While preforming a preshift exam of the mine a roof fall was observed at the #18 cross cut track entry along the #2 main belt. No injuries occurred from this fall.The fall measured about 25 feet long,20 feet wide and 4 feet high, This area was bolted with 4 foot resin bolts

While checking out 4701 hoist after weekly electrical PM, hoist tripped out while entering upper limits. Could not reset hoist to back skip out of limits. Electricians conducted an investigation and found the #2 overtravel track limit switch activated. Electricians exercised switch to working order.

A non-injury roof fall occurred during pillar recovery on the 002 section, #4 block, 70' inby S.S. 7697, entrapping the mining machine. The rock measured 15' W X 18' L X 4.5' thick and fell in the cut. It did not fall above anchorage. MSHA issued a 103k order which was modified to retrieve the maching machine and resume normal mining operations.

The mine discovered a roof fall on the 4 Main West, entry #3, at crosscut #151. The fall measured 18' X 18' X 7' thick. The area was developed in 2/2007 and bolted with 6' glue bolts. The fall will not be cleaned up. The 4 Main West is being recovered and will be sealed.

A roof fall was discovered in Entry #1 near Spad # 81. The fall measured approximately 30' long x 20' wide x 10' high. No injuries resulted from this fall.

A roof fall occurred in the 4A panel xc 23 spad #15+09 #5 and #6 entry. The size of the fall was 100ft long, 18ft wide and 6 1/2ft high.

Timber movement in Silver Summit Shaft. Damage or impairment is unknown at this time. Informational update for 7000-1 filed on 1/29/10. Upon further examination a major timber failure was found in the Silver Summit Shaft. At this time ventilation has not been compromised, but major shaft repair will be required to re-establish safe travel. Per 2/5 doc 220100340005.

An unplanned roof fall occurred in the #2 gas mine in the #9 entry, brk 36-37, 60' inby spad #1298 that measured approx. 90' long x 19' wide x 9-12' high. There were no injureies reported. The area was developed on 2/17/09.

At approximately 9:30am the elevator was shut down for testing. At approximately 9:50am it was discovered that the bolt to the tension spring bracket to the emergency brake system was damaged. New part was order from outside the United States and arrived at the mine on 01/28/2010. The elevator is back in service on 01/28/2010.

It was reported that the north hoist motor for #3 hoist was making noise. Upon investigating, it was determined that something was rubbing on the inside of the motor so the hoist was shut down for inspection. It was determined that a piece of metal got into the hoist motor so the hoist motor was replaced. There were still two escapeways throughout the incident (#2 and #6 hoists.

An unplanned roof fall occurred in the G - Butt rooms from #1 entry to #3 entry at spad #1092 (#2) entry. Fall was approximately 18' wide, 130' long, 10' high. Area was bolted with four (4) foot and six (6) foot fully grouted bolts and with ten (10) foot cable bolts throughtout. Area was timbered and dangered off.

A roof fall was found in the #6 entry of #1 NW mains one break inby SS #105. The fall was found at 1:45am on 1/26/10. The fall measured approx 5 1/2'H x 19'W x 60'L. The fall did not interrupt ventilation nor was any equipment caught. The fall will not be cleaned up and will be breakered off.

Found a roof fall at x-cut 25 in #8 entry on our M# zone. The roof fall was 20'L, 18' W and 10'H. No miners were hurt or trapped ventilation was not affected and it was not adjacent to an escape way. The area was supported with 6' fully grouted roof bolts prior to the accident. Approaches to the fall have been timbered and barricaded off to prevent entry to the roof fall.

A roof fall occurred on the #4 section in the #8 entry 1 x-cut inby spad #50513. The fall was about 35' long X 6'-8' high. The area was bolted with 5' torque tension bolts and 2-12' rope bolts every 3rd row. The fall will not be cleaned up it was cribbed and dangered off.

A roof fall occurred on the N. Mains Section in the face of #8 entry inby spad 10399 for 80'. The fall extended outby the intersection of spad 10399 for approx. 90'. The total area was approx. 170'x20'x8'. Area was mined 1/22/2010 & was supported w/5' fully grouted torq. tension bolts. The area will be partially cleaned to retrieve CM. The area has been dangered off and cribs

On 1/25/10 mine examiner found a roof fall in #5 entry between #1 and #2 crosscuts. This is a continuation of a previous fall. Fall approx 20' X 20' X 7'. Area timbered and dangered off. No ventilation interuption, no injuries. This is an idle section. Fall was 1 BK inby spad #730.

