Accident type, without injuries

Rock fall occurred in 1 West Mains at break #99 in the #3 entry. Fall measured approximately 25' wide x 20' long x 5' thick.

During an MSHA investigation of a squeeze area located at the junction of the 1L/2ME submain and 1R/1L/2ME panel, a roof fall was found in entry #10 at crosscut 7. The fall was the result of the squeeze condition.

A roof fall occurred in the 2nd West Submain at spad number 5+90 in the pillar line between #8 and #9 entries measuring approximately 25ft. long x 18ft. wide x 7ft. to 8ft. high. This area was supported with 4 foot fully grouted roof bolts, 8 foot cable bolts and square cribs.

Hoist tripped on ""Drive Converter A"" and ""Vibration"" faults. Electrician inspectd teh hoist and reset the faults and hoist was back in service at 5:20 AM. There were still two escapeways during incident.

An unintentional, non-injury roof fall was discovered at 03:25 pm one break inby SS 960 #5 entry. Size of fall measured 35' X 19' X 7' and does not effect ventilation or travel. The area has been timbered and dangered off and will not be cleaned up.

A roof fall was discovered in the 2 North 8 Panel, #4 entry, @ 11 crosscut. The fall is approximately 30 feet long, 20 feet wide, and 8 feet thick. The area is supported with 5 foot torque tension rebar bolts. This panel is worked out.

A roof fall occurred in the 1st South Panel in #5 entry in 1/3 the intersection of spad number 0+50 and continuing to the inby edge of spad number 0+150 measuring approximately 90ft. long x 19ft. wide x 8ft. high. This area was supported with 4 foot fully grouted roof bolts.

An unitentional, non-injury roof fall was discovered at 01:57 pm one break inby SS 752 #7 Entry. The fall measured 35' X 19' X 7' and does not effect ventilation or travel. Area has been dangered off and timbered. It will not be cleaned up.

An unintentional roof fall occurred two breaks inby spad # 1184 of the # 7 entry on the east 002 panel. The fall measured approx 18' wide 24' long and 7' to 8' high.

A roof fall has occurred in the primary intake in the #6 entry at break #43 in the intersection. The fall measured 20' wide, 30' long and 6' in height. Persons, ventilation, nor equipment were affected. This area will be rehabilitated.

An unintentional fall of the roof was discovered in the #7 entry at 55 cx in the North West Mains. This fall was approximately 20' long X 20' wide X 7' high. Located at Spad #1002.

An unplanned roof fall occured in the #6 entry of the 001-0 section. This fall occurred at spad #4154, 3045 feet from the active face. Roof fall was in the intake. Ventilation and passage was obstructed. Fall measures 20 feet long x 30 feet wide x 8 feet high. Area was developed 5/2009. Roof support consisted of 48"" resin roof bolts. State & Federal agencies notified immediately.

A roof fall occurred at 2:30 pm in the # 3 entry of the Cool Hill 2 at spad number 70 + 30. This is a main belt entry. The fall was approximately 30' long by 8' high by 18' wide. The fall blocked the passage of men and material. The area was mined in December of 2006 and bolted with 8' expansion bolts that are resin assisted.

A ROOF FALL was discovered on the 001-0 section during preshift examination. The fall measured app. 4' thick X 20' wide X 65' long. The fall occurred in the crosscut from the No. 5 to the No. 4 heading & through the No. 4 intersection. The crosscut had been cut through at the end of the 2nd shift on 9/26/09 & had not been bolted prior to the end of the shift & had been left down.

Hoist #2 went down at 2:35 a.m. Because there was only one means of escape (Hoist #1), the mine was evacuated. MSHA number (for immediately reportable accidents) was called. Surface electricians found a failed coil on the brake system and had the repair completed by 4:30 a.m.

The fire boss found the roof fall on the South Mains B Section. The fall was approximately 80' x 16' x 13'. The cover was approximately 230' in the area of the fall, and the top was laminated shale and sandstone. Area was bolted with 6' fully grouted resin bolts with 12' cable bolts installed sporadically in the area. The fall occurred on an idle shift.

