Pick up truck and fuel truck were pulled into the shop at 3:50 pm. Everything seemed normal when the last two employees locked the door and left. At 4:10 pm smoke was seen coming from all the overhead doors. Fire dept was summoned but when they arrived the bin was fully engulfed. The state fire marshall determined that the fuel truck battery cable had shorted out causing the fire.

North 6, 14 block, 3 entry, 4 way intersection, MMU 004 18 ft wide by 17 ft long by 8 ft high. Support 6 ft 3/4 inch resin bolts, roof mats, cable truss bolts. Fall dangered off, additional roof support installed around fall.
During a routine inspection, the guide rope at the escape shaft was found to have a bad spot. Underground miners were evacuated from the mine immediately and a replacement cage was installed. Miners returned underground on 2/1/05 at approx. 10:15 am.
A fall of supported roof has occurred in by spad # 2589, 001 section. Number 15 crosscut, #2 entry. No injuries resulted from this fall. The fall measured 45' l by 15-20' w by 10' h.
A ROOF FALL WAS FOUND BY THE 3RD SHIFT PRESHIFTER ON HIS PRESHIFT. THE ROOF FALL WAS IN THE #5 ENTRY ON THE MAIN SOUTH ENTRIES AT SPAD #35+10 TO 35+55. THE FALL WAS 40'L X 17'W X 5'THICK. 5' DOUBLE LOCK ROOF BOLTS WERE USED IN THIS AREA.
A roof has occured on the 8 South section between the #11 break and the #12 break in the #1 heading. The fall is 80 feet long by 18 feet wide and 7 feet high.
Thr roof fall was approx 4-5 feet high, by 20 ft wide and 20 ft long. It was discovered one crosscut out by spad #193 in the #7 entry.
There was a rock fall at spad #4221 in return interupting ventilation; timbered and dangered off, set brattice line over one break re-establishing return.
An unintentional roof fall occurred in the #2 entry of the 3 left section, just out by sta # 1824. The fall was 20' wide by 30' long by 6' thick. The area was supported with 5' tension bolts. No men or equipment was included and ventilation was not interrupted. No escapeways were affected, all accesses to the fall were timbered off.
Methane ignition occurred in face of #2 left crosscut on Grassy Creek Mains section, 78' left handed of survey station #23273. Ignition occurred as miner was loading the std. shuttle car ont he left side of the face opposite of curtain. flame duration was approx. 3 minutes, and was extinguished with the wash down hose off the miner and fire ext. were no injuries or equip.
D11N Cat dozer caught fire in the engine compartment. Dozer appears to be a total loss. No reportable injuries.
A roof fall occurred on the 1LMS-2L1R panel at crosscut 22, entry 3 to 4. The cause of the fall was weak laminated roof shale. The fall was between the roadway and the belt.

sediment pond at the Oakhill Mine D65p bulldozer became. 24 inches below the surface and was buried in soft ground. the d9l sank in the soft material and ruptured the pipeline with it right grouser.

Roof fall occurred in No. 4 entry 150 ft. inby spad No. 1511, 400ft. from active face. The roof fall is 60 ft. long, 20 ft. wide and 6 to 8 ft. thick. All entries are breakered off. The roof is laminated and was bolted using No. 5 x by 48 in. resin grouted re-bar.

An unintentional roof fall was discovered on the 001-0 (West) Section in the #2 entry, spad #2576. Size: 50'L x 4-9""H x 18-20""W. Had been supported with 4' fully grouted resin bolts with supplemental support consisting of 9' rope bolts. No injuries, no equipment caught, ventilation was not disturbed. Fall will not be cleaned up.
Driver Error: Driver coming over 20 per cent hill and lost control of haul truck. Haul truck struck berm and rolled over onto the drivers side. No occupational injury to driver. Equipment was badly damages.

Discovered damaged tail rope on service hoist. Rope was damaged by bracket on dividing timber. Removed tail rope and replaced with new. Back running 11:45 am. Evacuated mine at 7:35 am to repair tail rope.

The pressure switch that powers the brake motor on and off, was out of adjustment. This caused the brake system on the hoist to lock up, and we could not release the breaks. Down time was 17 hours used to trouble shoot and adjust pressure switch. Hoist back in service 1/24/05 at 10am.
Roof fall occurred on the #2 main belt. The 42"" full grouted and 48"" full grouted rods failed to hold the questionable roof.
The elevator would not operate properly for over thirty minutes. A repairman from Schindler was called and a faulty door switch was replaced.
A fuse blew in a control circuit for the man hoist. No one was trapped but the hoist was out of operation for more than 1/2 hour. The two back-up hoists were fully functional.
The Tyro Creek service elevator went down after the first group of miners entered the mine. Mine management contacted Bagby Elevator Service and a technician was dispatched. The technician stated a problem with the govenor switch which he then repaired.
During a pre-shift of the mine following a weekend idle period a roof fall was found on the No. 2 belt conveyor. The fall measured approximately 18 feet by 24 feet by 10 feet and was located in an intersection 60 feet inby survey check point # 8074 The area had been bolted with 48 inch resin bolts.

This area was bolted with 5' tension rebar roof bolts and was reinforced with some 8' pins. Fall will not be cleaned up. It is cribbed & timbered at approaches.

During routine muck hoisting operations, the hoist operator noticed abnormal noise coming from the hoist rope drum assembly. the hoist was shutdown and a maintenance inspection revealed the #1 side automatic drum greaser had worked loose. the hoist remained inoperable for 2 hours while necessary repairs were made.
There was a PLC system failure which caused the primary air fan to stop. This caused ground coal to settle which was shortly thereafter ignited causing an explosion.
A FALL OF SUPPORTED ROOF WAS FOUND ON THE 002 SECTION NO. 9A BELT AT #2 CROSSCUT INTERSECTION OF NO. 7 ENTRY. THE FALL MEASURED 20'W X 20'L X 6'H. NO INJURIES RESULTED FROM THE FALL.

Fall in the no. 4 entry sometime between Jan. 20 & Jan. 21, 2005. The main power was off and all the men were knocked off. Fall approximately 60' long, 5' high. Bolted with 4' glue bolts; grade 60; 19' wide. Single crack on left rib, slate top. Miner was in #4 when power knocked. Rock on the miner and the retriever was used to pull miner out Saturday.

#8 entry was left not bolted, started with a small amount of rock falling. We made a power move up and a belt move up, during this time 14 rode out taking somebolts with it. this fall was stimbered off and left. this fall is approx. 22'wx 30'l x 48"" thick near #218.

A roof fall and subsequent tailgate blockage occurred at approx 4:35 pm. The fall obstructed travel off the tailgate end of the longwall face.

A WELDER WAS USING AN OXYGEN-ACETYLENE TORCH TO CUT A NOTCH IN SPILL PLATE IN THE STAGELOADER WHEN APPARENTLY A SPARK TRAVELED ACROSS THE STAGE LOADER LANDING ON MINE FLOOR IGNITING A METHANE BLEEDER. FLAMES WERE VISIBLE FOR 10 TO 15 SECONDS AND PUT OUT BY A FIRE EXTINGUISHER.

AREA WAS SUPPORTED WITH 8' FULL RESIN BOLTS AND PROPS. NO INJURY OCCURRED BUT MAIN LINE BELT WAS COVERED UP. FALL WILL BE CLEANED UP.

The elevator at our Hart street portal malfunctioned. One of the brakes got out of adjustment and caused the elevator to stop ten feet from its normal stopping point at the bottom of the shaft. The brakes were re-adjusted and the elevator was returned to normal operations.

UNINTENTIONAL ROOF FALL AT #24 CROSSCUT ON THE SECTION BELTLINE. FALL WAS APPROX. 40' LONG, RIB TO RIB & FROM 0' TO 6' HIGH.
Timer relay failed on dual brake system. Secondary brake system would not release. Down time of 25 hours was used to trouble shoot and replace timer relay. Hoist back in service 9pm on 1/12/05.
Roof fall anticipated due to visual observation of area, occurred in the area of the last open crosscuts and intersecting entries no. 1 through 5. The fall occurred as a result of the coal rider seam dipping toward the seam being mined in a low cover area.
Slope brake car was removed from service to replace speed sensor which had malfunctioned. Brake car was placed back into service at 7:56 a.m.
THIS IS A STACK SHALE AREA WITH THIN STREAKS OF COAL AND WEATHERING CONDITIONS, WHICH ALLOW THE THIN LAYERS TO SEPARATE & PULL AWAY FROM THE MAIN ROOF.