During a rock dust survey, a roof fall was discovered in the #5 entry of the 1st north east mains section approx. 1200 feet outby the face, adjacent survey spad #11950 which is located in the #4 entry. The fall was approx. 160 feet in length and 4 to 6 feet high.

Crabapple elevator was unoperational due to the breaks needing adjustment. The Elevator was able to be operated in Manuel. No injurys were reported due to this incident. Elevator manufacturer was contacted and performed maintenance.

An excursion of SO2 gas was released from the sulfite scrubber exhaust stack. The SO2 gas carried a distance of 350 feet causing three contractor employees who were assembling a product silo, to the east of the sulfite building to experience nose, eye and respiratory discomfort.

Due to a break in the sump discharge line, an overflow from the surface sump entered the alternate escapeway and adjacent entries to the left of this escapeway. Workers were withdrawn from the mine.

Stack rock fell out above bolt length. 45in resign bolts 40 feet X 19 feet. The roof fall occurred in #8 entry of 001-0 M.M.U. 120 feet inby survey station #1972. Non-energy roof fall no equipment involved.

Access cage lowered to sump. After troubleshooting, determination of faulty lower limit switch. Switch replaced, but was bad. Installed another switch. During investigation, found foreign material on brake pad it was cleaned. Mine was evacuated. No injuries or other problems. Back in service at approx. 2:00a.m.

An unintentional roof fall occurred in the face of #4 entry, it crosscut on the 11 north mains section. The fall occurred in an unbolted 20 foot cut and pulled two roof bolts in the last row of permanent support. Fall 20' x 19' x 2'.

A roof fall occurred on the #4 section in the #2 face. The fall was about 40' long X 18' wide X 5'-7' High. The area was bolted with 5' torque tension bolts. The fall will not be cleaned up, it was cribbed and dangered off.

Freeze/thaw and heavy rainfall caused surface rock to fall on and just inby canopy on #1 portal (intake) entry. Ventilation is affected.

West hoist motor was deenergized to replace brushes on the motor. Upon reenergizing at 10:20 am, it was found that there was no power to the PLC controls. Mine evacuation began and MSHA notified about 10:35am. It was found that the uninterruptable power supply (UPS) was bad in the control power line. This unit was replaced and the hoist was put back into operation at 10:55 a.m.

Roof fall found during weekly exam. The fall was on old 002 section in belt entry starting in intersection @ BK #2 and going to BK #3 inby corner. Fall 1 BK inby spad #170 to 18K outby spad #193. Area cribbed and dangered off on all sides. This is an idle section. No injuries. Ventilation still passing through area. Fall found by EE A-70-01. Fall approx 100' X 19' X 7' in size.

An unintentional roof fall occurred at the Main West return Entry #2 CC 32, 4217 W. The fall was approx. 17' long X 8-9' thick. The fall did not affect the ventilation. The fall will not be cleaned up.

At approx. 11:50 two miners working near rail loading facility noticed a private car enter fac. and proceeded to travel between our rail and Burlington Northern main line. Car became stuck on the tracks. Driver got out of the car and off the tracks just before train struck his car.

An unplanned roof fall occurred in the No. 2 entry of G - Butt. Fall was located one (1) x-cut outby survey station #994. Fall was determined to be approximately 18' wide, 40' long, 8' to 10' high. Area bolted with four (4) foot fully grounted resin bolts with eight (8) foot to ten (10) foot cable bolts installed throughout the area. Area posted and dangered off.

Access cage experienced problem with lower stop switch. After changing switch & extensive investigation when that didn't fix, a slightly bent support arm for switch was discovered. Mine evacuated; no injuries or other problems.

At approximately 4:33pm a non-injury roof fall was found in the No. 1 entry on the MMU No. 001-0 section. The roof fall is located in the intersection at crosscut No 20 @ survey station No. 4739. The roof fall is approx. 20 ft wide x 30 ft long x 6' - 10' high. The areas adjacent to fall dangered off w/timbers and/or cribs to prevent access. MSHA was notified immediately

An unintentional roof fall occurred in the 12 I longwall tailgate entry. The fall measured approx 35ft long, 16ft wide and fell above the anchorage. No Equipment, injury or equipment damage from this event.

