This is being filed at the request of MSHA. On 3/31/10 torrential rains & flooding throughout S New Jersy overwhelmed the area surrounding the mine & ultimately engulfed the dredging pond. This regional flooding resulted in declaration of federal & state disaster areas. The mine dredging pond is not an impoundment. Co disputes any reportable accident. No impoundment, no failure.

Non employee was riding as a passenger in a privately owned vehicle on the access road approaching the main gate, the driver lost control of the vehicle, left the road, rolling the vehicle and damaging the vehicle, our fence and seepage well pump. There are no reported injuries to passenger or driver of the vehicle.
The East Finley Elevator was down for more than 30 minutes. The elevator was down for a fault in the contactor relay. There was no damage or entrapment.
The main upper hoist contactor coil failed causing the hoist to malfunction. The mine was evacuated at 12:20 a.m. and completed at 12:30 a.m. The faulty coil was replaced and the hoist was tested. The miners returned underground at 1:45 a.m.
A weekly examiner found a roof fall in a worked out panel. The fall was found in our M4-1st Left D Panel in entry #1, X-cut #5. It did not affect ventilation, passage of miners and is not in or adjacent to the examiners route of travel. The fall is 20' X 20' X 15' high and was supported with 5' fully grouted roof bolts. The area was developed on 2/4/2010.
CONTINUOUS MINER WAS LOADING GOB IN THE NO. 1 L/W FACE ON THE NO. 5 SECTION. THE MINER WAS APPROX. 3FT. FROM THE FACE LOADING GOB WHEN THE IGNITION OCCURRED. IGNITION LASTED APPROX. 20 SECONDS AND WAS PUT OUT WITH WASH DOWN HOSE AND 1 FIRE EXTINGUISHER. THE FIRE WAS LOCATED OFF SIDE MID FACE IN THE NO. 1 LONGWALL FACE.
A welding crew was working on the middle of three stacked screens, which had some rubber/urethane screens. The crew failed to mitigate the fire hazard posed by the hot work near/above flammable materials. During welding, slag or other hot debris fell to the lower screen deck, setting it on fire. The flames were out in less than 30 minutes. There were no injuries.
A roof fall measuring 95L X 18W X 8H occurred outby survey station 2908 in the No. 2 entry 3 breaks outby airshaft.
At 5:30 am pre-shift found a rock fall in #5 entry on 004 section(in active section) at spad #7147. Fall is approximately 40' L x 16' H x 20' W. Area timbered and dangered off. No injuries reported.
Roof fall was discovered by Fireboss in the 8N Bleeders betweeen #43 and #44 break in #1 Entry. Fall was approximately 30Lx18'Wx5'T. Travel way was rerouted and no ventilation was affectted.
Govenor overspeed switch on elevator tripped, which caused the elevator to stop. MSHA was notified via phone call. No injuries.
A fall was discovered on the C-4 Beltline, #4 Crosscut, 300' outby the feeder. No injuries resulted from the fall. No travelways, ventilation, or equipment was affected. The area of the fall was bolted with 8' Super Twist bolts and 12' post tension cable bolts.
Lost power (capacitor grounded) to elevator, evacuated and removed capacitor from curcuit
At approximately 7:41am the rupture vents on the #2 coal system opened due to an ignition of coal dust inside the coal grinding circuit. Prior to the ignition the coal weigh belt stalled causing a reduced amount of coal feed into the grinding mill which resulted an elevated temperature at the mill exit. There were no personnel injuries or equipment damages.
MSHA Inspector was in worked out area of mine with Mine Foreman, collecting dust samples when a roof fall occurred measuring 80' x 20' x 6' at Break #16 in the 002 South Mains off the 1st West Mains of the 002 Section.
A fall was discovered in the A Mains Section, #6 Heading, #32 Crosscut Intersection. The fall was 320' outby the feeder. The area was bolted with 6' Super Twist bolts and 12' post tension cable bolts. No people were injured as a result. the travelway, ventilation, and equipment were not affected.
While mechanic was cutting on roof bolter of continuous miner, sparks ignited methane that was escaping from floor. Flame was put out immediately with water hose from the continuous miner.
Unintentional roof fall approximately 3,000 ft outby face.
Govenor overspeed switch on elevator tripped, which caused the elevator to stop. MSHA was notified via phone call. No injuries.
A fire was discovered at 1:30 am in the control room of the existing Dan's Branch Load-out. The fire was extinguished by 2:15 am.
#2 hoist tripped on ""Supervision UHM Fault"". Maintenance Coordinator and two electricians were called out of the mine to inspect and reset the hoist. There were still two remaining escapeways throughout the incident.
Elevator would not operate due to a sticking door switch at 7:24AM and was repaired at 8:10AM. No one was trapped.
A small spon-comb activity was discovered with minor intermitted flames on highwall @ J-19 pit - Ramp #44. The spon-comb activity was not a hazard to workers and no pit activity was in progress. MSHA inspector was @ the site when flames were extinguished and under control. This report is submitted under protest.
Roof fall was at x-cut 30 in 1 west mains between no. 6 and No. 8 entries. Fall was 20'W x 100'L x 5' Thick. Set posts at all approaches to fall. Area was bolted with 5"" torque tension bolts with A3 resin. Area was mined in 10/09.
A grass fire was discovered at 4:10 pm. The fire was south of the mines Mid Pit. No structures, personnel or active mining areas were threatened by the fire. A strong wind from north to south kept the fire from being extinguished within 30 minutes, so at 4:40 pm the fire was reported to MSHA. The flames were extinguished at 5:56 pm.
A loader operator backed away from the dig face with a full bucket. When the operator moved forward, they articulated with bucket in the air. The loader began to rock side to side. While applying the brake, the operator went to put the bucket on the ground and the loader came over on its left side. The operator was wearing their seat belt at the time of the incident.
The header on the cage needed to have a flange welded on. This task must be done in the shop area. The doors to the cage can not be secured without the header in place. The header was repaired and replaced. The hoist was operational at 2:30 and employees returned to the mine after the test run was complete.
Flint Ridge Mine #2 experienced a non-injury unintentional roof fall on the MMU 001-0 section while retreat mining. The caved area on the Continuous Mining Machine was approximately 5 to 7 feet thick, 11 feet wide and 15 to 20 feet long. There was no damage.
One of the hoist motors for the 16' Hoist had a short in the windings.
Fuse blew on drive of Number 1 motor of Production Hoist. Had to replace fuse. Hoist was down for 1 hour and 15 minutes so mine had to be evacuated because Production Hoist is second escapeway.
EE got into end loader, put it in reverse, went back 8 to 10 feet and struck an unoccupied parked 2004 Ford F-150 pickup truck. EE hit the drivers side rear quarter panel and driver side back cab door.
During the process of lowering a CM cutter head and opossum belly car into the mine. The hoist being run on automatic failed to slow down adequately at the shaft bottom and struck the shaft bottom causing damage to the underside of the cage.
A non-injury roof fall occurred in the West Main 8 Crosscut 25' in the #3 Entry. The fall was 6 1/2 to 8' high, 20 to 25 ' long and 25' wide.
Discovered a roof fall in the GL4 panel in the intersection 1 break to the left of spad 2652 to the next outby intersection. Fall is approximately 4' thick x 20' wide x 100' long and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 9400' from the active section.
The #1 hoist speed control contactor stuck on control #3. An electrician was called to replace the entire switch with a new one. The miners were evacuated at 9:38am the evacuation was completer at 9:58am. The contactor relay switch was replaced and the hoist was tested. Miners returned u/g at 10:40am.
Discovered a roof fall in the GL4 panel in the intersection 1 break inby spad 2608. Fall is approximately 4' thick x 20' wide x 20' long and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 8900' from the active section.
Discovered a roof fall in the GL4 panel in the intersection 1 break outby spad 2662. Fall is approximately 4' thick x 20' wide x 20' long and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 9600' from the active section.
The #1 hoist speed control contractor stuck on control #3. An electrician was summoned and freed the switch. Miners were evacuated at 8:22am. Evacuation completed at 8:39am. The hoist was repaired and tested. Miners returned u/g at 9:05am.
An employee was stuck in the concentrator elevator for 1 hour 5 min. MSHA was notified. Repairman was contacted-no injury.
Discovered a roof fall in the GL4 panel in the crosscut to the left of spad 2715. Fall is approximately 4' thick x 19' wide x 30' long and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 8700' from the active section.
Roof fall at spad location 209 extending about 4 crosscuts outby in main west headings
While retrieving High Line and other mining supplies in the Northwest Mains section of our mine that is in the process of being sealed, 6 falls and a large rib roll have been discovered. The falls did not interupt the ventilation in any of the bleeders. They range in size anywhere from 15' long, 20' wide to 70' long, 20' wide. All are at or above anchorage.
Unintentional roof fall. Fall was 5' to 6' high 20' wide 33' long.
Hot water wash #1 was locked out by cross cleaning solution EE, #2 hot wash unit was being utilized by Benchmark EE's to wash equipment. Benchmark EE's took a 15 min break. When they returned, one of the Benchmark EE's turned on switch for unit #2, a propane gas ignition occurred blowing the four 3""x4' doors off the cabinet of #2 and displacing bent stack.
An unintentional roof fall has occurred on the number 10xc of the 2nd Main East in the number 3 entry. This fall is approximately 25 feet in length, 7 feet high and approximately 17.5 wide. The fall had a slip, with slikenbacks in the top.
A non injury unintentional roof fall occurred on the MMU No 1003 section.The roof fall occured during retreat mining. The roof fall was located in the #13 entry 2 crosscuts inby survey station # 8263. The roof fall entrapped the cont. miner and the shuttle car. The roof fall was approx. 4-8' thick X 18' wide X 18' long. MSHA was notified.
During the weekly inspection of the Infinity # 4 UG mine which is in non-producing status, a roof fall was discovered in the 1st left panel which is a worked out area. This fall was in the No. 2 entry intersection. The fall was above the roof bolts anchorage. This area has been dangered off with timbers and danger signs.
A non-injury roof fall occurred in the #4 right crosscut and extened to #6 entry of the last open crosscut on 001 mmu for approximately 100 feet long and 5 feet high. Survey station 5128 located in #2 entry of the last open crosscut.
The mine has experienced an unintentional roof fall at the 14xc of the 2nd main east between the Number 9 and 10 entry. This fall was approximately 70 feet long by 9.5 feet high and 17.5 wide. The fall was called in at 10:03 am to MSHA.
8 Haulage Elevator was down for more than 30 minutes when a door pulley failed to operate properly.
The mine has experienced an unintentional roof fall at XC-11 of the 2nd Main East in the #8 entry. The area had been dangered off prior to the area falling. This fall was called in to the district office at 7:07 am.
A gas lance was inside of Kiln 2 preheating, when the cooler fan was opened. This caused a draft within the kiln that extinguished the flame from the natural gas lance. Natural gas was released into the kiln, and an ignition caused the reaction. No persons were injured nor was there any exterior property damage.
The #8 elevator was reported down at 2:31pm and was out of service for a period of time exceeding 30 min. A bad wire that connects the car leveling switch to the controls in the penthouse was changed. The car was put back into service at 6:30pm. All necessary agencies were notified and no individuals were on the elevator at the time.
The mine discovered a roof fall on the Main South, entry #4 at crosscut #50 - #51. The fall measured 35' L x 18' W x 4' T. The area was developed in 3/2000 and supported with 4' resin bolts. The fall shall be cleaned up.
The elevator serving #6 portal was inoperable from 3:00 to 3:27 p.m. and again fron 3:35 to 4:00 p.m. due to failure of the stop (interlock) switch.
The #8 elevator was reported down at 3:53pm. The elevator was above the top landing. The car had faulted on counter with switch and car up over travel. The counter Weight buffer switch was cleaned and lubed and the car placed back into service @ 8:30pm. All necessary agencies were notified and no individuals were on the elevator at the time.
While making the weekly exam of worked out areas Mine Foreman and an MSHA Inspector encountered a roof fall at break #43 measuring app. 20' x 20' x 4' on 1st East Mains. It was phoned outside and the 800# was notified at app. 2:10 pm. The fall has been dangered off.
A roof fall occured int he No. 7 entry of the 2nd subpanel north off the 4th panel north. The fall is located at x-cut 40 spad 20 + 32 approx. 10' high x 20' wide x 30' long. The were no injuries, withesses or disruption to ventilation.
Making the weekly exam of worked out areas, mine foreman and MSHA Inspector discovered a fall was encountered at Break #41 of the 1st East Mains 20' x 10' x 4'
A roof fall was discovered just outby No. 7 entry spad location 178. The fall measured approximately 20'x50'x4-1/2' thick. No injuries resulted from this occurence.