GARDEN B SHAFT ELEVATOR WAS INOPERABLE DUE TO JAM CONVEYANCE BEING OUT OF ADJUSTMENT.
An ignition occurred on the f-10 longwall section lasting 15 second and was extinguished by a water hose. there was no damage to equipment and there were no injuries.
Intersection 3-way fall in 18 block, #7 heading, 1 right section. Top gray laminated shale and sandstone. Area bolted with 6' resin bolts. Over 350' - 400'. Fall will not be cleaned up.
An unintentional roof fall occurred on H-mains, #5 entry, inby spad 8095, between #19 & #20 block. It measured 5'high x 18'wide x 45'long.
Employee found rock fall while conducting preshift examination. Rock fall had occurred in the #3 entry of the #3 West Submain between break 25 & 26 from the #7 Headdrive which is located in the #4 entry of the #1 North Mains. Rock fall measured approximately 20' x 30' x 6'.
Intersection fell in at 19 block, #7 heading, 1 right section. Top gray laminated shale & little streaks of sandstone intermixed. Cover of 370' - 400'. 3-way intersection. Area bolted with 6' bolts & supplemented with 9' install II bolts. Fall will not be cleaned up. Moved sect. from this area.
A roof fall occurred on the 2L4W-2L panel at cross-cut 2 in entry #3. The cause of the fall was weak laminated roof shale commonly referred to as stack rock. This fall blocked the alternate escapeway which was rerouted.
A fall of supported roof occurred on No. 5 belt (south mains) No. 4 entry (belt enrtry) at No. 14 crosscut. The fall measured approximately 35' long x 20' wide x 12' high. No injuries resulted from this fall.
At 1:55 pm the electric company experienced some type of problem which resulted in the loss of transmission of electricity to the area that included Independent Salt Comapny. Three employees who were in the mine at that time were brought up through the escape shaft.
A tailgate blockage occurred as the face reached survey station 2507. The fall was approx. 10 ft high, 20 ft wide and 25 ft long. Initial support included 7 ft torque tension bolts, welded wire mesh supplemented by cable trusses and roc props.
Roof fall occurred 2000 ft outby section. Roof measured 40' long, 20 ft wide and 7 ft in thickness.
Joy miner mining the ""A"" run between #12 entry and #13 entry. Top began to work and fell on miner. The ""A"" run was opened into the #13 entry. Miner is moving at time of investigation. The first cut out of the x-cut #12 to #13 was bolted. The first cut was 20 ft.
A new roof fall was found in the No. 6 entry of South Mains ""B"" intake. The fall is approx. 60 ' long by 10' high by 16 to 18' wide. Bolted with 8' install bolts and resin. Fall did not obstruct ventilation or escapeways. Area was dangered off and re-routed to other entry (#5). Location of this fall 30 to 31 xcut. Cause was deterioration of roof through time.
A roof fall measuring 180' L, 60' W, and 9.5' thick was observed during an examiation 14,000' from the working section in the old 4 left panel in the # 1 entry. No injuries occurred, travel was not impeaded, and ventilation was not disturbed as a result of the fall. The area was developed Jun 05, 5' torque tension roof bolts were in use.
An excessive amount of snow, sleet, and rainfall Wednesday, 1/5, and into Thursday, 1/6, caused flooding of the quarry at this location. Weather permitting, normal operation is expected to resume Friday, January 7, 2005.
UNINTENTIONAL FALL OCCURRED IN PILLAR LINE. CONTINUOUS MINER WAS CAUGHT AND SAFELY RECOVERED.

Plant had melting snow and 5 3/4"" of rain the week of January 2 to the 8th. There was wide spread flooding in Noblesville. There was no damage to equipment of to any buildings. There were no injuries.
Surface cracks had been encountered and area had been bolted with 4' resin bolts and T-3 channell. A crosscut was turned right out of #3 and when crosscut holed into #4 entry the fall occurred in #4 entry in the intersection being developed. Fall approximately 5.5' thick, 20' wide & cannot determine length.
ON 1/6/05 WHILE OAK MINE EXPERIENCED AN UNPLANNED ROOF FALL. THE FALL MEASURED 30'X18'X6' AND WAS 3,400 FT FROM THE ACTIVE FACE. THE FALL WILL BE REHABILITATED.

A roof fall occurred on the 1LMS-2L1R panel at cross cut 17, entry #6. The cause of the fall was weak laminated roof shale. The fall was adjacent to the primary escapeway for MMU-001.

NO VENTILATION OR TRAVEL WASYS WERE INVOLVED. WILL NOT BE CLEANED UP, WILL BE CRIBBED OFF. FALL IS APPROX 1.5 MILES FROM MAIN FACE AND APPROX 1 MILE FROM 1ST RT FACES.
An unintentional entries 8 & 9. no one was injured and ventiletion a passage of mess was not affected. the fall was caused by a hidden slip.
AT 2:00 PM A FALL WAS DISCOVERED ON THE #6 TRACK ENTRY OF THE 001 SECTION, 80' OUTBY SPAD #17314. THE FALL WAS 80' X 18' X 5' HIGH. THE ROOF IS MIXED AND SUPPORTED WITH 5' TORQUE TENSION BOLTS.
Fall app 30' long 20' wide and 10' thick- scoop was parked in heading in by fall, no damage, no blockage of vent or escapeway, no injuries, no equip damage. Will be cleaned up, breakered off and dangered, plan for cleanup posted, plotted temp on mine map. Fall cleaned up and area re-bolted.
Top fell for approx 30' in length, 6' high between 60-61 bl. on 1 rt belt. cover approx. 350' I called and reported this fall, but forgot to do the report.

Employees noticed Crosby clamps had turned. Called evacuation at 5:45am. Conducted full length inspection of rope including diameter, and connections. Changed out 9 Crosby clamps on #1 rope. Torqued to 250ft-lbs. Ran test trips. Mazella W.R. technicians completed a full length inspection, diameter check, torqued to factory spec of 430 ft-lbs. tested and documented. All ok.

THE FIRST SET OF SEALS ON THE RETURN OFF THE ACTIVE SECTION STARTED TO MAKE WATER AT SEALED ENTRY #1. THE ACTIVE SECTION IS 20'HIGHER IN ELEVATION THAN SEAL #1. THE WATER BLOCKED THE SECTION RETURN & THE MEN WERE PULLED FROM THE MINE. AT NO TIME WERE THE MEN IN DANGER, W/THE ESCAPEWAY UNOBSTRUCTED TO THE SURFACE.
AN UNINTENTIONAL ROOF FALL OCCURRED ON A-MAINS, #4 ENTRY(#1 BELT), AT #7 BLOCK, 1 BLOCK LEFT OF SPAD 31. IT MEASURED 6' HIGH X 8' LONG X 12' WIDE.
Brakes were dragging on elevator and elevator was shut down to correct problem. Elevator was put back into service at 9:45 a.m.

A fall occurred on the plow block side of the #1 entry. This fall began at the inby corner at #57 break and extended inby for 30 feet. The fall is appr. 10 feet wide and 5 feet thick.

A RIGHT TO LEFT SEQUENCE OF PILLARING WAS BEING USED. WHILE MINING THE LAST LIFT OF A PILLAR FROM THE NO.5 ENTRY OF 6-LEFT OFF 1 EAST, A PILLAR FALL RODE ACROSS THE MOBILE ROOF SUPPORTS INTO THE INTERSECTION (1 BREAK INBY SURVEY STATION 1190). FALL MEASURED 20'WIDE X 40'LONG X 12' HIGH. FALL WILL BE CLEANED UP FOR RECOVERY OF THE MOBILE ROOF SUPPORTS. THERE WERE NO INJURIES.
Roof fall occurred in track entry at #15 break in a 4-way intersection. The fall measured approx. 6'H x 20'W x 20'L. The fall was discovered at approx. 11:30pm.
At break 61 on the 008 - 009 mmu, a rock fall occurred on the belt and track. The fall was approximately 8' to 10'. It fell from the no. 2 entry to the no. 3 entry then up 1 break in the no. 3 entry. Area has been mined out under the 008 - 009 mmu.

The rock fall was in a x cut between the supply road and belt entry. the support in this area is 5' tension rebai roof bolts, both approaches have been cribbed. this was in the best panel at 7425 the face on unit is 2240.