A continuation of a roof fall located between #1 and #2 entries at spad 2509. Continued into # 1 entry and fell up to spad #2520. Fall measured 16' high X 95' long and approx. 3500' outby section. Fall was breakered off.

A ROOF FALL was discovered one break inby survey station 2642 in the No. 3 heading near faces of an old section - on a weekly examination. The fall measured app. 70' long X 20' W X 8' - 10' thick and fell between the blocks into an intersection. The fall was composed of laminated sandy shale.

The man cage lily control went out of sync due to faulty lower limit switch. Mine was evacuated. After trouble-shooting, it was corrected and placed back in service for the next shift. No injuries or other problems.

A roof fall occured in the #11 seam in the 2 Main North at crosscut #8 starting in the #1 entry and traveling half way through the intersection of #2 entry. The fall measure approximately 100'x0'-16'x10'-12'. The area was supported with 30"" fully grouted bolts. A slip with the presence of water was determined to be the cause of the fall.

At approx 11:20 am belts went off. At 11:50 am a fall of roof was found between #7 cc and #8 cc on 5 belt. This is non 4 entry on 2 left off of north mains. Fall was between spad #5, 6871 and 6872. Fall is approx. 50' long x 18' wide x 8'-10' high. No injury occured. Area was timbered off and dangered off. Federal was notified. via 1-800 number at 12:05 pm

A roof fall occurred in the 2 West 4 Mains, from crosscut 41 to 42, in # 5 entry. The fall measures approximately 50' long, 18' wide, and 5'-6' thick. The roof fall covered the 2 West 4 belt flite at this location.

Roof fall in tunnel 86E of primary escapeway impeding travel. No injury and no equipment damage. The roof fall happened under normal mining conditions. The area was immediately barricaded and employees notified of restriction of travel and evacuated. MSHA was notified within 15 minutes (07:36 PM) via Hotline.

A roof fall occurred in the Main North in #2 entry starting inby the pillarline of crosscut 32 and extending through the intersection of crosscut 35 then extending through the pillarline of #3 entry measuring approximately 250ft long x 19ft wide x approximately 8ft high. This area was supported with 4ft fully grouted roof bolts and 8ft cable bolts.

There was a roof fall #1 unit beltline,between X-cut 36 and 37. ( Had been truss bolted and cribbed previously). Length 60', width 18', thickness 10'.

Rock fall occurred in the bleeder entry on 1 right off 5 West Mains at break #26 between #3 and #4 entries. Fall measured approximately 20' wide x 30' long x 6' thick.

On 01/14/2010 at approx 5:30 P.M. on # 4 Unit, MMU-006-0 the continuous miner operator was extracting coal from the # 8 Entry. The head of the continuous miner came in contact with a pyrite inclusion above the coal seam and an orange glow was detected at the head and approx 10ft outby the head. It was of short duration and extinguished itself. No injuries resulted from this.

The elevator stopped running and the elevator doors would not open. Elevator was down for more than 30 minutes due to slack rope governor chain had iced up, stopping the chain motion. This was not an accident but elevator was down more than 30 minutes and interfered with the use of it. Back in operation at 7:00am 1/14/2010.

An electrical contactor failed on the production hoist. Quite some time involved in locating specific unit. Employees evacuated from mine. Problem found and repaired. Hoist back in service at 11:00pm. No injuries or other problems.

An unplanned roof fall has occurred at entry #5, spad #396, approximately 5,000 feet from the active face in the return. The fall measured 30' x 19' x 5'. No equipment or miners were involved in the fall. Note: This fall is a continuation of a fall that was previously reported.

An unplanned roof fall occurred in the #5 entry on the #1 section @ Shonk Round Bottom. The fall occurred in the #5 entry spad #3989-4002. Fall measured 20' long x 17-18' wide x 4-8' thick. Area was previously supported by 5' Torque tension bolts with 8' cable bolts in intersections. This fall occurred 500' from the active 001-002 section. Area was originally developed 12/23/2010.

The main hoist park brake set and would not release after loading a skip. Initial inspection indicated the problem was mechanical in nature and the mine was ordered to begin evacuation procedures.

Several roof falls in the 2nd left panel off of Sycamore Mains, ranging from 30' to 150' in length and 18' wide, 4' to 5' in heigth. This is a worked out panel, requiring weekly exams approximatly 4000' from an active section. Location of the falls are at spad numbers 3650, 3653, 3688, 3831, and 3662. No injuries or equipment involved.