First cut in # 4 entry. Developing barrier. Was finish and awaiting the bolter when the fall occured. Fall extended from the fresh cut and 16 ft outby pulling approx. 4 - 5 rows of roof bolts. No miners or equipment were damaged in fall.
A roof fall occured one break inby survey station number 3960 in the #8 entry. The fall was approximately 3600 feet from the working 003/004 mmu. the fall was approximately 19' wide x 65' long x 10' thick. This was found at 9:35 am on 3/18/2010. The fall was report to MSHA's hot line.
Rock fall in a panel (spad 10+50 #2 entry) off holt road submain. Fall size in #2 entry 60 ft by 18 ft by unknown height and across cut 2- right for 50 ft by 18 ft by unknown height. This is an L shape fall. Section moved from this area appr. 2 weeks ago and producing in B panel approx. 1700 ft away from fall area.
Roof fall in #4 entry approximately 150' inby fan house. 8' H x 30' L x 6' W.
A 30' x 25, x 4' thick roof fall occurred in the mains at break 18 in the #5 intake entry. The fall will not be cleaned up. The area was breakered and additional cribs set in area.
A roof fall was discoverd in the 2nd Northeast parallel spad #16+80 the #3 entry. The fall was 22ft long, 18ft wide and 8ft high.
A new skip was installed in the 14' shaft. After installing the skip a series of test were performed. The skip was released. The hoistman was bringing the empty skip to the mine level when it tripped. The skip was brought to the surface with no damage. An adjustment was made to prevent further tripping.
A roof fall was discovered in the #0 entry in the #280 intersection. The fall was 40' long x 18' wide x 6' high. No persons or equipment involved. The 3 sides were cribbed and it will not be cleaned up. This entry was already stopped.
A roof fall was found while making the return airways of the old 2 West sub-Mains near spad 8009 between #4 and #5 entries. The fall was approx. 20'W x 20'L x 5'H . The area was supported with 5' torque tension and was mined approx. 9/2008. The area has been dandered off and timbers have been set.
Pumping BASF Foam from the surface to the No. 4 Entry of No. E Panel Inby Spad 16503 to seal off entry. At approximately 10:46 AM, the foam seal started smoking and then started flaming from the right to the left side and the flames and fire went inby down the entry approximately 200'. It took several fire extinguishers, water and the foam generator to put the fire out.
While welding on the shear a spark rolled off the welding blanket and briefly ignited a methane pilot fire. The small flame extinguisher itself after a second. Bore bicarb was spread on the floor to prevent reocuurance.
Roof fall above ancorage occurred in the primary escape way at break 11 along #1 belt at spad #161. The fall was 6 to 7 feet high 19' wide and 22' long. No injuries, no equimpment involved, and ventilation was not impaired.
An unplanned roof fall was discovered by the weekly examiner in the #2 return entry, #3 x-cut it was 15'long by 12'wide by 7'high.
An unplanned roof fall has occurred alone the No.2 conveyor belt entry at break No. 56, survey spad No. 504 the fall measures 20' in lenght, 20' wide and 6' high, the fall does not impede travel or ventilation in this area. The fall will be cleaned up and resupported.
Had an unintentional roof fall 20X20X4 in White Oak Mains.
The roof in the last cut of No. 3 place fell, taking out the last row of permanent supports above the anchorage.
Rock fall occurred in return on 1 West Mains adjacent to the #3 belt conveyor at break #100 in the #1 entry. Fall measured approximately 20' wide x 50' long x 5' high.
Unintentional roof fall. Fall was 6'1/2"" to 7' high, 20 ' wide and 80' long. 20' inby spad #10747 track and belt entry.
At apporox. 2:15 a.m. the elevator began having problems while running in automatic. (would run in manual mode). Brought men outside - called vendor for repairs.
Mine Flooded at number 2 conveyor - unable to operate
During a lease inspection a fall was discovered in a gob area located at spad 2097. The fall occurred in an intersection. The fall is approximately 5 feet high x 19 feet wide x 30 feet long. It was discovered at approximately 10:30 AM. The fall has been dangered off and this area is going to be sealed.
The #8 elevator was out of service for a period time exceeding 15 minutes due to a fault on the motor controller drive package. Two individuals were on the elevator at the time. The motor controller drive package was changed and the elevator was placed back into serive at 5:52 PM. All necessary agencies were notified.
An unintentional roof fall of the mine was discovered in the #3 entry, #107 X-cut in the North West Mains. The fall was approx. 10 to 15 feet high, 20' wide and 55' long.
Elevator outage, no injuries, no emergency, no entrapment. Discovered CPI transformer primary side fuse was corroded. Clean it and the elevator worked. Out of service greater than 30 minutes.
The results of the X-ray testing of the ropes on 8 Hoist showed one tail rope with metal loss in excess of 10%. The hoist was locked out and the tail rope was changed.
A roof fall measuring 19' wide x 16' high x 30' long. Occurred on the Jenny Branch Section near spad No. 5094.
Roof fall occurred appx. 2200' outby 2 section in the #3 entry at spad #4480. The roof fall was appx. 20'x50'x4'. The fall was investigated by MSHA inspector.
The elevator was taken out of service due to a malfunction with the Bodie brake. The elevator was out of service from 3:20 pm until 4:10 pm.
An unintentional roof fall was discovered by MSHA inspector and mine examiner. This fall is one break outyby survey spad # 1584. The fall measured approximately 20' wide X 35' long X 10 to 12' high. No injuries or escapeways blocked.
#3 hoist tripped on ""Drive Converter A"" fault. Maintenance coordinator and electrician were called out of the mine to inspect the hoist and reset the hoist. There were still two remaining escapeways throughout the incident.
During exploration/rehabilitation pre-existing fall was observed @ spad 208 for 2 break inby. Per instructions from local MSHA office it was called to them.
A roof fall was discovered in the Main West # 2 return aircourses, at # 83 crosscut, between # 2 and # 3 entries. The fall measures approximately 35' long, 18' wide, and 8' thick.
#1 entry 003 section at spad 31450 fall was 20""w x 20'l X 8 "" high fall was checked by MSHA and will not be cleaned up area was dangered off.
During exploration/rehabilitation pre-existing fall was observed @ spad 140 runs 2 break inby. Per instructions from local MSHA office it was called to them.
During exploration/rehabilitation pre-existing fall was observed @ spad 90 in mains. Per instructions from local MSHA office it was called to them.
During exploration/rehabilitation pre-existing fall was observed @ spad 226-227-228 (2 breaks only). Per instructions from local MSHA office it was called to them.
A fall of roof occurred in the South West Sub Main, # 26 cross cut, # 6 entry, @ 15+70. The fall was approximately 20 feet in length x 20 feet in width x 12 feet thick. The area was supported by 6 foot fully grouted roof bolts.
A roof fall was discovered in the number 4 entry adjacent to 2C belt line at break 18 near spad 25493. Estimated size of the roof fall is 40 foot long x 18 foot wide and 7 to 8 foot high. This area was mined 3-12-2009. No injuries were reported as a result of the roof fall.
During exploration/rehabilitation pre-existing fall was observed @ spad 126 in mains. Per instructions from local MSHA office it was called to them.
During exploration/rehabilitation pre-existing fall was observed @ spad 241. Per instructions from local MSHA office it was called to them.
During exploration/rehabilitation pre-existing fall was observed @ spad 65 in mains. Per instructions from local MSHA office it was called to them.
During exploration/rehabilitation pre-existing fall was observed @ spad 104-105 inby. Per instructions from local MSHA office it was called to them.
A roof fall occured on the 002-0 mmu during retreat mining. The continuous miner was taking the left cut in the coal pillar in the #3 entry when the mine roof fell on the continuous mining machine. This was 150' inby survey station # 2480. The fall was approximately 16' wide, 20' long, and height is unknown.
The #2 coal weigh belt stalled due to a drive sprocket catching on a guard. During the period of time the coal mill was starved of coal feed and explosion occurred inside of the #2 coal mill sytem. Both rupture vents opened to vent pressure. No equipment damages or personnel injuries occurred.
While traveling our return air course to EP's 1 state inspector and 1 msha inspector found a roof fall located left of spad 338 in the old panel at an intersection mined around Nov. 2003. Area posted off and danger sign hung.
An unplanned roof fall has occured from cross cut #26 to #27 in the # 7 entry, in the 3rd Southeast Submain off the 1st Southwest Submain. The area was fractured stack rock, bolted with 8' & 10' point anchor roof bolts. The fall will not be cleaned up, and MMU 002-0 has been moved from the area.
A roof fall was discovered on this date. The fall was located on B-4 Mains in the #2 entry between break 4 and 5. The fall measured 18' by 20' and was 6' in thickness. The area was previously supported with 5'Hy-Tech bolts and 12' cable bolts. Ventilation was not affected.
At approximatley 1:05pm a face ignition occurred in the face of the No. 8 entry on the No. 1 section approximately 250 ft inby spad 5570. While the continuous miner was operating in the face. No injuries occurred.
A roof fall was discovered adjacent to spad location 286 between the No. 1 and No. 2 entries. The fall measured approximately 70' long x 20' wide x 4-1/2' thick. No injuries resulted from this occurrence.
A fall occurred on this date. The fall measured 20' by 20' and was 8' in thickness. The fall was located on 6-North mains in the #3 entry in the #6 crosscut. The area was previously supported with 5' Hy-Tech Bolts with supplemental 8' cable bolts. Persons inby the fall were moved outby the area until the secondary escapeway was rerouted.
The slope hoist had an electrical malfunction it was found while doing pre shift.
Roof fall 20' X 30' X 8' at spad # 3565. 3500' out by an active section. No injuries, equipment involved or passage blocked.
Roof fall approximately 18'w X 25'l X 41/2-5'h on 1A belt near brk #6
At approximately 12:37 pm, on 03/7/2010, the shift foreman reported a waste hopper being used by contractors was found to be smoking and on fire. The cause of ignition is believed to be lime that reacted with the rain falling at the time.
8 Haulage elevator was down for more than 30 minutes due to an electrical problem.
Crosscut and partial intersections between the L2 and L3 entries one crosscut outby the LOX in the 3 Butt off 2 Right working section experienced a roof fall. The L3 entry was supported with 6' x 3/4"" fully grouted resin bolts. The crosscut and L2 entry supported with 4'x 5/8"" FG resin. The L3 entry had bed separation at 40' and water running out test holes & vertical cracks.
The east finley elevator was down for more than 30 minutes due to a blown buss feed circuit breaker. There was no entrapment.
8 Haulage elevator had a door switch malfunction and the elevator was down for 45 minutes.
The roof fall occurred in an outby area approx 3,000 feet outby section in an abandoned area of the mine. The fall will not be cleaned up.
A roof fall occurred on the 1st West Submain,spad # 14+00, entry #6. The fall measured 19ft W x 19ft L x 6ft H.
On 3/5/2010, an unplanned roof fall occured at cross cut #24 in #5 entry of the 3rd S.E. Sub Main, off the 1st S.W. Sub main. The fall measured 17'Wide X 18' Long, X 11' high. The area was bolted with 8' & 10' point anchor roof bolts. The fall area was fractured stack rock.
A roof fall occured in the 2nd Northeast Parallel xc- 112 spad #78+40 between #7-8 entries.
A roof fall occured in the 2nd Northeast Parallel #8 entry xc-111 spad #77+ 70.
A fire occurred on a Letourneau 2350 loader. The fire was not extinguished within 30 minutes and was called into MSHA. No injuries associated with the fire.
An unintentional roof fall was discovered at the Page Service Shaft bottom in the No 6 entry in the intersection at survey No. 100. The fall measured 40 ft long x 20 ft wide x 7 ft high. No one was injured, the ventilation was not disturbed, and no equipment was caught. This fall will not be cleaned up.
A unplanned roof fall occurred in the #2 entry at spad #4424 approx 210' from the active face. Fall measured 28'Lx20'Wx6 1/2"" thick. Area was previously supported with 5' Torque Tension Roof bolts. area was developed on 3/1/2010. Ventilation and passage of personnal was not affected. Federal and state agencies were notified immediatly.
While mining in the inby 3x4 crosscut everyone heard and felt a roof fall. Upon investigating it the Foreman found a fall in #4 Entry of the section adjacent to the fail in #4 Entry. This fall was 20' wide and 55' long. It came above the 4' resin bolts and also took cable bolts out in the intersection. Roof was very thinly laminated and broke up.
There was rock fall on 1 left panel about 7' thick 20' long 18' wide in NO. #6 acress from spad 6689 in #5 entry it was found on weeky exam.
Roof Fall found on 3/3/10 on 2 east panel@bk56 between 3 and 4 x-cut it was 20'w x40' lx 6'high investigated by MSHA dangered off and will not be cleaned up,didn't affect any ventillation or travelways.
Found Roof Fall on weekly bleeder examination that was blocking the bleeder. One break inby spad number 5053, Fall was 175 feet long, 20 feet wide, 9 feet high.
An unintentional roof fall occurred on 3/3/10. The fall occurred on old #2 unit at spad #2241. No injuries occurred from the fall (length 60' to 80', width 20', and thickness - above anchorage) (support 6'3/4"" resin bolt).