THE TYRO CREEK ELEVATOR WENT DOWN AT 4:44 AM DUE TO EXCESSIVE WATER IN ""PIT"" AREA. THE WATER REACHING A MAXIMUM OF 40"", CAUSED THE GOVERNOR TENSION SWITCH TO SHORT.
Unsure how accident happened. Best guess is that the one or both of the left side wheels of the tractor caught a rib and caused the tractor to overturn. The employee was found under the left fender with his head and chest crushed. Do not, at this time, have autospy results to confirm whether the tractor caused death or something prior to the overturned tractor.
Employee became entangled between a return idler and the bottom conveyor tail pulley located in the primary crusher basement.
EE WAS SUPERVISING INSTALLATION OF A 125 H.P. PUMP AT THE FAN SHAFT. AS THE SCOOP WAS PULLING THE PUMP, EE SLIPPED IN THE MUD. TO KEEP FROM FALLING INTO THE FREEZING WATER EE GRABBED THE ROPE WICH PULLED THE PUMP, AND HIS RIGHT LITTLE AND RING FINGERS WERE PULLED INTO THE PULLEY. (AMPUTATION OF LITTLE FINGER: DISTAL PHALANGE.)
TRUCK DRIVER WAS ATTEMPTING TO DISLODGE FROZEN COAL IN TRUCK BED BY SWINGING TAILGATE. HE LOST FOOTING ON SLIPPERY GROUND AND HIS FINGERS WERE CAUGHT BETWEEN TAILGATE & TRUCK BED. 3 FINGERS ON LEFT HAND WERE SEVERLY INJURED.
Employee had tub ring chained to loader bucket and elevated in air. When reaching to retrieve hammer under tub ring, chain shifted and dropped on his finger.
Rolls were plugged with compacted dirt. Dirt gave way and caused the tension on the rolls to roll forward and pinched or pulled his left foot into the 3/4 inch gap between the shells.
Lifting pins to install into loader frame and felt pain in stomach.
EE INJURED GROIN WHILE PULLING ON BIG ELECTRIC WIRE FOR ROCK CRUSHER.
Was tightening a belt to align a motor.
EE was cleaning out a rotary valve. Located by the valve is an on/off stop station. EE did not stop the rotary valve and the valve fins caught his right middle finger. The edge of his finger was removed at the fingernail length. (AMPUTATION AT MIDDLE OF DISTAL PHALANGE.)
Electrcian erected ladder; extension 14' base 5.5' from wall. Man proceeded to climb ladder to tie off, when ladder slipped, causing man and ladder to fall apx. 7' to concrete floor. Mans hand was pinned between ladder & floor, servering rt thumb from hand. Man was wearing harness but had not yet reached top to tie off. First aid administered, ambulance required
Employee was attempting to remove plugged material from dust feed line with a 8' -10' steel rod when the rod came into contact with a rotating agitator in the bottom of the mixing tank. This resulted in the employee's glove becoming entangled with the rod and causing the injury.
EE THOUGHT THE CLASSIFIER REJECT LINE WAS ""STOPPED UP"". EE PROCEDED TO INSERT RIGHT GLOVED HAND UP A PIPE ATTACHED TO A ROTOR LOCK THAT WAS NOT LOCKED OUT. THE TIPS OF THE 3 MIDDLE FINGERS WERE CUT OFF.
THE INJURED WAS OPERATING THE SCOOP, HE HAD HIS LEFT HAND ON CANOPY EDGE. WHILE TRAMMING SCOOP, THE CANOPY CAME IN CONTACT WITH THE MINE ROOF, MASHING OFF THE END OF (2) FINGERS ON HIS LT. HAND. INDEX & MIDDLE FINGERS, JUST BEHIND FINGER-NAIL.
TRYING TO UNTHAW A PLACTIC LINE USED TO TRANSFER CAUSTIC SODA FROM HOLDING TANK TO TREATMENT PLANT DURING THE PROCEDURE EE MELTED THE PLASTIC LINE EXPOSING A MIXTURE OF GELLED CAUSTIC AND ICE. THE EXPOSED PRODUCT REACTED TO THE HEAT FROM THE TORCH PROPELLING DROPLETS UP AND UNDER SAFETY GOGGLES. EE PROCEEDED TO RUB EYES WHILE STILL WEARING A PAIR OF CONTAMINATED RUBBER GLOVES
Employee was helping load 48"" rigid top structure onto a gator for a move. While lifting the 18th piece onto the machine he strained his lower back muscles.
EMPLOYEE PERFORMED THE PRE-OPERATION INSPECTION & CLEANED THE FRONT WINDOWS. AS HE WAS IN THE PROCESS OF DESCENDING THE LADDER ON THE RT.SIDE OF THE MACHINE, HIS HAND SLIPPED OFF THE HANDRAIL & HE FELL BACKWARD TO THE GROUND, LANDING ON HIS BACK. EE HAD A FLASHLIGHT IN ONE HAND, NOT ALLOWING COMPLIANCE W/INSTRUCTIONS GIVEN ACCORDING TO .11011- USE OF LADDERS.
A rock pinned his hand against canopy.
Employee was walking in parking lot , slipped on the ice and fell to the ground, striking his head. Another employee found him unconscious in the parking lot.
Person was walking to piece of equipment and stepped on a rock and twisted ankle and fell.
Scaling ribs - rock fell approximately 15 ft. above - hit employee on top of head (hard hat was on). Cut top of head.
EE was surveying area and slipped on ice while walking down a slope. Initial diagnosis was knee sprain. EE continued to have problems with knee. Later determination was ligament damage. Physical therapy was attempted to avoid surgery. EE had orthoscopic surgery on 5/24/05 and returned to full duty on 6/29/05.
He was air lancing the ends of BC-205, also jack hammered the end - was using his left arm to hammer with.
While operating a shuttle car to transport coal, he ran over a rough road and was thrown into a canopy top hurtin his back and neck.
Employee was lifting a piece of metal and felt a pain in his lower back. Started missing work 3-8-2005.
EE was replacing V-belt on short head crusher. His glove hung in the belt and when the crusher broke over it pulled his hand in between the belt and sheave. His right had was bruised and his thumb was cut.
EE WAS FLOATING THE LEFT SIDE OPERATOT OUT. EE WAS INSTALING THE FIRST BOLT IN #4 RIGHT WHICH WAS A CORNER BOLT. THE LEFT SIDE DRILL BOOM FELL STRIKING EE JUST ABOVE THE KNEE ON RIGHT LEG. AT THIS TIME, OUR INFORMATION IS THAT FEMUR IS FRACTURED.
EE WAS DRIVING A SHUTTLE CAR. AS EE WAS TRAMMING IN #7 ENTRY TOWARDS THE MINER, HE NOTICED THE MINER CABLE IN ROADWAY. EE GOT OFF THE CAR TO MOVE THE CABLE OVER AGAINST THE RIB. AS HE PULLED ON THE CABLE HE STRAINED GROIN AREA. THE NEXT DAY, 02/01/05, EE HAD HERNIA SURGERY. EE WILL BE OFF WORK FOR SEVERAL WEEKS.
EE injured his back while attempting to change a center section on a scoop. (EE WAS UNDER SCOOP; TWISTED HIS BODY WHEN USING WRENCH.)
EE WAS ON LONGWALL WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF SUPPORTS STRIKING EE ON HEAD AND RIGHT FOOT CAUSING A BRUISE
The ee was operating the cont miner and backed miner up after cutting right side of face and he split the cats on the miner to move it toward left side of face. The right rear corner of the miner mashed the ee against the rib.
Injured was tightening bolt on brake head of #00-5 man Brookeville mantrip when he slipped on mine floor causing rt wrist to become fouled between brake head & frame of mantrip. He was brought outside by EMTs & transported to Boone Memorial. He was diagnosed w/contusion to rt wrist & released. He went to see another dr. later that day & was advised to take physical therapy.
Cold weather, icy frozen ground conditions. Worker had both feet on the ground when she lost her footing, fell backwards to the ground striking her tailbone on the frozen surface.
EE slipped on expanded metal walking surface because of outside weather conditions being foggy and cold, made walking surface extremely slippery. Fell backwards on back and neck. Lost time reportability.
While in the process of jacking a haultruck, the truck rolled forward and tipped the superjack up in the air. EE exited the superjack and landed on his wrist, fracturing bone and dislocating his left shoulder.
Employee bent down at the knees to pick up a 13"" length of 12"" pipe weighing about 100 pounds to place it on a table for grinding. As he stood up, he felt a burning sensation with the knee and tingling down into the leg which worsened over time. Employee did not seek medical treatment until 2/2/05 when the pain didn't subside.
Loader was pulling on 58 HP pump when 1/4"" steel cable broke and struck employee on left side of face.
Employee was cutting a piece of screen rib rubber when the utility knife slipped and cut his hand.
Employee slipped getting out of boat, stepping on to dredge. Fell on dredge - broken ankle/leg.
EE WAS STRUCK BY ROCK FROM ROOF, ON B0TH LEGS AS EE WORK ON A SHIELD ON THE LONGWALL RECOVERY AREA
The employee was using a chain fall to hoist a 3""X3""X8' angle into place. Tim reached down to lift the angle to its' final position - straining his neck. The incident occured on Saturday, 1/29/2005, but was not reported until Monday, 1/31/05, prior to going to the Dr.
Employee was in the rotor breaker coal chute prying on a piece of frozen coal material when it broke loose and fouled him against the side of the chute. Coal material was approximately 1 to 1.4 tons.
EE was mining the left side of # 6 Block when he was struck in the back by a piece of falling rib. He was knocked into the side of a shuttle car.
Employee was lifting nozzle to fuel 966 loader & pulled muscle in back. Transported to NNRH ER for eval. Given 2 days restricted duty & then return to work full duty. He contacted Safety & Health 1/31 requesting further eval still in pain. Follow up 2/01 at Golden Health; xrays of back normal. Prescribed anti-inflammatory, narcotic pain med & put off work.
EE WAS STEPPING DOWN OFF OF A TRACK FLAT CAR AND WHEN HIS FOOT TOUCHED THE MINE FLOOR HE TWISTED HIS ANKLE. CAUSE OF THIS INJURY WAS SAFETY AWARENESS. COMPLIANCE WITH RULES/REGS. WAS NOT A FACTOR, MINER ATTITUDE A FACTOR, PERSONNEL PROTECTIVE EQUIPMENT WAS NOT A FACTOR, MINING EQUIPMENT/SYSTEMS NOT A FACTOR.
EE cleaning mud from under conveyor slipped & fell back & hit his head. Cut to back of head. Bar EE was using hit him in chest. Foreman called Moriarty ambulance & EE was transferred to University Hospital (Albuq) to be checked out. It's Southway policy that any EE hurt be checked out by a Dr or ER. EE was wearing fall protection, taken off at hospital.
EE WAS STACKING CRIBS IN THE ENDLOADER WHEN SOMEONE THREW A CRIB ON TOP OF THE ONE EE STILL HAD HIS HAND ON. LOST TIME INJURY.
While stepping down from trackhoe, foot slipped and right knee struck the trackhoe.
EE (company electrician) was working on the control box of the secondary cone crusher. While moving some wires he felt an explosion which resulted in a second degree burn on his left hand due to the flash created by the explosion.
CHANGING CUTTING EDGES ON A SCRAPER, THE HOISTING CHAIN CAME LOOSE FROM A LIFTING EYE AND HIT EE ABOVE THE EYE, STITCHES WERE REQUIRED.
EE was loading belt framing. 2 belt stands were stuck together - EE jerked on framing to try to get them apart and felt pain in his R. groin. Went to ER that morning & saw specialist 1/31/05. Diagnosis was from previous hernia surgery (at age 16). Had exploratory surgery and hernia was surgically repaired on 2/2/05. Fax from doctor attached.
EE WAS WALKING IN HIGH INTERSECTION THEN STARTED INTO PART WHICH WAS LOWER STRIKING A ROOF BOLT PLATE, PUNCTURING HIS EYE, EE WAS WEARING SAFETY GLASSES.
EE WAS WALKING DOWN AN INCLINED ENTRY, SLIPPED AND SPRAINED LEFT FOOT.
Was carrying tarp on trailer & stepped on rock that was on trailer causing injury to left foot.
Supervisor was assiting ee in opening 2'x2' metal door from the jig vessel bin when the door un-expectedly blew open after bolts were removed, due to water. pressure. behind door. employee was push back against a stationary ladder resulting in a shoulder injury.
Employee was putting a cable on a truck to pull it out of the mud. Employee stepped in a hole on frozen ground and turned his ankle. Employee left work and went to the hospital and then called his supervisor that evening and told him that he injured himself on the job. He received a bad sprain to his left ankle. He was given an air cast. Pending per workers comp.
EMPLOYEE WAS OPERATING A DOZER, PUSHING IN THE SHOT & HE SLIPPED OFF A ROCK, JARRING HIS BACK & NECK.
Injured person was operating a shuttle car when the tires hit a hole in bottom. this action jarred his back. he sustained a bulged disc in his lower back which requires physical therapy and time off work.

