An unintentional roof fall occurred on 7/31/09. The fall occurred from #1 entry on #1 unit near spad No. 2236 to spad No. 2364 in #4 entry on #2 unit. No injuries occurred from the fall (3/4"" 6' resin bolts).
On 07/29/09 a methane ignition occurred on the 1-rt. panel off the #6 east mains on the mmu#001 section. On 7/31/09 a trace of smoke was seen at the location of the methane ignition . There was no evidence of a mine fire. MSHA was notified by phone of the trace of smoke on 7/31/09.
There was a hyd. leak with the fluid in the gear box, this stalled the hoist and it would not budge, so I called MSHA and reported the hoist down, the hoist this time really never went down. We allowed it to cool a few minutes and as the ATF Fluid cooled, the hoist functioned as required.
Moisture accumulation in the proximity switch causing it to fail.
The hoist car servicing the #1 track slope was taken out of service from 4:50 p.m. until 9:00 p.m. when it was discovered the roll back was not working.
A roof fall was found 6 sub panel north. The fall took out a return brattice in a panel adjacent to one their in now. The fall is app 175 ft lontg 8 ft high 19.5 ft wide at spad #10+96 on old #3 unit. The fall was discovered by the fireboss.
The #8 elevator was out of service for a period of time exceeding 30 minutes due to the govenor overspeed switch being ajar. No individuals were on the elevator at the time. The overspeed switch was reset, the elevator examined and placed back into service at 4:15 pm.
At approximately 3:20 pm some people got on the Hart street elevator at the bottom of the shaft. They attempted to come up the shaft to the surface but the elevator would not move. The doors could also not be released to open them. The persons were trapped on the elevator until electrical switch repairs were made. The elevator was returned to service at 4:10 pm.
Discovered a roof fall in 1 nw butt when doing weekly air course run. Fall was in #2 entry 2 X-cuts inby spad #173, extending about halfway into the intersection. Fall was about 40' long by 18' wide and 4-7' high. The area was bolted with 4' fully grouted resin bolts. The area was dangered off with two rows of posts and danger signs and all sides.
Preshift examiner discovered a roof fall in 2nd SW Butt #1 Entry (room neck) Inby spad #547. The fall was in the intersection extending into #1 entry and x-cut 2-1 in both directions. Fall was approx. 60' long by 18' wide & 6'-8' high. Area was bolted with 6' super twist bolts. The area was dangered off with cribs and danger signs on all sides.
The fall occured in the #4 belt entry spad #162. Fall was 0"" to 8' high 20' wide 30'long. the fall will be cleaned up and bolted to std.
Roof fall 1 x-cut inby station #7990 on D-Butt beltline. 70 ft long, 18 ft wide, 10 ft high.
On 7/29/09 a methane ignition occurred in the last rt. cut while retreat mining between the #3 and #4 headings on the 1-rt. panel off of the #6 east main on mmun #001 section. MSHA was notified by phone of the methane ignition. Both the MSHA call center and the supervisor were notified.
An unintentional roof fall occurred in the #1 entry of 4 South mains. The fall was located at SS 175+20 in a return entry. The fall measured approx. 25 ft long by 16 ft wide by 8 ft high. No injury or equipment damage as a result of fall. Fall will not be cleaned up.
A roof fall occurred in the South East Mains at cross cut #31, #3 entry. The roof fall was approximately 18 feet in length x 18 feet in width x 14 feet thick. The area was supprted with 6 foot fully grouted roof bolts.
A roof fall occurred at #4 Mains Section (#12 Crosscut, #2 Heading, 3500ft outby the face). No ventilation, travel ways, or equipment were disturbed. No employees were injured. The fall was approximately 16ft x 16ft x 7ft. The area was bolted with 6ft Super Twist bolts.
10' of intersection at station 6989 on 10' radias from the solid. The area was bolted with 6.5' bolts and T-S Channel 10' cable bolts. The area is cribbed up.
An unintentional roof fall was discovered by a mine examiner making a weekly examination. The roof fall was approximately 8' high x 16' wide x 50' long and occured at the 106 crosscut in the #1 entry of north mains.
at 5:24 pm the mine manager and operation mananger noticed a grass fire in the DIII mining area. The fire was located in an area behind the mining activity near the intersection of abandoned hiway #850 and county road #421. The local volunteer fire department was called out and extinguished the fire at 6:15 pm
The roof fell in the 2 Left Submains at break #25 in the #4 Entry. The fall is 20 ft. wide by 100 ft. long to the left and right of the intersection.
Rock fall occurred in 1st Northeast Paralle #8 Entry Spad #1960 app. 14,000 from nearest section.
A roof fall occurred in the #2 entry at 3B 5 north longwall at 93 XC, survey station 116+00. Approx 20' X 20' X 15'. Investigated.
Firebosses were pre-shifting the mine for the midnite shift on Sunday night in 5th West entry #2 at x-cut 47-48 when they found a roof fall. It was dangered off and reported to shift foreman and MSHA. The fall was 20' wide by 75' long and approx. 8' high. A double row of timber was set at all accesses to danger the area off.
Hoist down due to electrical problems. It was decided to bring out the factory rep from Siemens to go through the hoist and look at all settings to make sure it will stay operational.
A fall was discovered at 8:40 pm on 7-26-09 by foreman who started his shift at 6pm to preshift. The fall was in the #3 belt entry at SS No 881 of the No. 1 main west set of entries. The fall measured 20' wide by 25' long by 15' high. The fall will be cleaned up and the area supported.
A roof fall occurred in the #7 entry 250 feet inby spad #18010. the fall was about 20 ft wide, 40 ft long, and 8 foot high. The area was bolted with 5 foot torque tenison bolts. The fall was crib and danger off, and will not be clean up.
Rock fall occurred in the 1st Southeast Main in #5 Entry Spad #28+00 approx 10,000 ft outby nearest section.
Electricians were in 4330 north post splicing cable to a MCL back together when the power went off to fan. Electrician and supervisor walked through drifts to locate problem. They stopped by barricade for an Inactive area and saw muck from a fall of ground. The fall of ground affected ventilation to heading downstream, no one was working in heading.
Roof fall between #44 and #45 brk at 8HLG Extension Track and Belt Entry. 50' Inby #45 Brk. Approximately 50'Lx20'Wx8' to 12' H. Had fell out above the anchorage. Area has been breakered off at this time.
Our hoist used to hoist a mantrip car at the Wheatland portal experienced a blown hydraulic hose while hoisting the mantrip car out of the mine. The brakes automatically set on the hoist and the hoist became inoperable. The hoist was repaired and returned to service.
An unintentional roof fall occurred on 7/24/09. The fall occurred on #6 entry (intake) near spad No. 2587. No injuries occurred from the fall (3/4"" 6' resin bolts).
An unplanned roof fall occurred on MMU No. 003 section entrapping the continuous mining machine while or during retreat mining in the No. 3 entry. The roof fall occurred in the rt. run of the coal pillar inby survey station #9086. The roof fall is approximately 8 ft long x 15 ft wide x 2 ft thick. (Note: Roof fall reported to MSHA by phone.)
An unintentional roof fall occurred on the old 1A return @ station 31+80 between entries 9 and 10. The fall did affect the ventilation on the #4 unit which was idled until all work was completed and ventilation re-established. The fall did not affect passage of men or cause personal injury. The fall had been bolted with 6 ft. fully grouted roof bolts and 12 foot cable bolts.
The roof fall was discovered at 6:18am in the #4 Mains Section (#16 Crosscut, #5 Heading, 3200 ft outby the section). The roof was supported by 6ft Super Twist Bolts and 12ft Cable Bolts. The fall was 10ft x 10ft x6ft. Ventilation and machinery were not disturbed, and no one was injured.
East side of 23W at 4N intersection roof fell above the anchor point of the bolts. Mechanical scaler was working on northeast corner of intersection at the time of the fall.
On 7-22-09 at 8:35 pm a roof fall occured on the # 2 section in the # 7 entry.
A roof fall measuring 75' long, 20'wide and 4' thick was found in the return entry near spad 23874 near EP 24. It occurred due to layers of slate, 48"" glue bolts had been used. The area was mined 12/2/1997. Ventilation and the passaage of men was not affected
#1 Production hoist down electrical 75% speed switch was out. Called MSHA at 5:05 pm. Had to wait for 10 minutes for someone to answer, asked him to make note of this. Called MSHA back at 8:31 to report hoist was running.
A roof fall occurred in the #9 seam located in the 2 East off of the 2 Main North between crosscut #54 and #55 in the #6 entry. The fall measured approximately 45'x18'x7'. The area was supported with 5' fully grouted bolts.
An unintentional roof fall occurred in the 2nd East panel off of the 1st South sub main in the #8 entry at station 23+75 in one of the return entries. The fall did not affect ventilation, passage of men or cause personal injury. The fall will not be cleaned up and was timbered off and dangered out.
A roof fall occurred in the 2nd Norht Main at cross cut 72 in the main intake in the #9 entry. It is unknown when the roof fall occurred, it was observed by the Mine Inspector while examining the main intake.
An unintentional roof fall occurred at the Galatia Mine New Future Portal at the Main West #1 Belt Crosscut 21 2845 W between #2 and #3 Entries. The fall was approx 20' long, approx 18' wide and approx 6-8' thick. The fall did not affect ventilatio or passage of personnel. The fall will not be cleaned up.
#3 hoist kicked on ""East Motor #1 Bearing Overtemp"" fault. Still had two escapeways during the incident.
An unintentional roof fall occurred in the 13 I section Return at SS# 26+40. The Fall measured approx. 60ft long by 16ft wide by 10 ft high. *** No injuries or equipment damage as a result of this fall.
Rock fall occurred in the 1 East Mains from break #45 to break #47 in the #5 entry. Fall measured approximately 20' wide x 180' long x 5' thick.
Examiner found a roof fall in the 8N bleeders during his weekly examination. The fall was at #30 crosscut and measured approximately 18.5'W x 25'L x 5.5'H. The area was mined in April of 2006 and is 7,120 feet from the nearest face.
