A fire was called in and upon arrival to the location of the fire, it was found a generator was on fire. The location was at Hoop's Hill Radio Towers.

An unplanned roof fall occurred in Powelton mine 33 entry at spad #658 that measured 80'long X 20'wide X 7/8'high. No injuries were reported. The area was developed on 2/9/10
A Roof fall occurred in the intersection, last open cross cut, inby the tailpiece on unit #1, Entry #1,of the 1st South Sub-mains. The fall measured approximately 15'L X 18' W X 9' H.
A methane gas igintion occurred in the #3 face of the 12 east continuous miner section.
A roof fall was found at xcut #58 on the #2 track entry at 22 east gate section. The fall was approx. 40 feet long by 17 to 18 feet wide by 2 to 8 feet high. Area was bolted with install III bolts resin assisted. The area was screened and we used bacon straps. Fell above anchorage point. Section is idle.
A roof fall was discovered in the 2nd Southeast Parallel #3 entry xc- 13 spad # 8+94.
A methane ignition occurred in the #3 face of 12 east continuous miner section.
At approx. 11:33 pm an explosion occurred inside the #2 coal mill system resulting in the opening of both explosion vents. There were no personnel injuries or equipment damages at a result of the accident. Metallic objects were found in the coal mill which created an ignition source.
On 2/25/10, it was reported at 9 pm to the Libby Rural Fire Department, that the Stimpson Plywood building also know as Kootenai Business Park Industrial Center located at 990 E 5th Street Extension Libby, MT 59923 was on fire. Genesis Inc. stores railcars of concentrate at the facility. These railcar were in the building waiting shipment from the Port Authorities to the smelter.
Unintentional roof fall was found in the 1st south east main return air course. 12,000' from the B&C sections. Heizman Jacks were already set in that area. The fall occurred behind the jacks. Travel way was not blocked. Fall measured 20'l X 5' W X 3-4' thick.
(Not on mine property) Property owned by Kootenai Business Park Industrial District in Libby, MT. Fire started in collapsed section of building where demolition work was being done. Fire department did not try to extinguish fire and it spread to railcar load area.
There was a malfunction in the main contactor. The mine was evacuated at 4:33 a.m. and completed at 4:55 a.m. The hoist was repaired at 6:55 a.m. and the miners returned underground at 7:00 a.m.
An unintentional roof fall occurred at the Galatia Mine New Future Portal at the #6 seam entry #2, 1475 So. In the belt entry. The fall was approximately 35'long, approximately 17.4' wide and approximately 10' thick. The fall did not affect ventilation or passage of employees. The fall will be cleaned up.
Approximately at 9:08 am, while the dozer operator was pushing coal at the raw coal stockpile, he observed a signifcant amout of smoke coming from underneath the coal pile. No fire was observed, just 'smoldering' coal.
An unplanned roof fall occurred in the Coalburg mine at spad #1182 that measured 80' long x 19' wide x 8' high. No injuries were reported. The area was developed on 9/30/09.
At approximately 8:15pm the material hoist went down and showed motor field/lost fault. Trouble shooting was conducted and the hoist was put back in operation.
There was a minor fire started when a piece of heated metal was discharged from a belt magnet into a plastic collection bin. The piece of metal was likely coming from a plastics shredder, where it was likely caught and discharged - then collected by the belt magnet. The damage was minor and corrected in the same day. No one was injured during or as a result of the incident.
Hoist rope broke while lowering a load of supplies.
8 Hlg. elevator was inoperative for 52 minutes due to an electrical problem. No one was on the elevator at this time.
The material hoist went down at approximately 6:07pm and showed motor field/lost fault. Trouble shooting procedures was conducted and electrical parts was changed out. The material hoist was put back in service at approximately 8:30pm.
The fall was discovered on the A Mains Section travelway (#8 Block, #5 Heading, 2000' outby the face). The fall was 10' high x 16' wide x 15' long. The fall area was bolted with 8' Super Twist Bolts. No people or equipment was affected by the fall. The travelway was rerouted to ensure traveller safety.
A rock fall had occurred on 14 beltline break 39 in Holley Submains, 3 entry, the size of the fall was approx. 6'H x 40'L x 60'W. Additional roof supports of cribs, beams, and 10' rope bolts were added to the effected area, also additional rope bolts were added to surrounding area's inby the fall.
The fall was discovered on the C Mains Section (300' inby the feeder in the #21 Crosscut, #3 Block of C-4 Panel). The fall measured 7' high x 16' wide x 60' long. The area was bolted with 6' Super Twist Bolts. No equipment, people, or ventilation were affected as a result of the fall.
A roof fall occurred in the neutral entry of the main North at X-Cut 37. No injuries or disruptions to ventilation. The fall was approx. 20' long X 8' high X 20' wide. Fall was at Spad 26+40 X-Cut 37 on Main North.
An employee was trapped in the warehouse elevator for approx. 49 minutes when the elevator car was unable to spot itself at the proper level.
During exploration for rehab. various roof falls were observed. These were called into local MSHA Office, per their instructions falls being reported on one form due to pre-existing condition.
East Autron drive communication board bad. Changed board to return to normal operations.
IN 4-5 AT 14 BREAK THERE WAS A ROOF FALL, 18 FEET WIDE, 30 FEET LONG, AND 5 FEET TALL.
A roof fall was discovered @ 13-11 XC, #9 Entry of the Main East Return. Approximate dimensions of 20 X 20 X 12.
The auxillary hoist stopped as it was coming to the surface. The hoist was changed to manual mode and continued on out. It was found that 2 wires in the umbilical cable had come apart. The MSHA hotline was called as requested by an inspector on the scene, but it was later detemined that it was not reportable in that the hoist was never out of service.
The maintance truck was used to travel to a tunnel to unclog a feeder jam. The employee left the vehicle idling, and when he returned to the truck observed smoke coming from the hood. The employee notified the shift supervisor and began spraying fire extinguishers. Local fire department was called as employees continued to suppress the fire.
Weekly examiner found an unintentional roof fall measuring approximately 18'x50'x8'high in the 8N bleeders in the #20 crosscut between the #1 and #2 entries. No travelways or air passage was impeded.
A fall of roof was discovered in the 1ST Southeast Mains in the #5 Entry extending from x-cut 18 to 19, the fall was approx 110ft long x 19ft wide x 6 1/2ft high. This fall was previously reported on EDOC #1066129. Per a discussion with D10 Roof Specialist it was determined that (2) separate falls could not be reported on the same 7000-1.
Elevator stopped with one EE on the elevator at eh #1 portal. Start 2:54 AM-off the elevator at 3:42 AM.
Elevator went down at 11:12pm the initial cause appear to be electrical. After a investigation was conducted it was determined that a fuse was bad in the control transformer. Fuse was changed and the elevator was back in service at 12:37am.
A roof fall measuring 27'L X 20'W X 6'H occurred in the #1 West Mains across from survey station 434 in the #2 Entry (Mudlick outby panels).

A federal inspector deemed a smoldering area on a refuse pile berm a fire. Based on this determination, a ""fire"" was reported.

Roof fall, one crosscut inby spad #24062 in the track entry. Fall was 2500' outby the Headgate. Fall was 20' X 20' X 7'. No injuries or equipment involved.
Fall was discovered at 9:56 pm on 2/21/10. The fall is approx 750' outby active section in the #2 entry 1 crosscut outby SS 867. The fall is approx 20' long X 20' wide X15' high. The fall did not affect ventilation travel didnt not cause any injuries and did not trap any equipment. The area has been timbered and dangered off and the fall will not be cleaned up.
A roof fall occurred approximately 80' inby 14XC of 33 5 North Longwall. Approximate dimensions of 15' X 20' X 12'.
A non-injury roof fall occurred on the 002 MMU during pillar recovery covering the Joy continuous miner. The rock fell from the cut and did not pull permanent roof supports. The fall measured 20' X 20' X 4'. A 103K order was modified and terminated to allow normal mining operations to continue after the miner was retrieved.