They decided to try blaster cannons on floor 5. 1 EE left floor 6 & went to floor 5 to initiate blast. He didn't call on radio to make sure everyone was clear. The injured EEs were standing in front of kiln door when he fired cannon.
Employee states that while changing bits on the miner he tripped over a bucket of bits that were setting on the mine floor.

EE was using a cutting torch, laying on his side, cutting on a beam at the #1 conveyor Belt Tail. A piece of hot metal flew into EE's ear. He did not fill out an acc. report at that time. He started having trouble with the ear, so he went to the doctor & did not fill out a report until 3/29/05. He also did not miss any work until 4/21/05. Surgery, hole in eardrum.
Employee jumped down from a 2 foot high material chute and sprained ankle.
At the time of the incident, EE was changing out the transmission cooler lines on a CAT off-highway truck. As EE was moving along the frame rail of the truck, slipped and struck left knee on the frame.

EE WAS MOVING SHOT DOWN MATERIAL WHERE CONSTRUCTION OF OVER-CAST WAS TAKING PLACE, AREA HAD BEEN BOLTED. EE BACKED SCOOP INTO THE LOW AREA, BUMPED A LOOSE ROCK WITH THE LIGHT GUARD CAUSING ROCK TO FALL ON SCOOP, A PORTION OF THE ROCK STRUCK LEFT LEG.
While employee was pulling a feeder cable out of the way, he slipped & fell- thus twisting his knee.
While operating a scoop the ee lt. middle and ring fingers were pinched between a stop block and the outside wall of the scoop cab. ee fractured the tip end of lt. ring finger, 8 stitches to pad of middle finger and 6 stitches to pad of ring finger.
Injured person slipped and fell. As he landed on bottom his ribs struck his self-contained self-rescuer. He sustained bruised ribs which have required time off work.

Injured had just completed servicing scrapers on the #8 belt head and was walking toward the track entry. As he walked behind the belt take-up he stepped on a piece of conveyer belting that was laying on the mine floor and covered with water and rockdust causing him to slip and fall backwards straining his left knee.

Employee started dragging an oxygen bottle and felt a sharp pain in lower back. He worked his next scheduled on Feb 27 but started missing work on Feb 28, 2005.
EE STATED THAT HE WAS GETTING READY TO CUT THE CORNERS OFF OF A X-CUT OF A BLOW THRU IN 5-4. HE HAD BENT OVER FOR A BETTER VISUAL TO SET HIS MINER ON THE FAR CORNER WHEN A ROCK CAME OUT OF THE TOP FROM BETWEEN THE BOLTS, STRIKING EE ON THE HEAD AND THE LOWER RIGHT LEG. EE RECEIVED MEDICAL TREATMENT ON 2/1/05.
DRILLER SLIPPED ON MUDDY TRACK OF DRILL & USED HIS LEFT HAND TO STOP HIS FALL. HE STRAINED HIS LEFT ARM & SHOULDER WHEN HE GRABBED THE DRILL.
EE was trying to move an empty pallet at #4 header. The pallet was stuck in mud and when he picked up on the pallet, he felt something pop in his right shoulder
While loading timbers into the bucket of a scoop felt pain and pulling in lower back. Determined to be an acute lumbar strain. Unable to work for 10 days.
While sweeping floor on kiln floor, the employee developed lower back pain and muscle spasms.
Employee alleges that he injured his back while operating a shuttle car over what he claims to have been a rough roadway. He did not report this accident until 1/29/2005. Mine mangement feels that this accident should have been reported before the employee left the mine- if that is where the injury occurred.
Employee started downhill in forklift leading from Mill to shop. He turned forklift around to traverse the decline in reverse for better visibility. Forklift began to slide due to wet road conditions. Forklift would not stop so employee turned forklift into berm where it stopped and overturned.
WHILE EE WAS GOING THROUGH A MANDOOR, HE FELT PAIN IN THE LEFT SIDE OF HIS STOMACH. DIAGNOSED AS HERNIA - SURGICALLY REPAIRED ON 1/28/05.
The injured was operating a shuttle car and ran into a hole in the mine floor. The shuttle car bounced and the jolt resulted in bruising and a sprain to his right hip. He continued to work his regular job until June 14, 2005 when he started missing work from this injury.
Employee had raised the lid that covers the air tank to CS11 when the lid fell shut, smashing his finger on his left hand.

SPECIALIST ON 1/31/05.

The ee was injured as he was stepping down from the basket of a manlift. as the ee stepped down from the lowered manbasket to the ground, he stepped onto a rock the size of a softball causing him to twist his ankle.
EE WAS LIFTING AND CARRYING HYDRAULIC CYLINDER. EE ATTEMPTED TO LIFT CYLINDER FROM A KNEELING, TWISTED POSITION AND FELT DISCOMFORT IN CERVICAL AREA OF NECK AND LOWER BACK WITH SOME TINGLING SENSATION IN ARMS.
Employee was walking with some difficulty due to gout pain of right knee. On way to get toolbox, EE slips and fell, and twisted knee. EE continues working regular shifts; on 1/31/05, was sent to get medical attention. EE knee condition was aggravated by the excess tension of the knee when EE tries to avoid the fall.
Employee was cutting grass with a weedeater eater and he pulled his back.
EE was driving a 35 ton backdump. He was going uphill with a full load. The load shifted back and the transmission shifted down at the same time allowing more torque to pull wheels. This allowed front wheels to lift off ground into standing position. When load came off truck came down.
Employee and co-workers were in process of loading chain into metal tub (container bin) when metal pipe on bin fell off striking injured on right foot.
Employee was in the basket of a manlift shutting the machine down. When he reached to push the off switch, another contractor's crew dropped a 4# hammer from approx. 25' above and it landed on employee's (R) index finger. Finger tip was lacerated and fractured.
EE REPORTED: A FALL HAD OCCURRED & HE AND COWORKERS WERE ADDING TIMBERS AT BROW OF FALL. EE WAS CARRYING A TIMBER; TIMBER WAS RESTING ON EE'S RIGHT SHOULDER. TIMBER SLIPPED & EE CAUGHT IT WITH HIS LEFT HAND, RESULTING IN A TEAR TO THE BICEP TENDON. EE CONTINUED TO WORK ALL SCHEDULED SHIFTS UNTIL 3/4/05, WHEN HE STARTED MISSING DAYS.
Before starting to clean, he went outside and fell on the ice.
EE was palletizing slabs of stone which weight to 100 lbx. when he sustained a strain to his lower abdomin and right groin area. ee will be off work until further examination on 2-1-05.
Counterweight on shovel was being removed. Three of the bolts holding the weight were off, the fourth bolt was loose. Employee placed left middle finger under washers and the counterweight was slightly lifted pinching fingertip. Pinched off 1/8 inch off end of fingertip.
Employee was walking down a belt line when he stepped on a discharge pipe, twisting his knee. First lost work day did not occur untill 3/3/05 due to surgery.
Eyes burned from welding all day, employee was close to another welder and was catching arc from behind.

EE WAS CLEANING ROCK OFF OF A SHIELD WHEN A ROCK FELL BETWEEN THE GAP ON THE SHIELDS, STRIKING EE ON HIS RIGHT ANKLE. LOST TIME INJURY.
EE was welding near freshly applied spray expansion foam and ignited a small area of the foam. Employee blew onto the material and extinguished it and claims to have inhaled some smoke. Employee ended his shift at 6 am without claiming injury. He reported the injury at 3 pm, 9 hrs after his shift ended.
The employee was carrying a conveyor belt idler to the scrap metal bin when he stepped on a patch of ice. The employee fell fracturing his left ankle.
WHILE CLIMBING UP THE 2 STEPS TO ENTER HIS TRACTOR TRAILER HE SLIPPED ON THE TOP STEP AND STARTED TO FALL OFF. HE GRABBED THE HANDRAIL ON THE OUTSIDE OF THE TRUCK AND HELD ON WHILE HIS LEGS KICKED OUT FROM UNDER HIM THIS CAUSED A MUSCLE IN HIS LOWER BACK TO TEAR.
EE hit head on half-header; he fell down on his right arm.
EE was walking along a 33 ft section of track rail that was being pulled to the work area by a locomotive. The piece of rail bounced over and struck EE on left foot causing a contusion. EE did not receive a release from the physician for next shift.
Employee was moving 8"" water discharge hose out of a ditch. While lifting and moving the hose on uneven ground he felt a pain in his right knee. He was diagnoised with a sprained right knee.
Driver got out of his truck to approach dispatcher's window to see if he was needed for a load, he made 2-3 steps, slipped and fell on icy road.
While traveling outside, coming down grade mantrip struck a belt hold down roller that the outby crew was moving into place, roller caught mantrip causing it to come into operator deck hitting him on left side of hip, causing bruise.

