Two employees were in the process of unloading two boxes of metal clamps from the back seat of a SUV to transfer the boxes to a pick up truck so they could be delivered to another location. He slid one of the boxes out, then turned to allow other room to assist him in lifting the box. He thinks the box tilted and the weight of the box shifted, then felt pain in his lower back.
MECHANIC WAS INSTALLING FEEDER DRIVE BELT WHEN WRENCH SLIPPED AND TWISTED THUMB. EE BEGAN MISSING WORK 3/13/06.
Co-worker hit the victim's finger with a jack bar where they were trying to get pieces of wood from between transfer rail and main rail at a track switch. Cause of this injury communication. Compliance with rules/regs not a factor, protective equipment (gloves) a factor, mining proficiency a factor, mining equip/systems not a factor.
EMPLOYEE WAS SHOVELING DIRT IN A LIME PLANT WHEN DUST GOT IN HIS EYES. THE EMPLOYEE WAS WEARING SAFETY GLASSES AND RESPIRATOR AT THE TIME OF THE ACCIDENT. EMPLOYEE RECEIVED A CHEMICAL IRRITATION OF BOTH EYES.
After pushing a bolt into a hole he stepped back into mud and got his left foot stuck. When he attempted to free his foot he twisted his left knee causing a strain to the left knee.
Carrying a ladder, employee stepped on a stone submerged in water.

Injured was crossing over flow thru when he slipped and fell striking shoulder.
Contract truck driver entered property to pick up load. Pulled onto truck scale for light weight. Got out of truck, entered scale shack, light weight taken, exited scale shack to reenter truck. Came around front end of truck and slipped on a patch of ice and rolled left ankle. Got in truck and pulled over to loading door. Reported slip to forklift operator.
Employee was moving the right miner to the #9 Entry. When he moved the miner cable out of the roadway he pulled a groin muscle. He completed his shift on 1/16. He called on 1/17 stating he was going to the doctor. He called on Wednesday and stated he was scheduled for surgery on 1/20/2006.
Employee was running shuttle car - started around corner & hydraulic locked up causing car to hit rib.
On 1/25/06 it was reported to us by Cougar Trucking that one of their drivers had fallen off of his truck at our Ernest site on 1/16/06. The employee was fixing his tarp outside of his truck & slipped and fell to the ground (bruised groin/tailbone). He continued to work the rest of the day & did not report this incident to anyone at the site.
Brought breaker down and sat it on ladder. Picked up with aid of other employee. Felt stinging sensation in right groin area.
Foreman stopped to pick up EE from location where he was working as a front-end loader operator for a ride to lunchroom for dinner. EE climbed down out of Cat 966 loader to get into pickup and slippped on icy surface. Bruise back.
Employee was tramming an excavator up a hill and was turning onto a road when the cat slid down the hill and the excavator turned onto it's side. Causing the employee to hit his right shoulder on the bars across the inside of the window.
He was handing another employee a piece of metal when his foot slipped and he fell approximately 4 to 5 feet landing on his back.
Steering jack hose burst on ram, spraying hydraulic oil on indiviual, running into eyes. He was wearing safety glasses.

Employee was walking beside roof bolter to move machine, rock fell from top and hit him on shoulder, neck and left arm. A rock approx 20""x 30""x 4"" thick fell from between the rib bolt and the center bolt. First day missed due to accident was 1/24/06.
EMPLOYEE WAS TRANSPORTING WASTE IN A 777 ROCK TRUCK. WHEN THE LOAD SHIFTED, THE TRUCK CAME OFF THE GROUND AND BACK DOWN. AT THAT POINT THE EMPLOYEE SUFFERED STRAIN TO THE BACK. HE SPENT OVERNIGHT IN THE HOSPITAL.

Employee was exiting crawl space area and stood up going forward striking his head on beam jamming his neck. Employee broke his hard hat when he struck beam. He worked for 3 weeks after accident until pain was only getting worse and he decided he better see a doctor.
WHILE EMPLOYEE WAS DOING PERMISSIBILITY ON A VACUUM BREAKER, A COWORKER HIT A LOOSE TIMBER AND CAUGHT IT BUT THE HEADER BLOCK FELL ON THE VACUUM BREAKER, STRIKING THE EMPLOYEE ON THE HAND (FRACTURED LEFT THUMB).
Employee was standing by AL Lee machine when co-worker tripped over some metal. Co-worker knocked into employee. Employee fell against AL Lee, striking head against the boom and scraping left arm on the forks. Complaining of numbness in neck, arm, hand and leg on left side. RTW 01/20/06. Lost 4 Work Days.
Employee struck rib with LHD and his face struck cab frame. Bridge of nose was fractured.
The employee was stepping off of a stair case and sprained his ankle.
EE was assisting the section electrician make a cable splice when his razor knife slipped and cut his left thumb. EE was taken to ER and recived 3 stitches.
Dismounting crane, foot slipped off bottom step, jarred back. Experienced back pain. Accident occurred 1-14-06. Pain persisted. Returned to doctor 2/17/06. Off work from 2/20/06 with disc problem.