While traveling air course with KY inspector a roof fall was discovered at spad 2559. The fall is approximately 40' Long x 19' Wide x 6' High. Ventilation was not interupted. Fall was reported to MSHA field office and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
An unintentional roof fall occurred on unit 3 in the right side rooms @ 4+00 room #3 at station 28+70. The fall was at a 3 way intersection and mesured 18.5 feet x 20 feet x 6 feet. The fall was inby the tailpiece. The area had been bolted with 4 foot fully grouted roof bolts and is now timbered and cribbed off.
While traveling air course with KY inspector a roof fall was discovered at spad 2532. The fall is approxmately 30' x 19' x 6' high. Ventilation was not interupted. Fall was reported to MSHA field off and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
A roof fall measuring 50' x 18' x 8' to 10' thick occurred in the last open crosscut of #5 entry. The area was roof bolted with 3/4"" dia. x 6' and 8' long tensioned rebar bolts. A roof bolter cable and continuous miner cable were under the roof fall.
Rock fall occurred in 2 East Mains cut through to 1 East Mains #5 entry. Fall measured approximately 20' wide x 60' long x 6' thick.
A roof fall was discovered in the 1st North Mains, #5 entry xc-15 spad # 10+66.
While traveling air course with KY state inspector, a roof fall was discovered at spad 2382. The fall is approcimately 40' Long x 19' Wide x 6' High. Ventilation was not interputed. Fall was reported to MSHA field office and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
A rock fall measuring approx 20' long X 20' wide X 3' thick was discovered at 1:30PM on 2/19/10. The exact time of the fall is undetermined. The fall was located in the 9th crosscut inby the north portals in the #5 entry. The #5 entry was dangered off on all accessible entrances and the #4 entry will become the designated travel way and primary escapeway.
While traveling air course with KY inspector, a roof fall was discovered at 2520 spad. The fall is approximately 30' Long x 19' Wide x 6' High. Ventilation was not interupted. Fall was reported to MSHA field office and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
8 Hlg. elevator had a grounded final stop switch and was inoperative for 45 minutes. There were 5 men on the elevator during this time.
A roof fall was discovered in the 2 West 2 Mains, # 4 entry, at # 87 crosscut. The fall measures approximately 30' long, 17' wide, and 6' thick.
This roof fall was discovered in the 4A panel xc-16 to 17 in the #11 entry.
A fall of roof was discovered in the 4A Panel at x-cut 14, in the #2 entry extending to #3 entry. The fall was approx 50ft long x 19ft wide x 6 1/2ft high. The fall was previously reported on E-Document #1066138. Per a discussion with D10 Roof Control Specialist it was determined that (2) separate falls could not be reported on the same 7000-1.
Roof fall was discovered in #9 entry of 1 North Mains. The fall measured 30' x 19' x 5' thick. Fall was dangered off and will not be cleaned up.
The primary power to the escape shaft hoist was interrupted due to a customer truck contacting power lines that provide power to the hoist. A backup generator is available and was started at this time. The backup generator did not provide voltage and thus the escape shaft hoist did not have a power source. Replacing generator with new. Currently wired in a rental generator.
Employee smelled abnormal odor while performing cleanup operations in the coal system building. Identified small amount of smoldering coal and promptly flushed with water. Outside emergency services was contacted and confirmed that smoldering coal was extinguished by mine personnel.
Note:Report is being filed as a precaution. A fire developed in the exhaust stack of the 3744 Locomotive, that lasted 15 mins. and was blown out by throttling engine up. Approximately 10 min. period of time past without a fire. Then a fire started again in the exhaust stack area and was extinguished by local fire dept. using 1 extinguisher. The second fire burned for 15 mins.

Fall was found on weakly at 9:00am 2/17/10 it is in the #1 entry in the north main 2 breaks inby spad 6600 it is about 70' long 8 to 10' thick.

There was a rock fall found on weekly exam in the return on 1 left panel - 1st set of room to left at #8 entry 2 breaks to the right from spad #6638 it is just in the intersection 80' square 8' thick.

A fall of roof was discovered in the 3rd Northwest Submain in the #2 Entry at x-cut 24. The fall was approx 25ft long x 20ft wide x 6 - 8ft high. This roof fall was discovered in the 2nd Northeast Parallel, between #2 & 3 entries xc-7.
A roof fall was discovered in the crosscut between spad locations 248 and 256. The fall was approximately 60' long X 20' Wide x 4-1/2' thick. No injuries, damage to equipment or disruption to ventilation resulted.
Replacement and cutting of structural steel on #3 Stacker caused hot slag to fall between belt and rubber lagging on drive pulleys, igniting belt. The stacker acting as a chimney caused flames to climb the belt to the upper floor igniting wooden floor boards, with slight damage to the floor.
An unplanned roof fall occurred in the 5-block mine at SS#481 between #6 & 7 entries that measured approximately 40'long X 18' wide X 5' high. No injuries were reported. The area was developed on 9/30/08.
Unintentional roof fall that occured on HG #3 West rooms #2 entry x cut #3. The fall measured 20'lx20'wx10'h.
Approx. 6:30a.m. on 2/16/10 a continuation of an existing roof fall was found in the #9 entry at spad 10400 approx.600' outby the working section. The fall was approx.20'W X 40'L X 6'H. The area was supported with 5' fully grouted resin bolts. The area will not be cleaned up. The area has been dangered off and timbers set.
During a weekly examination of our D1 Zone, a worked out area, a roof fall was found in X-Cut #5 entry 00. The roof fall is approximately 15' high, 10' wide, and 10' long. The area was supported with 6' fully grouted roof bolts and developed on 1/18/2010. No miners were injured and ventilation was not affected.
A METHANE GAS IGNITION OCCURRED ON THE 12 EAST SECTION WHILE CUTTING A ROOF BOLT OFF.
Due to inclement weather two trucks were parked behind Sigmon Coals Shop without Mgt. permission. This area was not intended nor designed for coal truck parking. During the night the trucks caught on fire & were destroyed.
A fall of roof was discovered in the 2nd Northeast Parallel at x-cut 94 between #1 and #2 Entry. The fall was approx 20ft long x 18ft wide x 6 1/2ft high. This fall was previously reported on e-document #1066149. Per a discussion with Roof Control Specialist in D10 it was determined that 2 separate falls could not be reoprted on the same 7000-1 form.
Rock fall occurred in the bleeder system in the 6 Right off 1 North Mains at break #22 in the #4 entry. Fall measured approximately 20' wide x 80' long x 5'-7' thick.
This fall was discovered in the 2nd Northeast Parallel, the #7 entry xc-109.
Three miners were trapped while chipping ice from the Sugar Run air shaft for approx. 1 hour. Ice fell and hit the counter weight buffer switch cable.
After some non-related start-up problems at beginning of shift, the newly installed skip began binding in the scroll plate. Finally, it stuck and would not move. The mine was evacuated; no injuries; no other problems. Day shift crew held on surface; adjustments made; back in service at 7:30am.
Upon inspection of James Creek Emergency Hoist, the examiner discovered that a switch needed repair so the hoist would operate as intended. The hoist was set up manually and was manned by an operator until the repairs could be made at 7:00pm.
#3 hoist tripped on ""Drive Converter A"" fault. Electrician was called out of the mine to inspect and reset the hoist. There were still two remaining escape ways throughout the incident (#2 and #6 hoists).
The Kuhntown Elevator wen tdown at 7:12 AM with no persons on board. The agencies wer notified at 7:25 M, MSHA and State was on site at this time. The elevator was down until 1:55 PM. The problem was that the ropes needed adjusted and cables re-lubed.
An unintentional roof fall occurred at station 23+80 entry #3 on the 4C intake and return. The roof fall took out a stopping that separated the intake and return. The stoppings were rebuilt and the primary escapeway was redirected. The fall measured 100 feet x 19 feet x 8-10 feet high. The area had been bolted with 4 and 6 foot roofbolts.
Though I listed this as a roof fall, it was in fact a rib failure. I listed it as a roof fall because you do not have a category for rib fall. About 10 feet of the right rib in 14E Stope fell out. It was roughly measured at 10' long x 9' high x 4' deep. no one was injured.
Employees were cutting bolts from a feed plate on the ""C"" standard crusher. During the process, a piece of hot metal landed on the screen and started a fire.
A continuation of a roof fall was discovered in the 2 West 4 Mains, # 7 entry, at # 42 crosscut. The fall measures approximately 30' long, 17' wide, and 8' thick.
Odor of smoke was smelled near 1225 PM bay. Location investigated (1225 MCC); Mine Emergency pronounced to dump stench, evacuate mine. Exhaust fans went down to due to power outage. MSHA notified of Incident via telephone and called once again to allow Barrick to restart the exhaust fans. MSHA agreed for Barrick to restart fans and monitor any gas levels.
Rock Fall 5 belt Bk-31 between 3&4 entry.Fall is 5'highx40'longx18'w fell between the #3and 4 entry dangered off will not be cleaned up. MSHA and OMSL was notified of the Rock Fall on 2/13/2010.
Ice build up in elevator shaft prohibited the elevator from running properly causing it to be inoperative for more than 30 minutes.