A wire supplying power to the upper limit switch was compromised by a short circuit. Miners were evacuated at 7:50 p.m. Evacuation completed at 8:04 p.m. The encoder and track limit switch was replaced. The hoist was tested and the morning shift miners entered the mine at 2:10 a.m.

A roof fall occurred on the 1st West Sub-mains @ spad # 4 + 20, cross cut #11, between #4 & #5 entries. The fall measured 20'L X 18.6'W X 6'+ H.
15-W Section long wall face ( Please note that this 7000-01 form is being filed for reporting compliance purposes only. As we do not agree that an inundation occurred) De-gashole 16 WHD - H- 6 was intersected and by following the vent plan the hole was grouted and mining resumed.

A roof fall occured in the #11 seam in the 2 Main North at crosscut #11 starting in the intersection of #1 entry and extending to the edge of the intersection of #2 entry. The fall measured approximately 80'x18'x10'-12'. The area was supported with 30"" fully grouted bolts. A slip was determined to be the cause of the fall.
The hoist operator had droped the Barney car into the bottom. The right rear wheel had came off and caused the car to wreck. When discovery of the wheel missing, all employees were notified to evacuate the mine. The hoist was our secondary mechanical means of escape.

The fall measures 20 ft in length, 20 ft wide and 3/6 ft thick, the fall will not be cleaned up, all areas into the fall will be supported.
Employee was in dump truck waiting to be loaded with bulk salt from under 150 ton bin to transfer it to the stockpile pad on Cargill Deicing property. He was being loaded with salt when the brackets from the bin gave way and landed on the cab of his truck pinning him inside.
Welder/Repairman checking operation of Firebal grease pump after rebuild. Applied air to 50 psi pump would not actuate but air was escapiing around housing. Repairman was feeling area where air was escaping when pump engaged slicing off end of right thumb.
Raw Material Handler was performing equipment check on a Komatsu loader and was checking belt tension in engine area and did not realize engine was still running when his middle and ring finger on his left hand became caught in V-belt. The ends on both finger were amputated just below the finger nails.
The EE was closing coupling covers on a dragline swingcase. He bent down to close one when he became dizzy and fell from the case, bumping his head and separating his right shoulder. The ER doctor determined his dizziness was from sinusitis. CT scans were negative. He had surgery to repair his right shoulder. *A 5% whole person impairment rating was issued.
The employee attempted to catch a Duke that was falling over. The duke pinched his finger against the face conveyor pan.
EE was lifting two tools out of a crate and while doing so felt pain in his abdomen. A bump developed and was diagnosed as an umbilical hernia.
The outside contractor was onsite working on the rock drill. A piece of drill steel started to fall out of the carousel. The contractor grabbed it to push it back in place as the operator engaged the grabbers. The carousel turned pinching the contractors little finger.
Employee was picking up belt structure when he felt pain in back.