Left leg moved toward right. This action pulled him down slightly & to the right; he felt his left knee pop. Fell to ground away from moving belt. Unable to walk, crawled to power center for help.
Employee was cutting shaft from the drag drum and inhaled chemical fumes.
Member was cutting bottom coal, a piece of rock came from head drum stiking member in the right arm.

EE was loading overcast rails onto mine track, rail slipped, caught finger causing a fracture to left little finger.
Employee was walking down stairs when he felt a pop in his left knee. This resulted in a tendon tear of the left quadracep where it attaches to the knee. The Physicians said that the incident was caused by 20 years of use and the tear was going to occur at work or at home. He said it was going to happen how ever.
Two employees were unloading a pump out of the bed of a utility truck when one employee slipped and the other employee took all of the weight in an awkward position and twisted his back.
Cutting a tree down and there was a rotten tree above the one being cut down. The limbs of the two trees entangled and the one being cut down pulled the rotten tree down causing the rotten tree to strike employee in the spine area of the neck.

While working on the 2nd step of a 6' step ladder, I began climbing down the ladder and missed the 1st step of the ladder and fell to the floor. When I landed on the floor, I struck the floor with my left elbow, causing me to jamb my left shoulder.Although this accident occured 1/13/06, there was no reportable medical treatment until 9/14/06.
Employee was pulling on a pipe wrench towards his face and wrench slipped off & struck him in the face.
Two employees were loading rail road ties in a truck, one pushed his end into the truck then pushed down making the tie fall on the other's finger. He smashed his finger between the railroad tie and the bed of the truck.
INJURED WAS OPERATING THE LONGWALL SHEARER. THE BRETBY FELL, AT THE MIDFACE TRANSITION AREA, FROM THE CABLE TROUGH. INJURED WAS PULLING THE BRETBY BACK IN PLACE WHEN A ROCK WAS THROWN BY THE TAILGATE DRUM STRIKING INJURED ON HIS LEFT HAND.
Employee was installing roof bolts was drilling a hole and a piece of draw rock fell, hit canopy he was unless and bounced off and landed on right leg.
The shop millwright was pulling the overhead crane control box towards his work area when his shoulder began to hurt. The doctor prescribed therapy for his shoulder strain.
EE was standing behind bolter dropping trailing cable when bolter backed up to miss s/car cable, while doing so smashed EE foot between bumper and floor.
EE WAS GETTING OFF LOADER AND TWISTED HIS LEFT KNEE WHEN HE STEPPED TO THE GROUND (DISLOCATED KNEE).
Injured was installing an 8 foot bolt into the hole when he accidently bumped the rotation lever causing the bolt to suddenly bend and rotate causing him to hyper-extend his left arm. Originally diagnosed as a strain. New diagonosis is rotator cuff tear requiring surgery.
Stacking mill balls into a bucket, the bucket became top heavy and fell over pinning employee under the bucket. It took a crane to lift the bucket off of him. Drug test results pending.
Operating Bobcat, he positioned Bobcat to hook onto trailer. Trailer has a jack w/wrench. He lifted tongue of trailer to hook onto ball of the Bobcat.
EE was changing light bulb in bottom of raw silos. After completing job EE turned to go out of area and slipped on wet raw material, injuring his right knee.
Employee was welding grating to ramp. A nearby loading hose had small leak that sprayed caustic on to employee.
Injured was dropping steel rope (3/8"" x 120 ft long) which was run through a shive block to the ground from top of load-out bin when slack slipped in rope, catching finger against handrail causing injury.
While performing repair work on a 992G Cat Loader, employee was twisting his body while working with a wrench to tighten a bolt on a transmission and felt pain in his abdominal area.
DRIVER OF WATER TRUCK WAS HAULING WATER TO JOB SITE WHEN HE ENCOUNTERED ICE ON ROAD WAY CAUSING ENGINE BREAK TO STALL THE TRUCK ENGINE, RESULTING IN THE DRIVER LOSING CONTROL OF TRUCK. THE TRUCK OVER TURNED. DRIVER COMPLAINED OF PAIN IN LEFT SHOULDER, LEG AND NECK.
Employee was pinned against wall by equipment while bolting a place.
3 Factors contributing: 1. Employee has a narrowing of the spine where the injury occured. 2. Bin was packed up. 3. Injured used too heavy of a sledge hammer (8#)