An event defined as a bounce occurred in the B-West Mains room area where floor coal in the bleeder entry around the second mining area heaved into the entry. Floor heave is estimated at 3 to 4-feet in the south bleeder entry.
A roof fall was discovered in the main return air course 55 crosscuts outby MMU 002-0 while making a weekly examination. The roof fall was approx. 4' thick 20' wide by 30' in length. The roof fall did not interfere with the air flow. The roof fall was cribbed and the travelway was moved to the other entry.
Employee change trailer caught fire from an apparent electrical malfunction. No injuires involved. Structure destroyed.
A fall of roof was discovered in the 1st North West Mains in the #3 Entry from spad 127 + 23 to spad 126 + 53. This fall was previously reported on form # 1064450. Per a discussion with D10 Roof Control Specialist it was determined that 2 separate falls could not be reported on the same 7000-1. This form is filed to abate citation #8495781.
This roof fall was discovered in the 1st Northwest Mains in the # 3 Entry. From x-cut 158 to 157 it was (120ft L x 19ft W x 6 1/2ft H). This area was developed in July of 2006. This form is being submitted to abate citation #8495781 issued on 2/11/2010.
Elevator was shut down at approximately 2:48pm due to vibration. After an investigation it was determined that the vibration was due to slack in the hoist rope. The hoist ropes were tighten and the elevator was put back in service at 10:30pm.
A fall of roof was discovered on 2/11/2010 in the 1st Northwest Mains between the # 1 and #2 entry at x-cut # 162. the fall was approximately 45ft long x 19ft wide x 6 1/2 ft high. This form is being submitted to abate citation # 8495781 issued on 2/11/2010
An unintentional, non-injury roof fall was discovered on the North Mains section in the return. The fall measured 100' X 19' X from 10' to 12'. The fall will be cleaned up.
Door switch on the elevator was frozen due to freezing outside temperatures. The elevator was down for more than 30 minutes.
An area of the roof fell approximately 12 feet long, 20 feet wide, and 3 feet high, in the belt entry on number 1 belt approximately 400 feet underground.
A fall of roof occurred in the 1st Northwest Mains in the # 2 entry, spad 131+43 to 130+73. The fall was approx 70ft long x 19ft wide x 6 1/2ft high. This area was developed in August of 2006.
Roof fall at No. 4 entry at spad 257 on 1 south mains. Fall was 18'L x 20'W x 2'5' thick, did not clean up fall-area dangered off. Fall was in a non mining area and no airway blockage. Area was bolted with 5' torque tension bolts.

Employee was following other employee up the ""Woods Road"" to the Twilight Mine. At the 31 curve. He stopped to let other employee go up the hill 1st, almost at the top he (other employee) spun out sliding down the hill striking his truck on the front drivers side(fender and door).The roads were snow covered and slick.
A belt line fire was noticed at 9:17am. The fire occurred approx 50' upstream from the transfer building in the 201 belt gallery. The fire was called under control at 10:2 am. It is unknown at this time what caused the fire. The belt was not running and had been idle since 4:45am.
Upon inspection of Cook's Branch Elevator Shaft, it was discovered that the elevator doors were not properly functioning. The rollers on the right door were broke and required repair before the elevator could continue operation. This shaft is not used to portal miners or supplies and is not the primary or secondary means for escapeway from the mine. The elevator was repaired.
Two employees were stuck in an elevator for approx. 30 minutes. The elevator service was contacted. No injuries occurred. MSHA was informed at the time of the incident.
The roof fall was found at 10:50 AM during a return run along 7th South East Butt at wall #41 #4 entry into 4-5. The fall was approximately 45' long 20' wide and 4' high. The area was bolted with 42"" bolts.
A piece of ice fell an unknown distance off the wall of the elevator shaft damaging the bottom slow down switch and disabled the elevator for more than 30 minutes.
During removal of rock on the east side of the Camp Creek Portal site are. The back-hoe struck and detonated an unfired detonator. At the time of the accident it was unsure what had happen. After an investigation was conducted it was determined that an unfired detonator had detonated. A call was made to Shoal Creek portal around 1:10pm, then a call was made to MSHA at 1:18pm.
At 11:45 am a roof fall was discovered at Break #50 on the track by the examiner and called outside. The fall measured approx. 2 ft thick, 8 ft wide, and 5 ft long. No employees, equipment or air were effected. No roof support was effected, fell between the row of bolts. Travel was available around the fall. The fall was cleaned up and the area re-supported.
The foreman discovered a rock fall on the belt line during the pre-shift. It was on the #1 belt flight at the #10 cross cut on the #2 entry. The fall was approx 35' long, 25' wide and 8' thick. This fall was at an intersection. The foreman reported to the safety director at approx 6:10 am.
Door switches had frozen due to the below freezing temperature.Switches were thawed and repaired and elevator was operating at 2:30PM.
During pre-shift exam. a roof fall was discovered. The fall was in the cross cut. Fall measured 160' long X 16' wide X 7-8' high. No persons trapped, no roadways or airways were blocked. The fall was approx. 2400' outby the working section.
The #6 portal elevator was inoperable due to a power interruption brought on by a seasonal snow storm. It remained that way until the current from a back-up generator was supplied.
The #3 production hoist tripped on ""Check Point UHM"" and ""Drive Converter A"" faults. Electrician was called out of the mine to inspect the hoist and reset the faults. The hoist was back in service at 4:40 PM. There were still two escapeways throughout the incident (#2 and #6 shafts).
A roof fall was found in the No. 3 entry at the No. 4 crosscut intersection. The fall measured approximately 20' wide x 20' long and 5' thick. No injuries or equipment damage resulted from this occurrence.
At approximately 3:15 P.M. employees noticed smoke and saw flames. It appears to be an electrical short in the generator but is still under investigation. MSHA was notified before problem was verified and the mine was evacuated.
A roof fall was discovered on this date. The fall was located on MMU-005 in the #3 entry at break #49. The fall measured 20' by 20' and was 8' in thickness. The area was previously supported with 5' Hy-Tech Bolts and 12' Superbolts. The fall occurred during an idle shift, with no personnel present.
Setting up the continuous miner to rehab this area when the rock fall occurred. It was 20' long x 16'wide x 6' high. This area was being rehabed for this mine. This area will be cleaned up and rebolted. This fell in the secondary escapeway. The lifeline was rerouted and the men trained on the change of the escapeway. The fall was at Break 38 on the #6 belt in the #3 entry.
Rider seam rolled up and bolts failed to 6 anchored in above rider. Bolts broke around edge and intersections fell. Occurred in worked out abandoned area.
A non-injury event occurred to Camp Run Portal elevator due to the severe weather occurring at the time, 8 miners became trapped on the elevator when power was lost. Back-up electrical power was set, one of the 2 rope grips failed to release. Schindler Elevator service was called to correct the condition. The miners were freed at approximately 12:01 AM 02/06/2010.
There was a loss of power at the plant. When power was restored operators in plant noticed that a power pole from the local Electric Utilities Substation to Tower Rock Stone was on fire. This power line goes across our property. Local Fire Dept and power company were called.
Aroof fall was found at xcut #25 at South Mains ""B"" #6 entry escapeway being used for the 6 1/2 West Section. The fall was approx. 40 feet long by 17 feet wide by 2 to 10 feet high. Area was bolted with install III bolts resin assisted, along with bacon starps & screening. The area was mined 4/15/2003. Escape way was re-opened. Cause was deterioration and weathering.
An unplanned roof fall occurred in the coalburg mine on #18 belt, #27 brk, #2 entry that measured 14' long x 19' wide x 6' high. No injuries were reported. The area was developed on 8-4-08.
Unintentional roof fall occurred on the MMV #003 section during retreat or pillar mining. The roof fall entrapped the continuous miner in the #4 entry two crosscuts inby spad # 3648. Roof fall was reported to MSHA by phone on 02/04/2010.

No miners or equipment were affected. This section has been moved to another location in the mine. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts.

We started to rehab a crosscut in our bleeder system in order to re-route walkway. After beginning re-hab we found that the center of crosscut had fell above roof bolt anchor.