Construction crew was dismantling a fan housing when a piece of duct work approximately 8'x 12' x 8' fell on an employee.
EE was assisting in changing out screen decks. They were placing this deck into the upper slot when the ee injured left shoulder.
Was pulling cable and stepped in hole twisting leg & hip.
Employee was walking when he stumbled on a rock and fell. He grabbed for the bumper on a truck and caught his right arm. As he fell with his right arm on the bumper, it caused him to dislocate his right shoulder. Reopened see DCN# 220101200018.EE is out having surgery.
Operator was pulling coal from pit floor when the loader bucket came into contact with solid ground causing the operator to lunge forward. Operator complained of neck and back pain and was transported to the hospital for doctor evaluation.
Individual was picking up a bag of ready mix when he felt a sharp pain in his lower back. That caused him to let go of the bag and feeling sharp pain in his back. He was taken to the office were he was picked up and taken to the local Emergency room for observation. It was discovered that the individual had had an earlier back injury in a car accident in 2005.

EE was operating a front end loader and was picking up a rock with the end loader the rock fell out of the bucket and caused the rear of the loader to fall down. This jarred the EE causing him to have pain in his lower back.
Employee got hit with a piece of rock that scaled between the roof bolt plate and the right rib. The rock fell against his ankle. The employee worked the remainder of the shift. Got up this morning with a lot of swelling. Went and was diagnosed with a sprain. He is on crutches and will probably be off a few days.
While bolting on the left side of the miner in 14I, drill steel fell out of the roof and struck the employee on the left index finger. 5 sutures to close.
Employee was in beside a Ram Car, in a sump, washing the machine off. He finished what he was doing, shut the water of and threw the water hose and valve up on the bank. The handle to the shut off hit just right and caused it to open. When this happened it sprang back under pressure and struck the person in the mouth. He's off work, lost 6 teeth.
A piece of roof rock approximately 1' x 1.5' x 3.5"" thick slid off of a piece of ventilation tubing and struck employee on left shoulder.
Employee squatted down to lift a prop and felt a sharp pain in low back when he stood up. EMPLOYEE LOST TIME FOR ONE DAY ON JUNE 3, 2010 DUE TO HAVING A PROCEDURE DONE.

Employee was making up roof bolts at the Main West 1, 64 crosscut, between # 5 and 6 entries (outby), when draw rock fell from between pattern bolts striking employee. The rock measured approximately 7' long, 2.5' wide and 6"" to 8"" thick. Employee sustained a fractured left forearm, and a torn right patella tendon.
Employee strained his middle part of his back as he was greasing a SR-1 mantrip.
Injured employee was bending a roof bolt when he slipped and strained his lower back.
EE was removing an old dust cover inside the chute under #3 dryer from the 5th floor. Another member was on the 6th floor,EE told him to reach him a torch down thru the chute when he did this he heard EE ignite the torch and a cloud of dust came up onto the 6th floor. EE received burns to his head.
Employee alleges that he injured his back at work on 3/28/2010. He did not follow up with an accident report as required by employer. He is now off work with alleged back injury. No detailed information is available. This is being investigated to verify if this is a work related injury.

Crew was spot bolting outby face in #2 entry. EE was operating the helper side of a Fletcher RRII. He swung the drill out to spot a bolt between the outside bolt and the rib, when loose rock fell onto his right foot. X-rays revealed fractures to ankle. Surgery on 3/28/10. A plate and screws were inserted. Rock measrued 5""X12""WX30""L. Wearing tiger-tip metatarsal boots.
IE was struck on the left little finger by a hammer while making a belt splice.
Employee was lifting a drum and the drum slipped catching his finger between the drum and a fire extinguisher box
Employee stated he was operating the Cont. Miner and had just set the machine over in the cut and was waiting on a car to come in to load. He stepped up beside the machine and looked back to watch for the car coming in, when a piece of slate slid off the top on the machine and struck his foot. He finished his shift, but was taken off work on the 29th of March.
Employee was attempting to remove buildup in preheater. He was using a piece of pipe about 3 ft and smashed his finger in the process.
Was getting on truck,slipped foot went backwards.
At end of shift, while tramming Cat. 992K to parking lot, EE drove through a dip in the road, causing a quick jerk to his back. Diagnosis is pending an MRI.
Employee was attempting to drive in a conveyor hinge pin with a 20lb sledge hammer. The hammer glanced off the pin an hit the employee in the left ankle area.
Was prying metal with pry bar, when it slipped and jarred his body.

Employee stated while bending a 6' roof bolt to get it in the hole, he pulled something (muscle) and the pain went into his back also. He worked the rest of the shift and stated there was even discomfort pulling drill augers out of the holes. He was treated by a family dr. and taken off work.
Was checking miner head to see what was making gear case hot. Operator started miner to raise head, hit wrong switch on remote box causing miner to slew over hitting right foot.
When climbing back on dozer after filling up with hydraulic oil, operator slipped on the tracks and fell onto the dozer/ground. Conditions were wet and muddy. Leg and ankle were hurt during the fall. Was treated and sent to hospital for X-rays.
Employee felt pain in left forearm and wrist after loading bolts into the magazine of roof bolter. Employee did not seek medical attention until 5/25/2010.
Employee was moving flypads in #3 entry. Was to move the fly-pads straight up entry but went in the cross-cut where the shuttle car was running. The shuttle car ran over the employee's left foot.
EE was cleaning screens in a PEP Screen.EE was finished cleaning and was climbing out of screen when his pant leg hooked on a piece of metal in the discharge chute, pulling his groin muscle.
Employee was walking to his toolbox and felt his knee pop. EMPLOYEE DID NOT START LOSING TIME UNTIL 4/7/2010 DUE TO INCREASED PAIN.
Employee was stepping from a vehicle, did not notice drop in floor level. As his feet engaged the floor, he felt pain in his back. He was released to work but the condition continued to worsen. Employee received surgery on May 10, 2010, causing this to be a lost time. Medical professionals had to advise whether this was an illness or injury before reporting.
While alligining a cylinder on the shield hauler, cylinder slipped mashing soft tissue on left middle finger off.
The injured party was standing on the jaw deck pulling pipe and preparing to drop a section of the pipe to the ground. The section of the pipe that was to be dropped rolled and crushed the ankle against the mast.
Putting on downspout bucket on end of auger, truck operator moved auger too quickly, pinched end of ring finger.
On March 23rd employee felt tightness in both shoulders. On March 25th he was bolting his second place of the day, while straightening out an 8ft fully grouted bolt, he felt a pop and piercing pain in his left shoulder. It was determined on 10-4-2010 that the incident above was a work related injury.
While in the process of making a hydraulic hose with a Hydraulic hose crimping machine, Injury was struck in the neck with a piece of the ring from the hydraulic crimping machine. He was taken off work for 2 days and also received sutures.
The employee was shoveling a belt when he caught two fingers of his right hand on a lump of coal pulling his fingers back. The employee continued to work until 4-15-2010, when his finger started to hurt him worse.

EE reported to the nightshift foreman that he had stepped off the ladder of a 992G and stepped on a rock causing him to twist his hip. He was asked if he needed to go to the doctor. He declined and said that he had an appointment with a specialist on Saturday. He was informed to get a release from the doctor before retruning to work.
Employee was working on changing pins and bushings in a low trac. He was picking up timbers to block the low trac up when he felt a pain in his lower back. He continued working until he started missing on April 22nd making this a reportable injury.
While removing asphalt on the Freeport-McMoRan Road, the employee placed his left hand on a large piece of asphalt, while another employee tossed a piece of asphalt that struck his left hand.

The employee stated that he hit his head on a cable bolt, twisting his neck. The employee worked his normally scheduled shift until his surgery. The employee began missing work on 5/14/2010.
An employee allegedly strained his back as he was climbing into 5551 service truck. He was evaluated by a Physician on 3-24-10, taken off from work, resulting in lost time.
This form is filled out due to a Part 50 audit. The company does not agree with the decision. He was under doctor's care when incident occurred. To the best of my knowledge. Employee claims to have walked into the TRS and jammed his neck.
Employee was standing outside a garage door to the warehouse, when he took two steps backward and stepped on a wooden pallet and fell, fracturing his ankle.
While operating a loader, hit a bump in the road, felt severe pain in lower middle of back.

The injured employee stated that he was installing a spill rubber at the No.2 head drive. He was between the head drive and backboard in a small area. He tried to turn around and he twisted his back.
Employee was attempting to drive his vehicle around a parked Shuttle Car that had been left out in the middle of an intersection. There was gob on the opposite corner of him and when his front tires hit this gob, it caused the vehicle to slide sideways into the car. The victim was pinned between the dump end corner of the car and the transmission housing.
EMPLOYEE WAS STRETCH WRAPPING PALLETS USING THE WRAPPING MACHINE. THE STRETCH WRAP RAN OUT, HE WENT TO REPLACE NEW ROLL, AND AS HE LIFTED A NEW STRETCH WRAP ROLL, HE FELT A SHARP PAIN IN HIS LOWER BACK.

