A fall measuring 15' long x 19' wide x 5' thick was discovered while firebossing the #2 belt at 22 x-cut south mains between #3 & #4 entries at spad #154. The fall will not be rehabilitated and all entrances are breakered off.
Rib/roof fall in an old overcast area spad 6204. No injuries, ventilation not obstructed, outby area. This area will be re-bolted and a walkway established.
Employee stated that he stepped into a coal crevice created by a ripper dozer. He further stated that he was stuck at about waist deep and was unable to free himself without help for more than 30 minutes.
A roof fall was found by the pre-shift examiner @ approx 10:45 PM. The roof fall was on the 1-A belt conveyor. Roof fall is located (1) crosscut outby survey station 4095 on the mmu n 0/ 001-0 section. The roof fall is approx 65'-70' long X 20' wide X 6-9' thick. This location is in the No/4 hdg (3) brks or crosscuts from belt drive.
While making a pre-shift examination discovered roof fall along #6 belt line at way # 246 and extending inby aprox 75'.
An unintentional fall of the mine roof was discovered in the Northwest Mains, at #28 x-cut between #3 and #4 entries. The fall was approximately 60 feet in length, 18 feet wide and 5 feet to 6 feet high.
The #1 elevator was out of service for a period of time exceeding 30 minutes. No individuals were on the elevator at the time. The escapeways were re-routed to the slope. Shindler repairmen were called, the bottom door switch needed lube and adjusted. The elevator was placed back into service @ 7:35 PM.
IGNITION: A methane friction ignition occurred in the No. 3 Entry slant on No. 5 Section while the Miner was cutting in the middle of the entry. The ignition lasted 15 to 30 seconds and was put out with water hose.
A small fire occurred on the tail pulley of the waste belt. The fire took more than 30 min. to extinguish due to limited access. The fire caused minor damage to belt conveyor and hyd. hoses. No injuries resulted from the fire.
Rock fall occurred in 1 East Mains at break #47 in the #6 entry and the #7 entry. Rock fall in the #6 entry measured approximately 20' wide x 60' long x 4' thick and rock fall in the #7 entry measured approximately 20' wide x 50' long x 4' thick.
Had an unintentional roof fall in Main West return at XC 4 . Did not impede travel or affect ventilation. Was reported to MSHA at 9:00PM
An unplanned roof fall was discovered on a weekly exam in the #7 entry on the 5 a belt return entry at 5.5 #3963. The size of the fall was 210'long x 8' high x 20' wide. The area was dangered off & does not affect travel of ventilation. EE of MSHA & fire boss ee discovered the fall. This area will not be rehabilitated.
An ignition occured at 80+42 on the F-18 Longwall Face at # 129 Shield. The ignition lasted approx. 2 seconds and was self extinguished by the shearer water sprays. There was no damaage to quipment and there were no injuries.
A roof fall 750' from face in #6 enrty 5' heigh 18' wide 100' long.
An unintentional fall occurred in the 2nd main west #4 entry at the 63+00'. The fall was 18' wide by 25' long by 11' high.
The elevator would not stop even with the landing. After examination it was found that a contactor was bad, this contactor was replaced at 1:33 pm.
Smoke was observed coming from the level 3 motor control center, mine was evaculated and the MSHA Immead. report number was contacted. Prelim. investigation revealed there was no fire. The smoke was the result of a flash arc that had occured. No injuries/illnesses as a result of this incident.
An alleged dust ignition occurred on the 16A Voest Alpine miner while mining coal in the face of #2 entry. No injuries to personnel nor damage to equipment occurred.
A roof fall has occurred in the #6 Entry on the #1 Section. The fall measures approximately 20ft long by 20ft wide by 6ft high. No injuries resulted from this fall.
The hoist went down due to computer issues due to recent power bumps in the local area. The hoist was back up at 5:00 pm of the same day 26 Aug 2009.
A roof fall occurred in #2 entry at #20 break one break inby spad #133. The fall was approximately 90' long X 18' wide X 7' high.
A fall of roof and rib in the tailgate entry of the 5 Right Longwall at approximately 47+00 caused the entry to become impassable & impaired ventilation of the longwall face. Immediate notifications were made to both MSHA and the WVOMHST and production ceased on the longwall section.
Elevator door switch malfunction, caused elevator outage from 2:15pm until 8:00pm.
Roofall occurred on the No. 3 belt, survey station #17298. There were no injuries associated with this roofall. The roof fall was approximately 40' x 20' x 7'. The mine roof strata is laminated with a coal rider seam present. The fall will be cleaned and rebolted. MSHA inspectors Rick Runyon and Randal Thornsbury along with OMSL inspector James Tackett investigated the fall.
The roof fall occured at the 1st of left hand rooms number 6 belt, 3 crosscuts to the left of spad 4244. The length of the fall was 60""long, 18' wide, 5 1/2"" high. The passage of the men were not eggected. No injuries occured from this fall. The fall will not be cleaned up. The time of the roof fall was 10:20 pm Person reported the fall at 10:50 pm ...fall have been supported
Rock fall occurred in 3 East Mains on #7 belt (track entry) at break #34 to break #35. Fall measured approximately 20' wide x 50'-60' long x 6'-8' thick.
Roof fell on 23 break on 4 belt. It was on the left hand side of an intersection towards #2. It was 17 feet wide, 6 feet high, and 25 feet long.
At spad #312 - 90' inby - fall about 30' long & about 4 1/2' thick Cribbed off & dangered off
A fall appx. 24'x20'x7'10"" occurred in the intake entry (33) on 001-0 MMU. This section is in the process of butting off and the fall will not be rehabiliated. This area was timbered and dangered off.
An unintentional roof fall was discovered on the #2 West mains in the #5 entry at #23 brk at spad #9780. Size estimated to be (5'H x 18-20'W x 80'L). Area had been supported with 4' fully grouted resin bolts with supplemental support consisting of 10' cable bolts. Fall will not be cleaned up. No injuries, no equipment caught, ventilation was not interrupted.
A roof fall was discovered in the 2 West 1 Mains 60 to 61 crosscut, #7 entry. The fall measures approximately 25 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts.
20'W X 20'L X 5-8' thick. Roof fall on #5 South belt (Brk 27) near spad 13431.
Roof Fall in first left in #4 entry around 6600' from loading section of 3 south mains, Roof Fall was around 50' in length, 6 feet in height. EQ involved and one piece of high voltage was replaced and rebolted after cleanup.
The roof fall occurred at the #21 Block of the #7 Entry in the #4 Mains Section. The fall was 2,800"" outby the face and measured 18"" x 18"" x 6"". The area was bolted with 6"" Super Twist bolts and 12"" cable bolts. Ventilation was not interrupted, and no equipment was damaged. No persons were injured as a result of this roof fall.
A roof fall at spad #59 inby for 180' 5' heigh 18' wide above the encor zone.
A roof fall was discovered in the #5 entry between spads 3989 and 3991 on the 4 southwest panel. Approximately 100 foot long, 18 foot wide, 6 foot high. The fall was in an intersection and reached into the cut-throughs toward the number 4 and 6 ertries. The air flow was not effected.
An unplanned roof fall occurred in the #8 Entry, 2 breaks inby spad #3155. The fall measured 20'LX18'WX4-5'High. This area was developed in August 2001 and suppored with 48"" resin bolts. Ventilation was not interrupted and passage was not hindered. Fall will not be rehabilitated. Jacks will be used to danger off area. Located approx 7000' from active section in return airway.
An unintentional fall occurred in the #8 entry of the 2nd main west at the 59+50'. The fall was 18' X 35' X 11'. Fall was due to deterioration of the roof above bolt anchorage.
Roof fall, Main North at Spad #56+64 between #8 and #9 Entries. Approx. 70' long, 18' wide and 8' high.
Security guard was traveling up the hill to Allegiance mine bench. The transmission was slipping. He parked it and start to walk to the mine office. He heard a popping noise turn around and saw the explorer on fire.
Pulverized coal at the coal mill self ignited, causing a fire at the mill. The coal mill electrical system has been severely damaged. Also, some of the mechanical components of the mill have been damaged (mill motors, conveying screws, expansion joints, air heater, etc.)
A roof fall measuring approx. 3'x20'x40' occurred on the mains section-mmu 001-0
Power outage occurred in the power and production department due to a tripped breaker. Evacuation started at 10:31 a.m. was completed at 10:41 a.m. The breaker was reset, the hoist was tested and the miners returned underground at 10:57 a.m. and mining operations resumed.
An unintentional roof fall occurred on 8/24/09. The fall was on the 2nd left panel off #3 belt in #4 entry near spad #1570. No injuries occurred from the fall (6' resin and 10' cable bolts).
A roof fall occurred in the track entry located along the # 2 belt inby spad # 251 at the # 19 crosscut. The fall measured about 16ft wide, 10ft long and 5ft high. No injuries resulted in the fall.
An alleged dust ignition occurred on the 16A Voest Alpine continuous miner while mining coal in the face of #1 entry. No injuries to personnel nor damage to equipment occurred.
An unintentional roof fall occurred on Unit #2 at Entry #4 at 46+50. The fall occurred due to roof failure, cracks in the immediate strata. The fall was originally supported with 4 ft. conventional roof bolts. The fall is approximately 16 ft. wide and 16 ft. in length and 0 in. to 4 ft. high. The fall was cleaned up and resupported with longer roof bolts than originally installed.
An unplanned roof fall occured in the #2 gas mine in the #9 entry, between brk #27 & 28, inby spad #1411 that measured approximately 20'long X 10'high X 18'wide. No injuries were reported. The area was develped on 12/16/08
A roof fall was found in the main return (13 North secondary escapeway) near a previous fall area. Fall was approx. 75-100' X 40' X 0-8'. No equipment or personnel involved. Persons brought outby and rehab work process began.
Roof Fall occured in #3 entry.Fall from 18' inby right rib of #2 entry to 15' left in the #4. The fall was approx. 120'x18'x8'-9' High.
Power feed lines going to the production hoist failed due to corrosion at the connection. This caused a power failure to the hoist. Cleveland Public Power arrived to repair the problem.