A ROOF FALL was discovered in the No. 5 Entry at survey station No. 454. The fall occurred in an intersection and measured approximately 20' wide X 24' long X 5' - 8' thick.
An unintentional roof fall was found in the 003 section return air course of the 2nd south west mains. Travelway is not blocked due to multiple entries. The fall was breakered off with timbers. Size of fall 20' x 20' x 6' thick.
A loud noise originated from the area of the coal fan for #5 kiln. Upon inspection, it was determined that 13 out of 42 housing bolts had failed on the fan housing. There were no injuries and no fire as a result of this event. MSHA was notified at 6:52 a.m.
A fall of roof occurred in the South East Submain, # 59 cross cut, 32+46, entry # 2. The fall was approximately 18 feet in length x 18 feet in width x 6-8 feet thick. The area was supported with 6 foot fully grouted roof bolts.
At approximately 3:55 P.M. a loader caught on fire at the Fletcher Mine. The mine was evacuated, mine rescue team was activated and MSHA was called. Cause not yet determined.
Mine examiner encountered a roof fall in the return entries (L3 & L2) just inby SS#5612 in 2-East off 1-Right section at appx 5:10 AM. The L2 entry was supported with 6' super twist bolts with T-5 channels and 12' cable bolts on 8' centers. The L3 entry was with 4' resin bolts and then center cribbed on 5' centers. The area was at the bottom of a dip in the coal seam.
During a routine inspection of the escape hoist, a large piece of ice was noticed on the mine guide that prevented use of the shaft. The mine was immediately evacuated and MSHA was contacted. The mine went back into production after the ice melted.
A longwall employee was walking along the shield line when he noticed that the shear had stopped cutting and proceeded to walk along the shear line to assist employees with the rock jam. A rock released from the tailgate drum striking the employee in the left hand causing a fracture and partial amputation to the middle finger and a laceration to the index finger.
Employee was attempting to dislodge a frozen chunk of rock that was stuck in the chute. Rock was dislodged and employee tried to catch it before it hit the shaker screen. Little finger was crushed against a ""J"" bolt. Tip of little finger to first knuckle was crushed and had to be amputated.
IW was cleaning pulley shaft in preparation of changing bearings/cum-a-long slipped off wing pulley pinching IW's middle and ring fingers against bearing support
Member had trammed the roofbolter into #3 left to begin bolting sequence, and as he prepared to get out of the operator's deck a rock , fell onto the ATRS of the bolter, causing the bolter to tilt up in the rear. Member placed his hand on top of the canopy to assist himself out of the deck, and when the bolter tilted up his hand was caught between the canopy and the roof.
While operating scoop, employee placed his hand on top of the scoop canopy, and his fingers were caught between the canopy and coal rib.
EE was inserting a piece of drill steel in the coal drill, lever was bumped causing the drill head to advance, mashing his right index finger.
Employee was climbing down off of a piece of mobile equipment, when he stepped on the ground the surface was slippery and he lost his balance, twisting his right knee.
Employee emptying trash - as he lifted bag of trash to put into dumptster - felt pain rt. arm/shoulder. Reported to MIS 4/5/10.
Employee was fueling a welder on a maintenance truck. He was fueling it with a gas can and when he went to fuel it he slipped and injured his knee as he fell.
Employee was grinding steel when a piece of metal lodged in his left eye. Employee was wearing a face shield.
Miner was walking from baghouse to showerhouse and slipped on ice spraining right shoulder. At time of doctors visit, was issued prescription pain meds.
Slipped on ice & twisted ankle.
The employee was driving 2-man 4-wheeler, striking a solid coal rib, causing the steering wheel to dislocate his third finger of the right hand. He is missing work and finger injury may require surgery.
EE was shoveling the ribs in the face as part of the clean up cycle, when a piece of rock sloughed off the rib near the roof and struck the EE in the right ankle.
The man was walking out of the outside shop when he slipped and fell to the ground.
Injury suffered a lifting injury to his left arm while attempting to service the sweeping machine he was operating. He attempted to raise and latch the rear door of the sweeper containment compartment in order to dislodge stuck swept material in the compartment.
Another EE pinched the EE on his leg, EE jumped and twisted his left knee.
EMPLOYEE STATED THAT HE WAS BENDING AN 8' ROOF BOLT WHEN AFTERWARDS HE HAD A PAIN IN HIS LOWER BACK.
While getting out of the scoop deck, EE got his ankle tangled up in a piece of line curtain which was being used to cover the seat. As he got out he twisted his ankle.
EE reported on 3/4/10 that while lifting a 50lb bag of lime he felt something pull in his stomach.
WHILE ROOF BOLTING, EMPLOYEE WAS ADDING EXTENSION STEEL TO STARTER STEEL WHEN OPERATOR'S PLATFORM SHIFTED, CAUSING HIM TO LOSE HIS BALANCE. IN AN EFFORT TO CATCH HIMSELF, HE LET GO OF STARTER STEEL WHICH FELL FROM ROOF, STRIKING HIS LEFT RING FINGER RESULTING IN A FRACTURE AND LACERATON.
While observing a shield being set on the 4B set up face, the employee started to stand up, pushing up with his right hand on the handrail, when a piece of stone fell out of the top smashing his right index finger resulting in an avulsion requiring sutures.
Employee was standing on a platform at the feeder belt and was attempting to turn the tail pulley with a long bar. Another worker was on the ground behind him helping to turn the bar. When the employee placed his foot on the bar he stepped on the other workers hand. this startled the employee, he lost his balance, fell backwards, landing on some cobbles.
EE was dropping bolter cable that was hung from the roof. Tension from the cable caused it to hit the ee in the head, causing him to stumble and twist his right knee.
EE was using a board as a pry to loosen a turbo from a piece of equipment. This lead to him tearing his rotator cuff.
Employee was walking around the forklift and tripped over the forks and fell onto his left wrist.
While using a wrench to remove filter housing the bolt broke loose causing victims elbow to hit hydraulic pump.

The employee was walking across a blast pattern and took a misstep. He twisted his knee resulting in swelling and bruising. Based on ongoing evaluation by doctors the injury was deemed reportable on 3/2/2010.
Operating a front end loader, ran over a pothole which caused the loader to jerk resulting in a strain to his neck.
Employee driving company truck, patrolling property, wiring in truck caught fire. As employee tried to put out fire, truck went into ditch. His left vehicle and started walking down hill, he fell on pavement and hit his head. He was found in roadway to approx. 5:30 am 2/25/10. Transported to hospital by ambulance.
The outside stacker belt needed a splice, Injury was removing bolts from structure and pulling on the belt with tools when he said he had pain in his right shoulder and neck after pulling on the belt.
He was walking to the continuous miner when he tripped on a rock in the tram road. He fell and braced his fall with his right arm and jammed his shoulder, causing a strain to the right shoulder.
EE was lifting a 3/4"" plywood sheet top board to put between layers of pallets. Hit his left upper thigh with plywood sheet.