Employee exiting ""change house"" missed the bottom stair step, his ankle rolled under him and he broke his right fibula.
EE reported on 3/29/10 that on 3/20/10 while getting on the dozer he pulled a muscle on his back. EE stated that he did not report the accident at the time because he was going to treat the injury his self. On 3/22/10 he went to Cline Chiropractic for treatment.
Employee was walking across the 001 section in the #5 entry when he stepped in a hole and hurt his right ankle. Employee works through Superior Elite Coal Services.
EE lost his balance when dismounting a screen and when he grabbed a pole, he twisted at the same time and tore the labrum in his left shoulder.
EE was drilling holes in top of fender to move auto greaser, when drill bit broke through, he lost his footing and fell from top of fender to the ground. EE was taken to ER by ambulance and was diagnosed with a heel fracture.
EE reached up to grab rail to climb into a straight truck when he felt his left shoulder pull out of socket. He was taken to hospital to check it out. He was given a brace for now and may require surgery in the future.

A customer drove his pickup truck onto scale to get an empty weight. He got out of the cab and walked around the pickup instead of walking to the end of the scale as the signs say to do. He tried stepping over the scale side rail, it was snowing and he slipped and fell on the rail.
Walking from one piece of equipment towards another, kicked track and tripped and fell, scraping nose from his glasses.
Employee stated that as he was moving miner cable out of roadway that he felt a pop in his back & felt pain. Diagnosed as a strain. Company notified on April 1,2010.
Went in early on this day. Walking across the section to see what areas needed cleaning. Walked in the crosscut between #2 & #3 entries, he stepped across a rut in the floor made by a scoop tire and hyper-extended right knee. Continued working and did not seek medical attention until 3/26/10 on scheduled day off. Saw dr. due to pain, 1st day missed was 3/29/10.
While walking out of the lab on the firing floor employee stated he missed a step and fell against a wooden table. He was diagnosed with a rotary cuff tear and had surgery on 06/02/2010. June 2, 2010 was his first day missed and worked full duty until surgery.
Employee opened the door of his D11 dozer and stepped out onto the deck, he straddled the door to retrieve an air hose that was attached to the cab. The hose had been used for repairs. He went to throw the hose off and lost his grip and fell. He suffered a broken arm.
Employee was leaving the building after line-up tailgate meeting. As she exited the building, she stepped on a small rock, twisting her left ankle, causing her to fall down and scrape her right knee. Employee was able to perform normal work duties until 4/26/2010 when she had surgery on her right knee. Employee's incident did not become a lost-time incident until 4/27/2010.
Employee was attempting to straighten a 50lb packer that was sitting on a pallet that had shifted. The employee jerked on the leg of the packer and pulled a muscle in his mid back.
Associate noticed that his back became sore sometime during the afternoon as he operated a haul truck. He does not recall any particular event causing the pain. He left the location to go to his doctor thinking it was from a pre-existing medical condition. The doctor determined that he had a bruised rib that could have been caused by operating the haul truck.
Employee was pulling on a 60"" top belt idler when he felt pain in his lower back.
Employee indicates that she was jostled while driving articulating truck causing injury to her neck and back. We were made aware of her injury when we received a fax from her attorney on 4/13/10. She did not inform her supervisor that she was injured.
While in the process of installing permanent roof support a 6x16 plate was not uniform with bolt pattern, employee placed his right hand on plate to straighten plate, his right index finger was caught between plate and mine roof, causing a fracture and a laceration to the right index finger.
EE was pressure washing a screen when he slipped and hit his knee on the screen rail. The deck was wet and muddy from the use of the pressure washer.
Employee & coworker were attempting to move a set of wooden steps in order to scale some loose roof. While lifting the steps, employee stated that his bicep popped & he immediately felt pain.
Backing versa track out of x-cut. Ran over 2"" pipe. Pipe struck man and pushed him forward. Punctured him in buttock area. Surgery required.
While placing a pull cable on a D11 Dozer, miner said he felt a pain in the upper inside of right leg (groin area). Miner did not seek treatment until 3/18/10 in ER, for soreness and discoloration of affected area.
Employee was exiting a 777 rock truck. He had 3 points of contact, backing down the ladder, with nothing in his hands. As his foot got to the second step from the bottom, he slipped and fell to the ground.
Employee was dismounting 735 hauler, missed the last step resulting in a fall to the ground.
On 3/17/2010 the employee was in the process of traveling through a personnel door in a stopping when he struck his right elbow against the door frame and felt pain in right shoulder. The employee has continued to work at his regular job at full capacity until 5/17/2010 when he started missing work due to his injury.
While in the process of installing a 6 foot resin bolt, the 6x16 plate was not uniform with bolt pattern, employee placed right hand on plate to straighten plate, right index finger was caught between plate and mine roof, causing a laceration to finger.
Employee was working on an axel off of a backhoe. He had taken the axel off & set it on jacks. The employee checked the axel to make sure it was stable on the jacks. When the employee stood up the axel fell off of the jacks, landing on his hand.
EE stepped on a piece of loose material causing his left ankle to turn causing a bad sprain.
Medical condition unbeknownst to the EE (heart-related issue) EE was standing against a wall in Finish Mill Bldg, was not involved in any job activitiy at time of event - felt faint & blacked out falling face forward to floor. EE was not working in normal job nor full-duty capacity at time of this event.
He was attempting to recover a drill steel from the roof when the steel suddenly fell striking him on the back of the right hand causing a contusion to the back of the right hand.
Two employees were trying to untangle a 200' length of 1 5/8"" wire cable. Employee injured back while pulling cable.
Employee was roof bolting when a piece of roof rock measuring 10.5"" x 5"" x 2"" thick fell and struck his canopy and bounced into his right shoulder.
While assisting the mechanic make a splice on the miner cable, EE cut his left wrist with a belt knife, requiring 4 stitches.
While changing out a Bad order return idler on #6 rod mill feed belt, employee dismounted a man basket and rolled his right ankle.
Employee had parked a Cat 938 at the service dock for service. He decided to clean his front glass & stepped out onto the top of the wheel fender. As he finished, he grabbed the top of the light housing on the fender & it broke. He jumped to the ground when he lost his balance. Originally a knee sprain. Did not miss work until 7/7/10 for arthoscopic surgery
Employee was transporting parts to the rais bore project after arriving the employee was attempting to release the binders with a bar. The binders released and the bar hit the employee in the nose.
Injured was taking block walls down when top block fell and struck left hand.
Employee was doing the work place inspection, when he noticed the area was wet with debris. Employee turned around to leave when he lost his footing causing the employee to fall through the mill hole 40'.
Employee stated that while swinging a sledge hammer, he lost his balance, slipped and injured his back. (Started missing work on 3/28/10).
While trying to straighten a bent bolt that was being installed, the bolt spun in the operator's hand causing a strain to his abdomen. Employee started losing time on 03/24/2010.
This 7000-1 is being submitted under protest in order to abate citation 8032396 issued on 11/4/10. The ee alleges an injury while on mine property while carrying K wall panels and experiencing back pain. As a result of a part 50 audit MSHA has deemed this a reportable injury.
Employee stated that he was working on the feeder when a ram car trammed near him. The oil compartment caught his foot between the compartment and the coal floor. His FLWD did not occur until 03/17/2010 due to surgery.
On March 16, an EHS Specialist suffered a back strain while ascending a ladder during a MSHA inspection. This incident has been classified as a recordable injury based on the medical determination that his work activity aggravated a pre-existing condition, incident was deemed work related on 4/23/10. Injury-RDI. Injury-Lost workday incident.
EE slipped on a miner cable while hanging a curtain, which caused him to fall on his left wrist.

While working on the shipping docks forklift forks, the fork rack came off hitting him in the head and shoulders.
While in the process of installing a new belt head to extend the refuse belts, the forks of a IT28 Catapillar forklift fell off when they were raised up, stricking EE and causing a concussion.

The employee was examing the belt when he tripped on the 6"" water line causing him to hurt his lower back. The employee continued to work with pain and continued to see the doctor for it until it got to hurting him alot worse, then came out on 4-13-2010 due to the pain then the doctor took him off work.
While operating a continuous miner, he was struck by a rock that fell from the rib on both shins. The rock was 10 feet x 3 feet x 8 inches thick. He sustained contusions and lacerations to both shins and a left achilles tendon strain.
Employee slipped and fell while walking in mud and water.
Operator was pushing overburden on a drill bench over a spoil bank. The operator was backing the equipment up, when it struck a small rock ledge, causing a jarring motion to the equipment. The operator complained of minor discomfort and finished out the shift.

Employee stated that while moving a loader bucket pin, he injured his back.
Worker was removing a leg brace on a conveyor, was standing on a concrete block went to step off block and came down on his left ankle wrong and twisting it.
EE was working on the original side of the MCC secondary crusher roof repairing water leaks around the air conditioner ducts. At approx 12:45 he claims as he was cleaning around the duct with a vacuum, he stood up and his right knee popped resulting in the alleged injury.
Employee lifted a 3"" water pump to set it up to pump water and when he turned he injured his back.
EE was installing roof bolts when a piece of draw rock fell striking the EE on the head and upper torso.
Employee was using a chain hoist to pull on the miner chain when the hoist came loose striking the employee on his right leg resulting in a fracture to the Fibula bone.
Employee was using a set of come-a-long to set a motor in place, when the hook slipped and caught left hand.