A roof fall was discovered in the intersection of #6 entry adjacent to 2A beltline, inby spad 24269. Estimated size of roof fall is 12 foot high, 20 foot wide and 20 foot. No injuries were reported as a result of this fall.

A roof fall approximately 18 feet by 40 feet in size was found in the return of the 3 right panel. There were no injuries or interruption in ventilation.

A roof fall ocurred on 003 section during retreat mining. The fall ocurred inby the 65th x-cut in the pillar block extending out over the continuous miner. The size of the fall was undetermined and the fall pulled 2 roof bolts. MSHA was notified immediately.

Overhead radiant heater element burned, fire on spool table below. Burned table, 2 chairs, some electrical wires on wall. No damage to building or other contents. No people on site at time of fire.

IGNITION: The Continuous Miner was cutting coal in the No. 2 Face on the No. 8 Section, when the ignition occurred. The operator stated he was cutting bottom rock on the right side on the No. 2 Entry when the ignition occurred. The water sprays on the miner head put the fire out.

A rock out burst in the main entry to the 4600 down ramp area caused the withdrawl of miners and disrupted normal mine activity for more than 1 hour. The estimated amount of material knocked down is 4 to 6 railcars. Estimated weight 15 to 20 tons? Estimated time of repair is 3 to 5, 8 hour shifts.

At 13 belt drive at a ( 500 KVA power center) there was an electrical fire in the transformer of the power center located in 67 mains, the power center was de-energized, miner's were evacuated, fire was extinguish, all local agencies were contacted, no injuries, and power center has been removed from service.

During an examination a roof fall measuring 20' L, 20' W, and 5' thick in the intake entries 4000' from the working section. The fall was located in the #9 entry inby spad 58227 along #3 belt which was developed Aug 07. The type of roof support used was a 5' glue bolt. Fall did not impede travel or ventilation and occurred due to laminated rock and coal in the immediate roof.

A roof fall occurred in the 2 West Mains, at 46 crosscut, in # 7 entry. The fall measures approximately, 40' long, 20' wide, and 12' thick.

A roof fall occurred in the #11 seam in 2 Main North in the #1 entry starting at crosscut 153 and ending at crosscut 154. The fall measured approximately 60' x 10'-18' x 2'-3'. The area was supported with 30"" fully grouted bolts.

A roof fall occurred in the 2nd East Panel in the #1 entry in the intersection of spad number 8+00 measuring approximately 20ft. long x 17ft. wide x 8ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

A roof fall occured in the #3 entry, about 1400 ft from the face of the #5 section, at spad #49615. The fall was about 18 ft wide, 25 ft long, and 4 to 7 ft high. The area was bolted with 6 ft torque tension bolts. The fall was crib and danger off and will not be cleaned up.

#3 hoist tripped on ""#4 West Brake Not Applied"" fault. Maintenance foreman, electrician, and mechanic were called out of the mine. The brake was checked and adjusted and the hoist was inspected and reset. There were still two remaining escapeways throughout the incident (#2 and #6 hoist).

#3 production hoist tripped on ""Drive Converter A"" fault. Maintenance coordinator and electrician were called out of the mine to check the hoist out and reset the hoist. There were still two remaining escapeways throughout the incident (#2 and #6 hoist).

The brakecar was frozen to the track and the slack in the rope after the brakecar was sent in. This shut the hoist down at the top of the track slope. The hoist was down from 3:50a.m. until MSHA released the trip at 8:30a.m.

The roof fall was discovered in the #1 Mains Section, #12 Crosscut, #2 Heading. The fall was 8,800' outby the nearest intersection. The fall was 18'x 18'x 6' and was bolted with 6' fully grouted bolts. No equipment or ventilation was affected, and no individuals were injured.

Hoist operator noticed arc flash on brush end of electric hoist motor. Motor was shut down and an electrician was called and the mine evacuated. The lug connections to the brush rings were tightened. The hoist was tested and placed back into service. Miners returned underground at 7:15 am.

A transport line containing liquid sodium bisulfite solution separated at a joint causing a release of SO2 gas. Contractors working in the adjacent area of the S-300 plant were exposed to the SO2 gas. The area was evacuated and flow to the line was isolated. The released liquid sodium bisulfite was neutralized by applying soda ash and the area was secured and flagged off.