A roof fall occurred in the #11 seam at 1 North panel at crosscut #21 halfway in the intersection of #6 entry and into the pillar line towards #5 entry. The fall measured 25' x 19' x 6.5'. The roof was supported by 3' fully grouted bolts.
An unplanned roof fall occurred in the #130 mine on #4 belt brk #17 that measured approximately 20'long X 9' wide X2'high. This is a continuation of a fall. No injuries were reported. The area was developed on 8/10/05.
IGNITION: The Continuous Miner was cutting the bottom in the No. 3 Entry Slant on No. 5 Section when a frictional ignition occurred on the mine floor on the right side of the entry. No injuries were incurred.

Roof Fall located by Federal mine inspector, fall is approx. 140' long and 4' high. No injuries.
Fall occurred on #2 unit, MMU 002*0 #2 entry, crosscut 15. The entry contained a localized horseback. The fall measures approximately 20 feet long, 17 feet wide, and 2.5 to 4 feet thick. the area is supported with 5' torque tension rebar B. lts.

During a preshift examination by a fireboss he found a roof fall in the 1st North section at #3 entry x-cut #9. The roof fall was approximately 15' x 15' x 6' high and the area had been bolted with 6' resin bolts. The fall was not in an escapeway and did not impede ventilation. Travel was rerouted around the cave area.
A roof fall measuring 20 ft. x 15 ft. x 6 ft. occurred 50 ft. inby #4 belt head in the roadway entry, approx. 10,800 ft. from face.
A Fall of the roof occured on the south east life of mine mains B at x-cut 144 on the supply road. Ventilation, personnel not equipment where involved on affected.
A roof fall occurred at #14 wall 3 North in #3 entry at survey station 153 + 23. The roof fall was above anchorage. There was no damage to equipment. The fall was 45 feet long, 25 feet wide, and 15 feet high.
The mine elevator at the #1 portal was descending into the mine when it became inoperable. There were no ee on the elevator at the time but the elevator did not become operational until 8:35am. The brake contactors were changed and the elevator was tested and found safe. The elevator was down for a total of 40 min.
An unplanned roof fall occurred in the coal burg mine 1 brk inby spad #1035 that measured 30'long x 9' wide x 5' high. No injuries were reported. The area was developed on 5/29/09.
A roof fall occurred in the #9 seam in the East Main Intake, between crosscut 27 & 28 in the # 7 entry. The fall measured 20.5' x 10' x 6'. The area was supported with 6' tension rebar bolts, 9' mega bolts, and 12' cable bolts.
An unintentional rooffall occured 600' off no.2 section in the no.1 return. The fall measured 35' long, 18' wide and 4' to 5' high.
An unintentional roof fall has occurred at xc 11 entry 1 of the main north west return. The fall is approxiamately 25 ft long, 18 ft wide, 5 ft height.
Main hoist motor for Service Hoist #1 malfunctioned. MSHA (800 number and Green River office) was notified at 11:45 a.m. There were 50 miners underground. Two hoists were still operable (#2 & #3 Hoists). Motor was replaced and functional at 5:10 p.m. on 8/18/09.
A roof fall occurred in the #1 west A beltline inby spad #75 and outby spad #84. The fall was 60 feet in length and 4 1/2 feet in height. No equipment was involved. The area was cleaned up and re-bolted and supported with cribs and posts.
A non-injury unintentional roof fall was discovered in #2 entry of the 2nd Southwest Mains. The fall measured approximately 40' X 18' X 7' and does not interfere with ventilation and will not be cleaned up. The area has been timbered and cribbed around the parameters of the fall.
A non-injury unintentional roof fall occurred on 001-0 section at cross-cut #30 between #1 and #2 entries. The fall was approximately 5' to 7' thick, 22' long and 18' wide. Using 60"" 3/4 fully grouted rods. Laminated roof gave way.
The mine elevator at #1 portal went down with a man on it. The elevator dropped approximately 25 feet and stopped. Elevator was put on inspection made and was brought back to the surface. The elevator brake contactors were cleaned and the elevator was tested and placed back in service. The ee was trapped on the elevator for approximately 3 min.
A roof fall approximately 20' x 19' x 6' occurred at 12:05 AM in the #3 entry (belt entry). The area had been bolted using 5' torque tension bolts. The strata is mixed. The area will be rehabilitated with approval from investigators.
A roof fall measuring 75'L X 20'W X 12'H occurred in the #4 heading 1 break inby spad # 1527 in the NO. 3 NE MAINS off of the NO. 5 SE MAINS on the 005 MMU (003 Section). Area was bolted with 6' TT and 10 cable bolts.
An unintentional fall of the mine roof was discovered in the #1 Entry at #43 X-Cut on 2 Right. The fall was approximately 6 feet high, 20 feet wide, and 35 feet long.
A small roof fall occurred in the tailgate entry of the longwall. The roof fall was not above the anchorage zone of the bolts, however, it partially blocked travel out the alternate escapeway. The fall measured 17' x 16' x 3'.

An unintentional roof fall occurred in the 2B intake at crosscut #31 entry #4. The fall did not affect ventilation passage of men or cause personal injury. The fall measured 40 feet x 18 feet x 12 feet high. The intersection had been bolted with 6 foot fully grouted roof bolts.

During inspection of our travel road, also our secondary escape way. A roof fall of the intersection 1 block outby spad 641 was noticed. Fall measured approx 8'H x 17'w X 24'L. There were no visual warnings. However there was a slip in the strata that may have contributed to the fall. The fall has been cleaned up and area timbered around.

Slate fall in the #5 entry at spot #1180 in the east section. the fall was from 6'to 7' high x 18' wide x 37 feet long. Approximately 500' from face. The area was slick sided rock.
Unplanned roof fall occurred in #6 entry a S.S. #4099 app. 1050' from the #2 working section. This was breakered off & will not be rehabilitated the size of the fall was 160' long x 20' wide x 6' thick. Phoned in at 7:22 am
An unintentional roof fall occurred in the 1st East Panel (001-0) off the Main South Entries in #2 entry at #1 cross cut at the +120 foot mark. This fall was located in the center of the intersection approximately 18' wide by 18' long and approximately 30 to 36 inches high. MSHA was notified within the prescribed time frame.
Conditions contributing to the Accident;1 West section,#22 x-cut, #2 Entry, 4-way intersection; 25-30 ft. long by 25-30 ft. wide by 7 ft. thick. Appeared to be in a slip area; bolted with 6 ft. fully grouted bolts. area mined in 2004.This area has been barricaded with cribs timbers and boards
A roof fall occurred on the 2-East Return in the #1 Entry between the #27 and #29 crosscuts. The fall started inby X-cut #27 and ended outby x-cut #29. The fall was around 8' in thickness. The area was previously supported with 5' Hy-Tech Bolts supplemented with 8' cable bolts.
An unintentional roof fall was found at approximately 7:25 p.m. in H-Return aircourse, No. 2 entry from Crosscut 29 (Spad 112540) to Crosscut 30 (Spad 11254). The fall was approximately 20' wide x 100' long x 8' high.
A rock fell from the mine roof approx 35' inby survey station No 13356 as the 6-left crosscut was being mined on the 020-0 MMU, East section. The rock measured approx 4'6"" to 5' high x 15'6"" long x 5'6"" wide. 6' roof bolts were being used as primary roof support in the No 6 entry. This report is being submitted to prevent any additional citations.
A roof fall was discovered while traveling the intake. The fall occured at break 12 in the intake. The intake consist of 2 entries, the primary escape way was not affected. The fall measured approximately 30'long, 18' wide 6' thick. Area has been timbered and dangered off.

8/11/09 at 7:14 am #4 Hoist went down due to SPS power outage. MSHA notified at 7:18 am via telephone.
Kuhntown elevator broke down at 10:40am. Surface doors would not open. Shindler came to repair elevator. Problem found was contactors and auxilery switch replaced for contactors. Elevator back in service at 6:10 pm.
A roof fall occurred in an intersection on the Main Northwest haulroad at 39+90, which also serves as the secondary escapeway. In addition to the primary roof support of 12' and 5' point anchor roof bolts, this area also had cable trusses installed.
An unintentional roof fall was discovered in the return air course on the 3rd south west mains of 002 section return. Travelway is not blocked due to multiple entries. The fall was breakered off with cribs & Heintzman jacks. Size of fall 80' L x 19' W x 6'8' Thick.
Our examiners found a roof fall during their weekly examination of a worked out area located in the 1 Left, 1 Right, 3 Left, 4 Main North panel. The fall was located adjacent to their route of travel into the panel. The fall was located at croscut #7 between entries 4 and 5. The fall was confined to the crosscut and did not fall in an intersection.