A roof fall was discovered at 23 breaks on 10 head gate in the #2 entry. The roof fall is approximately 6'h x 20'w x 20'l.

The #8 elevator was taken out of service @8:15am to change out the hoist ropes. The ropes were changed by EE and the car placed back into service @ 6:00am on 9/27/09. No individuals were on the elevator at the time, the escape ways were re-routed and all necessary government agencies were notified.

The 15 W. section entry cut through a planned horizontal gas well at 6:17 a.m. The entry #1 at the face well gassed off following the planned cut. The power was removed from the section and all agencies were notified. Note that this form is being filled out for reporting purposes only. We do not agree an inundation occurred.

An unintentional roof fall with no injuries occurred in the #7 heading at the last open crosscut. The fall measured 60' X 19' X 12'. The fall does not affect the emergency escapeways or ventilation and will not be cleaned up at this time.

An unplanned roof fall occured in the #2 Gas mine in the #9 entry at brh #22 that measured approx. 22' long X 19' wide X 19' high. No injuries were reported. The area was developed on 10/27.

A roof fall occured on the West Mains section MMU 005-0, approximately 40 feet inby spad number 6329 in number 4 entry. The fall was approximately 8 feet thick and 30 feet in length. The depth of the fall could not be determined. The fall occured during retreat mining in the 4th lift in number 4 entry.

A roof fall occured in the #9 seam in the 2 Main North at crosscut #13 in the pillar line between #5 and #6 entries. The fall measured approximately 30'x18'x6'. The area was supported with 6' tension rebar bolts.

A roof fall was discovered in the #5 entry of x-butt near spad #10,117. Cribs and gob were removed to prepare to build seals when the fall was discovered. The fall area was 4 feet in height and 40 feet in length. Affected area is being cleaned up and re-bolted.

Employee was pulling around some ruts in the pit to get to the dragline for greasing. When he was finished, he started backing up. He thought he was following the same tracks he used coming in, but instead backed up roughly 60 ft staright into the pit. Another employee was able to get him out of the truck, but as soon as he was out, the truck went into the pit and was submerged.

A roof fall was discovered on this date. Fall was located on 3 North #2, the fall started in 3 right and extended over into the intersection in #4 entry in the 4th crosscut. The fall measured 8 Feet in thickness. The fall was originally supported with 5 foot High Tech bolts with supplemental cable bolts in inetrsections.

Roof Fall occured on pillar line outby.Roof fell about 20'in front of jacks blocking the path of entry #6. Fall measured approx. 7-8' high.Area was bolted with 5'resin bolts. Fall was slick and slided about 20-25' long and 19-20' wide.

Contractor Van Damme Farming not involved in normal mining operations was delivering hay at Empire tailings basin. When unloading hay the contractor employee noticed a fire starting near the driver's side door of the truck. His supervisor called 911 at 4:58 pm. Fire depart's arrived at 5:19 pm, put down the initial fire, stayed past midnight to ensure smoldering was out.

No men were on section at the time of fall. Fall was on the way in and fell between pre shift and workers getting there. Fall measured 4 ft high X 85 ft long. Fell at 2 cross cut 1 intersection 3 to 1 across from end of tall piece 2 cross cuts outby (spad #831 which is located in #4 entry).

An unplanned roof fall occured in the Coalburg Mine that measured appro. 30' long X 6-8' high near spad # 940. No injuries were reported. The area was developed on 4/17/2009.

A Roof fall fell measuring 20'(W) x 20'(L) x 5'(H) located 60' inby break 165. It was found @ 11:00pm on 9/22/2009.

Rock fall occurred on 3 East Mains return at break #36 in the #2 entry adjacent to the #8 belt conveyor. Fall measured approximately 20' wide x 15' long x 4'-6' thick.

Roof fall found by safety man and federal mine inspector while traveling through an abandoned worked out area that is not normally traveled or examined. Roof fall 110' x 18' x unknown. Area supported by 4-foot fully-grouted resin bolts on 4-foot x 4-foot centers.

Over the weekend of 9/19-09, we experienced a sand failure in 5500-12 Stope. The failure was approximately 3' high x 11' long x 12' wide. There was no one at the stope at the time of the failure and no machinery involved. It failed in the intersection of the stope.