Employee was dismounting the steps from the launch deck of the highwall miner unit. His right foot slipped from the last step causing his left foot to get momentarily caught on the step just above it. He felt pain in his left ankle. He was diagnosed with a tendon strain of the left ankle.
As employee was trying to lower the boom on the roof bolter, he hit the wrong lever and closed the steel guide catching his finger between the guide and steel. He suffered a fractured left middle finger. He began missing work due to the injury on 3/15/2010.
EE tripped on a water hose and landed on his right hand causing a fracture to his right radius.
EE dropped wooden crib block and it struck his right middle finger causing a fracture. The EE began losing time as of 4/29/10.
EE was welding pipes and felt pain in his low back.
EE's had the power center in a scoop bucket and were repostioning the power center.The ram of the scoop got caught on the bumper of the power center. The injured EE held a crib block between the ram and the power center to push it loose. The EE's hand slipped off the top of the block and was caught between the block and the transformer. Laceration-Fracture - finger.

A truck driver was walking into the East Ramp (access 1) from the main haulage. He saw an LHD unit coming out of a heading toward him. The driver then turned to walk back to his truck. When he turned his body, his left foot was firmly planted on the ground and he felt popping in his ankle. By turning quickly his left ankle and fibula broke.
While watching the dump area where scoop dump coal, the person injured did not turn off belt and tried to remove a piece of rock from feeder.
Employee was working on the roof of an office building. Taat roof was approx. 12' from the ground. A hi-power electrical line approx. 15' above the roof ran parrallel to the bldg. He was installing a piece of metal trim when he was hit by hi voltage. His clothing caught fire causing severe burns.
Employee stated he was cleaning windows on truck - stepped on lower step mounted to the fuel tank causing it to break. He alleges his right leg hit the ground while his left foot remained on step.
Employee stated that he was roof bolting from the miner in 4B 8North section when he bent over to look at a hose connection on the ATRS when a piece of stone approx. 10"" x 6"" x 1/2"" fell striking him on the left shoulder. This resulted in a non displaced fracture of the scapula.
Employee was performing maintenance on product Pb slurry filter press chain drive assemblies when employees left hand made contact with a bracket causing a minor puncture to palm.
A cylinder was lodged in the welding cart. In trying to move/dislodge cylinder against advice, miner injured back muscle.
Employee stated that he was exiting his Ramcar when his left foot got hung/caught in the car controls. This caused him to slip and spread his legs apart (like the splits), straining his hip and also a muscle in his groin area. He has continued to work but was taken off by his doctor starting 9/25/2010.
As EE was helping the mm's place the hazelton in the hole, he was splashed underneath his left armpit with slurry. EE was treated at medical and returned to work. EE developed 2nd degree burns after second day.
Employee was welding on a excavator bucket. Employee reached over to pick up a shank and felt a twitch in his lower back. Employee did not miss any work until 6/11/2010 and had surgery on 6/14/2010 to repair disc L5/S1.
Maintenance employee was underground and stopped the tractor to visit the out house. He stepped down from the tractor and landed on a rock. He heared a pop and felt immediate pain. He was transported to Hospital and diagnosed with a Severe sprain to the Left Ankle
Employee and co-worker were lifting a belt clamp that weighed approximately 60lbs. when the employee felt pain in his lower back.
The EE had gathered a five gallon bucket of stalite material and emptied a five gallon bucket of coal. Minutes later he was walking to the EE's break room and felt a pain to his chest area and he developed a cold sweat. He thought he was having a heart attack.
EE was installing roof bolts when his glove (leather) was caught between the head (drill) and pinner steel dislocating the thumb on his right hand.
Employee stated: He was standing at the side tram controls of his roof bolter, when he was struck by a piece of roof rock. The rock pushed him down and his lower left leg was caught beneath the rock - fracture to left ankle. His first scheduled work day missed is 3/12/2010.
He was using a 3/4"" ratchet and socket to break bolts free. When the bolt broke free he felt pain in his left shoulde.r Diagnosed with a strain to the left shoulder.
EE was building up a lift with rock and soil bounced in dozer, back stain. On 3/10/2010, EE said his back was hurting, never missed any work till 10/29/10.
Employee stated he was walking to the cage from the hole carrying two channel samples. He hit his head on the mine roof. Claim to fell pain in his neck and shoulders.
While bending over to pick up a socket, Injury stood back up and struck his back on a metal plate causing a contusion to that area. He continued to work and was seen at the Doctor's Office on March 17th and was taken off work at that time.
On March 10, 2010 EE was rock dusting w/scoop and duster. To get out of the dust, he backed-up the scoop and ran over a rock resulting in pain in his lower back. He completed his shift and went to have it checked. He was released and returned to work on his next scheduled shift. He returned to Dr on 4/19/10 due to continued pain & was placed off work. An MRI was performed.
The employee stepped into a hole causing him to twist his ankle on 3/10/2010. He started missing work due to a sprain injury to his left ankle on March 22, 2010 from the event.
EE was stepping into filter house when his foot caught the bottom part of the door causing him to fall forward. He struck the left side of his face and ear on the angle iron that holds the filters in place, causing a laceration to the face and ear. He had surgery on his left ear on March 12, 2010. His first full day of lost time is March 11, 2010.
EE was loading scoop with waterline and heard a popping sound .
EE slipped in mud while pushing wheel-barrow full of product outside to dump. He strained his lower back. He has Doctor's slip releasing him from work 03/11 to 3/18/2010.
While cutting a section of rail on a cutting table and attempting to roll a six foot piece off the table it landed on top of his left foot causing pain and swelling. Transported by personal vehicle to Springfield Memorial ER where X-rays revealed several fractures in the foot. Treated and released to see an orthopedic surgeon for surgery to re-align the fractures.
While hanging siding on the east side of the mill, EE was dismounting the ladder he was using and during that process slipped down the ladder and pulled his left knee. EE was doing his regular work until June 8, 2010 when he was taken off work for arthroscopic repair.
Employee was walking from the office to the plant. Ground was covered with snow and ice. Employee slipped and fell to the ground. Employee was taken to the doctor and diagnosed with a broken ankle.
Employee was roof bolting when a small piece of draw rock fell and struck him on the right shoulder.
EE alleged that he was injured while driving a scoop to the battery station. EE said he hit a hole causing him to bump his head on the canopy of the scoop. EE worked the entire 9 hour shift.

EE stated that he and a co-worker were driving rivets into a belt using a spicing machine. They were using hammers to drive the rivets and were both kneeling on the ground at the same side of the spicer. The injured EE had a muscle cramp that made him move forward placing his hand on top of a rivet that was being driven. EE's left ring finger was struck by the co-workers hammer.
Employee was manually spinning the crushing drum at the Crushing Plant when his foot slipped and got hung between the crushing drum and wear plates. His right foot may be fractured, but is definitely sprained and has soft tissue injury.
EE got off the mantrip and started walking,his back started hurting.
Injured employee was trying to reposition the suction hose on water pump. While the injured employee was pushing other employee was pulling. The hose went through the upright and the hose sprang back and caught his finger between the foot valve and the upright of boom truck. Resulting in a fracture right index finger.
Employee was running a loader when he stated that he hurt his back. Employee refused any first aid help or transport in an ambulance to the hospital. The employee left the job against the advice of the foreman.
The EE alleges that while he took the stand that holds the strut hydraulic hammer with a 16-pound brown, he felt a sharp pain and cramp in his right arm.

Employee was drilling a roof bolt hole when a small piece of cap rock slid down the drill steel and broke in half. A piece of rock hit employee in the upper lip causing laceration to the upper lip that required 13 stitches.
EE was moving two 13"" tires from the back of the mantrip to the front deck of the mantrip. When he placed the tires on the deck he felt pain in his lower back. The EE went to Med Express where he was examined by a Physician Assistant. EE was seen by a Doctor on 3/22/2010.
On 3/8/10, EE was in process of removing the Miller Welder from #7 belt head. EE set the welder on some hanging cables. The welder started to fall and EE grabbed the welder w/his right hand to prevent it from hitting ground. EE heard his shoulder pop and felt pain. EE was referred to Dr where he was placed off work on 8/17/10, 1st day of work missed.
Loader operator was dismounting from loader when his right hand slipped from handrail. Supv. saw the operator falling and broke the fall with his own body before he hit the ground. Employee stated he was okay, but was taken for medical exam. On 8-9-11 operator went to Dr. and was taken off work duty for two weeks.
He climbed up on the front of a shovel track to get into the cab and hit his head on a mirror bracket. He did not use the step mounted on the machine and came up under the bracket from the wrong direction. The ER doctor gave him 5 sutures and said he suffered a concussion.He was seen again by the doctor on 3/9/2010 and cleared to return to work on the 10th.
Employee states he stepped out of a mantrip in a hole and fell face first to the ground. He landed in a push-up position but his right shoulder took the most impact. Employee continued to work but ultimately went off for shoulder surgery on 08/11/2010.
On 3-05-2010 EE stated that as he exited a Brookville Personnel Carrier he twisted his right knee. He continued to work until 04-30-2010. On 04-30-2010 due to continued problems with his knee a knee arthroscopy was preformed. This was not reported to Marfork Safety by the EE or the operation until 05-20-2010.
The employee was walking down stairs to begin his shift. When he stepped down with his right foot the heel of his boot caught on the stair causing a tear in his right quadriceps. On 3/16/10 the employee had a surgery making the injury reportable.

EE was whole watch at bag house, ee removing bags. EE was warned 3 times to put respirator back on, was sent home after 3rd warning. 7:30pm ee could not breathe and went to emergency room. ""See attached letter"".
Neutral safety switch was not working when co-worker started super-4 with transmission in gear. Basket went into rib and he got hand caught between basket and fork mast smashing first 3 fingers on on left hand. He was taken to hospital and was not released back to work making this a lost time accident.
EE experienced chest pain while shoveling material. Pain seems to be heart related as no injuries were reported. Further testing as outpatient is scheduled for heart condition. Tests showed no blockage but irregularities to the vessels were noted. Also drinking power drinks may have brought on symptoms.