A fall of mine roof has occured at 74 Brk along #2 south belt in #4 entry. The fall measured 20'L x 20' W x 6-8' Thick. the area was cribbed off on all approches, the fall did not block any travelways or airways.

A roof fall was discovered at 8:50 p.m. during a pre-shift. The fall was located at x/c 10 7th South panel, crosscut 4-5. The fall measured 20 feet long, 18 feet wide and 5 - 5.5 feet high. The fall did not affect ventilation, impede persons or affect any equipment.
An explosion from apparent coal dust and fire occurred in the coal mill baghouse area. It occurred when the coal mill was shutting down for maintenance. The damage was confined to the coal mill baghouse exterior doors and internal components.
Wheel on supply car broke causing car tto de-rail resulting in damage to 2 guide rollers. No one was injured and no other damages occurred. Was reported to MSHA 800 number as required.
The roof fall occurred in the #4 Mains Section (#9 Block, #3-#4 Crosscut, 3200ft outby the face). The fall measured 25""x16""x8"" (LxWxH). The area was bolted with 6"" Super Twist bolts. No one was injurd as a result of the fall. Ventilation and travelways were not disturbed.
On August 8, 2009 an unintentional roof fall occurred at the 48+50 plus marker of the main east entries track entry. The roof fall measured approximately 50' long x 14' high & 17' wide. The fall did not cause any injuries, no entrapments and ventilation was not disturbed. The area had been supported with conventional roof bolts, crossbeams with lagging and 12' cable bolts.
A roof fall ocurred in the intersection of the 006section,The fall measured about 19 feet wide 19 long and about 8 feet high.
Mine Examiner found a roof fall in the left return off of Byrd Mains # two belt at # 50 cross cut between # 1 & # 2 entry. Left of survey point # 1556. Air movement is not impaired & no employees were present in the area.
A roof fall occurred on an intersection in the old #6 entry of 33Mains. The fall has occurred in an inactive area of the mine. There were no persons injured, or equipment damaged as a result of this event.
There was a rock fall in the intake inbetween the black of spad 5922 and 5929 about 5 feet thick and about 20ft long it did not about air flow, the air was that to go over it and the was a rather entry for except relay.

The Gil-8 longwall section, MMU 010-0, experienced an outburst while in the process of mining the tailgate side of the face. Ventilation and travel on the face were unaffected, however, production was halted for more than one hour due to equipment damage.
An unplanned roof fall occurred in the #2 Gas mine at brk 4 in the #3 entry at spad # 1018. The fall measured approximately 20' long x 20' wide x 14' high. No injuries were reported. The area was developed on 7-15-08.