An unintentional roof fall occurred at the No 4 belt entry, inby spad #1364 one break. The fall measured approximately 30' x 19' x 8'. The roof fell on beltline and belt structure. The fall has been timbered off and will be cleaned up. The fall effected the passage of men but not ventilation. The area was supported with 5' fully grouted roof bolts.
A roof fall occurred above the roof bolt anchorage in the Prop Entry ""1 Entry"" in the Intersection at Spad 18422 on No. 2 Longwall. The area of the rock fall was approximately 40 feet by 22 feet wide. The roof fell approximately 4 feet above the roof bolt anchorage.
A roof fall was found on the South Mains ""A"" mainline belt inby 54 cross cut. Fall approx. 30 feet long, 18 feet wide and 12 feet above the normal roof horizon. Area was bolted with Install II bolts and resin assisted. The supplemental support consisted of bacon straps, wire mesh screen, steel beams and metal jacks. Cause of fall age that is was mined and weathering of the roof.
A roof fall measuring 4' X 20' X 50' occurred in the intersection of #5 and #6 entries near spad #2842. This area is outby the active section.
Fell on top of the belt power center, 5'Hx12'Wx20'L. Rock was hot to the touch. Power center was vented to return with about 1,000 CFM. Power center lids were bent in, but still operative.
An unintentional roof fall occurred in the 13 I miner section belt entry. The fall measured approx 80ft long by 16ft wide by 12 foot high. No injuries or equipment damage as a result of the fall
An owl shift repairman discovered a non-injury, non-intentional roof fall 1 break inby the section power center on the 002 MMU. The fall measured 40' x 18' x 8'.
Unintentional roof fall occurred in 4-Right (001) Section while mine was idle. Fall found by pre-shift examiner at approximately 8:45 PM - called outside at 9:05 PM. Fall from x-cut R-2 entry to L-1 entry (175 feet x 19 feet x at least 8 feet in height). Area supported with 4-foot fully grouted bolts, except belt entry supported with 6-foot super-twist & T-5 Channel.
Rock fall occurred in the 1 East Mains in the #1 entry between break #57 and break #58. Fall measured approximately 20' wide x 30' long x 5' thick.
Rock fall occurred in the 3 East Mains at break #34 in the #2 entry. Fall measured approximately 20' wide x 40' long x 8' thick.
Hoist at the main shaft had power problems - lack of power reaching the hoist itself. Found through troubleshooting process that a voltage phase monitor (electrical component) had failed. Replacement of the failed part returned the hoist to service some 2 hours later. Five miners underground were evacuated out the secondary escape shaft.
A roof fall occurred in the #9 seam located in the East mains at crosscut #26 in the #7 entry. The fall measured approximately 15'x15'x10'. The area was supported with 6' tension rebar bolts and 12 cable bolts.
The #8 elevator was out of service for a period of time exceeding 15 minutes during an electrical storm when two fuses blew. Two individuals were on the elevator at the time. The fuses were replaced & the individuals removed from the elevator @ 12:40 PM. The elevator was placed back into service @ 12:50 PM. No injuries were reported.
An unplanned roof fall occurred in the old works area behind 17 kickback in the mno 2nd east. The fall was above the anchorage zone. There was no one trapped or ventilation blockage or disruption.
2:57 AM #1 hoist down. #1 converter is out.
5:15 PM #2 hoist down. Oil temperature switch is out.
An employee turned off the main underground fan in order to apply shotcrete near the fan. Underground mobile equipment still operating caused elevated levels of NO2. The miners were brought out of the mine, the main fan restarted, the mine reventilated, and gas tests completed in all work areas before returning miners to work.
Operation of the man/material hoist was interupted for more than 30 minutes due to a thunderstorm, which caused fuses to blow in the hoist electrical system.
A roof fall was discovered in the 4 North 3 Returns, #4 Entry between Crosscuts 6 & 7. The fall measures approximately 20 feet long, 17 feet wide, and 5.5 feet thick. The area is supported with 5 foot torque tension rebar bolts.
An unplanned roof fall occurred in the #2 gas mine in the #3 entry at spad #1395. Measured approximately 40' long x 20' wide x 12' high. No injuries were reported. The area was developed on 3/27/09.
A non-injury roof fall was discovered at 12:10am on 07/15/09. Size of the fall is 75' X 8' X 19'. Fall will not be cleaned up and ventilation is not affected or travel. All areas around the fall have been cribbed off.
A fire ignited in the bin of contractor's wood chipper & spread to a telephone pole storage area. Inventory level was approximately 2,000 poles. There were no injuries/illnesses resulting. The wood chipper/grinder & poles were destroyed.
An unplanned roof fall occurred in the #2 gas mine in the #2 entry that fell across to the #3 entry at spad #1410. Measured approximately 115' long x 19' wide x 12' high. No injuries were reported. The area was developed on 4/1/09.
A roof fall occurred on #4 Unit, #2 Entry, from 47 to 49 Crosscut. The fall measures approx 150' long, 17' wide and 7' thick. The area is supported with 5 foot torque tension rebar bolts.
A roof fall above the anchorage zone occurred in the 7th East No. 1 tailgate entry to the Gil-8 Longwall Section. The fall included the tailgate of the longwall and approximately 14' outby. The fall blocked use of the alternate escapeway, but did not adversely affect ventilation.
Fuel truck began leaking fuel from the hose. The truck caught fire and burst into flames destroying the fuel truck. On August 19,2009 the insurance co. told the owner they would pay $36,000.00
A roof fall occurred on the #1 section, entry #6, spad # 372 of the Allegiance Mine Cook Portal. The fall measured 20ft long, 20ft wide and 6 ft high. No injuries resulted from this fall.
On 7/14/09 a dozer operator contacted Operation Supervisor to inform him of a grass fire on the NW side of the DI Area Auxiliary Coal Stockpile. 2 water trucks were called to the scene & 2 dozers were used to cut a fire break. The fire was not under control within 30 minutes so MSHA was called(3:45pm). The fire Dept. was called & arrived at 3:50pm. Fire was under control by 4:15
Elevator stalled for 129 minutes. MSHA was call on 7/14/09 @ 7:25 AM.
An unintentional fall of the mine roof was discovered in the #1 entry at #37 X-Cut in the Northwest Mains. This fall was approximately 6 to 7 feet high, 20 feet wide and 50 feet long.
While conducting his fireboss run the mine examiner identified a roof fall in the old Zippo Mains #7 entry near spad #10587. The fall was approx. 60'L X 20'W X 4' High. The area was supported with 4' fully grouted resin bolts. The fall will not be cleaned up and the area has been dangered off and timbers set.
Rock fall occurred in the 3 East Mains at break #19 in the #5 entry at spad #5439. Fall measured approximately 20' wide x 40'-60' long x 6'-10' thick.
Roof fall in 23-m section #4 block. spad location 9658. This is a weekly air course and ventilation not change. Fall was dangered off on the outby approach. This is an outby area that does not effect a working section.
Fall on 5A belt intersection and 2 ft Inby - Spad # 3988. Fall is 1 x cut inby this spad. Fall was approx. 18 ft wide x 50 ft long x 8 ft high. No rider was present. Slate top and laminated.
Had a hyd. problem on the main hoist and when it became obvious that it was going to be an issue, the hoist was shut down, then the crews were pulled in the secondary and MSHA was notified of the down shaft.
During weekly exam, the mine examiner found a roof fall on old 2 belt. (Not working section). Fall was in #3 entry on roadway @ 34 cc in intersection. Fall approx 20' x 20' x 4' thick. Fall is cribbed off and dangered off. A-1312-78
Maintenance personnel, responding to a dust emission report at coal mill #2 dust collector, found pressure relief panels (2 of 4) were open. Believing that an over pressure event occurred, MSHA was notified @ 2:25 AM. At 5:00 AM, MSHA was again notified of a fire that had begun @ 5:00 AM. This fire was quickly extinguished prior to 30 minutes.
A rock fall was discovered during the preshift exam in the last open break in #3 entry of the 001 section. The fall measured 20' x 25' x 8'. The fall has been cleaned up and arches installed for support.
Kiln GK3 pushed due to lack of fuel to calciner. Drive belt for calciner coal feeder broke at 4:15 AM. Cntrl Rm Opr received an alarm for coal feeder speed 40 sec after the belt broke. Lack of fuel caused kiln to push (rush hot feed thru kiln) and hot material went to the clinker conveyor system. Diverter gate was opened too late to avoid burning belts.
The EE's were removing a joint of pipe from a dewatering hole and they were using torches to cut the pipe in two. While cutting the pipe, there were several small pops heard. The EE's did not see any flames.
Rock fall occurred in the 2 North Mains main return at break #22 in the intersection of the #2 entry. Fall measured approximately 60' wide x 40' long x 6' high.
A roorf fall 9' W x 540' L x undetermined H fell at 12:30 AM in 9 #3 rooms section. The fall does not impede ventilation or travel.
A roof fall was found at 9:30 pm on 7/9/09 at xc 244 entry 5 main south. Fall was in four way intersection that had been mined August 1997 and supported with 8' resin bolts. Fall measured approx. 20' wide x 30' long x 12' high. No injury or property damaage occurred. There are no plans to clean up this fall.
An unintentional roof fall has occurred at the MMU 007-0, 1 South 2, #47 Crosscut, in #9 Entry. The area was roof bolted with 5' torque-tension, fully grouted bolts as primary support with 8' fully grouted bolts as supplement roof support. Roof fall was 20' long, 16.5' wide and 10' thick.
A roof fall occurred #5 Entry--spad #185, 20ft x 20ft x 5ft. Timber and Cribs were set.
At 7:30 am during preshift examanation a rock fall was found on #3 belt line. at spad # 852 the fall measured 6 ft. thick by 18 ft. wide by 80 ft. long.
A roof fall was found in the abandoned C-1 butt in the intersection of the #3 entry approximately 300' outby the face. The fall was 60' long x 6'-10' high. Area was bolted with 6' fully grouted 3/4"" bolts. Area was dangered off and timbered.
