Accident type, without injuries

A roof fall occurred in the Unit #1 intake aircourse at entry #6 at 18 xcut. The fall is approximately 20'L x 18'W x 10'H. The fall will not be cleaned up. The intersection was supported with 10' point anchor bolts.

The mine slope belt motors stopped due to power failure. The brake on the slope belt failed to engage allowing the slope belt to run backward. This caused the fluid couplings to rotate backward. The fluid couplings failed and blew apart.

(2ND OF 2 ROOF FALLS DISCOVERED THIS DATE.) A roof fall occurred in the 2 Main North in the intersection of #1 entry at cross-cut #27 and extended approx. 80' inby. Fall measured 100' x 20' x 6'. Area was supported with 6' tension rebar bolts & timbers. An undetected slip was determined to be the cause of the fall.

(1ST OF 2 ROOF FALLS DISCOVERED THIS DATE.) A roof fall occurred in the 2 Main North in #2 entry in the intersection at cross-cut #26. Fall measured 20' x 20' x 6'. The area was supported with 6' tension rebar bolts & 6"" square timbers. Area was mined in Sept. 2000. An undetected slip was determined to be the cause of the fall.

The 018 joy miner was caught by a roof fall, it was in the final push in the #2 block when a pocket of rock fell and caught the head of the miner. The fall was about 20 wide by 30 foot long.

A rock burst occurred at the bottom of the 4000 level 184rs. in the main drift & the top of the 4300-190rs approx. 20'below the 4000 level. The rock burst was triggered by the blast from the 43-190 development raise heading. The rock burst was picked up on our seismic system & due to the activity was not looked at until the next day.

At about 10:16 a.m. a fire occurred on 62BC11 & 62BC12. Clinker cooler failed to sufficiently cool the clinker--the source of ingition. BC11 is the 1st rubber belt in the system that feeds BC12, a tripper conveyor that runs the length of the clinker building. Kern County FD & LA County FD responded. Fire was extinguished by 11:27 a.m.

A ROOF FALL WAS FOUND AT X-CUT 267 ENTRY 7 MAIN SOUTH. AREA HAD BEEN MINED OCT. 1997, AND WAS SUPPORTED WITH 8' FULL RESIN BOLTS. FALL WAS APPROX. 20' WIDE X 30' LONG X 10' HIGH. NO INJURY OR PROPERTY DAMAGE RESULTED. FALL WAS ON MARKED ROUTE FOR WEEKLY ESCAPEWAY. ESCAPEWAY WILL BE REROUTED.

At approx. 7:15 pm, the tyro creek service elevator was out of service due to the door lock contacts not working. Bagby elevator repalce the interlock fixed safety edge, and replace one guide. elevator fully operational @ 1:45 am on 3/28/05.

A roof fall measuring 180' long, 20' wide, and 6' thick was found in the return entry near EP-6 spad 48861. The fall occurred due to subnormal conditions above the 5' torque tension roof bolts. Ventilation and the passage of men was not affected. The fall entrances were barricaded to prevent travel.

A roof fall measuring 30' long, 20' wide, 6' thick was discovered in the return near EP-H spad 49101. The fall was 4,600' from the active section and occurred due coal and rock streaks above the 5' torque tension roof bolts. Ventilation and the passage of men was not affected. The fall entrances were barricaded to prevent travel.

AN UNPLANNED ROOF FALL OCCURRED IN THE BELT LINE ENTRY OVER AND OUTBY 1 CROSSCUT FROM SURVEY STATION 30. THE FALL MEASURED 40' X 20' X 6'. THE AREA WAS BOLTED WITH 4' FULLY-GROUTED BOLTS AND ROOF MATS WERE USED

A roof fall measuring 20' long, 20' wide and 5' thick was found in the #7 neutral entry at spad 49879, 5000' outby the working section. The fall occurred due to subnormal coditions at or above the 5' torque tension roof bolts. Ventilation and the passage of men was not affected, no personal injuries resulted from the fall. Fall entrances were cribbed and barricaded.