Inby spad #10787 in the #1 entry of 001-0 MMU a roof fall occurred in the bleeder of the section. The fall was approximatley 19'x45x6'. The strata is mixed & wet.
Kuhn Town Elevator was down, door would not unlock at the top of the shaft. Shindler was called and found the problem. They cleaned contactors to correct problem. Elevator back in service at 2:30 PM. No persons were on the elevator at this time.
Unintentiional roof fall discovered on the above date and time. Fall measured approximately 20'LX10'HX20'W. Location of the fall is No. 11 Zone, 18 block, 9 entry, 1 block outby station 837. Fall location previously supported with 6' full grout resin and 10' cable bolts. Overburden approximately 180'-200'. Area mined on or about 1/11/07.
During a storm a lightening strike caused the main power source to the plant to trip causing a total power outage tripping both the #2 & #3 hoists. The emergency backup generator was brought on line to get the miners out of the mine at approximately 7:15pm on #2 hoist
An unplanned roof fall occurred at the #2 Gas mine at brk 39 in the #9 entry spad # 1336 that measured approximately 100' long x 19' wide x 13' high. No injuries were reported. The area was developed on 2-24-09.
Roof fall was discovered on 1 Northwest Mains, 140 feet outby SS#3184, in #2 Entry. It measured 18' X 30' X 7'. Ventilation or travel was not affected.
An unplanned roof fall was discovered at approximately 2:20am 8-4-09 in the 1 south belt and track entry break 86. The fall measured 60' long by 20' wide by 5' thick. The fall was immediately reported, breakered off and a clean-up plan put in place. The fall area has been rehabilitated.
15 W #1 Entry 2863 inby 208. A planned horizontal de-gas hole was intersected at 12:30. The horizontal hole had water coming out as per plan. After water ceased we had excess of 5% which we followed the plan to clear the area. (Please note that this 7000-1 form is being filed for reporting compliance purposes only. We do not agree that an inundation occurred.)
Fall in the Rt return airway 1 bk outby spad #557. Fall was 0"" to 8' high 20' wide 32' long. Fall will be cleaned up and rebolted to std.
Rock fall occurred in 3 East Mains at break #13 to break #16 in the #1 entry. Fall measured approximately 20' wide x 270' long x 6'-8' thick.
At approximately 6:35 am on 8-4-09 the mine examiner found a roof fall on the # 2 section return J panel.
Around 1:00pm men working in section heard rock falling upon investigation found a roof fall inby S.S. 5965 that was appro. 180' long x 20' wide and 5' to 6' high
A roof fall was discovered in the 3 North 1 Mains, 54 Crosscut, #2 Entry. The fall measures approx 25' long, 18' wide and 6 1/2' thick. The area is supported with 5 foot fully grouted resin bolts.
On 8-3-09 the shift foreman discovered a roof fall on the old # 1 section. This section of the mine has been abandoned.
Fall of roof in the 3rd East Mains at 21+60 between #4 & #5 entry. (20'x15'x6') The area had been mined in March 2008 and supported with 6' tension rebar roof bolts. No injuries or damage as a result of this occurrence.
Area not in active workings. 3-way intersection roof fall in #1 entry of the West Submains between crosscut 14 and 15. Cause of the roof failure is not determined. The dimensions are approximately 35'Lx24'WX7'H. The area was bolted with 6-foot fully-grouted resin bolts with some 7-foot torque tension spot bolts.
Electrical issues with SCR modules and Amplification cards.
322 operator noticed a red warning engine light, saw smoke and flames coming from the engine compartment. Engine was shut down, he then hit the fire suppression and called for a water truck. Operator exited the cab and hit the fire suppression again. Fire was too intense to put out, area evacuated.
The fall was discovered by the mine foreman & MSHA Inspector as they made 001 section on 3 east mains. The fall was approx. 20' wide x 20' long and was up to 5' thick in the middle. No ventilation, people, travel or equipment was effected. The fall was between the blocks of #2 & #3 entry across from spad # 2230. This mine had been idle since May.
The tailgate travelway became impassable off of the 5 Right Lowngwall section. A portion of the rib rolled out into the walkway. Ventilation was not impeded.
The Gil-8 longwall section, MMU No. 010-0, experienced an outburst while in the process of mining the tailgate side of the face. Ventilation and travel on the face was not affected, however, production was halted for more than one hour due to equipment damage.
The individual was struck by a rib roll measuring approximately 20' long X 2' thick X 4 1/2' wide causing crushing injuries.
Employee was lifting a grease barrel and felt a strain in abdomen area. He later sought treatment and learned he had a hernia and would need surgery. Scheduled for surgery on 10/23/09. Will be off work after.
The employee was using a hydraulic metal shear to cut strips of stainless steel metal. He reached around the back side of the shear to grab the metal to slide it into position when the actuating lever to lower the shear was activated. His right thumb was underneath the cutting edge resulting in the thumb being amputated just below the first knuckle.
EE was guiding inside part of breaker into case of main breaker when it pushed forward pinching finger between both parts cutting end of little finger almost off.
The injured person was helping to hang ventilation tubing. As he lifted the tube, his footing slipped and he tried to catch himself by reaching for the roof bolter canopy. The canopy was being lowered at the time and his left index finger was caught between sections of the canopy mast, severing the tip of the finger.
EE was lifting a heavy bolt from the crusher spider with an overhead crane, when the lifting eye on the Bolt failed. The bolt pinched the first two fingers of his right hand.
Employee Reported: He and 3 co-workers were assembling steel arches. When placing the arch across a conveyor belt, the victim's right middle finger was caught between the arch and the steel belt frame. The extent of injury was an amputation of the tip of the finger.
Employee was helping direct a motor which was being lifted by a 2/1 directional pulley rigging. Employee had left hand on stationery cable & right hand reaching for motor, when left pinky finger went into traveling pulley, removing tip of finger just below nail.
Employee was flipping a brace, using forklift. While flipping, the peice slipped off forklift and pinched employees right index finger between peice brace and gurter.
Employee was tramming roof bolter when he bumped his left leg on the roof bolter.
Employee fell in hole trying to relocate discharge line on a pump straining medial collateral ligament of right knee. Physician did not issue any restrictions until 10-13-2009. Restricted to no climbing or descending equipment. Cannot operate heavy equipment.
Employee was operating tender boat. He bent over to pick up. rope tie off small crew boat to tender boat. When he bent over, he stated something in his back pulled.
Taking down a kenndy stopping and a piece of rock fell out and struck the employee across the nose
Employee stated that while getting on the dozer he slipped and fell. He used his right hand to catch himself. This accident was not reported until 9-1-2009.
EE runs off side bolter, another EE pulled bolt up and set ATRS. EE moved to swing boom out to start drill, when a piece of rock approx 25"" X 36"" X 4"" fell on his lower left side and back.
WHILE BOLTING HAD HIS HAND ON THE CANOPY POST THE SPACER SLID DOWN PINCHING HIS FINGER
EE was cleaning screen on back of screener with a hoe, when hoe reached the back of the screener EE tryed to clean off the dirt. Bottom screener plate bkoke and EE fell about 8 ft and landed on the screenerplate and screen metal.
Struck on left thumb by rock which had jammed in crusher; sustained fracture of left thumb.
While climbing into the trackmobile the employee's hand slipped causing him to have a pain in his back. The employee reported the accident to the company on 9/2/2009.
Employee was riding on a mantrip when they ran over a rock or hit a hole in the roadway and he twisted his back.
There was a roll of fly pad beside rib. Employee picked up roll of fly pad & threw it on to roof bolter & felt sharp pain in left lower back.
Employee was preparing to start drilling the hole for a roof bolt when he slipped and hit the rotation lever, causing him to twist his wrist.
Employee hurt his knee while he was walking through mud, when he slipped and fell.
Employee was bitten by a copperhead snake as he was walking by a shield car loaded with crib blocks.
Was bitten by a Brown Recluse Spider - only being reported due to lost time. Was bitten on 8/30/09, did not go to doctor till 8/31/09, went back to doctor on 9/2/09 and that is when they requested time off.
Injured employee was pulling the 6"" water line together with a chain rachet. The chain rachet slipped and hit the employee over the right eye.
At the end of shift employee was stepping off of D-11-R dozer. When stepping to ground, he turned his left ankle on the uneven ground.
Employee was pulling cable reel off of the roof bolter, when he slipped and fell, twisting his knee. Started losing time 9-9-09.
EE reported: Attempting to pull slack cable off a Flecther roof bolter machine's reel, ee lost her balance and fell backwards landing on left foot. The foot began to swell a couple days later, ee sought medical treatment and was determined to have a hairline fracture to a bone in the foot.
Employee drilled hole in top and steel lodged in hole. As he let down drill pot steel fell, from the hole landing on his left index finger smasheing it against drill chuck.
EE was drilling with a rib drill on the miner when his right thumb was struck by a bolt in the drill chuck. EE was wearing gloves and protective equipment. EE had a (hairline) fracture and skin tear to the right thumb.
EE had slipped and fell 1-2 feet trying to decend to the next level of planking. Instant swelling had occcured at the time of the accident. Proper equipment was present for a safe decend.
Employee was splitting a large block using a hammer to drive wedges into shims. Block separated and shifted striking his foot. He ignored training and procedures by not using S.L.A.M. method and not properly bracing/shimming block prior to splitting.
Employee reported to work on 8/29/09 with complaint of a bulge on the side of his abdomen. Employee was sent to his personal physician for diagnosis & treatment. As of this date, employee is alleging the bulge is related to his work activities on 8/28/09. There is no confirmed work relationship at this time.
The fan blades on the exhaust mine fan broke out. A piece of a fan blade came through a small opening between the fan housing shroud & the connecting tubing & struck the contractor employee on the left foot behind his big toe causing a laceration that cut tendons & fractured his foot.
Employee stepped out on catwalk of loader to clean windows when he felt something pull in his left knee area.
EE was removing a pin on a bucket and he lost his footing and slipped.
EE was helping others replace springs pads on 800 Mack truck (ST140), when one of the jacks kicked out and caught ee right index finger between spring and crib block.
Employee was standing by No. 4 belt at belt switch and was turning the belt off to spot the belt, when a rib rolled and struck him on his right shoulder.
After checking the engine oil in the machine, EE walked along the tracks to enter the cab, EE then reached for the door, slipped off the tracks and fell to the ground causing multiple fractures to EE's right heel.
The employee was going up the steps, on the third step he missed the step, causing him to lose his balance and fall.
EE was exiting the scoop, slipped and twisted his ankle.
Was cleaning tail pulley on BC 14 when roller came back and pinned his arm.
Employee was using a hydraulic wrench to tighten bolts on brealer plate at the rotary breaker. The location of the bolt was over his head and behind access door. The tool was slipping off the nut, when pressure would release. Employee's hand slipped a little enough while holding tool and his thumb came between the edge of the reaction arm of the tool and the I-beam.
Injured employee was hitting his drill steel to unstop it with a wrench, when he felt pain in his right forearm.
EE twisted left knee while crawling from under 50B haul truck while installing a rear u-joint. Swelling of left knee which did not occur for 3 days, EE worked on 8/28/09 normal duties.
EE was exiting small elevated ramp when his foot slipped due to rain, his knee popped tearing his ACL in his right knee.
Employee was at toolboxes getting tools and parts, when he turned around, he hit his head on roof bolt.
Employee was operating a D-10 Dozer in a shot when he says that his back began to hurt from being bounced around.
He was taking down loading machine cable when he slipped and twisted his right ankle causing a sprain to the right ankle.
Employee strained his back slugging in 19 pebble mill. He was evaluated by Physician taken off from work and a referral was made to another Physician. It was made known on September 3 that it was a lost time injury.
EE's were moving the buggy cable, EE had his foot in a loop whey they pulled the cable it jerked his feet out from under him and he landed on his back. EE was diag. with a sprained ankle and given time off from work.
While stepping out of the high lift in Crabapple yard, he stepped off of the last step and his foot was on uneven ground. His leg twisted and felt pain in his left leg. *** Lost work days began 10-22-2009
The EE was carrying a rock to a pallet and slipped. The rock landed on the EE's left foot. The EE was prescribed ibuprofen and put on modified duty.
EE WAS LIFTING A 60-70# BUCKET OF AGGREGATE TO DUMP IT INTO THE SAMPLE SPLITTER. BUCKET WAS SITTING ON THE FLOOR AND NEEDED TO BE DUMPED IN SPLITTER (~39"" HIGH). EE REPORTED THAT HE FELT SOM ETHING ""PULL"" IN HIS LOWER BACK OR RIGHT HIP WHILE ATTEMPTING TO LIFT. THIS BECAME REPORTABLE ON 9/22/09 AFTER EE WAS MOVED TO DIFFERENT POSITION DUE TO ADDITIONAL RESTRICTIONS.
Driver climbed up the ladder of the truck to do a pre trip inspection, he bent over to check the oil & water and fell off the truck. He landed on his left side resulting in a hair line crack of his hip, scraped left elbow and hand. Contributing factors: It rained last night so there was moisture on the truck and on his boots from walking to the truck.
EE stated that while getting a knot out of the miner cable, it fell from the hanger and struck him on the head and right shoulder.
Member was in the process of building a 7' high stopping. Right corner was tied in and he was stacking block across the top when the middle blocks fell out and struck him on the shoulder, causing it to become dislocated.
Employee stated he bent over and picked up a 7"" x 7"" x 12' timber and felt pain in his right shoulder and chest.
Employee was standing on a wooden block examining an MRS when he slipped and fell to the mine floor injuring his left shoulder.
Employee was recourving 4/O cable when the employee stepped and twisted his back.
Employee was helping put a rub rail on the miner, when the employee felt pain in right elbow.
Emplyee was inside a tank car, cleaning the car. Local railroad came on site, accidently releasing rail cars above the car the employee was inside. The released cars struck the car being cleaned, knocking the employee down, bruising his lower back. No medical treatment was deemed necessary by attending physicians.
Employee felt pain in his right shoulder when he was installing the roof bolt into the drill chuck.
2 ee were conducting normal maintenance, replacing screens, greasing and servicing which includes a lot of manual labor. They did nothing outside of the normal practices. These activities was too strenuous for one and he was advised to take time off in order to rest his back muscles.
Employee was setting timbers. Timber fell and hit employee under his right eye and across the bridge of his nose lacerating his jaw & nose.
On 9/15/09 the employee reported that he could no longer work on his ankle that he had twisted by stepping into a water hole on 8/26/09.
Helping to lift miner cable off of the boom of the miner in 4 South left and felt a pain in his left knee. Lost workdays began 10-28-2009.
Left side groin area started hurting while flipping a headnut for a crusher.
EE was removing hydraulic pump from Mack Truck, the pump was suspended by a rope which broke allowing pump to fall on EE's fingers (2) on right hand.
Mechanic was installing a motor in a water truck. Upon installation, the motor got stuck in the engine compartment. The mechanic was prying on the motor to free it when the motor shifted pinning his hand between the motor and frame. He received 7 stitches in his right hand.
Injured worker was cleaning in the area of 302 drag conveyor. He slipped on wet material and tried to catch himself, when his left hand went into hot material, causing 2nd degree burns. Injured worker was wearing gloves at the time of the accident.
EE was disassembling a CMI EB-42 dryer, while standing on top of the unit - he repositioned his feet - his right foot stepped into the opening causing him to fall while his foot was still in the open center section - the EE fractured his right tibia.
Injured employee stated that he had been pulling the miner cable and upon arriving at home, he experienced back pain.
Employee started up from th epit on an access road in order to construct a drill bench. He was operating dozer approx 15 ft up the access road the dozer got over too far left over the berm and started to slide towards the pit. Immediately material was being placed under the dozer where it had partially went over the edge. He tried to turn his dozer toward the pit.
Employee was returning from break. He was remounting the Dozer when his back gave out which caused him to fall back twisting his left leg and left arm.
Warehouseman reported a shore left shoulder after lifting boxes. He was evaluated by a Physician and on 10-13-09 Physical Therapy was started.
EE stated he was riding in Wallace 14 person man trip. Ran over pot hole in supply road. Causing pain to employee's back. Started missing work on 8/27/09. Scheduled to return to work on 8/31/09.
DISASSEMBLING SCAFFOLDING, STEPPED ONTO PLATFORM AND SCAFFOLDING COLLAPSED. HARNESSED EMPLOYEE FELL, CAUGHT ARM ON RAILING STRAINING RIGHT SHOULDER.
EE was struck by a rib roll on the left ankle while tramming the miner on 008 section. No broken bones, pain and swelling only.
EE was lifting a coalhauler battery lid in order to place it on another battery, when he felt pain in his abdomen. NOTE: EE did not miss work as a result of this incident until 09-29-2009.
As ee was standing in intersection of the last open crosscut, a piece of draw rock approximately 4'x6'x5"" fell, striking him on the back and left side.
Emp was working in #5 Finish Mill removing mill liners. Emp was prying under liner while other emp was using a 20lb. sledge to loosen liner. Pieces of liner broke off striking emp in the upper Rt. arm and upper leg. Sm piece was removed from upper arm stitches were required. The piece in his upper leg required surgery- LT as of 08/28/09.
Employee was working on Kenworth truck and told driver to start the truck up. As soon as he started the Kenworth it lunged backward knocking him over and rolling on top of his ankle. Vehicle would not pull forward until it had built enough pressure for brake to release.
employee was crossing the track, tripped on rail and fell onto the ground and rib.
EE was cleaning sideframes on the shovel and claimed he burnt himself on right leg. Under further investigation another welder was told from him that his knee was sore before the shift occurred and we believe it was alleged.