Due to an electrical short , the magnetic upper limit switch failed, causing the access cage to become inoperable. Two new switches were installed before operations could continue (one was faulty). Mine evacuated. No injuries; no problems. 37 persons underground. Back in operation at 2:50pm.
The Cedar Creek elevator was taken out of service due to a DC Fault caused by the brake liners coming out.
Discovered a roof fall in #7 entry, 1 break outby spad #2991. Fall was in the intersection and measured 5-1/2' thick x 20' wide x 30' long. This area was bolted with 4' and 6' fully-grouted resin roof bolts. Fall is 2000' from the active section. The fall has been timbered off.
We had a roof fall in the 3 West Mains in the Return in intersection 6258 spad number about 40' long & 9' thick. Fall has been timbered off and there are 2 other entries still open that can be traveled.
Eight miners were stuck on the Service Elevator due to a DC Fault due to the brake liners.
On 07/08/2009 the firebosses found an unintentional roof fall in the 5th West section at x-cut 46-47 in the #5 entry (return). The roof fall measured 30-40 feet long by 12 feet wide and was 5-6 feet high and 5 foot resin bolts had been installed in this area. Not an escapeway-no ventilation impeded-no injuries.
Solid fuel ignited at the solid fuel facility. Fire was extinguished within 30 minutes.
The No. 2 entry at spad 283 roof fall. 7' high x 15' wide x 60' long.
Roof fall approximately 19'W x 20'L x 5-6'H in #2 entry 1 brk inby spad 324. The area had been bolted with 5' mechanical anchor resin assisted bolts. Fall creates no hazard.
Hoist Nordberg Model # 70301271 went down due to some power bumps in the area. The power bump caused the cooling fan for the DC drive to burn up. the hoist was back operating on 7/8/2009 at 12:10 PM
A roof fall was discovered by air-way examiner during his weekly run. No one was injured, no interuption in ventilation and no equipment involved. The fall was in return in #2 entry 1 block inby spad 5163.
In the process of making the weekly exam, a roof fall was discovered by the examiner. The location of the fall was in the cross-cut immediatly outby #22 Seal. No persons were injured or equipment damaged due to the event.
An unintentional, non-injury, roof fall was discovered in the #7 Entry of the right return aircourse of 001/002 section. The roof fall was 20 feet X 8 to 9 feet X approximately 50 feet. Ventilation was not interrupted.
A fall was discovered in the worked out No. 1 Northeast marker panel. The fall was in the No. 2 return entry at survey station No. 6527 and measured approximately 20' long x 20' wide x 6' high. There were no injuries or equipment involved. The fall will not be cleaned up.
A fall was discovered in the worked out No. 1 Northeast macker panel. The fall was in the No.2 return entry at survey station No. 6507 and mneasured approx. 20' long x 20' wide x 6' thick. There were no injuries or equipment involved. The fall will not be cleaned.
An unplanned roof fall occurred in #5 entry, spad # 4155 on the intake airway approximately 950 feeet from the working section. The fall measured 20' long, 40' wide and 5-8' thick. Ventilation was not interupted and no persons were affected. The strata is comprised of shale and sandstone, laminated with no rider seam present.
Unintentional roof fall that occurred on HG #3, #2 entry, x cut 173. The fall measured 35'l x 18'w x 8'h.
Encountered rock fall on preshift of the belt line 80' outby spad (751) (26 brks outby face) (18 brks outby power center) soft long 6-7' high includes intersection and inby 3/4 or 30' of block #4 entry beltline (no injuries) (no men underground). Fell during miners vac.
Possible lightning strike to wheel loader causing a fire. We were notified by a previous employee at approx. 4:00pm. We immediately called dispatched and headed to mine site. We arrived at 6:15pm and the loader was still smoldering. No one had been at mine site since Thursday. An insurance investigator is coming Wednesday to investigate the scene. The loader was a total loss.
A ROOF FALL WAS FOUND AT X-CUT 41 ENTRY # 8 MAIN WEST INTAKE.
At approximately 11:00am a mine examiner discovered a roof fall on the 9F section in the No. 1 belt and track entry at crosscut #11. The fall was in the intersection and was approximately 20'L x 23' W x 8"" H. The area was 2,323 feet from the nearest active working face and was mined in August of 2008. The supply track for the 9F section was blocked off by the fall.
A non-injury roof fall was found in the #1 entry, #1 crosscut of 11 west, wooden crib and cable bolts had been installed in the intersection. The fall measured 25' long X 25' wide and 6 to 10' high.
Hoist was down due to an electrical problem (dead batteries).
A roof fall occurred in the East Mains in the intersection of crosscut 23 in #2 entry measuring approximately 25ft. long x 18ft. wide x 8ft. high. This area was supported by square cribs, 4ft. fully grouted roof bolts and 8ft. cable bolts.
A fall of roof occurred in the 2nd Northeast Parallel in the 3A belt line at spad # 74+ 90 cross cut # 107. The fall was approximately 20ft long x 20ft wide x 8ft high.
An unintentional fall of the mine roof was discovered in the #5 entry at 34 x-cut in the northwest mains. the fall was approx. 6 to 7 feet high, 19 feet wide and 60 feet long. The fall was immediately reported by phone. (note: this occurred during mine vacation period and this mine returned to work on 7/13/09).

A reportable roof fall was found in entry 4, crosscut 4 of F South Mains. The fall was estimated to be approximately 50' long, 30' wide, and 16' feet high.
Employee was driving a loaded hauler when he backed the hauler into a pond with no hesitation.
The victim was operating a walk behind block saw on a ledge. The victim and the saw went over the 9-foot ledge and the saw fell on him. This mine closed in 2005 and the operator did not notify MSHA that mining operations had resumed.
Finger caught between drill pipe and upright stake.
Mechanic was working on Mack road truck adjusting clutch. He asked the other mechanic to depress clutch from inside the cab. The mechanic had not moved his hand when he asked the other mechanic to depress clutch and when the clutch was depressed the mechanic caught his middle finger on his right hand severing the top of his finger .
Injured employee was troubleshooting the alternator on a CAT D( N doxer when he instructed the operator to reenter the cab and start the dozer so he could check the voltage on a multi-meter. He then used his right hand to check the alternator belt tension at the same time the dozer started resulting in the partial amputaion of his right index finger.
Installing roof bolts, hit lever with elbow causing head to raise cutting off tip end of his thumb on right hand.
While tramming a battery powered Fairchild 35C Injury's middle finger of his left hand became tangled in a nylon cable hanger causing it to pull the distal phlange off.
The tire company was having trouble mounting the tire. EE went to see if he could help, the tire bar slipped and his right pointer finger was caught between the tire and the rim resulting in the loss of his finger tip.
The individual was operating a roof bolting machine. He had his hand positioned on the boom lift of the roof bolting machine. As he raised the drill head on the machine, he caught the tip of his right ring finger. The tip of his finger was surgically removed by the physician.
The mechanic was prying the top of the LR suspension out of its pinned position. The suspension was supported by a come-a-long near the bottom of the barrel. As the weight began to shift, it caused the prybar to recoil rapidly, pinching/severing the tip of his right pinky finger.
Employee was at 140 Shield waiting to repair ram hose. Another employee was pulling shields and told him that he was putting a prime in to pull shields. The employee got his toe of right foot caught under relay bar and when the shield started to pull, it pinched his lower leg between the relay bar and the shield.
Employee was holding a bolt while another employee was knocking a pin out and his thumb was hit with a sledge hammer.
The miner operator was rockdusting the face with a machine mounted hydraulic Bantam duster. The dusted became plugged with damp rock dust, so the operator removed the guard to clear the plug with his hand. In the process he accidentally leaned against the toggle switch on his remote control, activating the auger.
Rock fell from roof and landed on upper back (4' x 4' x 2' thick). Employee struck head on rock on ground.
Employee was changing tires using a tire machine. He had changed several tires and developed pain and swelling in his right knee.
EE operating truck was turning right out of a heading into the main haulage & got too close to the right rib & when turned the truck back to the left, right corner of the truck bed caught 6"" air line breaking it apart. When pipe broke apart it was bent by truck bed. The bent part of the pipe was against pinning him in the seat, breaking his left leg below the knee.
Employee stated that he finished cleaning the area and reached in scoop to get a bag of rock dust when a rock, measuring 12"" X 18"" X up to 2"", fell from top striking his left hand. He was initially released to regular duties but after a follow up was taken off duty. He began losing time 8-12-09.
Employee was replacing seals in tilt cly. Pin of the 990 loader slid off the frame and landed on employee's right edge of foot. Causing a fracture in the right little toe.
Employee stated that while he was lifting miner cable to hang he felt a pain in his groin area. He was released to regular duty but had surgery and began to lose time on 9-01-09.
Back dragging with dozer from the B2 stock pile when he backed off in a dip from a previous drag. This caused a jar, causing his neck to start hurting.
Employee was working on a screen in the plant when his knee pain worsen.
EE was driving key wedge down in the jaw crusher using a piece of steel shaft & a sledge hammer. A piece of steel came off shaft where it was being struck with a hammer. The piece of steel went into right leg above knee on inside of leg.
Replacing 60"" parts on the 9th 60. Lifting a top part up into place and felt a pain in shoulder, neck & hand. Wasn't reportable until 8/26/09 when it became a lost time incident.
Employee was struck in knee with muck bucket when it was being hoisted and received a knee contusion.
While cutting a bracket on some conduit, the conduit shifted, striking the ladder that the employee was on. The ladder fell, causing the employee to hit the floor resulting in a laceration to the head, requiring sutures. It was later found that the employee had suffered a non-displaced compression fractrue to his lower back.
Employee was working to pull CM slack up and as he was putting the pull rope on, the corner of the rib fell of and struck his right hand.