The elevator #8 was taken out of service @ 10:45 a.m. for ANSI 17 testing (5 year test). No EE's were on the elevator at the time and all escape-ways were re-routed. The elevator was placed back into service @ 8:20 p.m. All neccessary government agencies were notified according to law.

A mud trap and water hole broke loose in the east end of the pit, causing shovels and dump trucks to have to evacuate the area.

On 9/19/09 at 4:30AM, the interior doors on the Somerset elevator failed to close. The incident occurred as an employee stepped off the elevator at the mine level landing. The elevator repair service repaired the door lock mechanism and replaced a guidewheel on the elevator in order to put it back in service. There were no persons trapped, or injured as a result of this incident.

A roof fall was found at 2:12 am on 9/19/09, at survey station 70+50 entry 66RMS. Fall was in a four way intersection that had been mined 9/15/09 and supported with 8' full resin bolts. There are no plans to clean up this fall. No injury resulted, approx 200' of cable was lost.

At 6:45 pm on Sept 18th, an ignition of gas occurred at the site of the gob vent bore hole number 2. Upon discovery, the gob vent bore hole pump and engine were taken off line. The incident is still under investigation to determine cause and corrective actions.

A roof fall occurred on the main east supply road at x-cut 32 spad # 19+96 and is approximately 8' high x 19.5 wide x 20' long. There were no injuries, witnesses, or disruption to ventilation.

XCut 3-4 at 5+37 at Intersection in #3 started working approximately 9:00 AM. Employee called to inform me and said that they were cribbing & timbering. On the way in I contacted the outside and by 9:30 AM I was informed that the above area had caved in on the LD Box.

Solenoid sticking on one of the brakes of main hoist. When the computer senses too little or too much brake pressure, or an abnormality it sets all the brakes, until a system check is complete in the logic. Call made to MSHA 10:19am. Hoist returned to service approximately 10:40am. Parts odered for repairs.

The center high voltage regulator shorted out causing one phase to 'drop out'. Our three phases motors will not operate without all three phases in operation.

An unplanned roof fall occurred in the 12 I longwall section tailgate entry. The fall occurred at 43 x-cut. The fall measured approx. 20ft long by 16ft wide by 10ft high. No injury or equipment damage as a result of this occurance.

An Unintentional roof occurred on the (004 MMU) 4 Mains section in the 110 pillar block #5 entry inby spad #952 in the #3 lift, resulting in the Company #17 remote control continuous mining machine being caught by mine roof.

A roof fall occurred in the #11 seam in the 2 East Panel in the pillar line between crosscut #5 and #6 in the #3 entry. The fall measured approximately 60'x18'x10'. The area was supported with 4' fully grouted bolts and T3 Channel Straps.

Pressure on roof and floor was causing the roof to sag and bottom to hoove up in the area. Dangered off the area and allowed an intentional roof fall to occur to relieve the pressure off the adjacent entries and crosscuts. The intentional roof fall was 80' inby spad 455 and was approx 120' long and 19.5' wide.

A Roof Fall was discovered in the worked out 1 south 2 panel at 39 crosscut,#3 room setup. The fall measures approx. 50' long X 18' wide and 7' high. This area is due to be sealed pending plan.

A roof fall occurred in the #1 west main return, #2 entry 1 crosscut inby spad #2273. The fall was 50'x4'. No equipment was involved. Area was posted off.

A roof fall was discovered in the 1 south 2 worked out panel in #2 entry. Crosscut 42-43, the fall measures approx. 95 feet ong, 18 feet wide, and 7 feet thick. The area is scheduled to be sealed pending plan approval.

Roof fall occurred in #3 to #2 crosscut at spad #2016 and #2030 measuring 60' long 19' wide and from 7 to 9 foot high. The fall was break red and dangered off, area will not be cleaned up.

Fireboss discovered the roof fall when making his examine. 8O Section #1 Entry, #6 Break had fallen. 18'L x 18'W x 30L, this is an approximate measurement. Area has been breakered off. Travel way and ventilation was not changed.