EE was standing near the junction of a bulk hose that had just been repaired, checking for leaks. The hose connection broke and the end struck the ee in the knees, causing a bruise. EE placed himself in the line of fire.
The injured was cleaning an entry with a scoop. He backed out of the way for a shuttle car to pass, when another shuttle car came around the corner and ran into the back of his scoop. The injured didn't start missing work until 09/02/2009.
While hitting a support beam for catwalk repair with a sledge hammer, the employee struck the metal and his hand slipped off the metal and got caught between the hammer handle and steel.
The employee was working on the slope belt when the employee slipped and fell an strained his left shoulder area.
Employee was carrying 2 cans of hydraulic oil, when he felt a sharp pain in his groin area. The pan subsides but employee defected lump on groin the night of 9-14-09. He sought medical attention the next day 9/15/09
While walking from #5 entry toward #6 entry, employee struck right side of his forehead on a 10 foot cable bolt that was installed in mine's roof.
EE was crawling to get back on his scoop when a piece of draw rock fell & hit him on the neck & back.
Employee was attempting to loosen a brake line fitting on a surface drill when the fitting suddenly broke loose causing him to strike his right elbow on the roller frame of the drill.
Employee was pushing a jeep underground at the start of shift and later into the shift his back became sore. Upon further investigation and medical evaluation the injury to the back was deemed to be worked related as of September 9, 2009.
Employee was walking from point A to point B on uneven ground and lost his footing. He never fell but tweaked his lower back causing a muscle strain. Employee went to panel physician and was put on restrictions.
Employee was washing down vibrator with 1 1/2 high pressure hose. He pulled hose and slipped on wet screens.
Employee was lifting engine hood on excavator. The hood hinges were not working properly causing employee to jerk up on hood to open, which caused injury to lower back.
Lab Tech was mixing nitric acid and potassium chlorate under fume hood and inadvertantly used potassium chloride. The chemical reaction caused approximately 500 mls of the soluton to overflow out of the fume hood and vapors were emmitted. The lab tech and lead lab tech cleaned up the spill. After clean up the lab tech complained of a burning throat and was taken to the ER.
The ee was standing next to a supply car full of belt structure and belt while other men were pulling belt off of the car. The belt flipped over and slid off of the car striking the mans left foot. The belt fractured 2 bones in the top of the ee foot.
Lead lab tech was asked to determine what was wrong with the solution the lab tech was preparing in the fume hood. While observing, approximately 500 mls of nitric acid & potassium chloride overflowed the fume hood and vapors were emitted. The lab techs cleaned up the spill. After the cleanup the lead lab tech complained of a burning throat & was taken to the ER.
Fireboss examining #4 belts, slipped and fell. Caught himself with left hand, landed on left side, dislocating his left shoulder.
While cleaning scrubber in the #0 entry on #1 section a piece of drawrock fell out between rib and roofbolts. The drawrock struck member on head and neck. Member was attended to on the section and transported outside and taken to hospital by ambulance.
EE working on bolting down the symonds crusher motor at the finish plant holding the bolt on the bottom and turning the top with a impact wrench the wrench turned and jammed his left thumb into the frame.
I do not know the date of the injury. EE complained of back pain n 8/24/09, went to the doctor, they said he had kidney stones according to EE. I have not seen any medical information. The 9/15/09 date was when I was told he was filing a workers comp claim.
In trying to lower the shoot while setting up EE unchained one side, the other side came loose also. This knocked EE down from an elevated (4-6') position and the shoot fell on him causing his broken leg, also L-3 compression fracture.
Employee stated he was riding in a manbus, a track switch was left open and the trip ran into the side track and struck another trip. The employee worked the entire shift and worked the following day and then came back and said his lower back area was injured.
Employee was helping change springs on a shaker. When he lifted a spring with another employee, he felt pain in his back.
Employee developed back pain after adding 5 gallon pails of oil to crusher.
EE was measuring timber and the top timber fell on his ankle.
Hydraulic hose end blew off causing a flash fire. Employee received slight burns to arms. Treated and released for the burns. While leaving truck, employee struck his right knee and bruised his heel. Company doctor took employee off work until knee and foot healed.
EE was performing belt scraper maintenance on #4 belt head while standing on #3 belt, #4 belt was locked and tagged out, #3 belt was not locked and tagged out. #3 belt started up.
While shoveling the slope belt the injured pushed his shovel into a pile of coal. The shovel struck a large piece of coal causing the shovel to stop suddenly and the injured felt pain in his right shoulder.
Employee was picking up 12lb sample bags, when she felt a pain in lower back. Resulted in a lumbar strain.
Employee was tramming a battery powered scoop in reverse when he run over a section of scrap water line, the pipe bent upward and came into the operator compartmant striking the operator in the left rib area.
EE was doing area check when his left foot got caught in grading and twisted his knee.
Unplugged a hose that had steaming water in it. Burned his face.
While installing roof bolts a piece of rib fell and struck ee on the back and right arm. EE was transported to General Hospital where he was treated and released and referred to a specialist. To this point a diagnosis has not been provided by the doctor or the specialist.
Injured employee, while on a rubber tired man trip, came off the track jarring his right wrist and hand. X-rays showed two breaks, one in the middle finger, one in the ring finger of his right hand.
While roofbolting, a small rock fell off rib and hit the employee on his right knee EMPLOYEE DID NOT START LOSING TIME UNTIL 2/11/2010 DUE TO SURGERY
Loading parts on mechanics buggy ee was bitten on right hand by spider.
Crack opened up unexpectantly and employee lost his balance and fell.
The jeep he was operating collided with a motor, as he caught himself he struck his right ring finger against the frame of the jeep causing a fracture to the right ring finger.
Injured was cutting a hammer handle with a knife and cut his finger.
While using a jack to change a tire the tech pinched his thumb between the jack and the truck tow hook.
Employee was ascending up the ladder of his loader, when he slipped due to wet conditions from the coal pit, causing employee to hurt his right arm.
Employee was working at the recycle plant, when he slipped off the crusher and fell onto the ladder. He hit a kick plate and got a contusion on the right side, between his rib & hip.
Employee was carrying explosives to the face when he felt discomfort in the groinal region.
Employee walked across parking lot when his knee went out. We did not initially submit this injury because there was much uncertainty about whether this injury was work related or personal (employee originally reported as non-work comp) and whether it happened at work or not.
Employee was getting into #17443 shuttle car when employee struck his left forearm on a piece of metal causing laceration to forearm.
When he was shoveling he felt a pull in his right forearm.
EE reported after removing nylon straps form low boy, walked over to slope shaft wall and hung on storage rail, turned to walk away, foot become entangled and fell on left elbow.
Employee was shoveling belt. The employee told the witness that he was having chest pains. Employee stated that he threw a piece of rock against the rib and continued to shovel, then started having chest pains.
Two employees were leveling the rakes on #5 Thickener. They had a wrench on one of the adjusting bolts for the rake. Employee 1 was pulling and employee 2 was pushing on the wrench. Employee 1 felt a sharp pain to the lower back that traveled to the lower abdominal area. * Started losing time on 8/25/09
The employee stated he was bent over and swinging a sledge hammer connecting water line (8"") and strained his back.
Employee was pulling on 100' bucket elevator chain with a pipe wrench and strained himself.
The EE was helping move a roof bolter the operator was hung up into a cable,the ee stopped to get the cable off of the ATRS. While the operator was removing the cable from the ATRS the other employee stepped between the wheels of the bolter. The operator then stepped back and started to turn running over the other EE's foot and leg.
Employee was working on a continuous miner in the #2 entry on 003 sect. The employee walked to the opposite side of the machine and ran into a roof bolt plate hanging below the roof. When employee ran into the roof plate it stubbed up his neck.
The man was lifting the end of a cross bar when the other end fell off the pile causing the mans arm to jerk. NOTE: man continued to work regular duties until 9/17/09 when he reported off work.
Service man was taking a cap off to put a hose on a tank and there was some pressure behind it. The oil sprayed in his face and eyes.
Employee was working in the shovel house as he lifted a two step ladder he felt a pain in his back. Employee was taken into the ER to be checked out. He was taken off work.
Weather was clear. EE was getting out of 1845 skidloader & twisted his ankle on bucket. Co-worker was standing on a screen catwalk, noticed EE limping & not in control of his balance. Thru hand gestures EE indicated he'd twisted his ankle. Co-worker went to help him after finishing his task. EE said he twisted his foot & was dizzy trying to gain his footing.
Employee twisted right knee while climbing over rock at quarry.
Pulling drill steel out of hole and felt pain in left shoulder.
While pulling the low belt tight with a scoop, the chain broke, letting the tail fly back and strike EE in the hand.
While trying to move a piece of rock out of a travel way, the employee felt pain in his lower back. He began missing work on 8/20/2009.
EE reported that he and co-workers were working pulling a conveyor motor off a CM when a piece of draw rock fell from between roof supports striking the top of his mine cap.Contacted mine after the end of the shift. EE was treated by a chiropractor on 8/19/09 after work.EE saw doctor on 11/10/09 due to continued headache/neck pain. EE began missing work on 11/12/09.
Injured was building stopping when a coal hauler came through the curtain and knocked stopping down onto injured. Injured's right knee hyper-extended.
EE was plastering brattice when he went to stand up he felt pain in his leg.
Employee was originally diagnosed with bursitis of the right shoulder. After reviewing the surgeons report we feel this may be a reportable event. The surgeon repaired the employee's rotator cuff in addition to removing osteophytes.
While moving a rock, another rock rolled on to employee's right foot
EE was removing an oxygen cylinder from a pick-up truck, to install on a torch cart in the maintenance shop. EE reported to his supervisor 1/2 hr later that he strained his back lifting the cylinder.
He was carrying a crib block, when he tripped over a rock and fell, landing on wrist.**started missing 09-22-09**
EE was working on overcast, EE was stacking blocks when he picked up a 8"" block he turned to the left to place the block on top of a 48"" overcast when he said he hurt his back. Accident occured 3 brks outby #16 headdrive. No EQ involved. EE did not slip or fall
Employee had been lifting belt structure during his shift on 8-18-09. At approx. 5:00 A.M. he complained of severe pain to his lower back and was taken to Lonesome Pine Hospital for evaluation. He was treated and released to the care of his regular physician.
The employee alleges he was reaching a crib block to a co-worker when he slipped on a rock lying on the mine floor, twisting his back. The employee started missing work on August 31, 2009
Employee was working under dry screen and a small rock bounced off the screen and struck employee in the back. Employee was taken to medical clinic. Has a bruise on back. Doctor took employee out of work for two days, August 19th and 20th. Employee returned to work today, Thursday, August 21st.
EE was walking to underground work area a rock 56"" long, 9"" wide 2"" thick fell from mine roof striking him on the left elbow.
EE was holding a punch while other EE was swinging the hammer trying to loosen bolts when the EE with the hammer missed hitting the EE finger on his left hand.
The employee was dismounting a P & H 4100 shovel. As he stepped from the last step to the ground he lost his balance and fell. On 08/24/09 he saw an orthopedic specialist who diagnosed a compression fracture to his T11 vertebrae. He was prescribed a back brace and physical therapy.
Employee has a pre-existing, non- work related neck illness. Employee filed FMLA case in January of 2008. Employee now states that his disability is a result of a work place injury on 8-18-09. The US Gypsum Company does not support his injury claim that his illness occurred on the mine site.
EE reported after dropping the bolter cable to tape spices, he was attempting to rehang the cable. When lifting the cable up to rehang over the cable hangers experienced pain to his shoulder. The following day, should became sore and experienced pain in the joint area.
Co-worker was drilling metal into ground, chuck (apparatus on end of drill) broke loose causing drill to come down on injured employee's arm, resulting in a fracture.
Operator received material in left eye when an inlet hosed to a diaphragm pump ruptured.

