A fall of supported roof occurred at #4 head drive, west mains, in by spad station 547. The fall measured 18' w by 40' l by 5' h. No injuries resulted from this fall.

A ROOF FALL WAS DISCOVERED IN #2 ENTRY (1 BREAK INBY SURVEY STATION #4481) ABOUT 1200' OUTBY THE MMU003-0. THE FALL WAS IN AN INTERSECTION AND MEASURED 30' LONG X 20' WIDE X 10' HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED, VENTILATION WAS NOT AFFECTED. THE FALL WILL NOT BE CLEANED UP. THE AREA WAS BOLTED WITH 5' RESIN GROUTED BOLTS, SUPPLEMENTED WITH 8' BOLTS

An unintentional roof fall occurred at the Millenium Portal, at the 5N Longwall headgate between entries 2, 3, and 4 at crosscut 53. 4518N. The fall was approx 100' long, approx 18' wide and approx 12-15' thick. The fall did not affect ventilation or passage of people. The fall will not be cleaned up. Fall #22F.

An unintentional roof fall occurred at the Millenium Portal, at the 4N Longwall Headgate between entries #1 and #2 at crosscut 40 / 2721N. The fall was approx 20' long, approx 16' wide and approx 8' thick. the fall did not affect ventilation or passage of people. The fall will not be cleaned up.

A fire occured inside of the Hydraulic Roll Crusher building and the basement area below the Hydraulic Roll Crusher building. The fire was extinguished by a local fire department. There were no personal injuires, but property damage did occur.

The cable of the belt wrap itself around an idler. Bent a support beam & forced a pillow block bearing out of roller shaft, frozen idler w/friction from the tolling belt created heat ignating the rubber coating of the roller,the belt communication line, stopping the belt and burt 2 section of the conveyor belt. fire extinguished 45 minutes later & water truck.

A roof fall occurred in a worked-out area of the 1LMS-2L3L-1R panel in entry #5 at crosscut 11. Cause of the fall was a geological slip in the mine roof. Addl. mine roof had been cut during normal mining operations & the area supported. Mine roof averaged 10 to 11 feet in height in this area, when it is normally around 6.

Fall occurred at app. 6:45 pm in the #1 entry. Area was bolted with 4' resin & 6x6 plates. Fall measured 15' x 30' x 5' high. Area was a transition area from slate to sandstone roof located in low cover area. Equipment was moved outby & area timbered & dangered off.

Roof condition occurred in an area where several cracks were present. When a cut was taken out of the crosscut to the right it tied the cracks together in a triangle. The top started working in this area. A crib was built in the intersection. In time the laminated roof broke out around the crib.

A roof fall occurred in the 2 Main North at cross-cut #20 in the intersection of #2 entry. The fall measured 20ft. x 19ft. x 6ft. Six foot tension rebar bolts and square timbers were used to support the area. An undetected slip was determined to be the cause of the fall. This area was mined in December 2000.

Scaler operator experienced loss of hydraulics and then noticed smoke and fire at rear of machine. Operator exited machine and initiated mine evacuation while scaler continued to burn.

Bull Wheel alignment wasn't right & reported it.Inspection was noticed that S.Support pedestal had $ bolts, broke & was allowing pedestal to move, which was throwing sheave alignment to be off, also found 2 broken bolts, in S.Pedestal supporting N.Sheave. Each sheave is supported by 2 pedestal switch 8 bolts. mining shut down immediately & wait for special bolts due 2/25/05.

Based on the information received from those present it appears that the ignition was caused when the continuous mining machine was trimming footwall (mine floor) when a spark ignited an unknown methane bleeder near the footwall. MSHA notified timely.

A roof fall occured in #5 entry of the main south east at survey station 18 and 90. The fall measured 30' long by 19' wide by 10' thick. This area had been supported with 6' fully grouted resin bolts and 9' double locks. All approaches have been timbered.

A roof fall was discovered on 2-16-2005 at 10:05 am in the 3rd North Mains in #5 entry, between crosscut 11 and 12. The fall measured 16' wide, 80' long and 8' high. The fall was reported to the Delta field office at approximately 11 a.m. on the same day.

No injury, accident, illness or equipment damage occurred. A grass fire which started outside the permit burned into vegetation on reclaimed ground. It is not anticipated that any additional reclamation work will be required as new growth was not destroyed. No effort was made to extinguish the fire due to its widespread but low intensity nature. It burned out in roughly 12 hrs

The 001 longwall shearer was preparing to ""cut out"" on the tailside of the face when a bump occured that apparently forced a pocket of methane from the gob onto the face at the shearer resulting in an ignition that caused a mine fire.

As the 7LS1 Joy Shearer was cutting coal on the #1 L/W, an ignition took place on the T/G Drum which was cutting bottom rock as they went toward the H/G. The flame lasted approximately 10 to 20 seconds and was extinguished with the water sprays and a fire extinguisher. There were no injuries or property damage.