A Pittsburgh second mining break was within 100 feet of the roof fall location. Adverse conditions were encountered when the cross cut from #5 to #6 Entry was turned on the South A Section at #12 Break. There was 380' of cover present, block size was 60 X 60 feet.

A roof fall was found in the cross cut between the #5 and #6 entry at crosscut #4 on the MMU. N9002 section on the 2-left panel. The roof fall is approximately 30'-40' long x 20' wide x 12' thick. The roof fall has been dangered off and will not be cleaned up at this time. Roof fall reported by phone to MSHA on 9/14/09.

An unintentional roof fall above anchorage occurred in the 1st left panel of old mains. This area was mined by an operator/contractor prior to wvmp inc. The intersection @ spad 1880 fell and covered fresh water pump. At this time the fall will not be cleaned up.

On 09/14/2009 an unplanned roof fall occurred at cross cut # 23, in #4 entry on the 3rd S. W. Panel off the 1st N. W. Submain. The area was roof bolted with 6' and 10' point anchor roof bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.

A roof fall was discovered in the left return entry at cross cut number 36. The fall measured about 30 ft long, 20 ft wide and 6 ft high, This area was dangered off and no clean up at this time. No injury from this fall

A roof fall was found on the 002 section MMU. The roof fall is on the belt conveyor e cross cut #5 of the 2 left panel. The roof fall is approx 30'-40' long x 20' wide x 10'-12' thick. The roof fall was reported to MSHA by phone. The roof fall will be cleaned up and the roof re-supported (See map for location)

During the examination of the Cook's Branch Elevator Shaft the electrical examiner noticed that the governor rope needed replaced. The governor shaft also needed straightened. This elevator is not used as an escapeway or primary method to portal men into the mines.

At approximately 5:30 am on September 13th, an event occurred that triggered a release of methane. The areas of most significance were directly behind the shields and tailgate side of the longwall face and the stopping line that separated the longwall gob from the return air course (bleeder). Employees were evacuated from the mine.

#3 Shaft south skip hung up and is damaged. It will take several days to repair. There are two hoists available to take employees out of the Mine. MSHA was called.

Hoist #3 (south skip) had an open door at the apron feeder. Skip would not clear at the clam shell, which tripped the hoist. MSHA was called. Two hoists were still operable. Hoist #3 was back in service at 2 p.m.

A Roof Fall was discovered in the 2 west 3, #4 entry at crosscut 7 to 8. The fall measures approx. 75 feet long X 17 feet wide and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts.

Rock fall occurred at break #13 in the 1st Right off 1st Left off 1 East. The rock fall started in the #1 entry and extended across the #2 and #3 entry and then in-by the #3 entry at break #13 to break #15. The rock fall was approximately 20 feet wide, 4 feet thick and approximately 295 feet long.

An unintentional roof fall occurred at the Galatia Mine New Era Portal at the 5W Headgate #2 set up at +150'. The fall was approximately 20-25 feet long, approx. 17-18 ft wide, and approx. 8-10 feet thick. the fall did not affect ventilation or passage of workers. The fall will be cleaned up.

An unintentional roof fall above anchorage was found in the old mains #5 entry. This area was mined by an operator/contractor prior to wvmp inc. At this time, the fall will not be cleaned up. Approaches have been timbered and dangered off.

Rock measuring 5 1/2' X 10' X 18' wide fell ont CM while cutting coal. No vent. or travel of people or equip were affected. Both MSHA & OMSL were notified, and investigated.

Mechanic was cutting out bent bit from miner drum and a spark ignited methane gas which was being released from floor. Mechanic extinguished fire with water hose from continuous miner in less than 1 minute. MSHA hotline was called.

An unplanned roof fall occured in the #130 mine at spad #4634 that measured approx. 40' long x 20' x 6' high. No injuries were reported. The area was developed on 11/30/2005.

WHILE CONDUCTING HIS WEEKLY AIRWAY EXAMINATION THE FIRE BOSS IDENTIFIED A ROOF FALL IN THE OLD NORTH MAINS RETURN. THE FALL MEASURED 19'L X 50'W X 4'H. THE AREA WAS MINED APPROX. 12/8/06 ON 80' X 80' CENTERS. THE AREA WAS SUPPORTED WITH 4' RESIN BOLTS. THE AREA HAS BEEN DANGERED OFF AND TIMBERS HAVE BEEN SET.