A roof fall was discovered on the intake entry 5,200' outby the active section near spad 49694. The fall measured 40' long, 20' wide, and 5' thick. The fall occurred due to subnormal conditions above the 5' torque tension roof bolts. Ventilation and the passage of men was not affected, the fall entrances were resupported and barricaded to prevent travel.

A roof fall occured in the 1 Main East, #4 Entry, in the pillar line between crosscuts 23 and 24. The fall measured approximately 25'X19'X6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.

A screen fire was started during construction of the new secondary plant. Belt 26 was started & it was rubbing on the structure, so EE trimmed a small piece of metal with a torch. Some of the slag fell to the screen deck below & started a fire on the rubber screen deck. The fire was extinguished in about 45 minutes. No injuries.

A ROOF FALL ABOVE ROOF BOLT ANCHORAGE WAS DISCOVERED IN THE INACTIVE HEADINGS OF THE 2 NORTH AREA OF THE MINE. THE AREA WAS BEING EXPLORED FOR REHABILITATION WHEN THE FALL OF 20' LONG X 10' WIDE X 5' HIGH WAS FOUND.

A ROCK FALL OCCURRED ON THE #1 BELT AT CROSSCUT 23. FALL WAS APPROX. 30' LONG X 20' WIDE X 4"" HIGH, PULLING THE 4' PERMANENT SUPPORT. NO PERSON OR EQUIPMENT WAS AFFECTED BY THE FALL. THIS ACCIDENT WAS REPORTED TO MSHA ON 3/23/05 & WAS INVESTIGATED BY MSHA INSPECTOR. FALL WILL NOT BE CLEANED UP.

On the 088-0 MMU 6 north mains section, an alleged ignition occurred - an orange flame, round in shape, 18"" to 24"" in size, lasting 2 to 3 seconds. Self-extinguishing was observed. No injuries to persons or property.

THE ENLOW PROTAL ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE OF DEFECTIVE DOOR SWITCH. THERE WAS NO ENTRAPMENT OF PERSONEL OR DAMAGE TO EQUIPMENT

A roof fall was discovered on 3-22-05. The fall measured 80' by 20' by 6-8'. The fall will not be rehabilitated.

While mining X-Cut belt to L1 the miner operator cut through an abandoned bore hole @ 11"" in diameter. A small amount of water was encountered. Oxygen was 20.9% and Ch4 was 0.0%. The hole is properly plugged in the roof and floor.

An unintentional roof fall was discovered in the #2 entry one break in by spad #58. Size estimated to be 75' l by 18-20' w by 8-10' h. The area had been supported with 6 foot torque tension bolts with supplemental support consisting of 9 ft cable bolts. No injuries, no equipment caught, ventilation was not interrupted.

A roof fall occurred along the old travelway of the 3W at crosscut 3 in entry #5. The cause of the fall was weak laminated shale and water was dripping from the mine roof in this area. The intersection adjacent to this fall was bolted with 8' tension rebar bolts and it did not fall. The fall was restricted to the crosscut.

A ROOF FALL OCCURRED IN THE 1ST WEST, #8 ENTRY, #33 CROSSCUT. THE FALL WAS 30' LONG, 30' WIDE, AND 8' HIGH. THE FALL DIDN'T AFFECT VENTILATION, DAMAGE EQUIPMENT, BLOCK TRAVEL, OR CAUSE ANY INJURY. THE AREA WAS BOLTED WITH 6' & 8' POINT ANCHOR BOLTS.

A ROOF FALL OCCURRED IN THE 1ST WEST, #1 ENTRY, #22 CROSSCUT. THE FALL WAS 25' LONG, 25' WIDE, AND 8' HIGH. THE FALL DIDN'T AFFECT VENTILATION, TRAVEL, DAMAGE EQUIPMENT, OR CAUSE ANY INJURY. THE AREA WAS BOLTED WITH 6' & 8' POINT ANCHOR BOLTS. NO ONE WAS REQUIRED TO WORK OR TRAVEL IN THIS AREA.