The machinist was tightening the bolt on the dozer part that was in the boring machine. He was using a pin wrench to turn the bolt with his right arm when he felt pain and a burning sensation near the shoulder joint. Employee was not restricted and did not miss work until today, 12/2/2009, when he was scheduled for surgery.
EE was cleaning on tailpiece on his knees out of sight of foreman. EE told foreman to throw him a crib block when he threw the crib block it struck ee in the lower back. EE wasn't scheduled to return back to rockhouse, accident occured on 7/31/09 he was to return back to solid his next scheduled shift. He suffered broken vertebra tips and hasn't returned to work.
Ee was changing iron onthe crusher impactor when he sustained a pain to his stomach. Doctor's visit confirmed a hernia
EE was operating a John Deere 40 ton Artic truck. While dumping a load of rock, a large rock flipped back and hit truck. This caused the employee to be jarred.
Employee was cutting curb using a steel hand chisel. A piece of steel broke off and went into his left knee.
Carried a heavy roll of vapor barrier from pickup to office, was squatting to set roll down on the floor and dropped roll on floor. Next day was experiencing some pain in groin area. Two days later was not able to walk very easily.
Employee states he was measuring for a cover for the drill box on the bolter. As he stood up he banged his head on the bolter canopy jamming his neck. He did not seek immediate medical care then, but after seeing a doctor he was put on restriction and his first missed shift was 08/11/2009
The man was prying a roof strap with a drill steel and the steel slipped striking him on the forehead.
Employee was working on a piece of equipment on his knees in the maintenance shop.
EE stated he was walking down slope. Slipped and fell, causing pain in lower back. Started missing work on 7/31/09
Employee was assisting in the installation of a cylinder pin when his hand slipped and was caught between the pin and a punch.
Sand on bottom step of loader. Slipped into rung and twisted knee.
Medical received 8/12/2009 the employee was setting a de-watering pump and strained his lower back. There was no injury/illness report filed with the company. He has missed 9 days of work and continues to be off.
While the employee was working on the left rear brake drum on the mechanic's truck, the brake drum slipped and smashed his right index finger, causing a laceration to occur.
Dismounting a rock truck, missed hand rail with right hand, foot slipped and he fell. Machine was wet from rain.
Injured put his hand behind the seat to a place where there was an opening to the air cond. blower in order to cool his hand. There was no barrier and the fan cut his finger.
Employee stated that while dismounting a shuttle car on the 11 Left section he stepped in a hole and twisted his right knee. Medical records were released allowing for the determination that this incident has been deemed reportable on 8/10/09.
Employee was pulling rib with slate bar and part of rib hit his leg when pulled loose. His first doctor visit and first day missed was 8/10/09.
Employee was unloading a big bag of rockdust when the corner of the bag gave way causing the employee to fall backwards onto the scoop. This caused a injury to the lower lumbar area.
Was lifting timbers and pulled his back. The conditions were good, no mud or water. Was not reported until 8-3-09.
Employee complained of back pain he said was caused by driving a haul truck. He noted that he had been having back pain for about a month, and was under the care of a doctor for his back. Also he thought at first his pain may be caused by his mattress. WC Denied claim per state law.
The company received medical notification on 8/12/2009 that the employee alleged he was struck by an I-beam in the stomach. He missed four (4) days and has returned to work. Please note that an injury/illness report was not filed.
A hydraulic hose burst striking injured on the side of his left resluting in blood accumulating within the eye ball.
Injured was stepping over the tailgate of his pickup truck, he placed his left foot on the bumper of the truck and his left hand on top at the tailgate. He swung his right leg over the tailgate. His left foot slipped off the bumper causing him to fall on his left side.
Employee jumped off knuckle of Rail Car and injured his right knee.
The injured was cutting a gear case off of a continuous miner, he was laying under the head of the miner when a piece of hot slag fell into his left ear. The injured sought medical treatment and returned to work. On 9/23/2009 surgury was required to close a hole in his eardrum.
Employee was performing maitenance and stepped off ladder onto uneven ground and fractured right ankle.
Employee was installing a roofbolt when a rock fell from between the coal rib and another roof bolt striking him on the lower leg.
Bobcat operator appeared to be having difficulty getting back up the ramp out of the way of the Loader. Loader operator was going to assist, Bobcat operator put Bobcat in reverse, making the Bobcat jerk. Jarred back of Bobcat operator.
After further investigation by the mine company on 01/15/09, it was determine that this incident was an injury. EE was pulling an empty water hose that was caught on a ladder. When the hose caught on the ladder it caused the ee to abruptly stop and fall backwards. Where the ee fell it was dry, even and clear of debris. EE sprained his right shoulder.
Three men were pounding splice on 4 left belt. 1 man was moving plunger while other two were pounding. He thought they were done and reached for plunger and was struck by hammer.
The injured employee, a section manager on MMU 001 was rolling up curtain in entry #1 when a piece of roof rock measuring 52"" X 36"" X 1 to 3"" thick fell from the roof striking him on his hard hat and knocking him to the mine floor. The injured sustained a compound fracture to his lower right leg.
EE was lifting and straining right wrist while lifting blower bagging to hang. Strain caused cyst to form and after treating with physician, EE had surgery on 2/10/10 to remove cyst.
Loading pipe on a pipe trailer, when he stepped on a small chunk of coal and rolled his ankle.
Employee was hanging the roof bolter cable and the operator pulled the lever to take up the slack. The cable took his feet out from under him and he landed on his back.
EE was checking sensor on belt. EE stepped over handrail onto a beam. A wire nut had came off sensor and when he touched sensor it shocked him causing ee to fall approx. 15 feet.
Walking beside 002 sec head drive, tripped and fell into the main line belt hurting his lower back and right arm and elbow.
Injured was removing reel from shuttle car when he felt pain in right shoulder
Got out of loader to check belt position, entered under hopper, put right hand on conveyer for stability. Belt grabbed glove, pulling hand under belt. It mashed some fingers, no broken bones according to physican. Did require stitches.
Employee was installing motor belt on the mill and while turning the sheave the wrench slipped causing him to lose his balance and fall down. (fell from even ground not from a height)
Employee was walking in the UG shop wash bay when he stepped into an eroded area of the salt mine floor. His ankle creeled and he fell. He was in the process of washing a piece of equipment with a pressure washer.
Miner stated, ""I was loading high-line cable into a sled and raised up striking my head against the roof. I was just ""stoved"" up."" Mine site began investigation to determine reportability. Please note that the initial medical report on miner was received on 7/30/09. A medical diagnosis was received on 8/4/2009 diagnosing the miner with a lumbar sprain/strain.
Employee was bagging hydrate lime. A bag busted and the hydrated lime went into his eyes. The lime irratated his eyes.
The injured tripped over the anchor chains for the belt tailpiece and fell against a belt stand and injured his right shoulder. The injured continued to work till 8/6/09 when he went to the doctor and was diagnosed with a torn rotator cuff. His 1st day off work was 8/7/09
EE stated he was crossing a belt, hung his right foot on belt rope, causing him to fall off belt. Struck 3"" water line, causing pain to his lower back. Started missing time on 7/28/09
Employee bent down to turn on water valve located approximately one foot from the ground. Twisted or tweaked his back in the process. EE is currently off work pending further medical evaluation and release from multiple doctors restrictions.
Person was using a water hose to wash off an area in which he was getting ready to work. Person lost his footing and fell onto left side hurting his hip and shoulder.
The employee had moved the CM from #3 heading to #2 heading. He then moved the cable from one side of #2 heading to the other side of the heading when he sprained his back.
He was climbing down a feeder and fell and hit right shoulder.
Employee was attempting to climb into a moving maintenance truck, when the open cab door struck a parked end loader counterweight, closing the door on the employee and pinching his pelvis between the door and door cab frame. Pelvic fractures.
Employee was operating a Dresser haul truck when the truck struck a hole in the road, jarring employee's lower back.
Parked loader, climbed down ladder and stepped one foot on the ground, pain in lower back.
Employee was walking to the water pump when he stepped on the hose and slipped and fell.
While roof bolting EE attempted to shove a 48"" resin bolt up he was holding on to. While spinning the bolt, he pushed it up. His glove became tangled, causing a fracture to four fingers. He refused emergency transportation and transported himself to Raleigh General Hospital Emergency Room where diagnosed with the fractures and referred to a surgeon.
Walking on uneven surface dragging a hose.
Employee was climbing up the ladder on a 777D haul truck, when he stated that his back gave out, causing him to fall about 5 feet to the ground.
EE parked mantrip, got out, was getting chock block out of rear compartment. Truck started to roll down hill, EE tried to get back in truck and fell dwon or was knocked down by rear compartment of truck. This resulted in 2 broken ribs and broken bone ni right foot (metatarsus).
Injury happend on 7/24/09, but did not get medical treatment until 8/05/09. Injured employee was assisting other employees, swinging a 16lb sledge hammer trying to loosen a nut on the liner of the head of the crusher. Resulting in a shoulder stain.
On 7/24/09 the intial accident occured, employee was attempting to mount a D-9 Cat Dozer. As he was mounting, he heard a pop in his knee. He was taken to the hospital where he was referred to a specialist for an MRI. He was returned back to work full duty. On 8/7/09, he went for the MRI reading and found a tear in his meniscus and was taken off work.
On July 24, 2009 employee was kneeling at the power center trying to hook the cat head up for the Bridge. As he pulled on the cable for slack, he twisted his right knee. He continued to work and on October 16, 2009 he had to have surgery for a lateral meniscal and has been missing work since then.
Truck coming down hill lost control and collided with highwall. Cracked rib - bruised knee - mild concussion
The employee was pulling and pushing on miner cable and strained his back. The event resulted in a lost time injury.
Employee taking slab off polishing line with jib crane/grabs to place on A frame. Slab was leaning and not stabilized. She tried to reposition the grabs but the slab cut her left leg and the weight of the slab or the impact caused her heel to fracture.
Doesn't know how he got hurt. Said his back started hurting after bringing mantrip outside.
The employee was preparing to spray the roof and ribs to fireproof for a charging station. He checked the machine for blockage and plaster sprayed into his eyes.