While cleaning sprayer to apply stain the hose lifted out of the bucket and sprayed acetone on the left side of EE's face. EE immediately flushed his face with bottled water and reported to the job trailer for eye wash solution. EE contined to experience burning in his eye and was taken to the emergency room for treatment.
Bolting two pieces of 10"" steel pipes together. Using loader to move one to the other. Loader operator failed to watch mine manager's hand signal. Dropped loader bucket on the head of employee trying to bolt the pipe.
Raising and setting #2 shield using manual controls. Hose fitting broke which allowed the hose to fling back and forth striking right arm at the elbow area.
Employee began missing work on 12/18/09 from an injury that occurred on 8/17/09. He was moving a miner when he was struck by the conveyor boom.
Employee tripped over a rock and fell at approx. 1:30 A.M. Later that day he went to a local clinic for an x-ray & treatment due to pain in his index & middle fingers of his right hand. The x-ray was neg. for fracture & he was diagnosed with sprain to his fingers & put off work for 10 days.
It had been raining all day and when he stepped out of skid steer, he slipped and kicked the bucket.

Employee was painting the face preparing to drill, when a small portion of rock fell from the face. It first struck him on the hard hat, right shoulder, rolling down glancing off his mid thigh, calf & ankle. Superficial abrasions were noted on these points of contact. After x-rays, right ankle fracture.
EE was drilling a roof bolt hole. As he was putting the two drill steels together, his right hand got caught in between the two drill steels, causing a fracture to his right hand.
Worker changing bearing on the press roller. While removing the seal, the inside was very sharp and cut his right thumb when the seal came loose.
EE was installing center pins in getman. While prying with mining bar he felt pain in right groin. EE was taken to company doctor on 8/21/09 and diagnosed with a hernia and not to return to work until he has surgery.

The employee is an end dump driver. He got out of his end dump to pick up a rock that had fallen off his truck onto the haul road. When he picked it up he felt a pull or twist in his foot.
Employee fell over a masonary block and fell onto a pull trailer hurting his ribs and chest.
Employee was spotting for another employee that was operating a forklift. The forks on the fork lift got in a bind on the load that was being picked up. The fork came off the lift and smashed the spotters finger between the fork and the load that was being moved.
Employee was sitting on the bench in the washhouse changing clothes when he said a basket fell and struck him on the back of his head. He has continued to work and was taken off on 06/03/2010. MRI's of the neck and back were done and showed Multilevel degenerative areas and Minor Lumbar degenerative changes. Spurring was also found.
Lifting water tree for belt water line felt pain in upper back between shoulder blades. Cause is improper lifting technique. PPE not a factor,Compliance of rules/regs not a factor,mining equipment/system not a factor, miner profiency was a factor. This person worked after the incident and now is off due to injury 10/10/2009 was his last day worked.
The continuous miner operator was standing between the second and third to the last row of roof bolts in the No. 1 entry (Gil-10 Headgate), completing a cut, when a rib roll occurred. The rib struck his left leg. The rib measured 6' x 6' x 0-24"".
While standing on a ladder (4 foot off ground) Employee stepped backwards and fell. He was holding onto the ladder handrails but due to his size he could not support his weight. Employee complained with knee pain. Initially classified as First Aid Injury by Doctor's office. Status changed to Recordable during follow up visit on 8-17-09, due to prescription medication.
Injured was shoveling a rib between a skidsteer and a 3 1/2 scoop. The skidsteer operator shut his machine off and as he was getting out, but before he could place a wheel chock, the skidsteer rolled downhill and caught the injured person between it and the bucket of the scoop.
EE pulled pin to seperate hydraulic trailer from forklift and the tongue moved back falling down missing the blocks that were set to keep it up in the air. Tongue landed on his right foot.
Coal truck driver was coming out of pit where there was a rut, driver straddled rut and overturned. Driver said he was dizzy got emt to him they looked at him ,gave him oxygen, called 911 for ambulance.
Employee states while unloading structure weighing approx. 70-80 lb. he stepped off a 2-foot ledge and felt a pop in his right knee.
Employee was removing steel with a cutting torch. When relocating, he lowered his torch. Once he did this, the flame from the torch touched a paint can that was under dust and rubble and the can ignited. Flames came at employee and he jumped off platform (approx 4 ft) to the main floor and twisted his ankle and received first degree burns (partial neck and chin).
While installing a bearing on a conveyor, the jack slipped out and pipe fell on left index finger pinching it between the pipe and metal.
Mechanic was pressure washing cooler carrying wheels in steam room. The wheel had been cleaned, standing upright. The employee turned around to clean the next wheel. The wheel behind him began to roll towards him. His partner yelled a warning but the employee couldn't hear due to noise. The wheel tipped over, fell, pinned employee's right leg/ankle to the pallet and floor.
Two employees were pulling out shell liners inside the ball mill. A liner flipped over on its side and struck a 5' pry bar causing the pry bar to strike employee on his ankle.
Employee was pulling pump coupler off front of engine when foot slipped causing him to go down on hood. When he went down on hood a bolt was sticking up and it struck left testicle. He has been treating continuously with co. dr and specialist and he was given an injection and taken off work effective 3/2/10. He will have surgery on 3/10/10.
EE was throwing block off a supply car, he slipped and fell on his right shoulder. EE started missing work on 08/27/2009.
EE was commuting home on his motorcycle, he was with his job, when he was hit by another contractor on site who was driving a truck; that contractor is responsible for the accident and injury to EE.
While carrying a 55 ton shackle up the steps to the tool trailer, the employee experienced a sharp pain in his lower back on the right side.
EE was checking pump floats when he crossed the discharge line he stepped on a rock with his right foot and twisted his right knee. EE has been having injury treated and recieved an injection on 9/29/09 making this injury reportable.
A piece of plate steel was stuck in the jaw crusher and was being removed by an excavator and chain. The steel plate sprung free and slipped from chain upward. When the steel plate came down it hit a cross brace then struck the victim who was still in the feeder.
Electrician was doing prev. maint. checks on cont miner, when he bent over to check the traction breaker, a piece of rib rock measuring 6' L, 15"" W and 1-4"" T, struck him on the right hip and upper leg. He was taken to hospital where X-ray was normal. He was referred to follow up w/specialist and MRI. Contacted Safey Dept 9/4/09. MRI on 9/2 showed ruptured discs/1 bulging disc.
EE strained stomach while lifting a piece of stone.
Injured slipped on top of roof bolter while cleaning it. He went to ER and was released to return to work on 8/17/09. On 8/18/09 he returned to orthopedic dr. and was taken off work until 8/24/09. No diag. was made therefore the co. had not made a determination on reporting until 08/26/09. The mine was idle until 08/24/09.
Tearing out a mine fan, the top half of the shroud had been unbolted from the lower half with several bolts left in the conndu without nuts. As one piece was being lifted by a 15ton crane ee stepped onto the top of the shroud closest to the blade. The shroud moved ee jumped off and the shroud rolled onto him pinning him under it.
Bent over and picked up a cement block for a wall and felt pain in his back.
Employee was grinding on a piece of metal. While doing so a small piece of metal shaving flew through the side of employee's safety glasses, resulting in the metal shaving embedding in his right eye.