The injured was operating the continuous miner in #6 left when a large piece of rock fell onto the mach. He then turned to run and was struck by a second piece of rock that fell from between the last two rows of bolts. Rock was 75""long by 52""wide by 6""thick. Taken to hospital, being treated for fractures to right leg, left ankle and pelvis.
EMPLOYEE WAS RECOVERING HIGH VOLTAGE CABLE, HE PULLED ON THE CABLE & IT TIGHTENED UP, SWUNG OVER & HIT HIS RIGHT ANKLE. ANKLE SWOLLEN WITH CONTUSION.
Employee was in mill using hand shovel when he scraped his finger against a steel chute. EE received a 3/4 inch cut on his middle finger of his left hand. employee immediately cleaned cut and went back to work. Several days later cut became infected. Went to Dr. on 18 & 19 Jan 06.
While plastering stoppings got b-bond on skin.
EE & co-workers were attempting to lift FCT machine using both wood & air bags. The machine was partially lifted, pieces of wood had been placed under the machine for support. When more air was pumped into the air bags, the weight of the FCT shifted causing a chunk of wood to pop out under pressure striking his left abdomen. Sought treatment 1/23 & started missing work.
USING PRY BAR TO ALIGN GUARD ON DRAG PINNION PUSHED ON BAR, FELT PAIN IN HIS NECK.
Employee was changing a piece of 3"" water line when uncoupling, waterline snapped up and hit him in the mouth. Required five stitches in lower lip.
Employee was standing on top of tailpiece greasing 3C header, got off balance, slipped, falling 3' to mine floor. Injured's left thigh struck a belt tail rachet jack resulting in a deep bruise to the thigh.
Employee stated he was pulling the miner cable back with one hand. He stated, he fell down while holding onto the cable. He stated, he felt a pain in his left shoulder.
CHANGING BITS ON MINER & STRUCK FINGER WITH HAMMER.
Employee pulled too hard, on a 7' long 4x4 piece of hardwood that was being used to create a lever to close gate on crusher, receiving sharp pain in groin area.
While preparing to hoist grease barrels to the E-21 Conveyor deck with the use of a hydrolift, EE's left foot was pinned between an outrigger and the ground causing a contusion to the affected area.
This report is being submitted for the sole purpose of abating violation #7098808, event #4054919. Employee allegedly suffered flash burn to both eyes when roof pan (pie pan) contacted the trolley wire. The reportability of this alleged incident is being contested.
INJURED STRAINED BACK WHILE PLACING RAIL IN SLOPE BOTTOM. EE DID NOT REPORT INJURY UNTIL 1/17/06.
EMPLOYEE WAS USING HOSE FROM VAC TRUCK IN CLOSE QUARTERS BETWEEN 2 & 4 DRAG. HE STATED THAT HE STEPPED ON UNEVEN SURFACE AND TWISTED HIS ANKLE.
EE: Incline Belt Tail, Mother Belt Transfer - A long piece of rock was wedged in discharge. Employee was jerking on the rock (4 1/2' long, x 1 1/2' wide x 8"" thick) trying to dislodge. After getting rock loose, employee picked up one end to remove from chute. Alledges feeling popping sensation and pain in right shoulder.
While tramming miner into the #3 entry, injured stepped into a hole, twisting his right knee.
While removing a layer of screens, employee strained his back as he was pulling and pushing and climbing around in deck.
The employee was repairing the primary feeder chain. The chain was twisted. When he tried to straighten the chain, it was loose on the bottom. When it stretched, it moved trapping the little finger of the right hand against the sproket.
The injured party was pinning top when a small piece of draw rock fell. He moved to get out of the way and hit his elbow on the side of the pinner. Just bruised and a very small cut. Two days later it was swollen because of infection. He had to spend time in the hospital.
EMPLOYEE WAS ATTEMPTING TO REMOVE A RETURN ROLLER THAT HAD FALLEN AND WAS LOCATED UNDERNEATH A MOVING BELT. WHILE PULLING THE ROLLER OUT IT CAME IN CONTACT WITH THE MOVING BELT JERKING THE ROLLER IN EMPLOYEE'S HAND. HIS LEFT HAND WAS CAUGHT BETWEEN THE ROLLER AND A SUPPORT BRACE CAUSING A COMPOUND FRACTURE TO THE EMPLOYEE'S LEFT INDEX FINGER.
EE fell through handrail opening that was created to input materials into ball mill.