Right of feeder between 5 & 6 entry. He was taking a shuttle car tire off. Shuttle car tire fell across both feet.
Left rear step had been welded on the right side, when operator steeped on ladder the weld broke on right side causing the left shoulder to be pulled more than normal
Employee was building a brattice when he twisted his back.
Employee was cleaning highwall with front end loader when rock came off of highwall and struck the front end loader bucket causing pain in employee's back.
EE was walking along the #4 unit FCT low framing when he stepped on a piece of slick draw slate. He slipped and twisted his left knee. He continued working, notified the mine on 1/5 he would see his doc on about 1/8 to have his knee checked out to continued knee pain. He had surgery on 3/5 for a menisci tear of his knee. His 1st scheduled work day missed was 3/5/10.
Employee was climbing down the ladder of a CAT 777D Haul unit and lost his footing. He fell backwards and hit his head on the ground resulting in a 1 inch laceration to the back of his head. The injury required 3 stitches.
He was working on a gear box. He had to stand on a tail pulley guard to reach it, which is about 3.5 ft off the ground. While working on the gearbox he was bringing up a wrench and slipped as he was climbing the tail pulley guard. He landed on a rock as he fell which caused him to loose his balance and fall on another rock on his hip.
DID NOT START LOSING TIME UNTIL 8/20/2009. SURGERY REQUIRED ON THIS DATE. Stepped in a mud hole and pulled with his leg. Felt pain in groin area. Hernia.
He slipped going up the stairs to the truck and fell. He injured his lower back and his left ankle.
Employee was walking on a muck bound draw point hole, when the muck gave away. Causing the employee to fall approximately 10' from his heading.
Attempting to lift an 80 lb. bag of gunite when employee felt a sharp pain in his lower back.
EE was unloading belt material out of the back of a ramcar, when he failed to acknowledge an approaching ramcar and was struck from behind.
Employee had washed hydraulic oil from getman and when getting off of getman he lost his grip and fell to ground. He was seen by comnpany dr and he was diagnosed with a torn rotator cuff. He was taken off work effective 7/28/09
While employee was hanging miner cable he felt sharp pain in his lower back.
Injury and 3 other workers had the task of moving the kiln's pusher block from the N to S side. Upon reaching the S side, the worker's, except for injury, let go of the block. He tried to steady the block himself causing him to strain his back under the tremendous weight of the block (300-400 lbs).
EE was working on a BMR(crusher) liner bolt replacement. Crew was placing the top lid on the BMR. 2 cranes/hoists are used to move the lid. The first positions the lid approx 3' short of its final placemen. The lid was placed on top of the BMR while the second chain hoist was rigged. While resting on the machine, the lid teetered and fell approx 18"" on a mechanics foot.
Employee was rolling a 6x6 beam and caught the end of his ring finger in between them.
Employee was pulling fire hose from scoop bucket when he felt a ""pop"" with a burning sensation in his lower back and right side. Employee finished shift and drove himself to the doctor the following day - 7/23/09.
EE was unbinding timbers to load on loadboy. When the last bind snapped it landed on his leg.
Employee was walking in entry when he slipped in low spot causing him to twist his back.
EE states he was assisting rib pin a corner when a large chunk of coal fell from the rib and landed on his right foot. The injury resulted in two fractured bones in the right foot.
#3 E. #4 belt had just been completed & power was being restored at the belt power ctr when incident occurred. 1 ee was pushing the reset button, & injured attempted to set/latch the breaker. Breaker did not set/latch on this attempt & process was repeated 2 add. times. 3rd attempt there was electrical arc within power ctr w/flames, causing burns face & forearm.
Employee was running the 330 around the powerlines. He got out to see how close they were and slipped on some loose rock/dirt.
#3 E. #4 belt had just been completed & power was being restored at the belt power ctr when incident occurred. 1 ee was pushing the reset button, & injured attempted to set/latch the breaker. Breaker did not set/latch on this attempt & process was repeated 2 add. times. 3rd attempt there was electrical arc within power ctr w/flames, causing burns forearm.
Employee Failed to follow Posted Lock Out TagOut Procedure Wrench fell into electrical box and caused Arc Flash
Working on a belly dump trailer between the side rails, trailer started rolling and as he was exiting from under the trailer he hit his head on the side frame and lacerated his scalp.
Employee was dismounting a 944 Cat loader. When he stepped from the last step to the ground, he pulled his fingers on his left hand.
As a Norfolk Southern empty train pulled under the loadout facility a 9'8"" piece of rail broke causing the train to wreck. The cars struck and dislodged the concrete and steel supports of the facility resulting in it's collapse. The victim was on the second floor checking for debris in the cars when the collapse occurred.
Bolt men were suppling bolter. Injured EE was on offside reaching across bolter to place bolt supply. The operator thought he was out of the way and trammed bolter forward onto injured's left foot.
Replacing conveyor belt on VSI new belting was on forks of a loader with a pipe in a cradle pipe sliped out of cradle and fell on employee.
EE was inspecting dozer, stepped off track onto step and twisted his right knee.
EE reported: FCT belt had stopped, he was kneeling along side the machine's computer screen attempting to determine the mechanical problem. A piece of draw rock (14"" x 19"" x 2""-3"" thick) fell from between roof bolts striking the top of his head, sliding down his upper back & falling to the mine floor. The area was bolted with resin bolts using 14"" aluminum pie pans on each bolt.
Employee was working to reposition the brake on a shovel swing motor when the brake struck his hand resulting in fractures to the thumb and index finger of the left hand.
A rock fell from the roof while the employee was bolting the top. The rock stuck him in the right arm, causing a severe bruise.
EE reported: Walking from return air entry back to the face area, after stepping thru a pull curtain, a small piece of draw rock fell striking the top of his head and sliding down his upper back. Complained of sore shoulders and stiffness to his upper back region.
Employee was lifting a piece of belt on #1 tailpiece to put in for skirt rubber when he felt something pop in his back. He reported this accident on 07/27/09 and is scheduled for an MRI on 08/04/09.
While trouble shooting a stack conveyor, where the belt was not moving, employee disconnected the power source. When he pulled the lever down, it failed, causing a flash fire.
Member had stopped to hang miner cable out of the roadway, when he was struck by a rib roll
A mobile crane was used to lift wire rope reels into a cable sled. During this lift, the whip line ball was two-blocked. The wire rope guide was broken free and fell, striking the hardhat of the employee, knocking him to the ground. He was transported to the Colstrip Clinic by ambulance and air lifted by Help Helicopter to Billings for tests and observation.
Employee was installing battery in generator. Battery fell while employee was moving it around to install it and cut middle finger on left hand. Employee is off work.
While adjusting the bolts of the grinder in Planta I, the bar slipped and he lost his equilibrium hitting his left hand against the conveyor & injuring small finger of left hand.
Section foreman was lifting oxygen bottle from buggy to hook up torches to work on mine equipment. When he turned with bottle his lower back popped and injured his lower back.
The employee was stepping out of a motor. When he stepped out his foot landed on uneven bottom causing him to strain his left knee.
Injury reported on 7/20/09, and became reportable on 10/14/09. Employee was breaking rock, and broke through rock. Machine tilted forward and back straining employee's neck.
Employee was burning with a torch when a sprak went in his ear. He continued working until 7/24/09 when he missed a day of work.
The ee was lifting a welder from the back of a shuttle car. When he lifted the welder he felt pain in his lower back. The ee did not begin to loose time until 8/18/09.
The employee was going to look into a preheater door, after inspecting the door he stepped in some hot material.
Injury was doing mine rescue training on the mine rescue training field. He was exploring the No. 1 entery turned with a stretcher stepped on a rock an twisted his right ankle.
Miner was checking level of water in tank of Ledwell Water Truck, when he slipped off bottom step, injuring right arm and right side of his ribs.
Hanging miner cable to the #2 miner at the #1 crosscut in front of power center when a piece of rib came out between the timber and the mine roof, catching injury on the left knee pinning him to the bottom
The employee was loosening and removing bolts on a vessel door using a 12 inch pipe wrench and felt a kink in his back.
EMPLOYEE WAS PULLING TORCH CART, TRIED TO PULL UP APPROX. 3 STAIRS, STATED HE PULLED SHOULDER MUSCLE
When installing a stull rock hit him on his right shoulder.
While watching someone else hammer on a bearing, a piece of metal flew off of the bearing and struck the employee on the arm.
A Mechanic was helping to finish the tire job on a haul truck, he was using a torque wrench on the right front wheel and felt a sharp pain in his left lower abdomen or upper groin area. He checked himself and saw a ""bulge"" in the lower abdomen area. The mechanic was transported to hospital for treatment.
Employee was struck on the shoulders by a piece of rib rock. It pushed him into the miner causing a bruise. He was initially released to full duty, but on 8-7-09 began to lose time.
While working on Cowl blade, a rock approximately 6"" X 5"" fell on back of his head and neck and drove him to the ground.
While employee was drilling a hole for a bolt, a small rock fell from the top, striking him on the left foot.
EE was picking up wheel and rim to put into back of vehicle, when he hurt his right arm.
Employee was pulling pin to unhook duster from getman tractor when right index finger was inadvertersly placed in pinch point and mashed.
Employee was pulling grease hose to grease haul trucks, slipped, fell on her knees and strained her back.
EE was on scaffle opening valves, when the operation handle he was using slipped off the valve, causing him to lose his balance and fall and land on the floor.
EE was walking across the road at x-cut 54 in the right mains when his foot slipped out from underneath him and as he fell to the ground he injured his left shoulder. EE continued to work and visit his doctor until 11/17/09 at which time he underwent surgery on his shoulder.
As the driver was ascending a hill on the haul road, the truck stalled and slowly rolled back approx 60'. The coal truck then overturned due to the slope of the berm, it was resting on the passenger side of the vehicle.
Employee was shoveling tailpiece, tripped over 2"" waterline, falling and injuring back.