The employee stated that tramming a bolter into the #3 Entry on the A Mains Section. A piece of rock fell from the top, striking his lower back and right side.
Area was plowed of snow and not yet sanded.
Employee was setting 8 X 8 Timbers and one slipped out of his hand and fell on his right foot.
EE had just finished replacing a bulk line. As he stood up and walked away from the job, he felt numbness in his right leg and foot. Diagnosis was sciatica.
Was securing the cover over the reel of a shuttle car when the cover fell on his left foot.
The mantrip buggy got stuck. The men were pushing the buggy out of the hole. The injured employee had his left foot between the buggy and rib. As the buggy came out of the hole, it got traction and the wheel turned and caught his ankle between buggy and the rib. The caused impairment to his left ankle, chipping the bone and contusion on the surface.
Driver was hauling coal from the pit at the mine via pit haulroad. While descending a grade, driver attempted to downshift to a lower gear. Driver missed the gear and noticed that he was drifting towards the road berm. He tried to correct his steering and at the same time the right front tire blew out. The truck upset onto the roadway and stopped.
Employee stated that he was stepping off of the ladder on the fork lift. As he stepped down he slipped causing him to twist his knee. The area was muddy and wet.
He was walking across the floor in the prep plant when he bent over to walk under a pipe. He stepped on a hose and felt pain in his left knee. He continued to work at his regular duties but continued to have pain. He was removed from work on March 22, 2010. His physician report indicates moderate degeneration of the lateral menicus. Occupational illness.
EE was operating a rock truck coming out of the coal pit and the tire hit a hole in the road spinning the steering wheel and it jammed the EE's right shoulder.
Employee was bolting on the right side of the continuous miner when a rock fell out of the rib above the rib protector striking the employee on the back of the neck. Employee was diagnosed with a non-surgical fracture of the C6 vertebrae in his neck.
Employee was removing a steering cylinder from a Letourneau loader. Both pins were removed and had a come-a-long installed to assist in lowering the component down. The lifting strap was installed somewhere in the middle of the str cylinder barrel. When the steering was rocked loose from position the cylinder swung and crushed the left hand of the employee.
Part 50 Audit. Employee was installing belt rollers when he began to experience back pain. Employee was excused from work for 4 days.
EE was fastvacing on the platform above fm2 pump. He was tied off with his safety harness squatting down fastvacing around the duct. As he tried to advance forward, his lanyard was at its limit and slightly threw him off balance and his right knee twisted a little bit.
Employee was working on loader. He was grinding out cracks on bolt holes on front rear end. He was wearing safety glasses. Piece of foreign matter (metal) got into eye.
Employee twisted his back while helping load a shot at the quarry. The impairment is unknown.
Employee was lifting thumper to place on the power center, when his foot slipped, causing him to twist his back.
EE was using a small jack hammer to remove the castable material from the #5 firing hood. A piece of castable material fell and hit the top of his left foot.
EE was operating the roof bolter when a rock fell and struck his left forearm and wrist. Last day worked 2/25/10.
Employee was assisting in patching holes/cracks on top of the dragline. When he was walking across the roof he slipped and fell on a patch of ice next to the dry walkway.
Inappropriate personal fall protection, he tied off in a unsafe area, caused employee to fall while taking down a surge bunker off the side of a wet tower. Injuries were a small laceration to the back of his head and a broken right forearm.
EE was installing rub rail on the miner, rail slipped and fell on his right hand.
Employee was carrying two metal track ties, weighing approximately 5lbs. apiece, when he lost his grip and tried to catch the ties, injurying his lower back.
Employee was walking to his assigned equipment after he had attended his pre-shift toolbox session and instructions when he slipped on icy conditions in the parking area and fell on his back.
The man had a coal chain laying on the mine floor and was installing a new flight. While hammering a pin throught the flight, a piece of coal flew up and into the man's eye and scratched the eye.
He was changing ladder on excavator, when he fell off the ladder to the ground. He was transported to hospital and diagnosed with a sprained left ankle. On 03/01/2010 he returned to doctor and re-diagnosed with a fractured left ankle.
Dropping water hose from slope tower over the hand rail the end hose fitting struck him on the side of the head lacerating the right ear. PPE was not factor, compliance rules/regs not a factor, miner proficiency was a factor.
EE was removing cover from pump compartment his hand became caught between the cover and the pan line on the longwall resulting in a laceration to the palm of his hand.
A piece of roof rock measuring approximately 22"" x 43"" x 1"" thick fell out between 2 roof bolts on the 9E working section and struck an employee on top of his hard hat. Employee didn't miss any work due to this incident until 3/05/2010.
Climbing down a ladder and twisted his knee.
He was leaving the analyzer building, walking around the sample building when he slipped on ice and landed on his left shoulder, twisting his back and causing a tear of the supraspinatus tendon. He continued to work at his regular duties until 4/22/10, when he was removed from work for surgery to repair the torn supraspinatus tendon.
The employee stated that while pulling belt on the C-4 tail, the belt jerked, causing the channel locks to dislocate the thumb.
EE was assisting in the installation of rib boards when a piece of coal and rock 1.5' x 1.5' x 5"" thick fell striking his right shoulder, neck and head.
EE was completing a stopping in the #9 entry outyby the 002 section. He stumbled over some concrete blocks. While falling, he caught himself with his left hand. He completed the stopping and finished his shift.
Operator backed over existing berm, truck overturned while going down bank. 8' height 50-75' vertical.
He was putting roller in belt, rib rolled out pinned hand against belt structure. *started missing work on 7-15-10* (This individual was employed at Riverview mine when he started missing work.)
Injured employee was in the process of removing a bent drill steel that was stuck in drill pot, when he accidentally pushed the rotation lever. the drill steel struck the employee resulting in medical treatment.
Employee injured his right knee when the employee jumped onto the bucket of the loader to help another miner and hit his right knee. Employee did not start missing time on this injury until 06/15/2010.
Employee was stepping up on a block when it gave way. As the employee grabbed a rib bolt to catch himself he pulled a muscle behind elbow. Employee did not start missing time until 02/24/2010.
The man attempted to remove by hand a drill steel stuck in the mine roof without success. While preparing to add a second steel into the bolter chuck to raise into the stuck steel, the man inadvertently raised the drill head and caught his finger between the chuck and the drill steel.
Employee was driving 900 pit truck to the 5 dump when he struck a pothole. This bounced employee striking his head on the roof of the cab. Resulting in a cervicle muscle strain.
EE fell and fractured L3 vertibrace while chipping on top of an overcast to increase the size of on opening.
Employee allegedly twisted his right knee while stepping on a rock. He was evaluated by a Physician, had surgery on April 6, 2010, thus resulting in lost time.
EE stated that for the last few weeks he had been having back and leg pains when he was hanging the cutting machine cable.
The ee was changing 6' water valve, stepped over the eater line and struck his knee on bottom. This 7000-1 is submitted to abate citation #8182156 and as a result of a Conference-Litigation decision. The mine operator reserves the right to expunge the record if a court decision is favorable to the mine operator. This accident has been reported by the contractor MVK
Ice had built up at base of sand screw where start/stop panel is located. After starting the sand screw EE was walking toward the roll crusher and his right leg slipped forward and his left leg twisted as EE fell to the ground.
Employee was stepping off of ladder when he slipped off ladder an fell down approximately 3 feet and landed on right leg and heel.
Jumped out of pickup over side of pickup bed. Caught right foot on strap tie down and landed on left leg straining left knee.

We were lifting 20' sections of steel and bolting it to the conveyor system. EE complained of feeling sick around 4:00p.m. EE called next morning 02/19/10 said his stomach area was hurting but was going to try and work. At 9:00a.m. he left work and went to Highlands Med. Center for a check up.
Employee injured his back while installing a DC drive motor in the reclaim tunnel on October 28, 2008. This injury was filed in October of 2008. This is a reoccurrence of an old injury.
He was riding in a jeep when the jeep slid and collided with a mantrip that was parked at the end of the track. He was thrown backward and sustained a strain and contusion to the back and neck
The employee stated that while bolting the #4 entry, rock fell from the top, smashing finger against the bolter head.
Employee stated that he was carrying pin boards to the scoop when he stepped in a uneven place in the mine floor and fell on his knee. He continued to work for a couple hours and asked to be taken to Med. services. He then was taken off by his family doctor.
Employee stated he was standing in a notched in place, when a rock fell from between the roof bolts and struck him on the head and shoulder. Employee has continued to work until he was taken off until a MRI is done. Last day worked was 03/19/2010.
EE was washing sweeper truck hopper out with high pressure water hose, when a piece of dry concrete fell down on top of his hat. EE had pain in neck and extending to mid back. After treating with company doctor, EE was referred to a specialist and due to restrictions, he could not return to work on 3/22/10.
Employee was walking towards warehouse slipped on ice covered walkway twisting left ankle.
Employee was working on miner, as he turned he felt Rt.leg slip twisted knee.
EE was carrying a door for a scoop and felt pain in his lower back, this door estimated to weigh about 40 pounds. He called in on 3-3-2010 and stated that the doctor had removed him from work and he was referred to a neuro-surgeon for futher evalation.
Two shuttle cars meet at an intersection unexpectedly and collided boom to boom. The collision did not affect one of the car drivers but the other car driver was shaken up. He did not report for work the next two scheduled shifts.
Employee slipped and fell on mine floor as he was attempting to enter the deck of a parked scoop.
Employee stated that had just put up his outside bolt in the seventh row of the cut. He was walking beside the machine to retrieve a half header to support a kettle bottom when the coal rib rolled out striking him in the lower back. He was placed off of work by his treating physician for physical therapy on 2/22/2010.
EE cut finger while changing motor on shuttle car.
Employee stated that as they were pulling a rock off the miner with a chain, the rock came loose from the chain, struck his hand and pinched his ring finger against the side of the continuous miner.
Moving the blue station up, a piece of loose draw rock measuring app. 8' x 2 1/2' x 8"" fell from between rows of roof bolts knocking employee down & injuring his left arm, leg, & ribs (no broken bones).
Employee was using a cutting torch to remove old bolts when he passed the torch over his off hand. The heat from the torch melted a nitrile glove he was wearing as a liner under his leather gloves. This resulted in 2nd degree burns on two of his fingers.
He was pulling a 54 inch v wiper and felt pain in his lower stomach
Employee stated he was removing the tongue on a Rescue Chamber, to install it on the other end. When he lifted up on it he felt a sharp pain in his right elbow. he continued to work until the 24th when he was taken off work by the doctor with a torn tendon on the bicep.
The employee was removing pipe from a wire rope spool when his left hand pinkie finger was smashed between the pipe and a cable spool. It was determined he had broken the third bone of the left hand pinkie finger. The employee was unable to return to work for his next scheduled shift on 2/17/10. Surgery was scheduled for 2/18/10.
He was doing some maintenence using tie wire while the screen was running and the shaft or the pulley caught the end of the wire and pulled the wire tight, trapping his fingers between the frame and the wire.
Stated: He had walked to his personal vehicle in the parking lot to get in after the end of his shift. As he started to get into his vehicle, his foot slipped on snow/ice and he landed on his right leg/hip. He has been diagnosed with lumbar and right hip contusions. He started missing scheduled work days on 2/17/10.
Employee never filled out an accident report on the 16th day of February 2010 or he did report an accident to his foreman or tell any one on his crew he was hurt. I'm filling out this report because of a citation.

Injury initially sprained his right knee on 2/15/10 lifting mine belt for set up and has worked until today, date (3-2-10) he has become a reportable lost time injury after an MRI revealed some tissue damage in his right knee.