Employee was crossing over the longwall pan line with power off when employee twisted right knee. Employee did not start missing time from this accident until June 14, 2006.
On 1-9-2006 employee was loading holes at a shot location and injured left shoulder. The next day the employee was using a pry bar on a drill steel and injured right shoulder. MRI was performed and Doctor diagnosed rotator cuff injury of both shoulders. Reported information to employer on 1-25-2006. First lost day was 1-11-2006.
EE was loading oxygen & acetylene tanks onto personnel carrier when he felt pain in his lower abdomen. EE did not start missing work until 2/2/06.
While releasing the roof screen mesh from the top of the ATRS individual had right elbow pinched between mesh support plate and ATRS main frame. This caused a severe bruise to right elbow area, resulting in missing work to allow swelling to subside.
Employee was setting 8' timber-while standing on cribs to reach roof the cribs went out from under him causing him to fall landing on right shoulder. He continued working until 1/16/06 at which time this became a lost time accident.
EE WAS GETTING MATERIAL OUT OF THE BACK OF UTILITY TRUCK, SLIPPED ON BUMPER, GRABBED CANE HOOK FOR SUPPORT, JABBED HOOK IN RIGHT WRIST. REQ'D 3 STITCHES TO CLOSE WOUND.
Put scoop on charge and the scoop rolled over the employee's foot.
EMPLOYEE WAS SHOVELING ALONG THE FEED BELT DRIVE, WHILE LIFTING THE MATERIAL UP TO THROW IN ON A BELT HE FELT A PULL IN HIS LEFT SHOULDER. LOST WORK DAYS BEGAN 1/11/06.
While operating roof bolter, EE caught left thumb on boom arm, resulting in mashing left thumb tip.
EE was loading loose cribs in the scoop bucket when he felt burning in his right shoulder and arm. Lost time injury.
While putting a sling on a liner, the liner shifted striking employee on upper right thigh.
EMPLOYEE WAS WALKING FROM THE UNIT MAN TRIP. WHEN HE ENTERED THE BELT ENTRY HE STEPPED IN A HOLE CAUSING HIM TO SPRAIN HIS ANKLE.
Employee claims to have injured his back while operating a tractor/mancarrier. Employee reported incident 01/13/06, but does not claim it an injury. Employee requests light duty on 03/22/06 but employer has no light duty position to offer. Employee does not return for next scheduled shift, 03/27/06, claiming unable to work and he has surgery scheduled for 04/17/06.
While performing routine maintenance work, EE twisted his knee. EE reported injury to EH@S manager. EE was taken to occupational clinic to be evaluated. At that time EE was released (same day) back to full duty. After injury began to hurt again, EE returned to clinic and was released to light duty.
Employee was using a cutting torch - a piece of slag entered the employee's LEFT eye. The employee was wearing eye protection.
EE was assisting a crew of repair personel changing out the rear end of a battery scoop. When the pry bar that he was using to align the rar end slipped, he fell to the ground and his right foot was caught between the differential and the battery lift arm.
Employee was getting rock dust off of his 4-wheeler when he lifted and twisted with a bag of dust and something popped in his back.
Injured was unloading steel beams from scoop bucket when they slid together mashing fingers.
EMPLOYEE WAS DRIVING A SCOOP WHEN HE HIT A HOLE IN HAUL ROAD. HE SAID HE FELT A BURNING AND GOT SICK AT HIS STOMACH.
Stepped off left side of dozer onto window rock rolled and twisted left ankle.
EE was picking up the belt splice board when he turned around and fell over the belt knife. *EE started missing again due to this injury on 2/7/06. **EE started missing again due to this injury on 3/27/06.
Employee was changing rigging on a box beam that was to be carried to hoisting bay. Beam was lowered to floor by rough terrain fork lift. Beam rested on rods through the beam holding anvil beams. The box beam slipped 1"" to 2"" on the rods resulting in pinch point between the shackle and box beam. Tip of employees right idex finger was pinched requiring 4 stitches.
Member was operting mantrip, the mantrip jerked. Member hit top of canopy of mantrip hurting his neck.