Member stepped out between the hand rails after cleaning the head shaft on the coarse vessel. As he stepped out, he placed his foot unexpectly on a small piece of wood that was on the floor, which caused his ankle to pop. He has been taken off work until his injury has been properly diagnosed.
A large piece of rock fell from the rib striking a joint 8"" x 20' waterline lying on the pavement. He was crossing the waterline at this time to clear himself from the area. The rock caused the pipe to bounce and tripped him causing him to fall. He suffered a broken tibia and fibula to the lower right leg.
When lifting a piece of 1 1/2"" thick stone from the ground to stand on it's side, a ""sharp"" pain was felt in his middle lower back with tingling down left leg. Pain/ache continued and worsened.
Employee was using a box end wrench to tighten a 1 1/4"" nut on the overland belt flip over. The wrench slipped off and struck him on his abdomen. This resulted in a hernia which will have to be surgically repaired.
Haulage train, powered by Trolley #685, loading ore cars on the haulage level was struck from behind by a second train entering the same loading panel. No damage or injuries were reported at the time. Several weeks later, employee in Trolley #685 complained of back pain associated with the incident.
While bolting top, a piece of roof fell on his ankle. #3 left break, spad #341.
The employee alleges his regular duties create a back condition.
EE stated that while hanging a miner cable, he injured his stomach.

Employee hurt his shoulder/neck area on 08/07/2008. Employee was lifting a pump to be installed on a haul truck and felt a pop in his left shoulder. He returned to work on 4/7/09. He re-injured himself on 7/15/09. He did not report until 7/16/09. The incident has been under investigation by GAB. The claim was accepted 11/18/09.
Employee was operating manbus on west track and got something in his eye.
Mechanical failure on a haul truck engine resulted in an engine fire. Employee operating the truck injured the heel of his left foot exiting the haul truck.
The excavator walked in front of the truck, then reached over and put the bucket in the bed of the truck and began to walk backwards, pulling the truck out. When the truck was freed, the truck came forward, striking the windshield of the excavator. The truck struck the walking levers of the excavator which caused it to walk forward. Excavator then pushed truck til it went off.
After using a 4 1/2"" angle grinder to grind weldments on channel steel framework for a platform under the breaker's shaker arms ee's eye became increasingly irritated from an apparent minute piece of welding slag or dirt.
EE was operating a shuttle car when he ran over some coal on the roadway causing him to slide into the coal rib. Sprained his lower back.
Stepped off of scoop and crilled ankle and broke it on left foot.
Employee had set a timber by the section Power Center, x-cut 10 of 2 north and was walking between a pallet of dust and picnic table and tripped and fell down, hurting his right shoulder.
EE was carrying a timber from a timber pile to a pick-up truck, when he tripped over another timber, resulting in a twisted ankle.
Employee said he hurt his back sometime during the shift while doing his maintenance work.
Employee was preparing to swap pumps and was exposed to hydrocholric acid mist from a leaking flange that was approximately 15 feet away on adjacent equipment to what he was working. He had difficulty breathing
EE was operating a mantrip buggy transporting workers to their U.G. work location. The front wheel of the buggy caught in a rut and the steering wheel pulled out of ee's hand and hit the ee on the right hand.
Threw lunch pail over seat in water truck and felt a rip in back.
Injured was rolling curtain up and a piece of the rib fell and struck him in the left shoulder.
The injured was a utility man on a continuous miner section, he started complaining of back and leg pain 7/14/2009. He sought medical treatment and continued to work until 8/13/2009 when he was diagnosed with Piriformis Syndrome, he continues to be off work.
Employee notified foreman about 8:30 pm on July 14, 2009 that his back had started to hurt. He asked the foreman to fill out accident report Jarring of back from truck was mentioned He was offered a ride to the ER but declined. He went to his personal Dr. the next day and was excused from work. He returned the 17th but left after 4 hrs. 8/3/09 accident determined to be chargeable.
EE was helping lift a metal beam when he lost his grip, causing it to land on his hand. Bruising and swelling occurred, with no broken bones.
Employee was pushing a rock and injured's hand got in between the rock and side of chopper.
EE was cleaning sticky coal from the buckets in the coal elevator. While cleaning a bucket, a piece of coal fell and struck him on the arm.
Employee lost balance and grabbed railing on screen and pulled muscle in upper right arm.

Employee was performing an installation of the inspection registry in the discharge hopper of belt 3 in the FM#2. During installation he was looking for support when he stepped back and his left foot fell down thru the vertical ladder opening.
Employee was busting rock at section loading point with a sledge hammer. He swung the hammer and his hand hit the feeder and broke the bone at the end of his finger.
Employee lifted and was carrying a bucket of miner bits and twisted his back.
Truck was backed up grade to place material in a washed area of the road. When operator raised the truck bed, the truck began to roll and operator lost control. The truck traveled over an embankment and rolled. As a result of the incident the operator received a cut to the head and broken knee.

Employee was tramming a ride backwards to turn it around. When the front wheel hit a rock and spinning it out of his hand. When the handle hit his left hand. Spring left hand and bruising it.
Employee was lifting an Omega block to the top row of a stopping and felt pain in his left shouder, but continued woking. He called the mine office on 8/18/09 and informed them that he had gone to the doctor and rotator cuff surgery had been scheduled and that he would be off work from that date.
He was attempting to throw a set of steps over a hand rail, the steps came back and struck him on the right wrist, causing a laceration that required 8 sutures.
Empoyees left boot became stuck in the mud in 12 I. As he attempted to free himself he felt pain in his left knee and hip. Lost time began 7/23/2009.
Employee was spot bolting and a small piece of draw rock fell and hit him on the right hand and fractured his pinky finger.
Uniy 9595 Pit F250 service pickup truck was run over by a water truck. The incident happened by the lower pit stand pipe, the one used for filling the 190 ton water trucks. The employee operating the pickup truck jumped from the pickup truck to avoid being run over. The result of this is a sore knee requiring medical attention.
Employee called Stillwater Mining on his days off, telling us that he went to doctor & was diagnosed with a torn ligament to his left shoulder. This was not reported to us until 7/20/2009. He was pulling machine out of a lifter hole & felt his shoulder pop.
Employee was replacing a damaged track on a ASV loader. As he was attempting to pry the track into place with a bar, the bar slipped and caused the employee to strain his abdomen which herniated.
Employee was pinning in #1 Entry Face and a rock approximately 12"" X 4"" fell from the top and struck the employee on the right shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 7/16/09.
A screen cover (wire cloth) fell into the ""Jaw Rock"" chute. The victim was removeing the screen cover and a rock approx. 4"" x 8"" came traveling down the chute and pinched his fingers btwn. the rock and chute wall. He suffered lacerations on his L/H middle and ring fingers. The middle finger was broken behind the fingernail.
After being investigated, it was determined that the employee could have jammed his hand into the door handle, when exiting the men's restroom as someone was entering it. The results of the investigation were obtained on 08/6/2009 from GAB.
Employee stated that while carrying two five gallon cans of oil through mud he slipped and twisted his back and light leg.
Technician opened the container to investigate whether or not the ozone process was active (i.e bubbling) on the sample being tested. His proximity to the container was close enough for him to breathe in some of the ozone gas. Accident just determined Work Related 11/23/09 by Workers Comp.

Walking to the Cage through mud, employee stepped in mud and hyperextended his left knee. EMPLOYEE HAD SURGERY ON HIS KNEE 11/19/2009 WHILE ALREADY OFF WITH HIS RIGHT SHOULDER SURGERY. ALSO DUE TO THE UNDERLYING ARTHRITIS THIS IS BEING CLASSED AS OCCUPATIONAL ILLNESS.
Employee was cleaning pieces of roof rock out of the walkway on the 2A belt and caught his middle finger, on his left hand, between two pieces of rock causing a fracture to the finger.
Assembling pipeling in river and ring to attach pipeline together, fell on hand and broke thumb.
Employee stated that he was assisting another employee with a wrecked jeep when the jack they were using, came out from under mantrip and struck the employee on the left elbow. EE FLWD was 7/13/09.
While removing a cover from the shear the employee lost his balance and fell backwards and pinched 3 fingers on his left hand between the cover and the panline causing a fracture and laceration.
Injury was coming down a return ramp into the pit to get loaded again when he stated his brakes failed. He yelled for the loader man to move out of the way He ran his 777B end dump into the highwall. He received a broken left leg.
Miner was about/preparing to clamp a 27"" X 72"" X 1 1/4"" thick slab. Slab was not braced properly. Miner pulled slab against himself, pinched his thumb, moved back away and slab fell toward him. He fell as slab slid down his leg, eventually landing on his boot. His lower leg was fractured.
Employee was digging hole with post hole diggers, and felt a pull in his left shoulder. The employee did not report an injury until 7/14/09 at which time he was sent out for treatment for a left shoulder strain.
On 07/10/09 emp. was rolling up cable when he felt a hard pop in his right shoulder. He was taken to the ER and was looked over and given a full duty release. He continued to be in pain so he was sent back in for a check up and was then scheduled for a MRI. On 09/09/09 it was discovered during the MRI that he had a torn rotator cuff. He became a Lost Time on this date.
EE had stopped to help the motorman move a sc tire that had vibrated off the flatcar and fell onto the track. While moving the tire he slipped and fell and the sc tire fell on his right leg. EE worked from 7/10/09 until 8/19/09 when he had surgery.
While water blasting on the kiln exit area, injured worker stated that he knelt down to shoot the lower northwest port. Injured worker said when he was shooting the port that he must have twisted wrong while using the gun and felt a pop in the middle of his lower back. Injured worker also said that his back felt fine to finish, but his back felt sore after he took a break.
EE was operating a forklift to load a tractor/trailer container with product when the spring loaded dock plate came up out of position. He backed out of the container, he did not look in the direction of travel and hit the dock plate. Rushing and eyes not on task. Forklift and dock plate sustained minor damage.