Employee stated that he was walking toward the face, watching the miner cable as it was being pulled up. He walked through a pull thru curtain next to the rib and stepped on a pile of small rock and gob, causing him to slip and trip, twisting his left ankle and knee. He has been scheduled for surgery 05/03/2010 after a MRI that showed a torn Meniscus.
Employee was working on extending the hydoclone line at the tailings impound. He said when he pushed up on the pipe his back went into spasms. He was unable to work his next scheduled shift.
The injured and another employee had changed out a canopy post on the right side roof bolter machine. Both had loaded the old post on a 3-wheeler to take it to a diesel ride that would be used to take the post outside. While they were loading the post from the 3-wheeler to the diesel ride, he felt and heard a tearing sensation in his right upper arm (bicep).
EE stated he was walking and hit his head on a roof bolt plate. EE hurt his neck and back.
Driver was exiting vehicle to converse with other drivers, slipped and hit head on unknown object requiring a few stitches
The EE stated that he tripped while walking towards another EE, hyperextending his right elbow as he fell forward onto it. The EE continued to work and began missing work on 3/8/10, this incident is being reported as a lost time, reportable date of 3/8/10.
When the water pressure was turned on to a newly installed piece of 6"" fresh water line a coupling failed and the water line struck the employee in the right knee.
EE was working in the #2 intake entry close to the fan shaft. Cleaning up and re-bolting a slate fall, bolter had been backed out of the fall area, EE and a co-worker were under supported top evaluating the next step. Co-worker walked away, staying under supported top. As EE started to follow co-worker, he was struck by a rock originating from the fall area.
Employee was attempting to rehang a roller hanger from the top. The employee dislodged the roller hanger and it fell hitting employee on the right hand.
Employee was a passenger in a mantrip. EE was parked in a switch when an oncoming mantrip jumped track and struck EE's mantrip. Employee felt pain in EE's lower back. EE continued to work all scheduled shifts. EE has continue to treat for lower back pain and has been taken off of work for surgery on 6/24/2019
Ee was hanging cable (for tracking) in the intake air course when he fell hitting his back against the man trip. He worked two days before he decided to go to the doctor. Doctor said he had cracked his ribs and should be better in a few days.
EE stated that he was roof bolting in #9 entry where some loose shale was on the ground. He was installing the second row of bolts in this working place. He was walking towards the rear of the bolter when he stepped into a hole in the ground. EE cont. working until 3/5/10. EE stated he went to Dr. on 3/5/10 and was diag. with knee strain. He began missing 3/5/10.
EE was hooking his welder leads up, it grounded out. The brass connector blew up and put 2 pieces of brass into his left eye.
On 4/29/10 EE reported that back on 3/3/10 he was sweeping rocks off of the pit floor with the 988G loader he was operating when he caught the bucket on something solid causing a sudden stop of the loader. When this happened he said it felt like it ripped his insides. The accident was not reported to the supervisor until 4/29/10.
EE was shoveling on #2 belt. Claimed shovel hit on a splice causing the shovel to jerk back - aggravating an old injury where a plate had been installed in his shoulder approximately one year ago.
EE was helping to move the forks on a fork loader his hand was caught between the forks and the back board of the loader.
Employee called on 3/3/10 stating that he hurt his back on 3/2/10 while loading a shot with wet hole bags. He stated that he was going to the doctor. He returned to work on 3/4/10 with no doctors excuse. He said the doctors office was full and that he did not wait. He worked the 4th, 5th, and 6th. He called on the 8th stating he could not work.
Employee felt a pain in his left abdomen area as he was pluging in a high voltage cable.
Employee was standing in a parked scoop bucket when he got his foot caught between the bucket and a pin tray. He fell and twisted his ankle.
EE had been operating the miner & a piece of roof rock (approx 5'L x 3 1/2'W x 6"" thick) was on top of the machine. The rock was on the miner overhanging slightly over the right side. He backed the miner up under supported roof to the left side of the entry to remove the rock. As he approached the mid-point of the miner, the rock slid off on its own striking his left foot.
Hook chain up from dozer kumtso to unihauler pulled about 60 yards pulled into a ditch when tried to pull the hook broke traveled about 46 to 47 foot striking ee in the face.
Cutting tree with one tree against it the other tree came down and brush right shoulder and he fell down on elbow and knock shoulder out of place. He also had a cut on left side of head and put three staple in head.
Employee slipped and fell from the back of a flatbed truck. He landed on his left side causing pain in his hip, elbow, and shoulder. Employee was restricted from all work activities.

The man was removing a J-hook from a roof bolt plate when he caught his hand against the plate and lacerated his hand.
EE got off of shuttle car tripped and fell on his right knee at the feeder.
Foreman and roof bolter were scaling loose rock. A rock hit the panic strip. While trying to free it a piece of rock fell hitting the roof bolter operator on the shoulder and arm.
EE was tramming the continuous mining machine across the 003 section, when he stepped in a hole in the mine floor and twisted his right knee.
The employee stated that he was injured when he was taking apart a water hose nozzle in the plant with two large pipe wrenches. He states he had one on the floor and was pulling on the other for about 20 or 30 minutes trying to fix a leak. The employee did not notify the employer of his injury until 3/11/10. The employee began missing work on 3/30/2010.
Employee was driving passenger out to work from Pit operations building. Employee placed the van in reverse and felt the brakes grind, he then placed the van back into drive to park and down vehicle. When placed in drive the van jumped the curb, and hit the building. Passenger felt back pain and was sent in for medical treatment. Diagnosis back pain.
Employee was using the three point of contact method of mounting a D3C, his right foot was on a cleat of a track pad, his foot slipped off of the cleat, his shin hit the cleat and track pads.
Employee was carrying 2 full 5 gallon oil cans and experienced sharp pain in his back.
Employee stated that his loaded haul truck slid into the ditch along the haul road during icy road conditions and he strained his lower back while holding on to the steering wheel. He finished his shift on the day of the incident, returned to work the next day and completed that work shift. Medical records were obtained on 3/22/10. Incident is under investigation.

Employee was driving a skid steer.He looked behind him & drove over a rock which jarred his neck. He didn't report this. On 3/4/10, he came to work reporting discomfort in his neck & shoulder. He was sent to the medic on site. He worked his shift & then went on days off. He reported to work his next shift w/restrictions from doc.
Employee was attempting to step off a Kobota tractor and did not look at where he was stepping. As a result, the employee stepped onto a medium sized rock and rolled his left ankle as he stepped down.
Injured was loading shear jack for miner and felt pain in neck and right shoulder. *Did not start missing work until 6/8/10.
The employee was helping the section repairman pick up tools and put into bucket. The employee lifted the bucket of tools to put them on a ride when he felt a sharp pain in his lower back.
Employee tripped/slipped and fell while dismounting a portal bus in the parking lot, injuring his right shoulder. Note: Employee did not start missing work until 6/23/10. Full medical records were not available until 6/29/10.
The employee was shoveling muck into a chute, when he felt pain in his stomach. After our investigation, it was determined that it is a work related injury.

Ee was lifting a large water bottle into a vehicle and his elbow started to hurt afterwards. The Ee tried to treat the discomfort with rest and OTC medication. After no relief, the Ee went to the doctor and was placed on modified duty for two weeks with re-evaluation on 6/28/10. (Original injury date was 3/30/10 and when he finally went to the doctor on 6/18/10)
EMPLOYEE WAS LIFTING A CAN OF OIL AND ALLEGEDLY FELT PAIN IN HIS RIGHT SHOULDER.
Employee was installing screens on pep screen. Two other employees were told by the employee installing the screens to hand it to him and release it. When the two employees released the screen, the employee inside grabbed it and it slipped out of his hands and down onto his right leg pinching it between the screen and the cross member of the pep screen.
Mechanic was working on the A/C on a Motor Grader 16M he was he was stepping down of the machine and twisted his left knee. The mechanic continued to work for an addtional 30 min before he noticed any pain in his left knee. He notified his was supervisor and was taken to the doctor office and was told he had a sprained left knee.
Employee was removing the Gas Inlet Analyzer from the port for clean up at the Kiln, when a pulse of hot gas released and came through open port where the Gas Analyzer is located. As a result, employee sustained some epidermal burns on his arms.
EE was bent over a handrail using a pry bar to remove a stuck dart plug when he strained his low back
Employee states he was tightening the wheel and the wrench slipped, causing him to fall with right leg under him. ""REPORT BEING FILED UNDER PROTEST""
On March 24, 2010 at approximately 6:05 pm the 150 ton bin collapsed, falling onto a truck below. Employee was in the bin operator's booth and was injured when the booth collapsed with the bin.
Observing co-worker drill a hole for a rib bolt and was struck on the left knee by a rock that fell from the rib and rolled into him. The rock was 12"" X 18"" X 5"" thick. It struck him on the left knee and caused a contusion.
Employee was working in the raise cleaning up. He reached out and grabbed a piece of lagging. When he lifted it up, he felt a pop and pain in his shoulder. He continued to work about 4 more hours before the pain became intolerable.
Employee was cutting a section out of the floor in the mill. He stepped on a piece that he had already cut and fell through the floor.