Helping coworkers assemble FCT. Had been lifting conveyor flights across to a coworker as part of the assembly when he experienced pain to his low back region. Did not recall one specific flight that caused his pain, thinks repetition may have caused the muscle strain. Contacted the mine the following morning he was going to family Dr. his 1st day missed was 2/17/10.
Putting safety glasses on, something went in eye. Either came off glasses or could have come off cap.
Treating bed at stock pile back, Started back down steps and slip causing sprain, strain to right wrist, lower back and right hip.
Fell down stairway. Talked to man that fell. He had no recogonition of how he fell. No metal skids on wooden steps and improper illumination. Was violation statements.
While employee was hanging miner cable, he felt a strain in his lower back. Started losing time 02/20/2010.
He had refilled the propane tank for his forklift and was placing it on the back of the lift when the fuel shifted slightly causing him to slightly lose his grip resulting in a sprain or strain. The full tanks weighted 65 lb & there is a slightly different placement on these new lifts.
Employee was exiting his dozer. Employee was getting out of his dozer and he stepped from the platform of the cab to the next step. His foot slipped and he hyper extended his left knee.
Lifting a skid plate on heavy equipment. Employee ""twisted"" to install a bolt and hurt his back.
While walking up stairs to perform maintenance in a screening tower, a step failed causing the employee to fall. The step may have been missing a bolt and when he placed his weight on the step, step fell out from under him. His left knee hit the step above, thus injuring it. The employee caught himself from falling any further.
Pulling emulsion hose from bore-hole when employee experienced pain in low back.
EE was cleaning the walkway along the 15-W Longwall stage loader, when a large rock rolled from the top of rib striking EE and pining him beneath the rock. EE sustained fractures to both sides of the pelvis and a cracked scapula on the left side.
Employee stated that while bolting off of the miner in 8 South rehab area, he drilled a hole about 8"" into the roof when the drill steel became plugged. He removed the steel and cleaned it and put the drill steel back into the chuck and started to raise it into the hole when the steel hit the lip of the hole causing the steel to bend hitting his right wrist.
Picking up a board off the roof bolting machine and felt a pain in rt shoulder. Continued to work until 1/14/11. Not reportable at the time of injury.
The individual was replacing hammers in the crusher when a hammer dropped off the shelf. The individual caught the hammer to prevent it from falling onto the belt. The individual did not go to a doctor until 03/04/10 and started missing work on that day.
EE was installing boom back in saddle on the service truck, winching cable up when the hook caught on the bumper of the service truck, came loose and hit the EE in the head over top left eye.
Stepped off concrete pad at MCC building onto snow and ice. Ankle gave way on snow and ice. EE fell onto ground and felt a strain/pain in his lower back.
Employee had removed a plate from the chip chute and was clearing the build-up from the chute when his jacket became entangled in a rotating shaft. He did NOT get pulled into the shaft. His injury was caused from the tearing of the jacket.

While Security officer was walking down the coal silo's catwalk his left foot slipped on the frosted grating. He caught himself from falling but twisted his right knee while doing so.
Offside bolter operator was putting up the inside bolt of an entry in the face on West Mains when a piece of drawrock fell from inby. The drawrock fell onto the T-bar over his canopy onto a rib and on his lower legs. The rock measured 4'x5'x4"" and took several workers to remove from the operators legs. He was taken to the ER and has been hospitatlized due
Security guard had come into the guard shack, tripped and missed a step and fell onto her knees. She extended her arms to break her fall. Her left arm got jammed in between the desk and a blue water cooler crate. Afterwards she felt no mobility in her arm and shoulder.
EE was operating shuttle care, turning from #4 entry toward #3. Hit a hole, throwing him into the roof.
Employee was trying to clean windshield of backhoe. He opened the cab door, placed his left hand on the outside handle by the windshield and placed his left foot on the top step, he tried to swing himself out but lost his balance and fell. His right foot caught between the door frame and fender in the cab. He broke the Tibia and Fibula in his right leg.
Employee was moving a light plant when he walked to his truck, stepping on some ice causing his feet to slip out from under him causing employee to land on his back and back of his head.
EE states that he was sitting on top of picnic table conducting weekly safety meeting. As he finished meeting he jumped down quickly while leaning forward and landed with most of his weight on his right leg. He states that he continued to have problems and went to doctor. Doctor has removed him from work as of 10/28/10. He was having surgery today 10/29/10
EE was working on a tandem axel dump truck.One EE was in the cab and was operating the PTO the other EE was knelt next to the PTO pump.The EE placed his right hand on the pump for support and balance. His left hand using a tool trying to push the dump plunger.The tool slipped off of the plunger and struck his right hand forcing the hand into the pump shaft.
Employee was operating the shuttle car,the shuttle car bumper rode up on the miner bumper lifting the tires about 1 foot off the ground. The shuttle car rolled back and fell off the miner bumper causing the employee to bounce and strike his head on the cab of the shuttle car.
While supplying fuel from the maintenance truck to the excavator, the employee jumped from one to the other falling to the ground hurting his shoulder.
EE was traveling on the roadway to the mine to begin work. As he started up the hill his truck lost traction . He attempted to get out of the truck, slipped on the slick roadway and fell, causing him to slide under his truck, hitting his chest.
Injured was getting into surface truck; truck came out of gear pinching side of his leg.

At the end of the shift, employee was riding in a 4 passenger rail mounted mantrip exiting the mine. The operator of the mantrip failed to stop as the trip approached a set of air lock doors resulting in the mantrip crashing into the doors.
While adjusting grizzly bars EE had his right arm overhead pushing on bar to position. There was a ""pop"" in his shoulder. Shoulder aches and hurts with resistance.
Employee was walking to the elevator when employee stepped on small rock and twisted right ankle.
walking across the yard, slipped on some packed snow and fell, causing a strain to his left hip.
Employee was roof bolting when he drilled a crooked hole into very hard roof strata. When the drill pot was lowered, the steel snapped back and struck the employees right forearm, resulting in a simple fracture to the right radius.
Employee stated that he was unstopping a piece of drill steel. When he hit it on the drill head trying to unstop the steel, it flew up and hit him under the eye.
Employee was driving haul truck down the west main drift when truck stopped suddenly and jarred employee's back. Was released to full duty 02/10/10 and then was given a restricted duty on 02/13/10. Follow up MRI was completed on 02/25/10 and it was found he had ruptured disc. Employee's injury will become a lost time.
On 2/9/10 a water valve broke causing water to push employee in a low spot in the ground resulting in a knee strain. At the time he denied medical treatment. On 10/15/10 employee requested to seek medical treatment and was taken off work.
Employee was working in the bottom of the elevator shaft up under the elevator when a huge piece of ice fell off the wall of the shaft from an unknown distance and struck the bottom of the shaft. When the ice landed it shattered and a piece struck the employee on the right side and foot.
As the injured was going to install a roof bolt into the top. As he twisted to install the glue and bolt he heard and felt a pop in his back.
Employee was getting inside Dozer when he slipped on the track resulting in jerking his back and neck.
ring finger on left hand was fractured when his hand was forced between 2 pieces of metal as he removed the cap on the sand processor plant.
Employee reported he was driving a haul truck when he hit a pot hole jarring his back causing severe immediate pain. As a precautionary measure, the treating physician restricted the employee from all work activity during the initial diagnostic phase of evaluation and treatment. At a subsequent appt, the work restriction was sustained.
EE was loading cinder blocks in scoop and felt a pulled muscle.
Employee alleges he was lining up holes to drill and he slipped on some ice hurting his right knee. Employee started losing time on 02/16/10 and was reported to supervisor on 02/15/10.
Employee was preparing to service dozer. He climbed up the dozer tracks from the outside and then started climbing down the inside of the tracks when he lost his footing and fell a distance of aprox.2.5 feet.His body landed on his right hand and arm area. The trauma from hitting this area with his body caused his right hand/wrist/lower arm to swell and became painful.
Employee was moving a rock off the miner when he felt a pop in his right shoulder. Employee has worked and recently was diagnosed with a partially torm labruim in the right shoulder. The employee has worked full duty. He is now doing 10 days of restricted duty before arthroscopic surgery to repair the tear.
EE was operating a Joy SC10 Shuttle Car when this accident happened. EE was getting off of his shuttle car when he stepped on the ground (bottom) the bottom was uneven, causing him to twist his knee.
Ran to turn plant off because it had stopped. Slipped in the mud and fell, straining back.
Employee stated that while walking down truck ramp he slipped and fell on ice.

The employee was climbing out of the operator compartment of the scoop and felt pain in his lower back. He began missing time due to continuous pain as of 3/2/10
Attempting to lower batter off of joy ram car, he failed to have car properly positioned. Car was titled and battery slid off the rear of car. This allowed the car to bounce causing his head to strike against the canopy of the car. On the following Monday, on the 2nd shift at mid shift stated his neck was sore and he wanted to be taken from the mine.
Employee hurt shoulder while pulling on drill and steel.
Employee accessed an unauthorized area on a screen to free a frozen belt while the conveyor was running. His coat became entangled between the conveyor and belt pinning him against the two. The breaker popped due to the resistance. Employee was taken to hospital where x-rays indicated no broken bones or sutures required. Bruises to his upper left arm.
EE was stepping out of grease truck when the wind blew the door shut and it hit him on the arm.
Employee got off slope car at bottom of slope. While he was walking he slipped and fell hurting his left knee.
Employee was working on Stamler Feeder Breaker to clear a plug and a hydraulic hose blew spraying employee with hydraulic oil. Employee was burned on right forearm and face. Employee was transported to ER for treatment.
Two employees team lifted an 8-inch pipe valve to carry it. The injured employee stepped into a mine floor hole and sprained his heel ligaments.
While looking for a skirtboard, employee stepped in a soft spot and his heel went down, causing a tear to his achilles tendon.
While in process of using a auger drill, employee was drilling a hole for anchor pins, drill got hung in hole, causing drill's handle to spin, striking employee's left wrist, resulting in a scaphoid fracture.
At approx 1:30 PM on 2/6/10 while preparing a belt splice on #5 belt, EE was struck on the right side of his head and face. The steel cable being used to pull the conveyor belt slipped, striking EE.
Employee was rodding out the O2 analyzer port at the kiln discharge. The port must be opened and the opening cleaned of any debris. Injured was working from the ladder and was removing the cap that covers the port. Hot material blew hot material out onto the left side of his face. Material went under his safety glasses and got in both eyes.
Employee was walking on the section after building a stopping when he slipped and broke his right ankle. He went to the hospital and they put a plate and screws in it.