Employee was refueling a truck mounted compressor engine with gasoline from a five gallon approved safety can. The fuel splashed onto the hot exhaust manifold and was ignited. The resulting flash/flame burned the employee on both arms, chest, and face. The employee was transported to the local hospital and then was transferred to the burn unit at the Univesity of New Mexico.
Injured employee had left hand on a stop bracket while lowering a dust collector hose. The dust collector hose was being lowered by hand, the hose assemble is counter weighted. The hose got away from injured employee and smashed the tip of his left thumb between the jig and stop bracket.
Injury was watering location around Crusher he was told to stay away from soft spot until cobble rock was placed in hole and covered. He forgot and backed next to hole where it was soft & the right side of water truck sank. The load shifted and truck tipped over on right side. He climbed out and seem to be ok. Worked 7/10/09 lite duty. went to dr 7/12/09/
Two EEs were injured in an accident involving a pickup truck & a highwall drill. The truck traveled down the drill bench slope & struck the drill. The two EEs were apparently outside the drill, both on foot at the time of the accident & were struck by the truck. The exact location of these 2 EEs have not been determined at this time & the accident is stil under investigaton.
EE was helping install a drive chain on #2 jeep. He was pulling on the chain when it broke loose, he caught the left pinky finger between chain and jeep frame. He went to hospital where received sutures to the finger and referred to umpl hospital for finger surgery on the small fracture.
After zoning in at the top of the #9 slope, we started down the slope and approximately 1/4 way down it felt like the brakes went out on the 8 man mantrip. We made the first turn to the right on the bottom and was not able to make the left turn and we ran into the coal rib. Co-worker was the operator of the 8 man mantrip.
Employee was going to mantrip at end of the shift and hit his head on low hanging brow. He continued to work until 7/21/09, this is the first day he missed.

Employee was working on a screen and it shifted and hit him in the knee. He says he didn't think it was hurt and didn't want to go to the doctor. Since then, it has gotten very sore and he went to have it looked at.
Moving scrap to lower storage, was stepping down from loader tripped backward over a pipe in the tall grass landing on back.
Employee was helping to unload a roll of belt on the surface. When they got to the end of the roll, the steel pipe they were using to hold the belt came off and hit him in the left foot.
miner was sitting while welding, turned to get a welding rod, felt pain in back.
EE was looking for bad belt splices with the belt on slow inspection mode. A loose strip of belt caught the glove on his left hand and pulled his hand and arm between the bottom belt and roller. This resulted in a fracture to his left arm and lost work days.
The EE was getting of the excavator from the chain to the floor and felt pain in the lower back.
An unsecured belt caused a backlash when a failed tail pulley bearing was removed on a 15 ft. conveyor coming off of a roll plant.
Quarry laborer strained a muscle in his lower back while shoveling built up crushed stone during plant cleaning.
Injured was struck by a slate bar that was hanging out of the side of a mantrip and came in contact with a wooden crib that had been set beside the track for roof support. As the slate bar contacted the crib, it swung around and struck the injured in the lower part of his left leg just below the shin causing a fracture to his fibula.
Employee was jacklegging out of high scaler when he noticed a slab of loose from back. Loose fell out of back striking him on head and shoulders. He has taken to Salem ER and later transferred to Barnes in St. Louis. He has a fracture to L-2 vertebre and cut to right fore finger requiring sutures.
Employee stated that he was removing drill steel from a hole that had been drilled. A piece of finishing steel got stuck and he was trying to remove it, when it came loose and struck him in the mouth.
Employee was repairing a steering clutch in a bent over postion. As he raised up. He felt a sharp pain in his lower back. He went to dr and was placed off work on 7/8/09.
Climbing down off loader and back went out. Lower left back.
Employee was pushing cable up in the hole when he felt pain in his left shoulder. Continued to work regular job but had surgery on 01/07/2010.

The individual was installing a roof bolt when a piece of draw rock fell. The draw rock struck the edge of the tray of the roof bolting machine which caused the rock to break. A piece of the rock struck the individual's left wrist.
Bag of rock dust fell off belt line. Employee grabbed bag as it slipped and pulled his left shoulder.
Employee reached with his arm to guide a beam in place that was hanging on a 3/8 cable from the roof and felt pain in his upper arm.
Employee was exiting Cat. 990-G end loader. When he stepped onto the ground with his right foot, his left foot got hung on the step causing him to fall to the ground; resulting in injury to the left knee.

Employee was operating an excavator in the clay pits. He brought the back end of the machine down hard on the ground while sitting on the edge of the seat and felt pain in his lower back.
Employee was helping and instructing another employee on how to change hose fitting and installing covers on shields. Employee helped him during the shift and starting feeling pain in his neck. EMPLOYEE DID NOT START LOSING TIME UNTIL 11/03/2009, DUE TO SURGERY.
Rolling culvert into trench and bolt caught employees glove and pulled him into the trench. Bolt went into thumb area below wrist
He was helping another person to remove a clamping cylinder from 7' symous crusher, when he was disconnecting cylinder, he felt his back go out.
Was using jack hammer to clean aglomerator when it slipped and fell on his foot.
Employee was standing between large tire truck and metal dumpster, as tire truck backed up he was pinched between truck fender and dumpster, sustaining a compression injury to shoulder.
EE pulled muscles in his neck and shoulder area while climbing into an excavator.
There were excavators working in the area. The Labor thought he was going out far enough around the excavator. He was on the blind side of the excavator of the boom. EE was going along the side of the pile when the operator put another rock on the pile the rock rolled down. He saw the rock coming toward him & proceeded to move out of the way. The rock hit his lower right leg.
While ee was rock dusting, part of the rib fell and hit him causing injury to his hip.
EE steam cleaning 992 hilift which was under repair. Inadvertantly sprayed foot with steam.
Employee was operating a Joy 10 shuttle car. While in the process of making a left hand turn toward feeder, the operator's compartment rubbed the left rib, causing some coal to be loosened from the rib, striking the employee's left knee. Employee stated that when rib roll started to come inside operator compartment, he twisted his back. This accident is still under investigation.
Employee was lifting a lever that operated a flop gate to change the flow of stone from conveyor C-2 to RC-4. It got stuck and he had to lift the lever twice. During the lifting of the lever the second time, he felt something pop in the middle of his back.
Employee was tightening a bolt to the miner panel when he felt pain in his left shoulder.
EE was checking Off Spec Door on # 4 Grizzly on the # 4 Kiln. The door was stuck in the open position and the EE pulled on the door to get it back on the hinges. He felt a pull in his lower back. Conditions and work conditions were normal, no Slip, Trip or Fall Hazards and Poor Housekeeping was NOT noted.
Mechanic was loading drill on lowboy to leave mine. He jumped off the lowboy and landed on a loose rock. He fractured his heel of his left foot and sprained his left ankle.
EMPLOYEE WAS CLIMBING DOWN A LADDER AND WHEN HE STEPPED TO THE GROUND HE SLIPPED AND SPRAINED HIS LEFT ANKLE.
Employee was on his way to the clinker ranch to collect lab sample; inside the ranch after collecting the samples he triped over clinker gravel, lost his balance and felt to the ground on same level. As result of the fall the employee landed over his left arm and suffers a hairline wrist fracture.
Miner was pulling 4"" hose up steep bank and his right knee popped out of place then popped back in. Went to take another step and right knee popped out again. Miner waited a little bit and rested the knee, went back to work. Miner states his right knee has been hurting ever since, and on 7/11/09 he sought medical treatment in hospital ER.

The employee was trouble shooting a hydraulic problem on a piece of equipment. The employee stood up from a stooped position and felt pain in the right knee. The employee continued to work until surgical intervention was required on November 12, 2009, at this point the incident became an MSHA reportable ""accident"".
Injured employee was roof bolting along the left side of the continuous miner on the 6 Right section when a section of rib measuring 9' long x 33"" high x 5"" thick fell and struck the employee on the back of the right leg just below the knee causing a bruise.
EE was climbing down machine, when he caught his foot on the lower step and fell about 2 1/2 to 3 feet on his backside, bruised back.
July 2, EE was on E floor watching wire saw. Employees on top of C wall were trying to clean up for a spot to drill. A 4'x4' piece of wire barrier fencing came loose and fell from the upper level, striking EE on the hard hat, knocking him to the floor. Created intense headache - he went home because shift was over.
Employee was walking down the steps at the Prep Plant. She slipped on the last step twisting her left knee. She has continued to work until 10-2-09 when she started missing work for a torn meniscus.
Slipped in drain ditch, hit his left shoulder on a large rock.
Sheared bolt on one end of brace while installing.
It had rained and the employee was climbing down from the service truck when he got his foot tangled up in a chain and slipped and fell. He landed on his hip. It was also muddy since it had rained.
Employee was removing fuel tank and as he climbed into operators compartment he jammed his neck when he hit his head on bottom of seat. After continuous treatment with company physician, he was diagnosed with a fracture in the neck. He was taken off work effective 08/14/2009.
Employee was attempting to empty a trash drum when he heard a popping sound in his right elbow. The initial doctor evaluation reccomended immobalization of arm until further testing. A subsequent examination by orthopedic specialist revealed a damaged right bicep muscle.
Employee was in the process of changing a tire on a 1 ton pickup. The tire iron slipped off the lug nut and cuased the employee to fall backwards. During this, the employee's left ankle rolled, resulting in a fracture.
Employee was pumping on jackhandle when the jack kicked out & metal stand hit him in the head.
Injured and co-worker were unloading 8 inch aluminum pipe from a scoop. As the co-worker picked an end of the pipe up injured's left middle finger was caught between the pipe and scoop smashing the finger tip. He sustained a left distal phalanx fracture and began losing time from work on 07/02/09.
trouble shooting feeder circuit inadvertently touched meter lead to top of breaker meter flashed.
Employee was troubleshooting C.O. system when a rock measuring 8""x30""x8' rolled off the rib striking him along the left side.
Two employees were moving material, one employee dropped his end of the load, causing the other employee to have his legs injured.
Employee noticed irritation to his eyes a couple of hours after welding bolts and studs to a guard at the railroad clean-out. This injury occurred on July 31, 2009 but the employee did not seek medical treatment until August 2, 2009.