An electrical glitch occurred in the elevator operating program causing the elevator to become stalled. Montgomery Kone (elevator service contractors) fixed the problem. No entrapment or injuries to persons or damages to any equipment or any ventilation disruption occurred. The elevator was repaired and tested & found to be operating properly by Montgomery Kone personnel.

It was discovered at approximately 8:00 AM that the trailing cable for the Nordburg auxiliary hoist had been damaged. The hoist was out of service for over 30 minutes. The mine was idle.

1ton dodge truck was up on jack stands with front wheels off ground,employee was under front of truck bleeding the power steering system,he asked another employee to get in truck and start the engine. The truck was started and the employees foot slipped off the clutch and the truck fell off the jack stands falling to the floor crushing the employee under truck.

The employee was cutting small pieces of steel in the metal shear. He was using a piece of metal to push the pieces into place. Rams, which hold the metal flat just before the cut, came down forcing the metal he was holding to crush the tip of his finger between the metal and the table surface of the shear. Two mechanics were working on a Kubota Tractor trouble shooting an ignition problem. One mechanic was testing the starting wires and the other was checking tension on the alternator belt. The ee entered the manvan at shift change and proceeded to the rear seat. The ee put his seatbelt on and the man van proceeded out of the main pit. The chain pulling the tailpiece broke and allowed the tailpiece to travel toward the header under tension. It pinned the victims leg between the ratchet jack chain and the mine rib.

The surgeon informed him on 2/24/10 that he had a torn tendon in his shoulder.

Employee stated that on 01-09-10 he lifted a crib block on to his shoulder and turned and stepped off a ledge in the mine floor near the section feeder and twisted his knee and causing him to fall. Employee finished his shift and did not start missing work as a result of this incident until 01-12-10.

EE stated that his tailbone was inflamed due to seat in the dozer.(Unit #1242) he operates is flattened out. This was not reported until 01/12/2010.

A dozer operator was pushing coal cleanings into a berm in a drilling and blasting area. While backing to reposition, the operator misjudged her distance from the crest and backed off the highwall. The dozer fell approximately 33 feet onto a slough pile. The operator was dignosed with one ruptured disc.

The injured person was rock dusting by hand in the #4 entry. The scoop operator stopped and gave the person notice that he would be tramming through the area. The scoop operator thought the man was clear and began tramming when the scoop bucket struck the man on the foot.

Injured person was pulling on a sheet of plastic to cover hay stack, when tension released on plastic, causing injured to fall striking elbow on metal culvert band. No lacerations reported only bruising. Entire body struck ground.

Emp haul truck getting loaded. Huge rock fell from shovel bucket, hit headache rack of truck, jostling emp in cab of truck. Emp was thrown up in seat, caught by seat belt, and slammed back down. Seen at ER, CT scan showed compression fracture of L1, referred to orthopedist who placed emp off work.

The locking mechanism failed as he was hitting it with a hammer. Air and ground sand struck his face when the lid blew open.

Employee alleges a knee injury when he stepped off the back of a truck and his knee buckled. Due to extenuating circumstances surrounding this event, an investigation ensued. However, the claim was determined compensable.

Operator was getting on 375 Komatsu Dozer, he put his foot on the step on the side of the push arm and grabbed the handle on the tilt cylinger, his hand slipped off the handle and he fell backwards onto the ground on his backside.

While working on 24s dump, employee moved a big boulder off of the road and spun his dozer around causing him discomfort in his lower back. He was seen at the Emergency Room and taken off work.

While catching himself, he jarred his right shoulder, experiencing pain. He has worked continually since, but underwent surgery on October 6, 2010 to try and alleviate the constant pain.

EE alleges on 01/06/2010 he was walking thru a curtain and hit his head. He did not report this to anyone. On 02/01/2010 he was sent for x-rays, still not telling anyone it was work related. On 02/02/2010 he requested an accident report be filled out. He did not miss work until 06/07/2010. His claim is being investigated to verify if this is a work related injury.

Flash burns (welding)

Employee was holding angle iron between two piece of channel for x bracing while another employee welded. He was looking away from the welder but the arc was too close and caused welders flash.

He was walking along the long wall face when he tripped and fell striking his right knee on a pontoon. He continued to work at his regular duties but continued to have pain. He was eventually removed from work on 3/19/10 for a possible right meniscus tear.