On 9/9/09 at 8:30pm an unplanned roof fall occurred on the #1 section. Roof fall was in the #1 entry at Spad #3527. Roof fall measured 90'L X 20'W X 9-10' in thickness. This fall occurred 500 feet from the active face. Ventilation or passage of persons was not affected.

A roof fall was discovered in the Conners Run return @ 0+75 between 3-04 and 3-05 6s. Approx. 8'high and 100'long.

Unintentional roof fall on the 8R longwall setup between #1 and #2 entry behind the longwall head drive. The fall was approximately 20'Lx18'Wx2'H and did not fall above the anchorage.

The # 1 elevator was out of service for a period of time exceeding 30 minutes due to the bracket for the buffer switch being damaged. Water in the pit caused the chain to strike the bracket. The water was pumped, the bracket repaired and the elevator placed back into service at 3:34a.m. No individuals were on the elevator at the time. The escape-way was re-routed.

A roof fall was discovered in the intake entry of the number 7 entry at spad # 376. The fall measured about 20 ft wide, 20 ft long and 6 ft high.

In process of making weekly exam; Mine Examiner discovered a roof fall 2 cross cuts outby the ""AT"" Evaluation point. The roof fall was a failure in the inersection and partial failure of cross cut. There were no persons injured or equipment damaged as a result of this roof fall.

A roof fall occurred on the 5D belt spad #18+54 the #5 entry in the 5A panel. The size of the fall was 26 ft long, 24 ft wide and 8-9 ft high.

An unintentional non-injury roof fall was discovered during a weekly examination. The fall was approximately 40' X 5' X 17'. This fall does not interfere with ventilation or escape travel and will not be cleaned up at this time.

On Tuesday the 8th of September one of our mechanic's had a trouble call from Tilden Mines that number 2 pellet plant elevator is not working. EE arrived on site at 2:30pm, found debris in first floor hoistway door track, causing doors not to close. EE was inside the elevator assessing the doors, when they went fully closed, trapping EE in the car for 1 hour. .

A roof fall was found on 9/05/2009 at 4+90 room 5 in 6R MS MMU 007. The fall was in a four way intersection that was mined August 31,2009 and had been supported with 6' full resin bolts. No injury resulted and fall will not be cleaned up. An intake stopping and approx. 100' of highline was lost,

The #2 Hoist was shut down to replace a broken hydraulic gage. Miners were evacuated at 11:00 a.m. and evacuation was completed at 11:11 a.m. The hoist was tested and miners returned underground at 1:30 p.m.

She got out of her truck, began walking toward the loader as the operator was backing in her direction to take delivery at the truck as usual. She had never left the truck before. She walked in the blind side(left rear)of the loader and was not seen. She was struck & run over by both left side tires causing fatal injuries.

Employee had just gotten off work, he was driving out of the mine on the haul road and was trying to pass two other employees and got off the road and hit an old gate post.

Employee was holding and pushing stone forward on splitter table, when the splitter was engaged, rock table came up and caught right hand index and middle fingers, crushing them between table and stone. Middle finger suffered partial fingertip and nail amputation and slight fracture to bone.

Employee was throwing a belt roller under the bottom belt during a belt move in J tailgate section and felt pain in his left groin. Employee has continued to work but was diagnosed with a hernia on 10/5/2009.

Working in a scissor lift, got off of it ot carry angle iron. As he was walking with the angle, his safety harness lanyard got caught on the heal of his boot, which caused him to fall foward over some pipe and land on the ground.

EE reported that he bent over to pick up one end of a jack to set it for roof support. When he raised up, holding the end of the jack, he said he felt a ""pop"" in his mid lower back area. He finished his shift and sought medical treatment the following day, due to continued pain to his lower back.

The loader man directed the truck driver on where to back in, loaded him and then told him to pull forward. He pulled forward approx. 25 feet and turned over on the right side.