Walking out on shop apron step on frozen mud. His knee shifted and while lifting his other leg, placed upper body weight on left knee. Felt a pain in the knee joint and visited a doctor. The incident happened on February 5, 2010 however had surgery done on March 2, 2010 and started the miss day from schedule.
Employee was operating continuous miner all day. Near the end of shift he experienced pain in shoulder, limited movement in shoulder.
While drilling a hole for a roof bolt, the drill steel hit EE in mouth. This cut his lip and chipped his teeth. Stitches fixed his lip and a dentist took care of his teeth. Missed one day of work. Back to full duty with no problems.
EE was installing ridged rail belt structure. He was lifting with both hands when he felt a strain in his groin. He tried to work thru this for weeks and weeks, but then decided to consult a doctor, who found he had a hernia. His first day off work was 9-21-10. He is doing well now.
He was pulling on a solid hold down roller that he was reclaiming and felt a pain as if he had strained himself.
While waiting to enter Homer City Power Plant, Injury noticed that his tarp was stuck, the driver tried to actuate the tarp 3 or 4 times and it stuck in the same position, he then climbed onto the side of his truck while trying to free the tarper it released and knocked his arms off of the truck and threw him onto the ground.
Employee was prying a rock loose from the 9E section feeder when he lost his footing and fell on the slate bar fracturing a right rib.
Replacing screen on sieve. Weld broke on rail, fell to floor, injuring back.
The illness person was helping another ee to cut a piece of metal with an oxyacetylene burner. When they were cutting the piece of metal it bent and with the burner he burnt him in the left hand and between his fingers. The illness person went to the hospital the same day.
Employee was pulling belt in head drive with a yellow rope. The rope broke and came through the deck and hit him in the eye.
Employee was drilling a hole when the drill steel stuck in the hole. He raised the head to reengage the steel and caught his right hand between the drill head and the roof causing injury to his thumb and index finger
Employee previously had rotator cuff surgery on his left shoulder. On February 4th, 2010, he slipped walking down a flight of stairs and attempted to prevent falling by grabbing onto a handrail. This motion re-injured his surgically-repaired shoulder.
On 2/4/10 EE was trying to get ton bag off the run. He squatted, put his back to bag and pushed and felt pain in right knee (he declined medical attention on that day). On 3/3/2010 EE reaggravated knee, while stepping off portable ladder to the ground his knee gave out. Sought medical attention on 3/3/2010.
EE reported back pain on 2/4/10 due to repeatedly bending over while coiling up heat trace cable. He did not want to see dr at that time. On 2/9/10 he reported he had reinjured his back while lifting his personal tools from back of site truck. He still did not want to see a dr. On 2/10/10 he notified his supervisor he was not coming in due to back pain. He was then taken to a dr.
Twisted ankle on stairs in hoist house.
The employee stated he was walking along B belt at CC#65 and contacted a monkey face with my head jamming my neck. The employee started missing work on March 15th of 2010 due to the event.
While dumping R-85 Eculid rock truck, main hydraulic hose burst, oil caught fire on hot exhaust. Flames were around cab. Operator did not set brake or stop engine. He exited cab started down ladder, jumped from third step, injured right leg.
Loading the anfo from the magazine into the shot truck, the bag slipped out of co-worker's hands coming down on EE's left arm.
Employee stated that he was using a metal pry bar to pry a piece of rock off the top of the miner. As he was prying the rock slid very quickly off the machine and struck the victim's foot. Started missing after his regular 3 days off which was the 8th of Feb.
A D-10 track dozer started pushing chunks off of the east side of the cut between the two shovels. The dozer operator started working back and forth to try to get back up their push. The dozer at that time started sliding forward on the material that had built up, and fell off of the bench.
Putting BOP on wellhead. BOP was started and was not threaded on far enough when it fell to the ground striking employees foot.
EE was following another employee into Bin #8 (plant area) and some dirt and/or rust fell into his face. EE went to the E.R. first thing the following day-2/4/2010 after his injury. This is when the supervisor was notified of the injury.
EE was binding or securing load on flat bed trailer when prybar slipped striking him on his right wrist.
EE was loading a roll of 3/8"" steel rope onto the #2 digger truck. He turned to put the wire into the truck and felt severe pain in his lower back and down his right leg. He worked until 4/14/2010 and went off work until 7/6/10.
While moving the roof bolter head forward, the canopy caught against a roof strap. While attempting to release the strap from the canopy, the man caught his fingers between the canopy and canopy extension when the strap came free.
The employee was lifting a welder up to put in the back of a supply jeep whn he felt pain in his lower back. The employee continued to work until he got the results of his MRI back on 2-11-10, showing a bulging disk in his lower back.
Haul Truck Driver slipped on ice spraining his left ankle and effusion to left knee.
EE exited cab of articulating truck after he had raised the bed. He walked behind the truck to hook a chain to a pipe to drag up a hill. A piece of frozen material broke loose and rolled out of the truck bed and struck him in the left knee.
Employee was helping 2 other employees change a return roller on a stacker. He was lifting the belt and twisting while placing the roller. He was lifting approximately 20 pounds at the time. It affected his lower back which he has had issues before.
Employee was dropping a cross under guard loose from a belt line. The chain that was holding the guard in place was wrapped around a 4 inch water line. He dropped the guard and the chain unrolled very fast hitting him in the face. Employee received 3 sutures to the nose.
Employee told his foreman at the end of his shift that he might not be in the next day because his back was hurting. Employee called the office the next day and said he had been to the doctor because he had hurt his back driving his end dump over rough roads.
On February 2, 2010 an employee was helping pick up a pump barrel when he felt a pain in his left groin. He continued to work and on March 3, 2010 due to complications he started missing work.
EE was prying on augar steel with a pry bar when the bar slipped, pulling his low back.
Climbing back on truck after lunch, missed hand rail and fell to the ground backwards.
A drill basket was being picked up by loader to move off door. Bucket slid off fork lift, employee tripped and fell underneath fork. Right knee caught underneath forks, causing strain and swelling.
Employee was carrying two slate bars and he was stepping up on a flat car. As he stepped up he felt pain in his right knee.
Was working on overcast turned to walk away from overcast his ear struck a piece of metal screen that was hanging down which cut his left ear lob. Compliance rules/regs was not a factor, mining equipment/system was not a factor, ppe was not a factor, miner profiency was a factor.
Employee was dragging screen to be installed as supplemental roof support and stepped on a rock, twisting his right ankle. He sprained it.
At 12:50 the Site Supervisor called main office and advised that he had been told by his contact at the plant that an employee from night shift had went to the hospital as a result of lime burns around his neck. Employee had work his shift from Monday, 02/01/10 at 6:00pm to 4:00am Tuesday, 02/02/10.
The operator of the 544 haul truck was descending the lower steps, when his left foot missed the rung. He then fell to the ground.
There was an alleged event as per WV Workers' Compensation dated 2/2/2010. There were no records or information provided to the company except through WV Compensation on 2/15/2010. He claims he bruised his hand riding in a mantrip, but is missing work.
Employee was removing a rock in the walkway and felt a sharp pain in low back. EMPLOYEE DID NOT LOSE TIME UNTIL 8/3/2010 FOR 2 DAYS. **Employee started losing time again on 9/3/10 due to surgery.**
Employee sampling for Quality Control to ensure compliance. The Employee lifted 5 gallon buckets of material to spread evenly into a sample pan & felt a snap in his shoulder. 5 gallon buckets of material weigh about 65 pounds. He didn't need treatment until now, his condition worsened. May 18 2011 the employee had surgery & missed his next scheduled shift on 5/19/2011.
He had climbed up onto a dryer in the plant. When dismounting the dryer he stepped down to the effuent box and then down onto the effuent pipe. The soles of his boot were wet, his foot slipped on the pipe, resulting in a possible sprain to his knee. Started missing scheduled work days 2-8-10, became a reportable incident on that data
The EE was wiring a terminal strip and contacted a hot surface causing a burn to his hand.
Man was lifting man trip with floor jack. Jack slipped out from under man trip pushing his arm back. Man felt pain in left shoulder. First day off work was 6/8/2010.
Employee was in process of running dewatering pump & twisted/reached for the control panel & felt pain in back, hip & leg.