EE was installing the outer bolt of the last row in No. 1 entry when a piece of rock 2' x 4' x 4"" thick fell on his back.

Employee stepped on a piece of coal lying on the mine floor and twisted his right ankle. He continued to work until the ankle began to swell and was transported out of the mine to a medical facility. X-rays revealed a lateral malleolus fracture to his right ankle.

Employee was operating the hammer. He noticed several spiders on the hammer and killed them. Apparently one of the spiders crawled up his pants leg and bit him on the thigh. By the 6th his leg had swollen and puss had formed. He went to the Medical Center where the bite was lanced and packed. Not sure what kind of spider it was. He is still undergoing treatment

The drill head swing jack on the diesel power roof bolter had to be repinned to the machine. The operator caught his right thumb between the boom jack and frame as he positioned the head. This resulted in a fracture to the thumb.

Employee was pulling down on the outside elevator door located on the 4th floor of the pre-plant, when employee felt pain in lower back.

Employee was dismounting his articulating haul truck at the end of the day. While proceeding down the steps his foot caught awkwardly in one the steps causing his hand to slip off of the railing and fell about 5 feet to the ground. Refused treatment initially, but the next morning wanted to get checked out and was diagnosed with 2 broken ribs on his right side.

The employee attempted to remove a concrete stopping block from the roof by stepping up onto the rib mesh. When he hit the block a piece of rib fell behind the rib mesh and struck the emploee's fingers which were through the mesh.

After break at 4:00 AM, the EE was heading back to the job site from the break room and ran into Jersey Block type berm, hitting head on the right side window of the truck. EE was found at approximately 6:00 AM where he was semi-responsive.

Employee was coming down off C22 ladder and there was snow and ice at the foot of ladder. When he stepped on the ground he slipped and twisted his knee. Injury was moving a manlift into position when the manlift basket caught onto an instrument box. The boom swung around and his finger was smashed between the manlift basket and the drain valves on the bottom of the heater. The resulting injury was a fractured and lacerated finger which required medical treatment and lost time.

While attempting to unplug a chute feeding a coal mill, the platform on which he was standing, tilted, causing the employee to fall about four feet, to his knees.

Employee was holding curtain up so the scoop could get through, his foot slipped into a rut in the roadway and the scoop ran over the end of his right foot.

Employee was removing a screen panel which requires pulling the screen panel toward you. The panel can vary in weight form 25-30 lbs. He then felt a pull in the lower left groin area.

While painting I-beams in the shop EE attempted to clean the tip of a high pressure spray gun with his index finger. He inadvertently pushed the trigger which caused a small incision that allowed paint thinner and aluminum paint to be injected under his skin. Surgery was required for treatment.

Employee was walking down the stairs in the mill plant when she slipped and fell, landing on her buttocks and hitting her lower back on a step. She felt she was OK and returned to work. On Jan 28, 2010 she visited the doctor and was put on work restrictions and given physical therapy medical treament.

Employee working from step ladder fell approximately 3.5 feet to the ground, landing on his right leg hurting his ankle. 911 was contacted, ambulance responded, and transported to local hospital. Doctors at hospital evaluated his injury and determined he had a broken ankle. Employee is currently off duty until doctor's release him to return his job.

He sought out a chiropractor on 1/25. He has continued to work everyday and the only time he has missed has been for chiropractor visits. After 3 visits he states his back is sore but is better

EE was shoveling and got clinker dust in his eyes. He was given first aid and was alright. That evening he went to the emergency room without notifying company representatives. Because of the patient privacy act, we were not able to obtain medical information until today 2/5/2010, at which time this became reportable.

He slipped on muddy track while mounting dozer, fell to track & hit L knee on blade rail & stomach on the track. Reported fall the next day, saying he was just sore. We sent him to dr on 1/27/10 after he complained of stomach discomfort & swelling knee. They performed x ray of knee and ct scan of stomach. set up an appt. for him to see a surgeon on 2/2/10 about stomach hernia.

EE was doing engine oil change on D10 dozer while he was coming down off machine instead of coming down the approved 3 point of contact method he stepped down front words not using the hand rails and fell hurting his right arm.

Injured was assisting in the construction of a new portable field office. While attempting to reposition a roof joist, the opposite end fell off its support crushing his right index finger between the joist and a metal support, breaking the bone.