AN UNINTENTIONAL ROOF FALL OCCURRED ON B-SOUTH MAINS IN CROSSCUT 4-5 AT STA. 13278. PRIMARY ROOF SUPPORT - 5' RESIN. HEIGHT-6'. WIDTH-18'. LENGTH-25'. ADDITIONAL SUPPORT WAS ADDED IN ADJOINING ENTRIES AND ROOF CONTROL PLAN CHANGED TO ADDRESS THE CONDITION.

A roof fall occurred at #28 x-cut of the Main Northwest in Entry #3. The fall measured 17' long x 17' wide x 12' thick. This area had been supported with 8' fully grouted resin bolts and 12' double locks. All approaches have been timbered.

A roof fall occurred on the 1LMS-2L1R panel in entry #3 at crosscut 36. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was located on the return air course of MMU-001.

At 7:15 pm a fall occurred in #6 entry out by spad #17610, in the neutral. Fall was 75 ft l by 19 ft w by 5 ft thick. Entrances to fall have been breakered off. MSHA and KOSL notified. MSHA investigated.

(2ND OF 2 ROOF FALLS DISCOVERED ON 3/15/05.) A roof fall occurred along the 2L4W-2L roadway at crosscut 14 between entry #2 and #3. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was adjacent to the roadway in the crosscut about 10 feet off the roadway.

(1ST OF 2 ROOF FALLS DISCOVERED ON 3/15/05.) A roof fall occurred along the 1W belt line at crosscut 27 between the belt and the return stopping line. The cause of the fall was weak shale. This area was developed 8 years ago.

The elevator at our Hart street facility failed to operate properly due to a brake adjustment problem. The elevator would not operate on automatic with the brakes deagging. The brakes were adjusted and the elevator returned to service at around 10.00 P.M..

DURING #2 SKIPS LOADING CYCLE, A ROCK LODGED IN THE SKIPS GATE PREVENTING IT FROM CLOSING PROPERLY. THE PARTIALLY OPEN GATE LODGED AGAINST THE LOADING POCKETS TIMBER FRAME PREVENTING RAISING THE SKIP. THE SKIP HAD TO BE MANUALLY EMPTIED TO FREE THE GATE. NO INJURIES OR DAMAGE RESULTED FROM THE INCIDENT. MSHA WAS NOTIFIED IMMEDIATELY OF THE SITUATION.

Roof fall occurred on track entry of 33 mains between crosscuts 112 & 113. Fall was 50' long, 20' wide & 10' high. Ventilation was not impaired. No equipment was damaged and no people injured.

AT 5:05PM ON MARCH 9, 3 MINERS ON 4000 LEVEL REPORTED TO THE #3 SHAFT HOIST MAN THAT THEY SMELLED WOOD SMOKE IN THE 4000 LEVEL DRIFT PAST THE TOP OF 43-190 RAISE. HOIST MAN IMMEDIATELY INJECTED STENCH INTO THE COMPRESSED AIR LINES AND THE VENTILATION STREAM TO EVACUATE THE MINE. SHIFT SUPERVISOR ALSO SMELLED SMOKE COMING FROM THE TOP OF 190 RISE. EVACUTATION COMPLETE AT 5:45P

at 11:20 pm on 3-8-05 a rock burst outbust occurred int he 5500-31 stope and the 5500-36 east stope. the rock burst in the 55-31 east stope consisted of damage int he hanging wall approx. 20 ft in from the main interseciton, three sill caps were damage along with the cement fill coming in approx. 8 to 10 feet up and 10 foot wide.

DUE TO UNDETERMINED CIRCUMSTANCES, METHANE IN EXCESS OF 2.0 PERCENT WAS FOUND TO BE LIBERATING FROM THE 1ST SOUTH MAIN SEALS. THE METHANE WAS ALSO FOUND IN THE WALKWAYS DOWNWIND FROM THE SEALS.

EE was tramming D8L dozer to parking area. The dozer began to run rough and EE stopped to check fluids. While closing guard on engine compartment EE lost his balance and stepped back onto track. There was a pad missing, causing EE to fall between the tracks and the dozer. This fall resulted in injuries to EE's lower back and left knee.

When scale was removed from the clean out, hot water exited into the scrubber cone filling the base.