EE, a roof bolter on Unit #1 was on the operator side of #38 Fletcher bolter in Entry #9. He walked to the back of the machine to get a bag of rock dust. He stepped in a hole/depression in the mine floor twisting his right knee. He informed his Supervisor. EE was transported by Company vehicle to Good Samaritan Hospital where he was diagnosed with a right knee strain.
Employee was lifting a piece of I-Beam with right arm to put a block of wood under it.
He was installing a 16' cable bolt, the bolt was pushed into the hole by hand. He was reaching for the drill when the bolt fell out of the hole and struck him on the right middle finger causing a laceration and fracture to the right middle finger.
Employee was riding Lee-Tram when equipment hit hole in roadway and employee jambed his neck and shoulder.
An employee and a co-worker were pulling a 3'x4' sheet of metal plate. The employees had placed a chain through two holes in the plate and were dragging the plate across the mine floor. After letting go the employee turned to walk when he felt the pop in his knee. ON March 8th he was diagnosed with having a torn meniscus requiring surgical repair.
REPORTED ON 3/4/10 - tapping holes, walking from hole to hole on the shot, hurt right foot.
The Miner left a note on his time card at the end of his shift 2/26/2010 hurt right rib laying on his rt side working on a motor no medical treatment 3/1/2010 The Miner said he was ok and incident report was filled then. 3/4/2010 he claimed re-injury himself after he coughed. 3/5/2010 he saw the Dr. he return to the next work day and put on lite duty for the week.
Employee was drilling hole for roof bolt when a piece of draw rock fell from top striking his right hand between pinky and ring finger. Rock was approx 12"" x 6"" x 4"".
02/25/10 Employee was rigging concrete blocks and positioned himself in a pinch point between two blocks. When block was lifted, it pinched his calf. Shift was over at the time of accident, employee called immediate supervisor and reported he pinched his calf but no major injury and would be reporting to work in the morning.
While pulling up miner cable the employee's foot got tangled up in the miner cable and when the cable pulled tight it caused the employee to fall forward striking his left knee against the miner.
Forklift slipped on ice. The fork hit employee on ankle.
Employee was lifting a screen, and felt a pain in his groin area. Employee didn't seek medical attention until March 25th. Upon a medical examination, a hernia was discovered.
The employee was walking from a work location carrying a shovel and broom. The employee reportedly slipped, fell, and struck his head while walking from the pay loader to the main office. The employee was disoriented and did not clearly remember the exact location he fell, or what events led up the fall.
EEs were replacing supporting pipes for head protection inside the ""kin riser"". Debris came loose, struck EE on the head and shoulder, knocking his hard hat off and causing a contusion on his shoulder. Another piece then fell and struck his head, he was wearing a Tyvek suit hood and a head sock. He was treated and released from the hospital.
While operating a mantrip struck a battery ramcar on the travelway. The impact caused him to strike his head leaving a laceration. EMTs placed him on a backboard and transported by ambulance to Springfield Memorial ER. Treatment included staples to close the wound. A CT Scan revealed transverse process fractures. He was admitted for observation and discharged 2/24/2010.
Employee was carrying a chain hoist and stepped on unlevel ground causing his knee to buckle. Torn cartlidge.
While exiting the loader the employee tweaked his knee. That night his knee swelled up and he began to experience pain. He was taken to the doctor the next day. No restrictions were placed on the employee, but he was referred to a specialist for further diagnosis and an MRI which found he had a torn meniscus. The employee was placed on restricted duty as of 4/16/10.
Employee was sitting on the top area of the #5 cooler fan damper housing using a rachet to tighten bolts. Employee's hand slipped off rachet handle causing employee to loose his balance and fall 10ft to the floor area below. Employee was not wearing / using proper PPE.
Injured employee was bolting on the #3 section when the bolt he was installing bent and struck him on his left forearm. On 02/22/2010 Orthopedics diagnosed him with a Ulna fracture.

Employee slipped while loading a truck causing a sprain to his knee. It is thought that the employee tore his meniscus.

While gathering and burning brush, the injured's leg became entangled in some vines. When he attempted to pull it free, he immediately felt pain in the leg from a strain.
Employee was inspecting and cleaning the ice from the rollers on K4 stone belt when he stepped on a pry-bar on the catwalk. He was using a sledge hammer to beat the ice from the rollers. The bar slipped out from under him and he fell. As he fell His right hand became lodged in between a return roller and the belt. He was able to pull the e-stop and call for help.

At his follow-up appt. on 2/25/2010, he was prescribed physical therapy thereby changing the incident from 1st aid to medical recordable.

A garage mechanic suffered a hernia to his right groin while performing a 300 hour service on Cat 992G loader. The injured miner had to shovel snow off the deck of the loader to gain access to a door. Then as he attempted to lift the door he slipped and felt a pull in his groin. He did not seek medical treatment until March 12, 2010.
EE was uncoupling a Vogelsang trailer mounted pump from the MS4 truck as the hitch cleared the ball the trailer tongue begin to move sideways away from the ball. EE realized the jack was falling and jumped backwards. As he landed he came down on his right ankle rolled. EE was taken to the Front Gate to be checked out and transport to the ER. Diagnosed with a broken right ankle.

Employee was installing a dump body lift cylinder on a 773 haul truck. When the support rigging shifted the cylinder fell against the wheel hub of the truck. The employee's wrist was caught between the cylinder and hub, resulting in a broken wrist.
Employee parked her truck, climbed down, walked 5 feet, slipped on the ice, and fell to the ground. This action caused a hip to fracture. Weather was light to moderate snow falling all day.
The employee was using a pipe and a hammer to break up clumps of material in an outside hopper. The employee put one end of the pipe on a clump and went to strike the other end of the pipe. The hammer head glanced off the end of the pipe and the employee's hand that was holding the hammer hit the end of the pipe. The employee broke the second digit on his right index finger.
EE was driving a Genie Lift around a corner. As he pressed the lever to move forward, he pushed too hard and the lift basket jerked, causing ee to jerk front then back. Employees were moving an eyewash station building with a fork lift. As the fork lift operator began lifting the building, an employee bent down to pick up an electrical cord. The building tipped over and pinned the employee against a concrete barrier. The employee was transported to the hospital, where it was determined that they had 2 pelvic bone fractures.
EE started work in the morning feeling fine. His lower back suddenly began to hurt on the right side. EE cannot pin point the activity that may have caused this. EE was prescribed Motrin, vicodin and medrol and put on temp. restrictions.

The employee had been working on a piece of equipment. As he went to stand up he started to turn to his right. As he did so all of his weight shifted to his right knee and it gave out. He sustained a medial meniscus tear.
Employee was grinding slag off angle iron, the grinder caught the slag and jumped causing the employee's hand to slip resulting in the grinder cutting through the employee's glove and cutting his thumb knuckle. Employee was placed on modified duty.
EE was stepping into a skiff (anchored on shore) to then step onto the dredge. His supervisor told him not to go to the dredge. When EE turned to get off the skiff, he lost his balance,fell and dislocated his shoulder.

EE WORKED 2/26 - 3/2 AND WAS ON VACATION FROM 3/3 - 3/9. THIS INCIDENT BECAME REPORTABLE ON 3/10/2010 BECAUSE MGMT HAD TO MODIFY HIS WORK DUE TO RESTRICTIONS.
Employee was roof bolting in the North 3 Parallel Section when a piece of rock fell out striking the bolter head and landed on employee's right knee.
When the employee got injured, he was sanding a shaft to a tail pulley of a conveyor belt. The employee had the shaft proped up about two inches off the floor to sand the shaft. The shaft slipped off the prop and crushed the employee's left middle finger. Employee should have been using a different means to prop the shaft up when sanding.
EE was cleaning the overchute above the cone crusher and was pulling the rock away and then felt a sharp pain starting at his shoulder and ran down his arm. This incident is considered an illness, due to the fact that a previous injury occurred to the same shoulder back in 1992.
Employee was air lancing FL02, while standing on a 22"" step. Employee lost balance falling backwards. Caught herself from falling to the ground, in the process twisted left ankle. Left ankle sprain.

EE was spitting sand samples and got hand caught in sand spitter causing bruising to all 4 fingers on her right hand. Possible Fracture on 3rd finger.
EE WAS CHECKING VOLTAGE AT THE CONTINUOUS MINER CAT HEAD AT THE POWER CENTER WITH A SMALL VOLT METER AND IT BLEW UP IN HIS HANDS.
EE was installing a 6.5 X 6.5 square steel tubing core over shaft of a belt winder. EE and coworker placed the winder shaft onto cribbing and placed the core over end of shaft and proceeded to push core onto shaft. As core slid onto shaft ee pinched his left ring finger between core and shaft. Fractured bone in finger tip and laceration, 8 stitches to close wound.
Miner was walking in from parking lot and twisted his ankle when wearing athletic shoes and not boots. He was taken to the clinic and the doctor indicated an ankle sprain. For 1-week, doctor did not want him to climb and take breaks if job had lots of standing/walking. He did not have any jobs that week that would require him to climb or with excessive standing/walking.