Operating bobcat on snow removal. Bucket struck old railroad rail, stopping momentum. Employee thrown forward striking his head on windshield. Employee was wearing seat belt.
Employee was stepping onto the conveyor walkway when his foot slipped off. He then grabbed the handrail which spun him around and twisted his back.
Injured received contusions to his left leg when it was pinched between the pickup he was attempting to get unstuck from the snow and a snowy berm. Snowy, icy weather caused the loader used to pull the pickup out to slide forward into the pickup and pin his leg between the truck and bank.
Went to get a wire brush out of tool. When a foreign object flew into his eye.
EMPLOYEE EXPERIENCED A BURNING SENSATION IN HIS LOWER RIGHT ABDOMEN & SWELLING JUST UNDER HIS BELTLINE. SEVERAL X-RAYS & AN M.R.I. WERE TAKEN & A RUPTURE WAS DETERMINED. THE INJURY DID NOT BECOME REPORTABLE UNTIL 5/9/05 WHEN SURGERY WAS PERFORMED TO REPAIR THE RUPTURE.
Employee was told to clean snow off belt. Instead of cleaning belt front catwalk, employee decided to step up onto belt causing him to slip and fall.
EE DRIVING ROCK TRUCK UP OUT OF STEEP PIT WHEN THE LOAD SHIFTED TO THE REAR CAUSING THE TRUCKS FRONT END TO RAISE UP 4-5 FEET AND THEN SLAM DOWN JARRING THE TRUCK CAUSING PAIN IN LOWER BACK.
EE AND COWORKER WERE CLEANING OUT FINES CHUTE AT #2 STOKER PLANT WHEN A FROZEN LUMP OF COAL BROKE LOOSE AND HIT EE IN THE EYE. EYE WAS FLUSHED WITH EYEWASH AND EE WAS TAKEN TO THE EMERGENCY ROOM. (EE WAS BELOW COWORKER. COWORKER WAS USING PRY BAR TO CLEAR COAL. EE WAS NOT WEARING SAFETY GLASSES.)
Climbing out of roll crusher holding on to a brace going across roller when brace slid off the side & EE fell backward onto roll crusher.
The EE was working on a pulling fixture for removing a pin when he felt a pain in his left eye. The EE was chipping a weld with a chipping hammer. Subsequently, a metal particle became lodged in his left eye. He was wearing required PPE.
EE ALLEGES THAT A LUMBAR STRAIN RESULTED FROM LIFTING A TIRE CHAIN. COMPANY DISPUTES THIS CLAIM OF INJURY
EE was attempting to lift a roll of 6"" discharge hose into the 910 loader & bucket. His foot slipped causing him to fall to the ground with one leg in front & one leg behind him.
EE was attempting to open a mandoor between the intake and the return when he felt a sharp pain in his right shoulder.
Employee stated that he and another employee had made a belt conveyor splice in no. 4 belt and were using chisels and hammers to spread the belt clamp apart to release the tension from the belt conveyor, when the belt clamp came loose and struck him in the upper right arm.
EE was walking in fresh snow up a grade to the upper level where his vehicle was parked. He slipped in the snow and fell to both knees, which one leg went behind him. Swelling happened in both knees.

Scraping bottom from #4 entry to #1 entry. EE was helping with cable on the 14-15 Joy miner. He bent over and was trying to pull cable over one side out of the shuttle car's path. He said he felt like something pulled in his back.
Employee was working in prep plant replacing fly hts in a-bath. new flights were banded together. as ee broke bankd 1 flyht rolled and dropped 18 to 22 inches-striking the ee right foot just behind metatarsal-xray revealed no breaks-contusion.
Getting out of loader and his left foot slipped on last step and he fell to the right and landed on his right hand hurting his wrist.

EE WAS GRINDING A PIECE OF METAL WHILE WEARING SAFETY GLASSES. A PIECE OF SLAG FLEW INTO HIS EYE.
EE CLAIMS BACK INJURY WHILE OPERATING AN R-35 HAUL TRUCK IN THE PIT. WHILE DRIVING INTO THE PIT, THE TRUCK STRUCK A POT HOLE, CAUSING THE SEAT TO RISE AND FALL SUDDENLY, RESULTING IN THE INJURY.
He was lifting a #50 bag. Bag slipped, he attempted to catch it.
EE WAS OPERATING BATTERY RUBBER TIRE MANTRIP. EE STRUCK COAL RIB CAUSING THE STEERING WHEEL TO SPIN SUDDENLY STRIKING THE EE ON THE LEFT HAND. THE RESULT WAS A FRACTURE TO THE RING AND MIDDLE FINGER ON THE LEFT HAND.
EE was sitting on a conveyor belt while the belts were down. When he attempted to stand up his knees buckled causing him to fall & twist his left quadracep muscle.
THE MEN WERE CHANGING THE BELTS ON THE MOTOR DRIVE FOR THE FEEDER. THEY HAD TO LIFT THE MOTOR UP WHEN THE MOTOR WAS LIFTED THE LIFTING EYE BROKE AND THE MOTOR FELL STRIKING EMPLOYEE'S LEFT HAND. HE WAS TAKEN TO THE EMERGENCY ROOM WITH ONE BROKEN BONE AND A BIG GASH ACROSS THE LEFT HAND.
PERFORMING ROUTINE ELECTRICAL HELPER INSTALLATION TASKS. EE WAS IN THE PROCESS OF CARRYING CONDUIT FROM A STORAGE PILE TO THE WORK SITE. WHILE CARRYING CONDUIT ON SHOULDER, STEPPED INTO A BLIND HOLE APPROX 12"" DEEP. EE FELT SORENESS IN HIP AREA. NEXT DAY, EE WAS IN PAIN WHEN REPORTING FOR WORK, SENT TO HOSPITAL. XRAY REVEALED A FRACTURE OF THIRD VERTEBRA.
THE EMPLOYEE BENT DOWN TO PICK UP ROCK DUST HOSE. FEET GOT TANGLED UP IN HOSE, TWISTED RIGHT KNEE. DIAGNOSED ON 3/4/05 WITH TORN ACL OF RIGHT KNEE. FIRST DAY MISSED WAS 3/4/05.
A steel plate weighing approx. 250 lbs. had fallen through a chute onto a coveyor. Two employees were moving the plate out of the tunnel, when the employee who was moving backwards suddenly began to feel pain in his lower back. He was taken to a doctor and diagnosed with muscle strain. He was prescribed medication, off work for 3 days and then released to modified duty.
Roof bolter operator drilled 5' hole while putting steels out of the roof, the bottom steel stuck in the head (chuck). The steel stayed in the roof, he lowered the head and pulled on the steel in the chuck with right hand. His left hand was resting on the boom. When the steel came out of the chuck, it fell striking left middle finger.
Throwing cable and felt pain in back and left leg. This did not become lost time until 01/26/05.
A cylinder was hung so employee & his co-worker could pick it up with a forklift. EE's index finger on his left hand was caught between cylinder & frame.
Individual was reaching to remove a chain used to transport a longwall shield located within a shield haul vehicle, when the chain lift arm was activated to lower the shield pinching individual's right upper arm (humorous). The pinching of the lift arm fractured the individual's right upper arm.
EE driving H10 coal truck, stopped at old 9A mines to tighten chains up on the truck. He slipped and fell while walking around his truck. He was transported to Brayton Hospital were he was diagnosed with a sprained knee. He will be off from work until 1/24/05.
Man was uncoupling supply cars which were setting in 4 North back switch. After pushing on cut-off lever - he stepped back. His left came down on side of rock causing foot to turn.
EE was standing on the lower level bumper area on the front of the Cat 777 End Dump and had placed his lunch bucket on the top platform and apparently blacked out and fell approx. 4 feet to the ground. This ""accident"" is under investigation as to possible medical conditions of ongoing concern. EE could have been on the ground and fell over.
EE STATES THAT EE WAS TRYING TO ROLL THE HAMMER DRUM IN THE ROCK CRUSHER WHILE REPLACING THE BELT. THE DRUM BEGAN TO ROLL PINNING LEFT ANKLE BETWEEN THE HAMMER AND THE ROCK CRUSHER HOPPER. THE EQUIPMENT HAD BEEN LOCK OUT AND TAGGED BUT EMPLOYEES WERE MOVING THE DRUM TO REPLACE THE BELTS.
482 Scoop. He was scooping rocks got head over scoop and caught head between scoop and top of roof.
DUMP TRUCK WAS LEAVING THE QUARRY WITH A LOAD OF STONE WHEN IT LOST TRACTION AND SLID BACK DOWN ROAD. EE WAS WALKING UP OUT OF THE QUARRY & WAS STRUCK BY TRUCK.
We were installing several side panels on the bucket elevator. The supplied rental equipment erratically moved and hit a panel that was about to be installed, knocking it over on worker's right leg.
EMPLOYEE BUMPED HIS ELBOW AGAINST THE HEAD OF A ROOF BOLT. ***REPORTABLE ONLY DUE TO ASPIRATION OF THE ELBOW.*** Amendment: Started missing time 2/16/06 due to surgery.
EE WAS WELDING ON A CONVEYOR IN A TUNNEL. WHEN FINISHED, EYES BEGAN TO HURT AT THE END OF THE DAY
Injured was shoveling spillage along belt line when a piece of rib rolled striking him on the right wrist causing a bruise.
EE was shoveling along 50W stacker area. While shoveling sand next to concrete wall, the corner of the shovel hit the concrete wall, stopping the EE's motion. EE's right hand went numb and his shoulder started hurting. Diagnosis from medical clinic is strained right shoulder.
Employee was pulling a piece of curtain away from a stopping which dislodged a cinder block. The block fell 5 feet striking the injured on the top of his right foot.
Employee was bolting brow of roof fall. On left side of bolter using two pieces of steel. Upper section of the roof steel hung up in the hole when EE dropped the pod. Steel dropped out after it cooled & struck rt. thumb mashing it between upper steel & drill pot causing a fracture and laceration. NOTE: Amended Narrative per misc. mail 1/25/05 #1.
EE WAS DRIVING HAUL TRUCK. IT STARTED RAINING ABOUT 8:00 A.M. AND WHEN IT HIT THE GROUND IT FROZE. EE WAS PULLING A SMALL HILL AND SPUN OUT. EE LOCKED THE TRUCK BRAKES AND THE TRUCK SLID BACK DOWN THE GRADE AND TURNED OVER TO THE RIGHT SIDE. (EE WAS NOT EJECTED.)
EMPLOYEE REPORTED BROKEN SEAT WAS THE CAUSE OF HIS INJURY TO HIS BACK.
Employee stepped in an open 8"" drill hole, fell backwards to the ground and his foot became stuck. His foot was quickly dislogded from the hole and he suffered strains/sprains to his right hip and lower back.
MOVING CONVEYOR MOTOR THAT COMES OFF MINER. HE WAS MOVING IT SO THAT HE COULD PICK IT UP WITH SCOOP & CLEAN BEHIND THE FOOT PADDLE.
Employee was removing frost from around track idlers on Akerman excavator with a hammer & chisel. A glancing blow off chisel head struck his middle finger on left hand against track frame, resulting in a deep cut & bruises to his finger.
Employee was descending a stairway. When he stepped off the last step, he slipped on some mud and twisted his left knee. His personal physician took the employee off of work on 01/22/05.
While unhooking a chain, he was in the scoop bucket, coal slack and wet, he twisted his knee. He missed the first day due to the accident on Feb 17, 05. He is scheduled to see the doctor on Feb 28, 05.
Employee was cleaning a ditch line to a pond with a D-5 dozer. He was attempting to move a caustic soda feed line to prevent damaging it with dozer. While attempting to move the line, the line broke and sprayed caustic into his face.
EE was operating a continuous miner. He was loading some push-up loose coal cleaned by a scoop when a pinner steel came out of the tail of the miner striking him on the left shoulder. He continued to work but his shoulder got worse. He went back to DR on 2-8-05 & was told to stay off work until an MRI can be made. He is scheduled for an MRI on 2-14-05. Missing work on 2-8-05.
Empoyee was in process of helping another ee pull 1 1/4"" pressure hose thru a stopping when he felt pain in his right leg and groin. (HERNIA SURGICALLY REPAIRED ON 2/11/05.)
While bolting top, a rock fell from the top striking him on the top of the right hand.
As ee stepped off the walkway of the roof bolter, when he stepped on the mine floor his foot slipped, he felt a sharp pain in his lower back.
Tearing down and cleaning engine parts. He was lifting and moving heavy parts and twisted back.
Employee picked up a 4 ft section of 6"" schedule 80 pipe, twisted his back and pulled a muscle in his lower back.
EE went up on the wet bins to start the belt and screw. As EE was coming down the stairs a puff of smoke from the dryer (scrubber emission exhaust) came up the staircase. With the below freezing temperature and the exertion from climbing the steps, this caused EE's breathing to become difficult.
Employee was stepping on the floor after running a cable under the drive pulley of belt in prep plant when his right ankle twisted.
He was sitting on rear of roof bolter with head down on arms when a rock fell hitting his head.
EMPLOYEE WAS GETTING OUT OF FUEL TRUCK, GRABBED THE HANDRAIL WITH ONE HAND & THE SEAT HANDLE WITH THE OTHER. WHEN HE TOOK ALL HIS WEIGHT OFF OF THE SEAT, THE SEAT WENT UP & CUT HIS FINGER WHICH REQUIRED SUTURES.
After filing sanders of his 50 ton motor located on the #2 empty track, he walked to the wire side of the track where he slipped on packed snow between the rails. He fell, landing on his right elbow and shoulder. At the end of his shift he felt a little sore and filed a report in case something became of it later. He worked until an MRI revealed that the rotator cuff was damaged.
While installing roof bolt, a piece of roof material approx. 3'x3'x3"" fell from between the roof bolts. struck the bolter canopy broke apart and a smaller piece struck ee are the right leg regs not a factor
Employee was working off a 4' step ladder cutting off low hanging roof bolts. The ladder was not set up properly and when he reached out the ladder slipped causing him to fall to the ground resulting in a muscle bruise in his lower right leg.
While checking oil on loader stepped back and fell off elevator platform.
Cutting bolts out of roller cap. Blew holes out with air hose. Cutting blew in l/eye.
Employee was going around a piece of machinery and struck his head on a cross section of angle iron
Employee was in process of removing bolts and hardware from a 992d loader cab area with a 1/2"" air gun. Air gun was left on cab floor area, while employee exiting cab area to ground by means of a portable ladder caught air gun hose and pulled air gun down against right jaw and face area.
EE was pulling the bolter cable, as a result of pulling the bolter cable He strained the muscles in his neck and shoulder.
Operator was pulling a dozer out of a stuck position - got dozer out - hit the tram leavers - was back further that he thought - excavator flipped backwards & rolled - landed on the cab - cab glass was broken - removed broken glass & got ee out
EE PLUGGED IN THE CATHEAD FOR THE #4 BOLTING MACHINE AND SET UP THE BREAKER. THE BOLTING MACHINE WAS TURNED ON AND AN ARC FLASH CAME FROM THE CATHEAD. THE FLASH CAUSED A BURN INJURY TO THE EE's FACE.
EE was lying on the ground on his back installing cutting edges on the blade of a 16-G grader when he says he had a sharp pain in his lower back going down his right leg.
Employee got his feet tangled up in cable when loading a shield, twisting both knees as he fell down.
EE WAS STRETCH WRAPPING PALLETS OF LIME. WHILE PULLING PLASTIC FROM THE ROLL, EE FELT A SHARP PAIN IN LOWER BACK