While tramming shuttle car from miner to dump, shuttle car dropped in a hole. The deck of car kicked up a rock, hitting EE in the eye.
Employee was removing metal that had been cut from the floor when it slipped and caught his thumb between the handrail and piece of metal. He received a laceration that required stitches.
EE was running the button house when a big rock plungged the throat of the feeder. He attempted to break the rock with a 12# sledge hammer and pry with a bar to allow the rock a position where it would go on into the crusher.
Employee was climbing off dozer when he missed the top step and fell on his back, hitting the dozer track.
Employee states he was moving steel plates on take-up and smashed his left ring finger between pieces of plate suffering a laceration. Employee was taken off work pending surgical consult to remove bone fragments from the top of finger.
Employee was checking the railcar and was sitting on the plant floor with railcar directly in front on him. He jumped to the ground using his arms to support him. When he jumped down, he landed on his right ankle and knee.
EE stepped onto the uneven surface of the stone covered yard as he exited the manlift basket, his right foot slipped into a hole causing his knee to twist resulting in a severe sprain to his right knee.
On dredge there is one hand winch used for moving tail section. The operator went to release winch and the cotter key holding the gear reduce shaft broke sending the handle free and it swung around hitting operator in arm causing a slight gash and bruise on his arm.

EE was helping put up supply doors when the rib rolled off and struck him on the left leg. EE continued to work until 09/09/09 when he had surgery.Injury was not LTA or medical reportable until 09/09/09.
EE was tending miner cable in the #1 entry of 11th west, when a piece of the rib was dislodged and struck the EE on the right side of his body and knocked him into the shuttle car with the left side of his body.
Employee was finished loading a truck and started down the ladder and his left hand lost its grip, causing his left knee to strike a ring on the ladder.
EE was in a lift basket scaling the roof of the mine. He stated that he twisted while trying to pull down a piece of rock with a pry bar, and pulled a muscle in his neck/back area. EE has been prescribed muscle relaxers, anti-inflammatories and pain aid. He will not work until the Dr. has released him. Dr. will reevaluate in one week.
Was removing a lock nut on a jaw toggle spring. The bolt securing the tension on the spring broke and trapped his left hand between the air wrench and motor base.
Employee had cut off the No2 and No3 belts and was in the process of crossing the belt when he fell. His left side in the rib area striking top belt structure.
Employee was assisting the disassembly of a crane when the outrigger lock lever struck his right index finger.
The roof bolt crew finished bolting the face of #6. The miners backed out of the face and turned the machine off when a rib rolled off from behind the line curtain. The rib measured 6' L, 8"" W, and 20 "" H. The rib struck the miner in the lower back/legs. The miner had pre-existing back problems and was diagnosed with a lumbar strain. Medical was not released until 8-18-2009.
The man was connecting a rock duster to a battery powered cart when he felt a pain in his low back.
EE stated that after hanging CM cable against the rib, he experienced pain in the right thigh area.
Employee was uncoupling 2 RR cars from each other. He grabbed the lever bar to release the coupler and it would not release all the way. He then grabbed the coupler itself at the same time as the lever and pulled on the coupler. This is when he felt a strain on his upper right shoulder.
Cutting off bolts at base of roof fall at 48+50 on main line track when a piece of stone rolled off the pile & hit him in lower back causing abrasions & contusion at tailbone area. Injury occurred 8/12/08 & he went to the hospital 8/13/09. He worked 8/14/09 & 8/15/09. After still having pain he went to ER 8/17/08 & dr put him off till 8/24/09. He started missing 8/17/09.
EE was using a combination wrench to loosen a bolt; the wrench slipped causing ee to strike his right hand on a channel support.
EE was packaging 50# bags on a turn table. He pulled a muscle in his lower middle back. EE reported the incident on 8/17/09
While driving a mantrip inside of the mines, the EE hit a bump causing his right hand to be struck by the ball on the steering wheel.
EE was walking towards face from power center, when a piece of rock 1/2 inch thick, 7 inch x 8 inch fell from the roof, striking him in the left side of his head. 90' inby spad #1691 (area of accident is 10'high)
EE was working between shifts to install belt hangers. The power had knocked. EE sat down on the T-bar of the roof bolter and said it shocked him. He was transported outside and then to the hospital via ambulance. MSHA, State, and Company officials investigated all aspects of electrical systems at the mine site and could not determine the cause of the accident.
The injured was removing loose rock and coal from roof screen with a slate bar. A piece of rock that he dislodged struck him on the left little finger. The injured suffered a fracture to the left little finger.
Mantrip #2 was coming outside & cautioned to watch for #1 mantrip which was checking the belt. At airlock doors mantrip #1 was traveling back to track switch when it slid into mantrip #2 & member twisted right knee by getting tangled in nascar netting attempting to get in clear. Member continued working then had been seen by specialist & MRI & had meniscus tear.
Employee was changing batteries on 516 scoop. He was trying to pick up the charge battery, when the jumper cable got pinned between the scoop and the battery. The employee then tried to remove the jumper by hand. The cable arced and the employee's left arm was burned.
The employee was backing across the run with supplies in his bucket for the roof bolter, when he ran over something that threw him up in the air. The employee reached to grab something to brace himself. When he grabbed the outside edge of the canopy, the scoop hit the rib at the same time. The employee's pinky and thumb were broken and received several stitches.
Employee was bolting top and his drill steel hit his left hand.
EE's foot slipped as he was descending the ladder of the D-9. EE caught himself with one hand and had pain in his shoulder blades. EE did not seek medical att. at this time. During routine visit with his Dr., he informed the Dr. of his pain and an MRI was ordered. EE has not returned to work since.
He was trying to pull cable off of roof bolter, when he felt pain in his lower back. He did not start missing work until 9/2/09 because of incident.
Employee lifted up the face shield attached to his hard hat, and where the shield attaches to the bill of his hard hard, accumulated dust fell into his eyes.
Employee was lifting a 6"" x 6"" x 8"" wooden block to use as a cribbing to secure the boom of a loader, when he heard/felt his right shoulder pop.
DID NOT START LOSING TIME UNTIL 10/22/2009 Moving miner cable, felt a pop and crack in his left shoulder. Surgery required at this time.
While opening the door on the tool trailer, the door stuck and EE felt pain in lower back.
was walking from vehicle in parking lot at cameron portal to portal building for afternoon shift when ee stepped on stone twisting right ankle.
The employee was hanging a cable up on a roof bolter when the reel locked up causing the cable to be pulled down. The cable caught the employee's foot and leg twisting it. The employee continued to work until Sept 18, 2009 when he started missing.
EE started going over the hill to seed hollowfil when he stepped on a rock and fractured his little toe.
While drilling a roof bolt hole, a piece of rock fell from the roof striking the drill head and hitting the employee on top of the hard hat.
Working on the heavy media prewet screen. 6x6 beams were cracked in half. Screen sees a lot of water, so it was rusty. Took the screen off deck of prewet to be grinded. Was wearing safey glasses and hard had with light attached and also had another light there. Had dust rag, steel could have been on rag when he wiped face. Did not get medical treatment until 8/17/09.
EE was mixing concrete sluray when mix got into his eyes causing burning sensation.
Employee was bitten by a brown recluse spider on the top of the right hand. Employee originally did not report the incident and turned it in on his own insurance. When notified that it happened on company property, we turned it into worker's compensation and reported.
Employee was pulling fuel hose out of truck & pulled something loose in Right Shoulder.
Victim was bending a roof bolt when he felt sharp pain in back
EE was trouble shooting air conditioning system on a Cat 24H motor grader. When checking air flow in an air duct, the employee did not realize the exposure to a moving fan. When retracting his hand it caught the fan, injuring two fingers. The finger tips were cut and fractured requiring medical treatment.
Walking down the stair and stepped wrong causing victim to fall and hit his right arm on step.
The injured was scaling down loose ribs from behind the ventilation tubes, when a piece of rib struck him in the legs. The injured suffered a right knee sprain and a left ankle sprain.
EE was operating the continuous miner under supported roof on #4 unit, #4 entry crosscut 58 to 59 when a piece of roof rock dislodged from between the row of roof bolts and the right coal rib. The ee sustained simple non displaced fracture of the right transverse process.
While installing lifeline along belt loop of line came in contact with roller. He was holding onto rest of line and it pulled his arm into roller.
The #11 motor was pulling the track duster to the James Creek Section while being followed by the #10 motor. The #11 motor ""saddle bagged"" in a dip and was struck by the #11 motor. The employee that was riding as a passenger in the #10 motor was diagnosed with a lumbar strain and contusion to the left knee. Medical diagnosis was not received at the mine site until 8-20-2009.
He went to back of miner to check his center line. He got down on his knees and a piece of rock fell from the top, 2'x 2'x 2"", which punctured his boot and cut his ankle. Injured received 5 or 6 stitches to close wound.
Employee was hit on the right side of the face by a rock that fell from the rib and came inside the scoop canopy.
Injured at shift change while stepping into launch's plywood floor, caught foot in floor, tripped and twisted his knee.
EE was getting out of the scoop. He had one foot out of the scoop (left foot), as he was pulling his right leg out of the cab he struck his shin on the cab frame, cutting his shin.