EE with history of chronic lower back pain, back surgery (see E-Doc 118619), lifted large wrench and felt pain in lower right side. Then swung 20# sledge hammer several times to tighten nut. Reported injury 08/03/09 and referred to physician same day. Physician removed employee from work for one week.
The employee was moving a rock with the assistance of another ee and felt pain in his back. He was put on temporary lifting restriction and prescribed vicodin.
The employee alleged that he aggravated an ""existing"" lower back problem when lifting a bucket with approx. 25 lbs of wet sand in it. (This claim is being investigated by the workers comp insurance carrier.)
Two employees were replacing UMHW Chute Liner at Vulcan Materials Stafford Plant. One employee was holding the liner while the other employee drilled a 1/2"" hole in it. While the hole was being drilled the liner moved causing the employee holding it to move it back in place at which time his glove got caught in the drill bit pulling his hand into the moving bit.
Employee was replacing damaged conveyor. Employee made contact with metal conveyor that was energized.
The employee was carrying timber when he tripped over a wiper box, twisting his back.
The injured and another miner were setting roof jacks along the haulway at SS #888. When the other worker took a jack from the jacks piled along the rib, another jack fell on the left foot of the injured.
Employee stated that he was mounting the loader. His left foot slipped off of the second step. He slid down the steps onto the ground while holding onto the hand rails. Remounted loader and proceeded to work. Felt pain in left leg later. Went to doctor, checked out and returned to work. Became reportable when he missed work on 8-24-09.
Employee was closing tailgate on dump truck and strained back.
Restacking a pallet of 50# bags. While restacking he felt a pain in his back. Didn't become recordable until 8/17/09 when he started lost time.
July 28, 12:15pm employees at 10 dragline socketing dump rope. Oiler using pry bar to set rope in socket groove. Bar slipped, oiler fell forward & tripped over drag socket. Dragline operator reached to catch oiler; oiler's boot contacted operator above right ankle. Operator reported 7/31 at 4pm. Mosaic denies this event constitutes a recordable work-related injury under Part 50.
A side arm on 740 Articulated Haul Truck was being cleaned in wash area when the arm slipped off blocks falling on right foot.
The ee was supporting mine roof with a twin head bolter. He had his hand on the starter steel and used the hyd. to insert the pusher steel. The steel snapped back striking the employees hand.
EE was retrieving a tool from the loader and started descending the loader. He missed the handrail and lost his footing and landed on the ground flat on his back.
He was repairing bridges and installing stone in tramway, he twisted his left knee, causing a left MCL strain and derangemant.
Employee was assisting another employee with lifting a piece of plate (1""x6""x10'). He felt a sharp pain in his Rt. Shoulder and was sent to the doctor. The doctor released him back to work, but set up further testing. On 8/18/2009, the specialist put him on light duty restriction and set up an appt. with a surgeon.
Employee fell approximatly 15 feet to ground. Material discharged from blocked chute that he was working on to un-block.
Miner slipped and fell on loose gravel on sloped 3' berm which encloses diesel tank containment area. Left ankle was dislocated and small bone broken.
EE was pulling cable for the roofbolting maching. EE felt left shoulder pop and had a lot of pain.
EE states he was holding a pc of steel to cut it and steel fell pulling his arm down causing pain in his right shoulder. EE did not report this injury or seek medical treatment until 8/7/09 when pain worsened. There is no est time associated with this injury. EE on modified duty pending orthopedic consult.
Employee was walking in shop & his bad knee gave out causing him to fall & re-injure his back. Didn't receive medical information to determine if recordable until 12/10/09 per DH.
Employee was attending a training session in a classroom environment. He reached for a bagel at the refreshment table and stated he injured his back.
The employee was going to look into a preheater door and after inspecting the door, he stepped in some hot material that remained on the steps.
Employee was observing a miner 3 slushing. Nut on the dydag holding the idler block & cable unthreaded itself, hitting employee above the right knee. He was sent for x-rays, which were negative. He returned to work, missing no days. His knee continued to bother him. MRI was done on Sept 09. ACL tear. Surgery scheduled for 11/11/09, which was his first day missed.
EE was dumping sacks of AG711 in the pellet plant. He raised a sack over the dumping bin and the sack tore allowing the load to fall. As it fell, it pushed his right arm under the sack and pinned his arm between the sack and the safety rack.
EE was using a sledge hammer to tighten bolts on raw mill drive motor, and reportedly missed target, straining muscle in his back.
Employee was the last of 3 rides going into the mine through a second set of airlock doors when the front ride hit the close switch on the doors. His left leg was pinched between the doors and the ride he was on. He sustained contusions and abrasions to the left leg.
Employee tripped and fell, fracturing his right arm.
Employee was driving a tractor-trailer, loaded with fines, down into the pit, as he reached the intersection of the ""New Ramp"" and ""4th Level Ramp"" the tractor-trailer overturned into the bank. The employee was pinned in the overturned cab and had to be removed by emergency personnel. The employee sustained a fractured pelvis.
LATE REPORT: Employee stated his ankle rolled over while walking towards the time clock on 7/16/09. The injury was not reported to his supervisor until 7/18/09.
While three mechanics were loading a torch box onto a flatbed truck, one mechanic heard a pop in his left arm near the bicep resulting in a ruptured bicep tendon.
The injured was operating a radio remote controlled continuous miner when a rock fell inby the last row of roof bolts, it rode through the last row in a manner that it struck the injured as he truned to get back, resulting in an injury to his neck and a fracture to his left ankle.
Injured was setting the breaker for the feeder at the section power center after a belt move. The breaker blew up when he attempted to set it and knocked him against the rib. There was no apparent injury at the time of the incident. Approx. 2 days later, he noticed that the hearing in his left ear was deteriorating. Dr told him that his hearing loss may be related to the accident.
Employee stepped down off of step and felt something pull in his calve.
Employee walked behind truck and struck his right knee on a trailer hitch. Initially, this was classified as a first-aid case; however, employee continued to feel discomfort. After further examination by a specialist on 7/23, employee's work status is restricted until further notice.
Employee was removing cleaning rod from 02 analyser on kiln #4 and felt a sharp pain just above the left pectoral muscle. Discomfort returned and further evaluation revealed a slight muscle tear which required surgery for repair. Surgery was performed on 1/13/2010.
The employee was trying to free a cathode that was stuck in the reciever of a stripping machine. While trying to free it, the cathode sprung back and caught his hand between the cathode and an I-beam. This caused a laceration to his ring finger.
Employee was throwing a piece of metal grate and it caught on his glove and he overextended his back resulting in lower back strain.
EE was using a blow pipe on rock for cleanup. He stepped off a 6"" uneven surface bumping the air turning it to full force causing him to lose his footing. He fell fracturing his right tibia.
Employee was pulling klinkers when the puller stopped, it hit the steel plate holding up the door. This caused a jolt and strain the the body.
Employee was greasing and checking guards and walkways and crossed over blocked off part of conveyor. Fell through the walkway.
Employee was drilling rock when he says he turned and felt his knee twist.
Maintenance Mechanic was dismounting D10R Dozer on right side and slipped on bottom push bar step resulting in injury to left bicep area. Employee treated at local emergency room and returned to work on light duty/modified work. He remains on light/modified duty. There have been no lost work days on this injury as of the date of this report.

Removing shaft from S-60 Derrick Stiff Leg crane draw works. Shaft slid inside and at the same time shifting weight upwards. Injury either tried to hold the shaft and it pulled him in or he did not have time to get out of the way, smashing his left forearm between the shaft flange and draw works.
Loose material fell from the back striking employee, knocking him down. Diagnosis is Sprain and Strain of knee and leg, left shoulder.
Employee was bolting using a jackleg. He grasped the machine at the saddle, balancing the machine with his right hand as he was reaching for the 2' driver, the machine pivoted back mashing his hand between the leg & hammer, resulting in a fracture on his right hand.
Miner was resting left hand on frame of Jaw crusher. A lid was in the open position and closed unexpectantly striking the miner's hand.
Employee was lifting a guard and strained his back. This is ongoing from a previous injury dated 11/22/07. He missed 5 working days and has returned with restrictions.
On Wednesday, July 8, 2009, employee was stepping from one small boat onto another small boat. When he stepped off of one boat onto the other he slipped and fell. He dislocated his left shoulder.
Employee strained lower back while moving wood.
Employee was working on an air conditioner on a D8 Dozer. He was kneeling and looking in the engine compartment. When he stood up he could not straighten his left knee. He stated he did not slip or twist his knee. Possibly a torn or strained meniscus. Employee returned light duty. Waiting for a MRI.
Employee was preparing to service a dragline cable tractor that was approaching the area where his service truck was parked. When the tractor parked, the three point hitch-mounted cable handling implement (D-Ring) was lowered onto the employee's right foot, hitting between his steel toe cap and his ankle. The employee sustained bruising and minor swelling.
Worker was picking up a piece of stone and felt a pull in his lower back. Went to Dr. and was given meds. and restrictions.
Checked load fleet, came back to dredge in yawl stepped onto dredge, knee started hurting, told them he wanted to get it checked.
Employee was refueling a D65 Komatsu Loader when he slipped off of the tracks and landed on the ground. Employee had no broken bones, but complains of complete body soreness.
While performing rounds, an operator went to climb two steps. The operator stepped on the first step and felt his Achilles tendon pop. He was transported to the emergency room, where his was given medication, crutches and the ankle was immobilized. The operator injured this same tendon 3 years ago in a non-work related event.
Employee was air arcing a heel band off of a 4100 shovel bucket. He was a making a cut with the air arc when the band fell from the bucket landing on his right wrist and hand.
The employee was changing a hydraulic hose on a continuous miner. The head pin plate was in an upright position to allow access to the damaged hose. The head pin plate fell on the employee's hand.
EE was scaling rock when a rock fell striking him on the head and shoulder causing a fractured collar bone and a possible compression fracture.