Mechanic, was climbing the access ladder of a CAT 992C loader to check a reported problem. Upon climbing the access ladder he felt a ""pull & burning"" in his abdomen. He reported the incident to supervisor the next morning. EE saw his doctor at a later date who said he would need to schedule a ""hernia"" surgery.
Employee was in the process of changing screens. He noticed that the screen appeared to be pulled more to one side. He reached his hand into the front of the shaker to check if it was still hooked properly. At the same moment the back side of the screen came off and the screen recoiled. The recoil pinned his right hand between the front hook point and the shaker wall.
Operating a shuttle car and traveled around a corner striking a rock in the roadway. He struck the canopy with his head and came down hard on the seat. This resulted in a cervical/thoracic strain.
The EE (miner) was loading prill into face round with a prill pot. Walking toward the prill pot and weld on top of prill pot came loose and blew prill into his face, a lot of prill.
Employee got his arm caught between a chain and sprocket on the head pulley of the apron feeder.
Employee was carrying jacks & timbers to timber off a roof fall. While carrying a heintzmen roof jack, employee's foot slipped causing the employee to twist his knee. Employee had knee arthroscopy on 8/25/2010. Company received medical evidence that an injury occurred and determined the incident as reportable.
Injured worker was taking a cooler for the cems monitoring unit up a flight of stairs using a two wheeled cart when he strained his left knee. Injured worker didn't request medical attention until 06/03/10. His treatment will be a knee support, and physical therapy. Restrictions are no lifting over 50 pounds and avoid kneeling, squatting
Employee pulling on wire for wire saw & felt a pain in lower abdomen.
Employee reported on 3/22/10 that he injured his back on 3/19/10 when lifting panel for an audio booth. Employee under chiropractic care consistently prior to this incident.Remains unclear if this is a new injury. Will know when medical records are received. In light of the elevated enforcement, this report is being generated and may be requested to be removed from the record.
The ALLEGED incident happened on 3/19/2010 EE stated on 3-23-2010 that he had twisted his ankle at work the employee did not tell the company or report it to anyone for a few days. There were no witnesses and he told no one he was ever hurt for four days.
As EE was operating a Gehl skid loader the Gehl started to buck and his feet came off of where they were positioned at the same time as he lowered the forks, thus pinning his feet between the forks and the frame of the machine.
Employee was moving 2 pieces of metal on top of each other the top piece shifted and caught his thumb.
Mechanic caught foot and tripped on floor protectors located in the quarry garage. As employee turned to leave shop his foot caught the edge of the belting and caused him to fall onto the rubber. Employee's right forearm was sore as a result. Did not go to doctor immediatly
Employee stated that he had just checked the oil levels in the motor grader and was starting to get into the cab when his foot slipped off of the tire and he fell approximately 5 feet onto the ground injuring his right shoulder.
Equipment service technician stepped of the back bumper of his service truck. Right foot caught on the step. Left foot slipped and technician landed on the ground on his left knee. Twisted his knee.
EE was picking up a tooth for the 1800 hitachi shovel and pulled a muscle in his upper left arm.
EE was moving a 55 gallon drum off of a tommy lift to put the from in the storage. The drum fell and landed on the ee left foot. The ee was taken to the hospital and it was determined his foot was broken.
Employee was working on feeder in blending tunnel. After completing the repairs he stepped down off the conveyor and felt a pain in his right knee. He was taken for evaluation by company doctor who diagnosed him with degenerative joint disease of the knee.
Employee was attempting to free a drill steel. The employee exerted considerable force expecting resistance. When the steel freed easily, the employee tumbled backward straining his shoulder.
Shoveling rock with a long handle shovel and strained his back.
Employee using cutting torch on torch ring of HP 400 crusher to relieve tension between locking bolt and mantle, a burst of flame came out of torch ring, causing 2nd & 3rd degree facial burns. Hospitalized overnight.
EE states he dislocated his right shoulder as he was attempting to pull himself up to another beam from an elevated position. EE received medical treatment and was placed on modified duty.
Employee felt pain in his knee while descending a ladder. The employee resumed normal work duties from the date of this incident until 6/8/10 when he had surgery.
The individual was walking beside the low-trac when he fell and struck the low-trac
EE was injured, when the mantrip he was riding to the outside of the mine, rubbed the side of the rib. EE's left foot, which was outside the mantrip, was caught between the rib and the mantrip. Lost time injury.
Holding a sack that was filling with product.
The employee was standing on a platform attached to a track drill. As he moved the drill he backed it into the side of an attached air compressor which pinned his foot between the two units.
Employee was bent over the face cleaning out hole to put in lifterpipe. Pieces of rock fell from face, striking hardhat & right hand. He was seen by medic on site & sent to ER Room for x-rays, which showed 3 fractures. Surgery was required.
A 3 foot x 2 foot x 6 inches Rock fell out between the bolts on the roof boting machine and then striking the worker on the hand hat and shoulder.
Employee stated as he turned to sit down in his CAT excavator he felt a pain in his lower back - right side
Employee was in process of removing a steel hopper side. He was cutting what he thought was 1 of last 4 bolts securing panel when it was actually the last bolt. He may have been leaning on panel when it gave way creating opening causing him to lose balance & fall thru opening 21ft to ground. He was wearing fall protection but was not tied off to the provided life line.
Employee stated that he was using a chain hoist to reposition the flop gate on the main conveyor belt. As he was pulling on the chain hoist, he felt a pop in his right shoulder. Went to doctor on 3/17/10. Continued working but became reportable when lost time was incurred on 6/9/10 due to having surgical repair on right rotator cuff.
EE was walking in parking lot,tripped on gravel falling on left knee.EE reported falling awkward and twisted knee.EE worked until 4am at which time pain and swelling hindered walking.EE refused ambulance transportation.EE RTW on RD on 3/12/10. On 3/14/10 mine mgt. decided EE was not to RTW until released to full duty. EE quit on own terms on 3/21/2010.
Employee was rocking the sheave on the grizzly feeder with his right hand because he thought the bearings were going out. Turned to talk to the employee beside him and caught his free hand (left) between the belt and the pulley, cutting off the very tip of his finger.
Employee was handling miner cable in the 4 west section and felt pain in left shoulder **** started missing work days on 3/24/2010
Employee was replacing a screen and pinched his hand between the screen and a ladder guard. Hand was bruised and employee was prescribed nabumetone for pain and swelling, and instructed to use an ace bandage and ice for 48 hours. Light duty was prescribed for 48 hours, and was provided at work site. After light duty employee returned to full duty for next scheduled shift.
EE experienced discomfort in upper leg after shift on 3/9/10. Went to chiropractor on 3/15, 3/17 and 3/19. On 3/21/10, EE went to ER and was administered muscle relaxer, painkillers and anit-imflammatories. Scrip from ER stated no work until 3/29/10.
Employee was getting off the crane and lost his footing. He caught his fall by hanging onto the handrail. This caused him to wrench his shoulder. He didnt think he needed to go to the doctor. On 3/15/10 he explained that his shoulder was still sore. We set up an appointment where they diagnosed a strain. No meds given. It was recommended he attend physical therapy for one week.
The emplyee stepped up on a coal hauler battery, lost his footing and slipped off twisting his knee.
Employee was cleaning mud from the tracks of a D11 dozer. He moved toward the front of the dozer and slipped on some mud which was on an incline causing his left leg to fall in a drain.
Employee was struck by a piece of metal that was dislodged when a mechanic struck a drill deck bushing while attempting to free it from the drill steel.
Two employees were moving a powercable for an undercutter by way of tractor. One employee was watching the cable at the machine. The employee pulling the cable dropped off the wrong strap to add slack. When he took off again the power cable and cable swing arm pinched the employee against the back of the cutter.
Walking across a piece of grading and fell about 10 feet. Cut right arm on the grading.
Employee was walking to mine office and slipped on rock, falling to the ground.Employee fractured right distal fibula.
He was putting a scoop on charge. While connecting the charger, his foot slipped off the rear axle and he twisted his left knee, causing a strain to the left knee.
Standing in manbasket, basket suddenly swung to the left and jarred employee's back.
Injured was with crew loading belt structure into scoop bucket. When EE twisted his body his foot remained stuck in approx. 8"" of mud, twisting his left knee.
Employee stated that on 02/17/2010, he was moving a 2.5 gallon tote of water and twisted wrong, causing a pain in his back. The employee did not seek medical relief until 03/03/2010 at which time the doctor placed him on restricted duty that cannot be accommodated.
A miner that in 2009 was diagnosed with ""STRAIN LUMBO SACRAL, HNP L4-5, L5-S1, RADICULOPATIA L5-S1 BILATERAL"" did not report to duty on March 4, 2010. The miner was referred to El Fondo del Seguro del Estado to receive medical treatment. Allegedly the vibration of the 966C Front Loader caused by the dumping of the bucket caused the injury or aggravated his previous condition.
Replacing bucket elevator chain. Removing buckets from chain and bucket smashed left thumb between bucket chain.
Employee reported on 3-1-2010 he felt pain in right groin while assisting in lifting a steel plate..he did not request medical treament at the time of report. 12 days later he did seek medical treament and was diagnosed with a hernia and scheduled for surgery on 3-26-2010 and estimated to be off work for 4 weeks.
Employee was attempting to remove a pipe to the suction/holder when the pipet broke and lodged in his right pointer and middle finger. Per ee, the pipe was stuck in the holder and he had to use extra force to remove the pipe. The pipe broke, and the section that remained in the holder struck his fingers.
Climbing down from engine platform of water truck when foot slipped from ladder causing employee to hit platform on left chest wall. causing contusion.Treated as first aid. EE seen for chest cold non industrial 4/8/10. reported soreness in chest with coughing had,industrial contusion evaluated on 4/13/10 placed on restricted duty for 2 days.