A non-injury roof fall was discovered in the #2 entry of the 005-0 section approximately 1 break outby spad #22476. The fall occurred while the mine was idle. The fall was about 6' high, 110' long and 20' wide. The roof fall was partially on the roof bolting machine but bolter was later removed.

Brake shoes were grabbing on elevator mechanism. Elevator was taken out of service, brake shoes were changed and elevator was put back into service at 11:40 a.m. on 2/12/05.

A roof fall occured in the 1 South Mains, #9 entry, starting in the middle of the intersection of crosscut #36 and ending in the middle of the intersection of crosscut #37. The fall measured approximately 80'X19'X6'. The area was supported with 6' tension rebar bolts. An undetected slip and dripping water was determined to be the cause of the fall.

An unintentional roof fall occurred int he number 3 entry, 2 breaks inby survey station 1604 in the belt entry. the fall was approx. 60 ft long x 20 ft widex 5 ft high.

A roof fall occurred in the 1 South, #8 entry in one-third the intersection of cross-cut #36 and extending in the pillarline 6ft. towards cross-cut #37. The fall measured approximately 12ft. x 19ft. x 6ft.-9ft. Six foot tension rebar bolts and 9 foot mega bolts were used to support the area. An undetected slip was determined to be the cause of the fall.

A roof fall occurred on the 1LMS panel at crosscut 38 on the 1LMS-2L1R in #2 entry. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was in the return air course and adjacent to the section roadway.

THE ELEVATOR AT THE #1 PORTAL WOULD NOT OPERATE PROPERLY AND WAS TAKIEN OUT OF SERVICE AT 1:OO AM ON 02/10/2005. THE PARTS WERE ORDERED AND THE ELEVATOR WAS REPAIRED AT 7:00 PM 02/11/2005 BY REPLACING FM2 AND FM3 RELAYS AND ALSO THE DBC CONTACTOR. THE #1 ELEVATOR DOES NOT AFFECT ANY DESIGNATED ESCAPEWAYS.

As the 7LS1 Joy Shearer was traveling to the T/G, the H/G drum was cutting bottom and the T/G drum was cutting top. A frictional ignition occurred at #50 shield on the H/G drum that lasted approx 30-40 seconds. It was extinguished with water and a fire extinguisher. No injuries or property damage was incurred.

A leaking natural gas valve allowed gas to accumulate in the Dorr-Oliver dryer control room. The gas was apparently ignited by an electrical device inside the room. The resulting blast severely damaged the walls of the control room and caused a partial roof collapse.

The torch struck EE on the left cheek and forehead. Laceration to the forehead that required four stitches to close. EE is still off work due to a slight headache, awaiting doctors release to return.

Employee injured his back loading posts by hand into the bucket of a scoop. He finished the shift and continued to work every day until 6/30/05. On 6/30/05 he was operated on for a lumbar disc herniation and/or lumbar stenosis.

THE LOOSE PANEL CAUSED HIM TO LOOSE HIS BALANCE AND WAS COMING OVER ON TOP OF HIM. EE THREW THE PANEL TO THE SIDE AND JUMPED TO THE GROUND, LANDING IN FROZEN RUTS.

Employee was inside the shaker replacing main support beams. Employee was welding, grinding and cutting to replace the beams. Foreign debris got into his left eye. He had his welding shield on. Later that evening his eye started to hurt and the next morning he woke up with a lot of pain and matting in his eye. He received a severe abrasion.

The employee was operating D motor (20 ton) on the tail end of two other cars. The D motor ran off track causing the employee to fall from the operator's seat striking his left shoulder and upper back.

He continued to have pain and recently (on 7/18/05) went off for shoulder surgery. He worked from the accident date to the date of surgery.

EE was installing a blade on a Caterpillar 16-G Grader. The chain holding the blade upright slipped off and the blade fell forward. The top edge of the blade struck his left foot just behind the steel toe. He incurred a deep laceration to the top of his foot and fractured three metatarsus bones (big toe and the next two). He is still off from work.

Actual lost time didn't start until 8/4/05; we were waiting for medical report to determine whether incident would be recordable or not. Employee lifted tongue of trailer and felt pop in right shoulder.

While moving a pump and its cable, the employee grabbed a spot that had a nick in the outer jacket. He got shocked and burnt his two little fingers on his left hand.

The job was to recover rail & while dragging a rail down the entry, EE noticed a piece of rib starting to roll. EE tried to jump back, but the rib caught his left lower leg and ankle. The x-ray was negative, but his lower leg & ankle show damage to the muscles & tendons. A brace was applied to his foot & ankle. EE was unable to return to work his next scheduled shift.

Employee was helping unload a roll of belt from a trailer by rolling it off. He got in front of it to pull on it and when it freed he slipped on the ramp. The roll of belt knocked him down and rolled onto his right leg, twisting it. He went to the doctor on 2/7 who diagnosed it as a knee and ankle strain and took him off work until 2/14/05 and he returned with no restrictions.