EE called on 1/19/05 & said he had hurt his back on 01/17/05 while operating a dozer.He states that when he backed over a rock, it caused him to be jerked around in the seat and strained his back.We have not been able to contact him since to get further information on his condition.Noone is sure of his length of experience in mining, so I have entered 0's for now.
Member was carring his cap light in hand and tripped over his cap light cord falling off of tailpiece he was welding on, landing on his shouler.

HE WAS TAKING THE SCOOP OFF CHARGE, THE TOP IS LOW AT THE SCOOP CHARGER AREA & WHEN HE POSITIONED HIMSELF TO UNPLUG THE BATTERIES HE STRAINED HIS LOWER BACK.
EE SAID HE WAS TRYING TO RAISE A PIECE OF BELT STRUCTURE BACK UP ON SIDE RAIL WHEN HE FELT A SHARP PAIN IN HIS BACK. EE CAME BACK TO WORK ON 1/18/05 AND CONTINUED TO WORK UNTIL 3/8/05 WHEN HE SAW A DOCTOR BECAUSE OF HIS BACK. EE RETURNED TO WORK ON 3/14/05.
Member was walking to section from end of track and stepped in a hole hurting his right knee.
EE WAS CHANGING DRILL BIT ON RIG WITH WRENCH WHEN WRENCH FLEW BACK AND HIT HIM ON THE RIGHT WRIST.
Employee was walking from his car to the bath house and slipped on ice in the parking lot. Injured fell to the ground injurying his lower back.
IW was stndng on the lft side of the trlr. The drvr chked the side mirror, he saw IW begin to walk away from the trlr. The drvr then noticed a pu truck approaching on the rt side. The drvr looked to see that the pu was not pulng in front of him. He then began to pull fwd. While pulng fwd he rechckd his mirrors and saw IW falling to the grnd. The drvr stopped the truck.

During packing, a bag burst and sprayed powder on the side of employee's head. Employee reported incident and pain on 01/24/2005.
Injured was rolling a rock out of the roadway when it slipped from his hands and fell back onto his leg
Handling miner cable when his feet gangled in wire screen the continuouse miner had just cut down. He tripped and fell striking his left knee on the mine floor. Did not go to a doctor until 6/30/05. Dr. diagnosed as bursitis and prescribed one month off work.
Employee was standing on a trailer (approximatelt 18 inches high) that a 6 inch water pump was mounted on. He went to exit the trailer and his right foot got caught on a metal bracket.
Employee was roof bolting at the face, as he swung the boom to install a bolt a piece of roof rock 4 in thick by 1 ft long fell and struck ee on top of left foot.
Employee was crossing under a piece of water line and raised up and struck his head on a piece of ventilation tubing. This caused pain in his neck and back.
Injured was traveling from the E2 to the E3 manbar, as he entered the E3 curve he took curve at too low of an angle, therefore trailer began to push the tractor across the curve. Thr driver tried to correct the situation, but failed. the truck overturned onto the driver's side.
EMPLOYEE WAS LOADING BAGS OF ROCKDUST ON TO PERSONNEL CARRIER & PULLED MUSCLE IN HIS BACK.
EE was starting first bolt in the fourth row of the entry. They were in the 1st L. X-cut. A rib-roll occurred, falling onto the right ankle, causing a fracture.
Injured was moving cables and curtain out of the travel path of the mobile bridge carrier when he stumbled over some rocks lying along the rib which had just been removed from the top of the continuous miner. He fell, breaking his fall with his right hand and knee. RTW on 1/16/05. Missed work 1/1905 and 1/20/05
Employee was standing on tracks of 992 excavator talking to supervisor. The tracks were muddy and when he moved to grab the handrail to climb down, he slipped off. He struck his left knee on running board, causing swelling, brusing and possible ligiment damage.
Employee stated that while helping lift a pipe from the belt magnet he felt pain in his lower back. (Employee continued working, started missing on 3-13-05.)
Employee was riding a mantrip out of the mine on overtime. the mantrip operator knew that motor and shieldcar was sitting on the track ahead of them. the mantrip ran into the shieldcar. cause of this injury was safety awareness. compliance with rules/regs not a factor, protective equip not a factor, miner attitude a factor, mining equip/system not a factor.