EE had a problem with his hitachi shovel and was trying to get the ladder down, he jerked on the latch to lower the ladder and injured his back. Frozen mud was keeping the latch from releasing, the mud was removed and ladder lowered.

The employee stated that he was shoveling coal off of the catwalk of the collecting belt. He pushed forward to get a shovel full, and he felt a pop in his right knee. The employee continued working until he was scheduled for a MRI. It was determined that he had a tear of the MCL. He is scheduled for surgery on 2/8/10.

NOTE: Safety Dept was not told of his surgery or light duty until 6/10/2010.

EE and two other employees were setting a new belt header. Header frame was sitting on a duck bill of a scoop. Employees were attempting to level the header frame when EE crawled under the boom. The header suddenly tipped forward and struck EE.

Employee was installing a roof bolt with a pin board attached. As the employee depressed the boom raise lever with his left hand, he attempted to adjust the pin board with his right hand. The employee did not release the boom raise lever or the pin board and his right index finger was caught between the pin board and the mine roof at the first joint.

Employee was bolting roof in 4 Right at break no. 35. Top rock fell and struck him on head and left shoulder.

Was pulling cable off cars so they could be inspected by MSHA. He did not have pain until the next day,and filed a report on the 13th. He said he woke up with pain on his right side/hip/and leg.

On 10/30/08, worker was changing a screen in the mill and felt pain in upper right arm. He was diagnosed with a strain and lost no time from work. During the next year he sought further medical attention and was diagnosed with a torn rotator cuff. An employee sustained injuries to both his right and left hands while operating a Fletcher Roof Bolter placing bolt in a corner section. The drill steel became stuck and seperated from the extension chuck. The employee then grasped the drill steel with his right hand and began pulling it back into alignment with the chuck extension. His glove became entangled in the steel.

The employee allegedly slipped on a patch of ice while walking an alternate route to the maintenance ship. He was OK and no medical attention was needed. On 2/25, the employee claimed he was injured and placed on modified duty. This report is being filed even though there are serious doubts that this is work related. The case is being investigated by Workers Comp.

Employee was installing pin on strut and was using short 4' step ladder. While descending ladder and standing sideways on lower step, he spun to left and with his right foot stepped off ladder and onto a 20 lb. sledge hammer that was standing with handle in vertical position and slightly to the left of the ladder. The employee had been using the hammer and placed it on the ground.

Employee was welding from an awkward working position inside a feed chute. Employee took a scheduled break for lunch and when he was returning to the job site he felt a sharp pain in his hip and leg.

Struck by falling object

Injured member was installing impact rollers in the #1 tailpiece when the lid on the side of the tailpiece slid off and fell onto members right foot. Injured member was diagnosed with a Right Foot Contusion and was placed on restricted work activity.

Emp. was spotting bits on miner. He was using a raw hide brass hammer and bit wrench, as he was coming down to hit the bit wrench, the outside of his hand between the knuckles and wrist came in contact with another bit. Sent to ER. Positive X-Ray for fracture, placed on restrictions.

The employee was using a jack hammer to loosen up hardened clinker dust from around a belt conveyor.Exposure to the wet dust and rain water caused irritation to both knees.

Employee was in the process of disconnecting a clevis from the end of the boom of a mule and a material trailer when his hand got caught between the boom of the mule and the trailer causing a laceration to his left hand which required stitches.

Emp. changing bits. While leaning over and using hammer, felt pain in between shoulder blades. Severe pain began as he started to adjust the position of the roll fan. Tips of his fingers on his left hand were numb and tingling. Sent to company dr, placed on restrictions. Muscle relaxant, MRI and P.T. ordered.

Employee strained his back as he bent over to pick up an air chuck off the ground.

Fall to the walkway or working surface

Employee was cleaning tracks on a dozer. He had cleaned the tracks on one side and was walking around the back of the dozer behind the ripper and slipped when he stepped in the ripper hole. He fell and landed on his hand and wrist.

Struck against a moving object

EE was pushing rock screens with a 375 Komatsu dozer when he began to experience problems with the dozer. Mobile mech.came to diagnose the problem. EE raised the blade put the dozer in drive,the dozer moved forward 5' and then jerked hard twice.The EE continued to push screens up the pile when his neck stiffened up. EE was seen by the doctor and given RX meds.and work restrictions.