A miner stepped off a tractor onto a rock and rolled his right ankle. The ankle was severely sprained with a possible fracture. The doctor placed the miner on light duty.
Trip over door jamb going into building.
Employee was tramming the roof bolting machine. When the machine went through the check curtains, the drill steel got caught on the curtain, causing the drill steel to strike the employee on his left neck and shoulder.
Employee was loading a piece of belt structure into a scoop, when the bucket was lowered on his right foot.
Employee claims he was cutting a large hopper and while cutting the beam, he cut completely through. He stated he did not know he had cut through and had turned to walk around and cut the other side of the beam. He states when he turned around, the beam fell and hit the right side of his shoulder.

The man was installing a bottom roller in the conveyor belt when he felt a pain in his right shoulder. The man continued to work regular duty until 11/10/2010 when he reported off for surgery.
While walking, injured his knee on the I-slope. Sent for x-ray determining strain of ligaments in left knee. Returned to work next scheduled shift and worked until MRI app 8/10. Worked on and off complaining going to the doctor several times. On 9/20/09 took additional MRI and then again on 9/30/09. Received results 10/5/09, determined it requires outpatient surgery 10/14.

Injured was sliding 1 1/2"" pipe inside a 4"" dewatering pipe. Hand struck 4"" pipe and caused injury.
EE was backing a 35C Fairchild Scoop down the #3 entry along the #2A beltline. At the #7 crosscut there were five fly boards lying to the side of the entry. The rear tire of the scoop ran over one of them, causing one end of it to flip up and enter the operator's deck, hitting EE in the ribs and back pinning him in the deck.
EE was traveling on personnel carrier, when a piece of draw rock fell from between bolts, hitting his left hand while his hand was on the steering wheel. Contusion to his left ring finger. The draw rock measured from paper thin to 1"" thick.

Employee was watching slack on miner cable when a piece of rock fell out between the bolts and the rib and struck him on the right leg.
While changing oil, something went in employees eye. It went unnoticed until later. Employee was off Thur. 8-6-09.
Employee was operating roof bolter machine. He went to place the roof bolt wrench in the drill pot, the drill steel fell from the mine roof and pinched his left index finger between the drill steel and the wrench, resulting in a laceration.
Employee was cleaning in the prep plant with a shovel. When he twisted around he felt pain in his knee. Employee worked until 08/15/2009. It took until 09/16/2009 for it to be determined from a peer review if this was a new injury and not a re injury. This is why the 10 day limit was passed.
Tightning lug nuts, wrench slipped and he fell onto wheel. Fractured right orbital.
EE was roof bolting off the miner in #280 wall crosscut,#5-#6 entry, 3 N. left section. While reaching to pick up a drill steel that had fallen across the drill head to use in retrieving the drill steel still in mine roof, the steel in the mine roof fell & struck him on the L. hand resulting in a fracture to the metacarpal of the left little finger. Also a laceration-3 sutures.
The employee was assisting another miner to load utility doors onto a scoop when he said he felt a pop in his lower back. The area became increasingly painful over the next couple days and he sought medical attention.
Employee unlocked the tool trailers, set the lock down on the top step, went inside, gathered up the tools he needed and upon exiting the trailer stepped on the lock that was laying on the top step. Result - twisted left ankle, hyper-extention of left arm, contusion on forehead, glasses pushed up against head resulting in minor scratches.
The employee cut the band securing a load of timbers inside the scoop bucket. As a result of the incident the timbers rolled knocking him down spraining his (L) leg. The employee is missing time.
EE was walking to an area to determine the source of a water leak when he slipped and fell to the ground, injuring his wrist.

Employee was walking down the long wall face when he slipped on a rock and fell on right arm. Employee did not start missing time from work until 11/17/2009 because of surgery on his right elbow as a result of the accident that occured on 08/04/2009.
Employee was coming down from having cleaned material from a conveyor belt. When he stepped down, he stepped awkwardly on a block and fell to the ground approximately three or four feet.

Employee was pulling a 1.5"" dia. water hose and felt a ""pop"" in his side as he was placing the hose (90 feet long) into the holder that ran along side the conveyor. He was not lifting or pulling the entire length. Employee is being scheduled for surgery to repair hernia and will lose time as a result.
He was starting to drill hole with bent steel, causing him to jar shoulder **started missing on 08/31/09**
Employee was helping tireman lift a medium sized tire to the bed of the tire truck when he experienced a burning sensation in his stomach. He was examined at medical center and diagnosed with ""Umbilical hernia"", and scheduled for surgery August 13, 2009.
Employee was drilling a hole in the roof with a roofbolter when a piece of rock broke loose and slid down the drill steel. The rock flew off of the drill steel and struck employee in the mouth causing him to receive stiches in his bottom and top lips.
Employee was replacing a support shaft. While putting the shaft into place, the shaft shifted and his fingers were caught between the shaft and the wall resulting in fractures to 2 fingers. He was evaluated by a physician and was taken off of work.
EE climbed up on ladder to hang bolter cable, when leg of ladder broke causing him to fall striking his shoulder on the ground and leg twisted up in ladder. He on eveshift reported putting damaged ladder with trash to be brought outside. He didn't know ladder was damaged and used it.
EE was cutting a tarp. Cut palm - left hand.
2 employee's were rolling a drum that was filled with absorb-all and oil. As they rolled the drum, it tipped over and fell on one of the employee's. As the other employee lifted the drum off of him, he strained his back. The drum weighed approximately 400#.
Washing down floor, stepped back and fell through danger tape. Fractured fibula.
EE was loading bolts on a pillar block, when the wrench slipped off, catching the right ring finger between the wrench and the rib. He received 4 sutures to the right ring finger.

While walking toward a personnel carrier, injured employee tripped over a rock. He fell, causing a strain to his ankle. Employee saw a specialist on Wednesday, August 12th. The doctor took him off work on this day.
Injured was getting out of a scoop operator's compartment when he slipped. He fell against the scoop and the door closed on his left wrist fracturing it.

The man was installing belt structure when he felt a pain in his low back.

Ee was using a hand shovel trying to pry a rock from the track on a dozer. He knew his shoulder was hurt immediately. He had to have surgery for a torn rotor cup in his shoulder.
Mechanic was loosening the end of an engine hose when his wrench slipped, causing his thumb to be pinched against the haul truck structure. This resulted in a cut to his right thumb.
Employee had walked the main belt-line and came outside. He was at the drift mouth at A-1 belt. Employee bent over to pick up a piece of chain link wt. about 20 lb. He developed a sharp pain in his low back. Dr. at the E.R. examined him and said he had acute muscle strain, told follow up with family Dr., he saw family Dr. 8-5-09, and was taken off work. (Mine Closed 2-10-10)
Employee was removing a U-Joint from a scoop with a hammer. While hammering a piece of metal, neddle bearing, embeded into his left forearm.
EE tripped while stepping over a screw conveyor and rammed his toe into the housing.
On 6/16/10, the employee underwent surgery on his left shoulder as a result of an injury on 8/2/09. On that date, the employee attemted to carry a jackleg down the longwall face and complained of pain in his left shoulder.
Employee was standing on batteries at battery charging station. His hardhat tangled in the mesh and he was attempting to untangle his hat when he fell back, striking his head against a roof bolt, causing a laceration to his scalp.
Reported: Operating roof bolter in face. A piece of draw rock fell to the mine floor. The rock flipped over striking his left calf muscle resulting in a deep contusion to is calf muscle.
Employee states the bottom roller bracket hit him on the right knee. The blow caused the knee to buckle sideways to the left. EE did not request medical attention until later and was scheduled to see Yampa Works August 10, 2009 and his first missed shift was August 14, 2009.
The foreman, who had prior soreness in his back, was operating a dozer. At the end of the shift the miner could barely move due to increased discomfort. Two days later the miner sought medical attention where a doctor treated the miner for muscle spasms. The mine operator was not made aware of this reportable injury, through medical documentation, until 8/11/2009.
Employee lifted a light generator to attach to the ball on a truck. When he set the generator down, he felt something pull in his back.