EE LET OFF THE TRAM OF HIS MANTRIP TO ALLOW THE GOPHER TO ENTER THE SHED AND AS HE DID, THE FRONT WHEEL OF HIS VEHICLE HIT THE TRAILER HITCH ON THE GOPHER, CAUSING THE STEERING WHEEL TO JERK & THE STEERING KNOB ON HIS VEHICLE STRUCK THE SIDE OF HIS RIGHT HAND.

Employee started a 58 hp water pump. The rush of water into the flexible discharge hose cause it to raise up suddenly, striking and jamming his right ring finger. Employee started missing work on 4/4/05 due to surgery.
The employee was investigating smoke coming from a fuse disconnect box located in the surface substation when an electrical arc flashed burning him.
DID NOT START LOSING TIME UNTIL 4/26/05. While hanging bolter cable, felt pain in left shoulder.
Removing a piece of chutework, the chute became lodged in the hole from which it was being removed from the plant, the ee pryed the chute to free it. Due to the forces of the rigging the chute sank into the ee's left foot when it was freed.
After positioning roof bolter to drill along. the rib the ee inadvertently set stab jack down of right foot.
EMPLOYEE NOTICED A SLIGHT PAIN IN HIS GROIN AREA AND POSSIBLY A SMALL LUMP IN THIS AREA ALSO. HE WAS DOING NON-PHYSICAL WORK AT THE TIME. (HERNIA WAS SURGICALLY REPAIRED.)

Employee was injured when the shuttle car that he was operating in #2 Entry (near spad # 10686) hit a hole in the bottom & jammed his head into the canopy. Management became aware of this accident on 1/17/05 & he started missing work on 1/20/05.

EE was removing the feed chute liner from a primary mill 7 feed chute when the D-ring broke off of the liner-he was struck by the clevis attached to the D-ring resulting in facial trauma/fracture. Lost time started on 1/14/05.
EMPLOYEE WAS TRAVELING DOWN A SLIGHT GRADE WITH A TURN TO THE RIGHT AT THE BOTTOM WHEN HIS FRONT TIRE SLID AND THE BED OF THE TRUCK TO JACK KNIFE SIDEWAYS CAUSING THE FRONT END TO STAND UP ON ITS NOSE THROWING HIM FOREWARD IN THE CAB. SEAT BELT WAS WORN.
Employee was trying to move a battery charger stand by hand (as opposed to using the scoop) and strained his lower back. He started missing work as the result of this event on 1-18-05.

THE INJURED EMPLOYEE WAS JUST BEGINNING TO GO UP AN EXTENSION LADDER, LOST HIS BALANCE & FELL FROM THE 3RD RUNG OF THE LADDER. THE EE STRAINED HIS LT. GROIN AREA.
Ice on ground caused employee to slip and twist his left knee. Saw Dr. on 1/31/05, torn ligaments.
EMPLOYEE STATED HE WAS ROOF BOLTING WHEN A PIECE OF ROCK 2'X 3'X 6"" FELL & STRUCK HIM ON THE RT. LEG. EE WAS TRANSPORTED TO THE HOSPITAL WHERE IT WAS DETERMINED THAT HE HAD SUFFERED A BROKEN LEG.
EE WAS GETTING OFF 3-WHEELER AT CHARGER. HE CAUGHT HIS FOOT ON CHARGER LEADS AND FELL, TWISTING HIS LEFT KNEE. EE STARTED MISSING WORK ON 1/13/05 DUE TO A STRAINED LEFT KNEE.
Employee stated that he was attempting to lift a belt roller. Two rollers were stuck together. He felt a pain in his lower abdomen. He was diagnosed with a left hernia. his first lost work day did not occur until 1/20/05.
Employee was drilling the roof to install a roof bolt. When he had drilled approximately 2"" into the roof the drill steel bent and broke causing it to strike him on the eye.
WHILE ATTEMPTING TO INSTALL (HANG) A HOPPER, FINGER BECAME PINCHED BETWEEN VALVE OF HOPPER AND THE HOPPER FEEDER.
Employee was cutting reject chips inside ball mill when debris flew into his eye. Examination revealed an embedded fragment.
Employee was walking through a 4'X 6' mandoor at the submains belt starter (3 break). He slipped and fell causing injury to his ankle.
Ee was picking up a gallon of oil for the equipment and his back went out. attempted to reach over extending his arm.
Employee was riding mac 8 mantrip, mantrip hit coal rib; coal rib rolled off hitting right side of employee.
Belt line broke - Employee was picking up structure to level it when he pulled something in the left side of his back.
VENTILATION TUBING SLIPPED AND TRIED TO GRAB IT AND STRAINED BACK
IT WAS A RAINY AND WET DAY. EE WAS WALKING ON CONCRETE AREA NEAR THE RAILROAD TRACK WHEN EE TRUNED TO GO BACK THE WAY EE CAME, EE FOOT SLIPPED OFF INTO THE DRAINAGE AREA. EE SLIPPED AND FELL TRYING TO CATCH SELF WITH RIGHT HAND, SUSTAINING A FRACTURE IN ARM.
EE was climbing ladder to perform engine service. EE slipped on the second to top rung of the ladder. While falling to the ground EE grabbed the handrail and felt something pop in right arm.
EE SLIPPED AND FELL ON CONCRETE FLOOR OF COAL PREP PLANT DURING A SCUFFLE WITH EE SUPERVISOR
Employee was carrying a 5 gallon hydoil can when he experienced pain in his low back and groin area. He has continued to work until 2/11/05 at which time he had surgery to repair a tear in his groin area.
Employee was performing backfill operations when the machine tipped. Employee experienced minor back pain the next day. He was seen by a chiropractor making this a lost time accident.
Injured was using a 1 1/2 ton chain hoist to pull the conveyer chain on a 10 s/c. The chain hoist was attached to a belt hanger and the chain. As the conveyer chain was pulled the hook attaching to the conveyer straightened out. The hook then flew striking the victim in the left rib cage.
EE was putting breaker in on pinner - burnt left hand when breaker flashed.
EE WAS HELPING WITH A BELT SPLICE WHEN THE NYLON SLINGS USED TO LIFT THE COUNTERWEIGHT BROKE. AS THE COUNTERWEIGHT FELL, THE BELT TENSIONED, CAUSING THE EE TO HIT THE STATIONERY ROLLER.
Injured was rockdusting, he stepped on a piece of belt that was covered with mud, slipped and fell causing a fracture of ribs left side.
Employee stated he was shoveling loose coal onto the belt line, as he turned to throw another shovel of coal onto the belt he twisted his left knee, causing pain to left knee. Started missing work on 1-21-05.
EE WAS FILLING THE ACCELERATOR TANK ON THE SHOTCRETE SPRAY TRUCK. EE LOOKED DOWN AND SAW ACCELERANT COMING OUT OF THE VALVE. EE CLOSED TE VALVE BUT THE PLUG WAS NOT PUT BACK IN THE TOP OF THE TANK, THIS CAUSED THE ACCELERANT TO BLOW OUT INTO EE EYES.
The operator claims that while he was operating a hydraulic excavator, the machine made an abrupt movement and he felt pain in his lower back.
EE WAS APPLYING BLOCK BOND TO 2 AIRLOCK BELT STOPPINGS. HE WAS WEARING SHORT RUBBER GLOVES AND THE BLOCK BOND MADE CONTACT WITH HIS WRISTS. EE WORKED THROUGH HIS SHIFT. HE TOLD HIS SUPERVISOR THAT HIS WRISTS WERE BURNING AT THE END OF HIS SHIFT.
EE WAS OPERATING BAGGING MACHINE FROM A SEATED POSITION. HE ROSE TO STEP DOWN AFTER FINISHING A PALLET OF SAND BAGS. HE DID NOT PLANT HIS RIGHT FOOT SQUARE AND ROLLED THE ANKLE AS HE PUT HIS WEIGHT ON IT, CAUSING A SPRAIN OF THE RIGHT ANKLE.
EE was setting timbers when he experienced pain in lower back.
WHILE TRYING TO TURN A TRAM MOTOR FOR A 10SC SHUTTLE CAR, MEMBER HURT BACK AND LEFT LEG.
EE twisted knee 1-10-05 after going to Dr and not missing any work it was decided to do some cleaning of knee. Diagnosis was osteoarthritis of knee, but an MRI showed tear which was repaired on 3-3-05 while knee was being checked. EE's first day to miss was 3-3-05 original injury was 1-10-05.
on 1-10-05, He was driving a rock truck and was a process of hauling material from the electric shovel to the backstack area for dumping. At the dumpsite, as the bed was being raised up about 8 feet, the frame suddenly broke and the bed abruptly fell back down on the truck. The impact of the bed striking the frame caused his neck to take a hard jolt. miss work on 8/3/05
Employee had loaded his scoop bucket with a load of 8"" timber that were 10' long. While hauling them into the section, part of the load fell out of the bucket. The employee stopped to reload the fallen timbers into the bucket. When he picked up one of the timbers he felt a sharp pain in his groin.
Employee had rock fall from roof at last row of bolts and piece of rock struck him on right ankle.
EE was getting out of the loader, when he reached to grab the handle. He twisted his back causing it to strain.
SECURITY OFFICER AT THE MAIN GATEHOUSE WAS WALKING OUT OF THE BATHROOM AND SLIPPED ON A RUG LOCATED ON THE FLOOR IN FRONT OF THE DOORWAY. THERE WAS NO WATER/ICE OR FOREIGN MATERIAL PRESENT.
Employee was using a high-pressure water hose to wash out underneath the stageloader. He lost control of the hose, was knocked down and struck on the right wrist the hose nozzle.
During a 7H belt move, employee went to toss belt roller across belt when his foot slipped twisting his left knee. Lost work days began 3/31/05.
Muddy conditions caused steps on ladder to become slick when employee was exiting 777 D Rock truck to receive cleaning supplies from lube truck when he slipped on lower steps striking his head on lube truck.
EE WAS PULLING ON 1 1/2"" WATER LINE WASH DOWN HOSE STUCK IN MUD. WHILE PULLING ON HOSE, FELT SHARP PAIN IN LOWER BACK. DUE TO SCHEDULING AT PLANT, FIRST DAY OFF WAS 1/13/05.
While installing bottom structure cover (belly pan) to Cat. D11 bulldozer, chain slipped allowing cover to fall on left hand causing laceration & index finger to break.
Employee was shoveling on top of overcast NM belt, slipped and tried to catch himself. He said he felt pain in left shoulder and back. EE started losing time May 12, 05. On June 1, 05, after investigating incident, we were notified that this was work related.
Employee was lifting switch throw to throw for the curve as he was lifting the handle with force he strained right shoulder.
HE WAS CLIMBING DOWN A LADDER FROM THE OPERATOR STATION ON THE IROC PORTABLE CRUSHER. HE WAS ON THE BOTTOM STEP, HE PUT HIS FOOT ON THE GROUND & TWISTED HIS ANKLE ON A ROCK.
Employee knelt down and felt pain in his left knee.
The injured was positioned at the mouth of the 8K fault heading approximately 3 feet inby the outby rib corner. A catamaran jack was position at 56 crosscut in the #1 entry 1/2 crosscut outby the injured's locaton. The catamaran jack chain was tensioned and a chain link broke and ricochet into the break striking the injured.
EMPLOYEE STEPPED OFF THE BOTTOM STEP OF THE LADDER ON THE FRONT END LOADER. AS HIS RT. FOOT HIT THE GROUND IT TURNED, ROLLING TO THE SIDE & TWISTING HIS RT. ANKLE. INJURY HAS CAUSED LOST TIME & MEDICAL TREATMENT. EE SUSTAINED TORN LIGAMENTS.
While working on coal belt EBC-274 removing down-comer roll for conter-weight. We were waiting for crane so we started getting everything ready to come out. Took guard off and took bolts out of bearing base. Went to hook up chocker on bearing shaft and the roll slid back pinching hand between bearing & angle iron.
Pushing the separator of a belt storage unit and ruptured his left achilles tendon.
EMPLOYEE WAS CARRYING A ROLL OF PALLET BANDING MATERIAL ON HIS SHOULDER WHEN HE SLIPPED ON SNOW & ICE COVERING THE GROUND. AS HE FELL FORWARD HE PLACED BOTH HANDS IN FRONT OF HIS BODY TO BREAK THE FALL, AT WHICH TIME THE ROLL OF BANDING CAME OFF HIS SHOULDER LANDING ON HIS RT. HAND & THUMB.
Putting up last row outside bolt swung pot into right knee and leg.
EE WAS USING SCOOP TO UNLOAD PALLET OF 6"" BLOCK FROM FLAT CAR, WHILE PULLING BLOCK INTO BUCKET, CHAIN GOT UNDER BUCKET. EE LOWERED BUCKET TO RELEASE CHAIN. WHILE BACKING UP, SCOOP FELL OFF OF CAR CAUSING EE TO BE BOUNCED AROUND IN DECK STRIKING HEAD AND RT SHOULD ON CANOPY.
WHEN LOADING ROOF BOLTER, THE OPERATOR WAS RE-ADJUSTING GUARDS WHEN HE SAW THAT HE NEEDED A NEW HOLE CUT IN THE RUBBER MAT. WHILE TRYING TO CUT THE HOLE WITH A RAZOR KNIFE THE KNIFE SLIPPED & SLICED HIS PALM OPEN.
EE WAS SETTING A BARREL OF OIL UPRIGHT AND, FEELING IT SLIPPING FROM HIS GRIP, TRIED TO CATCH IT, SUBSEQUENTLY PULLING HIS BACK. LOST TIME INJURY.
While cleaning mud accumulation out of discharge chute the air lance got stuck in the muddy material causing employee a strain of the left shoulder.
Employee attempted to descend the side of the static thickener using a piece of grading as a handhold. Grading came loose causing him to fall.

EE WAS GETTING DOWN FROM THE BACKHOE/ROCK BREAKER WHEN EE STEPPED ON A ROCK, TWISTING ANKLE.
Worker was bending down to pick up a 5 gal oil bucket, when he stood up he became dizzy and fell down an approximate 12' slope injuring his neck and cutting the back of his head.
The employee slipped on ice and fell on the deck of the barge Alexandria. The weather at the time was sleet and freezing rain. There is the potential for a slipped disc.
EMPLOYEE STATES HE WAS WALKING UPHILL & HE FELT & HEARD A ""POP"". EE STARTED MISSING WORK ON 1/19/05.

EE WAS INSTALLING A DIFFERENTIAL ON A CAT 793 OFF-HIGHWAY TRUCK. RATHER THAT USING A LADDER TO COME DOWN FROM THE TRUCK'S A-FRAME, EE JUMPED TO THE GROUND, 3-4' BELOW. WHEN EE LANDED HE FELT A SHARP PAIN IN HIS ABDOMEN. BECAME REPORTABLE ON 1/21/05 WHEN EE WENT TO DOCTOR & WAS DIAG. AS HAVING HERNIA - SURGERY SCHEDULED. PHYSICIAN'S RECOMMENDATION TO NOT RTW PRIOR TO SURG.
ROOF BOLTER OPERATOR WAS CRAWLING BACK TO OPERAT0RS STATION ALONG SIDE OF THE MACHINE WHEN EE SLIPPED ON THE BOTTOM TWISTING KNEE. EE HAS NOT YET BEEN SEEN BY A SPECIALIST TO DETERMINE THE EXTENT OF THE UNJURY. APPOINTMENT SCHEDULED 1/18/05.
EE WAS POSITIONED BETWEEN LONGWALL SHIELDS #173 AND #174 WHEN A SECTION OF ROOF FELL FROM BETWEEN THESE SHIELDS STRIKING HIP ON TOP OF RIGHT HAND CAUSING A LACERATION AND FRACTURE.
EE WAS OPERATING DOZER. EE SAID HE HAD PUSHED SOME WATER OUT OF A SMALL HOLE IN THE CLAY. WHEN HE WAS BACKING UP AND THEN GOING FORWARD, THE DOZER BUCKED WITH HIM, CAUSING A SHARP PAIN TO GO UP HIS BACK.
On 1/5/05 EE stepped out of the barge & slipped & jarred his tailbone or back. It was sleeting & raining so apparently he lost his balance. He reported the acc. but said he didn't think he needed to go to dr. Plant was closed 1/6/05 so on 1/7/05 EE worked on barge. On Sat. 1/8/05 EE went to dr before he came to work. Dr told him to come back Mon & they'd do x-rays.
Employee was using a crane to lift a metal drum filled with iron balls used on the Cement mills. Suddenly his little finger was pinched between the pulley and the wire rope of the crane causing him an open wound. He was using safety gloves.
Employee started bolter. Bolter began moving, trapping employee between bolter and rib.
EE had a wrench and was tighting up a bolt on a crusher in the shop and there was no room for a air wrench to get into the space, he was pulling on the wrench and something popped in his neck.
While employee was using a grinding wheel it came apart and hit his right knee.
EE was pulling miner cable when he felt a pain in his groin. He told the miner operator and worked his full shift. He worked the next day and was in a little more pain at the end of his shift. He told his supervisor on this day. The next morning he was in a lot more pain and decided to get it checked. He was diagnosed with a hernia and referred to a surgeon.
Employee stated he was walking underground & struck his head against a roof bolt, jamming his neck. This caused pain to his cervical area of the neck & back.
EMPLOYEE WAS INSTALLING A DUMP CHUTE TO DIVERT WET MATERIAL. A PIECE OF GRATING WAS REMOVED TO INSTALL THE CHUTE. THE WEATHER WAS FREEZING RAIN. THE EMPLOYEE SLIPPED ON THE ICE FALLING THROUGH THE GRATE OPENING. EMPLOYEE RECEIVED A HEAD LACERATION, BROKEN COLLAR BONE AND BROKEN RIBS.
Air Lancing Hopper, Air pressure caused loss of balance. When stepping back to brace, his right foot slipped past edge of catwalk. Frame edge poked inner side of his right knee.
Employee backed rock truck through berm and over highwall. Employee jumped from truck, breaking his leg.
on 1-5-05 ee stated he slipped and fell on some snow and ice while cleaning fines around the rock crusher area and felt pain in his left shoulder. ee was taken to the dr and was released to return to full duty. on 2-15-05, ee was diagnosed and underwent surgery for a torn rotator cuff of his left shoulder.
EE WAS USING WALK BEHIND BOBCAT LOADER AND SLIPPED ON THE ICE. THE BOBCAT RAN OVER HIS RIGHT FOOT, RESULTING IN A FRACTURE. THIS IS A LOST TIME INJURY.
Employee stated that he was putting glue and bolt in the bolt hole and felt something pull in right groin area. Man was putting in 4 ft. bolt bottom-piece with glue on top of it. He was standing in sluffage off of the rib in poor body position (employee did not miss any work until 1-26-05).

EE RAISED THE ENGINE HOOD, HAND WAS LACERATED BY METAL PART.
EE was helping set breaker timbers on north mains when a rib fell and hit him on right knee, he worked until 3:00 pm and walked outside, reported swelling in right knee, undetermined at this time as to extent of injury.
